We discovered severe mold contamination and water intrusion in our 2023 Toyota RAV4 Hybrid. My wife, our [XXX] child, and I have been experiencing dizziness and nausea for months whenever we were inside the vehicle, but we did not know the cause until the dealer (DLL) showed us extensive mold throughout the cabin and standing water trapped inside the vehicle. The dealer confirmed that this was not normal wear and tear. The mold has spread into the floors, interior panels, and even the steering components. This is a serious health hazard. After doing research, we found many other RAV4 Hybrid owners reporting similar water leaks and mold growth issues. This raises the concern of a potential design or sealing defect in this model. This vehicle is unsafe to drive, especially with a young child. We are requesting a full safety investigation into the water intrusion, sealing integrity, and air quality risks associated with this model. Symptoms: • Dizziness • Nausea • Headache • Strong mold smell • Corrosion of interior components This is a serious safety and health issue, and we are asking NHTSA to investigate. When driving, I occasionally hear a sloshing sound. After inspecting the vehicle, I realized it was coming from the rear passenger door of the RAV4. After putting down large bowls to catch the water, I pulled up on the rubber weather strip at the bottom of the rear passenger door and water gushed out. I collected over 13 cups of water out of the door. This is not an isolated incident, as it has happened before. At first I thought it was debris/a fluke that caused this water to accumulate, but after reading forums, there are multiple people out there running into the same issue. It's only ever been the rear passenger door too; none of the other doors are holding water.

I was driving yesterday when we suddenly heard a pop on the roof. We pulled over and noticed that the sunroof was broken, with the glass sticking outward, as if something from the inside had exploded outward. The dealership would not take ownership of the damage, therefore i had to use my insurance. upon inspection I was told that it was pressure from inside the vehicle that caused it to explode and that they have seen it before in Toyota RAV 4 models

The issue is with the moonroof, which is part of the panoramic roof on the 2023 RAV4 hybrid driving down the freeway at highway speeds. The moon roof shattered. It sounded like a shotgun and it seemed to have broken and upward direction versus downward. There were no rocks from cars noticed in any vehicles in front of me to which could have caused this issue. If you’re looking at the photos all around the edge, there seems to be a seam on all four sides of the Glass giving away along with the hole protruding up.

I was driving down [XXX] The car was moving along fine then when I pushed the gas it would jerk back and shake. I pulled off the highway and was able to coast into a gas station, but the engine stopped working as I was driving while cars were coming behind me. I could have been stopped in the middle of traffic with no way to get out. The power steering light came on as well as a light that said the engine was cut off. I came to a stop on the highway due to a law enforcement traffic slowage. As I began moving again with the rest of traffic, my car suddenly started shaking and struggling to stay on. My check engine light came on and an alert popped up for “Engine stopped, steering power low” “pre collision system malfunction” and “parking brake malfunction”. I felt fearful for my safety as I was unsure if my car would safely allow me to brake and move over to the shoulder of the road. I was really scared and had so much anxiety that I wouldn’t be able to make it out safely. The dealer was not able to find any record of these codes popping up and could not recreate the check engine. I have pictures showing the codes to support my claim.

This happened on [XXX] in Oakland CA, it was a rainy day. I drove off the freeway and then parked the vehicle in the parking lot. The car was in idle mode. After about 10 minutes the engine started to stall, and then shut off completely. I tried to restart the engine but it failed. The dashboard showed an error message "Engine Stopped, Steering Power Low". I suspected that rainwater entered into the engine from the air intake. After waiting for about 2 hours, I was able to restart the vehicle, and then everything seemed to be normal. Toyota should announce a recall for this problem as this is very dangerous to operate the vehicle on the freeway on rainy days. UNKNOWN. I drove my car off the dealership lot and not even 10 minutes into my ride home, as I pulled of the highway, I noticed that the RPM of the vehicle was slowing down. I pressed down harder on the gas to get it to accelerate but it just kept jerking and slowing down. Eventually I tried to get off to the side of the road and the car stopped in the middle of two lanes. The Check Engine, Battery warning and Electric Power Steering lights all came on the dashboard. There was also another error message: Engine Stopped Steering Power Low. I called the dealership and they came and towed the car. Not sure what the issue is but saw on some forums that this has happened to other RAV4 owners on used and brand new cars as well. Someone encouraged users to report it to the NHTSA so Toyota is aware of the issue.

Toyota RAV4 2023 , had it not even a whole year bought it from the dealer 0miles. Today while running errands parked on the side of the road, car started to vibrate weirdly and then it shut off. I try starting the vehicle again and it just keep showing the same message “Engine Stopped Steering Power Low” with the check engine light and steering wheel light on. After 30minutes I let it sit and when I tried turning the car on again, it ran perfectly but I wasn’t risking it happening again. So I had to call Toyota roadside to tow it since the warranty still covers it. And now I gotta wait to see what’s going on. I did my research on google and a lot of people have the same problem going on with RAV4s.

The contact stated that while driving at various speeds, the instrument panel failed to display critical safety information. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 21,000.

The contact stated that while driving at an undisclosed speed, the passenger’s side blind spot warning system located on the exterior side mirror failed to function as intended. The light flashed on the side mirror; however, the beeping sound was inaudible. No warning lights were illuminated. The vehicle was taken to the dealer; however, no cause for the failure was found. The vehicle was not repaired. The contact stated that the failure persisted. The contact stated that the failure had occurred the same month the vehicle was purchased. The manufacturer was notified of the failure. The failure mileage was approximately 20.

Faulty instrument panel keeps going on and off. It will reset itself and then do it again like it’s trying to fix itself. But then we’ll go on and off again it’s been doing it for about six months now we’ve been putting off by telling someone because we thought it might straighten up and when we keep looking up recalls, it says there’s no recall on it.

