When braking to a stop at low speed (stopping at stop sign or at light), then car accelerates forward even though my foot is on the brake attempting to slow the car down. This unintended acceleration is dangerous and can cause an accident.

Brake Transition Issues: I have noted frequent jolts of power when braking, which creates at least the sensation of, if not actual acceleration while breaking. Web research suggests this may be linked to the vehicle switching between regenerative and mechanical braking systems, and that the problem is acute with 2021 and 2022 Toyota Siennas, which, if accurate, means Toyota has applied a fix to subsequent model years, but has not systematically addressed the first two years of the current Sienna generation.

Occasionally the brakes seem to stop working briefly, and the van even accelerates while the brakes are engaged. It happened again today but has happened since we got the van new in 2021. When braking, the brakes will unexpectedly slip. There is generally a loss of brake and a split-second surge that is rather like acceleration (since the car is no longer being slowed down by the brakes). This occurs unexpectedly, unpredictably and without any warning. Having the car suddenly move forwards without brakes is dangerous and could cause the car to hit a pedestrian or another car. This has also happened multiple times while angle parking which almost caused a collision. Two Toyota dealers and a Firestone Tire Center have been unable to replicate the braking issue and tell me that there is no problem with the braking system. I have had both of them inspect the car multiples times, and the issue continues to happen.

Pulling into paved parking spot on very slight uphill grade. Tried to apply brake pedal but car kept moving forward about 6ft over curb and crashed into cement wall creating slow speed impact damage to front end. Towed on flat bed to dealers lot. Tow truck driver noted brake seemed loose and had little resistance. Car still under warranty, 28.8k mi, All warranty required maintenance has been performed to date by Dealer. No prior accidents. Annunciation on screen after event saying "Parking Support Brake Malfunction Visit Your Dealer" Message has subsequently remained. Large number of same or similar annunciation messages over last 4 yrs reported on Toyota Nation web site. Messages indicate that TSB-019-19 may apply and/or DTC C13B0. However, Service Manager at Dealer has refused to investigate this and similar diagnostics to explain safety systems failure or otherwise repair under warranty agreement, unless we first sign a release for personal injury liability with Toyota legal department. Dealer has advised it is not safe to drive car in its present condition. When the sliding doors are left open for any purpose (cleaning, loading, unloading, sitting in a car, etc.) after approximately 60-minutes the sliding doors are disabled and cannot be closed. The key won't work. The button will not work. The overhead will not work. This poses a threat and danger to driver and passengers in possible and many times probable scenarios. If sitting the car at a park and there is a bee or bug swarm, you would be stung to death by the time you manually closed the doors that will only allow a few inches at a time. If there was a sudden weather change or flash flood. If there was an assailant approaching while you are sitting with your family for a bit with the doors open. Any number of things. I took it to Toyota and they wanted to charge me $150 plus for an assessment, to which I declined because this is their problem, not mine. My request is to have this fixed so the key will at least override the door shut off before someone gets hurt because of this. This has happened twice. I will enter the date it happened on the second time. It is Beechmont Toyota in Anderson Ohio that refused to service me.

The rear climate control unit has fallen out of the headliner several times. The climate control unit then dangles from the headliner, obscuring the vision through the rear view mirror and poses a potential safety risk as it could hit a passenger in the event of an accident or abrupt stop.

2nd row passenger seat recline does not work. My seat is very reclined now causing the 2nd row passenger to not be in the optimal position for SRS airbag performance that may result in possible death due to head impact during a side collision. Toyota likely has enough data to issue a TSB to resolve this issue preventing serious injury or death to one of my children.

I am writing to report a significant safety concern involving the second-row seat in the 2021 Toyota Sienna. The issue involves the seat base not locking securely into the floor. As a result, the seat remains unstable and sits at approximately a 45-degree angle, rendering it unsafe and unusable. This malfunction poses a serious risk to passengers, especially during braking or a collision, as the seat is not properly anchored. It also prevents the safe installation of a child car seat or booster. Despite this being a clear safety issue, there has been no recall issued by Toyota, and customer reports online suggest this is not an isolated incident. The inability of a seat to lock into its designated position is a fundamental failure in vehicle safety design. The vehicle has not been inspected and no warning of the issue was given.

