While driving on a city road below the speed limit, braking was compromised leading to loss of traction and failure of the ABS system. During the incident, the brake, ABS, and engine warning lights all turned on and a collision was only avoided due to pumping of the brakes. Further investigation by a mechanic revealed a failure of the brake actuator system. Failure of this system has increased the stopping distance of the vehicle considerably and makes braking unreliable and unpredictable.

While driving 60 mph in highway, ABS, traction control and brake lights lit up and vehicle lost its effective braking power. There were no prior warnings prior to this failure. It was a sudden incident. Since it wasn't rush hour, I could safely reduce speed and bring the car to stop. Situation could have been much different if it was normal hours. After taking the car to dealership, it indicated DTC code 1391, Brake Assembly Failure. The car was fixed at dealership at cost of $3,000. When researched further, it appeared that there is broader pattern of failures. Numerous reports have been made by owners of 2010–2015 Toyota Prius, Prius PHV, 2012–2015 Prius V, 2012–2014 Camry Hybrid, and 2013–2015 Avalon Hybrid vehicles, all of which share the same brake booster pump assembly design manufactured by Advics Co., Ltd. Dealerships, including Capistrano Toyota and Claremont Toyota in California, have previously petitioned NHTSA regarding this very defect. Their 2019 letter to the agency documented that Toyota: * Knowingly excluded hundreds of thousands of hybrids from prior safety recalls (DOH 13V-235 and KOL 19V-544) despite identical failures. *Issued “post-failure” repair programs (Warranty Enhancements ZJB, ZKK, ZKC, ZLM, 20LE03, 23TE06 and 23TE07) only after owners experienced brake failure. * Identified the same DTC codes (C1252, C1253, C1256, C1391, U0293) in both recalled and unrecalled vehicles—codes that correspond to accumulator leaks, loss of brake pressure, and failure of brake assist and stability control systems. * Admitted through Technical Service Bulletin TSB-0130-19 that these defects caused loss of hydraulic pressure, increased stopping distance, and total loss of brake assist in affected hybrids. In short, the defect pattern across multiple hybrid models is identical to those already recognized by Toyota and NHTSA under prior recall campaigns, but for reasons unexplained, Toyota limited the recall population and left many consumers unprotected

2012 Toyota Prius. The brake system malfunctioned and my daughter was almost in a wreck. The ABS, traction control, and brake warning lights on the bashboard all illuminated at the same time. The brake pedal feels soft and is less responsive requiring more effort to stop the vehicle. The brake actuator assembly is making a hissing noise. Ran a ABS test using a scan tool and received a C1391 error code. The Code points to a leak within the brake actuator system. Upon research I have found that this is a very common and costly repair that Toyota has known about for many years. Toyota issued a customer support program related to this issue that ran for 10 years or 150,000 miles from the vehicle's date of first use. This car, being a 2012, is beyond the support program. I contacted Toyota, hoping they would do the right thing and cover the repair. I was told that they would not cover the repair or supply the parts to cover the repair. I told the Rep. for Toyota that I did not feel like it was fair for Toyota to continue to put millions of drivers at risk for a product that they clearly realize for years now is defective and could hurt or kill someone. The code and the braking issue did not appear until after the customer support program ended and therefore we had no warning or way to anticipate this braking issue or the danger to my daughter and anyone else riding with her in the car or on the roadway near her travel routes. I traveled 300 miles, rented a Uhaul car tow trailer and brought the car back home. My mechanic confirmed the code C1391 and the fact that the brake actuator has failed. The mechanic has ordered the parts (the same defective parts) to replace the broken ones. Toyota is still selling the same part as a replacement part clearly knowing that it is not safe and no warning came with the replacement part explaining the potential for a braking issue. Based on internet research this same part is in many 2005 to 2013 Toyota and Lexus

While driving on the freeway I applied the brakes and the brakes suddenly felt soft and not very responsive. At that moment 3 dashboard lights turned on: ABS, BRAKE, and the slip indicator lights. I carefully got out of the freeway, parked the car, and had the car towed to an independent mechanic. The mechanic detected brake booster and actuator problems, code C1391.

I was driving on the internet doing 60mph and there was a very loud bang and then a shaking of the parts under the hood. I was right next to an exit and coasted off the exit to a hotel parking lot. I had car towed to my mechanic. He said that the piston went thru the engine and the engine is done and needs replaced. I am having a hard time trusting Toyota again to purchase another Prius. I have had Prius for over 10 years and never had anything like this occur. I had no warning signs and no dashboard lights lit up until after all of this occurred and scared the life out of me then the engine light came on.

Had blown gasket repair last year (which was fixed by my local Toyota Dealership and all cost paid for by myself). Dealership acknowledged that there were known issues with failures due to clogged EGR's and out-dated manifolds. Was never recalled and did not know about the issue until after the incident. Wanted to report issues since it appears countless other have been in the same situation with gen 3 Prius. Toyota needs to be offering EGR cleanings and updated manifolds at no cost and reimbursing customer's for previous repairs.

The vehicle was taken to the dealer who informed the contact that no issues were found; however, the dealer replaced the headlight bulb. The contact stated that the headlight bulb was replaced several times; however, the failure persisted. The vehicle was not repaired. The manufacturer was not notified of the failure. Headlamp Lens Issue Vehicle Manufacturers Must Correct ISSUE 1. The increase in pedestrian-vehicular accidents and fatalities is, at times, related to the driver not seeing the person in time to stop. Not seeing a person in time may be related to poor street lighting or a decrease in front visibility caused by fogged headlamp lenses. Current nighttime traffic has a variety of vehicles operating with varying front light visibility. 2. For certain vehicles, the lens alone is replaceable. With other vehicle models, the entire front lamp must be replaced. The exorbitant cost of replacing the front lamp is causing some people to opt to drive with densely fogged front headlight lenses risking pedestrian safety. 3. Some car dealership offer a "defogging" cleaning service for greater visibility but does not guarantee to restore the car's front illumination to minimum required safety standards. Recommendation Require vehicle manufacturers to make front headlights with detachable lenses, or make the lenses fog proof. A replaceable lens is cost effective for all income individuals. Making lens replacement affordable may decrease the number of pedestrian-vehicular accidents and fatalities. Discussion The headlight lens of my car became increasingly fogged, overtime, decreasing night visibility ahead. The Toyota dealership offered a $300 cleaning service for both lenses or replacing the headlamp at about $300 each; $600. I opted for the cleaning. When completed, the headlight lens of my vehicle looking much less fogged. When I asked, the dealership told me that their cleaning service does not guarantee compliance with minimum front safety illumination for night driving. I was told that my car does not come with removable lenses. To ensure minimum safety front night visibility, I purchase new headlamps total costing about $300 each; I spent $900 because the $300 cleaning service was completed and not deductible. Purchased 2012 Prius used. Replaced low beams and high beam headlights shortly after. Passenger side low beam is no longer working (bulb was replaced 3 months ago). Upon googling causes saw 2010-2011 Prius had a recall for recurrent headlight issues, the wiring harness was replaced in them. It seem the 2012 is having the same issue in the 8 pages of complaints on exterior lightning that I saw. Toyota should extend the fix to include 2012 Prius.

The headlights are not functioning properly as a result of severe discoloration of the lens material. This creates unsafe driving conditions at night since the road is not properly illuminated, especially in areas with inadequate street lighting.

On 12/25/2021, I was driving in the country side under darkness and under heavy rain and I had a hard time seeing the road. There were no road lights so I could not make out where the divider, curve of the road or side of the road was. It was quite a safety hazards because cars were coming from the front, or a few inches off the road would be dirt field. I discovered that the headlamps of my 2012 Prius are full of water bubbles and the water bubbles are blocking and distorting the headlights. I have pictures and videos of the headlights. It is six days already and the water bubbles are still there, and it is not safe to drive out to the field at night or where there is no road lights, with such minimal and distorted lights. I found on the web that a few people experienced water bubbles in their headlamps over the years, but don't know if they reported to NHTSA. My car has no accident and no modification on the front of the car. Toyota representative and dealers said my car is out of warranty. There were no indications or symptoms of water bubbles in advance, until rain leaks into the headlamps. I did not notice if this happened in previous raining seasons, as I didn't drive out much at night.

Odometer Fraud. The contact’s son recently purchased a 2012 Toyota Prius. The vehicle was a private sale. After the purchase, it was discovered that there was a mileage discrepancy based on the Carfax report. At the time of purchase, the vehicle displayed 137,000 miles. However, the Carfax report listed the mileage as 281,586 miles, and the current mileage on the vehicle was 151,103.

The hybrid system fails to enter “Ready” mode with no SMR engagement audible from the junction box. Dashboard displays ABS, brake, engine, and master warning lights. OBD codes include U0142, U0155, and previously U0073 . Rear brake lights function normally when pedal is pressed. Dealer said to pay 250$ for diagnostic and diagnosed incorrect voltage from brake light switch and quoted $348.94 to replace, but refused to proceed with open safety recall J0V / 18V684 (NHTSA 18V684000) citing inability to achieve “Ready” mode. Recall covers inverter failure leading to loss of motive power. Dealer declined to jump 12V battery or use external power to perform required software update and inverter inspection. Vehicle remains inoperable and poses safety risk due to sudden power loss potential. Component (inverter G9200-47141) available for inspection. Problem confirmed by dealer diagnostics. No police or insurance inspection. Warning lamps appeared after 12V battery replacement three months ago.

The accelerator wasn’t pushing down after I turned a corner then the lights in attached photo appeared. The power inverter needs to be replaced and since my car had the J0V software update in 2/2019, and the recall update came out in 3/2019, it was not linked to my vin as it should’ve been. The update to J0V (attached) should have been posted w my vin, correct? They are fixing it under customer support but refuse to tow it or give me rental bc they are saying it isn’t a recall issue, when the document below says it applies to all cars that needed J0V. I want to be sure all cars that had recall service before the March2019 update get this update linked to their vin.

I was driving going 45mph when my car shut off and wouldn't start back up. It almost caused an accident from it stopping in the middle of the highway. The master warning light, brake system warning light, warning buzzer ABS warning light and the slip indicator light came on as also the message center reading to have the hybrid system checked immediately. I did have ot towed on for a diagnosis and it came back saying i needed a new inverter power module this will cost $2000 . i cant afford that. I read all over the internet how there is. Safety recalls. When i called toyota they said there are no recalls so now im stuck paying this. Not happy at all!!! If there are anymore 2012 prius with this problem please write a complaint so we can get this on recall and have the company cover the cost of repair.

