Strong gas smell when the engine is turned on. I started feel this issue about 2 year ago, now the gas smell is more stronger every time engine is turned on.

Sudden, unexpected, surprise Surging Idle upon placing the vehicle in Park after driving a 15 minute distance at a top speed of 35mph. Loud Surge up to 2.5 (x1000RPM) Sounds like the Engine Shifted Idle Drops down and immediately begins Surging up again. Idle is high while driving. Some chugging upon acceleration. Immediately drove to Auto Parts store for Diagnostic Test. Started vehicle and showed Technicians what was happening. Diagnostic Test provided: Code P0171 Stored and Permanent System is Too Lean Bank 1 Code P0505 Stored and Pending Idle Control System P117B Stored Fuel Performance/Direct Injector Most Likely Solution: Replace Fuel Pump Primary DTC: System Too Lean Bank 1 Suggested Part: Fuel Pump Operating The Vehicle Could Result in Sudden Shut Down, causing a Collision. Could also Cause Compromised Gas Mileage and Engine Damage. Check Engine Light is On Additional Message states that All Wheel Drive Mode has been disengaged and 2 Wheel Drive Mode is active. No Warning or Any indication that there was any issues prior to the Sudden Idle Surging upon placing the vehicle in Park. I have searched existing recalls for 2019 Toyota Rav-4 and found a Fuel Pump listed. I checked my license plate and VIN only to see the result that there are Zero Unrepaired recalls for both searches. This vehicle is clearly having an internal battle between air and fuel. I find it concerning that 2019-2020 Toyota Rav-4 vehicles had a Fuel Pump recall and yet my search has Provided these results.

I was told on 6/28/2024 that my car needs a new coolant bypass valve at a cost of 900.00 on researching this I see there is a class action suit against Toyota because there are so many people with this problem and it should be a recall. Not sure where to go from here .

My RAV4 does not hold a full tank of gas. It only allows half a tank get full of gasoline. Toyota says they don't know how to fix, as well as two other shops I have brought it to. There are no engine messages or any other signs of issues. Gas will gush out of the tank when filling up if I do not stop at half way.

The engine maintenance required turned on at55k miles for the coolant bypass. After trying to get it repaired they had 500 on back order. Thousands of Rav 4’s are experiencing this with over $800 quote to repair , this cannot just be a basic issue this is bigger.

Was insinuating engine was over heating even in -4° weather has no hot air plenty of coolant. This is causing the vehicle to "buck" amd having to have to pull over and waiting several minutes to let the engine "cool off" I can make it about 3 miles in-between pull overs. After some research I feel like the issue falls under a current lawsuit against Toyota regarding the coolant valve based on reading other issues and what my rav is doing. It is a several thousand dollar fix through a dealership and one my family can not afford especially if it is a poorly manufactured part of the car. It is one of two vehicles we have and need to daily living especially having a child and no means yo offered another safe vehicle at this time. The coolant pump itself

2019-2023 model year Toyota RAV4s and Toyota Corollas are equipped with defective coolant bypass valves prone to premature failure which can cause coolant to leak out of a vehicle’s closed-loop coolant system, harming its ability to properly regulate engine temperature. This can cause overheating, stalling, increased emissions, and total engine failure and also allows coolant to leak into and damage surrounding engine components, including a vehicle’s electrical system, the complaint relays. The engine maintenance light came on and I took the car to my independent mechanic. The "soft code" indicates that the coolant system valve/hose (engine coolant bypass valve) is showing in the closed position when it should be open. My mechanic has tried to order a replacement part, but is having trouble finding one. After my personal research, I see that there is a Class Action Lawsuit regarding this part from 2024 indicating that it can cause the car to overheat, stall, and potentially severe engine damage. Toyota has no formal recalls on this, but it has been indicated that some owners have received support from Toyota on models built up to 2021.

The design of some Rav4 grilles (particularly 2019-2024 models) allows rocks and road debris to easily pass through and damage the vulnerable A/C condenser, or radiator behind it, which leads to very expensive repairs. This is what happened to my 2019 Toyota Rav 4. Now I have to pay 1300.00 to get it repaired.

26Aug2025 I notified Maplewood Toyota MN I had not been notified of safety recalls when nhtsa site confirmed 7 safety recalls. I had absolutly no response. Now nhtsa site says there are no unrepaired recalls when Toyota of Maplewood has done No repairs.

You get a message that said engine maintenance required contact the dealer, when you do the scan it's shows that there is a problem with engine coolant bypass valve. After checking online it was multiple people with the same problem.

There is a known TSB out for 2019 toyota rav4 and some select other years. The issue is a slow speed transmission issue. It is due to a programming issue. At slow speeds, if you try to go from the brake pedal to the gas pedal too quickly, there is a serious lag before power is delivered to the wheels. Which is very dangerous when making specific turns. An example is what happened to my wife and myself the other day. We were in a median turning lane, cars were coming, as we were coming to almost a stop, my wife went to apply the gas. There was a serious lag before power was delivered and it almost resulted in a T bone collision. My wife went to apply gas, there was no power for several seconds, which caused her to panic and hit the brakes again. In that process, the power was finally delivered to the wheels and caused the car to pull about halfway into the oncoming traffic before it stopped from her applying the brakes. She then hard to quickly reverse to avoid the collision. The TSB calls for a flash of the TCM and a relearn procedure to be done. Which I am having done today due to the severity of the incident we had. This could have 100% caused a serious or fatal accident. The two dealerships I spoke with severely down played it as an issue. This deserves a investigation and a recall to be sent out. I was never advised of the TSB or anything.

I’m reporting a safety issue with my 2019 Toyota RAV4 that has recently started and is getting worse. The problem seems related to the start-stop system or something in the powertrain. When I come to a stop—at a stop sign, red light, or roundabout—the car doesn’t always respond normally when I try to start moving again. Sometimes there’s a hesitation, and other times it lurches forward unexpectedly when I lightly press the gas pedal, like you normally would. It feels like a jolt, almost like whiplash. It’s especially dangerous in roundabouts or tight traffic situations because I can’t predict how the car will respond. I’m worried I could accidentally hit another car, or get rear-ended myself because the car doesn’t move smoothly or reliably when restarting from a stop. Even when I turn off the start-stop system, the issue still happens. It feels like a hesitation followed by a sudden surge. The RPMs don’t change, but the car still lurches. Yesterday, for the first time, I noticed the car slightly accelerated on its own after stopping, which was very concerning. I found out through Toyota’s official website that there is a technical service bulletin about this issue that came out before we owned the vehicle. But because it’s not a full recall, the dealership said they wouldn’t fix it unless we paid the $80 diagnostic fee. We’re now just out of warranty. We’re making $580 monthly payments on a car that I don’t currently feel is safe to drive. There are no warning lights or messages on the dash. I’ll be taking it to Le Mieux Toyota in Ashwaubenon, Wisconsin, in two weeks to have it inspected, but I wanted to report this because I believe it’s a safety issue that should be looked into. I have two small kids and I’m genuinely worried about getting into an accident, especially when merging onto highways or driving in busy intersections. I’ve seen people say this system can be recalibrated, but the fact that this is happening without any dashboard warning and is worse.

The rear differential stopped working in January, 2025 and went into two wheel drive only. The message lit up on the dash board saying the vehicle was driving in two wheel drive only. We were driving home and the roads were slippery. We brought it to Mountain States Toyota and they confirmed there was a technical service bulletin (0086-19) issued in 2019, due to a Toyota manufacturing problem. It was issued three months after we bought the new vehicle. We were never notified and a recall was never issued in 2019 when Toyota knew about it. I contacted Toyota Brand Engagement and they agreed to pay for half of the $4,800 cost to replace the entire rear differential even though they acknowledged it was a Toyota manufacturing problem and possible safety issue. We don't understand why we were not notified about the known manufacturing problem that affected the drive train of an all wheel drive vehicle. We also don't understand why they won't cover the entire cost of the required repair to make the all wheel drive operational when they acknowledge it is a known Toyota manufacturing problem. I have the 2019 Toyota RAV4 Adventure that was manufactured with a defective differential, per T-SB-0067-20, Toyota has acknowledged that the differential were defective and that affected thousands of 2019-2020 RAV4 Adventure and Limited trim models. The defective part number is 41110-42050 and the new, superceded part number is 41110-42054. I reached out to Toyota numerous times to rectify the issue has the differential clunks and could seize causing loss of control. My vehicle was deemed a "total loss" due to hail damage and Toyota refuses to assist because of the total loss, but the vehicle was manufactured with the defective part and my vehicle suffers the VERY same issue as that a non-total loss vehicle suffers. The title status is irrelevant as the defective differential was installed BEFORE the total loss, so the title status is inconsequential. This is a safety concern because the differential could fail due to seizure, shatter, explode resulting in a loss of control and injury to me the driver and others on the road. I've expressed this numerous times with Toyota.

Rear door liftgate hinges defective. They are too week combined with electric power struts. They are being pulled away from body of vehicle and become misshapen (bent, deformed,etc) and causes door/liftgate to be unbalanced and and stick out too far away from body. Now the door/liftgate can not close and be secured

Lift cylinders of power liftgate are operating at different times causing gate to twist. Liftgate fails to open and close automatically, causing prolonged beeping while liftgate is open. Hinges holding liftgate in place have started to bend and crack apart from the body of the car. An independent auto body shop inspected this issue and recreated the tailgate failure. Quote to repair included replacing hinges, lift cylinders and to recheck electronic connections.

One week ago one section of the heated steering wheel was very hot. So much so that I could not touch that area. I took it to the Anderson Toyota dealership in Rockford today. On the way there, I turned on the heated steering wheel, and it started smoking. They said they couldn’t find what was wrong with it, and to replace it would be 1800 because it isn’t in warranty. They suggested I not use it.

Got a "Hybrid System Malfunction" notification on my dash and took it to the dealer. I was notified that my high voltage hybrid cable is corroded, and that the vehicle is unsafe to drive. I understand that customer support program 22TE09 exists for this problem, but because my vehicle has over 100,000 miles, it is not covered. If this is a safety issue that Toyota has acknowledged and admitted is an issue, there should be a complete recall. I was FORCED to get the work done at a dealer because Toyota has a hold on the part. I attempted to get an independent garage to do the work, but they were not authorized to even purchase the parts necessary. Ultimately I had no choice but to spend almost $5000 for a safety repair on a 2019 vehicle that Toyota themselves have admitted to the engineering mistake, while also making it impossible to fix outside of a dealer. This is completely unacceptable. Thank you.

The DCM is nonfunctional, which renders the safety connect features of the car inoperable. Additionally, the Bluetooth microphone is powered by the DCM causing the driver to have to switch the call output on their phone manually taking their focus off the road. This seems to be a widespread issue with many reporting this on online threads and Toyota offering many internal service bulletins. To me this is a major safety concern that is extremely costly to repair.

I have been told by the Toyota Arapahoe dealer that the DCM in the car is bad. All the services the DCM supposedly provides have NEVER worked on the car, leading me to believe it has been faulty from day one. These include all safety assistance features.

A distracting message “a connected device is not supported” flashes on my console screen every 10 seconds. This occurs when there are no device or cable connected. It blocks the clock and driving directions if I am projecting them through CarPlay. The SOS emergency system lights flash back and forth from red to green.

A malfunction in the electronic control of the engine, throttle, or automatic transmission has been detected. The car then ran hot and started jerking. I took it to autozone for them to look and could not find anything, I had a mechanic come over to look at it, and he couldn't find anything either. I sent it to Toyota Wolfchase in Memphis TN. Who has stated it will be two or three days before they can even look at it. This seems to be a common problem with Toyota cars yet there's no recall I have seen numerous complaints on Google Reddit, TikTok, and more. I called corporate to make a complaint and they said they don't see any complaints which I know is not true

When I was driving the car to work the automatic system (malfunction ) failed to restart in red light, i have to stop to restart (Awd malfunction } says to take to the dealer, I was driving back tothe dealership , they are saying car had zero coolant in syystem, raditor was leaking engine overheat to blow head gasket, a week before i was taking car dealership to state inspection and found only one issue that was back brake to change, no leak nothing they found, now i spent $7679 dollars to fix it i had all the proof what the dealerships try to say from the begining even I fixed, i am trying get rid of the vehicle now , it isonly 47454 miles when the incident happend.

