Having to replace the radiator due to it leaking around a plastic connection to metal. Was told by the dealership this has an updated part from GM due to it being a known issue.

I am submitting this complaint to the National Highway Traffic Safety Administration (NHTSA) regarding a sudden engine failure in my 2021 Chevrolet Tahoe equipped with a 6.2L V8 engine, which posed a serious safety risk. I was driving on the highway at approximately 65 mph with cruise control engaged. Suddenly, the vehicle lost all power, disengaging the cruise control, and the check engine light began flashing rapidly. I managed to exit the highway safely. While stopped at a traffic light, preparing to turn into a nearby repair shop, the car started shaking violently from side to side, making it difficult to control. I took the vehicle to a Firestone shop and a local Chevrolet dealer, both of which diagnosed massive engine failure and indicated that the engine will most likely need to be replaced. The symptoms I experienced—sudden loss of power at highway speeds, flashing check engine light, and violent shaking—are in line with issues reported in other GM 6.2L engines that have been recalled. For whatever reason, my 2021 Tahoe, which falls in the middle of the affected model years for these recalls (e.g., GM recall N252494001 for 2021-2024 vehicles with 6.2L V8 engines due to defects in connecting rods and crankshafts leading to engine damage and loss of propulsion), is not included under the broader GM 6.2L engine recall, although I experienced very similar issues. The biggest safety concern was the engine essentially losing power at highway speeds, which could have led to a crash. I urge NHTSA to investigate and expand coverage if necessary to ensure public safety.

The contact owns a 2021 Chevrolet Tahoe. The contact stated that while driving at an undisclosed speed, several unknown warning lights illuminated. In addition, the contact noticed an abnormal ticking sound. The vehicle was towed to an independent mechanic, where the vehicle was diagnosed with a misfire in cylinder #7. The independent mechanic checked the spark plugs and other unknown parts for failure; however, there was no evidence of a failure. The contact was informed of a valve lifter failure. The contact was informed that the valve lifter and possibly the push rods and camshaft needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure but provided no assistance. The contact referenced Technical Service Bulletin: N212353840; however, the VIN was not included. The contact was advised to contact the NHTSA Hotline to report the failure. The failure mileage was approximately 87,893.

The contact owns a 2021 Chevrolet Tahoe. The contact stated that while his wife was driving at undisclosed speeds, there was an abnormal ticking sound coming from the vehicle. The check engine warning light was illuminated. The vehicle was taken to the local dealer, where it was diagnosed, and determined that the cylinder #1 left side lifters and push rod had collapsed. The lifters and push rod assembly were replaced. The contact stated that on a separate occasion, while driving at undisclosed speeds, there was an abnormal ticking sound coming from the vehicle, and the check engine warning light illuminated. The vehicle was taken to the local dealer, where it was diagnosed, and determined that cylinder #4 lifters had detached, and the push rod was bent. The vehicle was repaired. The contact stated that the engine was consuming an excessive amount of engine oil. The contact related the failure to TSB: 15-06-01-002A. The dealer and manufacturer were not made aware of the failure. The failure mileage was approximately 12,400.

FUEL/PROPULSION SYSTEM

I am writing to formally request a review of my vehicle’s eligibility for Recall N212332040. I own a 2021 Chevrolet Tahoe, which is currently not listed under the affected vehicles for this recall. However, based on the recall description and the symptoms outlined, I believe my vehicle experienced related issues and should be considered for inclusion. While driving down the highway my vehicle experienced power failure leading to a full stop in the middle of the highway, resulting in a 9-1-1 call due to safety concerns for myself and oncoming traffic. The car was later diagnosed with the fuel pump module failure that other vehicles within the same year & model experienced, which were included in the recall. Why mine wouldn’t be included is quite confusing. I respectfully ask that your team investigate whether my Tahoe meets the criteria for Recall N212332040. On 10/20 I sent in a request to the GMC recall department & on 11/11 I was denied anything further. I appreciate your attention to this matter and your commitment to vehicle safety.

While driving my vehicle shut down. It will not longer crank. Had to have it towed. I am told that it’s the fuel pump driver control module. I carry my two children in this vehicle daily. This could have ended very badly. Now not only is my family without a dependable vehicle, I am going to be out a lot of money on an issue that is already a common issue.

The fuel pump went out. We were driving, about to enter traffic, and the engine shut off. It would not restart, and we had to have it towed to a mechanic's shop. The fuel pump control module was removed and replaced.

I JUST HAD TO BUY A FUEL PUMP CONTROL MODULE 2021 CHEVY TAHOE HIGH COUNTRY THAT WOULD CRANK OVER, AND NOT START. PER INTERNET, THIS PART IS ON A MILLION CAR PART FAILING. I FOUND ON INTERNET THAT IT SHOULD BE REPLACED AT NO CHARGE. I WAS STUCK AT HOUSE AND HAD A FRIEND CHECK WITH A OB2 TESTER AND IT SHOWED FAILURE. I CALLED 2 DEALERS AND MENTIONED RECALL AND WAS TOLD THEY KNEW NOTHING ABOUT. YET ON SEPTEMBER 11TH OF THIS YEAR, GM WAS WELL AWARE! RECALL #252516900. I HAD TO GET A RIDE AND GO PURCHASE AT A GM DEALER. AND PAID $69.09 WITH TAX. PART NUMBER #13554821 GM 03900 MODULE 03.900. AND COULD HAVE BEEN A SERIOUS PROBLEM IF I WOULD HAVE BEEN DRIVING OR STUCK OR SOMEWHERE. COSTLY TO GET TOWED AND NO CAR TO DRIVE. IM HANDICAP AND CAN'T BE WITHOUT. BUT FOR A DEALER TO CHARGE. AND PLAY DUMB SINCE I WAS GOING TO HAVE TO BUY AND CHANGE AT MY HOME. WANT MY MONEY BACK FOR A NO CHARGE PART.!!!!!!!!!

Was in stop and go traffic. Car suddenly said “conditions not correct for shift” and engine died. The car would not turn on and it could not be put into neutral therefore was stuck in the lane stopping all traffic on the NJ turnpike. It created a safety issue for that reason as there was no way to move the car to a safe place until a tow truck came and had to drag it onto the bed. I have since had it repaired at a local shop not Chevrolet dealer and was told it was fuel pump failure. There was no warning or any other symptoms in facet I had just had an oil change a week prior. Had there not been stop and go traffic and I was going the speed limit (60mph), there could have been a major accident and injuries.

My Tahoe was making a loud noise, I took it to the dealer to look at it and they said I need to replace the rear driveshaft to the tune of $4000. I don't have $4000 to spend on this. This Tahoe is only 4 years old and was supposed to drive us to Minnesota next week for Christmas and now that is not possible. This is a known issue and a recall, but my VIN not included in that recall. How can this happen - you know it is a known issue in these cars! Issue related to the rear-differential fluid, TSB 21-NA-028. Its a known issue where the rear differential make moan/chatter noises on turning (including at low speeds). Dealers are instructed to service/replace the rear differential fluid and perform procedures to address clutch pack chatter. It is such an issue, the fluid to remedy this is on back order!

2021 Tahoe Z71 with 70000 miles making a roaring and whining noise on reverse and turning, especially when cold. Took to shop and they changed the rear differential fluid. Now its worse. Needs a whole new rear end. This veicle is 4 years old and does not tow or use four wheel drive. The shop said there has been so many complaints and services on the same issue. Why is there not a recall on this yet? We spend $70,000 and up and expect a more quality vehicle without having to spend $4000 dollars on repairs.

Based on dealer inspection, oil was found sitting on top of the #2 piston, and you’ve indicated the failure is originating from the bottom end of the engine—likely involving piston, ring, or cylinder wall damage. Combined with the stored P050D, P0300, and P0302 codes, this represents a clear internal mechanical failure. Vehicle is currently at the dealership. Mechanical failure while driving the vehicle. Sensor light for service ESC and service traction control both showed as the failure was occurring.

My 2021 Chevrolet Tahoe recently displayed a warning message that said “Service Transmission – May Not Be Able to Shift Soon.” The message appeared at approximately 132,754 miles. Since then, I’ve noticed delayed and rough shifting between gears. The vehicle still drives and shifts into gear, but the message and behavior are concerning. I have an appointment scheduled for a diagnostic inspection on Tuesday, which was the earliest available. I have not yet received a confirmed diagnosis, but I’ve seen that many other 2021 Tahoe's and similar GM vehicles have experienced the same transmission problem and were included in recalls. My vehicle’s symptoms match those reports, though my VIN was not included. This situation makes me nervous because I drive with my four children daily. Even though the vehicle hasn’t completely lost the ability to shift, a sudden failure while driving could put my family and others on the road at risk. The warning itself suggests a potential failure is coming soon. The vehicle has been properly maintained and has not had prior transmission work. I’m reporting this because it appears to be part of a larger pattern that should be investigated to determine if more vehicles need to be included in GM’s existing transmission recall.