I was driving with my 10 year old son on local city streets. I was approaching a busy intersection while going approximately 35 miles when my car just turned off with an error message about the hybrid system failing. I stopped the car and put it in park while trying to turn the car back on, but I kept getting the same hybrid system error message. While the stoplight was red, I got my son out of the car and over to the sidewalk because this was a dangerous situation where anyone not paying attention could've slammed into my stalled car (see picture of black Toyota Prius). I called AAA to get it towed. About 20 minutes later, a cop car pulled up to block my stalled car and then got it pushed to the side of the road (picture attached) because of how dangerous it was sitting there blocking traffic at a major intersection. My car was taken to Cabe Toyota dealership where they diagnosed it with a failed IPM card, which would cost around $4,000 to replace. I did a simple Google search and found that 80,000 Prius from 2012 - 2014 have been recalled for this EXACT reason. There have also been class action lawsuits surrounding this IPM failure due to its extreme safety risks to anyone inside a car that will just die in the middle of driving it, especially when their fail-safe system does not get triggered with this particular IPM failure. As of right now, the car is available for inspection because it's still at Cabe Toyota. The invoice shows that the IPM card needs to be replaced. There were NOT any warning lamps, messages, or other symptoms prior to the car shutting off while driving it. It had been running just fine.

I contacted Toyota about the peeling paint on my car. The Toyota representative told me to file a complaint with your agency. The paint is peeling on the roof of my car. There are many similar cars with the same problem. Toyota is unresponsive.

2012 Prius has a fault from the the manufactor where the electronic brake abs actuator and pump, which are two seperate parts working together, are faulty. Various lights on dashboard illuminate brake, tire pressure, abs, and traction control. When this occurs the car becomes very unsafe to drive as the brakes become almost non existant because the braking system on the car is electronic and controlled by both the pump and actuator. This makes the car dangerous to drive, even in short trip conditions as the brakes could fail to engage at any time. The dealer confirmed that it is a known issue on this model Prius and did not cover the cost to fix it even though the car is not safe to drive. This is dangerous and should have a recall so the car can be safe to drive on roads and potenially save lives if the brake system fails to engage,

My letter to Toyota: I'm writing this in regard to an inverter issue my 2012 Prius C recently experienced. While driving last month my car completely shut down, no warning and thankfully cars avoided hitting me and I was able to get from the middle lane to be able to coast to a side street in which I had to be towed. It turned out it was an inverter issue which had to be replaced. Granted my vehicle has less than 135k even though it is a 2012. I have done research and found models of Prius had the same exact issue(s) which was part of a warranty enhancement and I truly think my vehicle should be included. I want this to be looked in to and addressed. I will be contacting NHTSA also in regard to this. I filed with Toyota and ref#230323001904

I was driving on freeway when my vehicle shut off and wouldnt start again. It almost caused an accident thankfully not. I paid to have car taken tothe repair shop Viva Toyota in Las Cruces NM and they inspected it and gave me a quote on repair as well what was causing the problem. There was a saftey recall for the PCU and i believe the inverter is causing the failure. please help me to get this figured out.

The front windshield wiper no longer performs on high but does perform on the lower settings. We experienced very heavy rain yesterday for the first time in a while and needed the high setting to adequately visualize the road and vehicles. It once switched to high the wipers were inoperable and had to be reduced in speed to even function. This is a significant safety concern as it drastically compromises your visibility in heavy rain conditions, such as those we experienced. Yes it is available for inspection upon request.

I BOUGHT A 2012 TOYOTA PRIUS PLUG-IN. THE WINDSHIELD CREATES A GHOSTING EFFECT ON LIGHTS, ESPECIALLY AT NIGHT. LIGHTS FROM OTHER CARS AND STREETLIGHTS ARE GLARING THROUGH THE WINDSHIELD WITH A GHOSTED IMAGE ABOVE AND BELOW SO I SEE THREE LIGHTS. THE EFFECT IS SO PRONOUNCED THAT IT IS IMPOSSIBLE TO READ LIT HIGHWAY SAFETY SIGNS AT NIGHT BECAUSE THE GHOSTING OBSCURES THE LETTERS COMPLETELY. THE LIGHTS LOOK FINE THROUGH THE SIDE GLASS, SO IT IS CLEARLY AN ISSUE JUST WITH THE WINDSHIELD. I CONTACTED THE DEALER AND THEY DISMISSED MY COMPLAINT. The front and driver side air bags did not deploy (although the passenger side air bag did deploy). Initial impact came from behind, toward the passenger side, causing the vehicle to turn fully sideways, where the vehicle was then t-boned on the driver's side. The lack of driver side air bag deployment resulted in a lot of glass making it into the left side of the driver's head.

The dealer was contacted. The contact stated that the failure had occurred on several occasions. The vehicle was taken to Nico's Auto Repair where it was diagnosed and determined that the brake booster and accumulator assembly needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure but offered no assistance. The approximate failure mileage was 140,000.

After inspection I was told four things that are very disturbing: The steering shaft needs replacement; The cost will be more than $1100; There is no warranty or recall or cost coverage; The car is not reliable and should not be driven any more than essential until fixed (and they don’t know when the parts will be available). Although the car is out of warranty based on age, this car has only 66,525 miles after almost 9 years, and my wife and I have been the only drivers and not on any dirt roads. I have been told by an independent repair shop this is a known problem with Prius and should be a recall item since it is dangerous to keep driving. This is especially troubling since the dealer could not fix it when I was there because they have to order parts and they don't know when the parts will be in.

The contact owns a 2012 Toyota Prius. The contact noticed that there was a significant amount of water on the floorboards from the windshield whenever it rained heavily. Additionally, the contact stated while at a traffic sign on a hill, the vehicle would roll backwards while in drive when she took her foot off the accelerator. The contact did not notice any warning lights. The contact also noticed that while parking the vehicle, whenever the PARK button on the dashboard was not depressed while the vehicle was running, the vehicle would roll forward. The contact took the vehicle to the local dealer, where it was not diagnosed and was not yet repaired. The manufacturer had not been informed of the failure. I want this recall repaired as presently there is a safety risk. There are phrases in this recall of "1 unrepaired recall" and "Recall incomplete".

I WAS DRIVING MY 2012 PRIUS C AND I'M MIDDLE OF TRAFFIC MY CAR LOST ITS POWER, THANKFULLY I WAS ABLE TO PARK THE CAR. I GOT IT TOWED TO THE DEALERSHIP AND THEY TOLD ME IT'S THE INVERTER. I ADVISED THAT I HAD READ ON RECALLS ON THEM THEY SAID MY CAR DIDN'T HAVE A RECALL THEY ARE WANTING ME TO PAY 4K FOR A NEW INVERTER ( WHICH I KNOW IT'S THE SAME ISSUE OTHER 2012 PRIUS C HAVE HAD ISSUES TOO). SPOKE WITH BOTH A TOYOTA DEALER AND TRANSMISSION SHOP WHO SAID THE CVT FAILURE ON THIS MODEL YEAR AND THE FOLLOWING SEVERAL IS AN EXTREMELY COMMON OCCURRENCE. THIS IS A $5000+ REPAIR FAIRLY EARLY IN THE LIFE OF THE VEHICLE. THE TRANSMISSION SHOP SAID THIS WAS DUE TO POOR INITIAL DESIGN.

My wife hit a deer at highway speed. The airbags deployed. The drivers side lower airbag deployed in the footwell. The driver was unable to get to the break paddle to stop due to the airbag at het feet. She had to use the emergency brake to stop the vehicle. That worked, but she had limited vision of the road ahead and it took some time for her to think of using the emergency brake.

IT STATED I WOULD BE NOTIFIED WHEN THE FIX WAS AVAILABLE. 2 1/2 MONTHS LATER, I'VE YET TO HEAR FROM TOYOTA AND MY DEALERSHIP STATES THEY HAVE NOT RECEIVED REMEDY PARTS OR PROCEDURES FROM TOYOTA. ALL THEY COULD DO WAS GIVE ME A PHONE NUMBER AT TOYOTA CORPORATE. I CALLED THE NUMBER AND THE PERSON HAD NO FURTHER INFORMATION ON WHEN A FIX FOR THE PROBLEM WOULD BE AVAILABLE. This is a very common problem with this specific model, but remains low due to the low numbers the solar sunroof packages was produced. Afraid the sunroof will leak if used, so it stays inoperable in order to not damage broken seals.

SYMPTOM IS THAT DOOR DOES NOT LOCK OR UNLOCK FROM KEY FOB OR LOCK CONTROL ON DRIVER'S ARMREST, ALSO FAILS TO LOCK AUTOMATICALLY WHEN SHIFTING INTO GEAR IF THAT OPTION IS SET. DOOR MUST BE LOCKED AND UNLOCKED MANUALLY USING THE ROCKER SWITCH IN THE INSIDE DOOR HANDLE. CLEARLY THERE WAS A BATCH OF DEFECTIVE PARTS USED DURING MANUFACTURING.

THE REAR HATCHBACK LATCH IS WEAK AND THE LATCH ASSEMBLY NEEDS TO BE REPLACED. THIS PREVENTS THE REAR HATCHBACK DOOR FROM BEING OPENED OR MAKING IT DIFFICULT TO OPEN. A DEAD MOUSE WAS FOUND IN THE CABIN AIR FILTER. THE DEALER CHARGED ME TO CHANGE THE FILTER AND CLEAN OUT THE AIR SYSTEM. I THOUGHT IT WAS A FLUKE. I CONTACTED TOYOTA THE DAY AFTER THE INCIDENT AFTER BEING SEEN BY THE ER. I SPOKE WITH A CLAIMS ADVISER ON DEC. 1, 2014 WHO ASKED ME TO EMAIL PICTURES OF THE DRIVER'S SEAT AND PICTURES OF MY BURNS. I WAS THEN CONTACTED DEC. 9, 2014 BY TOYOTA TO BE TOLD A TOYOTA INSPECTOR WOULD INSPECT THE VEHICLE ON DEC. 18, 2014. Wednesday morning I went to start my car to go to work. I noticed the brake would not go down. It was difficult to press. I was unable to go to work safely. I brought my car to a mechanic where I was informed it was a faulty brake booster. When I researched online, I saw many many other Toyota owners went through the exact same issue. There is no recall but it’s a huge issue and a a safety problem.

While my daughter was driving brake pedal faded and rear brakes locked up causing her to lose initial control of car . Steering felt tight . Many warning lights , check hybrid system , brake lights , steering warning lights , along with abs lights came on . Took car to Toyota dealer to diagnose it needs brake booster and master cylinder module . Service consultant said known issue with TSB but it had expired . Many , many complaints of this same incident of Toyota and Prius forums .

Our 2012 Toyota Prius experienced brake failure due to the Accuator and Accumulator. I contacted Toyota Corporate office, which advised it is a known problem and had a customer support program that expired last year. The Toyota rep advised me to not drive the vehicle because of the brake failure and opened a ticket advising it is a known issue. Took car in 9/11/15 for software update for motor generator ECU and power management ECU as recall letter suggested. This is apparently common and needs to be corrected without cost to driver.