Toyota Sienna has 3rd row seats recall already. While I was at dealership I explained that second seats are also rattling when they were fixing the 3rd row recall. My concern is that since they have safety concerns on that they should investigate all the seats for safety.because I’m experiencing second row issues with my vehicle. Terrible dealership! Called corporate office Toyota they were also no help! Please! please!. My request please have Toyota investigate the second row seats as well..

The contact owns a 2021 Toyota Sienna. The contact received notification of NHTSA Campaign Number: 23V865000 (AIR BAGS). However, the part to do the recall repair was not yet available. The contact stated that the air bag sensor turned off and failed to function as intended. The local dealer was not contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue but offered no assistance. The manufacturer advised the contact to stop driving the vehicle. The failure mileage was approximately 30,000.

Toyota has been aware of an occupant classification sensor defect since at least December 2023. Their automated systems state the remedy should be available by 2024 Q3. And yet we were in Feb 2025 and no remedy is available. I don’t think they intend to fix this at all, leaving me with a car that is not safe to use the passenger seat.

Recall 23TB15 was issued on Dec. 20, 2023. The remedy was supposed to be out by the 3Q24. I just called dealer service, and there is still not a remedy out. I am driving a car without knowing if the front passenger seat airbag would deploy in an accident. This is unacceptable to go this long with no corrective action plan yet issued. Can you take any action to get the manufacturer to correct this defect?

The contact owns a 2021 Toyota Sienna. The contact stated that while his wife was fueling the vehicle, his 5-year-old son pressed the automatic rear driver's side sliding door switch and the door opened, bumped, and scraped the fuel door. The contact's wife immediately stopped fueling the vehicle. The contact stated that the vehicle was designed with an automatic door lock that should engage to prevent the rear driver's side sliding door from opening and bumping into the fuel door while fueling the vehicle; however, the automatic door lock failed to engage. The contact stated that he was concerned that the failure had caused metal fragments intrusion in the fuel tank. The vehicle was taken to the dealer, where the cause for the failure could not be determined. The vehicle was not diagnosed nor repaired. The manufacturer was notified of the failure and referred the contact to the dealer for assistance. The failure mileage was approximately 49,000.

The navigation/touch screen has turned off and on during driving. The navigation, audio system, backup camera view were unable to be used. The next available appointment is on September 15th, 2022 and it has not been inspected by anyone. There were no warning signs prior to the incidents.

Sometimes when each of us 2 are slowing to a stop while applying the brake, the Sienna will lunge forward. This unexpected event may cause a rear-end collision if too close. Also, it is enough to cause us to hit our head on the head rest. Can’t understand why this happening. We are sure we only have our foot on the brake and not the accelerator.

When my car is going down a steep hill and I have the brake applied, Suddenly the car will speed up and lurch forward instead of continuing to slow down. This is not how cars normally behave in the 30 years I have been driving. I could have slammed into the car in front of me. This has happened over 100 times. My vehicle has approximately 27,000 miles and I have mentioned it at the Toyota service center and it has been checked out and there are no issues that they see. No warning, lamps or messages appear at all

My young daughter got the seatbelt caught around her waist. The seatbelt was unable to give any slack and was locked up. It could retract a little bit but that would only squeeze around my daughter’s waist tighter. The seatbelt would not pull forward. We eventually had to cut the seatbelt to release her. It caused red marks around her waist and had we tried to retract it any further could have suffocated her. We took the van to the dealer to replace the seatbelt and they had said they heard of this happening before but it wasn’t a malfunction but just a weird design quirk. This incident has not been investigated.

The contact owns a 2021Toyota Sienna Hybrid. The contact stated that while her son was occupying the passenger side in the third-row seat, the seat belt locked and failed to release. The occupant was unable to get out of the seat belt. The occupant sustained marks on his stomach but no medical attention was provided. The contact had to cut the seat belt to release the child from the seat. The vehicle was taken to the local dealer but was not diagnosed or repaired. The manufacturer was notified but no additional assistance was provided. The approximate failure mileage was 13,000. Consumer's son sustained injuries from a faulty seat belt in 2021 Toyota Sienna van. Manufacture refuses to fix the cut belt. The contact has mailed Toyota Corporate Office as well but with no reponse.