While starting my rav4 prime, the instrument panel (gas gauge, speedometer, battery life, etc) went black. Took it to 3 different Toyota dealers; 2 sent me away and the 3rd said I can’t drive the car anymore. Service tech said it’s the 3rd one he’s seen with the instrument cluster black in the last year! They’re still running official diagnostics on it.

The vehicle's radio/ GPS navigation system shuts off and on and it is available for inspection upon request. My safety and the safety of others were put at risk because while in the middle of driving on a one-way street the radio/ GPS navigation system shut off in the middle of my trip and I was unaware of if I was going down the correct street because the navigation system completely shut off. I had almost crashed into another vehicle because the radio/GPS navigation system cut off in the middle of my turn so I was unaware of where I was supposed to be driving at the moment. The vehicle has been inspected by Toyota but has not fixed yet. There were no warning lamps but the radio/GPS navigation system did shut off a few times prior to this incident but it never occurred in a situation where I was in a time where I needed the GPS and feared for my life and the lives of others.

My Gas tank does not fill up properly. It seems to trigger the automatic shut off at gas stations when it is only 3/4 full. I then have to keep putting gas it with about 6 more shut offs to get the tank full.

2023 Toyota Rav4 hybrid (XSE) is said to have fuel tank capacity of 14.5 Gal with range in miles (city/hwy) 594.5/551.0 mi. When I first got my car, I can fuel it up to 520 mile driving distance. After a while, I can only fuel up to 430 mile, with no other accident. The tank cannot be fully fueled. This incidence has been widely observed in owners with the same year model. The actual reason is unknown. Some went to the dealership and replaced the sensor, some just got diagnosed with no issue. None of these has actually fixed the issue. The real answer could be the issue of the sensor or tank, or something else. This malfunction with unknown reason is risky for the use of the vehicle.

A strong fuel odor was evident in the cabin, and also under the hood after operation. Vehicle use was low, approx. 6K miles from new. My wife reported this to the dealer service department. They said nothing was wrong with the vehicle. Approx. 2K miles later, I reported the same problem to the dealer. Service rep said vehicle was unsafe to drive due to over torqued and leaking fuel line. They repaired it for no charge. No fuel vapors now, but if this is a production quality issue that others have reported, only to be told nothing was wrong, I think a major fuel-combustion safety issue may be overlooked. I am submitting documentation. Also, I am concerned about health effects of sustained fuel vapor exposure over months of driving. My wife rear-ended a stopped vehicle, possibly due to cognitive impairment from fuel vapor exposure. No warning indicators were ever triggered. Thank you.

My 2023 Rav4 is having same or similar issues previously described on tsb 010920 for models 2019 to 2021. Toyota of Iowa city is saying issue is under investigation for the last 5 months.

I bought a 2023 Rav4 Hybrid XLE brand new. On my first refill of the gas tank, I noticed that the gas needle would only go a smidge over the 3/4 mark. The RAV4 hybrid is supposed to have 14.5 gallons and around 540 miles per tank but I estimate it to fill around 10.5 gallons and the car shows around 413 miles per tank. I've attached two images I took when I filled up the tank, the car showed around 36 miles left to empty and it only pumped 8.8 gals.

The passenger rear tire experienced a catastrophic tread separation failure on the highway at speed limit speed. Since this was on the freeway, loss of control could have been experienced and caused a severe incident injury to drivers in the vicinity of the incident. The problem just happened and will be inspected by an authorized dealer. There were no warnings until the tire of the incident. These tires have been rotated according to schedule and have less than 21,000 miles.

(Original Tires) No warning light was illuminated. The vehicle was taken to a dealer. The mechanic suggested that the contact had hit a pothole. The front driver-side tire was replaced. Firestone Destination LE 2. 225/65R17. Tire has exploded on highway. This is a brand new car purchased 3 weeks ago and has 1584 mile on it. Tire run only about 1575 mile. When inflated the tire again, it had multiple bubbles, ball shaped swellings.

At approximately 11:30 AM on Saturday, August 5, 2023 I was traveling northbound on I-17 just before exit number 278 for State Route 179. I felt the vehicle shake violently and my wife saw a portion of a tire rolling along the side of the road into the desert. I was able to safely pull off to the shoulder, it was then that I noticed the driver side rear tire had completely separated from the rim. There were no warnings I had recently had my 5000 mile inspection/oil change performed at Right Toyota. At the time of the incident there was approximately 5355 miles on the vehicle which was purchased new in January 2023. I was luckily able to bring the vehicle to a complete stop without incident and notified Roadside Assistance.

Was driving in heavy rain after about a 35 mile commute and put the car in park. After about 5 minutes of sitting in park the car turned completely off by itself for a couple of seconds and restarted. Warning message “engine stopped steering power low” popped up on dash and was visible smoke coming from under the hood with a slight burning smell. This is a first incident for the car as it has been driven for about 2 years with a little over 35,100 miles on it.

UNKNOWN: It happened during a heavy storm on my 40 miles to work and all the symbols lights suddenly lit up on the dashboard. The car died and the steering wheel got heavy when I tried to pull to the emergency lane. Luckily I was able to pull over, but it almost caused an accident as I was in the middle lane. I tried to start up the car again, but it would crank up, but died instantly and the dash would show "Ignition on" on the top left and "Engine Stopped: Power Steering Low". I called the dealership and talked to the service department and the service advisor think it was the battery, but he told me to try starting it after 5-10 minutes, if not, to call the tow company and tow the car to dealership for further inspection. I let it sit for 10 minutes and was able to start it back up and drove it straight to the nearest Toyota Dealership. While driving to the dealership, the car kept choking/stalling when pressing on the gas after braking. They examined with their scanners and the battery was in good condition and my car has gas (about 3/4 tank full) and they said the scans reported no issues. This is a concerning issue as I could had lost my life or possibly cause major accident and involve other people life.

I was sitting in line at a drive-thru with my wiper blades on the intermittent setting when suddenly the engine shut off and I got an error message that said “engine shut off steering power low”. The check engine light, battery, and steering light all came on. I turned the car off and then on again. It rumbled and then shut off again with the same message. I had to shift to neutral and push into a parking spot.