Bought the car from a dealership in january 2022 with 158,000 miles and heard buzzing noise that I was told was normal with hybrid cars.no codes showing on car and passed dealer inspection.Driving down a highway and heading to a stop brakes are hard to mash and caused skidding. No brake lights or abs lights are on. Couple days later brake lights and traction lights come on and then went back off. Take to Tires Plus and I am told I have to schedule appointment with toyota dealer as they do not have the software to diagnose. Take to toyota dealership to see codes 1391c for brake system. After searching issue there was a recall/customer warranty extension for replacement of this part for 150,000 miles /march 19,2022 whichever comes first. This did not show up on a report for dealer to inspect before selling vehicle and causes a huge safety concern to people that did not own this car before 150,000 miles. If a less experienced driver would not have known to reduce brake pressure applied it could have slid and caused collision or if brakes would have been needed during an emergency situation. Car is now sitting at toyota dealership and advised not to drive as brake pressure was lowered while being diagnosed. After talking with toyota they cannot provide assistance due to car being slightly over the 150,000 mile mark. Also if the brake pedal is mashed for an extended period of time the dash lights for brake system and traction control go off and will not come back on until it is driven for awhile

The engine caught fire while sitting in my garage. I had driven it for a few hours doing deliveries for a store and parked it in my garage around 2:30pm. About 1.5 hours later, my wife was coming home and opened the garage door and saw smoke above the hood of my car and then a few seconds later it burst into flames. I believe that it was smouldering for a while and the gust of wind when the big garage door opened gave it the extra push to ignite. The car was bought used and had a front end accident prior to my purchasing it so the electrical system could have melted and started the fire but the inspector that Toyota sent did not give me any copy of his report. It is a 2012 model. I bought the car in 2014, and had it for about 5 years until this happened and completed burned up the car and all the contents of my garage. I had a T-Mobile Sync-up module in the OBD port and it recorded a drop in voltage from around 12 to 5 volts at the time of the fire which I believe was transferred to my phone when I was near it.

I was driving on the expressway and experienced a sudden loss of power. The internal combustion engine suddenly stopped and did not deliver any power to the wheels. I immediately received a message on the system that said 'Check Hybrid System Stop Vehicle in a Safe Place'. The electric motor still continued to provide some power with the available battery charge. I was able to make it to the shoulder of the road but it was challenging to get to the shoulder with no power or warning and high speed traffic. My wife and I were in the vehicle. The vehicle had no engine or error codes until the failure on the expressway. The vehicle has the code P0A1A and P0A94. This is consistent with the error codes in the NHTSA 19V491000 recall and Toyota Safety Recalls E0E and F0R.

There has been a safety recall on my vehicle for the years 2004-2009 models of the Toyota Prius, but my model year is 2012 and I have the exact same issue. 1) My check engine light comes on and off and it appears to be the hybrid electric water pump issue related to the coolant. The cap pops off and spills out. 2) When the light comes on, I am assuming the car is starting to overheat and is putting myself and my 3 year old daughter at risk as this is the primary car we use to transport her. We have just stop taking her out at all due to this issue. When the car overheats, I pull over and it can be risky as sometimes, I am on the highway or local streets. I do not want this issue to cause problems for other other drivers as well while I am trying to pull over where ever I am. 3) The problem as I stated above has been acknowledge by you and Toyota but just for 2004-2009 models. 4) The problem has not been inspected by Toyota, the police, or third party. I have been the only person checking the car and I am using a diagnostic check engine light tool to read the code and looking under the hood to find the coolant pump leaking and popped out just as indicated in the recall for other models. 5) Yes, the check engine light and high temperature light goes on and off. The issue first appeared around November of 2020.

AFTER THE LATEST HYBRID SOFTWARE UPGRADE ON MY CAR WE NOTICED THAT THE GAS MILEAGE DROPPED BY 10%. THEN AS WE WERE DRIVING IN HEAVY TRAFFIC ON A TWO LANES FOUR DIFFERENT WARNING LIGHTS CAME ON TWO WHICH WERE FOR THE BRAKING SYSTEM AND TWO OTHERS FOR THE HYBRID SYSTEM. TOOK THE CAR TO THE TOYOTA SHOP AND THEY FOUND THE HYBRID BATTERS NEEDED TO BE REPLACED AT 63,000 MILES.

I bought my first used car, 2012 Toyota Prius, from Fowler Toyota back at the end of October 2023, but I was not able to pick it up until September 21, 2023 due to extra time for re-conditioning. I did not haggle about the price nor complained about the 3 week delay. In the last 3 weeks of owning my car, the TPMS sensor light and check engine light infrequently turned on. February 28, 2024 all the fault sensor lights on my instrument panel are on, along with the loss of proper braking function. I was not worried about the other sensor faults, but brake malfunction is a matter of life and death to myself and others on the road. I have had the car for a little over 4 months and less than 10k miles. I brought the car in to diagnose all the error codes. The dealer said it will cost over $2600 to fix the brake malfunction. I chose this particular car because carfax had well documented service records and Fowler Toyota advertised that every used vehicle for sale has surpassed rigorous inspection. I voiced my complaint to the used car director and he offered numerous default excuses for unforeseen problems inherent in buying a used car. I said I did my due diligence by selectively purchasing a used Toyota Prius, from a reputable Toyota dealership that uses a Toyota certified service center to inspect and recondition vehicles for sale. All the cliche nightmare stories of buying a used car from the side of the road car lots came true in my experience at Fowler Toyota. The worst used car story happened to me, despite performing my due diligence: to buy a used Toyota from a Toyota dealership that uses a Toyota certified service center for re-conditioning vehicles. I even paid more to finance my Prius through Toyota Financial services, which did not offer extended warranty. For God sake, I was buying a Toyota with 121k miles. I only bought a prius because I work too hard to throw away all my hard earned money on gasoline. The only recourse the dealership offered

THE ABS BRAKE PUMP EXPERIENCED AN "INTERNAL FAILURE" AT MILEAGE OF 113K. THERE IS A CUSTOMER SERVICE NOTICE THAT COVERS 2012 TOYOTA PRIUS AND PRIUS PLUG IN FOR THE SAME ISSUE REGARDLESS OF MILEAGE UNTIL 2019, AND THEN UP TO 150K MILES AFTER THAT. HOWEVER, MY 2012 PRIUS V WAS NOT COVERED. THIS ABS BRAKE PUMP JUST SUDDENLY FAILED, THERE WEREN'T ANY LEADING INDICATORS. TOYOTA KNOWS IT SUPPLIED DEFECTIVE PARTS FOR THE ABS BRAKE PUMP. THERE IS ALREADY A LAWSUIT OF OTHER 2012 PRIUS V OWNERS COMPLAINING OF THE SAME. HOWEVER, TOYOTA HAS NOT CORRECTED THEIR SERVICE NOTICE. THEY WON'T RELEASE THE DETAILS OF HOW THEY CAN JUSTIFY COVERAGE TO ALL OTHER 2012 PRIUS, AND PRIUS PLUG-IN, BUT NOT PRIUS V. THE REPAIR IS SIGNIFICANT ~ $3,500. ALSO, LAST YEAR MY ENGINE BLOCK WAS CRACKED AND NEEDED TO BE REPLACED. THANKFULLY, THIS HAPPENED ~ 80K MILES, SO IT WAS STILL UNDER EXTENDED WARRANTY. BUT THIS FAILURE WAS SURPRISING FOR A TOYOTA. I HAD WONDERED WHY I HAD TO CHANGE THE OIL MORE QUICKLY THAN THE RECOMMENDED 10K, I WOULD CHANGE IT EVERY 5-7K, OR AT LEAST HAVE IT CHECKED. IT SEEMS THEY HAVE THE SAME ISSUES WITH THEIR 2020 RAV 4S. I'M SURPRISED THEY HAVEN'T FIXED AN ISSUE FROM A 2012 VEHICLE YET BY 2020.

THE WINDSHIELD CAUSES MOST LIGHT SOURCES, PARTICULARLY LED LIGHTS, STREET LIGHTS, AND ONCOMING HEADLIGHTS (BASICALLY ANY BRIGHT LIGHT SOURCE), TO APPEAR WITH A "GHOSTING" IMAGE. THE "GHOSTING" APPEARS IN THE WINDSHIELD VERTICALLY BELOW THE LIGHT SOURCE AS ONE, TWO, OR EVEN THREE IMAGES. THE DEALER HAS REPLACED THE WINDSHIELD TWICE ALREADY, BUT THERE HAS BEEN NO IMPROVEMENT. IN FACT, THE FIRST AND SECOND REPLACEMENTS ARE WORSE THAN THE ORIGINAL. I ALSO OWN A 2008 TOYOTA SIENNA, AND I DRIVE VARIOUS CHEVY EQUINOXES AND FORD ESCAPES WHERE I WORK - I DO NOT EXPERIENCE THE "GHOSTING" EFFECT IN THOSE VEHICLES. PERSONNEL FROM THE DEALER'S SERVICE DEPARTMENT AS WELL AS REPRESENTATIVES FROM TOYOTA HAVE ALSO OBSERVED THIS PHENOMENA WITH THE WINDSHIELDS IN MY PRIUS PLUG-IN. WHILE DRIVING, A LOUD POPPING NOISE (CRACKING) WAS HEARD WITH NO OBSERVED DAMAGE, AND LATER THAT DAY THE WINDSHIELD GREW A LONG CRACK IN WINDSHIELD WHILE THE CAR WAS PARKED. AN EDGE CRACK FORMED ON THE WINDSHIELD EXTREME OUTER EDGE IN THE CHAMFER ABOUT 2/3 OF THE WAY UP ON THE DRIVER?S SIDE A-PILLAR. THE WINDSHIELD BY IT?S DESIGN APPEARS TO BE RATHER THIN, AND THE WINDSHIELD EDGE IS EXPOSED, NOT COVERED OR EMBEDDED IN PLASTIC OR RUBBER OR TRIM MATERIAL. THE WINDSHIELD EDGE AS OBSERVED HAS A VERY SMALL CHAMFER AND DOES NOT HAVE A GENEROUS BEVEL (ROUND) THAT IS HIGHLY POLISHED SUCH AS FOUND ON THE PASSENGER WINDOW EDGES OR OLDER WINDSHIELDS I AM FAMILIAR WITH. I TOOK THE CAR TO TOYOTA OF BOWIE AND WAS TOLD BY THE SERVICE MANAGER, HE BELIEVES THERE WAS AN IMPACT TO THE WINDSHIELD AT THE VERY EXTREME EDGE OF THE WINDSHIELD WHERE THE CRACK ROOT IS ON THE CHAMFER AND THEIR DEALERSHIP WOULD NOT REPLACE IT UNDER ANY WARRANTY. THE SERVICE MANGER DID NOT LOOK AT THE CRACK WITH ANY TOOLS, FOR EXAMPLE, A MAGNIFYING CLASS OR THE USE OF A BALL POINT PEN TO DETERMINE ANY IMPACT LOCATION OR IF IT IS IN FACT A STRESS CRACK THAT HAS FORMED. THE SERVICE MANAGER DID NOT OFFER TO ARRANGE TO HAVE THE CAR REPAIRED AND TOLD ME TO GO TO A GLASS COMPANY. WHEN THE SERVICE MANAGER WAS ASKED ABOUT ANY PROBLEMS OR CONCERNS WITH TOYOTA WINDSHIELDS OR EXPOSED GLASS EDGE WINDSHIELDS WITH A SMALL CHAMFER, HE SAID THERE WERE NO PROBLEMS OR RECALLS. THIS IS ESPECIALLY HAZARDOUS FOR PEDESTRIAN FOOT-TRAFFIC CROSSING PERPENDICULAR TO THE CAR. PEDS IN CROSSWALKS OR IN PARKING LOTS ARE AT TIMES, COMPLETELY MASKED BY THIS BLIND SPOT BY THE COMBINATION OF THE FRONT WINDSHIELD PILLARS AND FRONT WINDOW SUB-PILLARS. Anti lock brake malfunction. Code c1391 leak in brake accumulator. This seems to be a common code for brake issues with Prius. The cost to repair is significant, more than the value of the car. This is a certain safety issue that needs to be addressed. This is clearly a Hazzard and will cost lives to those who cannot afford the cost of repair.