The driver-side second-row seatbelt goes slack for no reason while driving. All tension is released, and the loose belt falls forward. This is a serious safety issue for the child riding in that seat with no working seatbelt. This has happened about a dozen times on various days while driving. The dealer first denied there was a problem, then they claimed they fixed it. But it is still happening.

Simalar issue to Toyota T-SB-0121-20 December 14, 2020 Sloshing or Splashing Noise Caused by Water Inside Doors The lower door seals are to tightly fitted and obstruct the door drainage holes. Debris builds up inside the drainage hole. In hard rain or car wash the door fills full of water causing potential for corrosion and damage to electrical components inside door.

While driving during snowy weather suddenly smoke began to billow from steering column area and vehicle slowed down, then stopped, while simultaneously several malfunction indicators lit, I tried to restart the vehicle to pull to a berm, it was unable to restart or even shift into neutral, the vehicle lost all power and had to be towed. This alone could have caused an injury or accident. The problem was from an electrical short from a hole in the firewall made by VMI which permitted snow, road water, and road salt to enter into the cabin area of the vehicle which shorted out the Hybrid ECU circuit board. It also caused electrical arcing to the aluminum cover of the NiMH hybrid battery which had been relocated by VMI to underneath the drivers and passenger seats and can produce electrical shocks of 800 volts or more (the aluminum alone conducts electricity), this alone could have caused injury or death. Furthermore, several relays were shorted-out, the smart key computer. There were burn, arc and water marks on many components noticed. The vehicle has been undriveable and cumulatively in a repair garage for 43 days as of date of this report

Both side power doors become inoperable if they sit open for more than a few moments. They do not close by switch, key fob, or by pushing. Starting vehicle does not activate or reset. Doors must physically be forced closed. It takes considerable force to close the door. Two complaints to dealer were ignored or Toyota Dealer claims this is an intentional design. But has no rational reason for the defect being a design feature. This is a danger to people unable to physically close the door and presents a security flaw for users.

While driving my 2021 Toyota Sienna Hybrid, the vehicle suddenly displayed a low oil pressure warning, immediately lost power, and entered limp mode. I was able to pull over safely, but the engine then suffered a catastrophic failure — a connecting rod blew through the engine block, leaving large metal fragments inside the engine compartment. The vehicle is completely inoperable. Prior to this event the van sometimes idled rough and shook slightly on startup when transitioning from battery to gasoline engine after sitting, but it drove normally otherwise. I have performed regular oil changes and routine maintenance according to Toyota’s schedule. Safety Concern: This failure happened suddenly and without warning. If it had occurred at highway speeds, it could have caused a serious crash due to loss of power or debris from the engine. This presents a significant safety hazard for drivers and passengers. Toyota has issued TSB T-SB-0104-21 for the 2021 Sienna Hybrid’s A25A-FXS engine addressing “MIL ON with Low Oil Pressure” conditions and DTCs P05202A/P052477 with milky or discolored oil. This suggests Toyota is aware of oil pressure and lubrication issues on this engine family. My catastrophic failure appears consistent with a loss of oil pressure or lubrication. I am filing this complaint to alert NHTSA of a possible defect or safety issue affecting 2021 Toyota Sienna Hybrid engines that can result in sudden catastrophic engine failure.

The contact stated no warning light was illuminated. The contact stated that after decelerating the vehicle became controllable. The contact was able to drive to his destination. The contact stated that he had experienced the failure on three separate occasions. On the last occasion, while driving 70 MPH, the contact lost control of the vehicle and the vehicle ended up in the median striking the metal bars in the median. The air bags did not deploy. 2021 Sienna Hybrid Platinum AWD ~65K miles. Safety Issue: Fogging of windows increased, as AC was not working properly. Believe rear AC lines had galvanic corrosion from differing metals, steel bracket with aluminum line-sets causes corrosion hole in refrigerant line set, and leak causing loss of all Freon. No warning lights or messages, AC just stop blowing cold. Vehicle is driven in Michigan on winter roads with road salt, but stored in garage and only 4 years old. $1742 fix to replace line-sets, which can corrode again without proper design fix. Problem confirmed by dealer, and seen in other similar year Sienna's.