1. What component or system failed or malfunctioned, and is it available for inspection upon request? Answer: The vehicle’s 6.2L L87 V8 engine suffered a sudden internal mechanical failure involving the connecting rod, rod bearings, and crankshaft assembly, resulting in catastrophic engine damage. The failure mode is consistent with the defect described in GM Recall N252494001. The engine and all damaged components are fully available for inspection upon request. 2. How was your safety or the safety of others put at risk? Answer: The engine failed without warning while the vehicle was in motion, causing an immediate loss of propulsion. This created a hazardous situation where the vehicle could no longer accelerate or maintain speed in traffic, increasing the risk of a rear-end collision or loss of vehicle control. GM’s own recall documentation identifies this exact failure mode as a significant crash risk, and that same hazard occurred in my case. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Answer: Yes. A service technician confirmed catastrophic internal engine failure consistent with connecting-rod or crankshaft damage. The dealer also verified that the engine is inoperable and requires full replacement. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Answer: No, the engine failed while in motion and was taken to the closest mechanic. Inspections by an insurance representative or manufacturer representative can be arranged. The vehicle and damaged engine remain available for further examination. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Answer: No. There were no warning lamps, diagnostic messages, or drivability symptoms prior to the catastrophic engine failure. The failure occurred suddenly and without any advance indication. All service records are available

Crash: 1
Took the vehicle in for Airbag System recall in 2022. They said they fixed it and that there was a broken wire. The very next day the warning light came on again like before. I contacted the dealership and they said bring it in. I could not get it in again as I was very busy. I asked them to make a note in my file of the immediate call/report of still a problem. I took the vehicle in this month (October 2024) for issue of no starting (fuel pump failure, another recall they are denying). They stated that the Drivers Side Airbag module was shorted or bad and my cost would be approximately $1,250. They stated it was not covered under any recall. I contacted Chevy corporate that offered to cover 30% only of this. I believe this should be covered in whole due to the failure to repair the first time and that is is a known safety issue. The vehicle has only 10,800 miles.

I was driving with family (2 adults - including myself, and 5 kids - ages [XXX] ) The truck started to flash warnings and my brakes disabled. We couldn't stop and almost had an accident. I kept pumping the brakes and the E break I didn't give it any gas and we slowly came to a stop after some time. I shut the truck off as I was beyond terrified and tried to start it back up. The lights came on but it didn't turn over to start. Had to have it towed and find an uber for a 7 passenger car. The truck was towed to the nearest dealer (a little over an hour from my home) and I was told it was a battery issue. They replaced the battery didn't check the breaks and told me if the battery dies the breaks stop working. Yet, I had power in the truck just no brakes, when I tried to turn it on the lights went on as did the radio but that was it. I tried to take pictures of the warnings on the dash however we were in Mom panic mode. Chevy is acting as if this isn't an issue. I had to put out over $1,100 between the battery, the fees of the dealership, rental and an uber. If we were going at a higher speed we would have not been okay. It was beyond dangerous and the thought that if a battery suddenly dies you have no breaks, that is just unacceptable. I would love for someone to look into this. I am now looking to see what I can do as I don't feel safe in the truck. I don't want this to happen to anyone else hence why I am asking for someone to please look into this. Driver's side headlight adjuster screw breaks very easily, and makes the headlight beam be too low with inability to adjust it. There is no way to replace it... it requires a change of the entire headlight, which requires removing the entire grill. A cheap piece that makes customers pay thousands of dollars in repairs. There are a myriad of reports of this issue online, for example here: https://www.tahoeyukonforum.com/threads/headlight-adjuster-broken-again.144355/

We were driving our 2021 Chevrolet Tahoe (purchased Dec 2020; odometer currently right under 40,000 miles) out of town this past weekend and I noticed the headlights were not illuminating the road ahead very well once the sun went down. They seemed like they were pointed down and when i turned on the high beams, those were also lower than how the low beams are usually pointed. We were stuck in the middle of nowhere and I persevered with low lighting onto our destination. After researching the issue on the internet days later, I found a Tahoe Owners' forum/thread [XXX] ) where several other 2021 Tahoe owners had experienced the exact same issue as ours had. The culprit appears to be a sub-component of the headlight adjuster mechanism, but GM/Chevrolet does not offer a replacement for either this small white plastic piece that is failing and the only repair option they have for this issue is complete replacement of the entire headlamp assembly ($1,350 MSRP). This is a dangerous problem that occurred randomly on a vehicle with low mileage and appears to potentially be a common failure for other owners of the same headlamp design. I have not taken this vehicle into the dealership for inspection, as I expect they will only replace the entire headlamp assembly for $$$$. Info center with radio, vehicle info, on star keeps going black screen. When this happens I loose all control of info center and on star goes out where no location would be available if the vehicle was in an accident. This is safety concern because with no automatic notifications of accident no one would know if vehicle was in an accident. The vehicle has been to dealer 4 times for this problem since February of this year. Each time the dealer says it needs another update each time. GM is having this issue with many of the Vehicles like this Tahoe. On Star is also aware of the issue too. GM knows there's an issue but now they are saying I have to pay for an update because I am out of my extended warranty even though this has been an ongoing problem which started when I was in warranty. This has to be a programming issue that they need to fix. The vehicle has to be shut off for up to an hour for screen and controls to come back on which is safety issue because no way to change any settings for vehicle and safety backup because of no on star.

Fuel module failure while driving, engine stalled and would not restart.., while at highway speed. Very dangerous situation!!!!! Tahoe was towed to dealership kept for. 1day. Chevy dealer replaced fuel module. We have owned 6 different Tahoes starting with the year 2015 to 2024. The fuel module failed on the 2021, 2022 and now the 2023!!!!!!! Makes us question if we should purchase a new Tahoe every other year? Seriously considering other car brands such as Ford, BMW or other more reliable brands!!!!

On 2 occasions the dash lights and gauges do not turn on when the vehicle is on. The first time this happened I was driving and was no longer able to see my speed or any other details provided by the gauges. We changed the fuse and it seemed to work for 1 day. The next day I turned the vehicle on, was able to drive but none of the dash features were functioning again. Will be taking to dealership when available scheduling to have it evaluated.

vehicle infotainment system will randomly "restart" or shut off completely when vehicle is being drove, but will still be playing music also all time and temperature (outside temp) will freeze when this happens. it also make the battery drain when this happens. the only way to get the screen to come back on weve tried is to disconnect battery. weve replaced the battery, weve had a chevy dealership look into it. no one can figure out what is happening. Now it is happening more and more. we also do not receive updates anymore to the infotainment system. the dealership had to manually do it a year ago. also will not keep our proifile signed in at all. always says were stuck in guest mode. says we have full storage when there is not any apps downloaded.

My vehicle performed an update without my knowledge. When I got back into my vehicle after a quick stop the infotainment system said "Update Failed - Return to Dealer". I did so and was informed that a reprogram was attempted but failed so the entire system needed replaced. There is active recalls I'm told on newer model vehicles with the same issue but mine is not included. So, after the OTA update I didn't ask to take place I'm stuck paying $1,300 for a new infotainment system. I called GM who basically told me without there being a recall there was nothing they could do.

The affected components are the electrical system, engine, and power train. In March 2025, my 2021 Chevrolet Tahoe displayed multiple warning lamps across the dash, including traction control and check engine lights. The vehicle shifted erratically and sometimes entered limp mode. I brought the vehicle to AutoNation Chevrolet South Clearwater for inspection. The dealer replaced the battery, dismissed my concerns, and stated that a test drive was performed, but the mileage in and out on the invoice was identical, showing it never actually occurred. Diagnostic trouble codes (P0700, P0717, P2714, P2716, P0706, and multiple U-codes) were present but were not fully investigated. A GM Technical Service Bulletin (21-NA-149, December 2022) covers these exact symptoms (caused by engine wiring harness chafing), but it was never referenced or applied. Because the problem was not addressed, the condition worsened. By July 2025, the vehicle suffered a complete transmission failure. At that point, the dealer confirmed the issue, documenting transmission slipping, shuddering, noise, and additional codes (P0700, P077C, P17F6, P17FB, P1872). GM increased its goodwill contribution from 15% to 40% of the $9,925 repair cost, while AutoNation offered only $500 despite their role in the missed diagnosis. Safety Risk: My family and I were at risk each time the vehicle stalled, lost power, or shifted erratically while driving, including at highway speeds. These failures created a significant risk of collision. The drivability problem was not properly confirmed in March 2025 despite multiple codes being present. By July 2025, the dealer acknowledged and confirmed the transmission failure. The vehicle remains available for inspection, and I have retained all invoices, diagnostic reports, and communications with GM and the dealership.

