For over a year now, I have been experiencing a random issue in my van with the brake system. It feels like a slippage when I hit the brakes to slow down and it will randomly feels like something slips and just give gives out and gets loose and like you are free falling until the brakes grab and actually stop. I have had it in the dealership twice to fix this issue. Last year they reprogrammed the brake actuator assembly control module, and it worked for a while, but then once again after several months, it started again and the second time I took it in they drove it around to try to get it to mimic the problem, but was not able to so Couldn’t do anything about it and just said to watch it and see if it became more frequent. They also said that once you have the actuator reset the computer shows updated and can’t redo it.The problem with this issue is that it is random and sometimes it’s less and sometimes more and I can’t give specific dates because of the randomness of the problem. I have never not had it to stop, but I am including my two invoices from Toyota of Richardson where I’ve had it seen as well as the research that I have looked up in different forums where other people are describing the very exact same problem with their vehicles. [XXX] . It is scary because you don’t know if it’s going to stop or not but so far it has. [XXX] . There is an issue, but don’t know how to permanently solve it or know for sure what it is. The forum does explain some different things because of the vehicle being hybrid, but who knows whether that’s it. Please look into this and advise because I do not wanna wait until this is a problem and I have a wreck because I am not able to have my brakes work Correctly. P.s. Brakes release briefly when slowing to a stop. While slowing to a stop with steady pressure on the brake pedal, the braking action will momentarily give out, resulting in the vehicle lurching forward beyond the anticipated stopping/slowing distance before the braking action re-engages. This has resulted in near collisions with cars, bikes, and pedestrians as the vehicle suddenly stops slowing without warning and unexpectedly increases stopping distance. The problem is intermittent and difficult to reproduce, but has occurred repeatedly over at least 6 months of ownership. No warning lamps, error messages, or other related symptoms from the vehicle either before or after occurrences of the problem. Has been taken to the dealership for an update to the vehicle but it didn’t fix it. There are multiple reports online and multiple lawyers talking about this.

When braking and coming to almost a complete stop, the vehicle will randomly lurch forward. This situation has caused me to run stop signs and almost hit pedestrians on crosswalks. This failure has been independently observed by myself, wife and mother. The vehicle did not display and warnings prior to the failure. The vehicle has not yet been inspected or reproduced by a third party.

I am writing to formally raise a serious and unresolved safety issue with my 2022 Toyota Sienna. The braking system on the vehicle behaves abnormally: when the brake pedal is pressed, the van does not stop immediately. In fact, it momentarily accelerates before decelerating, an extremely dangerous malfunction that has nearly led to multiple accidents. I have brought this issue to your attention several times: • In 2022, I contacted O'Regan's Toyota, the Halifax dealership, which has been unable to diagnose or repair the issue to date after several visits. • In 2023, I contacted Toyota Canada directly. I was referred to the dealership in Halifax. • The Halifax dealership has been unable to diagnose or repair the issue to date. • Despite the unresolved nature of this problem, the dealership has recently asked to charge me diagnostic fees for the same issue, one that remains unsolved and is clearly a manufacturer-level safety defect. This is not a minor inconvenience; it is a fundamental failure of the braking system and presents a critical safety hazard. The fact that this issue persists without a resolution, and that I am being asked to pay diagnostic fees for a problem that Toyota has not addressed, is unacceptable.

seat retractor failure- white plastic part off and visible , seat is making popping noise when seat is being adjusted. Noticed this when i picked up from dealer after oil change. I feel this is a driving safety concern. Larry H Miller American Toyota (Albuquerque, NM) service manager states it's safe to drive, however I feel this is a huge safety concern. The seat was in working order did not have this issue during drop off of service. However I believe this is a failure on Toyota engineering. I would like some guidance on how to get this repaired as they want me to pay $2,000.

Back seats do not lock down when folded. This almost caused my pet to get hurt when going over a speed bump sent them flying up. Seats should lock down when folded to prevent injury to pets. Also almost hurt by daughter in backseat when the single seat came up flying up and re-latched into up position from folded position hitting her arm in the process.