Well, making my research it shows that the RAV4 2020 will have a recall with the brake booster that my car the 2019 is having the same issues and it’s a very expensive fix. My car only has 40,000 miles is still brand new and I don’t understand why am I having this issues with the brake booster RAV4 2020 has a recall for that same part so I would like them to honor the 2019 brake booster and fix it as well

The following information refers to recall notice 19V544000 "SERVICE BRAKES" and "ELECTRONIC STABILITY CONTROL" (recall number same for both notices) by Toyota for the 2019 Rav 4 Hybrid: "In 2019, Toyota issued a recall for certain 2019 Toyota RAV4 Hybrid vehicles due to a faulty brake assist system, specifically related to a potential issue with the brake booster pump which could lead to brake failure; the 2019 Toyota Prius was also included in this recall." The recall covered the manufacturing period 4/26/2019 - 6/3/2019 and a recall notice was issued on 7/24/2019 for the affected vehicles and owners were notified on 9/20/2019. We purchased our vehicle on 9/21/2019 with manufacture date August 2019 and on 12/19/2024 we suffered a brake failure causing damage to the vehicle and our garage. The Rav Hybrid Limited comes with sensors that will auto brake if it believes the speed/distance is insufficient and the brakes have not been applied. The brakes were self-applied at the time and no sensor action occurred. I believe the above recall should be extended to include August 2019 manufacture and should also include any sensors that involve the braking system and/or stability control system. Currently we are dealing with insurance and Toyota, no appraisal costs as yet

Noticed air in the brake piston boot upon brake inspection. This is a safety risk for our drivers when commuting on their travels as they transport medical patient to and from the facilities. No warning lights on the dash, initial cause of inspection was due to screeching and popping noise from the brakes.

Wipers would not turn on when activated. I checked all fuses were good and still not working. I drove home with no wipers in rain. Then next day I went to recheck fuses and relays, but the wipers came on.

The Panoramic Sunroof spontaneously exploded while driving on the freeway. There was no impact point and the glass is bowed outward. When it exploded it was a very load pop and pressure feeling that made everyone's ears ring. About 2 seconds later small glass particles and glass dust fell down on the driver and passenger. This was reported to insurance company and taken to independent service center for estimate and repair. There were no warning lights or prior issues. The sun roof was in perfect working order and the vehicle has less than 25,000 miles on it. Driving context - 10:30 PM, light rain, outside temp was 38degrees, inside car temp set at 72 with heater on, sunshade was back, radio off, very little traffic on freeway with no road disturbances.

The sunroof/moonroof in my 2019 Toyota RAV4 spontaneously exploded while parked overnight in my driveway. There was not any damage on or around that area of my car, nothing above my car. The following morning when I pulled out of my driveway, I noticed a strange sound , retracted my visor and noticed the roof glass was completely shattered in a volcanic like shape. Pieces began to fall as I began to pull back into the driveway.

DRIVING AT INTERSTATE SPEED WHEN SMALL GRAVEL BREAKS A SILVER DOLLAR SIZE STAR SHAPED CRACK IN WINDSHIELD ON PASSENGER SIDE. APPROXIMATELY 10 MINUTES LATER ANOTHER OBJECT HIT PASSENGER SIDE WINDSHIELD AND CREATED ANOTHER STAR SLAPPED CRACK IN THE GLASS NEAR PASSENGER WIPER ARM. HAD WINDSHIELD REPLACED WITHIN 7 DAYS UPON RETURNING HOME. THE NEW WINDSHIELD WHICH IS A GENUINE TOYOTA WINDSHIELD IS NOW CHIPPED IN SEVERAL PLACES. EVERYTHING THAT HOTS THE WINDSHIELD EITHER CRACKS THE GLASS OR CREATES A SIGNIFICANT CHIP THAT INTERFERE WITH PROPER VISION. I FEEL THE GLASS IS DEFECTIVE. TOYOTA JUST SAID I WAS UNLUCKY. BUT A WINDSHIELD SHOULD AFFORD SOME SAFETY FROM ROAD DEBRIS, THAT TIPICALLY DOESN'T CRACK OR EVEN CHIP THE GLASS. IF A QUARTER SIZE ROCK OR LARGER OBJECT HITS THE RAV 4 WINDSHIELD THAT I'M DRIVING, I HAVE NO DOUBT THAT IT WILL BURST OUT. I HAVE BEEN DRIVING FOR 53 YEARS AND HAS ONE WINDSHIELD REPLACED PRIOR TO THIS ONE. THIS RAV4 CURRENTLY HAS APPROXIMATELY 38000 MILES ON THE ODOMETER. A SHATTERED WINDSHIELD IS A SIGNIFICANT RISK FACTOR IN ANY ACCIDENT. I DON'T KNOW WHAT WOULD HAPPEN IF THE WINDSHIELD SUPPLIED BY THE MANUFACTURER PROVED DEFECTIVE.

It happened yesterday , November 3rd at 8:45 am Approx. speed was 35mph to 40 •Location: 934 York Road, Towson, MD •Description: Front-right impact at 40 mph, no airbag deployment, two children in back seat, vehicle had recent 50K service and roof-rail repair, CSP 22TE05, at Sheehy Toyota Cockeysville. Vehicle now at Caliber Collision Cockeysville. Claim #060381946 I am experiencing neck, shoulder and rib pain right now, my 5 year old complain about right ankle pain and apparently hit his head on the right side.My 3 year old complain about neck pain yesterday. There was no sign showing any kind of failure on my dashboard. I asked my vehicle to be investigated formally by Toyota engineer asap or forensic expert. I asked my vehicle to be freeze right until proper airbag inspection have been conducted. I have picture of the car showing clearly that none of the airbags worked.

The contact owned a 2019 Toyota Rav4. The contact stated while driving 15 MPH and making a left turn at night, the contact did not watch the opposite direction and another driver that was driving 40-50 MPH crashed into the passenger's side of the vehicle, between the rear passenger's side door and the rear passenger's side wheel. The vehicle was propelled 2-3 feet in the air and landed 10 feet away from the collision location. The contact stated that the other vehicle was propelled more than 100 feet away. The air bags failed to deploy. The contact's wife was occupying the front passenger's side seat while the contact's daughter was in the booster seat in the rear passenger's side seat. The contact exited the vehicle. The contact did not sustain any injury. The contact's wife sustained soreness in the right leg. The contact stated that he was able to take his daughter out of the vehicle; however, the rear passenger's side door was significantly difficult to open. The contact stated that his daughter had hit her forehead against the rear passenger's side window during the incident and was crying significantly. The contact's friend transported the daughter to the hospital, where several tests were performed. The contact was informed that no internal injuries were found and that the pain would subside. The contact stated that his daughter sustained emotional trauma after the incident. The vehicle was towed to the dealer, where it was deemed a total loss. The manufacturer was not notified of the failure. The failure mileage was 34,111.

My wife was traveling 45 miles per hour and hit a deer and the airbag did not deploy. After review, the air bag is supposed to deploy at 20 miles per hour. I am especially worried because Toyota has issued Technical Service Bulletin, noting that the rail line leaks and can cause air bag failure. I have brought this to Toyota Corporate and Legal's attention and they have completely denied responsibility and implied that this isn't a serious issue. I asked to have the airbag inspected and they refused, saying we could only do that before repairs had been completed. However, the airbag did not have any work done on it and should be in the same condition as it was during the accident.

The contact then stated that while the vehicle was idling, he heard a beeping sound coming from the front driver’s seat. The vehicle was taken to the dealer where it was diagnosed that the battery needed to be replaced. The vehicle was repaired however, the failure recurred. The dealer was notified of the failure, but no further assistance was provided. The vehicle was not repaired. The manufacturer was not notified of the failure. The 2019 Toyota Rav4 has a problem with Water leakage. Check the Internet for very specific details. Water is leaking from the roof mounted roof rails. This leak is causing water to enter the car and drip significant amounts of water down both A-Pillars. The A-Pillars support & Hold Airbags. The concern is that the water intrusion can cause damage to the airbags and/or the wiring to the airbags. This water damage could either prevent the airbags from deploying or cause the airbags to prematurely deploy. Toyota has yet to acknowledge the issue. They claim the water is coming from clogged sunroof drains. While this can be a cause it is NOT the sole cause. I had my vehicle into the dealer on 6/22/22 and they claimed the problem was the clogged sunroof drains. Last night was the 1st significant rain in the area since the visit. This morning BOTH A-pillars had significant water damage. So this means 1 of 2 things. If the leak is from the sunroof drains, they have a design issue as these were just cleared by Totyota. OR, as Many other owners of 2019 Rav4's have discovered there is a flaw in the roof rail design. Both issues need to be resolved ASAP, before there is a failure in the airbag system. I am writing to file a formal complaint regarding a manufacturing defect in my vehicle and the refusal by Toyota to honor its responsibilities for repair despite clear acknowledgment of the issue. In October 2024, I discovered a small water leak from the roof of my vehicle. I attempted to schedule an appointment with the nearest Toyota dealer, but the timing of their available appointments conflicted with my work schedule, and it was not possible for me to get a timely appointment. In an effort to mitigate the leak, I tried to fix it myself until I could get the vehicle to the dealership. Upon visiting the dealership, I was informed that the issue was a manufacturing defect, but they refused to repair my vehicle because I had attempted to fix the issue myself. This worsened the leak, as they did not return my vehicle to its original condition. I was then informed that I would have to bear the full cost of repairs. I had no prior knowledge of any recall associated with my vehicle. After conducting my own research, I discovered that Toyota had acknowledged the manufacturing defect affecting my vehicle model but had not notified me or issued a recall. I reached out to Toyota’s customer service, and they assured me that they would approve the repair through the dealership. However, after approximately a month of waiting, Toyota informed me that they would not cover the costs of the repair, and they declined to take any further responsibility.

Roof rack design and grommet failure allows for water to flow into cabin headliner and down a-pillars and other areas where curtain airbag and other electrical components are housed. This can cause malfunction and/or failure of the vehicles crucial functions at any given moment. A class action lawsuit is filed on this. You need to step in and make it a recall and hold toyota responsible.

Outside water (rain, carwash, etc) is leaking through the roof into the A-pillar and C-pillar areas and into the interior. Water is likely entering the air bag components in the front A-pillars on both sides, potentially causing a safety issue. Also concerned that water could be entering the electrical components in the dashboard. When researching the issue, found multiple complaints of this issue and also at least one class action lawsuit. I was recently driving my 2019 RAV4 when it started switching to mud and sand mode and turns of all my safety features. This would occur at random when I was parked and driving. I was able to find several discussion boards about this issue by searching ‘ 2019 rav4 switching to mud and sand mode.” I took it to the dealership and they discovered that there is a leak from the roof racks that is causing water to get in my car and is now impacting the driving ability and safety features on this car. I was able to find many discussion boards and even YouTube videos on how to fix this issue by searching “2019 rav4 roof rack leak.” It was reported in Australia, Japan, Taiwan, and the US. Many reported they had this issue fixed for free under warranty but now warranty has run out on the car and to fix the roof rack costs approximately $5000. Fixing the roof rack does not cover potential water damage that my airbags may have experienced as well and does not ensure there is not permanent water damage to the driving mode buttons

This as well as many many other 2019, 2020, & 2021 Rav4s develop water leaks in the roof (due to inadequate gaskets in the rood rails) which result in water entering in vehicle very close to the side curtain airbags. Toyota updated the parts on the current model year but refuses to issue a recall. This is dangerous due to the strong possibility of water contacting the airbags and or the electrical connections.