The contact stated that upon inspecting the sunroof he noticed that 80% of the glass was missing from the vehicle. The manufacturer was notified, and a claim was opened. The local dealer was notified of the failure and informed the contact that the manufacturer stated that the failure was caused by a road hazard such as a hit by a rock. The contact stated that there was no prior damage to the sunroof. The failure mileage was approximately 12,000. The VIN was not available.

About 25% of the time when I come to a stop and I am breaking, the car surges forward slightly. At first, I thought it was the way I was driving or the fact that it is an EV vehicle, which I had not driven before, but overtime it seems it is just the way the car operates. Others in the car have felt it happen as well as I have come to a stop sign or stoplight. I have not bumped into another car or anything but feel that that could happen if this is not corrected.

When pulling into a shopping center parking space, my mother applied the brakes to slow down, but the vehicle seemed to surge and hit the car in front. It then surged again and pushed that car into another. No airbags deployed, and no one was injured, but someone could have been hurt either by the van itself or one of the other 2 vehicles. The dealer is scheduled to look at the vehicle, but stated on the phone that they "hadn't heard of this happening". There were no warning lights showing. My daughter stated that after the impact, she looked at my mother's feet, and her foot was clearly on the brake. I looked on the internet later, and found several threads of people complaining about their Toyota Sienna hybrids doing similar things.

-Multiple instances recently of feeling like the car was accelerating or lurching forward lightly while I was braking - thought I was going crazy. Today however, the car very clearly lurched forward while I was braking coming into an intersection with my kids in the car. Checked my foot on the pedal, mats, etc. Will be taken to dealer on earliest appointment they have in 7 days. -If there was a car coming through that intersection, my family and I could have been T-boned due to the unexpected acceleration. -There are no warning lamps or messages active in the vehicle. -Symptoms likely started a few months ago, but the most severe instance was today, 9/16/2024. I finished driving my kids to soccer and while there I googled this and called my dealer. Apparently this is a known issue? -How can Toyota not notify owners of this serious issue? There needs to be a recall immediately. This is my family vehicle that we bought new just a few years ago, this is dangerous and absolutely unacceptable.

When breaking, mostly during downhill, the brakes feel like they simply “let go” for an instant. Most times this is very short and It feels like a typical downshift during breaking in an ICE car. However, last time it really gave me a lurch and I almost ended up falling off a cliff face. Many reports all over the internet but nothing from Toyota.

1st. Brake sometimes get very stiff can’t press down and becomes normal after a few seconds or a few press on the brake pedal. (5 months) 2nd. A lot of wind noise coming from driver side windshield starting at 65mph. Complain to 2 dealers both said they test drove the same car but I don’t see any other Sienna around. They lied (1st day I took possession of the car) 3rd. The middle row seat rail are very hard to slide back and forth. (1st day)

This incident had happened twice in the last 30 days. I have been stopped, then starting to accelerate after a stop sign, I push on the accelerator and the vehicle slows down, as if it’s losing power for about 5 seconds. I do not release the accelerator and after the 5 seconds of what feels like no power, the van begins to accelerate like normal. This poses a severe safety risk while crossing intersections when you believe your vehicle should respond when pushing on the accelerator. I did not have any notifications pop up on my van, I was in an open intersection on both instances, with no one in front of me. Speed limit was about 5mph, coming off a complete stop. This is extremely dangerous, especially when your vehicle feels like it stalls without any reason. The gas pedal is completely non-responsive for 5 seconds.

Since we have owned the vehicle (April 2021), we've had three separate incidences where, when pulling out into traffic from a stop, and the accelerator (gas pedal) is pushed, the vehicle begins to move, but then power is immediately lost, leaving you stuck for a moment. Each time has been temporary, and when the pedal is released and depressed again, the vehicle begins moving. However, this sudden loss of acceleration and few second delay in movement of the vehicle now in the path of oncoming traffic can lead to an accident. The most recent occasion happened on July 24, 2023. We've reported the problem to the dealer two times, the most recent today, July 28, 2023, with no resolution.