I was told that the power steering was electric, they could not duplicate the message, and to bring it back if it does it again. I’m concerned because what happens if it occurs again while I’m driving on the interstate?

I was sitting in a drive trough getting breakfast, I go to pull off and my car cuts off. Couldn’t get it to restart. Had to push it out of the way. Power steering low. Granted this cat is just 2 years old. It’s done this before last year after having it one full year same thing. Luckily after letting it sit for 20-30 minutes it started and I got it to Toyota. Low power steering shouldn’t cut an engine off. If anything just be hard to drive. These new cars need to be tweaked. If Toyota can’t fix this they will not get any more business from me.

Was driving to work, pulled in put the car in gear. The car turns off staying low power steering fluid. Couldn’t get it to start. I go in work my shift have my brother meet me. The car starts after sitting 9 hrs and I took it to Toyota. I have only had this car a month and it does this. Low power steering shouldn’t cut an engine off. If anything it should just be hard to steer. These new cars need to be tweaked. If Toyota can’t fix this they will not get any more business from me.

Car shut off while driving because warning light for the power steering came on p.451stating check power steering fluid level. Vehicle has either a sealed power steering system or its electric. It was raining at the time that this happened. This is a big risk factor that could have led to a wreck but thank god that it did not was able to get out of traffic and stop. It took an hour of waiting before the vehicle would start back up and run properly. Unable to reproduce the problem at this time.

A dealer or independent mechanic was not contacted. The vehicle was not taken to a dealer or independent mechanic to be diagnosed. The vehicle was not repaired. The contact stated that the failure had occurred twice. The manufacturer was notified of the failure and informed the contact that the vehicle responded properly because the gear shifter was not in park(P). The approximate failure mileage was 24,500.

I am an owner of new 2023 RAV4 and believe some of the newer 2023 & 2024 Toyota Rav4 models with ICE (non-hybrid) have a potentially dangerous defect with the transmission system, which causes these cars to jerk and stutter when the gas pedal is lightly depressed (light acceleration) when the car speed is between 15 and 40 mph and tachometer is between 1500 and 2500 RPM. The reason I believe this defect is potentially dangerous is that a few weeks ago I was going through a school zone with an active 20mph limit, and when trying to keep the speed at 20mph, experienced jerkiness, RPM jump and sudden speed bouncing (momentary jerking from lower to faster and back to lower speeds a few times in a row). Typically this issue is a lot more subtle but occasionally it is so bad that I have a momentary feeling of loosing control over the speed of the car and active school zone is the least place I'd like that to happen. Besides, these stutters are pretty bad disturbers when they occur drawing focus away from the road and onto the car itself, since it does feel like the car is about to stall. I have visited the dealership twice regarding this concern, however since these stuttering have a random nature I was not able to replicate the issue during my visits to the dealership. I have also passed this concern to Toyota USA Corporate office and they in fact have acknowledged that they received numerous complaints on the same matter and suspect the issue is in the newer 8 speed transmission system they are now installing. Unfortunately there has been no further action taken by Toyota to remedy this issue, although they are fully aware of it. Hoping you get more complains on this matter and force Toyota to take action. The vehicle several occasions been unresponsive to accelerate. This has happened on two separate occasions (days) in which the vehicle would not accelerate. The first occurrence was right after I purchased the car and drove it the first full day I owned it in September 2023. The second time was on Saturday December 16, 2023. When the vehicle is driven under 40 mph such as slowing for a turn, braking for a traffic light, picking up speed to enter onto a highway this problem occurs. It is when the gas is pressed down to accelerate, the vehicle does not respond by increasing the speed. The RPM's would go really high and the vehicle would receive the gas but would not "shift" and felt stuck. And the transmission was very loud. It felt like the vehicle transmission was slipping or not engaging/shifting. Adding more gas was not beneficial. The gas pedal could be pressed down to the floor and the vehicle was not responsive by shifting "gears" or picking up speed. The vehicle would not accelerate above 20 mph and the RPMs would go up. No lights or warnings were produced. The only way to get the vehicle to act normally once again was to pull the car over into a safe location, shift the car into park, shut the car off, wait for a few seconds, restart the car and proceed. In one session (driving the same day) the car repeated the inability to accelerate each time the vehicle was slowed down below 40 mph to come to a stop at a stop sign, take a turn, stop at a traffic light etc. This was dangerous as I was not able to proceed in traffic and caused other drivers to honk their horns at me, and left me stuck in intersections. This was very disconcerting and left me feeling endangered by not being able to follow the flow of traffic. Toyota Service did a diagnostic test on it twice now. The first time had torque converter codes. This 2nd time they claim there were no codes on the diagnostic test. They dropped the transmission pan and found no metal shavings.

For over month on multiple random events, the car shuts down while traveling at hwy speeds. The car light prompt to shift the vehicle into neutral and push the start button to re start the engine. We have had the car at the dealership 4 separate occasions and they can not find the issue because it does not cause an engine light to come on when this happens. They also supposedly can not get it to duplicate so they are refusing to go any further. We have video of this occurring at 50mph. This causes lost of steering and braking. We reported this to Toyota and opened a case. They to will not pursue fixing the issue as they are not getting a trouble code. I fear this is going to cause a major accident and this poses a severe safety risk as you loose control of the vehicle

For the last six months our vehicle has had it's infotainment system malfunction. It shuts off unexpectedly while driving or stopped. It shuts off and on repeatedly as if car is turning on and off. The bright warning sign will show and proceed to act normal then again 5-10 seconds later it shuts off again. This issue happens with and without a phone connected to it via bluetooth. While screen off the rear view camera does not work and nothing else is operable within the screen/ infotainment system. We've brough car to dealership three times in six months.