The affected systems are the power train, engine, and electrical. My 2021 Chevrolet Tahoe experienced failures consistent with GM Technical Service Bulletin 21-NA-149 (engine wiring harness chafing) and the conditions outlined in Recall N232404190 (valve body failure, November 2024). Beginning in March 2025, the vehicle displayed warning lights and messages including Check Engine, Traction Control, Service Transmission, and limp mode (Reduced Acceleration – Drive with Care). The vehicle shifted erratically, lost power, and occasionally failed to accelerate, creating a serious safety hazard at highway speeds. These problems were documented at AutoNation Chevrolet South Clearwater, where multiple diagnostic trouble codes (P0700, P0717, P2714, P2716, P0706, and U-codes) were recorded but not properly investigated. No test drive was performed despite being claimed by the service technician, and the applicable TSB was not referenced. By July 2025, the condition worsened into a complete transmission failure, with additional codes (P0700, P077C, P17F6, P17FB, P1872). The dealer’s estimate required a full transmission replacement at nearly $10,000. GM reviewed the case through its internal Policy Evaluation Tool and increased goodwill assistance from 15% to 40%, while AutoNation contributed only $500. GM’s partial contribution confirms the defect is recognized, yet this VIN remains excluded from the official recall. This issue placed my family and other motorists at significant risk, as sudden loss of power and shifting failures occurred during normal driving conditions. Dealer records, GM documentation, and photographs of warning messages are available. I request that NHTSA expand Recall N232404190 to include affected VINs such as mine, since the symptoms, diagnostic codes, and confirmed failures match exactly what GM has already identified as a known safety issue.

On 01/03/24, the vehicle stalled and smoked on the Suncoast Parkway. All dashboard warning lights and messages illuminated, and the vehicle entered emergency park mode. OnStar was contacted and the vehicle was towed to Jenkins Chevrolet in Homosassa, FL. The failure was linked to a broken pulley and belt system that damaged the ECM and wiring harness. No DTCs were pulled, and mileage in/out was 80042/80043—no test drive performed. On 08/06/24, the vehicle returned to Jenkins for a roaring noise. A pulley bearing was replaced, possibly related to the earlier failure. Again, no drivability evaluation or codes recorded. On 03/09/25, the same full system warning lights/messages appeared again. The vehicle experienced harsh shifting and delayed acceleration. It was taken to AutoNation Chevrolet South Clearwater on 03/10/25. DTCs (P0700, P0717, P2714, P2716, P0706, and U-codes) were found but dismissed. Only the battery was replaced. Mileage in/out was 99494—no test drive performed despite claims one occurred. On 07/08/25, the vehicle again showed full dashboard warnings. Brought back to AutoNation on 07/09/25. On 07/14/25, customer received a $9,177.85 estimate including full transmission replacement. Safety was at risk due to repeated stalling, loss of acceleration, and emergency parking mode while driving or merging. These failures could have led to accidents or left the vehicle disabled in traffic. Driver and children were present during each incident. The vehicle is enrolled in OnStar and the MyChevy app, which provide monthly health reports. Despite these incidents, no faults or warnings were ever shown in the reports. Vehicle remains available for inspection. A case is open with GM. Across 4 service visits, GM-certified dealerships failed to follow proper protocols, including code checks, drivability assessments, and test drives—contributing to delayed diagnosis of transmission failure.

This is the second engine I’ve had replaced since I bought the vehicle in December 21, 2023. The first engine failed in August of 2022 while I was driving on the highway. The vehicle began to smoke and shut down while I was driving on the highway. The vehicle shut down and I had to use the current momentum of the vehicle to pull over to the shoulder. The vehicle would not come out of gear and would not restart. I complained of transmission problems ever since the engine was replaced. The service department escalated the concerns to the GM engineers who never gave directions to check the transmission fluid or physically check the transmission. January 24 of this year I was on the highway driving to work when the vehicle again shut down. The dash stated restart the vehicle. While in motion the vehicle was trying to shift to neutral. I informed the dealer of this message and they claim they took note of it. With the vehicle not being able to start or move out of gear the tow truck had to drag the vehicle onto the bed of the tow truck. This also occurred on the first engine failure as well.

While driving in traffic the car shut off, engine stopped. When the car stopped it went into park. Could not restart the car. Starter worked and engine started but ran very rough then stopped several times. I could not completely shut off the car in an attempt to basically reboot. Car had to be towed to the dealer.

third row sealbelt buckle retention been a problem since 08/2022 and no one has given me a call. IF I get in a car accident it can increase the risk of injury for the people in the 3rd row. i have no accidents but I'm trying to prevent it from having one.

2022 Tahoe was inspected for the announced recall. The manufacturer Chevy has no timeline for when the 3rd row seat belts will be repaired but only said we CANNOT use any of the 3rd row seats till fully repaired. I called chevy and ask them what’s the suggestion what’s if the passenger is 5 or more. Will the give rental fee or a loaner vehicle. They said no. This is totally unacceptable. GM is offering nothing to customers that purchased $74000+ vehicles from them for the very reason that they have big 3rd rows. How are families of 5 or more supposed to function in the meantime? NHTSA should exert all available pressure on GM to prioritize on-road vehicles and do it now.

The contact owns a 2021 Chevrolet Tahoe. The contact received a notification for NHTSA Campaign Number: 22V617000 (Seat Belts) however, the part to do the recall repair was unavailable. The manufacturer was made aware of the issue and confirmed that parts were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact had not experienced a failure. VIN tool confirms parts not available.

Our Tahoe was inspected for the announced recall and failed the test. The dealer/manufacturer has absolutely no timeline for when the 3rd row seat belts will be repaired and said we cannot use any of the 3rd row seats until then. The dealer even said it could be a year before parts are available. This is totally unacceptable. Has GM rerouted new seat belt assemblies from their manufacturing of new vehicles to prioritize repairs of vehicles already on the road vs. keeping their sales pipeline full? I doubt it. Are dealers prioritizing vehicles on the road over those on their lots that are waiting to be sold. I doubt it. These are the very questions NHTSA should be asking of GM. GM is offering nothing to customers that purchased $70,000+ vehicles from them for the very reason that they have big 3rd rows. How are families of 5 or more supposed to function in the meantime? NHTSA should exert all available pressure on GM to prioritize on-road vehicles and do it now.

I am writing to formally document serious engine defects I have experienced with my 2021 Chevrolet Tahoe equipped with the 5.3L L84 V8 engine. 1.Crankshaft Failure: My Tahoe suffered a catastrophic crankshaft failure that required major repair/replacement. I am aware that GM has recalled certain vehicles equipped with the 6.2L L87 engine for crankshaft and rod-bearing defects. Based on my experience, the same or a very similar defect has also occurred in my 5.3L engine, which is currently not included in that recall. 2.Valve Lifter Failure: In addition, my vehicle has suffered from valve lifter collapse—a well-documented problem in 2021–2022 GM trucks and SUVs with both the 5.3L and 6.2L engines. This caused severe misfiring, drivability issues, and expensive repair needs. Why this is a concern: •Both failures occurred well before what would be expected of a modern engine. •The crankshaft failure raises a safety risk: sudden loss of propulsion while driving could lead to accidents. •The lifter issue is widely reported by other owners and has been the subject of GM service bulletins. •Taken together, these problems suggest systemic defects in the 2021 Tahoe’s 5.3L engine that have not been adequately addressed by GM. Requested Action: •I ask GM to extend recall and warranty coverage to include the 5.3L L84 engine in addition to the 6.2L L87. •I ask NHTSA to consider investigating whether the scope of the current 6.2L recall should be broadened to include 5.3L engines, given failures like mine. •At minimum, I request assistance in covering repair/replacement costs for both the crankshaft and lifter defects, which are clearly not due to normal wear and tear. Vehicle details: •Year/Make/Model: 2021 Chevrolet Tahoe •Engine: 5.3L V8 L84

I was driving along the highway at 11pm when I the vehicle started shaking and I heard strange sounds coming from outside the vehicle. I pulled over to inspect and realized it was coming from the engine bay. The engine was making knocking, rattling, and belt slipping noises and the entire vehicle was shaking and the engine also stuttering. I was able to drive the vehicle to a nearby plaza where I had it towed the following morning. There were no warning signs, the vehicle ran properly up until this occurred. The dealership inspected it and told me the engine needs to be replaced and it was likely that a rod bearing that went bad. This happened nearly 2 months ago and I am still waiting for a new engine.

Engine failed while driving down the road. There was not prior warning signs to this issue. Vehicle was unable to accelerate. The safety of myself and husband were put a risk as we were traveling on an Interstate 12 in Baton Rouge, LA. The vehicle was inspected by Pine Belt Chevy and the cause identified as Camshaft and lifters failed in the engine causing a sudden deacceleration.