Lots of people including me are having issues with 2019 or newer Toyota RAV4 the roof guard rails seals start letting water in on both the driver side and passenger side near the airbag. The roof rack rails have begun leaking due to failed weather sealants around them. Water is seeping through when it rains, getting water on the airbags, headliner, and moves along the top frame. I’m scared the electrical components will become damaged. Issue began happening right around 45,000 miles when I noticed. I have only owned vehicle for 2.5 years.

My roof rack rail assembly is leaking water down my A and C pillars, and into the inside of my vehicle. When it leaks into my car it also leaks down into my side curtain airbags to the point if I were to get into an accident where the side curtain airbags needed to deploy because of my windshield, they would not which could cause me to lose my life. I have asked my dealership about it and they told me they had numerous roof racks replaced because of the leaking but because my vehicle is no longer under warranty, I have to pay to have the roof racks replaced. I have tried to reseal my roof racks but did not work. The Toyota dealership told me that if resealing didn't work, the only other option was to replace. I cannot drive in the rain, even when it is just sprinkling or wash my car because any sort of water will leak into my vehicle. Toyota will not send a recall out for this issue but is known to be a safety issue with the Toyota dealership and numerous people that own a 2019-2021 Toyota Rav4.

The contact owns a 2019 Toyota Rav4 Hybrid. The contact stated while driving 35 MPH and making a left turn, the vehicle suddenly accelerated to approximately 45 MPH unintendedly. The contact stated that the vehicle failed to respond upon depressing the brake pedal, and the accelerator pedal felt stuck, causing the front of the vehicle to crash into a building. There was no property damage. There was no warning light illuminated. The air bags deployed. The contact was able to exit the vehicle. The contact sustained bruising and a fractured rim on the right hand. The emergency units transported the contact to the hospital, where the contact underwent surgery. A police report was filed but the contact did not have the police report information available. The vehicle was towed to a tow lot, where it was totaled by the insurance company. The dealer was not notified of the failure. The manufacturer was notified of the failure and informed the contact that they would have the vehicle inspected. The failure mileage was approximately 70,000.

Vehicle happens to accelerate involuntarily when driving, especially on highway. You'll be driving, and then suddenly it starts to accelerate faster than usual. Had another family member drove vehicle to work, just to confirm if they had the same issue, and they did confirm that the vehicle was involuntarily accelerating at a higher speed on its own.

Intermittent loss/delay of acceleration power from a stop. When coming out of a stop, pressing the gas pedal warrants no response except the slow creep that occurs just as a result of lifting off the brake. This has happened a handful of times when trying to accelerate from being stopped at a red light. The problem occurs intermittently and I never know when to expect it. The most recent time this problem happened, I pressed the gas pedal all the way to the floor, and still the car did not accelerate. When left in the middle of an intersection, a loss of accelerator power can be deadly. I am a sitting duck as oncoming traffic comes towards me and I just hope they see me. Furthermore, when stalled out at a red light or stop sign, other traffic has to drive around me, risking the safety of other drivers as well. I took it into my Toyota dealership for repair, but since they are unable to replicate the issue, they cannot fix it. I was also met with skepticism; however, searching online has shown that other people have experienced the same problem with the same vehicle. The vehicle or component has not been inspected by the manufacturer, police, insurance representatives, or any others as of now. No engine lights appear when this happens; this is also what led the dealership to believe there wasn't actually a problem.

I am reaching out regarding a structural issue I’ve experienced with my 2019 Toyota RAV4, specifically with the rear power liftgate. Recently, I noticed that the liftgate hinges are beginning to crack and pull away from the body of the vehicle. The alignment of the trunk has shifted slightly, and there is visible damage around the hinge area. This appears to be worsening over time and is beginning to affect the liftgate’s ability to close properly and securely.

The rear power liftgate metal hinges are peeling away from the body of the vehicle. My liftgate doesn't close properly and is out of alignment to stay safely latched closed and has become 100% unusable. There is a 1.5 inch gap (you can see light and water can enter) on rear pass. side and the door is tightly jammed closed on the rear driver side. After speaking with a Toyota service manager, he casually let me know he sees this frequently. I have learned this is a very known issue yet Toyota will do nothing to prevent these defective hinges from being installed or cover repairs. The service manager told to take it to the collision center to get quoted on a repair. I did and my out of pocket cost (because Toyota won't take responsibility for this defect) is between $700 and $2700, depending how much damage occurred to other components caused by the defective hinges (the collision company has also stated they've done many of these repairs). My vehicle showed no visible sign of failure prior to breaking. It would occasionally fail to close, as if an object was in the way, but upon re-trying to power liftgate button, it would close. Sometimes it would take 3 to 4 times to be successful. But, it never appeared as if it was out of alignment or damaged until last weekend when I tried to close it and it forced closed itself VERY incorrectly. Considering my vehicle was purchased less than 5 years ago, brand new, this shouldn't be happening. It definitely shouldn't be showing this kind of wear on a vehicle I haven't even had to change brakes, tires or a simple headlight on the ENTIRE ownership of the vehicle. When I use those items every single day. My liftgate is used 1 time or less a week. The cost of this repair is half what I still owe on it and I didn't even do anything to cause it, it happened due to faulty hinges that are NOT safe. How do I know my hatch won't open while driving? Or that it won't break opposite and my liftgate becomes unsecured from my suv altogeth

Vehicle has absolutely no safe way to hook up to tow. My wife got the car stuck in the snow and needed a tug out with a tow strap. The owners manual states that only some vehicle of this model are equipped with a towing eye. Mine is not, and from what i hear all subsequent year rav4 are not equipped. I crawled underneath the car front and back and dug snow out to find there is no place to hook to. No holes, frame members, nothing to put a hook on, a clevis around, or a loop of rope to loop to. All plastic and smooth sheet metal. I had to strap around one wheel and drag it fighting the allwheel drive system, against my owners manual instructions. How can this be that there is absolutely no recommended way to tow/recover this vehicle or secure it to a towtruck? I have heard stories of tow companies refusing to tow Rav4’s because of this.

Toyota has a coolant control valve problem on the RAV4 that it has known is a problem but will not issue a technical service bulletin or recall. It is only covered by the extended warranty. Any other is 800 dollars out of pocket. The warning ️ on the dash blocks the speed indicator and cannot be cleared until the fix is made.

The contact owns a 2019 Toyota Rav4. The contact stated while driving approximately 65 MPH, the "Engine Maintenance Required Please See Dealer" message was displayed. The contact stated that the vehicle was driving normally, however the message was intermittent. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the coolant bypass valve needed to be replaced. The dealer advised the contact that the failure was associated to DTC code: P26815; and that coolant was leaking into the engine. The vehicle was repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 47,800.

"Engine Maintaince Soon" light continues to go off while driving the car making the dashboard impossible to use. Took the Rav4 to the dealership and said it would be $536 to replace the coolant bypass valve. It is a know issue on 2019 and newer Rav4 Toyota vehicles and needs to be recalled.

I am submitting this complaint regarding a recurring issue in the coolant bypass valve of my 2019 Toyota RAV4 XLE Premium. Although there was no check engine light, the vehicle displayed a persistent warning: “Engine Maintenance Required – Visit Dealership.” This message repeatedly appears on the instrument cluster and blocks critical driving information, including the speedometer, fuel economy, and other key displays. Even when the driver manually cancels the message, it returns within minutes. This creates a constant distraction and obstructs visibility, which poses a safety risk during driving. I took the vehicle to the dealership, where they diagnosed the issue as an internal short circuit in the coolant bypass valve and retrieved diagnostic trouble code P268115. This code relates to a malfunction in the valve’s electrical control system, which affects the engine’s ability to regulate temperature effectively. Toyota currently offers a Customer Support Program (CSP) for the coolant bypass valve, but only if the valve is leaking coolant. However, my vehicle's failure was due to an electrical defect, and Toyota does not cover this under the same program. This creates an unfair burden on owners and fails to address the full scope of the issue, even though the results—loss of proper cooling function—are the same. Many RAV4 owners have reported similar problems online, but no recall or technical service bulletin has been issued for this kind of failure. I believe this defect raises genuine safety concerns, including: Driver distraction due to persistent warning messages Obstruction of critical dashboard information Potential for engine overheating or reduced performance I respectfully request that NHTSA investigate this issue as a potential safety-related defect and determine whether Toyota should be required to expand support or issue a recall for this component, regardless of whether the failure is due to coolant leakage or internal electrical malfunction.

The vehicle began displaying intermittent warning messages and slipping into "Mud & Sand" mode without cause. Later, multiple dash lights illuminated, including 4WD malfunction, check engine, and coolant system warnings. The vehicle became undrivable due to overheating. Dealership diagnostics indicated a failure in the coolant bypass valve actuator the electronic water pump was receiving a signal but not responding, resulting in loss of coolant circulation and overheating. There may also be a shorted control panel causing related system failures. This problem has been confirmed by a Toyota dealer, and the vehicle remains at the service center pending resolution. The issue appears related to a known Customer Support Program (CSP 24TE04), which covers the coolant bypass valve actuator. There was no prior accident or abuse of the vehicle, and the vehicle only has 52,000 miles. Safety was compromised due to unexpected power loss and overheating while in motion. The issue has not yet been resolved. The vehicle has also experienced prior water intrusion through the roof rails, which may have contributed to electrical damage.

Defective coolant bypass valves on my 2019 Toyota RAV 4 have caused coolant to leak out of my vehicle’s closed-loop coolant system, harming its ability to properly regulate engine temperature. Eventually the message “Engine Maintenance Required Visit Your Dealer” appeared on my dashboard indicating an issue with the engine. An internet search of this error message on the 2019-2023 Toyota RAV4 and Corolla models shows this is a widespread defect, affecting hundreds of Toyota owners. A class action lawsuit has been filed against Toyota regarding defective coolant bypass valves in the 2019-2023 Toyota RAV4 and Corolla models [XXX] et al. v. Toyota Motor Sales, U.S.A., Inc) but Toyota has still not addressed this defect with a recall. “Engine maintenance required” maintenance light on dashboard. Very common issue within RAV4s. This message blocks the odometer on the dashboard when visible. It shows warning lights on the display as well. The part that needs replacing is a “coolant bypass valve”. My dealer wanted nearly $1000 to replace this part, and told me this part was on back order nationwide. My car begins to overheat after 1.5 hours of driving. The dealer said to pull over when operating my vehicle and it begins to overheat. This problem is an inconvenience as I commute to work in a busy city. This problem was first presented last week after my scheduled oil change with my local dealer. The next day (5/17/2024) the light came on my dashboard—unable to be removed. I first brought it into the dealer for a diagnostic on 5/20/2024, this is when they diagnosed a coolant bypass valve. I brought it back to the dealer today (5/23/2024) to have the part replaced, and was told it was on back order. I am afraid to operate my vehicle for long periods of time due to this issue. It has become a major inconvenience in my personal life this week, causing me to call out of work twice. This needs to be addressed by Toyota immediately.

The seat warmer/heater burned a hole through my driver's side seat and my coat. Luckily, I was not injured but I have severe concerns about this problem also occurring in the passenger seat where my children sit. This problem causes me worry about my own safety and that of my children. I am disappointed to be unable to use a feature that I specifically purchased this vehicle to use and enjoy. I currently have my vehicle at a Toyota dealership for diagnosis and evaluation.

The Data Communication Module has failed in my vehicle. This causes several major distractions and increases the risk of an accident. The media screen in the center of the dashboard will send an alert message every 15 seconds saying that ‘the connected device is not compatible’. At night, this alert message is especially bright and distracting. The lights on the SOS button are constantly blinking. More significantly, the SOS button does not work anymore. The only known solution to this problem is either to disconnect the DCM, or to pay more than $1300 to replace it.