SERVICE BRAKES

1.) The component/system that failed/malfunctioned is the brakes and/or anti-lock braking system. 2.) My safety and that of the people in front of the vehicle is placed at risk because the vehicle's braking system fails, allowing the vehicle to surge forward for a split second, temporarily out of control. 3.) While the problem has occurred many times with various drivers, the Toyota service technician said they were unable to reproduce the issue. However, with the information (date/time) I have provided, they confirmed the computer indicates the ABS system was triggered each time. 4.) The vehicle has been inspected by the Toyota service center representatives. 5.) There were no warnings, messages or other symptoms prior to the brakes failing. The problem is the braking ability is failing for a split second, when the car hits a bump. The size of the bump can be as small as the crack in the asphalt where two roads meet. The problem occurs when I am driving at slow speeds and riding the brake because I am in a situation like being in slow-moving bumper-to-bumper traffic, approaching an intersection requiring a stop, or coasting down hill. On numerous occasions, the brakes release, allowing the car to surge forward for a split second, before re-engaging. Toyota has told me it is the ABS working as intended. To be clear, the braking failure is happening at slow speeds and on dry pavement. This is not how ABS normally works. I have driven numerous makes and models of cars for 40 years and I have never experienced this type of situation triggering an ABS system. I report this to Toyota each time I bring my car in for regular maintenance. During the last visit, my regular service technician told me that they have now seen at least 5 or 6 other cars doing what I have described above. I don't have a specific date of the incidence because it happens frequently and has done so since I bought the car (10/01/23), so below, I am just listing the last time I reported it to Toyota.

Under braking conditions and hitting a bump on a paved road, the vehicle will have a 1 to 3 second momentary loss of braking. At the same time, a grinding noise, similar to (if not the same as) the ABS system is heard, and the VSC light flashes. The occurrence of this problem is unpredictable, the severity is inconsistent. It happens across various ranges of brake pressure/deceleration rates and speeds. No accident has occurred to date with this vehicle, but we have had close calls. In some cases, our vehicle has inadvertently entered controlled intersections due to loss of braking. Dealer has inspected the problem but has been unable to replicate or resolve to date. Based on internet research, this appears to be a common problem. Very concerned this could occur at the wrong time and lead to an accident, causing injury or death due to behaviors of the vehicle's braking (and loss of) being outside the driver's control.

When I drive this vehicle i have to be very careful. The moment you take break pedal the vehicle doesn't stop, It started shuffling and showing a warning sign of slippery road. It takes extra couple space to fully complete stop. It is summer time there is not slippery road road around. I hope they will fix this issue before it gets bad or kill someone. This is a dangerous act when you can not control your break pedal. All break pads are new this is 2023 vehicle built in June.

The paint of the exterior body is peeling off after becoming bubble on the surface. It appears all over the body including hood, roof, doors, etc. For reference, please see attached photos. This can eventually start rusting. This has come to know only after using a car for a year and four months, which proves this needs to be addressed because it may be going to start rusting other parts of the body or structural components. Car is available for inspection, on request.

The contact owns a 2023 Toyota Rav4. The contact stated while cleaning the vehicle, the contact became aware that there was water coming from the rear passenger’s side door. The contact opened the rear passenger’s side door, and water poured onto the ground. Additionally, the rear passenger’s side door made a sloshing sound while being closed. The vehicle was taken to the dealer, where Technical Service Bulletin: 0084-21 (Sloshing or Splashing Noise from Inside One or More Door Panels) was performed. The manufacturer was not made aware of the failure. The failure mileage was 19,100.

At less than 8000 miles on this brand new vehicle, the paint started chipping away all the way down to the metal under the paint on the front of the car… The dealership that I ordered from the manufacturer through stated it will probably start to rust and that I need to have someone repaint it… They tried to refer me to someone that they use for paintwork… And stated it will be at my expense; although I was forced to purchase Toyoguard Platinum and the clear paint cover protection packages/warranties. I have owned multiple of this make and model vehicle in the past and have never had an issue with the paint peeling off and chipping as it is on this brand new vehicle.

Toyota OEM windshield glass has SEVERE distortions/waviness in glass. I have had my windshield replaced 3 times now with Toyota OEM glass and the issue is still not resolved all 3 have had this distortion/wavyness in the glass. I don’t want aftermarket glass in a brand new car especially with all the safety features that relies on glass clarity, but Toyota is making it hard not to choose that option. Toyota needs to do better on quality control of their glass. The distortion is happening in the line of sight for the driver causing dizziness and issues while driving, as well as BAD wavyness around the edges. Ive done some research into this as well and I am not the only Toyota driver having this issue with their glass. I’ve never owned a vehicle that did this and it’s very disappointing and frustrating that I am not able to have a clear windshield in my brand new car. I called Toyota of America but they basically pushed me off onto the dealership. Someone needs to contact the MFG and make them aware of this problem so I can get glass that is normal.

The moonroof on this vehicle shattered without warning as I was driving on the freeway. It sounded like a loud boom and when i looked up the tempered glass was falling on me and inside my 2023 Rav4. This is a huge safety issue and im not only concerned about my safety but how much glass is in my car - I take my kids to school in this car. Im going to take it to the dealership today to have them repair it - this should br a warrantied item but I have read there are multiple issues in other Rav4's.

Have had the vehicle three weeks and only drive on paved roadways. Visible chip cracked instantly during drive. Windshield seems weaker than our 2 4Runners and VW. I’ve never seen a crack instantly occur without a large rock or hail.