3rd row seats when folded in well: They don't stay put in well and they come out with a bang upon applying a sudden break or going over a speed hump. Why is it a problem: 1) if a pet is there on the back, it could cause a serious problem 2) It makes a big bang when came out of the well and It make the driver think like they are rear ended and stopping their vehicles suddenly. 3) any light weighted articles fly to the front creates a possible hazard. What am I expecting: they stay put in the well and locked. What did I do? I spoke to the dealer and they acknowledged the problem and simply said that's how it is designed with no solutions.

3rd row seats do not lock into place when folded down. As a result, when going over a bump or applying brakes, the seats can start to bounce. Upon hard braking the seats can actually fully pop up and transform to unfolded position, potentially injuring anything in front of it.

Right after parking my 2022 Toyota Sienna XLE, I heard a VERY LOUD boom from overhead. It sounded like an explosion. Upon getting out of the car to see if something hit the car, I noticed the glass of the sunroof had a shattered pattern (but none of the glass actually came apart). My and my family's safety is at risk because at any moment, glass could shatter and enter the car. Fortunately, my moonroof cover is closed. Other people are at risk because if glass breaks off while I'm driving, it could hit other cars or pedestrians. I am writing this within 30 minutes of the explosion and have not taken it to a dealer, independent service center, nor has it been inspected by the manufacturer, police, insurance representatives, or others. There were no warning lamps, messages, or other symptoms of the problem prior to this incident.

We just recently purchased 2022 Toyota Sienna XLE with REAR SEAT ENTERTAINMENT package which is a 11.6-in. display. We did not use this display until yesterday when my daughter wanted to watch something. After she opened the screen to watch, I realized that this display screen covered/block the rear-view mirror view 100% and can't see anything. Before I tried to tell that she needs to close it, I had just a near-miss where I could have colloid to car in backend. This is a huge safety risk due to poor design of covering 100% rear view by this REAR SEAT ENTERTAINMENT screen. There was no car available to test drive at nearest dealer where I would have catch this. If I would know this safety issue, I did not have purchase this car with REAR SEAT ENTERTAINMENT package. Higher Trim has the "digital rearview mirror" option that could have alleviate this issue, but they don't even give that option to select as optional package on XLE trim with XLE plus and REAR SEAT ENTERTAINMENT package. I paid ~$4,000 for XLE plus and REAR SEAT ENTERTAINMENT package and it does include the "digital rearview mirror". This is a huge safety issue in point view and requesting NHTSA to take an action on it. I just googled it and found others complain for this hazards concern. When using polarized sunglasses, visibility of the driver and passenger front windows gets obstructed (standard windows, no modification) . This means, that these windows are partly intransperant. In an irregular pattern clouds seem to be dispersed all over them. In total maybe one third or up to 50 percent of the window seems to be way less transparent, only while wearing polarized glasses. Without glasses there is no impaired visibility at all.

Driver and passenger front window have obstructed visibility, when wearuling polarized glasses. This might be no issue for sunglasses but with regular glasses, it could impact safety. In an irregular pattern clouds of low visibility are spread across these windows. Normal sight without my or my wife's sunglasses is not impaired. It was observed in full sun while being in clouds I didn't try it.

Note: I have filed a complaint with the Florida Attorney General's Office Health Hazard. Mold Issue. Flooding Issue. Failing weather striping. We had a weather strip on the rear door get misplaced or is failing to stay on our 2022 Sienna. Toyota won't cover it. And we will have to make a claim per their recommendation, unsure. Third seat rusted; it was submerged in water. Didn't smell it or see it. 20k in damages. We never use it for anything except for groceries in the stroller. The worst decision I have made is bringing it to Toyota, because with it closed up, in the heat, the Van now has as all this mold. Before it was just wet in the 3rd seat compartment. It also had a Frog in it as large as a small hand. Improper storage. Toyota corporation also won't help. Don't be fooled and think this vehicle is imperfect. The new and previous generations have had this problem. They leak from the top and bottom of this door seal. Toyota seams to refuse to make a TSB. You do not have to look far online to find the reported issues. If it can happen to normal people like us doing normal things and many other people like us reporting it, it can and will happen to anyone. A Vehicle less than 3 years old should not be leaking. I have spoken online to other owners experiencing the same. But now since I see Toyota will not assist, and made it worse for us by improperly storing the Van and taking too long to respond. Now we cannot use it and it is a health hazard. Instead of helping me, they instruct me to file a insurance claim I would otherwise not have had to file, if I just air dried the Van. They should have acknowledged the problem and taken the opportunity to improve the design. Not create a mold situation for the customer and tell them to file a claim. This is a quality control issue. My issue is not with my Sienna but an appeal to the DOT to do some regulation or oversight of all the bright head lights on vehicles that are now so bright and many colors that is is dangerous at night for some people with eyesight problems to judge distances of oncoming vehicles. In a mix of vehicles some will have awesomely bright lights and some dim lights that make it just impossible to gauge what is what and some of these vehicles have huge light bars too, some of the large pick up trucks with these lights when behind me are so bright I can't see behind me, some vehicles have red and blue lights facing forward that first responder vehicles normally use. The DOT has to get on top of this wild west on our highways of headlights that are harming other drivers. Even the headlights on my Sienna HV with the adaptive feature are blinding to other oncoming drivers.