The vehicle gets a jolt of power or speed while braking slowly or coming to a stop. This could result into an accident. This was addressed to multiple Toyota dealers and the problem was unidentified or undetectable to the technician multiple times. The problem is reported by several Sienna forum users but it is not being fixed by Toyota dealers.

We were driving down the freeway when we got rear ended hard by a large pickup. Then we lost electrical and engine power. The brakes stopped working. The hazard lights stopped working. Luckily we were able to coast to a complete stop as there was no way to stop the car. The police came and took a report. Then the car was towed to a body shop. We're awaiting the insurance company adjuster for inspection.

While traveling in the far left lane of a four lane highway with a speed limit of 70 mph my car started to fish tale but I was able to control it. The cars behind me saw the blowout of my rear passenger tire and slowed down so that I could move to the far right breakdown lane. No warning light came on at any time to alert me of any problems with any of the tires. I always maintain the correct tire pressure. I had a tire company replace all four tires because I was afraid of the other three tires doing the same thing. When I picked up my car the next day the service dept said the brakes didn't work. I had to have the Sienna towed to a Toyota dealer to repair a crushed brake line caused by the blowout. We kept the tire and have taken many pictures.

I purchased a 2021 Toyota Sienna (VIN: [XXX] ) from an authorized Toyota dealer in June 2024. At the time of sale, I was assured that the open safety recall (23TA15) would be repaired soon. More than a year later, no remedy has been offered, leaving me and my family driving an unsafe vehicle. Toyota’s own phase schedule indicates repairs should already be available in Washington State, yet I have been repeatedly told no repair exists. It doesn’t break until I put on the emergency brake. the seat broke only weighed 185 pounds! the wiper blades don’t do a great job cleaning the driver’s side after I bought new ones. At the begining of 2024, I received a recall letter from Toyota regarding the airbag malfunction. I was told to wait for a remedy by Q3 of 2024. I patiently waited and was very anxious driving my vehicle. I recently did talk to my local dealer during my maintenance and they stated that they do not have any information regarding that and that I should call Toyota. I tried calling Toyota and was instructed to wait as they still have not got a remedy and that if I was getting in any accidents Toyota will be fully responsible. I bought the car to drive for my safety an this is the level of safety that I am getting. Thus I am here looking for help on how I can get this matter resolved as driving knowing that airbag has failed on the passenger side is very unsafe.

This recall has been open for 7 months and a remedy has still not been issued - this doesn’t seem reasonable for a safety concern. I have also several times experienced a failure in the “extra” 8th passenger seat (middle row middle seat) sensor - it alarms that the seat belt isn’t buckled even when that seat is not in use and not even installed at the time.

Check engine light came on - Taken to dealer, diagnosed following 2 codes : P24517F and P14502A Both of these codes are known problems to Toyota, and as such a Service buliten for the Evap Emission System Switch Valve has been issued per NHTSA. This problem should be a recall, not a known service issue - it is obviously a common problem with Toyota in the Hybrid engines.

There has been several incidents when I step on the brakes and the vehicle seems to accelerate forward after what feels like an attempt to find the right gear when decelerating. This occurs infrequently but usually when decelerating along with an external factor. I was able to consistently replicate when I’m coming off the highway onto a service road then have to make quick turn at a corner. It feels as if the vehicle is trying to search for a gear and once it finds it there’s a momentarily acceleration forward.

2 issues to report 1) when under medium consistent breaking(slowing down for a light) there are instances where the breaks feel like they let up or slipping. This issue has been reported and with the car since the first year of ownership but Toyota says it’s all within spec 2) second issue is an issue with the rear captions seatbelt and has also been reported since 2022 where upon normal driving the seatbelt locks up and the car has to be stopped and seatbelt unbuckled to release tension. Also brought up multiple times with Toyota and no action. Kids sometime unbuckling while car is driving since they cannot move