I BOUGHT A BRAND NEW 2019 RAV4 LIMITED WITH 150 MILES ON THE ODOMETER, NOW HAS ABOUT 460 MILES. A FEW DAYS INTO THE PURCHASE, I NOTICED YELLOW TICK MARKED GLITCHES SHOWING UP ON MY DRIVERS CENTER MAIN CONSOLE. THIS MAIN CONSOLE DISPLAYS MY MPH, SPEED LIMIT, LTA, AND MORE. I THOUGHT NOTHING OF IT AS IT WENT AWAY FOR A FEW MINUTES WHEN I TURNED ON/OFF MY CAR, BUT IT WOULD SHOW UP AGAIN. Complaint Summary: Seatbelt failure on 2019 Toyota RAV4. Complaint Description: I own a 2019 Toyota RAV4, and the driver-side seatbelt has begun failing. The retractor and locking mechanism are not working reliably. Sometimes the belt won’t retract, sometimes it locks up randomly, and other times it doesn’t lock at all. This is a major safety hazard. I’ve owned many vehicles over my lifetime—older cars, newer cars, even vehicles over 50 years old—and I’ve never had a seatbelt go bad. This is the first time I’ve experienced a critical safety component failing like this. Toyota has issued seatbelt-related recalls on other RAV4 model years, but not on the 2019, even though the symptoms match what others have reported. This failure puts me at risk in the event of a crash, and it needs to be investigated. I am reporting this because I believe this is a safety defect, not normal wear, and it may affect more 2019 RAV4s than just mine.

At times it is difficult to connect seat belt into the lock I have to press release multiple times and try to lock in and click seat belt several times. It sometimes connect on first try and has taken 20 minutes to connect seat belt. As of 8/12/25 in for repairs at a cost $430 Driver and left set set belt lock replaced

The driver side seat belt has stopped functioning correctly. It engages just fine, but is very dificult to get it to release. I have to jerk on the belt to get it off. I am concerned that if I needed to exit the vehicle for an emergency, I wouldn't be able to get it to disengage. It started doing this around October 14, 2022. I will be making an appointment with a Toyota dealership to have it checked.

The seat belt for the driver's seat often wedges itself between one or another of the levers of the seat-adjustment mechanisms and the side of driver's seat. I am concerned that this may cause the belt to operate less than optimally.

I was driving to Kansas on a road trip and we stopped for gas when we noticed the rear drivers side tire tilting inwards (top of tire angled towards the car). We then drove it to a mechanic in Kansas to see if it is safe to drive further. She noticed the bolt for the rear suspension loose and almost out entirely. She then told us we could not drive the car any further because a few more bumps and the bolt would fall out and create major safely concerns. She also noted that she could see where the bolt was rubbing on the rear suspension sub-plate and could see the old rusted damage along with the new damage from recent driving. She also noted the uneven wear on the tire with the tread worn significantly more on the inner side of the tire. We towed it to Denver to have it looked at and the said the bolt and nut need to be replaced along with a tire alignment. There were no warning signs other than my car felt more bouncy than normal. The car bolt has been replaced but you can inspect the surrounding damage if needed. Both the safety of the people in my vehicle and those around me would have been at risk if we didn't notice the tilt and continued the road trip.

Toyota Rav4 2019 XLE - Engine Coolant Pump Performance/Stuck Off Error code P26CB milage 56,920 Toyota says this is NOT covered by the 5 year power train warranty. For years Toyota has manufactured this water pump that freezes up due to manufacturing defect that allows the magnetic base housing to rupture. Dealers want to replace entire pump (or at least charge for it) verses replacing the frozen plastic rotor. Dealer claims this is not covered under 5 year power train warranty. Why not? My car temp started climbing and took me hours to get home (10 miles) as I kept stopping for hours waiting for engine cool and sprinkling water from bottles over the engine and trans housing where it butts against the engine (silver brading). (This was late in weekend night - repair shops not open. I do see a recall investigation started (no recall yet) and just wanted to add my complaint to the issue. At first the temp gauge read normal but later started climbing. The engine light did come on the the DB2 said P26CB.

My vehicle 2019 Toyota Rav 4 Limited has transmission and engine issues as well as a roof rack leak. I have reported this to Toyota via 1-800-331-4331, which was not helpful. Israel stated that there is nothing that Toyota Cooperate can do about this issue. Both the transmission and engine had been previously repaired at Toyota of Laredo to no avail. Currently my vehicle is awaiting repair at Cavander Toyota in San Antonio. No loaner was provided despite me being a home health therapist who needs her vehicle to work.

1ST INCIDENT: SITTING AT STOP LIGHT FOR A FEW MINS AS NORMAL. WHEN TIME TO ACCELERATE, THE VEHICLE STARTED LURCHING FORWARD IN A HARD JERKING MOTION. 2ND INCIDENT: SLOWER TRAFFIC. WHEN DRIVING IN SLOWER TRAFFIC OR AT SLOWER SPEEDS, ~20-35MPH THE VEHICLE WILL START LURCHING JERKING FORWARD. TOOK BACK TO DEALERSHIP FOR INSPECTION, THERE WERE NO ISSUES FOUND. DEALERSHIP RESET INTERNAL COMPUTER, STATING THAT "VEHICLE IS OPERATING NORMALLY". NEITHER HAS ISSUE HAS YET TO CAUSE AN ACCIDENT WITH ANY OTHER DRIVERS AT THIS TIME. I DO TRAVEL WITH DOGS WHO ARE HARNESSED AND SEAT-BELTED IN THE BACKSEAT, THEY WOULD HAVE BEEN THROWN TO FLOOR OF CAR IF NOT SEAT-BELTED. FOR MYSELF, I GET TOSSED FORWARD SEVERAL INCHES AND HEAD THROWN FORWARD. WHEN DRIVING IN SLOWER TRAFFIC AND/OR STOP AND GO TRAFFIC, THE CAR LURCHING/HARD JERKING FORWARD WILL EVENTUALLY CAUSE AN ACCIDENT. ALL INCIDENTS HAVE HAPPEN ON DRY CITY STREETS. I WOULD ASSUME DURING THESE INCIDENTS OF THE JERKING FORWARD THAT THE BREAK LIGHTS DO NOT ALERT REAR DRIVERS THAT THE CAR IS IN A STOP/GO MOTION.

The engine on my Rav4 suddenly stopped without any notification while I was driving it on the freeway. I could have been severely injured from the risk of being rear ended by vehicles behind me. I have experienced this twice now, and both times, I have been on a busy highway in the middle of rush hour traffic. While driving, the engine would start sputtering and the vehicle will jolt and shake, eventually causing a warning message to appear that says power steering low, engine shutting off, place vehicle in Park. Then, when I turn the engine off, and try to restart it, it would flash a message that read Not ready to drive. After towing the vehicle to a service department, the vehicle would start up like normal, and the techs could not get vehicle to duplicate the issue. No codes would come up when they place it on a scanner, either. I researched the issue online, and it appears I am not the only one who has experienced this issue with the Rav4. I thought the issue was fixed after having my vehicle towed on 3/6 to Freeman Toyota Service where they determined the issue was due to a recall on the fuel pump. However, the incident occurred again on 3/8, and I had to have the vehicle towed as the engine shut off while I was driving on the freeway blocking one lane of traffic on the interchange that connects one freeway to another. I was very fearful of other vehicles crashing into me as some of them were squeezing by between my vehicle and the retaining wall to get around me. It took the wrecker over an hour to get to me due to high traffic time. I have uploaded 2 tow bills I had to pay, and the service invoice for the recall work done. My vehicle is currently at another Toyota Service department where they are telling me they are unable to duplicate the issue.

WHILE DRIVING DOWN I-25 SOUTH OF DENVER, CO IN HEAVY TRAFFIC AT 75 MPH, THE ENGINE ABRUPTLY SHUT OFF. I SUDDENLY HAD NO ENGINE POWER, POWER BRAKES OR POWER STEERING. LUCKILY I WAS ABLE TO COAST TO THE SHOULDER, BUT CAME CLOSE TO HITTING OR BEING HIT BY SEVERAL VEHICLES. WHEN WE MADE IT TO THE SHOULDER ALL THE WARNING LIGHTS AND MESSAGES WERE APPEARING ON THE DASH. AFTER LETTING THE CAR SIT A FEW MINUTES I RESTARTED IT, AND IT DID RUN, THOUGH ALL THE WARNING LIGHTS WERE STILL ON. THERE WAS A DEALERSHIP 9 MILES FROM MY LOCATION, SO I SLOWLY MADE MY TO ARAPAHOE TOYOTA. THEY RAN A DIAGNOSTIC AND SAID THAT SOMETHING CAUSED THE FUEL TO BE SHUT OFF TO THE ENGINE. THEY DID NOT KNOW WHY, AND COULD NOT REPLICATE THE PROBLEM. THEY SUSPECT IT IS AN ELECTRICAL ISSUE AS I ALSO FOUND OUT THAT THE RIGHT REAR BRAKE LIGHT DOES NOT WORK INTERMITTENTLY. SINCE THEY COULD NOT REPLICATE THE PROBLEM, AND HAD NO LOANER CARS, THEY ESSENTIALLY TOLD ME TO COME BACK THE FOLLOWING MONDAY AND HOPE FOR THE BEST IN THE MEANTIME. SINCE I WAS HEADING TO VACATION THIS IS IMPRACTICAL, SO I WAS STUCK DRIVING A CAR I'M AFRAID TO DRIVE. I'VE NEVER HAD A VEHICLE DIE WHILE DRIVING LIKE THAT, LET ALONE ONE WITH ONLY 4580 MILES. I'M APPREHENSIVE ABOUT DRIVING THE CAR 26 MILES IN THE MOUNTAINS TO THE NEAREST DEALERSHIP, AND SO FAR TOYOTA HAS BEEN NO HELP.

Sudden Unintended Acceleration....ONCE AGAIN! I was pulling into a parking stall of a shopping center at about 2-3 mph. I started to apply the brakes. Simultaneously, the car lurched forward hitting a retention pole. In an attempt to stop the car from going any further, I continued to press down on the brake pedal and shifted into reverse. The EDR shows that the gasoline engine reached 2200 rpms, while the hybrid electric motor reached 2300 rpms. The car performed a backward semi circle maneuver, eventually hitting a cement wall and coming to a stop. Even though there were two (2) impacts (one going forward and one going backward) no air bags were deployed. If there had been people in the area, they easily could have been killed. The damage to the car was extensive and the car was "totaled". The car manufacturer inspected the car and downloaded the Event Data Records. SUDDEN UNINTENDED ACCELERATION CAN'T BE REPRODUCED!! There were/are no error codes and all car manufacturers, including my automobile's, refuse to acknowledge the issue. At the very least, could NHTSA PLEASE REMIND THE PUBLIC THAT THIS IS A SERIOUS, ALBEIT INFREQUENT, DANGER?

Car hesitates when pulling out from a stop. The natural reaction is to depress accelerator more causing a shift feels like a lower gear and the car pulls out abruptly . This happens very often and I contribute it to lower gas mileage then car is rated at along with accelerated tire wear. I feel it is a safety issue when merging into traffic and causing a possible accident. I've taken my concerns to dealership on numerous occasions with complete disregard of condition calling it a " characteristic" of the vehicle. I feel there needs to be a recall to correct this issue.

Hybrid system malfunction. Repairs include replacement of motor cable #2 and frame wire. Advanced Corrosion was the explanation. Not covered under warranty (either 3yr/36K or platinum addt’l plan). Problem occurred at 36,366 miles (300+ miles just over standard warranty?) In the midst of delayed communicating with Toyota with no response after 3 days. Was told to reference TSB T-TT-0630-20. After researching, this problem is frequently referenced as being an issue. Should just be a recall at this point.

Engine maintenance light came on after a routine oil change. I took it to a toyota dealer, they said that it has something to do with the coolant system. The problem is ongoing with several toyota models, including rav4, Tacoma, Tundra, Camry, Sienna models 2018-2022. The problem is that the car can suddenly overheat and shut down on a busy highway, which is very Dangerous. Someone, can get killed, if this is not addressed. I don't know why there is not a recall already. The government should definitely get involved.