On three (3) different occasions, I put the SUV in Park and the car engaged, and the "P" lit up as engaged. As I took my foot off the brake, the SUV went into Drive and rolled. This last time, the SUV did this at my kid's school Ater putting it in Park, I took my foot off the brake and turned to get the kids unbuckled, and my foot accidentally pushed the gas pedal. The SUV went into drive on its own and the SUV drove over the sidewalk hitting a rock staircase causing an accident with all 4 kids in my SUV. This is the 3rd time, and I don't know what to do about it. Thankfully there were no kids in front of the SUV.

We just recently had the cam shaft and lifters replaced, as the vehicle began experiencing a knocking sound. The repair was supposedly completed, and they gave the truck back. We took the truck back in within a week, bc we continued to hear the sound. They sent it back saying they didn’t hear anything after test driving it. Over the weekend the car shut off after beginning to shake uncontrollably. Now supposedly the camshaft bearing is gone, and the truck experience engine failure. I spoke with someone previously, as the issue being repaired hundred thousands of cars were recalled for the same issue. I was told that it wasn’t on the same line, even though my truck was experiencing the EXACT issue and fell under the years. We were placed back on the road with our family, with a vehicle

I’m reporting a transmission failure that occurred unexpectedly on my 2021 Chevy Tahoe/the diesel version has been recalled so I don’t know why all the other 2021 Tahoe have not been recalled for this issue . The vehicle began shifting roughly and slipping between gears, followed by a warning message and eventual inability to shift properly. I had it inspected by a family friend who owns a body shop , and I was told the transmission needs to be replaced at a cost of nearly $7,000. I am no longer under powertrain warranty, but this failure feels premature and unsafe for such a new vehicle. I’ve seen many other 2021 Tahoe owners report similar issues online including transmission failure, harsh shifting, valve body problems, and complete breakdowns . This seems like a pattern, and I believe it may warrant investigation. I’m asking NHTSA to investigate whether this is part of a broader transmission defect that poses a safety or reliability concern. Sudden shifting issues and inability to accelerate or reverse are dangerous for drivers and passengers. Thank you for your time and attention to this matter.

2 incidents of complete engine power loss while driving. Luckily was able to roll off road safely to curb. Dashboard glitch/flashing multiple/various message, including check engine, conditions not ideal to shift, etc. Would not re-start/shift when attempted. Tried disconnecting battery cable completely and re-connecting and that seemed to 're-set' the system and engine re-started. Incidents were about 30 days apart. "This battery reset" was tried because approx 1 year prior after leaving Tahoe parked at airport for a 7 day trip, it would not start, completely dead. Battery charge/jump would not work...watched youtube videos and someone said to try disconnect/reconnect cable and it worked to start it up at that time & did take it into Dealership after that incident and no-cost repairs were made for that issue. Called Chevy 3/25/25 on this power loss while driving issue to ask for information/assistance, they advised to take to dealership for diagnostic info/report and depending on result/cause might assist with costs to repairs. Appointment set for 3/28/2025 with Salem Oregon Chevrolet.

The head rest in the third row seat is broken and will not remain in the up position when the seat is up. This is causing a safety risk in that there is no safety for passengers in a rear end collision. There is already a recall that Chevrolet has issued for other Tahoe vehicles; however, my VIN number has not been added to that recall. I am requesting to have my vehicle's VIN number added to the already existing recall.

When driving the car just stops losing power then the car immediately shuts down and stops driving. I had to call a tow truck when it’s taken to the dealership the fuel pump power control module is faulty. This is a 2021 Tahoe they had to replace the fuel pump, power control module, and this vehicle should not have to be replaced. Chevrolet/GMC knows that these modules will make the vehicle stop wherever it is driving. I happen to be on a freeway going 70 mph and could have been killed. They have not put a recall out to have these modules replaced in any SUV. This is a danger for any driver on the road. This is a lawsuit waiting to happen for people. You know that they are aware this is happening because they paid for the tow and replaced this part that is not warranty now for free. Yes, great that it was paid for but thankful I did not lose my life as I could’ve been rear ended when my car shut down going that fast in the middle of the freeway I am begging that they please put a recall so no one gets injured or killed.

To Whom It May Concern, I am writing to formally report serious defects with my 2021 Chevrolet Tahoe (VIN: [XXX] ) and to request assistance with repair costs. Approximately six months after purchase, when the vehicle had about 22,000 miles (I bought it used with around 19,000 miles), the battery and electrical system failed. I brought it to the dealer and was told there was no issue; they only performed a jump start. Two days later, the vehicle broke down in the middle of the road. I had to pay for a tow truck and replace the battery completely out of pocket. More recently, at about 92,000 miles, the vehicle developed severe engine problems. A certified mechanic diagnosed collapsed AFM lifters and camshaft damage. The repair requires OEM GM parts and costs approximately $4,500. This issue is widely reported by other GM owners with 5.3L AFM engines and is known to be a manufacturing defect, not a maintenance problem. Please note that my Tahoe has been well maintained, with oil changes always performed on time and using the correct oil and service intervals recommended by GM. Despite this, the vehicle has experienced repeated and costly failures that create unsafe driving conditions. I kindly request that GM review my case and provide assistance with covering the repair costs or reimbursement, given that these issues are the result of known defects and not owner neglect. I look forward to your prompt response. Sincerely, [XXX] [XXX] 33032 [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am experiencing loss of power just like the recall shows for campaign 22V036000 powertrain. Engine stall in the middle of the street just like it shows for the fuel system 21V73 9000. I have stalled in the middle of the street including today four times and a month. The dealership can’t seem to find what’s wrong. Nor other places. I don’t think they are looking hard enough at the dealership. I almost got killed by another car going at least 65 mph while I was stuck in the middle of the street at a stoplight. These are all different days. While working and people in my car.

2021 Chevrolet Tahoe original owner. Fuel control module failures. 1. Failed on 6/29/21 at 5,068 miles. Dealer replaced with GM Part #13537053. 2. Failed on 8/15/22 at 12,130 miles. Dealer replaced with GM Part #13544060 3. Failed on 8/1/25 at 41,247 miles. Dealer replaced with GM Part #13554821 On each failure, vehicle was unable to shift to neutral, power remained on, electrical component began shutting down to conserve power, left stranded and required towing with rear wheels being dragged locked up onto tow truck. Once power was shut down, could not operate windows or lock doors other than keyed driver's door. Th entire failure mechanism puts the driver in an extremely unsafe situation. Recall notice #21V739000 was issued on 9/23/21 with to impacted notified owners on 10/14/21. Did not receive this notification since first failure was prior to recall and assume GM considered vehicle repair completed. Note that the fuel module part number has changed 3 times in my case, but Part # 3554821 replaces 5 other listed part numbers. Is this the 6th iteration or of this part? What is the root cause of the fuel module failure? Is it the module itself failing or an electrical issue that causes the fuel module to short out and fail. GM/Chevrolet is not solving this issue. With 3 failures of the same component, this is a lemon and I have no comfort or confidence using this vehicle. When will the next failure occur and what unsafe situation will this leave me stranded in? Read this issue applied to 2021 and 2022 GM full size SUV's and now reading issues through the 2024's. Unacceptable and poor quality. GM needs to be held accountable to solve the root cause of these failures and stop putting a replacement band aid on the problem.

The fuel pump power control module failed. We were driving on the interstate going around 75mph with traffic. The car suddenly lost power dropped to 60, 40, 20 in 5 seconds. We barley made if off the hwy`s left side when the Tahoe died. Would not start back up and locked in park. Had to be towed on a flat bed dragged up the bed and brought to the Chevy dealer who confirmed the fuel pump power control module failed and replaced under warranty. We have a 2021 Tahoe, there is a recall for this part but it did not include our vin. Really scary breakdown and could have been hit by another car.

I was driving the vehicle and it suddenly lost power - going from 55 MPH down to 0 then the engine just stopped. No warning lights went off, no stalling, no engine noise- completely lost power. Would not start after that for 20 minutes. Started after 20 minutes- and ran fine -like nothing happened. Had it towed to dealership where they diagnosed the problem as electrical. Two weeks later still waiting for the part. Dealer says it could be up to 3 months before they could locate the part.

As a traffic crash reconstructionist, the issue is a MAJOR SAFETY ISSUE. My wife was exiting a residential driveway when the vehicle started to stutter and immediately shut down with no power or any capability to get the vehicle off the road. The shutdown caused the blockage of a residential street on a Friday afternoon, until the correct tow truck arrived. This was due to the fact the vehicle could not be moved due to not being able to start the engine nor the ability to place the vehicle into neutral, without starting the engine or crawling un der it with tools. It was determined the fuel module went bad. There was no warning of the failure of the component. Luckily the shut down of all components of the vehicle did not occur on a high speed or highly traveled road since I did not have any control of the vehicle after the fuel module failed. This is not the first report of the issue to NHTSA. Hopefully something will happen to find a fix to put the vehicle in neutral without tools, for the safety of the motoring public.