I purchased my 2023 Toyota RAV4 new back on March 13,2023. 10 days later on March 23 it was raining as I pulled into BJ’s parking lot, I parked and began fumbling around getting my bag and keys together when my car started shaking and sputtering eventually shutting down with an error message engine stopped steering power low. After a call to the salesman I was able to get my car restarted after 15minutes and many failed attempts. Car was bought back to service the following week and I was told car was not properly programmed at delivery, it was now programmed correctly and this should not happen again. April 17,2023 my son pulling into Tractor supply , raining out, pulls into spot and car again shakes, sputters and shuts off, same message engine stopped steering power low. After many attempts car restarts approximately 20 minutes later. Car serviced codes ARE FOUND and cleared, road tested and told car was ok. May 31,2025 I was driving down Rt 80, pouring rain when traffic comes to a complete stop due to accident ahead. Car starts shaking, sputtering and shuts down. Same message code as always. I turned the car completely off as not to kill battery and started to goggle the RAV4. I attempted to restart the car numerous times. Called for a tow, prayed that I didn’t get killed once traffic started to move. Finally 1-1/2 hours later it started. Traffic began moving 15 minutes after I got it started. Took in for service, charged $199.61 only to be told there were no codes. Problem could not be duplicated and in the future to always disable the start/stop button while using the vehicle. October30, 2025 raining, I stopped at Panera drive thru. Placed an order waiting in line and car shuts down, same error code, couldn’t restart car for 20 minutes. Took to Toyota November 5th same response no codes and cannot replicate issue, car is ok. Please help rectify this before someone gets killed!

While raining, noticed smoke coming from under the hood, engine seemed as if it didn’t have any power. Car shut off. While attempting to restart I seen a message in dashboard that said Engine stopped: steering power low. Took a while to get car cranked again after multiple attempts. Message in dashboard disappeared after multiple attempts to restart nor is there a message in remote application health status section.

Vehicle: 2023 Toyota RAV4 Hybrid Issue: Persistent brake noise, safety hazard, manufacturer negligence I leased a 2023 Toyota RAV4 Hybrid from Clermont Toyota (Florida). Well before reaching 5,000 miles, the brakes began squealing and grinding, posing a potential safety hazard. Multiple service visits failed to correct the issue. Toyota Motor Sales, U.S.A., Inc. and its distributor, Southeast Toyota Distributors LLC, have long been aware of this defect, as documented in Technical Service Bulletin TSB-0001-22 and numerous NHTSA consumer reports describing similar brake failures and noises in 2019–2023 RAV4 models. Despite repeated notice, Toyota refused to repair or replace the defective vehicle and denied my Lemon Law claim through an arbitration panel connected to manufacturer counsel. This unresolved brake defect creates danger to the driver, passengers, and others on the road. Toyota’s continued sale and lease of vehicles with known braking issues constitute a serious public safety concern. My 2023 Toyota RAV4 Hybrid has had squeaky brakes for many months (from 4,000 miles onward and consistently continues to be an annoyance, nuisance, and safety hazard while intensifying in frequency), but despite taking the vehicle to the Toyota dealer more than 4 times, they have been unable to repair the issue. At least twice the air bag sensors have come on for the backseat when there was nobody seated in the backseat, distracting me from safely driving. Toyota has done nothing to right the wrong mechanically, refund me for the vehicle (a lease) or offer a replacement. I have a 14 page document filed with the Florida Attorney General Lemon Law Division thoroughly indicating why these ongoing mechanical failures, safety hazards, nuisances and distractions to me the driver substantially impair the use, value, and safety of the vehicle—fulfilling key criteria under Florida Statutes § 681.102 and § 681.104, which I am happy to provide all consumers and attorneys willing to launch a CLASS ACTION against Toyota. Florida law defines "impairment of value" not by market collapse, but by any reasonable reduction in consumer confidence. Consumer disclosure laws require honesty about defects. Florida Statute § 681.102(15) defines a “nonconformity” as a defect that “substantially impairs the use, value, or safety of the motor vehicle.” My vehicle has been subject to repair more than 4 times for the same defect within 24 months and thus qualifies for relief. The manufacturer’s failure to permanently correct the squeaky brakes—even after attempted repairs—demonstrates a pattern of unresolved nonconformity. The squeaky brake issue is not a cosmetic inconvenience—it affects the vehicle's functionality, resale integrity, and safety assurance. Toyota acknowledges the problem exists (via TSB) but has failed to provide a lasting solution. If unresolved, this defect would materially concern any reasonable consumer and diminish confidence in the vehicle’s reliability and safety.

Driving NB when a Toyota Corolla drove into my lane of traffic hitting me head on. No air bag deployed in my RAV but all air bags deployed in the Corolla. No air bag light was on prior to the accident. My vehicle data recorder was copied by a Bosch technician and forwarded to Toyota Corp. The body shop manager stated that my vehicle was the first he had encountered with the extent of front damage where the air bag(s) did not deploy

I purchased a new 2023 Toyota RAV4 in March 2023, which is covered by Toyota’s 36-month/36,000-mile New Vehicle Limited Warranty. In August 2025, while the vehicle was still within warranty, a malfunction warning appeared for the pre-collision system and Road Sign Assist. The Toyota dealership diagnosed an internal failure of the forward recognition camera (DTC C1A95:00 — image processing module malfunction) and recommended replacement and calibration at a cost of approximately $2,300. Toyota denied warranty coverage despite the failure being internal to the part and occurring within the warranty period. I am seeking reimbursement for repair costs that should be covered under the manufacturer’s warranty.

Pre collision malfunction, RSA malfunction, LTA malfunction, Headlight malfunction. These warnings keep coming up showing the vehicle is not safe. Parts were ordered in February but are on back order. Might get by June. Vehicle has also rebooted the screens, tried to lane change, and failed to start once. To me this is a programming problem. This has been an on going problem for months now. The dealership claims that it is a wonky camera causing the problem.

I was sitting in a drive thru with my car in drive , foot on break and all of a sudden it starts to shake and struggle to stay on . Then it stalls out , engine shuts completely off and my check engine light came on! The error messages on the screen read : Engine stopped , power steering low , precollision system malfunction , and parking brake malfunction. As I tried to restart it , It wouldn’t start back up and stay on at all. After a good 20 min or so , and after being pushed out of the drive thru and my order canceled, my car starts back up as if nothing happened. Had my car checked for codes , and guess what ? NOTHING ! It’s happened to me about 6 times in about a year and two month of owning it ! Nobody can figure out what the problem is . I also have issues randomly with my radio screen . When my phone is hooked up to CarPlay , it will sometimes kick me out and reconnect by itself , the screen completely shutting off and turning back on by itself , there’s times I was listening to pandora and it won’t allow me to click my station and show the song playing , and other issues. Had this happen too many times to count and within the first couple months of owning it , I took it back and they couldn’t figure out the issue. It never happened while they had it .