Premium Audio System. I have a bunch of FM and XM favorites saved as presets on my radio. After the 3 month trial of Sirius XM expired, I am having trouble with my presets. Whenever I zap through the presets and coming from a FM to an XM station, I can't control the direction of zapping anymore. Hitting the left button triggers going right, hitting right triggers right as well. This forces to zap through all presets. The moment I hit the expired XM preset the whole system suffers from lag of more than a second for every click. The only reasonable way back to control is switching to the Audio menu and start over with selection of FM source. Best thing: I can't delete the XM stations on this premium audio system.. That's a bummer. Dealership found this behavior strange, and tried to reset the system, didn't work at all. So they suggested to store 25 FM stations to override the existing XM presets.

Got my 2022 sienna with the factory installed rear entertainment system. When the entertainment system is in use, there is ZERO visibility out of the rear view mirror or window. This is a huge safety issue as I have almost been rear ended 4 times now. Today while it was snowing, if I had my entertainment system in use, I would not have been able to see that someone was sliding in to me. Luckily I saw out of my rear view mirror and I was able to hurry and speed up to not get hit. I have taken the car to 2 Toyota dealerships who have both verified and took pictures to show that you cannot see a thing out of the mirror or window when the entertainment is in use. They all agree it is a major safety issue and reported it to Toyota corporate and opened a case to get it fixed. The fix is to install a digital rear view mirror for those who have the entertainment system (which again comes from the factory) Toyota brand engagement said that yes this is a safety issue but since there is no ope recall they won't fix it for me by giving me a digital rear view mirror. This is absolutely crazy that there is this big of a design/safety flaw over looked by Toyota. They also told me that there have been some that have been fixed however it is a case by case basis and basically depends on who you talk to. I just paid 52,000 for this car and I can't use the features it comes with because of this huge safety issue. If I had my back window taped up completely blacked out and couldn't see through it, I would get a ticket from the police for a safety hazard of not being able to see out of my back window. How is this different? This is unacceptable and should immediately be recalled for those vehicles that toyota put entertainment systems in. This issue could cause people to be rear ended because they cannot see anything out of their windows. I can send pictures that the dealership took to verify this safety issue. It needs to be addressed and corrected before people get hurt.

We have an rear entertainment center package in our 2022 Toyota Sienna van, when we pull the screen down it completely blocks our rear view due to screen is too big. I am wondering why Toyota didn’t install digital rear view mirror to all trim levels with rear entertainment center, but they automatically install the to limited and platinum trims. To us that is an safety issue, for instance if there is a cop, fire truck or ambulance coming up behind us we wouldn’t see them do to it blocking our view. I have a 12, 8 and 3 year old and everytime they get in van they want to always watch the rear entertainment center. Thanks.

The complaint was the failure mode of a no start condition while traveling. The issue was related to partial or improper function of the DSM which caused a drain on the battery and battery cell damage requiring full replacement. The fix was a software update to prevent recurrence which cost several hundred dollars (according to the Toyota Service Bulletin or TSB-0089) along with approx $200 for diagnostic analysis. The TSB work would have been covered if we had not exceeded 36,000 miles. We had the vehicle for 3 years and 1 month. I do not understand the mileage impact to the coverage of a firmware update as this was related to a no start condition and could have occurred when we stopped at a rest stop area as we were traveling across country at the time. I am very disappointed with the failure and the impact of getting several jump starts for our vehicle as well as the couple days waiting to get scheduled into a dealership and 4 hours to get fixed.