Vehicle is sputtering and jerking while driving When slowing to a stop vehicles engine will shut off. I do know that the 2021 Tahoe has recalls on the fuel pump issues but not sure why my vin number isn’t covered under this recall. This is a 2 year old vehicle and already has recalls on this year model on its fuel pump. At this point it is barely driveable and is a danger for myself and my family to drive.

I encountered the same issue which most of the other complaints on here saw where the engine lost power (due to one cylinder misfiring) suddenly and it was due to a faulty lifter in the engine. Fortunately, I was able to limp the vehicle back to my house where it could be towed, but had it been worse it could've stalled in an intersection and I'd be unable to move the vehicle due to how the engine must be running in order to put it in Neutral. GM really needs to recall the engines that are affected by the lifter problem as this issue is showing up on several forums online along with this site. My engine date code falls within their TSB, however they only replaced one bank due to my mileage being over 8,000 miles. That means that I most likely get to look forward to more issues soon with the other bank of lifters as they're of the same faulty design.

ENGINE,FORWARD COLLISION AVOIDANCE

On April 25th, 2025 we were taking our kids to their after school activities when the SUV started to shake, and accelerate very fast by barely pushing the gas pedal, the ETS light along with the forward collision warning also appeared on the dashboard along with the check engine light. This prompted my husband to get our vehicle towed to the nearest shop close to the street where our vehicle started having problems. After several days of diagnostics the shop let us know our vehicle needed the following repairs: Bank One lifters replacement, head gasket service, and a water pump replacement, we ok'd the repairs. Although these components were replaced, we were informed on May 15, 2025, that the vehicle continues to misfire and now requires a camshaft replacement as well. The total cost of the repair has now risen from $5,182.16 to $13,549.79, which is a significant financial burden as the vehicle is just outside of warranty coverage. We are aware that there is an active recall covering similar issues for 2021–2024 Chevrolet Tahoes equipped with 6.2L engines. However, our 5.3L engine model is experiencing the same mechanical failures, including misfires, knocking noises, low RPM idling, and performance degradation. There is growing evidence that the valve lifter issues are not limited to the 6.2L engine variant. Our experience—and the experiences of others reported online—suggests that the 5.3L L84 engine may also suffer from the same design or manufacturing flaws. We respectfully urge NHTSA to investigate whether this issue warrants expanding the scope of the existing recall to include the 5.3L engine models, and we ask that GMC/General Motors be encouraged to provide assistance. Given the severity and known pattern of these issues, we believe GMC should cover at least 75% of the repair costs, or ideally, offer to perform the repair at no charge. Thank you for your time and consideration.

We had a failed lifter that caused bent rods which caused scratches on the crank shaft which caused us to have to replace the engine. After a simple internet search I have found the lifter failure is very common with gms 5.3 v8. We were told oil needs to be changed at least every 5000 miles although the vehicle prompts you around 7000.

On [XXX] I was driving in the [XXX] in El Paso Texas at about [XXX] I was about to enter [XXX} and the vehicle suddenly stopped with no warning! I was able to pull to the side and put the blinkers because the traffic was heavy and could of had a vehicle crash into me if I had not maneuver the vehicle quickly(My two daughters where with me in the vehicle). The vehicle prompted to start the vehicle, which I did so and continued my way. As soon as I was able to get into the [XXX] the check engine light turned on, and the forward collision warning message on the display. I stopped to my nearest Autozone to get some help as the vehicle started to make a noise too. They connected their computer and told me that the vehicle was given a message of a faulty cylinder, they told me I should not move the vehicle until I had it inspected on a GM dealership. Which I did on Friday December 20, 2024, I am waiting for a full diagnostic to see what had happened to my vehicle. We purchased this vehicle on 11/28/20, and on 12/10/20 began receiving multiple error messages and loss of power on the accelerator. Shortly after the error messages began I was no longer to even start the vehicle. It needed to be towed to the dealer and eventually required a full engine replacement, and was out of service for 22 days. On 5/19/21 I was getting on the highway with my wife and 3 young children and began receiving many of the same error messages. The car was brought to the dealership the following day and I found out that this was another major engine issue. They need to order several parts, one of which is on backorder. As of 5/24 it has been out of service for an additional 5 days, bringing the total up to 27 days out of service, and there are currently 4800 miles on the car.

There is a serious cabin air pressure issue in this vehicle. When driving at any speed with the windows up, there is a booming inside the cabin, as if the passengers are inside of a subwoofer. My theory is that the headliner panel has too much flexibility in it due to the ceiling air vents. When driving, it is likely the headliner is bouncing up and down, creating the booming air pressure. Within 2 weeks of delivery of this new car and daily driving, my passengers and I developed serious ear issues which we attributed to the car. It got to the point where I would immediately have pain and ringing in my ears just driving a very short distance. None of my other cars including SUVs cause or have ever caused this issue. It is important to note that with all 4 windows rolled down, the problem does not happen. It's only when the windows are up. I have the LS trim without the sunroof.

WHAT FAILED: Main Radio. SAFETY CONCERNS: GM sent an over the air update to car around July 5, 2024 and the update broke the whole radio system to my car. I now have no access to OnStar, bluetooth for phone safety, maps, blinkers on side mirrors, radio etc. Dealer mentioned it is a very common problem with this particular GM product and the over the air update to cars. When I asked to turn this feature off so it doesn't happen again it is not an option. The Chevrolet Dealer and a sound system specialist has inspected vehicle. Sound System specialist said this part may only be replaced by the dealer as it is the high end radio system. There was no warning of the update happening. The car was in great condition until this update was pushed through in the middle of the day. Now I am being quoted $2000 to fix it with no guarantee that it wont happen again.

My tahoe has shut off several times. I am sometimes at a red light and it shuts off and won't turn on or itll eventually turn off but it has a ton of errors on the screen. This has happened about 6 times. Today, April 6th, it wouldn't even start. It has started at the dealership but the screens are blacked out. Also the back hatch is not working as well and it randomly happened. I just purchased this vehicle in September of 2022. And it only has about 40k miles on it. The first time it shut off on me it had less than 40k miles.

Received an Over-the-Air update that rendered my vehicle unless. My vehicle would no longer start, or power on, or power on in accessory mode. It was stuck in Park, the electronic Parking Brake could not be disengaged, the door locks could not be operated and left unlocked, no lights, no warning lights, the emergency lights did not function, no horn, and most everything else. The only things still working were the radio, the driver's side powered seat would move, and the light came on for the Parking Brake. The battery was fully charged. I also started receiving messages through my Chevrolet APP that the vehicle had no air in the tires, no oil in the engine, and many other items that needed servicing. I am not sure what exact components or computers have failed yet. GM could not find any dealer to render aid and come to my location to reprogram the vehicle. It had to be towed to a dealer. With the electronic Parking Brake engaged, GM had a hard time trying to find a tow truck that wouldn't just drag the vehicle down my driveway and onto a flatbed.

VEHICLE WILL NOT START. ONLY 6900 MILES. BATTERY FINE, LIGHTS AND RADIO WORK, WINDOWS AND TAILGATE WORK. CAR TURNS OVER, BUT JUST WON'T START. READ MANUAL, DID EVERYTHING SUGGESTED. CLEARLY AN ELECTRONIC ERROR. AND, BECAUSE VEHICLE COULD NOT BE STARTED, IT COULD NOT BE PUT IN NEUTRAL. TOW DRIVER HAD TO DRAG CAR ACROSS PAVEMENT ON FLATBED. COULD NOT BE GOOD FOR BRAKES OR ANYTHING ELSE DOWN BELOW. WHEN ARRIVED AT DEALERSHIP, COULD NOT GET TAHOE OFF FLATBED. WINCH ONLY PULLS UP ONTO FLATBED, DOES NOT PUSH BACKWARDS. OILED THE FLATBED AND SHOOK THE ENTIRE TOW TRUCK BODY TO TRY TO GET IT TO SLIDE DOWN. NO IDEA WHY THIS HAPPENED. BUT A COUPLE OF MONTHS AGO SAME THING HAPPENED TO A FRIEND'S GMC VERSION. CLEARLY ELECTRONIC FAILURE IN BAND-NEW CAR.

The contact owns a 2021 Chevrolet Tahoe. The contact stated that the vehicle displayed "Reduced Power," and the rear wheels started to lock-up. The contact was able to keep control of the vehicle and continued driving. The vehicle was taken to the dealer, who completed the repair under NHTSA Campaign Number: 24V797000 (Power Train). The contact stated that after the recall repair, while leaving the dealer and driving approximately 10 MPH, the vehicle veered off to the right and ended up partially in the next lane. The contact stated that the vehicle unintendedly shifted into neutral, and the message to "Press Start Again" was displayed. The contact was able to restart the vehicle and parked the vehicle in a nearby parking lot. The contact restarted the vehicle and drove back to the dealer. The failure recurred twice while returning to the dealer. The dealer diagnosed that there was a fuel pump drive failure. The vehicle was not repaired. The manufacturer was informed of the failure and confirmed the VIN was no longer under NHTSA Campaign Number: 24V797000 (Power Train). The manufacturer opened a case. The failure mileage was approximately 69,000.