Mckinnon Toyota in Clanton Alabama advised that this vehicle is not equipped to detect a stationary object behind the vehicle. Three individuals at Toyota customer service advised that it did. R/O Number for the visit is [XXX]. The reference number from Toyota customer service is [XXX]. Car was purchased on 6/2/23. Car was purchased with the expectation that it was equipped to detect rear stationary objects when in reverse. I consider this a safety issue if it will not detect a person behind the car in reverse. My wife was going at about 10 - 15 mph coming up about 2 car lengths to a small curve in a residential area when all of a sudden she heard a pop or snap and the Rav4 went to a super high speed by itself. The acceleration could not be stopped even though she had her feet on the brakes. She had both hands on the steering wheel to avoid hitting a house and ended up going through a wall in the yard. There were no warning sounds and no signals indicating a problem. it appeared that the display was blank. There was no pre-collision warning or braking except as my wife was pushing on the brakes. She also hit the right side into side wall. No air bags deployed even though Windshield and all door glass on sides shattered. My wife the driver was okay and not hurt, and the grandchild in the car seat behind my wife was fine also. Neither had injuries. There were checked by emt. The police looked but all would not try to get in except to remove my granddaughter. My wife was removed also because she could not get out the door was jammed against shed and rock, and handles broken off This happened after the recall work was done by toyota dealer on 12/04/2025. There were no problems prior to the recall. The vehicle was a 2023 Toyota RAV4 hybrid. When the driver attempted to park the vehicle in a parking lot, it suddenly began moving forward without use of the accelerator. The driver applied the brakes but the pedal went to the floor without braking. The vehicle seemed to be moving faster than expected without use of the gas pedal and went through the parking lot. The driver stopped the vehicle by running up over a curb, through a traffic island with soft dirt and mulch and finally crashing into a metal storage unit. Two air bags deployed. Toyota was notified of the crash and a Toyota representative made the 911 call. A New York state trooper and two fire trucks arrived. The vehicle was towed and eventually collected by our insurance company. The tow truck driver said the brake pedal went to the floor when he tried it. The car was declared a total loss. 1. The notable failure was in the braking system. The car also seemed to be traveling faster than it should and didn't slow down. The insurance company has the vehicle now. 2. The safety issue was loss of speed and braking control of the vehicle. Steering was normal. Fortunately the parking lot had few cars or people in it. 3. No investigation of the problem aside from assessment of damage that I know of. 4. Liberty Mutual assessed the damage. The NY state trooper made a collision information form and gave us a copy. 5. No warning lights or related symptoms were noted prior to the car traveling on its own. After the crash, a local Toyota dealer called and said that they were notified that the check engine light was on and there was a problem with the hybrid system, along with other warnings.

This problem has occurred twice now. First time: 4/19/2023 The car was brought to the car wash, I pulled into my parking spot the car shut off. When I attempted to turn the car back on the car would not start. The car showed a check engine light along with a steering wheel with an exclamation point. The dash said "Engine Stopped Steering Power Low". The car was immediately brought to the dealership after the car would start. The dealership did not find anything wrong along with no codes. The dealership assured me this would not happen while driving. Second Time: 5/9/2025 The car stalled mid driving in the rain and said "Engine Stopped Steering Power Low", car would not turnback on. Car has not been brought to the dealership again due to them not responding. This problem happening while driving puts not only my life at risk but everyone on the road.

After routine service at South Coast Toyota in Costa Mesa, CA, I unknowingly drove my 2023 Toyota RAV4 Hybrid for over a week with two missing lug nuts and one extremely loose lug nut on the front passenger wheel. The service performed on April 5, 2025, included an oil change, tire rotation, and full inspection. No issues were reported to me at pickup. Over the next several days, I heard increasing thumping and metallic noises from the front right wheel area, especially at highway speeds. On [XXX], while driving with children in the car, the vehicle made loud metal screeching noises and the steering became impaired—I could not complete a basic turn. I limped the car home at 4 mph. Upon inspection, I found two lug nuts missing and one so loose I could unscrew it by hand. I had the car towed back to South Coast Toyota on April 18. The dealership admitted two lug nuts were missing but failed to apologize or fully document the issue. I was told via text that the only action needed was replacing the missing lug nuts. I received no formal inspection report or confirmation that the vehicle was safe. I paid out of pocket for a third-party inspection at a separate Toyota dealership, which confirmed that the previously recommended sway bar replacement was unnecessary and that my tires and components were in better condition than initially claimed. This was a clear service-related safety failure that could have led to wheel detachment and a serious accident. I drove over 200 miles on freeways with children in the vehicle. The dealership failed to follow basic safety protocols and has not taken responsibility. I am reporting this to NHTSA in the interest of safety and accountability. On 2 occasions while driving on a flat straight Interstate Highway using the adaptive cruise control, with no vehicle in front of me, the vehicle has suddenly tried to stop. Pressing the Cancel button for the adaptive cruise control would not shut the system Off. When the accelerator was depressed to attempt to get the car moving forward, it continued to try to stop the vehicle even harder. Pressing the brake pedal also did not disengage the adaptive cruise control. I had to allow the vehicle to creep off the highway, then Shutdown the vehicle and then Restart to resume driving.

Sudden surge and acceleration upon parking. I had brakes depressed pulling into a parking space when the vehicle felt like it jumped and quickly accelerated. It veered left missing the parked van in front of me but crashing into the parked Honda Accord two spots over and one in front. It crashed into the back tire and fender area of the passenger side of the Honda pushing it completely out of its spot and sideways. My vehicle kept going even though pushing the brakes to stop. I had to force it into park to stop. The car’s air bags deployed but my Toyota’s airbags nor crash warning system worked.