THE FUEL TANK IS FAULTY AND DOES NOT HOLD 14.5 GALLONS OF GAS AS LISTED IN THE OWNERS MANUAL. THE GAS GAUGE IS FAULTY AND IS NOT ACCURATE. OTHERWISE TOYOTA IS COMMITTING FRAUD BY MAKING SUCH CLAIMS WHICH MY RAV4 DOES NOT DELIVER. THIS HAS BEEN GETTING WORSE SINCE I PURCHASED THE CAR IN JULY 2019. THE TANK CAN ONLY BE FILLED WITH 10 GALLONS OF GAS AFTER THE FUEL GAUGE IS ON EMPTY AND THE 'DISTANCE TO EMPTY' IS ZERO WITH REFUEL NOW WARNING. THERE IS A PROBLEM WITH A FAULTY GAS GAUGE WHICH IS NOT ACCURATE. EVEN AFTER FILLING WITH 10 GALLONS, THE FUEL GAUGE DOES NOT REGISTER FULL AND SHOWS ONLY 7/8 TANK FILLED. 14.5 GALLONS X 35.7 MPG SHOULD SHOW 517 MILES TO EMPTY, BUT MILEAGE COMPUTER ONLY SHOWS 392 MILE RANGE. 392 MILE RANGE DIVIDED BY 35.7 MPG IS 10.9 GALLONS IN THE TANK. IF THIS CAR HAS A 14.5 GALLON TANK IT IS FAULTY. I HAVE NO IDEA HOW MUCH GAS IS IN THE RAV4 AND RANGE. THIS WILL PREVENTS ME FROM TAKING LONG CROSS COUNTRY TRIPS IN RURAL AREAS UNLESS I HAVE TO GAS UP AT SMALL RURAL GAS STATIONS AND NOT ALLOW ME THE RANGE TO KEEP DRIVING DURING LONG TRIPS. I AM TOLD NOT TO LET THE GAS TANK RUN DRY SINCE THAT COULD CAUSE OVERHEATING OF THE FUEL PUMP WHICH COULD START A FIRE IF HOT ENOUGH. Roof rail leak known to Toyota leading to potential compromise of SRS airbags and any other electrical wiring within anterior pillars. Long term water damage can lead to corrosion of components within that area. Toyota assessed and confirmed water intrusion due to roof rail leak. Leak became visible to owner due to water marks on headliner. Of note, Toyota refuses to accept blame for water intrusion and provide remedy.

As noted by many other 2019 thru 2021 Toyota Rav4 owners, there is significant water leakage intruding into the vehicle passenger compartment. I can confirm that the water intrusion that my vehicle has sustained is leaking through inadequately sealed holes for clips that are part of the Roof-rack side rails. And that this severe water intrusion is likely damaging the electrical system connectors associated with the vehicles Side Impact Air Bags and possibly the tire-pressure monitoring system (TPMS) connectors in the side body pillar areas. Our local Toyota Dealership Service Center is aware of this problem. They tell me that our standard and extended warranty will not cover the repairs, and that we should contact our auto insurance provider. I feel that should not be the case, and that this leak issue is going to cause severe injury or death due to the water leaks incapacitating the Side Impact Air Bag System and possibly other safety systems with electrical connection affected by the water intrusion.

The issue is the stock roof rack that comes with the vehicles. The clips that clip on to the body of the car have gaskets that deteriorate. Water leaks in through those holes and into the headliner of the car. The front SRS airbags are compromised and potentially rendered useless. The wiring that attaches to the airbags is also compromised because the water will eventually corrode it.

Roof rack seals failed, allowing water in above the A-pillar, which damages the headliner and could damage electrical and side airbags, compromising safety. In talking with other 2019 Rav4 owners, many have experienced the same issue as the sealing mechanism is poorly designed.

My 2019 Toyota RAV4 experienced a dangerous electrical and transmission failure that ultimately caused the vehicle to stall while driving and made me lose control. For several weeks before the incident, the vehicle showed subtle hesitation when accelerating but no warning lights. After the car stalled and spun out coming off the Ben Franklin Bridge onto I-76 near downtown Camden, police responded, confirmed the vehicle had unexpectedly shut off, and verified there was no crash damage. I later brought the car to the Toyota dealership, where they discovered corrosion on the battery cable, failed ABS sensors, and a weak battery. I paid $668 for a new battery, new cable, and ABS sensor repairs. At that time, the dealership did not inform me of any transmission issues and released the vehicle back to me. I made it only a short distance up the road before realizing the vehicle could not accelerate past 40 mph and was still malfunctioning, so I immediately returned it. The next day, the dealership initially claimed there was nothing wrong with the vehicle, but after I insisted they re-evaluate the car, they then informed me that the transmission was failing and needed replacement. This sequence of electrical instability, combined with the transmission malfunction, ultimately led to the earlier stall where I lost all power steering and braking assist and spun out into the median. The vehicle, diagnostic records, and replaced components are available for inspection. The sudden loss of power and control created a major safety risk to me and other drivers in a high-traffic area.

On 01/29/2025 at around 5:30pm I was driving home from work when suddenly the car stalled. I was going about 45 mph on a secondary to Lane Highway that was completely black because there are no street lights in that area. There were all kinds of warning lights flashing on the dash. I let my car drift to the shoulder was able to break and put it in park. I tried turning the vehicle back on but it would not start. My cell phone showed no Wi-Fi connection but I know the area very well and I knew I could walk several feet to a connection. Therefore spite concerns for my safety I had to leave my vehicle in order to call for help. I walked about 30 ft in the pitch black on this highway dressed in dark clothing on a shoulder that was very narrow. There was a pickup truck coming my way who flashed his high beams in my face but did not stop to see if he could help. Actually I was grateful for this because it's a pretty secluded area and I was very frightened for my safety. I was able to get a connection about 30 ft away from the vehicle and called my husband who said he would be on his way. I walked back to the vehicle got in and tried to start it. It would not start but all the warning lights were flashing on the dash. I decided I should take pictures of the warning lights in case we needed that for the dealer. After a couple of minutes I decided to try to start the car again and it did start. I was able to drive down the road to a Wi-Fi connection called my husband to let him know I was on my way. Once I got home the car seemed to be in order but there was one warning light telling us to take it to the dealer because there was something wrong with the hybrid system. We made an appointment to go in on January 31st. They inspected the vehicle to the tune of almost $200 and diagnosed the problem as follows: hybrid EV battery voltage system isolation internal internal electronic failure rear motor corrosion on both hybrid harnesses. Cost to repair $5,500.00

Hybrid Harness Malfunction. As reported on FaceBook by my wife to [XXX] of LIA Toyota: "Well my husband's RAV 4 wasn't safe and you knew it. And corporate knew it. You are in the business of saving lives by informing your customers when you find a defect in your cars. This faulty harness is dangerous. And the repair is costly and there aren't many of these parts available to make the repair. You put my husband and countless others in danger. Shame on you." Not only has the problem been confirmed: On May 11th, I received a letter from TOYOTA Motor Sales, USA, Inc. entitled, Certain 2019-2022 Model Year Rav4 Hybrid Vehicle (AWD Only) Coverage for High Voltage Floor Under Wire Harness and Rear Traction Motor Cable Corrosion CUSTOMER SUPPORT PROGRAM 22TE09 NOTIFICATION... There was a YouTube Video made by [XXX] about this problem A YEAR AGO entitled, "CableGate," AND an article at TorqueNews. (I have links if needed) The vehicle has been inspected and repaired by LIA Toyota in Rockland County. OVER $5,500 that I'm looking to get a FULL refund. LIA gave me back $1,000 for over-charging me. I want the FULL refund from Toyota Motor Sales, USA, Inc. A warning lamp/message appeared on May 6. I have emails, reports to corporate & screenshots of conversations with LIA service...all lasting almost 2 months. Toyota Motor Sales, USA, Inc. and LIA Toyota BOTH new of this problem a year ago BEFORE they sent out the letter. The letter states they would repair it for free if under 100,000 miles. I had 114,000, BUT...if I knew a year ago, I would have brought it in...UNDER the 100,000 miles, and got a FREE repair. My 2019 Toyota RAV4 Hybrid (VIN: [XXX] ) suffered sudden engine failure due to coolant intrusion. On Nov 20, 2025, Johnstons Toyota in Harriman, NY (RO#XXX) diagnosed the issue and recorded DTCs P26CA31 and P26CB, which indicate failure of the coolant flow control/bypass valve. The dealer confirmed heavy white smoke from the exhaust with a strong coolant smell and advised the engine had failed, recommending a replacement. This vehicle is part of the early 2019 Japan-built batch widely known for premature electric water pump and coolant-flow system failures. Internal impeller swelling, seal leakage, and bearing seizure can cause coolant pressure spikes and ultimately head-gasket failure with coolant entering the cylinders. This defect has been reported by thousands of RAV4 Hybrid owners (2019–2021). Toyota has addressed it quietly through internal TSBs such as XXX and XXX but no public recall has been issued. The safety risk is severe: the engine began emitting thick white smoke and ran roughly, risking sudden power loss, overheating, or engine seizure while driving. A stall at highway speed could easily cause a collision. Toyota USA opened Case #XXX but declined assistance despite the defect being well documented. This failure was sudden, with no warning lights prior to the incident. Given the widespread nature of this defect and its clear safety implications, I request that NHTSA open a formal investigation into the electric water pump/coolant system failures in 2019–2021 Toyota RAV4 Hybrids and consider requiring a safety recall. Stopped for gas on 11/6/2025 immediately after getting gas the truck shook twice, the power steering light and engine shut down light came on and the truck turned off. Waited for a few minutes to turn the engine again it wouldn’t start. Got it towed to my nearest dealership who kept it for a day. Said there was nothing wrong with the truck, there was no code showing and they couldn’t determine the cause of the problem.

I stopped for gas; my car would not start. The power steering fluid light came on. I went inside to purchase some but when I returned to the car, there was no place to pour it. After a few minutes the car started. About a month later, my husband stopped for gas and the car killed right after he left the gas station, about 10 minutes later, it started. Took it to a mechanic, they couldn't find anything wrong with it because the check engine light wasn't on and the car wasn't giving any problem codes. About two weeks ago, I stopped for gas, the car wouldn't start. Finally got it started, and the check engine light came on. However, it's still not reading any codes.

While driving or stopped at a light, car will switch drivings modes to “mud and sand” turning off the vehicle stability control and pre-collision system warning light will illuminate. After pressing desired mode, car will switch back to “mud and sand” at varying intervals resulting in me pressing the desired mode constantly while driving.

A Hybrid System Malfunction warning appeared about 1 week ago. The car was taken to the Toyota dealer who said that the car was safe to drive and that they would need to order a part and it will probably be weeks before they get it because so many other cars have the same problem. Three days later the car will not start with several errors including Brake Power low, PKSB Malfunction, and Precolllision System Malfunction. Toyota is not providing any coverage for any of our repair, car rental or now the need to tow the car.

This vehicle has continued to have issues. The most recent and dangerous issue is the electrical problems due to water leakage. Water getting into the vehicle's interior can be hazardous with all the solenoids, modules, and wires. The roof rack leaking is a common issue for these vehicles. It does not become an issue until they have been exposed to rainy environments or washed. The water in our vehicle leaked down the driver-side A-pillar, behind the steering wheel, and into the floorboard. This caused a shortage in wiring for the mud/sand terrain modes and the safety systems for the vehicle. This would happen at random times, including down the freeway at 70mph. The adaptive cruise control would shut off and emergency braking would be defective if needed. The water does flow across the airbag in the A-pillar which could cause a more serious issue. Now, there is a possibility of the airbag not deploying in the case of a collision. The racks leaking is something Toyota should be recalling or publishing a TSB.