The contact owns a 2021 Chevrolet Tahoe. The contact stated that while attempting to start the vehicle, the vehicle failed to start as intended. The check engine warning light was illuminated. The contact attempted to start the vehicle several times, however the vehicle failed to start. The contact stated that the vehicle was parked in an underground parking lot. The contact stated that four tow trucks attempted to tow the vehicle; however, the transmission had to be disconnected from underneath the vehicle to remove the vehicle from the underground parking garage. The vehicle was then towed to the dealer where it was diagnosed with a failed fuel pump power control module. The contact was informed that the fuel pump power control module needed to be replaced. The vehicle was repaired. The contact was informed of NHTSA Campaign Number: 21V739000 (Fuel System, Gasoline); however, the VIN was not included in the recall. In addition, the contact stated that when the vehicle was retrieved from the dealer after the repair, the failure reoccurred. The contact stated the vehicle was started, and the vehicle was driven to the residence without the failure reoccurring. The manufacturer was made aware of the failure and a case was filed. The contact was advised to contact the NHTSA Hotline to report the failure. The failure mileage was approximately 49,064.

The contact owns a 2021 Chevrolet Tahoe. The contact stated that while driving at an undisclosed speed, the vehicle stalled without warning. The contact stated that the transmission independently moved into park (P) and the gear shifter would not move after several attempts. The vehicle was towed to a dealer who informed the contact that they could not repair the vehicle because they did not have the adequate tool to unlock the transmission. Upon receiving the tool days later, the dealer diagnosed that water had leaked into the fuel tank, which led to the failure. The manufacturer was notified of the failure and advised the contact that they would follow up. The contact was still awaiting a call back from the manufacturer. The vehicle was not repaired. The failure mileage was approximately 27,000.

Stopped for gas, then proceeded to leave store, the check engine light came on, and car immediately shut off while on the highway going ~25mph. Car would not turn back on, and then lights for fuel machine error started coming on. Car sat there trying to turn on, lights off, and then would say push start button, but nothing happened. Car began to shake and continued to try to turn on, yet nothing happened. Car was towed to home, sat for an hour and then started again, check engine light remains on.

We were driving down the road and as I tried to accelerate the car would not go. Then I realized the car said push to start. My husband tried to push the neutral button and told me to restart it but I said you have to push the brake. All this is happening as we are coasting down a main road with speed limits of 55mph and my family is in the vehicle (including 2 toddlers in the back seat). Once we coast to a gravel area off the road I push the brakes and try to restart. My dash goes all black and I try to restart. It tells me battery low push to start. The starter engages and stays engaged until I push the start button again to turn off. The screen tells me it is shutting down functions to save battery. The check engine light is on. We tried 3x with same results. Finally we get out and my parents come to us. Within 15 mins of the initial incident, my husband and dad get in and start my vehicle. My dad suspected the fuel pump. The fuel was right under 1/4 tank but not low enough for the fuel light to be on. They drove to the fuel station, approximately 2 miles away, filled up the tank and drove it to an auto parts store who connected it to a diagnostic machine and it read fuel pump. They drove it back to my home, approximately 2 more miles. The check engine light is still on. I will call the dealership tomorrow. The vehicle has 23,000 miles and is still under manufacturer warranty.

We have owned this 2021 Chevrolet Tahoe for a little less than a year. Since acquiring this vehicle, there have been multiple safety recalls and we have had to get the vehicle serviced 3 times due to electrical issues. Now for the major safety issue, was out of town waiting at a red light in a turning lane. As we were waiting, the Tahoe spontaneously turned off. All of the indicator lights on the dashboard were illuminated and my husband tried to restart the car but the engine just kept turning and the car would not start. We put the hazard lights on and wanted to get the vehicle out of the road where there was active traffic. However, the vehicle was unable to be shifted to neutral so we were stuck and also important to note the hazard lights were not working. We towed to the nearest dealership and tow driver had to apply WD40 on the ramp to get the truck off since it wouldn't shift to neutral. At the same time, my husband received an email from OnStar "An issue with the Engine and Transmission System in your 2021 Chevrolet Tahoe has been detected." Now being told by dealership this was caused by a fuel pump module cut off, causing the car to go into "limp mode" so who knows what is true. I do not feel safe driving this car nor having my kids in it. This would be a very different conversation if this happened on a highway. This vehicle should be recalled because someone gets killed.

TOOK DELIVERY OF VEHICLE ON FRIDAY, JANUARY 29TH, 2021. WHILE DRIVING HOME FROM PLYMOUTH, MA TO CONCORD, NH, I WAS USING THE ADAPTIVE CRUISE CONTROL. I WAS TRAVELING BETWEEN 65MPH AND 70MPH. ROADS WERE COMPLETELY DRY, THERE WERE NO OTHER CARS NEAR ME, AND THERE WERE NO OBSTACLES IN FRONT OF ME. I WAS TRAVELING IN THE MIDDLE LANE OF A THREE LANE HIGHWAY. OUT OF NOWHERE, THE BRAKES WERE FORCEFULLY APPLIED BY THE COMPUTER WHICH NEARLY BROUGHT THE VEHICLE TO A STOP IN THE MIDDLE OF THE INTERSTATE. AFTER REACHING MY DESTINATION I NOTICED THAT THE CAMERA IN THE DRIVER SIDE MIRROR WAS NOT FUNCTIONING. CONTACTED DEALER THE FOLLOWING DAY AND RETURNED BACK TO DEALERSHIP TO HAVE THE VEHICLE REPAIRED (NEARLY 2 HOUR DRIVE). DEALER STATED THAT THE ISSUE WITH THE CAMERA AND THE ISSUE WITH THE AUTOMATIC BRAKING WERE RELATED. PICKED VEHICLE UP ON FEBRUARY 8TH AFTER VEHICLE WAS SUPPOSEDLY FIXED. ARRIVED HOME AND DISCOVERED SAME ISSUE WITH CAMERA NOT FUNCTIONING. TOOK TO DEALER IN CONCORD, NH TO TRY TO REPAIR AGAIN ON 2/15/21 AND PICKED UP ON SAME DAY. DEALER FOUND LOOSE PIN IN CONNECTOR. DEALER STATED THAT THE CAMERA HAD BEEN REPLACED IN OCTOBER 2020 BEFORE I EVEN OWNED THE VEHICLE, SO SOMEONE KNEW THIS WAS AN ISSUE. TOOK VEHICLE ON A DRIVE ON 2/17/21. ALERT ON DASH NOW SAYS 'SERVICE DRIVER ASSIST SYSTEM.' I DO NOT FEEL SAFE DRIVING THIS VEHICLE AND MY WIFE DOES NOT FEEL COMFORTABLE DRIVING IT OR LETTING OUR TWIN 3 YEAR OLD BOYS RIDE IN IT. I HAVE A CASE OPEN WITH GENERAL MOTORS ON THIS BUT THEY HAVE DECLINED TO HELP WITH EXCHANGING THE VEHICLE. I'M THANKFUL THAT NOBODY WAS RIDING WITH ME AND THAT NO CARS WERE AROUND AND THAT IT WAS NOT SLICK WHEN THE BRAKING INCIDENT OCCURRED BECAUSE IT CERTAINLY WOULD HAVE CAUSED AN ACCIDENT. PLEASE HELP ME.

The safety system needs serviced/repaired and the alert has been present for months, and investigated by GM licensed dealership but not repaired, after several requests. The safety of drivers and passengers has been neglected and the high risk for injury and death while occupying the 2021 Chevy Tahoe. Also on 06/11/2025 the EPS Power Steering system failed, with the system alert showing up on the driver's dash. This is a cause for major concern and is noted that 2021 Chevy Tahoe vehicles have an outstanding recall for those affected. The licensed GM dealership was contacted on 06/12/2025 and again, denied the repair and/or ability for the vehicle to be operated safely. The Tahoe has been taken to the shop over the last 12-16 months and as of this day, occupants of this vehicle are at very high risk for injury and death due to the service safety system alert and the failed EPS system. I am compelled to take additional action, to prevent injury and death unless these issues are able to be immediately resolved.

The dashboard for the vehicle loses all power and does not regain power/connectivity. I've had this problem "fixed" 5 times by the Chevy dealership and the issues continues. They've replaced the entire dash, checked electrical components, changed out the electrical bank, and the issues continues. Driving down the road at 65 mph to have your dash go out is not only terrifying but extremely dangerous! I have no idea how fast I'm going, how much gas I have, no situational awareness related to vehicles that are close by... the ENTIRE dash goes out. This is a brand new 2021 Chevy Tahoe... I bought it with 4 miles on it and within a month of the purchase, the vehicle had this problem.