This vehicle is going to kill someone. Not only does it have this unintentional steering issue, but the electrical system keeps going out. The dash has been replaced and there are still issues. The adaptive cruise control used to hard brake now it pulsates when lane changing which isn’t cool if you’re passing people because then you’re turn in front of them and braking right away. The radio requires the vehicle to be turned completely off, wait, and then turn back on. The unintended steering will take you into incoming traffic, in the opposite lane, or even off-roading even when there is nothing in front of you. The compass randomly disappears. The Tim roof literally exploded while we were doing 80mph down the interstate. The curtain was closed or we would have had glass all over. The window blew out and up. If something hit the window it would have blown in and down. I have reported these safety issues to Toyota and they keep telling me that nothing is wrong. The steering issue has happened at least 7 times since I owned it. I have provided Toyota with pictures and videos showing these things and they’re still telling me that nothing is wrong but they even have a form that talks about the “unintended steering” issue.

I was pulling into a parking spot and my vehicle suddenly accelerated. I slammed on the brakes and the car did stop, but not before hitting a railing in front of me. There were also trees close to the railing, so the camera should have detected those as a barrier and I believe the veauto-braked, even if I had not hit the brakes. This is a safety concern because the vehicle would have plowed over any person in front of the vehicle.

The contact owns a 2023 Toyota Rav4. The contact stated while driving 35 MPH, there was a loud sound like a gunshot coming from the vehicle. There was no warning light illuminated. The contact's wife slightly opened the sunroof cover and discovered that the sunroof had shattered into pieces. The vehicle was not diagnosed or repaired. Neither the dealer nor the manufacturer was notified of the failure. The failure mileage was approximately 16,000.

While at stop light notices my 2023 Toyota Rav 4 LE engine was steaming. Pulled into a parking lot when Power steering fluid low warning light came on and said that the engine will stop. After I put in some power steering fluid the dash warning lights lit up, with the following warning lights: Pre-collision brake system failure, Collision brake system failure, Secondary collision system failure and check engine light came on. Had vehicle towed to dealership, they did an test and confirmed several error codes and they are waiting on Toyota engineers to provide information on cause and remedy. While at dealership was told by service adviser that there was sat least one other individual with the same issue. The dealership unfortunately don’t know the cause of the problem

When I do these checks I have my hands lightly on the wheel ready to correct the car when needed. The lane control assist tends to stay to the left of the lane, crossing over the left side lane markers, it also tends to head towards the right side of the lane, crossing over the right side lane markers without compensating. At times it does compensate but it causes the car to go back and forth in the lane without settling out. There have been times when the car is steered to the right with no compensation and without my correction would have gone off the road. The lane warning system doesn't always work. If I did not have diligence when using the system I would no doubt have gone off the road and crashed. The dealer has not checked the car, they say this is how it works even on their personal Toyotas. These problems have been prominent and have existed since the purchase of the car in 2023.

At estimated 25500 miles in the vehicle, I started to feel vibration at 65- 75 mph. Bouncy ride and slight bad bearing noise at 45 mph . After taking to the dealer, they also felt vibration and recommended tire balancing. After having that done, the problem still existed. I took the car next day and at first they where not willing to check my car but then when I forced, they test drove and confirmed light road noise and light vibration. After a little checking, they said the tires are feathering and should get new tires and or alignment. The tires only have 25500 miles and they were serviced by totota .I got new tires from from NTB and rebalanced and the issue still existed. I then also got alignment from another shop even then the vibration and bad bearing noise still exist. After searching, I have read many others had this issue with their rav4 and dealers just say it's normal . I as years of experience know it's not normal. The origin equipment tires that came with new vehicle were Dunlop grandtrek pt20. The new tires I got are Cooper Endeavor plus. Issue is definitely not the tires. Humbly request NHTSA to look into this as this is ongoing issue with many new Toyota Rav4.

The brakes failed, without warning, while parking; causing me to hit a pole/wall in my employer’s parking garage on April 12, 2024. I was injured, and obtained care from UChicago Medical Center. Toyota assigned an independent investigator to assess my vehicle; which was completed on April 29, 2024. Per Toyota, no brake issues were noted. My vehicle remained at Oak Lawn Toyota from April 12, 2024- May 31, 2024.

The incredibly frightening incident occurred 2 times within 3 days. My 2023 RAV unexpectedly and very forcibly accelerated on its own when I was pulling into a parking space. Luckily I am in the habit of parking my RAV far away from other vehicles in parking lots when the option exists. The first ‘acceleration and/or braking’ malfunction occurred 5/31/24 in a Walmart parking lot. When pulling into a parking spot my right foot was fully off the accelerator (I use my right foot only for the accelerator and brake pedals) and was gradually depressing the parking brake pedal to come to a complete stop. Instead my RAV suddenly and forcibly accelerated forward through the empty parking spot in front of me and beyond!! I would have absolutely smashed into the car and or person if one were there!! As the RAV shot forward I began desperately pushing full force on the brake pedal; the RAV did not stop! so, I began full force repeatedly pushing and pumping the brake pedal. My RAV eventually came to a halt as the tires may have locked up which it skidded about 12-18 inches. By God’s grace no one was hurt! On 6/2/24 ‘it’ happened again!! Pulling into a parking spot at an outdoor cafe, as I was gradually depressing the brake pedal to come to a complete stop my RAV instead quickly and forcibly accelerated forward!! I thank God that I did not pull into the other spot (my sister took that one) which was just a few feet away from around 12 people and families enjoying their outside lunch dining!!! This 2nd horrifying incident my RAV took longer to come to a complete stop. I fully and forcefully pushed on the brake pedal but my RAV kept going!! I continued repeatedly releasing and fully depressing the brake pedal but it kept going! The next thing my RAV was approaching was someone’s backyard and house!! I forcibly pushed the brake pedal one more time and also threw the RAV shifter into “park” finally the RAV stopped! I am so shaken up by these traumatic malfunction incidents.