The contact owns a 2019 Toyota Rav4. The contact stated that upon starting the vehicle, the messages "Parking Brake Malfunction", "Secondary Brake system Malfunction”, “4WD Malfunction", "Lane Assist Malfunction", "Pre-collision Malfunction" and messages indicating malfunctions of the electrical system, the engine throttle, and the transmission were displayed. The check engine warning light was illuminated. The vehicle was taken to an independent mechanic, where a spark plug and ignition coil were replaced; however, the failure recurred. The vehicle was taken to the dealer, where the contact was informed that all the spark plugs and ignition coils needed to be replaced prior to the final diagnostic test. The vehicle was not completely diagnosed and was taken back to the mechanic, where a specialist used a camera to diagnose that there was coolant intrusion in cylinder #4, due to a fractured cylinder head. The mechanic related to the failure of NHTSA Campaign Number: 20V064000 (Engine and Engine Cooling); however, the VIN was not included. The vehicle was not repaired. The manufacturer was notified of the failure, but no assistance was provided because the vehicle was not covered under warranty and the VIN was not included in the recall. The failure mileage was approximately 87,230.

After a recall repair order was issued, the repairs were made and now accelerating under load or uphill, the vehicle will accelerate and then bogs down or chokes (the vehicle then loses speed and no longer accelates). The issue has occurred several times while accelerating to freeway speeds on freeway on-ramps (it’s a safety hazard). 1st attempt to repair by the dealer was made on 11/22/2021. 2nd attempt by the same dealership is pending for today 12/08/2021 at 1:30pm.

Unknown component or system While driving on the Interstate during rush hour, traveling 60 miles an hour, another vehicle along side us entered our lane and we had to avoid a collision by aggressive braking and steering. Following this maneuver, our vehicle immediately shut down. An 18 wheel barely missed crashing into our rear. It took us approximately 1 minute to realize we needed to put the car in park, step on the brake, and push the ignition button to restart the vehicle.

NOW TODAY, I WENT TO START THE CAR AND IT WAS LISTING ABOUT 6 ERROR MESSAGES, EVERYTHING FROM THE BRAKE POWER BEING LOW, ENGINE FAILURE, ETC. The vehicle suddenly started to smoke and burn by itself when driving on the super crowded highway. I immediately stopped the vehicle and found that the burning site was the bottom of the front-left head of the vehicle (please see the images attached). The mechanician I visited after judged that the fire may due to failed electrical parts or system. He suggested to visit a Toyota dealer for recall. However, the Toyota dealer (Crown Toyota, 1201 Kettering Dr, Ontario, CA91761) declined my request with the reason that my vehicle has passed warranty time. The vehicle was just inspected in the Toyota dealer a few days ago. The dealer's inspection report shows that the vehicle does not have any problems besides suggesting to replace tires and front brakes.

My car frame floods every time it rains. This flooding includes my sunroof drain clogging. As a result of the water retention in my car’s frame, my cars safety system controller units are being corroded and thus I DO NOT have any reliable safety monitoring system. I have looked into this and this a COMMON complaint for Toyota 2019-2021 Rav4 sunroof cars. I have had the sunroof drain cleared multiple times but the issue remains. So far it has impacted my tire pressure safety monitoring systems. This is a MAJOR safety issue as the electrical safety monitoring system controller units are NOT properly functioning as a result of poor water drainage.

Vehicle is switching to mud and sand mode on its own, which according to a quick google search is a common electrical problem. Possible cause listed by other drivers is a roof rail leak. Upon inspection I see water stains on the inside of my headliner where the roof rails sit. So confirmed roof rail leak. I have not yet had this looked at to learn the full extent of the damage.

The roof rack appears to be leaking and water is coming into the car. The glove box was saturated with water and there was a puddle on the floor. There is water in the headliner. We're concerned about the side impact airbags no longer working and the wiring harness. I'm not sure how long it has been leaking but it was only just discovered yesterday.

I own a 2019 Toyota RAV4 and have experienced water leaking into the interior of the vehicle after rain and/or when taking the car through a car wash. I believe the water leakage is coming in via the small holes in the roof directly above the front driver seat and front passenger seats. The holes are used to align the plastic clips that help hold the roof rails onto the rood. They are leaking and allowing water to drain down into the interior of the car. This then allows the water to come into contact with the electrical wiring that runs along the top of the roof and down the front window pillars and possible the airbag system. Although I have not experienced a true accident or had a side airbag deploy for NO reason, I am extremely nervous about driving a car that could potentially detonate at any time potentially causing me to wreck! I am also extremely nervous about what affect water coming into contact with electric wiring and components might have on the proper functioning of the airbag system in the car. Although not purely safety related, I also wonder if the constant moisture penetrating the interior confines of the car could potentially expose me to BLACK MOLD and its long term dire health affects.

Initially thought a seal on the windshield was faulty, so contacted the business who replaced the windshield. Business owner came to owner’s house Mon, Nov 4, 2024. He did a thorough inspection of all areas where water was identified by owner (dash, under the dash, on the driver side floor) and confirmed it was not the windshield. He then inspected the roof rack and determined the roof rack was defective and leaking water from the ceiling of the vehicle, down into the dash, and ultimately onto the floor. He recommended contacting Toyota because this was a known issue. By 12pm on Mon, Nov 4, 2024 owner contacted Toyota alerting them to the issue. An appointment was made to bring the vehicle in for inspection on Mon, Nov 11, 2024. Owners do not have a garage or carport, so a canopy was pitched over the car in an effort to prevent any further water damage and moisture absorbing canisters were placed in the car. The vehicle was not driven in the rain during the period after first noticing the leak and bringing it in to the dealership. Evidence of Electrical Damage: On Friday, November 1, 2024 the driver side seat became extremely hot, to the point of needing to pull over and turn the car off. The seat heater was not turned on at any point during this time. On Saturday, Nov 9, 2024 and Nov 11, 2024 while driving on the highway, the car (normally driven in Eco Mode) unexpectedly switched into Mud&Sand mode and all safety features switched off. Owner was unable to manually override this occurrence. The “Normal” button did not work. After a few minutes, the vehicle switched into the default setting and owner was able to put it back into Eco Mode. On Nov 11, 2024 owner brought vehicle into Toyota for diagnostics. Owner requested that the electrical components also be inspected and was told it was not covered under the Customer Support Program.

A couple of months ago, I noticed water had somehow entered the inside of my car, collecting along the interior roof panel and inside the C-pillar close to the top of the driver door. I did a little research to see if I could figure out what the cause of it was. I found that many other owners of 2019 and 2020 Rav4's had similar problems. Their findings indicated that Toyota had equipped poor quality gaskets, along the the area in which the roof rail trim were fastened to the roof of the car. I also read that this problem could be fixed by the owner, but was unaware that the water leakage would come in contact with electrical components. Yesterday, I experienced first hand the result of this situation. When starting my car, the display board was indicating a multitude of warnings for system malfunctions. These included power steering malfunction, brake system malfunction, pre-collision warning malfunction, lane-monitoring, and on top of this the RPM gauge and engine temperature gauge were both malfunctioning. These warning/indicators persistently flashed and caused a loud beeping sound to go off. The warnings also informed me to not drive the car and see a dealer immediately. After taking it to the dealer, the technicians were able to confirm that water has leaked through the roof rail panels from the area I previously specified. They found water on fuse box/junction connectors. They also found corrosion on pins on driver front door connector, under dash harness connector and body harness connector. While the water damage did not lead to a car accident, it did cause serious concern for the potential risk of car malfunctions. This information will be soon be reviewed by my insurance provider.

Around 2:30 pm on Thursday, December 2, 2021, my wife and I were on our way to the gym for a workout. I was pulling my auto out of my garage. It was in reverse when it stopped half way and did not finish backing out. I put it in drive and then in reverse and I was able to back out. Once out of my driveway, I backed out into the street in reverse. From reverse, I put my car in Drive to start driving on the street. At this point, the car spun out of control and I could not control its movements. The car turned around on the street and drove into my yard. It ran through the yard, spun around through some hedges, onto my driveway and continued back into the yard. At this time it hit a tree in the yard, went back on the road, turned into my driveway before coming to a stop. We got out and inspected the damage. The entire front bumper and the assembly was hanging from the body of the car. The damage was extensive. Thankfully, no one was on the road and the only damage was to my car and my yard. shrubs and tree. This car is a Limited Edition with cameras and braking functions that stop the car if an object comes in its way. The car kept accelerating and did not brake to avoid collision. Note: We bought this 2019 RAV4 in March 2019. It was brand new, full loaded with many safety features. In October of 2019, we heard sounds coming from the front of the car. We took it to Toyota dealership to have it diagnosed. They determined that the Electromagnetic Clutch Assembly needed to be replaced. The car had 7100 miles on the Odometer.

2019 Toyota RAV4 Hybrid XSE with 42000 miles on it. Engine light with message came on. “Malfunction in the electronic control of the engine, throttle or automatic transmission has been detected”. Toyota dealership said it was Code P0128. Seems like this is prevalent in the RAV4 when checking forums and owner sites. Why is there not a Recall for this matter for my VIN number. Toyota has other “engine coolant” recalls and I believe this one should be included! I am being charge almost 600$ for this repair!

AT THIS TIME THERE AT LEAST 24 COMPLAINT ABOUT A "LURCHING" PROBLEM AT LOW SPEED AND RE ACCELERATION. A VERY SIGNIFICANT "LURCHING" ALMOST"STALLING" IS VERY APPARENT AS THE COMPLAINTS INDICATE. My check engine light is on due to O2 sensor being dirty. While this light is on some features in the car are disabled like cruise control. It might also be that the ABS braking system is turned off! I am not sure but once when the car in front of me abruptly stopped I was surprised my emergency braking did not kick in.

Pre-Collision System (PCS) failed. My daughter during sunset (sun glare) struck the vehicle in front of her, that suddenly braked due to sun glare. Toyota hasn't look at PCS system yet and the car is at the repair shop, but I did open a case with Toyota to investigate failure and access onboard data. Auto repair shop will check the system, but no malfunction warning on the car's warning display. There were no warning present prior to accident or post-accident.

The car transmission hesitates to accelerate at slow speeds and jerks or lurches forwards in a uncontrollable way. Dangerous at stop signs or on slow or hilly roads. Dealership does not directly acknowledge this issue, despite mechanic mentioning very clearly it is related to T-SB-0107-19 which Toyota dealerships charge to repair. No warning messages. Issue has existed as long as I have owned the vehicle 1.5 years from 61,000 miles to 71,000 miles.

Started normally, but had a message on the screen saying “Check Fuel Cap.” I needed gas anyways, drove about 3 minutes to the gas station, turned engine off, put gas. Turned engine back on and within 3 seconds it shut off. The screen displayed “engine stopped- Power steering low.” I tried a handful of times to start, it did not. After 20 minutes and some rest, it started. With no more messages. Battery and alternator were tested, both were good.