I’ve been dealing with an alignment issue since last June when I took in my 2021 Tahoe z71 with air ride suspension. It’s chewed up the stock tires at just 25,000 miles. It’s came out of alignment 3-4 times since then and Chevrolet is doing nothing about it. I’ve spoke with others that have the same vehicle and the same problem. I just purchased tires for $1,100 just over a month ago and I checked the alignment last week and it was out again after City Chevrolet fixed it on January 17th of 23.

I purchased a 2021 Chevy Tahoe from Autonation Chevrolet 11-3-2020. I have taken my Tahoe to the service department numerous times - as my truck has had a stubborn service safety restraint system warning error code that continues to come on and off. I have taken it to have all recalls serviced as per recall request. The service safety restraint system warning code has not been corrected after more than 5-6 visits if not more as not all visits have been logged properly. I have had this truck for less than 12 months and have experienced too many conflicting issues. I do NOT feel safe driving my Tahoe with the SRS light on. There's obviously a malfunction that you all have not been able to figure out. This is extremely dangerous!! What if I were to get involved in an accident or collision and my airbags do not deploy? I may have a problem with the airbags themselves, a loose wire, a faulty seat belt buckle, or a problem with the engine control unit which is causing the airbag warning to come on. There is obviously a major problem. I am putting my children and family at risk. I have not received an email or phone call from anyone at Autonation Chevrolet. The service safety restraint system warning code has come on and off 7x since I was last at the dealership (9/10/21) and I have pictures to support.

After turning off a major highway (65 mph) onto a side road after driving for approximately eight hours, the transmission shifted into N...on its own. The engine continued to run but the vehicle would not move. We could not shift into P, R or D. When attempting to shift into P, R or D, an error message appeared on the dashboard that said "Conditions not right for shifting." We turned the vehicle off and then back on and the error cleared. The vehicle continued to drive "normally" for another two hours and the end of the journey. Had this happened in the middle of heavy traffic, a deadly accident could have occurred. Mileage on vehicle is 4,900. Date: 13 July 2021 at 1800 hours. Dealer has been notified but has taken no action to date. There were no warning signs or messages prior to the incident. I'm guessing it is a sensor or software problem related to the engine/transmission. There is a paragraph in the Owner's Manual that states: "Under certain conditions, propulsion will be disabled. Try restarting after the ignition has been off for two minutes." It offers no other explanation as to what "under certain conditions" means.

May 6, on vacation, sudden engine failure, valve lifter and lifter rod. GM told dealer only to replace lifters on one side, Cyl 5 was problem. May 24 car ready, pickup and 68 miles later same problem while on I95 in SC. Did not trust engine and drove in right lane. GM played Russian roulette with our safety and that of other people on road. Sudden engine failure can result quickly in accidents with possible casualties at highway speeds. After pulling of I95, called GM Road assistance. Towed to dealer. They were of no help. Family member from CHS , SC send trailer to bring us all the way home in New Smyrna Beach Fl. While on phone with GM Road assistance we idled the engine to keep cell phone charged. Suddenly engine stopped and on restart heavy smoke came from tail pipe. May 25 car delivered to dealer in NSB where we bought car. May 27 they informed us same lifter/rod problem occurred on other side. Look at Chevroletforum.com and you will see 2021 6.2 engine having these problems within 8000 miles. Have been in contact with GM customer Assistance Center since May 14. We do no longer trust this engine. GM already decided to redesign the lifters as they are aware of the problem. NO recall has been issued yet. Consider this a serious highway safety issue. Do not consider the car a lemon, but yes the engine sure is.

On the morning of 1 Oct, I had left the house here in Cedartown GA to head to a meeting in Atlanta with a customer. I had started the truck 15 minutes before departing just as we have always done to our vehicles over the years to allow them to warm up properly. I departed Doyle Rd onto Hwy 27 South heading towards interstate 20. About 15 miles from the house I was at a steady cruise speed of 45 MPH, no traffic, acceleration or deceleration and the stability control system light came on, one second later the engine light came on and the engine all of a sudden started making a loud grinding and clunking noise. I was on the inside lane so let the car pass next to me and started to pull over onto the shoulder of the road. I immediately shut off the truck, popped the hood and got out to inspect what might have happened. There was no smoke, I checked all the fluids and all were at the normal ranges. The engine oil was at its full mark as well. The oil was just changed a month early and still of the same color as it was new. After checking, I got back on the truck, attempted to start and it ran for a few seconds then I immediately shut it off after hearing again the loud noise. A Polk county police officer stopped to check on me and offer assistance as well as some tow companies to call. Eventually after about at hour a tow truck showed, we tried to momentarily start the vehicle so it could be put in neutral but the engine had locked up. I’m glad I got off the road when I did, otherwise I feel I could have been driving and experienced that sudden lock up and loss of control which could have been a serious accident with log trucks up and down that highway all day long. This vehicle is my wife’s primary vehicle and always has been maintained to a T just like all of our GM vehicles have. My daughter just graduated and drives our passed down 2005 Tahoe with over 300k miles that we bought in 2006. This has never happened to us and I feel it is a serious safety issue on the 6.2

I drive a 2021 Chevrolet Tahoe. This occurrence has happened on three separate occasions and each time it was raining outside. The first occurrence, my vehicle was running and parked in the rain after driving approximately 5 miles. While parked my check engine light came on with a series of warnings: service ECS, service traction control, forward collision system unavailable, reduced acceleration drive with care. Two days later it went away. The second occurrence, I was driving in the rain for approximately 22 miles and briefly parked with my vehicle running. I then turned out of a parking lot and began to accelerate to speed up and the check engine light came on along with all the same messages, this time my vehicle acceleration had seized and wouldn’t go faster than 30 MPH. I slowly drove back to the parking lot and turned the vehicle off to let it sit, disconnected the battery and reconnected, and the issues went away. The third occurrence, I was driving on the highway in the rain for approximately 10 miles, I went to merge into a right hand lane, I took my foot off the acceleration peddle to slow down for the merge and as I slightly turned the wheel to the right, my vehicle spun out three times, throwing the back end of the Tahoe into the inner highway wall, which spun me again to face the right direction, next to a highway exit. I then went to accelerate to exit the highway but noticed my vehicle would only coast and realized my engine had cut out at some point. I started the vehicle and exited the highway and parked in a parking lot only feet away from the exit. After I parked I noticed all the same warning lights came back on. Each occurrence happened on [XXX], [XXX], [XXX]. My vehicle was towed to Gunn GMC Collision Center in Selma, Tx and will be looked at on September 3, 2025 around 8:00am. • What component or system failed or malfunctioned, and is it available for inspection upon request? The steering column lock system malfunctioned, triggering a “Service Steering Column Lock” alert. This failure caused a sudden loss of steering control. The vehicle and components remain available for inspection upon request. In addition, the incident appears to have caused damage to the trailer harness wiring. • How was your safety or the safety of others put at risk? The malfunction placed my safety and the safety of others at significant risk. When the alert appeared, I lost steering control of the vehicle and nearly collided, which could have resulted in a serious accident. • Has the problem been reproduced or confirmed by a dealer or independent service center? The problem has not yet been reproduced or formally confirmed by a dealer or independent service center. • Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives. However, I am prepared to make it available for immediate inspection. • Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. A “Service Steering Column Lock” message appeared at the time of the incident, immediately prior to the sudden loss of steering control and the resulting issue with the trailer harness wiring.

I am submitting this complaint regarding my 2021 Chevrolet Tahoe (VIN: [XXX] ) which has developed a severe transmission shudder at both low and highway speeds. The issue has been confirmed by my Chevrolet dealer, who provided an estimate of $5,492.18 for torque converter and valve body replacement. This failure is identical to the torque converter shudder problem documented in GM service bulletins and special coverage adjustments for other GM vehicles of the same make, model, and model year. However, my VIN is not currently included in those programs, leaving me without coverage despite experiencing the exact same defect. I believe this condition poses a serious safety risk, as the shudder occurs while driving at highway speeds, causing vibration, hesitation, and potential loss of control. These symptoms could lead to accidents if the vehicle fails to respond properly during acceleration or lane changes. Given that this appears to be a widespread defect affecting multiple GM vehicles, I respectfully request that NHTSA investigate and consider expanding any related recall or special coverage programs to include affected VINs such as mine. Please let me know if further documentation, service records, or dealer estimates are needed to support my complaint. Thank you for your time and for working to ensure vehicle safety. We are apart of the safety recall with the potential brake and engine issues, we have contacted the dealership and they have NO remedy at the current time for our defective vehicle. We contacted GM to request a buy-back due to the safety issues with the vehicle, told we did not qualify BUT they have no remedy for our defective truck. We do not fell safe in the vehicle, the brakes are sticking and the engine is making weird noises. We are currently seeking legal representation because we are being forced to drive around our family of 5 in an unsafe vehicle with NO remedy or option to trade it in.