In the 2-1/2 years than I have owned and driven this vehicle, I've had no problems. On Saturday, [XXX], driving home from a 80 mile trip, as I deaccelerated to make a left turn onto my street (speed was 7mph) I tapped the break pedal and the vehicle went bonkers! The Sienna veered right at a high rate of speed that the front end cleared the curb; the back end smashed into the curb flattening both rear tires and damaging the metal hubcaps; the vehicle speed tore off the bumper and both rear panels. I was able to steer the vehicle: missing a large 2-foot high tree stump; two other large trees; a 10-foot high solid fence at the yard back. I then drove down the sidewalk to the next intersection and was finally able to slow down. This entire time, my foot was on the break, but the vehicle would not stop. The Sienna is now at Faulkners Collision Center in Mechanicsburg with virtually no front end damage but colossal rear damage. The vehicle had 5-full maintenance checks since purchase. I was not told of any suspected problems and never received a vehicle recall. I could have been killed or may have killed others as there is a playground on the corner of this block. I BELIEVE THE COMPUTER CAUSED THE PROBLEM. THE CAR IS AVAILABLE FOR INSPECTION AS PERSONNEL AT GETTEL TOYOTA IN PUNTA GORDA CLAIMED THEY COULDN'T TOUCH THE COMPUTER AND HAVE DONE NOTHING. Purchased Sienna in August 2021. In October at about 5:30 a.m. we were in the far left lane on a PA interstate, traffic came to a stop for construction. A delivery truck in front of us rolled backward a bit. When traffic started up again, our car would not go forward, only reverse. We tried "drive"--nothing--only had reverse. Finally, with cars whizzing around us, shut the engine off. Upon restarting, all gears were restored and we continued on to Boston. We were shaken by this event. Since then, we have had numerous incidents of the car not moving when I put it (mainly) into reverse. I had to shut the engine off--restart--and it went into gear. Latest incidents--first week February 2024--I pulled into a parking space at Englewood Hospital. After visiting my husband, came out, started the car, put it into reverse--NOTHING. Shut the car off--restarted it, and it was ok. Ten minutes later, I pulled off onto the shoulder to take a call from one of his doctors and--no gears. Shut off the engine, restarted it and it was ok. I'M WORRIED I'LL STOP FOR A TRAFFIC LIGHT ON ONE OF OUR 4 OR 6 LANE STREETS AND GET HIT BECAUSE OUR CAR WON'T GO INTO GEAR.

Recall: OCS Sensors May Short Circuit; 3TB15/23TA15 ; I am a owner of 2021 Sienna Hybrid (VIN [XXX] ) and was getting alert for the airbag and seat belt warning even when plugged in. I took it to a dealer in Sunnyvale California (Stevens Creek Toyota; 4202 Stevens Creek Blvd San Jose, CA 95129; 408-984-8833) and they are not recognizing the recall. I was charged a diagnostics fee of $ 899.93 and am being asked to pay $ 2000 to replace the car seat to fix the issues. Your advise is solicited. Best regards. The contact owns a 2021 Toyota Sienna. The contact stated while driving at approximately 50 MPH, the rear end of the vehicle started to sway to the left and the right. The contact attempted to steer the vehicle however, the steering wheel was very difficult to turn. Additionally, the contact depressed the brake pedal, and the pedal was very loose and depressed to the floorboard. The contact pulled over and had the vehicle towed to a dealer. The contact stated that the dealer was not able to duplicate the failure. The vehicle was not repaired. The manufacturer was informed of the failure. The failure mileage was approximately 73,000.

1. The vehicle displayed the Rear Cross Traffic Alert (RCTA) warning light along with the Blind Spot Monitoring (BSM) warning light on the dash board. The vehicle was brought to Toyota of Ann Arbor for an inspection. After inspecting the vehicle, Toyota of Ann Arbor reported that road debris (snow, dirt, mud, etc.) was trapped between the bumper and the sensor resulting in the sensor to malfunction. The vehicle has been taken through a car wash with an underbody wash option, but due to the position of the sensor, the sensor is not able to be cleaned from a car wash. The sensor can only be cleaned by removing the bumper. But when the bumper is removed, the sensors have to be re-calibrated and the re-calibration of the sensors can only be completed by trained professionals. 2. Due to the malfunctioning sensor, the driver's safety is put at risk by not being alerted of obstructions (both objects or persons) behind the vehicle, nor oncoming cross traffic, nor detecting vehicles in blind spots. This could cause collisions or worse, fatality, due to the malfunction sensor not alerting the driver. 3. This vehicle has been able to reproduced the problem multiple times and as mentioned above, the vehicle was inspected and confirmed by the Toyota of Ann Arbor dealership. The dealership (Toyota of Ann Arbor) mentioned that there are multiple reports of this issue on other vehicles of the same make and model. 4. The vehicle or component has not been inspected by the manufacturer, police, insurance representative, or others. 5. No prior symptoms were seen or displayed prior to the failure.