Happened at ~644pm on 04/08/24- I heard the noise like a thump or thunk, The car initially pulled towards the roadside, then I corrected the wheel and slowly stopped the car. Upon exiting the car, it became apparent that the ball joint broke just above the plate that attaches to the axle.

Unintended sudden aggressive acceleration when I made a right as I was slowly pulling into a spot in the parking lot. Breast’s did not work fast enough and car ended up going up the hill and front colliding with it.

A car pulled out right infront of me at an intersection, I hit their back drivers side rear with most of the front of my car. I was doing approximately 41-45 mph. No airbags were deployed and my SOS button didn't work nor did it call for help.

I was driving on a highway posted speed limit 75mph on a clear day with no precipitation and without cause, the panoramic sunroof above my head imploded with glass shattering down upon my head. I did not hit anything and nothing hit me. No rocks, no debris, no outside force caused this. The panoramic sunroof showed a crack around the seal where it meets the body of the car and it imploded and cracked (shattered) inward from this cracking.

While driving down the highway at the speed limit in our town on a clear evening with no weather, the sunroof of our 2023 Toyota RAV 4 exploded. There was a loud bang and it shattered as if the glass had been under significant stress or pressure. The glass bowed up in the middle and a section the size of a small saucer dropped out. There was a rigged break along the front edge of the sun roof. There were no bumps or holes in the road - it was smooth. There were no other vehicles nearby - no one threw a rock at our car. The noise was so loud that it was disruptive and shocking and thankfully I was not in heavy traffic or an accident might have occurred. I researched the web and found numerous instances of this sunroof explosion occurring. Please get Toyota to fix this before someone is serious injured.

Check engine sing comes and all dash board alarms lights on and bleep and while driving it shuts off while vehicle is on motion. This vehicle operates as yellow taxi and this issues is serious concern about passengers safety on board . The vehicle took to bay ridge Toyota dealer and the technician was unable troubleshoot and solve the concern Dealer charged 200 dollars for diagnostic and car is still under warranty.

I was turning left pulling into my carport with my foot lightly on brake and the car suddenly accelerated forward !!! I slammed the break hard and was able to stop the car from hitting the concrete wall because my front tire hit the wall to the left and the car hit the cement parking bumper in my carport!! It scared me so bad I am afraid to drive the car in fear I could hurt someone. The dealer could not replicate the problem and sent me home with the car. I am not happy with Toyota !! The car is 8 months old with 7,000 miles on it!!

I was pulling into a handicaped parking space and the car just jumped over the curb and hit the handicaped sign plowed into the landscapping. it went from 5miles an hour to 20miles an hour in 5 seconds by itself . computer took over . no air bags were deployed .

Car loses traction over minor imperfections in the road. When braking and dropping speeds to 20-30 MPH, car will lose traction over imperfection in the road, causing the ABS to engage. The car shutters and grinds/vibrates until traction is regained. With an AWD system, a minor 6-inch drop in the road due to a pot hole or dip should not cause traction to be lost. The car consistently loses traction when braking and 1 tire runs over imperfection in the road (pot hole, dip, uneven pavement, etc.). If car drives over speed bump (which keeps tires aligned), car does not lose traction. Issue is only present if 1 tire different than the other 3 which shows instability of the vehicle. Issue has been verified by manufacturer and has shown up in the car log. Dealer said that the car recorded a loss of traction 4 times in the last month. Issue has been escalated to Toyota corporate and they do not see the sensitivity of the traction control as an issue. Issue has been persistent since the purchase of the car in early 2023. All components on the car are OEM and have only been looked at by Toyota dealers. Multiple online forums have shown that other owners face the same issues. Video proof of the issue can be provided.

Screen used for backup camera and radio was intermittently non-responsive. Issue has been seen/confirmed at the dealership. Safety concern is the failure/inaccessibility to the safety/visibility feature associated with the reverse camera. (Issue was intermittent upon ignition and remain non functional until vehicle would be powered off and back on again.)

As I was entering a covered parking structure, I came to a complete stop at the entrance of the gate to obtain my parking ticket. I was driving very slow less than 5 mph, and engaged the brakes as to stop to get my parking ticket before entering the parking garage. I have my foot on the brake pedal, I am at a complete stop of 0mph for several seconds now, and suddenly the engine revved and my car lunged forward a bit. I had not lifted my foot from the brake pedal, and yet the engine revved and my car lunged forwards. My passenger at the time yelled, WHOAA why are you pressing on the gas!!? So my passenger in the car at the time was also startled by this incident and felt the car surge forward. He thought that I had pressed on the gas pedal. If someone was to be in front of my vehicle they would have potentially been hit with my car, or if I had been closer to the parking entrance gate my car could have hit the gate. I am highly concerned this just occurred as this could cause a great accident or injury. I have 1,100 miles on the car. It was purchased brand new. Another issue I have been experiencing is when I engage the gas pedal, it does not accelerate and takes a few seconds to accelerate causing my forward momentum to stall out, this is an issue when I have been trying to merge into traffic and my car does not drive as I have anticipated. I am not sure if this is an issue with it being in "ECO" mode or not, but there is a stalled acceleration issue, that happens sometimes, but not always and I cannot pin point why it is happening at certain moments.

I paid for option on my vehicle but it was not installed in the factory. The software shows to be installed on my car. I alerted the sales person numerous times. I told the car had to be moving faster than 40 miles per hour to activate the pre- collision system and it still doesn’t work. This safety issue of the vehicle.

Was driving and made a stop at the stop sign. Vehicle stalled, got a message that the engine had shut off. Would not restart in the middle of an intersection. Engine would not turn over after stall.. Dealer was unable to replicate issue but said that the computer had thrown codes.