I was driving when I felt my brake pedal start to become soft and mushy. I was able to brake, but I had to pump the brakes to get full brakes to stop. I also noticed warnings began to appear on my dashboard. The warnings that were shown are the brake light, traction assistant light, and ABS light. I took it to Gates Toyota in South Bend, IN and paid for a brake inspection. They told me that my brakes themselves were OK (pads, rotors, calipers), but they were getting a C1391 error. I was told that the brake accumulator has an internal leak. I was told that I needed to replace the brake master cylinder and brake booster pump assembly. I bought the car used from the dealership about a year and a half ago. After doing research, I found that this same issue has been a common recall problem on a number of Toyota vehicles starting as far back as 2008. I also found a few class action lawsuits concerning Toyota vehicles with the exact same issue. My vehicle doesn't show any current recalls, but I'm having the same problem as other Prius V vehicles are having. I was quoted $3300 to fix this problem. I shouldn't have to be responsible for paying for a defect that has been recalled on a very large number of vehicles, and also has been the result of class action lawsuits.

THE LATCH SYSTEM DESIGN ON THE SEATS ON THE NEW PRIUS V IS FLAWED. INSTEAD OF HAVING FOUR LATCH POINTS, ONE SET OF TWO FOR THE LEFT SEAT AND ONE SET OF TWO FOR THE RIGHT SEAT, THEY HAVE ONLY THREE LATCH POINTS, ONE IN THE MIDDLE OF THE LEFT SEAT, ONE IN THE MIDDLE OF THE RIGHT SEAT AND ONE IN THE MIDDLE OF CAR. OUR CHILD SAFETY SEAT HAS THREE BUCKLES DESIGNED TO WORK WITH THE LATCH SYSTEM, ONE ON THE LEFT, ONE ON THE RIGHT AND A TOP TETHER. THE LEFT AND RIGHT BUCKLES CANNOT ATTACH TO THE SAME LATCH POINT BECAUSE THEN THE CHILD SAFETY SEAT WILL PIVOT. THIS MEANS ONE MUST INSTALL THE CHILD SAFETY SEAT AT AN OFFSET TO THE CAR'S SEAT, USING THE MIDDLE AND RIGHT LATCH POINTS FOR A CHILD SAFETY SEAT INSTALLED ON THE RIGHT SIDE. BUT THEN THE TOP TETHER, WHICH MUST GO OVER THE TOP OF THE SEAT DOWN TO A LATCH POINT, IS NO LONGER AS SECURE AS IT SHOULD BE, SINCE THE LATCH POINT IS THEN OFF-CENTER WITH REGARD TO THE TOP TETHER. IF THE CHILD IS OLD ENOUGH THAT THEY NEED TO USE THE CAR'S SEAT BELTS WITH THE CHILD SAFETY SEAT (E.G., A BOOSTER SEAT) INSTEAD OF THE RESTRAINTS BUILT-INTO THE CHILD SAFETY SEAT, ONE CAN'T BUCKLE IN THE SEAT BELT BECAUSE THE BUCKLE INTENDED FOR THE RIGHT SIDE OF THE CAR IS BURIED UNDERNEATH THE OFFSET CHILD SAFETY SEAT. THE MIDDLE BUCKLE IS DESIGNED TO NOT WORK WITH THE SIDE SEAT BELTS. IF ONE USES THE LEFT SIDE BUCKLE WITH THE RIGHT SIDE SEAT BELT, IT'S NOT AS SECURE, AND THEN THE LEFT SIDE PASSENGER (OR A SECOND CHILD SAFETY SEAT) HAS NO SEAT BELT. I was driving my 2012 Prius on the highway at around 55 MPH. The Hybrid System Warning came on and the car lost all power and immediately began to coast. I was fortunate not to have been in or caused an accident but cars behind me had to swerve to avoid hitting me. I was able to coast to a middle median lane. After waiting 15-20 minutes, the car never regained power. We were finally able to push it off the road and into a parking lot. It never regained power.

I WAS DRIVING THE 2012 PRIUS ON A DOWNHILL PARKWAY AT ABOUT 40 MPH WHEN THE WARNING LIGHT CHECK HYBRID SYSTEM CAME ON. WITHIN 10 SECOND, THE VEHICLE TOTALLY LOST POWER. IT AUTOMATICALLY SHIFTED INTO NEUTRAL. THERE WAS NO BACK UP FAIL-SAFE/LIMP HOME MODE. FORTUNATELY THE CAR WAS GLIDING DOWNHILL BY GRAVITY AND EVENTUALLY WAS STEERED INTO A DRIVEWAY. THEN IT REFUSED TO GO ON READY OR DRIVE MODE. ON THIS SATURDAY AFTERNOON THE CAR HAD TO BE TOWED HOME. IT HAD TO BE TOWED AGAIN TO THE DEALERSHIP FOR SERVICE THE FOLLOWING WEEK. IT TOOK THE MECHANIC TWO DAYS TO MAKE A DIAGNOSIS THAT A FUSE ON THE MAIN DRIVE BATTERY HAD BLOWN. ANOTHER DAY WAS SPENT WAITING FOR THE PART. THE DEALERSHIP CANNOT GUARANTEE THAT THIS EVENT WILL NOT HAPPEN AGAIN. I CONTACTED TOYOTA USA. AN AGENT NAMED MICHAEL WAS COURTEOUS ENOUGH TO LISTENED TO ME AND GAVE ME A CASE NUMBER 1604142245. TO ANYONE WITH COMMON SENSE, THIS TOTAL LACK OF AN EMERGENCY ALTERNATIVE PROPULSION SYSTEM FOR EVEN A CHEAP CAR IS UNACCEPTABLE. THE DEALERSHIP CANNOT EVEN COME UP WITH A PLAUSIBLE EXPLANATION AS TO WHAT MAY HAVE CAUSED THE FUSE TO BLOW. WAS IT OVER HEATING? WAS THERE A SHORT IN SOME INTERNAL BATTERY CELLS? THIS IS A SUDDEN LOSS OF 2 MILES OF CHARGE. I SUSPECT THAT SOME LITHIUM CELLS HAVE BEEN DAMAGED AFTER THE INCIDENT. I WAS STOPPED AT A RED LIGHT ON A HILL. WHEN THE LIGHT TURNED GREEN, I STEPPED ON THE GAS WITH NO RESPONSE. THE CAR SLID BACK AND I BRAKED. MY 2012 PRIUS V IS ONLY 90K MILES. I WENT TO THE DEALERSHIP HOWEVER THE DEALERSHIP STATES THAT MY CAR, BASED ON ITS VIN, IS NOT IN ONE OF THE CERTAIN CARS COVERED BY ZJB PHASE 2 WHICH COVERS BRAKE BOOSTER AND BRAKE BOOSTER PUMP ASSEMBLIES FOR CERTAIN 2011-2015 MODEL YEAR PRIUS VEHICLES. THE FAILURE OF THE BRAKE COULD BE A DEADLY DEFICIENCY TO ALL THE PEOPLE IN THE CAR WHEN DRIVING. NOW I HAVE TO THINK TO STOP USING THIS CAR BECAUSE OF THE SEVERE SECURITY RISK. ON JAN. 3, 2018 WHILE MOVING AT A MINIMUM SPEED (LESS THAN 15 MPH) I APPLIED THE BRAKES AND SEVERAL WARNING LIGHTS ILLUMINATED (BRAKES, ABS, TRACTION CONTROL) ALONG WITH A WARNING SOUND. THIS WAS ON A CITY STREET, DRIVING IN A STRAIGHT LINE. I WAS ABLE TO BRAKE TO A STOP. I WAS TOLD THE BRAKE ACTUATOR HAD FAILED AND WAS CHARGED $3288 PLUS TAX FOR THE REPAIR. I RECEIVED THE CAR ON JAN. 9, 2018 FROM THE DEALER. ON JAN. 12, 2018 THE CAR FAILED ONCE AGAIN IN THE EXACT SAME MANNER. THE DEALER KEPT THE CAR UNTIL FEB. 1, 2018 AT WHICH TIME THEY RETURNED THE CAR TO ME AT NO CHARGE, STATING THEY HAD CHANGED THE SAME EXACT PART AS BEFORE. THE CAR HAS 84,000 MILES AND I PURCHASED IT NEW. I UNDERSTAND TOYOTA HAS ISSUED RECALLS FOR THIS ISSUE ON OTHER MODELS BUT NOT ON MY VEHICLE. The contact owns a 2012 Toyota Prius. The contact stated that while reversing, the driver's side rear wheel flipped over. The vehicle was towed to the dealer, where it was diagnosed with a corroded I-Beam assembly on the rear wheel. The contact was informed that the driver's side rear hub and the BRG assembly, rear suspension, and bolts needed to be replaced. The vehicle was repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 196,347.

On 07-June-2024 @14:47 my 2012 Toyota Prius suffered a catastrophic failure. I was attempting to back my 2012 Toyota Prius vehicle a few meters back into my driveway while turning slightly. This was a very slow speed backup. Suddenly, the rear passenger side wheel collapsed inward, and rendered the vehicle unmovable. A video of the incident is attached for viewing The vehicle has low mileage approximately 58,000 miles. The vehicle is in excellent condition and the service records provide evidence of proper maintenance, often at Toyota dealership. This vehicle was originally from Cape Cod Massachusetts and perhaps suffered adverse effects from ocean salt air? Although the rest of the vehicle does not show any significant rusting. . I am impressed by the engineering of the Prius hybrid technology, but this point of failure seems to be an engineering weakness.