The sliding door on the passenger side opened while I was driving the vehicle. The door started out as closed. This is unsafe because I had a 6 yr old in the middle row of seats. I have not yet contacted the dealer or service center. The vehicle has not been inspected by anyone. There was no warning prior to the failure.

The contact owned a 2021 Toyota Sienna Hybrid. The contact stated that while driving at an undisclosed speed, the check engine warning light illuminated. Soon after the malfunctioning hybrid system "pull over immediately warning message displayed. The contact stated while he was driving the vehicle to a nearby parking lot he noticed smoke coming from under the hood. The contact then parked the vehicle and removed a 4-year-old and a 1-year-old from their car seats. Flames came from the hood area and he grabbed a nearby water hose in an attempt to extinguish the fire and property owners had a fire extinguisher that was also used. The fire department arrived and extinguished the fire. A fire report was filed and there were no injuries. The vehicle was towed to the dealer and an investigation was completed where it was mentioned that the origin of the fire was the hybrid component. The results of the investigation were not released to the contact. The vehicle was declared a total loss. The manufacturer was made aware of the failure. The failure mileage was approximately 23,500.

The breaks give out to a point where you run the risk of running into someone or something. We've visited Toyota 3 times now complaining of this issue yet they just state it's the regenerative breaks and how they operate. Although the breaking never got to this level of safety concern until we reached around 30,000 miles. I filed a complaint with Toyota that resulted in nothing being done. A couple examples would be sitting at a stop light and the breaks release almost causing us to run into the car in front of us. Pulling into daycare trying to stop while pulling into the parking spot and the breaks release. The last one is while going down hills the breaks seem to let go and the vehicle speeds up. It scared my wife so much that we actually sold the vehicle as Toyota deemed it safe. We loved the vehicle but can't risk the safety of our family.

The vehicle has had rocks get in the suspension 4 times so far which creates loud clunking noises and can cause a noticeable feel in the steering. The most recent time, after the rock entered the front end, the brakes were soft and required excessive pedal force followed by a clunk before the brakes abruptly applied. I like on a paved road with a rock driveway and it seems like the shielding for the front end is inadequate. I have friends with a 2022 sienna that live on a gravel road that have experienced similar issues.

I would like to make formal complaint in regards to a manufacture defect. I purchased my Toyota Sienna XSE brand new in August of 2021 at Cerritos Toyota in California. On December 24th at approximately 1:30 p.m. on Christmas eve, after filling up gas at Sam's club in Fountain Valley, CA. I pull out on to the street heading northbound on Warner approaching intersection as I come to complete stop on red. All of a sudden, my Sienna completely shut itself off and the engine light came on. I put it back on park and restart the car. As I attempt to accelerate forward. The car jerk and the brake didn't work and the engine light came on. Fortunately, I was able to make a U turn and pull into a gas station near by ( Shell) and I thank the lord almighty there was no car right behind me. I could of gotten rear ended. It was the most terrifying moment of my life. My wife and I was in the car at the time of the incident. We waited patiently over an hour for Triple AAA to have it tow the nearest dealership next to my house in Long Beach, CA. My 2021 Toyota Sienna currently has 52,023 miles and sitting at the dealership to be repair at Long Beach West Coast Toyota. I have contacted Toyota of this incident and they refused to fully covered repair instead they offered me 700 payment assistant the rest would come out of my pockets. I believed the car should be part of the campaign recall and nothing should come out my expense. I would like you you guys to investigate the incident and look into this matter as soon as possible because there thousands like my mind that could there have the same issue but was not part the campaign recall. While slowing down from highway speed 55mph to about 30 mph preparing to turn right, I heard a huge explosion (sunroof). Thinking it was overhead, I slowly pulled back the sunscreen which blocks out the sun from the roof, realized it was full of broken glass and immediately closed it. This was a terrifying event and could of easily caused serious injury or worse. My dog was riding in a secured dog crate in the rear of the van. The van was taken to the dealership for investigation and repair. ( McDonald Toyota, Greeley, Co 80634) although the dealership is still investigating and agreed to follow up with Toyota, they are currently denying warranty. I went on a trip to Missouri to visit my son and family. When going through St. Louis on 5/18/2022 I ran through a thunderstorm with a lot of rain. After I stopped, I noticed a puddle of water between the front seats. I could not find out where it came from. Later that same day I went through another storm. Again I found a puddle of water between the seats. This is a vehicle that was modified by VMI into a handicapped accessible van. Water coming in is concerning due to all the electrical components that could be shorted out. I don't want to find out later that there is rust damage nor damage to the electrical systems because of water entering the vehicle.