MY BOUGHT MY VEHICLE BRAND NEW ON DECEMBER 10, 2019. ON OCTOBER 29, 2020 THE BATTERY STOPPED WORKING WITH ONLY 10K MILES. I CALLED TOYOTA AND TOLD THEM MY BATTERY NEEDED TO BE REPLACED. TOYOTA TOLD ME TO JUMP START THE CAR AND DRIVE THERE TO REPLACE THE BATTERY. I ADVISED MY CAR WAS STUCK IN THE GARAGE AND WE WOULD NEED SOMEONE TO TOW THE CAR. WHEN THE 'TOW' ARRIVED IT WAS ONLY A MAINTENANCE WORKER THAT JUMP STARTED THE VEHICLE AND SAID I WOULD NEED TO DRIVE IT MYSELF THERE. THE WORKER ADVISED ME OF THE DEFECTIVE BATTERIES FOR RAV4 AND OTHER MODELS. ONCE MY CAR WAS TURNED ON AND OUT OF THE GARAGE, I WAS PULLING OUT OF MY DRIVEWAY WHEN MY POWER STEERING LOCK WARNING SIGNAL FLASHED. I CALLED AND EXPLAINED I WAS AN HOUR FROM THE DEALERSHIP AND NEEDED MY CAR TOWED TO THEM AND THE WARRANTY I PAID FOR SHOULD COVER IT. SUBSEQUENT TO GETTING MY VEHICLE BACK, I RECEIVED PAPERWORK IN THE MAIL WITH A RECALL NOTICE ON 12/9/2020 EXPLAINING THE POSSIBILITY OF MY VEHICLE STALLING WHILE DRIVING, WHICH COULD INCREASE THE RISK OF A CRASH. I have been experiencing water intrusion with my vehicle, I first noticed it by a mildew smell. Acknowledged under Toyota’s Customer Support Program 22TE05. This water leak has led to multiple secondary issues, including: Interior water damage Clogged A/C drainage system Sunroof drainage issues I reported this issue to a Toyota, but they denied repairs under 22TE05, claiming the issue was only due to a sunroof leak and clogged drains. [2/14] – Took my car to AutoNation Toyota due to a persistent water leak. The dealership diagnosed the issue as a clogged sunroof leak and clogged drains but did not provide clear documentation that they properly inspected the roof rail gasket. [2/18] – Dealership denied repair under 22TE05 but then stated they could replace the roof rail gaskets if my insurance paid for it—which contradicts their claim. [2/18] – I escalated my case to Toyota Corporate for review, but I am still waiting for a response. My car was left outside in the rain, potentially worsening the water damage. The dealership never provided me with a written repair estimate. Safety Concerns & Impact on Vehicle Use: I am worried about driving my car due to potential electrical system failure and airbag malfunctions caused by water damage. According to industry knowledge and Toyota customer experiences, a roof rail leak can directly contribute to all of the issues the dealership claims my car has: Clogged A/C Drainage System – Water leaking from the roof rails can enter the A/C system, causing drainage backup, mold, and excessive moisture buildup. Sunroof Leaks & Drainage Problems – If excess water enters through roof rail failure, it can overwhelm the sunroof drainage system, making it appear clogged when the root cause is actually the failing roof seal. Interior Water Damage – If the roof rail clips fail, water leaks into the headliner, side pillars, dashboard, and footwells, leading to electrical issues, carpet damage, and excessive moisture inside

Car randomly switches into different driving modes, BSM and anti collision goes off while driving. Seat warmers randomly come on high and can’t be turned off. Dealership identified water damage from leaking roof rack resulting in mold and electrical damage. Possible damage to airbags as water leaking into the pillar at front window. The vehicle was totaled due to a front end collision. The air bags did not deploy, the seat belt did not lock and the emergency alert when in an accident did not work. The driver was trapped in vehicle and unable to get door open, the vehicle was going about 40-45 mph at the time of the collision and the driver was flung forward causing whiplash and a concussion.

Dear NHTSA, I am writing to formally lodge a complaint regarding a recurring issue with my 2019 Toyota RAV4, which I believe poses a significant safety concern. The check engine light on my vehicle frequently illuminates, resulting in the disablement of the all-wheel drive (AWD) system. I initially brought my vehicle to a Toyota dealership, where they replaced the charcoal canister (EVAP) three times based on the diagnostic code. Despite these repeated repairs, the problem persists. Suspecting that the issue might be related to the dealership's service, I sought a second opinion from another Toyota dealership. Unfortunately, they provided the same diagnosis and solution, which again failed to resolve the issue. Given that the AWD system is a critical safety feature, especially in adverse driving conditions, I believe this issue should be considered a safety concern and potentially covered by a recall. Further given that the EVAP system is directly tied to the fuel it also should be considered a safety concerns. The repeated failure to address the root cause of the problem not only undermines the reliability of the vehicle but also compromises the safety of its occupants. I kindly request that the NHTSA investigate this matter and consider issuing a recall to address this widespread issue. [XXX] - Driving my 2019 Rav4 Hybrid Se nearing [XXX] [XXX] traveling 25-30mph suddenly felt like car was moving faster, stop sign came up fast, slammed on brakes with 0 effect! Yelled [XXX]? as I shot across the highway, down the shoulder, coming to rest against a couple small trees. No air bag deployed, wearing seat belts of course, still bumped head on ceiling causing small laceration, neck trauma, and Dr found I had also suffered a Vitreous Detachment (eye). Did not lose consciousness. Vehicle damage appeared to be front left corner but enough underneath to total vehicle. Very fortunate no other vehicles involved and that debris brought car to a stop instead of a big tree or wall. Two witnesses heard brakes... "Engine Maintenance Required See Dealer" message keeps appearing on my bashboard took it to my local Toyota dealer and they confirmed the problem is the coolant bypass valve/hose $150 to find the find the problem and $700 to fix apparently this is common in 2019-2022 RAV4s. Now my car won't drive for long without over heating I have only had my car for a year and a half and drive less than 5,000 a year.

THE STEERING WHEEL MOVED WITH FORCE AND SPEED--HAD MY REACTION TIME BEEN HALF A SECOND LONGER, I WOULD HAVE STRUCK VEHICLES NEXT TO ME. SECONDLY, THE VEHICLE'S FULL-RANGE ADAPTIVE CRUISE CONTROL BRAKED ABRUPTLY, ESPECIALLY AT LOWER SPEEDS, ON NEARLY EVERY OCCASION WHERE HIGHWAY/CITY TRAFFIC SLOWED OR STOPPED. After driving for 5-10 minutes, the center console screen stated: “A malfunction in the SRS Airbag System has been detected. Contact your dealer in order to have your vehicle inspected immediately” This continues to happen on subsequent trips. It only happens when somebody is actually sitting in the front passenger seat. There were no warning signs or symptoms prior to this message appearing. It makes me very concerned about passenger and driver safety and that the airbags may not deploy when needed.

The contact owned a 2019 Toyota Rav4 XLE. The contact stated that the vehicle was experiencing excessive lane assist efforts. The contact stated while driving at various speeds near a right turning lane, the lane assist engaged erroneously, causing the vehicle to jolt and pull towards the right turning lane. The vehicle was taken to the dealer several times; however, the contact was informed that the lane assist was operating as designed. The contact stated while driving 75 MPH with cruise control mode a rainstorm suddenly started. The contact stated he disengaged the adaptive cruise control mode and allowed the vehicle to decelerate. The contact stated that the vehicle jolted to the right. The contact stated while attempting to correct the steering wheel, the vehicle hydroplaned. The vehicle veered off the road at 50-60 MPH and hit several trees, was stuck in the trees, and became crushed. The air bags deployed. The driver's side windshield was shattered. The contact was able to push the door open and to climb out of the vehicle. The contact also received assistance from bystanders. The ambulance examined the contact but was not immediately transported to the hospital due to cost. The contact later went to the hospital where he underwent a brain scan, but no brain damage was found. The contact sustained nerve damage in left arm, injured leg, left arm twitching, head trauma, back, neck, and heart injury. The contact also sustained significant PSTD. The contact stated that a branch that pierced the engine was initially pointing toward the driver's side cabin. A police report was filed but the contact was irate and could not provide the information over the phone. The vehicle was towed and totaled. The manufacturer was notified of the incident, but no assistance was provided. The failure mileage was approximately 11,386.

I was stopped at a red light. When the light turned green and I pressed the gas, the engine sputtered and almost shut off. Just before I came to a stop, the engine revved up and the vehicle accelerated. The uneven acceleration is concerning. I recently replaced the gas cap and serviced the fuel system, which leads me to believe there is a defect with the engine. Later in the day, the check engine light went on and AWD system, cruise control, and lane assist were disabled.

Engine: I got the 100k engine maintenance done. I replace my coolant and got an oil change. The engine maintenance required warning is still on my dashboard and will not go away because the diagnostic reader states I need a new coolant bypass valve. This is not apart of the factory maintenance, alot on rav4 owners are dealing with this, and it is very pricey. I think it is best for this type of situation to be recalled. Brakes: I got my front brakes replaced and I had issues with my front calipers. My mechanic said both calipers had holes in them and caused my brake fluid to leak which has affected my braking alot. It takes a long time to brake and I’ve experienced this even after I’ve paid alot of money to get my driver said caliper replaced, still need to get my passenger side replaced. My mechanic only uses toyota brand supplies so I’m not sure why there isn’t any longevity in the quality of supplies. I’ve only had the vehicle for 2 years.

Maxi-Cosi Emme 360… MAJOR SAFETY CONCERN! When in the locked position with confirmed “locked” emblem visible, the car seat can easily be knocked loose and rotated. There are of course several warning labels on the side that say do not attempt to rotate the seat while in the locked position without using the appropriate release handle. The amount of light force I am apply is extremely minimal and nothing compared to the torsion that will be exerted to the car seat during a serious collision. We have already contacted the manufacturer directly, this is now the second car seat we have received with the same issue. We have been on the phone with tech support. They have no idea what’s happening and cannot resolve the issue. Again, this is a very serious safety concern with the car seat.

I bought this car last year on October 4th from carmax (mileage was 62k at the time of purchase). I started to have problems the next month on November 6th with the brakes which was rusted and had to replace the pads. That was very dangerous and could have hurt myself driving around the heavy rain at the time. I was scared for my safety and others wellbeing so I took the car to the dealership. Their mechanic checked the car and told me that this wasn't fixed or checked since April 2023 before I bought the car in october 2023. I had problems with the infotainment system. the map navigation for my android and iPhone doesnt work anymore. the mechanic at the dealership fixed the rear break pads and discs but couldnt fix the infotainment system even after updating the system. He issued me a 250 dollar refund and was charged 680 dollars for the rest of the job. Another issue happened on March 2024 for the front barring needed to be replaced from a carmax mechanic shop. Paid 380 to fix. This month I got a message saying " engine maintenance required, go to dealership". I took the car to the mechanic and explained that the computer code error message means the coolant valve is defective. I'm thinking of going to the dealer tomorrow up until I found out the car never got the recall repaired from this website. Could this be the reason why I'm having this may problems? The car hasn't been inspected by police or insurance, only the toyota manufacturer on November 6th who told me about the car not getting fixed since April 2023. I have the invoice of all these repairs done. The latest mechanic that told me about the coolant valve being defective thought I needed to change my oil. He did change the oil and charged me 189 dollars. I'm planning on taking the mechanic to fix that problem since the toyota dealership was gonna charge me to fix the transmission fluid which isn't the problem. I will upload the receipt for the oil change job.

Toyota service bulletin: T-SB-0079-20 (August 14, 2020) Outer Rear View Mirror Image Vibration While Driving (Causes distraction and could lead to and accident because image in side mirrors are unusable due to vibration) Dealer refuses to do this repair because it was only covered under a 36mo or 36,000 mile warranty. My car currently has 23,500 and this repair should be covered...or be a recall! Some 2019 – 2020 model year RAV4 and RAV4 Hybrid vehicles may exhibit a vibration/shaking of the image shown in the outer rear view mirror(s) while driving. Received multiple malfunction alerts. BSM & RCTW. Other safety features also inop. Dealership told me it was due to non-factory installed Cross Bars, faulty gasket allowed water into vehicle and flowed down onto the Engine Control Module. There was no way the amount of water said to have been in vehicle could have come from that 1 gasket, over 1" above roof. After $825+ and removal of cross bars water still leaking into car. Concerned issue will re-occur making safety features worthless, as well as damage side air-bags. These issues would all need to be corrected via out-of-pocket expense. Believe the issue is design flaw from Toyota.

Hatch hinge cracking, bending, and breaking causing hatch door to pull to one side. Hatch door pulling to one side caused the door to rub on the tail light when opening and closing and door doesn’t not seal when shut. This issue was not caused by any foreign objects or accidents. The issue is available for inspection. The issue has been seen by a dealership. The issue has not been inspected by any other entities. The first sign of the issue occurred in January 2023 with visible evidence the door was pulling to one side. The issue has continued to worsen.