WHILE STOPPING AT TRAFFIC LIGHT BEHIND OTHER CARS AND BRAKING ON SLIGHT DOWNSLOPE THE BRAKES LET OFF FOR A SECOND OR TWO, THEN RE-ENGAGED. FELT LIKE WHEN YOU HIT AN ICY PATCH OR OIL SPOT ON PAVEMENT. THE 2022 VEHICLE IS UNDER 3 YEARS OLD AND HAS 10,539 MILES ON IT AND THIS WAS FIRST TIME THIS HAS OCCURRED.

This van was converted to be wheelchair accesible by Vantage Mobility International. My parents have repeatedly had an error arise that indicates an issue with the hybrid battery cooling system. This system was modified by VMI, in partnership with Toyota, to allow for the accessibility features to be added to van. Neither the vehicle dealer, Superior Van and Mobility in Knoxville, TN, nor the local Totota dealer, Toyota of Knoxville, have been able to rectify the issue, despite mutliple attempts.We are concerned that this is a safety issue, and have asked for written confirmation of that and for the issue to be resolved. None of the three parties, Superior, Toyota, nor VMI, have been willing to do that nor rectify the issue. (I have filled this out for my parents because this has already become so stressful and in the wake of my dad's rapidly progressing disability, I don't want either of them to have to deal with more of the headache with this.)

The contact owns a 2022 Toyota Sienna. The contact stated that while his wife was driving at 65 MPH, a loud sound was heard. Upon inspection, the contact noticed that the sunroof had exploded; however, the sunroof panel was closed, and no glass entered the interior of the vehicle. There was no impact on the sunroof that could have caused the explosion. The vehicle was taken to the dealer, and the dealer referred the contact to take the vehicle to Vantage Mobility International (VMI, 5202 S 28th Pl, Phoenix, AZ 85040) due to the wheelchair van conversion. The vehicle was taken to VMI, where it was determined that a small impact caused the sunroof to explode, and the sunroof needed to be replaced. The vehicle was not yet repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 750.

The contact owns a 2022 Toyota Sienna. The contact stated that the vehicle was designed with an 11-inch screen located in the roof between the front driver’s and passenger’s seats. The contact stated that the screen was designed for movies and entertainment however, the screen obstructed the rearview mirror while flipped down. The dealer was notified of the failure and referred the contact to the manufacturer. The manufacturer was made aware of the failure and informed the contact that they could not assist. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 90.

LANE DEPARTURE,BACK OVER PREVENTION

While driving approximately 25 mph my dash lit up with the cross traffic sensor is unavailable and my blind spot monitor is unavailable. I got out and cleaned off my sensors on the outside of my vehicle all around the vehicle front and back and the warning has stayed on. It was not raining or snowing and I was on dry pavement at the time. My vehicle was not hit nor did I hit anything. There is no damage what so ever to the sensors or anywhere on my vehicle.

We are not talking about malfunction, since it's underperforming by design. Rcta and BSM don't make any audible alert. Every other vehicle I owned had this. Here BSM is silent and rcta can't be heard, because volume can't be adjusted. This is dangerous, since relying on visual with rcta doesn't make sense, when you try to watch you back in reality through the rear windows. Please recall this toyota safety issue because it doesn't utilize its potential and poses an unnecessary risk for everyone.

While operating the vehicle in the city of Helena, Montana, foreign matter entered the interior of the rear bumper. This caused the RCTA and BSW systems to fail because the sensors were obstructed by mud. This failure condition happens to my 2022 Toyota Sienna every three weeks. Toyota has designed the vehicle to allow the ingress of debris to the *interior* of the rear bumper, therefore the failure of RCTA and BSW is inevitable. Toyota can and should know that this failure is inevitable but has offered no repair, nor has it offered advice on how to restore operability after failure. Yes, mud can interfere with safety sensors on any vehicle, but only this 2022 Toyota Sienna intentionally allows mud to accumulate on the *interior* of the bumper resulting in this otherwise avoidable failure.

Pre-Collision System Malfunction, Hybrid System Malfunction, AWD System Malfunction suddenly when restarting the vehicle. Towed to Toyota of Downtown LA dealership. Toyota of Downtown LA Service tells me that the CPU on my relative new Sienna 2022 was fried from jump start when Sienna starting-battery was low. It was done by a AAA professional, I doubt that is the reason for the fried CPU. I believe Toyota engineers would not design/allow the CPU vulnerable to being fried from a jump start. This is a horrific design flaw if it is this way. This happens now, just 6 months after the 3 year warranty coverage. Toyota of Downtown LA Service is telling me it is going to cost me $2900 to replace this CPU. I find this unacceptable.