Our vehicle was rear ended and the front passenger seatbelt did not lock or (possibly) locked too late. The front passenger's face hit the dashboard, as well as their knees and feet. The vehicle that hit ours was estimated to be travelling at about 45 mph and we were rolling at around 5 mph in traffic. The failure has not been able to be reproduced by the collision center, the sensor callibration contractor, nor the dealership. No sensors showed any failures and the tension appears to be correct on the seatbelt. We have been unable to determine why the seatbelt failed during this incident and if the vehicle will react appropriately in the event of another collision.

Instrument panel has failed twice and is unresponsive. The gauges move and stay to the left after moving left to right. All lights begin to cycle through error messages. I have video of the issue both times it happened. When it occurs, you don't know your speed you have to guess, the full tank reads empty etc., and it is dangerous to back up. The vehicle has been diagnosed by one independent mechanic and is currently at the dealership. There was no warning before the instrument cluster failed other than a cycling through of warning messages service transmission check 4X4, service suspension system, service Power Steering Drive with care. -The center screen goes in and out. -When in reverse, the rear camera is black, the screen blinks intermittently. - Dealer has stated that the wires are put on incorrectly- if that is the case, I am not sure who would have done that other than the manufacturer. -previously reported there was a strong gasoline smell in the cabin that made it really hard to breathe. Dealer corrected. Right passenger side is intermittently wet with no cause from the interior creating stale smells, making it hard to breathe. It is important to note this is the side where the electrical wires are "put in incorrectly" according to the dealer. I also have a video of what occurs

While traveling on the highway one day, my 2021 Chevrolet Tahoe began warning lights on the dashboard. Shortly after, it running roughly,, and jerking, so I pulled over to investigate. There no visible smoke, so I drove to the nearest auto parts store, where they ran a diagnostic test that returned a code of P0306. I then took vehicle back to the dealership where have always received oil changes and repairs. I have never visited any other dealership, relying on certified GM/Chevrolet dealerships. After inspecting the vehicle, they informed me that the lifters needed replacement advised that it would be best to replace the engine entirely. Upon further research, I discovered that issue has been affecting many people in our area and across the United States. This needs to be addressed, as the vehicle is only three years old, purchased brand new with zero miles, and I have not done anything out of the ordinary to cause this problem. The vehicle being 70,000 and 12.000 for repairs is insane.

GM does not install TPMS devices in spare tires. I had a flat tire in snowy conditions 100+ miles from the nearest place for assistance. After putting on the spare tire the DIC (driver information center) displayed no air in tire. That’s because a TPMS doesn’t exist in spare tires. I started driving a my four wheel drive turned off. I turned it back on and after a very short distance it turned off again. Since the vehicle determined I didn’t have air in one of the tires (spare previously mentioned) the four wheel drive shouldn’t work to prevent damage. I needed the four wheel drive in the snow so I had to proceed at approximately 25 mph. This is a safety issue for not only my vehicle but others on the roadways. I contacted the dealer that had no idea about the problem and neither did the national GM customer support center. There is a tool that “tells” the vehicle where each tire is located so that is low tire pressure occurs the driver would know which tire needs attention. If a TPMS was installed from the factory and a flat occurred, spare installed then the four wheel drive would function. If a flat subsequently occurred the driver would still be alerted but wouldn’t know tire was involved. Visual check would find it. Either this or have GM provide an override so the trip could be continued safely. I believe that this situation applies to any four wheel drive vehicle not just GM and should be resolved if the NHTSB would initiate an investigation.

It’s happened about 10 times since I’ve owned the car but the dealer can’t recreate it so we are out of luck. The vehicle accelerates on its own. Scenario 1…with constant light steady acceleration, it will lurch forward like I’ve stomped on the accelerator a bit, for about a second then go back to my normal input. Scenario 2, same lurch but this time while coming to a stop at a stop sign WITH MY FOOT ON THE BRAKE PEDAL ONLY. scenario 3…I’ve been using the adaptive cruise control and had it disengaged by braking, coming to a stop, then parking and turning the vehicle off. Then after getting back in and starting up and slowly pulling forward, the cruise control suddenly and abruptly renegages and attempts to accelerate to the last demanded setpoint. Image this scenario pulling out of a spot at a crowded gas station with pedestrians. I almost ran someone the hell over just now at the gas station and I’ve had enough. The dealer won’t do anything and when I called Chevy engineering they put zero effort into it, it was a 20 second phone call stating if it can’t be reproduced at the dealer there’s nothing they can do. This is absolutely completely 100% dangerous and unacceptable

2 ISSUES - 1) vehicle repaired 5 times for "transmission" problem by replacing wiring harnesses. On 5th attempt, the GM factory part or the technician caused the Tahoe to catch fire. Worst part is that it happened 1 hour after test driving and parking the car at their dealership, thankfully, I would normally have driven it home and it would have destroyed my home too. GM investigated and was unable to determine cause. 2) each time the "transmission" error occurred the GM car went into what they call the "limp" mode. The car immediately loses speed control and drifts to about 35 mph until you have control of the accelerator. I was on an interstate going +70 mph to pass a semi and immediately after passing the car went into Limp Mode, the semi nearly rear ended me and then passed me blowing his horn and cursing me. GM could certainly correct this latter issue by creating a warning on the screen that gives the driver ample time to pull over and address the problem as they do when DEF fluid (this was a diesel) is low and will give warning to add DEF or reduce speed after so many additional miles are driven. The former issue needs to be addressed immediately too, I've noted 4 other drivers on social media that had similar fires. Both of these issues could have resulted in serious injury to me or my family.

Myself and my passengers were injured and received and continue receiving medical treatment for our injuries; our safety was definitely placed at risk. Before being involved in this accident, warning alerts/lamps would appear on the vehicle’s dashboard indicating to “service the safety restraint system” and also to “open then close passenger window” which in turn led me to take the vehicle in for service however the dealer said it could not be duplicated nor did the computer show any alerts. After the accident, I Contacted General Motors and filed a claim for the non deployment of the airbags, they in turn are conducting their investigation.

I have an on going problem with the emissions system on my 2021 Tahoe. The vehicle discharges raw diesel fuel and DEF from the exhaust tail pipe during cold startup. This discharge is overwhelming and is so strong it can be detected by smell from over fifty feet away. In addition to this issue the fumes enter the inside of the vehicle and at times it is so bad you need to open the windows. I believe this problem is unsafe as it poses a fire risk and a CO exposure risk to the vehicle occupants. The vehicle has been in the shop repeatedly for the past eighteen months with various parts replaced. To date none of the repairs have fixed the problem. In February GM issued a statement saying that this is normal operations.

The vehicle was manufactured with a push button transmission gear selector. If there is an electrical issue(ie. dead battery, electrical short, engine failure, etc) you cannot shift transmission gears. If I pull into my garage, notice a burning smell coming from engine area, possibly an electrical short, I cannot put vehicle in neutral to push out of garage in case it does erupt into a fire and not burn down my house. Recently my dead battery caused the tow company to lift the back end and pull it our of garage. General Motors did not install an override to put transmission in neutral in an emergency. The transmission will only shift if the engine is running. Imagine driving on a road or stopped at a traffic light and the engine stalls, will not restart, good Samaritans offer to push you off the road, but can't shift to neutral. Dealerships have a hydraulic lift they place under rear tires to move vehicle around service area. As a retired law enforcement officer, many times we had to move vehicles out of the way until wrecker serviced arrived. I feel this is definitely something that has to be addressed and corrected before major damage occurs or lives are endangered.

While driving on a residential street during daylight hours, on a clear day at approximately 20 miles per hour the right wheel on my car dislodged.  This caused me to lose control of steering the vehicle and strike a parked car.  There were people walking about on the sidewalk at the time and other cars traveling.  When I exited the car I saw that the passenger side was completely on the ground and there were broken pieces and liquid coming from underneath the car.  The axel was broken with a clean break.  The car was then towed to the dealership where I was informed it could not stay.  My insurance company then towed it to their repair shop where upon assessing the damage they could tell that this was not caused by a collision.  General Motors along with our independent investigator examined the parts and vehicle. Leaving me without a vehicle for 4 months.  Our Insurance investigator determined this was a defect in manufacturing and GM did not agree.  General Motors will not communicate with us any further on this matter.  Some of the repairs that were made was the left front and CV axel, also the left front aluminum steering knuckle was missing.  This should be investigated for metallurgical discrepancies by an independent lab.  While the car was being inspected there was a recall to the power steering that may apply to the vehicle.  I would like for the NHTSA to investigate this issue.  The car has been repaired and is in my possession and my insurance company has all of the reports, documents, and parts regarding the investigation.  I also have pictures immediately following and a witness to the incident. Had a been traveling on a highway at a higher rate of speed this could have been fatal.  Your assistance in this matter would be greatly appreciated.