While driving approximately 70 MPH the vehicle tends to "float" or wanted to change lanes on me. The vehicle would jerk to the left or to the right requiring corrective action on my part. At first I attributed the problem to the windy conditions, but later decided the wind was not the cause. It appears that when driving less than 40 MPH, there is no problem. This problem continued for my entire trip (about 1400 miles). This is a vehicle that was modified by VMI into a handicapped accessible van. I don't know if regular Toyotas do this or not.

Our Sienna has had incidents of unintendedly accelerated a minimum of 25 times. I have been unable to stop the vehicle several time to the point where I have had to go through a lighted stop intersection. There have been numerous "close calls” where I have nearly hit the vehicle in front of me. My wife, while driving to pick up my 4 children, actually bumped into a vehicle in front of her. It is a serious risk to expect that I drive the vehicle with my wife and 4 kids inside, yet Toyota expects I will drive the Sienna for over a month while they wait to get the report back about the vehicle. I am not willing to put my life at risk along with my wife and four children while we wait for Toyota to address the issue. We brought the Sienna in after the initial few close calls (March 14, 2022), and they were unable to find anything wrong. After receiving the vehicle back, the issue continued. Once my wife was in an accident with the Sienna, we have decided we will not be driving it until we are assured it has been resolved. It was brought back to Toyota on April 19, 2022). At this point and I not confident Toyota will be able to fix the issue. Toyota has deemed the car safe to use; even though they have not received back the report. It's reckless and immoral to knowingly place someone in a vehicle that has already caused 1 accident (although thankfully very minor) and something I am unwilling to do. Meanwhile Toyota has offered nothing to keep me safe or offered a vehicle in the meantime. The forward collision does not work. The entire vehicle is available for inspection. I am not certain what Toyota has inspected, but an inspection was performed on May 10th and the results will not be available until June 13th. Problem is not able to be reproduced as the car has not been driven. The warning sign prior to the accident was numerous other near misses.

Van has just over 5k miles. Multiple warnings occurred while at speed on a highway. Primary warning was 'Low Brake Power' and braking became extremely touchy as it seemed to utilize the emergency brake as a backup. This extreme braking is hazardous and could result in an accident caused by braking too quickly - with a possible rear-end collision by traffic to the rear. Additional warnings/malfunctions were listed as well, due to the low brake power: LTA Malfunction (Lane keep assist), Automatic Emergency Brake Malfunction, Parking Brake Malfunction, Check Engine. There were no symptoms of poor braking prior to the incident. Dealer filed a TAS report with Toyota to address the problem, and was subsequently inspected by the Toyota Field Representative. Initially could not duplicate/identify the issue and the van was returned. Within one week the incident reoccurred, again on the highway. Van is currently with the dealer and the Brake Booster and Brake Master Cylinder will be replaced. Expect the dealer will retain the parts.

With the rear seat entertainment system in use, the rear visibility is completely blocked in conjunction with the standard rear view mirror. This is a major design defect and is avoidable with available options that should be standard. Without being able to utilize all possible ways to view around the vehicle, Toyota has put its owners, occupants, and fellow drivers at unnecessary risk. An easy remedy is Toyota retrofitting and including going forward their digital rear view mirror.