While pulling out of a parallel parking spot going slightly uphill, I braked as I looked around the back of a double parked truck. I saw a car coming, braked hard, and suddenly my car surged forward across the street and hit a parked car. I looked down and saw that my foot was squarely on the brake pedal pressing down as hard as I could. The parked car was unoccupied, and I was uninjured. There were no warning lamps, messages, or other symptoms of the problem. I turned my car into the dealer for repairs and a fix to the sudden unexpected acceleration. They could offer no fix until a Toyota representative examined the car. On August 14, by an emailed letter, Toyota claimed it found no defects and it must be the result of driver error, specifically, my mistakenly hitting the accelerator. This is gaslighting denialism. (a) The EDR data sent to me does not indicate that I hit the accelerator unless that data is in the page of hexadecimal numbers. (b) Toyota's claim that I hit the breaks at "-0.2 seconds prior to the collision" is physically impossible. No one can move their foot from the accelerator to the brake that fast. (c) Toyota's claim is contrary to what I experienced and observed.

As i was driving, my breaks suddenly became spongey and unresponsive. I could hear a "hiss" sound come from below me. ABS and other warning lights came on on my dash. As i was near my auto mechanic i tried to make it there safely, and as i drove the brakes seemed to become softer and softer. By the time i had reached my mechanic i was so uncomfortable with its ineffectivness i felt it necessary to use the emergency brake to come to a complete stop. My mechanic confirmed that the issues included the failing of the Brake Booster, Brake Master Cylinder, Accumulator Pump, ABS, ABS Computer. According to him this was caused by a set of issues involving internal leaks in the brake booster assembly. These issues were replicatable, and my mechanic says they pose a saftey risk. Even after he gave it a "temporary patch job." Due to this the only times it has been driven were to get it home, to move it along our street so as to not get ticketed for having a stationary vehicle too many days in a row, and one time it was taken up the nearest rise in preparation for a Tsunami warning. When looking for replacment parts, my mechanic allerted me to a Technical Service Bulletin that should have been sent out to owners warning them of this issue and offering replacment parts. I never received this notice as I am the second owner of this vehicle, the notices were sent before i owned it, the previous owner claims not to have been aware of this issue herself, and because it was a voluntary service call, when researching the vehicle myself for any reported issues such as recalls, none had come up, meaning i had no way of knowing. To add to this, i contacted Toyota to see if they would honor the past offer to service this issue, but Toyota refuses to replace the parts now as it is too far past the voluntary service dates they set.

THEN THEY SAID IT WAS POLICY TO ALSO REPLACE THE BRAKE BOOSTER PUMP ASSEMBLY FOR AN ADDITIONAL $2,000 OR A TOTAL OF $4600. AFTER I COMPLAINED THAT THEY HAD RECALLED 972,000 OTHER PRIUS BUT DIDN'T INCLUDE THE PRIUS V THEY SAID I ONLY NEEDED TO REPLACE THE MASTER CYLINDER AND POWER BOOSTER AND SAID THEY WOULD DO IT FOR $1,516.89. SO THEN I GO TO TAKE THE CAR TO THE SERVICE DEPARTMENT TODAY 12/4/19 AND WHILE SITTING THERE THE IDIOT LIGHTS ALL TURN OFF AND THE BRAKE START TO FUNCTION NORMALLY OWN ITS OWN... I ASK THE SERVICE REPRESENTATIVE WHAT HIS THOUGHTS WERE AND HE SAID I SHOULD STILL MAKE THE REPAIRS, BUT HE COULDN'T SAY WHAT THEY WOULD DO IT CAME BACK. I am writing to express a complaint regarding the issues I have experienced with the brake system of my Toyota Prius C, 2012 model. For the past several months, I have noticed that the brakes exhibit strange noises, soft pedal sensation, reduced braking efficiency, were driving and the ABS, traction, brake, and that other light turned on and the brakes became slow these issues have been observed on multiple occasions and pose a safety risk to myself, other drivers, and pedestrians. I have taken the vehicle for inspection, but the problems persist or have not been definitively resolved. I believe these faults may be related to defects in the braking system, and therefore I request a thorough inspection and necessary repairs at no cost, considering that the manufacturer could be responsible for manufacturing defects.

In September 2022, my car, a 2012 Toyota Prius V with 79, 056 miles, began making noises in the engine area and the brakes became loose, meaning it took more pressure on the brake pedal to engage the brakes. In November 2022 the following warning lamps on the dashboard came on: Master Warning Light, Brake System Warning Light and Warning Buzzer, Brake System Warning Light, ABS Warning Light and SLIP Indicator. I took it to the Toyota Dealership near me: Lipton Toyota in Ft. Lauderdale, FL, and they diagnosed car with the trouble code of C1391-Abnormal Leak of Accumulator, Internal ABS Actuator Failure. To fix this problem, Lipton Toyota will charge $3124.29 for the repair. Toyota Motor Sales, U.S.A., Inc. issued a customer support program notification that would cover this repair if the malfunctions happened before coverage ended. Primary Coverage was applicable until December 31, 2021 with no year/mileage limitation. Secondary Coverage was applicable for 10 years from the vehicle's date of first use or 150,000 whichever occurs first. Date of first use on my Prius V was 03/01/2012 and my mileage is 79K. So as you can see, I am just out of the coverage window. My private mechanic and myself have appealed our case to Toyota that this problem should be a recall with no limits on coverage. (Toyota Motor Sales, U.S.A., Inc. - case reference # 230206000678) I believe that if I had driven my car more, (which was not done because of the Pandemic 2020-2021) this issue would have come up before the cut off date for repair coverage. I also believe that this problem with the Brake Booster and Brake Booster Pump Assemblies on certain 2012-2014 model year Prius V vehicles should be a recall.

The contact owns a 2012 Toyota Prius. The contact stated that while driving approximately 50 MPH, the ABS, brakes, and traction control warning lights illuminated. The brakes became inoperable, but the driver was able to regain control of the vehicle and pulled over. The vehicle was taken to the local dealer where it was diagnosed there was a hydro pump motor fault and hydro motor pump relay fault with codes C1252 and C1253 and the hydro boost brake pump kit and brake assembly needed to be replaced. The vehicle was not repaired. The manufacturer was contacted and referred him to the NHTSA hotline. The approximate failure mileage was 97,000.

I brought the car to the Toyota dealer already 3 times for the same problem,now happened again,they told me the first time,nothing wrong with the car,and I paid them,the second time nothing wrong they said,the third time they said it’s the 12 v battery has to be replace it,I did replace it,but still the same problem I have,the HYBRID SYSTEM problems ,the Toyota company should fix it ,because it’s something very unsafe it’s happening with my car,please help me to resolve this problem….. the car drive but stop anytime,I almost have a very bad accident with my daughter ON board

Hi, I have a 2012 Toyota Prius with around 120k miles that is completely inoperable. A local Toyota dealership is charging $3400 to replace both computers, but the more I research the more apparent it becomes that this is directly linked with past issues that were never fully resolved during previous recall attempts. The following article describes the exact situation my car is in (https://www.latimes.com/local/california/la-fi-prius-overheat-inverter-defect-20190414-story.html). When you combine that with the previous recalls back in 2014 & 2018, this quickly becomes a pattern. That pattern suggests that past software issues (1 & 2 below) were never fully resolved, and/or already caused premature damage to internal/computer components that then fail regardless. I spoke with the California Bureau of Automotive Repair who referred me to nhtsa. The agent I spoke with noted that because the Inverter wasn't actually replaced back in 2014 (Toyota only performed a software update), this part definitely could have been already damaged due to the software issues, which could then cause problems with the computers later on (which is what occurred). --- 1) NHTSA CAMPAIGN ID: 14V053000 - In the affected vehicles, the Intelligent Power Module (IPM) inside the inverter module (a component of the hybrid system) contains transistors that may become damaged from high operating temperatures. 2) NHTSA CAMPAIGN ID: 18V684000 - Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven. --- General: - Computers both failed, not currently available for inspection as they are being replaced - If this computer failure happens again later (while on the road - as it has for other consumer), it's very dangerous as the car essentially shuts down on the spot - Dealer is dismissing this due to the car's age - No previous warning lights or messages

ON JUNE 5TH, 2019, I TOOK MY CAR IN TO ROUND ROCK TOYOTA AND THEY PERFORMED THE 2018 TOYOTA PRIUS V RECALL FIX. SINCE THEN, MY MPG HAS GONE FROM AN AVERAGE OF 41 MILES PER GALLON, WHICH HAS STAYED CONSISTENT SINCE I PURCHASED THE CAR BRAND NEW IN JUNE 2012, DOWN TO 32 MPG. I TOOK THE CAR BACK TO ROUND ROCK TOYOTA FOR A DIAGNOSIS ON SEPT 30, 2019 AND THEY SAID THAT THEY CANNOT FIND ANYTHING WRONG, THERE ARE NO ERROR CODES, AND THAT MY NOW GREATLY REDUCED 32 MPG FELL WITHIN TOYOTA STANDARDS. I AM WRITING TO FILE A COMPLAINT THAT THEIR 2018 RECALL FOR THE TOYOTA PRIUS V HAS A PROBLEM AND NEEDS TO BE REVISITED OR THE VEHICLES SHOULD BE TAKEN OFF THE ROAD. THE NAVIGATION, BLUETOOTH & RADIO SYSTEM WAS TURNED OFF ON IT OWN AND BECAME CORRUPTED WHEN THE CAR DEALERSHIP TRIED TO UPDATE THE SYSTEMS SOFTWARE. THE HARDWARE WAS THEN RENDERED INOPERABLE AND THE SYSTEMS COMPUTER WAS THEN REPLACED, THE WARRANTY DID NOT COVER THE DEFECTS IN THE SYSTEM. THE ESTIMATED REPAIR EXCEEDED $1000US. DUE TO THE DEALERSHIPS ACTIONS IN TRYING TO RUN A SOFTWARE UPDATE, THE REPAIR COSTS WERE PAID BY THE DEALERSHIP. THE FAILURE OF THE COMPUTER SYSTEM SEEMED TO BE KNOWN BY THE CAR DEALER. TOYOTA WAS NEVER DIRECTLY CONTACTED BY CAR OWNER AS ONLY THE CAR DEALER HANDLED THE REPAIR ISSUES.