Yes, was replaced once under of warranty and failed again. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Vehicle will not start every single winter! Has been in the toyota repair shop multiple times. Told me it was possibly the kick sensors that are activated in the cold, stay on and then run battery dead. It is currently 20 degrees outside, but was parked in the garage all night, and STILL won't start in the morning! Completely unreliable. Nearest dealership is 1 hour away from me. Kick sensors were replaced last year, battery has been checked. Still happens. The vehicle is completely unusable and unreliable. Other issue- battery is in the back of the vehicle and you cant open the back door to access the battery to charge it! You have to get an external battery charger, charge front battery to open back door then jump back battery. If I was out and about I would have no way to do that. Absolutely unacceptable and a reoccurring problem. This needs recalled. Many others online report the same issue. Please respond.

During heavy rain exhaust system become dislodged and fell to the road, only hanging on by the engine connection. We had the vehicle towed to the dealership only to be told that is not a warranty issue. They did reinstall it in the rubber brackets. Due to my persistence a warranty claim was opened. There claim was that it hit something as there were marks across the muffler however those were made by dragging on the road until vehicle could be driven off the road. Knowing it was most likely water pressure from the water on the road that lifted the exhaust up and out of the rubber brackets, I wanted this documented. Although if the exhaust system had completely broken off from the engine it would have not hurt our vehicle occupants, any vehicles behind could have been hit.

This vehicle accelerate suddenly while it is slowing down even if I have foot on break paddle. I have complained to Toyota dealers but they said that hybrid vehicle operates like that. This is very dangerous and nobody is taking seriously. I do not believe that this works like that. I have seen so many people complain online about this problem in 2022 hybrid sienna. Please help.

The contact owns a 2022 Toyota Sienna. The contact stated while driving on the highway at 35 MPH, she depressed the brake pedal however, the vehicle inadvertently accelerated. The contact stated that there was no warning light illuminated showing that the accelerator and brake pedals were being depressed simultaneously. The contact was able to drive home safely. The vehicle was not yet taken to be diagnosed. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 1,600.

Question 1 Auto Headlight continues to malfunction and it has been inspected 3 times by Randy Wise Toyota (Grand Blanc, MI). Yes, it is available for inspection (video & list of incidents provided to Randy Wise Toyota - as requested by Tech/[XXX] but no action taken). Question 2 High Beams have come on when oncoming vehicle is between 300 to 500 feet - potentially temporarily blinding the person in the oncoming car and causing an accident. The High Beams also have come on when following vehicles within 300 to 500 feet, also in the City (with city lighting) the high beams were on with a City Bus approaching 1 block away. We have had numerous cars flashing their bright lights at us as our high beams do not dim to low beams appropriately potentially temporarily blinding us and creating an accident. We also are concerned about a potential road rage incident due to others thinking we are intentionally turning on our high beams. Question 3 No the dealership Service Director [XXX] took our vehicle home (23 miles each direction - during the day time) but only test drove the vehicle for 6 miles to try to duplicate the issue. When asked how many vehicles he passed, was told he was not available (out of town) we asked for a phone call to discuss this - but never received a call. Tech/[XXX] stated that we needed to provide video documentation and to keep a list of when the issue happens and details - which was provided but since there was no "CODE" they determined there was no problem. Question 4 Vehicle has been inspected by Randy Wise Toyota 3 times. Question 5 No warnings prior to failure. First appears immediately after purchase. Add'l Info: Video provided to dealership shows us following a vehicle (never loosing site of their tail lights) when an oncoming vehicle approached and when the oncoming vehicle was approx. 500 Ft. in front of us our bright lights turned on and remained on until that oncoming vehicle was approx. 300 Ft. from us. A tire and wheel came off of my 2022 Toyota Sienna while driving over a mountain pass. I was going downhill and around a curve, heard and felt some clunking, then watched my tire bounce across the two lane highway and over the steep dropoff. This was a potentially fatal occurrence, for so many reasons. This malfunction put the oncoming traffic and the drivers behind me at risk for collision, or worse. There was no warning lamp on, my tire pressure sensors read steady, and no other system lit up before or during this occurrence. The dealer can not find a reason as to why this happened within a year of owning it.