I have had my RAV 4 since it was brand new. Today while i was driving, the mud and sand screen popped up automatic with traction control light, later i was able to turn it to normal then the light would go off for a while and it comes back again. It all happened while i was driving in I 355 going down to Lemont, IL from Palatine IL. I had the same problem but not with the mud and sand but with snow back in 2019 when it was a brand new, took it to a dealership and they said they have never seen like this before and they just reset the computer and it went off. I am pretty sure when I first bought it, even i push each and every button of either snow, sports or mud, the traction light wouldn’t come off. While driving on i 355 today, my car slide for a while which was the most scary part, first I assume it was raining but after when the rain stopped too, it was skidding while applying the brake. I have the regular maintenance, just did the brakes with rotors, changed tires, oil change every 3-4k miles. I tried to talk to Toyota 800 number but wasn’t able to connect with them.

On 12-April, I filed Report #11460592 regarding stuttering and uncontrollable acceleration of Toyota RAV4, 2019 model year, <22000 miles, incident occurred 11-April. Vehicle was taken to Toyota dealership service department on 12-April, returned that day as described in previous report. On 13-April, one day after repair, exact same issue occurred. Family (with 2 children) driving on road, 45MPH speed limit. Hybrid system malfunction alert occurred again. Speed limited drastically to approximately 15mph, with occasional reaccelerating uncontrollably. Vehicle exited as safely as possible and returned home. Vehicle then taken back to dealership for service. Full text of Report #114605 (11-April incident): Driving on 35MPH limit road. Vehicle began to stutter. Vehicle began to communicate warning lights - "PKSB Malfunction" and "Hybrid System Malfunction" and "Check Engine" and alerted to visit dealer. After stopping at red-light, received warning to pull over and stop driving. Vehicle could not exceed ~10mph no matter how much accelerator was depressed and received warning that vehicle had ~2.5 miles until it would cease being able to be driven. For rest of trip to home (approximately 2 miles), vehicle would alternate between 10mph limited state and returning to full acceleration which led to very unsafe driving conditions and abrupt changes in speed. Reached home and parked in drive way. After a few hours, returned to vehicle to take to dealer for service. All 3 error messages remained but no further limitation to 10mph or abrupt speed changes. Dropped off for service at dealership. Dealer notes: "Hybrid system malfunction, check engine light - SCAN FOR CODES, FOUND U010087 LOSS OF COMMUNICATION BETWEEN PCW AND ECM / CLEAR CODES, UPDATE SYSTEM, OPERATING AS DESIGNED." Upon receipt from dealer, no further issues and drove home safely. Vehicle is a 2019 Toyota RAV4 Hybrid Limited, bought new in Jan. 2020. Approximately 23000 miles, no previous issues.

Took my 2019 RAV4 to Toyota of Orange. Like couple months ago for fuel pump recal but my vehicle stalls when driving on freeway or streets. Looses speed for 4 seconds and its scary reported issues to Toyota of Orange and they don’t fixed anything they only want to sell. They offer me services I don’t need for my vehicle. Got 2021 RAV4 rental from Enterprise and that one work just find no problems had it for about a week. If could get another another dealer that will be great.

I bought my vehicle new in August 2019, it currently has approximately 30,500 miles. A few months ago I noticed a water stain by the roof, right on the A pillar on the passenger side. I took it to Toyota and they determined it was from the sunroof, according to them one of the drains was clogged with leaves and dirt (prior to this I had NEVER opened/used the sunroof, not sure if leaves are able to get in there without ever opening the roof). They cleaned the water stain and sent me on my way, without checking for water damage. After doing some research online found out this was happening to quite a few frustrated people and seemed to happen in the 2019-2021 RAV4 vehicles, most people were told it was the sunroof but further research revealed the possibility that it was the roof rail that contained bad seals. The water is now pooling on both A pillars right where the airbags are housed. I feel there is a good possibility those airbags will not deploy properly in case of an accident. In addition, multiple customers have complaint of this issues causing electrical problems as well.

The contact received notification of NHTSA campaign number: 20V682000 (Fuel System, Gasoline) however, the part to do the recall repair was unavailable. The contact called the local dealer and it was confirmed that parts were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not notified of the issue. Parts distribution disconnect. The approximate failure mileage was 25,000.

The contact stated while attempting to park the vehicle, depressing on the brake pedal but the vehicle began to accelerate independently causing the vehicle to crash into a tree trunk. The contact stated she had chest and neck pains due to hitting the steering wheel and did go to urgent care but did not seek medical attention. A police report was filed. The contact owns a 2019 Toyota Rav4. The contact stated while driving 40 MPH, the vehicle was making an abnormal humming and grinding sound. There were no warning lights illuminated. The contact received a recall notification for an unknown recall in regards to a humming sound coming from the transfer case of the vehicle; however, the dealer diagnosed that no issues were found and informed the contact that the recall repair did not need to be completed on the vehicle. The contact stated that the failure had been recurring while driving and he had been hearing abnormal bumping sounds from underneath of the vehicle. The dealer was made aware of the failure. The manufacturer was not notified of the failure. The vehicle was not repaired. The approximate failure mileage was 3.

Seat heater turns on automatically and randomly and gets way too hot. I'm afraid it might burn me. I have an appointment with the Toyota dealership however it's my only mode of transportation and I work 3 hours away from home. This puts me at high risk of getting hurt. None of the lights or any indicators came on. There has been no indication that there might be an issue before today. Today is 6-25-2021.

Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes, while driving on the interstate the vehicle began vibrating heavily. What component or system failed or malfunctioned, and is it available for inspection upon request? Upon inspection it was noticed that the inner tread of the left rear tire was damaged, worn away and scalloped or cupped. The tire was replaced with a spare, and vehicle driven to the nearest Toyota dealer for service. The damaged tire is available for inspection. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the servicing dealer informed us that the left rear camber was out of manufacturer specification being -1.8 deg. The minimum allowed is -1.7, and the minimum recommended is -1.3. We were informed that to be safe the vehicle required repairs; however, they were unable. We then took the vehicle to the selling dealer who also found the issue and were unable to repair. Two additional service centers also confirmed the rear camber was out of specifications, and posed a safety risk. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes, the dealer stated that a manufacturer representative from Toyota inspected the vehicle. How was your safety or the safety of others put at risk? Safety was compromised as the damage to the tire was severe and hidden. This damage could have resulted in a loss of control of the vehicle. The vehicle remains in the unsafe condition. It is also important to note that the tires were being rotated every 5,000 miles by a manufacturer service center as recommended.

WAS AT DRIVE THROUGH CASH MACHINE, STOPPED, PUT INTO PARK, DID MY TRANSACTION, PUT CAR INTO DRIVE LEFT TO GET TO STREET VIA A SIDE STREET. NOTED AT STOPS THAT THE BRAKES WERE HARD AS WHEN ABS IS NOT QUITE RIGHT. ON A TOWN STREET, TWO PEDESTRIANS STEPPED OUT INTO MY LANE OF TRAVEL. I PUT ON MY BRAKES, HARD. THE ENGINE REV'D AND CONTINUED FASTER AND I KEPT MY FOOT ON THE BRAKE, THE WOMEN GOT OUT OF THE LANE. I REALLY DON'T RECALL SEQUENCE, I WAS TERRIFIED (HAVE HAD A CAR RUN AWAY WITH ME LONG AGO). TL* THE CONTACT OWNS A 2019 TOYOTA RAV4. WHILE DRIVING APPROXIMATELY 2 MPH AND ATTEMPTING TO PARK, THE VEHICLE DID NOT STOP. THE BRAKE PEDAL WAS DEPRESSED AT THE TIME OF THE FAILURE. THE VEHICLE CONTINUED TO MOVE, DROVE OVER A CURB AND SIDEWALK, AND CRASHED INTO A STOREFRONT. POLICE REPORT NUMBER: [XXX] WAS FILED. THERE WERE NO INJURIES. THE FRONTAL AIR BAGS DID NOT DEPLOY. THE VEHICLE WAS TOWED TO A BODY SHOP. THE VEHICLE WAS NOT TAKEN TO A DEALER FOR DIAGNOSTIC. TESTING OR REPAIRS. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND STATED THAT THEY WOULD SEND A FIELD MECHANIC TO DIAGNOSE THE VEHICLE. DRIVING AT A SPEED OF 60MPH I HEARD A STRANGE NOISE ARISE FROM THE REAR PASSENGER SIDE OF THE VEHICLE AND SMELLED A BURNING (ELECTRICAL) SMELL. I CUT OFF THE HEATER AND SEAT WARMERS AS A PRECAUTION, SLOWING THE CAR TO A SPEED OF 30-40MPH. APPROX. 2 MINUTES LATER, MY DASHBOARD LIT UP WITH ABOUT 4 CONSECUTIVE WARNINGS 1. ANTILOCK BRAKE SYSTEM HAS MALFUNCTIONED. 2. LANE TRACING ASSIST HAS MALFUNCTIONED. 3. REAR CAMERAS HAVE MALFUNCTIONED. 4. PARKING ASSIST WITH AUTOMATED BRAKING (PA W/AB) HAS MALFUNCTIONED. BRAKES CONTINUED TO ENGAGE, BUT FELT STIFF AS THOUGH THEY MAY STOP WORKING. WAS ABLE TO SAFELY TAKE BACK-ROADS TO MY LOCAL DEALER (VANCOUVER MCCORDS TOYOTA) WHERE IT IS STILL UNDERGOING INVESTIGATION.

TL* THE CONTACT OWNS A 2019 TOYOTA RAV4. THE CONTACT STATED THAT THERE WAS AN OBSTRUCTION REFLECTING OFF THE DASHBOARD ONTO THE DRIVER'S SIDE REARVIEW MIRROR. CHEROKEE COUNTY TOYOTA (301 LIBERTY BLVD, CANTON, GA 30114, (770) 704-9525) WAS CONTACTED AND SUGGESTED THAT THE CONTACT TRY TO GET USED TO THE REFLECTION ISSUE. THE DEALER TRIED TO GET THE CONTACT TO SWITCH TO A CAMRY, BUT SHE REFUSED. THE MANUFACTURER WAS CONTACTED AND PROVIDED A CASE NUMBER AND A CASE MANAGER. THE MANUFACTURER PROMISED A CALL BACK AND SCHEDULED AN APPOINTMENT, BUT IT WAS NOT KEPT. THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE FAILURE MILEAGE WAS 48. *JS

1. TRANSMISSION SUDDEN JERKING/SHIFTING WHEN SLOWING DOWN IN CITY AT TRAFFIC LIGHTS THEN TRYING TO ACCELERATE ANYWHERE FROM 5-10 MPH WILL THEN JERK BACKWARDS THEN FORWARDS MAKING THE CAR JUMP ABOUT 10 INCHES FORWARD. CAN BE REPLICATED AT ANY TIME. 2. CARS FRONT END SUSPENSION SEEMS TO SQUEAK/GROAN WILL PULLING INTO MY BANK OR DRIVE WAY, ALMOST ANY UNLEVELED PAVEMENT. THIS HAPPENS AT LOWER SPEEDS WHEN I CAN HEAR IT WITH VERY LITTLE WIND NOISE. 3. MY SEAT IN THE VEHICLE BROKE AT 7,000 MILES. 4. MY BRAKES SOUND AS IF THE ROTORS ARE BENT WITH SLIGHT VIBRATION IN THE BRAKE PEDAL. 5. NO FIX FOR TRANSMISSION. TODAY THEY ARE REFLASHING THE PCM - POWERTRAIN CONTROL MODULE. WE'LL SEE IF THAT WILL FIX THE ISSUE BUT I HIGHLY DOUBT IT FROM THE DAMAGE THAT HAS ALREADY OCCURRED. I AM WORKING WITH TOYOTA CORPORATE WITH THE ISSUE.