MY PRIUS WAS JUST FINE THEN ONE DAY IT STALLED WHILE I WAS DRIVING IN THE MIDDLE OF THE FREE WAY. I TOOK MY VEHICLE IN TO ELMORE TOYOTA IN WESTMINSTER, CA, THEY FOUND IT HAD A FAILED INVERTER AND IT WAS STILL COVERED UNDER WARRANTY. IT TOOK 2 WEEKS FOR THEM TO FIX IT, BUT WHEN THEY RETURNED MY CAR, THE SLIP INDICATOR WAS NOW ON AND WOULD NOT TURN OFF. THEY CLAIM IT WAS LIKE THAT PRIOR TO THE FAILED INVERTER, THIS WAS FALSE, MY CAR WAS RUNNING PERFECT. I BOUGHT PRIUS V IN FEBRUARY 2012. I'VE NOTICED FEW TIMES ISSUES WITH BRAKES. WHILE DECELERATING WITH MY FOOT ON THE BRAKE PEDAL TO SLOW THE CAR TO A STOP AND IF THE CAR GOES OVER A POTHOLE OR SLIGHT BUMP ON THE ROAD THEN CAR ACCELERATES FORWARD BEFORE COMING TO A STOP. THE VEHICLE WAS TAKEN TO A DEALER WHERE A TECHNICIAN ADVISED THAT NO REPAIRS WERE NEEDED AND THAT THE ISSUE WAS NOT A DEFECT BUT RATHER A CHARACTERISTIC OF THE VEHICLE. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND ADVISED THAT THE VEHICLE WAS NOT INCLUDED IN ANY RECALLS. THE VEHICLE WAS NOT REPAIRED. THE APPROXIMATE FAILURE MILEAGE WAS UNKNOWN AND THE CURRENT MILEAGE WAS 7,200. UPDATED 12/10/12 *CN THE CONSUMER STATED THE VEHICLE'S WINDSHIELD WAS REPLACED ONCE. I am writing to bring attention to a critical safety issue affecting the 2012 Toyota Prius and other Toyota models equipped with the brake booster and actuator system. These components are failing at alarming rates, leaving vehicles with no functional brakes, ABS, or traction control. Additionally, this failure compromises the hybrid system due to the regenerative braking no longer working, potentially causing further damage to hybrid components and resulting in repairs costing several thousand dollars. Toyota issued Customer Support Program ZJB, which covered these issues for a limited period. This program was in effect from August 31, 2021, with no mileage or year restrictions, but offered secondary coverage for up to 10 years or 150,000 miles from the vehicle’s first use. Unfortunately, my vehicle, with approximately 130,000 miles and 12 years of age, was denied coverage when brought to a dealer after the program had ended. Despite the brake booster and actuator failing, the dealership did not advise me not to drive the vehicle, further emphasizing the gravity of this oversight. This is a significant safety hazard, as vehicles with these failures lack the ability to stop effectively, risking lives on the road. Furthermore, this problem is not isolated to my Prius; it affects thousands of Toyota vehicles equipped with these components. The estimated repair cost exceeds $3,000, an undue burden on owners for an issue that stems from defective manufacturing. I firmly believe Toyota’s decision to address this problem through a limited Customer Support Program, rather than a full recall, is inadequate and irresponsible. This issue meets the criteria for a safety recall and should be treated as such by the National Highway Traffic Safety Administration (NHTSA). I respectfully urge the NHTSA to investigate this matter further asap/mandate a recall for all affected vehicles. This is a big safety issue. FROM DATE OF PURCHASE, VEHICLE PERFORMED UNPREDICTABLY. TRANSITIONING FROM ELECTRIC TO MOTOR WAS ROUGH AND NOTICEABLE. AT 600 MILES, A SIGNIFICANT IMPACT WAS FELT DURING TRANSITION. DEALER FOUND NOTHING. BRAKE PEDAL TRAVELED TO THE FLOOR AND VEHICLE WENT THROUGH STOP SIGN AT LOW SPEED. ROUTE 44 TOYOTA INSISTED I NEEDED TO GET USED TO ELECTRONIC BRAKES. TOYOTA INSISTED BRAKES WORKED AS PRESCRIBED. RECENTLY, A AAA DRIVER DISCOVERED THE GROUND WIRE ON THE BATTERY WAS LOOSE. THE BATTERY WAS REPLACED AND THE VEHICLE NOW FUNCTIONS AS DESCRIBED, THE HILL HOLD FUNCTIONS AS DESCRIBED. NHTSA DID AN INSPECTION IN RESPONSE TO A DEFECT PETITION - VEHICLE DID NOT PERFORM AS DESCRIBED IN THE FEDERAL REGISTER. BRAKE PEDAL IS NOW HIGH, AS IT WAS INITIALLY. THIS VEHICLE WAS SENT FROM THE FACTORY WITH A LOOSE BATTERY GROUND AND IT WAS COVERED UP BY EVERYONE FROM TOYOTA, DEALERSHIPS TO NHTSA WHEN THE CORRECTION WAS SIMPLE. Upon driving out of my school parking lot at approximately 15 to 20 miles per hour I noticed a cushiony filled with a brakes and all of a sudden I heard a shhh all of my lights popped up on my dashboard. When I took my vehicle in to be able to find out what was going on with it I was told that I need a master cylinder and a brake booster. WHILE TRAVELING IN A RAIN STORM AT 17MPH, I TRIED TO ENGAGE THE BRAKES TO PREVENT FROM CRASHING INTO AN 18 WHEELER AND THE BRAKES DID NOT WORK. I DID CRASH INTO THE 18 WHEELER BECAUSE THE ENGINE WAS ACCELERATING AND I WANTED TO STOP.WHEN I CRASHED THE AIRBAGS DID NOT DEPLOY. EVEN AFTER IMPACT THE CAR WAS STILL ACCELERATING LIKE IT WAS TRYING TO PUSH THE 18 WHEELER. EVENTUALLY THE CAR POWERED DOWN AFTER REPEATEDLY PUSHING EVERY BUTTON POSSIBLE. I HAD THE CAR TOWED TO A TOYOTA DEALERSHIP. I PHONED YOUR HOTLINE BUT THE VIN NUMBER I GAVE HAD A NUMBER THAT WAS WRONG MY PRIOR CONFIRMATION NUMBER IS 10542285. PIVOT BOLT INSIDE STEERING WHEEL CAME LOOSE, WAS WARNED IT HAD DROPPED TO WHERE IT COULD INTERFERE WITH BRAKE LINKAGES. FILED COMPLAINT WITH TOYOTA AND NEVER HEARD BACK SO FILING THIS COMPLAINT. (1) A LARGE BOLT INSIDE A SEALED AREA SHOULD NOT BE ABLE TO COME LOOSE IF INSTALLED CORRECTLY (MANUFACTURING ERROR - SINGLE INSTANCE OR SYSTEMATIC?) AND (2) A LOOSE BOLT SHOULD NOT BE A RISK FOR A POSSIBLE CATASTROPHIC OUTCOME - LOSS OF BRAKES (DESIGN FLAW, LINKAGES SHOULD NOT BE SUSCEPTIBLE TO BEING JAMMED - THAT'S WHAT THEY WARNED ME ABOUT WHEN THEY ASKED FOR THE $330 TO FIX IT). MY HORN NO LONGER WORKS. I TOOK IT INTO THE DEALER AND IT WAS DIAGNOSED AS HAVING A FAULTY SPIRAL CABLE. ACCORDING TO INFORMATION ON THE INTERNET, THE SPIRAL CABLE ALSO PLAYS A ROLE IN THE DEPLOYMENT OF THE DRIVER AIRBAG. THIS PART WAS UNDER A RECALL FOR VEHICLES A COUPLE OF YEARS PRIOR. I FEEL THAT IF THIS IS A KNOWN ISSUE AND AN EXTREME SAFETY ISSUE THEN IT SHOULD ALSO BE FIXED UNDER RECALL. THE DEALER PRICE TO FIX THIS IS OVER $800. I HAVE OPENED A CASE WITH TOYOTA #210-128-2281, BUT THEY DON'T SEE IT AS A SAFETY CONCERN AND WILL NOT OFFER ASSISTANCE TO REMEDY THE FAULTY CABLE.

PROBLEM WITH THE FUEL BLADDER CAUSING THE GAUGE TO READ INCORRECTLY. THE GAUGE GETS STUCK AT A QUARTER TANK AND BEFORE YOU KNOW IT, YOU'RE OUT OF GAS. ONCE THIS HAPPENED, IT CAUSED THE HYBRID SYSTEM TO FAIL AND LOCKED THE CAR UP EVEN AFTER PUTTING GAS IN IT. HAS TO DO A MASTER RESET OF THE ENTIRE CAR TO GET IT TO START AGAIN.

I WENT TO START MY PRIUS V. AFTER IT WAS STARTED, A NUMBER OF THE WARNING LIGHTS CAME ON INCLUDING THE ABS AND STABILITY CONTROL. I WAS STUNNED SINCE I HAD MY 30000 MILE CHECK UP IN MAY WITH NO PROBLEMS. WHEN I TOOK IT TO THE DEALER FOR REPAIR, THE DEALER DETERMINED THAT RODENTS HAD EATEN THROUGH A NUMBER OF THE ELECTRICAL SYSTEMS BECAUSE OF THE SOY BASED WIRING. (I UNDERSTAND NOW THAT IT'S "BIOGRADABLE SOY BASED WIRING INSULATION.) I GENERALLY PARK MY CAR AT ONLY TWO LOCATIONS - MY HOME AND MY WORK. I HAD NEVER SEEN ANY RODENTS OF ANY TYPE! On 10-17-24 these warning lights came on: 1. Pre-collision System 2. Hybrid System 3. Airbag 4. Safety Connect red light And Intermittently: 1. Slip Indicator 2. Brake System Cruise control would not set, and displayed "E3". ------- Component failed: Dealer determined that the driver's side door harness had been chewed by a rat that had entered the under dash area "somehow". They said it couldn't be repaired but must be replaced. My insurance deductible was $500 and they paid the balance of the $1,387.06 bill. (I have retained the harness, which could have been repaired by splicing the two wires connecting to the driver's side airbag collision sensor. The labor to splice the wires would have been less than an hour.) ------ Safety: The chewed wire apparently disabled the Airbag, Pre Collision, Emergency Automatic Braking, Seatbelt Pre-Tensioning, ABS, and Stability Systems. ----- The dealer confirmed the problem. ----- My insurance adjuster did inspect the harness. ----- I placed electronic rat traps in the vehicle, which killed three rats. ========= NEXT ========= ALL THE SAME WARNING LIGHTS CAME ON A FEW MONTHS LATER! I didn't throw more money at the Dealer, and took off the panels to inspect the wiring myself. There was damage to the passenger side door harness wires. I removed the harness and ordered a $60 used replacement on Ebay. While waiting for delivery, I researched rat damage to Toyota wires, and how rats could possibly enter the interior of the car. I discovered that there is an UNSCREENED 12x9 inch opening to bring fresh air into the HVAC system. It is hidden under the windshield wiper cowls. Also, there are two UNSCREENED vents in the trunk area. Other Prius owners covered those openings with 1/4 inch metal wire hardware cloth. I spent a few hours disassembling, covering, and reassembling pieces to prevent further access through the fresh air intake, and will do to the rear next. TOYOTA should recall these cars and ratproof the harnesses

The local dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 106,019.