Occasionally there is a message popping up about a malfunction of the tire pressure sensor while driving. This appears in conjunction with the yellow flat tire warning. It is very inconvenient, since I have to stop the van just to find out that the tire pressure is fine. Actually it's more than inconvenient since this may lead to a kind of negligence of this system what poses a safety thread. On top of that, the Toyota connect is app keeps telling me, that my tire pressure is low. Went to the dealership, they say they can reproduce the error message and the problem is known to Toyota. However they can not fix it. It seems to be a software issue.

The wiring harness connector for both lights has had water accumulate in the connector which causes the rear tail lights to fail. Both of my tail lights have failed, and the cause was corrosion inside the wiring harness connector. I had to replace the wiring harness and connectors on both sides, driver, and passenger And I had to pay out-of-pocket. The dealer said that this problem is common on 22 sienna's and I have seen other 22 sienna's with out their driver and passenger rear running lights burnt out.

The Entune 3.0 System governs navigation, calls etc. The system on my car has been replaced 4 times and still does not work. This is a life safety issue as the system grabs phone calls while driving without being able to answer. It also makes it frequently impossible to use hands free on the phone as it grabs calls. In order to avoid it grabbing calls I have to disable bluetooth on my phone which then causes my phone to not work in handsfree mode (ok google etc.) Furthermore the backup camera does not always work. Finally, a Toyota Service Manager told me in his oppinion 25% of the entune units are "bricked," non functional and need to be replaced. Toyota has fired according to him Panasonic as the supplier and now has a new provider.

When decelerating, particularly on hills or into corners, there is an intermittent surge that occurs after the brake pedal has been depressed. It feels as though there is a release of the braking action, this is unsettling as if you are starting to approach a vehicle or pedestrian, this sudden "delay" of braking could result in contact with other objects. We have changed our driving behavior as a result of this. We reported this behavior at 5000 miles and there was not a solution until last year. We were not notified of this, and our vehicle is now out of warranty. Toyota has released as of last year a TSB related to programming and we are going to pay out of pocket $300 this week to get that TSB applied. This doesn't seem fair to consumers. You can see this is not an isolated problem here: [XXX] , a 2025 and 2024. In both of those vehicles, this behavior is not present. On May 18, 2025, 4:00pm, my wife was applying brakes to stop for a stop sign at a T-intersection. Suddenly, the car lurched forward and accelerated uncontrollably. Despite her attempts to stop, her vehicle crossed four traffic lanes, vaulted over curb, nosedived into turf, and then crashed through brick wall into a fire station. My wife was unable to brake and stop the forward progression of the vehicle. My wife steered to avoid hitting trees but could not avoid the brick of the fire station wall. The airbags did NOT deploy. My wife sustained back injuries and bruising of the front torso. Her right foot is bruised from pressuring the brake pedal. The vehicle sustained heavy damage as did the fire station.

When pulling out of a parking lot, the accelerator disables leaving the car unable to move in the middle of the rode for a few seconds. The happens intermittently and usually when there is traffic going both ways in the area. This has happened several times but has not been replicated with a dealership employee during test drive. After the accelerator disables, it will enable after a few seconds. It has left me sitting in the road with cars honking at me. Van is currently in the shop. Brake pedal was not deployed at the same time of acceleration.

When engaging the cruise control, the vehicle continued to accelerate even after I let off the button. During a trip with this vehicle on September 14, 2024, this vehicle did this twice. I have never driven another vehicle where this occurs. If this is supposed to be a "feature", it's unsafe and should not be legal. If it remains legal, there should at least be some kind of safety sticker.

I have a Toyota sienna 2022 Limited AWD, not been driving much recently. The last time I drove was 2 days ago but local around 2 miles. Today I tried opening my doors, and there is no response at all. At first I thought maybe my key fob is dead, so I grabbed my spare and tried, nothing. Pull both my physical keys out twist and turned, still nothing. Called Toyota Roadside service and they said in that case they will have to tow my van to the nearest dealer, so it seems Toyota is aware of this issue. What is the point of the physical keys if they can't open your doors when your battery is dead? What if my kids are stuck in the car????