2012 Prius in very good condition with 180k miles- only 12 years old. Driving on back roads (thankfully) and the drivers side rear wheel compelely fell off while driving. I was doing about 30 MPH and felt like there was a flat tire so I was in the process of pulling over when the whole rear assembly snapped. The wheel hit the rear quarter of the car and bounced about 100 feet down the road. I could have been on the highway and this could have been fatal I believe. Very, Very scary. As can be seen in attached photos the issue was rust from the inside. This is very concerning since there was no way for me to know this was an issue. I could have been killed or killed someone else. I believe Toyota needs to look into this since there are other Prius' out there the same thing happened to on this very thread. Although this is a Massachusetts car I have owed it since 25k miles and have maintained it impeccably. I always have the undercarriage cleaned after driving on salty roads. Safety inspection was current- only done the month before. Had that wheel bearing replaced about 2 weeks before the incident and the reputable mechanic reported no issues. There was no warning except for maybe a minute before it happened- the car was vibrating like I had a flat tire. The damage repair estimate on this was $8500. Catastrophic fail Toyota. Is this a rust issue like the Tacoma, Tundra, and Sequoia?????

Brake master control system failure with constant issues over 80k miles. Forward running lights control housing has not held a light fixture since day one and dealership wants to charge for light replacement. It’s a faulty part and burnt through the housing. Olathe Toyota refused to repair under warranty saying it was a customer issue but is clearly a safety problem. Irresponsible dealer management.

8/2/22 My ABS light came on. Took the car to a dealer. They kept it for six weeks and returned it on 10/3/22. No charge and no explanation. 11/7/22 engine light came on. Took in to dealer, they returned it 11/17/22 indicated incorrect air/ fuel mixture. 10/9/23 Traveling 47 mph, came around a mountain and encountered stopped cars. Slammed on the brakes, car did not slow down, steering wheel locked to the right, car yawed to the right, and turned over, hit a curb and the car righted itself. 3 occupants, 2 injuries , 1 death. Researching the 2012 Toyota Prius, found many instances of a faulty ABS system, mimicking exactly my experience. I was never informed by Toyota or the local Toyota dealer.

I have had a 2012 Prius since 2016. The entire time I've had it, it occasionally "hydroplanes" but without any water. It happens in wet or dry (but mostly dry) conditions. Usually at slower speeds so, though it makes me very uncomfortable, it doesn't happen with any regularity. I've taken it multiple times to my mechanic who always says "Prius' just do that", which is not reassuring. I know there was a recall and I was told my car did not fall under that umbrella. On Wednesday of this week, 10/25/23, I finally had an accident because of this. The brake lockup had been happening more frequently in the past week and in this instance I saw the car in front of me suddenly slow down - I had about 2/3 of a cars distance between me and the car ahead. I hit the brakes and nothing happened. I was pushing down on them long enough to think, "Ok, this is it. They aren't responding at all and I'm going to hit this car in front of me." I did and it crushed the passenger side of the car. Luckily I wasn't hurt. There were no warnings from the car at all that this was going to happen, as there never are. I took it in for possible repair today (the mechanic is not sure but thinks it may be a total loss). This car did everything they describe in the recall cases and I want to know if there's anyway to be repaid from the company for this as there was nothing I could do to stop this accident? Why wasn't my car included in the recall?

The glass sunroof spontaneously exploded or imploded. There was a loud popping sound and I looked up at the sunroof, and it was shattered. glass started falling on me as I was driving on the highway. Then the eps light came on & the dash stopped showing the mph. The ac stopped blowing cold air & the windows were working slow. I turned car off then back on a couple of times then more lights came on. No lights were on prior to this & I haven’t had any problems w the vehicle before this. I just got this car & I feel like it has something to do with a recall but no one wants to take responsibility for the issue.

TAKATA RECALL FROM THE DATE OF PURCHASE, VEHICLE PERFORMED UNPREDICTABLY. I TOOK MY CAR TO THE TOYOTA DEALERSHIP AND THEY FOUND THAT THE HYBRID BATTERY NEEDED TO BE REPLACED. WHEN I DRIVE 45 MPH, THE VEHICLE WILL STALL AND ALL INDICATOR ILLUMINATE ON THE INSTRUMENT PANEL. IN THE MIDDLE OF THE ROAD, THE CAR WILL STALL. TOYOTA DOES NOT STAND BY THEIR PRODUCT. MY CAR GOES COMPLETELY DEAD. EVERY WARNING LIGHT TURNS ON AND ITS BLINKING.

ON JULY 28, 2017, I WAS DRIVING ALONG WITH MY THREE CHILDREN. I PULLED INTO A DRIVEWAY TO PARK MY VEHICLE BEHIND A PARKED VEHICLE. THERE WAS APPROXIMATELY A 2 FOOT SPAN BETWEEN THE BACK OF THE VEHICLE AND THE FRONT OF MY 2012 TOYOTA PRIUS. TO THE RIGHT OF THE PARKED VEHICLE WAS A PARKED TRUCK. BEING THAT I WAS PULLING INTO A SPACE TO PARK MY VEHICLE, I WAS TRAVELING EXTREMELY SLOWLY. I BEGAN TO PRESS ON MY BRAKE TO PARK THE VEHICLE AND INSTANTLY KNEW THERE WAS A PROBLEM. UPON DEPRESSING THE BRAKE, SEVERAL LIGHTS BEGAN TO FLASH WITHIN THE DASH TO INCLUDE BUT NOT LIMITED TO THE ABS, WARNING LIGHT, AND THE LIGHT WITH VEHICLE SWERVE LINES. OTHER LIGHTS WERE ALSO FLASHING, BUT I DO NOT RECALL EACH ONE. THE BRAKE PAD INSTANTLY WENT STRAIGHT TO THE FLOOR WITH NO PRESSURE. THE VEHICLE WOULD NOT STOP AND CONTINUED TO TRAVEL FORWARD WITH ACCELERATION. TO AVOID HITTING THE VEHICLE IN FRONT OF ME I BEGAN TO TURN MY WHEEL TO THE RIGHT. THE VEHICLE CONTINUED TO ACCELERATE AND CAME INTO CONTACT WITH THE TRUCK THAT WAS TO MY FRONT-RIGHT. MY PRIUS FAILED TO SLOW OR STOP. IT CONTINUED TO ACCELERATE AND BEGAN PUSHING THE TRUCK UP THE DRIVEWAY AND INTO THE METAL GARAGE DOOR WITH THE BRAKE PEDAL BEING DEPRESSED THE ENTIRE TIME. EVENTUALLY, THE CAR CAME TO A STOP AND THE LIGHTS CONTINUED TO FLASH PRIOR, DURING, AND AFTER THE INCIDENT. AFTER THE CAR WAS TOWED TREAD WHEEL MARKS WERE FOUND ON THE DRIVEWAY WHERE THE WHEELS SPUN AND LEFT AN IMPRINT IN THE ASPHALT. I AND MY CHILDREN WERE UNINJURED HOWEVER, THE INCIDENT WAS INCREDIBLY TRAUMATIC. THE VEHICLE WAS DEEMED A TOTAL LOSS BY MY INSURANCE COMPANY AND AS OF YET TO BE PROCESSED FOR DIAGNOSTIC TESTING BY TOYOTA. THE TOYOTA CARE SERVICE WAS INFORMED OF THE ACCIDENT. TOYOTA BELIEVES IT TO BE ASSOCIATED WITH THE INTELLIGENCE COMPONENT OF THE VEHICLE, HOWEVER DETERMINATION IS CURRENTLY UNKNOWN UNTIL DIAGNOSTIC IS PERFORMED.

TL* THE CONTACT OWNS A 2012 TOYOTA PRIUS. THE CONTACT STATED THAT WHILE PARKED, THERE WERE RODENTS ENTERING THE INTERIOR OF THE VEHICLE. THE CONTACT STATED THAT THE CABIN AIR FILTER DID NOT HAVE A MESH GRILL WHICH WAS ALLOWING THE RODENTS TO ENTER THROUGH A HOLE IN THE CABIN AIR FILTER THEN INTO THE GLOVE COMPARTMENT CONSOLE AND INTO THE INTERIOR OF THE VEHICLE. THE DEALER WAS CONTACTED AND INFORMED THE CONTACT THAT THE WIRES WERE SOY BASED CAUSING THE ATTRACTION OF RODENTS TO THE HOOD OF THE VEHICLE HOWEVER THE VEHICLE WAS NOT TAKEN FOR AN INSPECTION. AN INDEPENDENT MECHANIC WAS ALSO CONTACTED WHO CONFIRMED THAT THE CABIN AIR FILTER DID NOT HAVE A MESH GRILL WHICH RESULTED IN THE RODENTS ENTERING THROUGH A HOLE INTO THE CABIN AIR FILTER THEN ENTERING INTO THE GLOVE COMPARTMENT CONSOLE ACCESSING THE INTERIOR OF THE VEHICLE. THE VEHICLE WAS THEN TAKEN TO A MECHANIC WHERE A HARDWARE CLOTH GRILL WAS INSTALLED ON THE CABIN AIR FILTER. THE VEHICLE WAS REPAIRED AND RODENTS WERE NO LONGER ENTERING THE VEHICLE. IN ADDITION, THE CONTACT WAS REQUIRED TO CLEAN THE FECES REMAINING IN THE INTERIOR OF THE VEHICLE UTILIZING A MASK, GLOVES, AND DISINFECTANT AND IN ORDER TO AVOID CONTACT WITH A VIRUS FROM THE FECES, THE CONTACT WAS ADVISED NOT TO VACUUM LET THE VIRUS BECOME AIRBORNE. THE CONTACT WAS CONCERNED THAT THE RAT FECES IN THE VEHICLE COULD RESULT IN A FATAL DISEASE IN THE EVENT A CHILD WOULD RIDE IN THE VEHICLE AND ACCIDENTALLY EAT THE FECES. THE CONTACT WAS CONCERNED THAT IF RODENTS ENTERED THOUGH THE AIR CONDITIONING UNIT AND DEFECATED INSIDE, ACTIVATING THE AIR CONDITION UNIT IF THERE WAS FECES, A VIRUS COULD BECOME AIRBORNE. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE THEN INFORMED THE CONTACT THAT THE FAILURE WAS NOT COVERED UNDER WARRANTY AND THAT THE FAILURE WAS AN OUTSIDE INFLUENCE WHICH WAS NOT THEIR RESPONSIBILITY. THE APPROXIMATE FAILURE MILEAGE WAS 40,000. THE VIN WAS NOT AVAILABLE.

AFTER TOYOTA POWER MANAGEMENT SOFTWARE RECALL INSTALLED "POWER MANAGEMENT UPGRADE" SOFTWARE IN MY CAR I IMMEDIATELY EXPERIENCED SERIOUS PROBLEMS WITH MY IGNITION /START OF HYBRID/GASOLINE ENGINE. AFTER PRESSING IGNITION, I HAD TO WAIT 15 TO 20 MINUTES AND PRESS START BUTTON ON/OFF REPEATEDLY UNTIL THE GASOLINE ENGINE IGNITE BUT HYBRID FAILED TO START. IT IS NOT NORMAL BECAUSE A PRIUS HYBRID ENGINE STARTS FIRST NOT THE GASOLINE ENGINE. TOYOTA HAS DISABLED SOME IMPORTANT FUNCTIONS IN ITS POWER MANAGEMENT RECALL SOFTWARE UPDATE THAT AFFECTS THE HYBRID ENGINE PERFORMANCE AND FAILURE TO START. I DRIVE MY PRIUS 3-4 TIMES A MONTH AND I DID NOT HAVE ANY PROBLEM BEFORE THE RECALL SOFTWARE UPGRADE BECAUSE PRIUS OWNERS MANUAL CLEARLY SAYS THAT THE BATTERY WITH BE DISCONNECTED BY BATTERY SAVING FUNCTION WHEN YOUR CAR IS PARKED FOR LONG TIME, SEVERAL DAYS, WEEKS. IN FACT, WHEN YOUR CAR IS PARKED FOR JUST DAYS, TOYOTA POWER MANAGEMENT SOFTWARE (RECALL) IS DRAINING THE CAR BATTERY AND RECONFIGURING THE RELAYS TO POWER UP THE GASOLINE ENGINE. TOYOTA OWNERS MANUAL CLEARLY STATES THAT WHEN YOU ARE PARKED, "POWER MANAGEMENT SOFTWARE DISCONNECTS THE AUXILIARY BATTERY FROM THE HYBRID SYSTEM." HOWEVER, AFTER THE RECALL, NOT ONLY AUXILIARY BATTERY IS NOT DISCONNECTED BUT THERE ARE LOTS OF "ACTIVITY" IN HYBRID ENGINE THAT DRAINS THE BATTERIES. I CALLED THE TOYOTA SERVICE AND THEY NOW SAY YOU HAVE TO KEEP THE CAR ON A CHARGER ALL THE TIME WHEN PARKED FOR SEVERAL DAYS. OF COURSE , IT IS A HUGE SAFETY PROBLEM TO KEEP THE CAR ON A CHARGER ALL THE TIME WHEN PARKED FOR DAYS BECAUSE THE BATTERY IS HOUSED IN THE CABIN AND THE VENTILATION DOES NOT EXIST UNLESS YOU PARK YOUR CAR OUTDOORS. AT THE SAME TIME TOYOTA RECOMMENDS KEEPING THE CAR ON A CHARGER , IT ALSO WARNS OF EXPLOSION CAUSED BY CHARGING THE BATTERIES IN A GARAGE.

MY WIFE WAS INVOLVED IN A TWO CAR CRASH WHEN A CAR COMING IN THE OPPOSITE DIRECTION MADE AN ILLEGAL LEFT TURN DIRECTLY IN FRONT OF HER. IT HAPPENED SO QUICK THAT SHE NEVER HAD TIME TO HIT HER BRAKES. SHE WAS TRAVELING BETWEEN 30 AND 35 MPH IN A 35 MPH ZONE ON A TWO LANE ROAD. I WOULD THINK THAT AT THAT SPEED IF ALL HER SAFETY EQUIPMENT WORKED SHE WOULD NOT SUFFER ANY INJURIES. HER SEAT BELT FAILED CAUSING THE SEAT LATCH TO FAIL AND SHE ENDED UP JAMMED INTO THE BOTTOM AND UNDER SIDE OF THE DASHBOARD. SHE SUFFERED A BLOW TO HER CHEST AND INJURIES TO HER KNEES, LOWER LEGS AND FOOT. SHE HAD SURGERY ON HER RIGHT KNEE SO WE COULD GET HER ON HER FEET AGAIN AND IS AWAITING SURGERY ON HER RIGHT SHOULDER. SHE HAD A PRE EXISTING HEART CONDITION THAT WAS RAISED TO EMERGENCY STATUS AFTER THE BLOW TO HER CHEST AND SHE HAD SURGERY FOR THAT AND THEY FOUND A SHORT CIRCUIT IN THE MIDDLE OF HER HEART AND ZAPPED IT. THERE IS ONLY A 2% CHANCE IT WILL EVER RETURN. I HAVE PICTURES OF THE FAILED SEAT BELT AND SEAT. THE SEAT BELT IS IN THE FULLY EXTENDED POSITION AND LOCKED AND CAN NOT BE RELEASED EVEN BY YANKING ON IT. I AM WRITING THIS REPORT SO THIS SITUATION CAN BE CHECKED AND IF FOUND FAULTY CAN BE CORRECTED BEFORE ANY ONE ELSE GOES THROUGH WHAT MY WIFE IS GOING THROUGH.

I OWN A 2012 PRIUS WITH THE TECHNOLOGY PACKAGE, INCLUDING RADAR ADAPTIVE CRUISE CONTROL. I BELIEVE THESE SAME RADAR SENSORS ARE ALSO USED TO DETECT IMMINENT COLLISIONS IN FRONT OF THE VEHICLE, EVEN WHEN CRUISE CONTROL MODE IS NOT ENGAGED. ABOUT A MONTH AGO, WHILE DRIVING NORTH ON INTERSTATE 5 IN DOWNTOWN SEATTLE AT APPROXIMATELY 60 MILES PER HOUR, AND WITHOUT CRUISE CONTROL ENGAGED, MY PRIUS WENT INTO "IMMINENT COLLISION MODE" DESPITE THE FACT THAT THERE WAS NOTHING IN FRONT OF ME -- NO VEHICLES, NO DEBRIS, NO OBSTRUCTIONS. BEFORE SLAMMING ON THE BRAKES, MY SEAT BELT WAS "TIGHTENED" AND THE BRAKES WERE AUTOMATICALLY APPLIED AT FULL FORCE, EVEN WITHOUT MY FOOT TOUCHING THE BRAKE. THIS WAS INCREDIBLY JARRING AND DISTURBING, AND COULD HAVE BEEN A SERIOUS ACCIDENT IF THERE WAS A VEHICLE BEHIND ME. FORTUNATELY, THERE WAS NO VEHICLE BEHIND ME AT THIS TIME. AFTER ABOUT TWO SECONDS OF FULL BRAKING, THE VEHICLE "DISENGAGED" COLLISION MODE AND ALLOWED ME PROCEED AND RETURN TO SPEED. I CALLED THE TOYOTA OF SEATTLE DEALERSHIP AND SPOKE TO THEIR SERVICE DEPARTMENT. THEY WERE UNAWARE OF THIS ISSUE, AND SUGGESTED THAT I BRING THE VEHICLE IN FOR SERVICING. THEY CONFIRMED THERE WERE NO OUTSTANDING RECALLS ON THE VEHICLE. THEY TOLD ME IT WOULD COST AT LEAST $100 FOR A BASIC DIAGNOSTIC, AND IF SENSORS HAD TO BE REPLACED, IT COULD COST HUNDREDS OR EVEN THOUSANDS OF DOLLARS. BECAUSE I AM CURRENTLY UNEMPLOYED AND LOOKING FOR WORK, I DID NOT BRING MY VEHICLE INTO THE SERVICE CENTER. I FEEL THIS IS TOYOTA'S OBLIGATION TO FIX, AND SHOULDN'T REQUIRE ME SPENDING MONEY OUT OF POCKET. I WOULD LIKE SOME KIND OF RESPONSE FROM TOYOTA ON THIS ISSUE. I WAS INVOLVED IN A FRONT END COLLISION WITH A BOBCAT FRONT END LOADER. I STRUCK THE IMPLEMENT BROADSIDE GOING 35 MPH. THERE WAS NO DAMAGE TO THE IMPLEMENT. IT DOES THE SAME THING IN REVERSE. THE ONLY WAY OUT OF THIS CYCLE IS TO PHYSICALLY PUSH THE CAR TO GET IT MOVING(SLOW WALKING PACE) AND THEN THE TRACTION CONTROL BEHAVES VERY MUCH LIKE OUR 2012 CAMRY SE V6. A COUPLE TIMES WE HAD OUR CAMRY RIGHT NEXT TO THE PRIUS(SIDE BY SIDE) AND THE CAMRY WAS ABLE TO GET ITSELF MOVING(WITH TC ON) AND PRIUS WOULD NOT BUDGE. BOTH HAVE THE FACTORY TIRES, THE PRIUS TIRES ARE IN BETTER SHAPE THAN THE CAMRY. DRIVING 70 MPH CAR BEGAN TO GO INTO HIGH RPM, THEN ACCELERATED TO 74 FROM 70 MPH. SO FAR DEALER AND TOYOTA HAVE NOT ADDRESSED THE HIGH RPM, THE ACCELERATION, LOSS OF ELECTRONICS AND HYBRID SYSTEM FAILURE AND TOLD ME THEY HAVENEVER HEARD OF ANY TOYOTA EVER HAVING A REPORT OF A HIGH RPM OR ACCELERATION WITHOUT INTENT TO ACCELERATE FORM THE DRIVER. VEHICLE IS UNSAFE, HAS 4944 MILES AND THIS PROBLEM OCCURRED ABOUT A MONTH AFTER PURCHASE. TOYOTA STILL DOES NOT KNOW WHAT THE PROBLEMS ARE AND IN POSSESSION OF THE VEHICLE. *TR *JS

HYBRID ENGINE SUDDENLY SHUT DOWN WITHOUT WARNING WHILE VEHICLE WAS TRAVELING 65 MPH ON A BUSY FREEWAY, CAUSING VEHICLE TO COME AN UNSAFE STOP. DEPRESSING ACCELERATOR HAD NO EFFECT. ONBOARD COMPUTER GAVE ERROR MESSAGE "CHECK HYBRID SYSTEM. STOP THE VEHICLE IN A SAFE PLACE." RESTARTING VEHICLE PRODUCED THE SAME ERROR MESSAGE AND STILL UNABLE TO SHIFT VEHICLE INTO DRIVE OR REVERSE. FUEL TANK FULL AT TIME OF INCIDENT. VEHICLE PURCHASED NEW 5 MONTHS PRIOR FROM TOYOTA DEALERSHIP. NO AFTERMARKET PARTS OR MODIFICATIONS PRESENT. *TR

WHEN DRIVING DASHBOARD "HYBRID SYSTEM NOT WORKING. PULL OVER..." THIS WAS 7/3/2012. IT HAD RUN OUT OF GAS EVEN THOUGH THERE WERE 2 BARS OUT OF 10 STILL PRESENT ON GAS GAUGE. ON 7/30/12 LATER THIS HAPPENED AGAIN WITH 4 BARS SHOWING. GOT THE FUEL SENSOR AND GAUGE REPLACED ON 7/30/12. 8/14/12 WITH 3 BARS SHOWING, CRUISING RANGE OF 60 MILES AND HAVING GONE 480 MILES THE SAME WARNING "HYBRID SYSTEM NOT WORKING..." I WAS ABLE TO GET TO A GAS STATION BEFORE RUNNING OUT OF GAS. THE PRIUS MANUAL STATES THE TANK HOLDS 9.5 GALLONS. FILLING UP THE TANK TOOK 9.91 GALLONS. THERE IS CLEARLY SOMETHING WRONG. THE FUEL SENSOR AND GAUGE HAVE ALREADY BEEN REPLACED, THOUGH. THIS IS A MAJOR PROBLEM FOR A NEW CAR. THIS IS A NEW MODEL. *TR