After starting the van, an error pops up: "SRS Airbag System Malfunction Visit Your Dealer." This occurs a day after turning the car on and off and driving for several hours. The next day, it may come back on, or it may not. I have taken it to a dealership, and they stated it was a firmware update they completed. After they updated the warning light, it came back on less than a week later.

Our 2022 Toyota Sienna would not start after arriving at a doctor’s appointment. Contacted active ToyotaCare for towing - no one showed up. Called a friend to jump start van; consulted the owners’ manual and performed jump. Van started but could only be driven about 100 yds in parking lot. Van stopped; multiple error codes showed up when trying to jump start again. ToyotaCare didn’t call again or show up so we ended up paying for tow service to the dealership - Haley Toyota of Richmond. Dealership checked vehicle; concluded the battery had an internal weld problem but that the jump start was not performed properly even though the manual was followed. There are no jump start instructions in the engine compartment. We were given a Tech Bulletin instruction sheet with more accurate information. We were advised the Engine Control Unit was burned out, causing all the error codes and would not be covered under warranty because “we” did not perform the jump start correctly. The cost of repair is almost $1100.00 but the battery, which started all the issues, is covered under warranty. The Tech Instruction sheet indicates to me that Toyota was aware of the inadequate jump start instructions and failed to inform customers of the possible issues. The Tech Instruction sheet we were given is dated April 6, 2022 - one month BEFORE we purchased the vehicle. The sheet lists all Toyota hybrid models as affected.

The contact owns a 2022 Toyota Sienna Hybrid. The contact had ordered the vehicle with modifications due to being a disabled veteran from a specialty vehicle modification for disabled persons manufacturer. The contact had picked up the van from a dealer and was driving approximately 60 MPH when the smell of something burning filled the passenger cabin. The contact pulled over and called the dealer who took the vehicle back and determined that the fuel line was rubbing against the rear passenger side tire. The vehicle was repaired. The contact stated that also the "check battery" light had illuminated, the light blacks out the entire instrument cluster and cannot be reset. The contact stated that he took the vehicle back to the dealer and they could not repair the warning light failure. The dealer had diagnosed that the main batteries were getting overheated making the vehicle dangerous to drive. The manufacturer had not been informed of the failure. The contact stated that the van modification facility had modified the vehicle with all of the defects and sold the van with the defects to him. The failure mileage was approximately 9,000.

The contact stated that the release button on the crotch restraint buckle, where the two arm and chest belts buckled, were cracked into two pieces rendering the car seat unusable. The failure occurred inside of the vehicle. The child was not in the car seat at the time of the failure. The manufacturer was made aware of the failure. The car seat was not replaced.

Ever since a tire was changed on the vehicle, it has had a recurring error message that states there is a malfunction in the tire pressure monitor system. As such, I rarely know the status of my tire pressure as the system has the malfunction at least 50% of the time. The car has been in service 2x for this issue and they have had it for close to 30 days and driven it all over saying they needed to drive it to make the error go away. They have replaced the sensors on the tire, nothing has worked and the problem still occurs daily.

car seat gets loose easily and quickly with simple vibration down the road. example going over speed bumps and appropriate speed causes the seat to oscillate making seat belt strap loose that keeps the car seat in place. in one instance the strap was so loose i could not use the rotate function without the possibility of the car seat falling out of place. this is the second unit i have had with the issue manufacture is not moving to fix issue.

Over the past 18 months, this vehicle has exhibited the following actions approximately 30-40 times. When slowing the vehicle, with brake pressure applied, the vehicle reacts appropriately until, at the slow speed( approximately 5-10 mph) it suddenly accelerates or surges ahead for approximately 1-2 seconds. The has occurred while in the process of turning as well as moving straight ahead. I have taken the vehicle to the dealer of the original sale, Shepard's Toyota, Thomaston Maine, and they have attempted to replicate this action. They were unable to do so. The reported to me that they contacted Toyota engineering and was told, I paraphrase, that when the drive battery is fully charged and the dynamic braking system is still creating a charge, the current is directed to the drive motors, despite the fact that the brake is applied to slow and stop the vehicle. These events have occurred inn the middle of turns, when attempting to stop at crosswalks, and stopping prior to entering a traffic stream. If I had not continued to apply the brake, the vehicle would have, in several case, collided with objects.

When breaking at a stop sign or light, the vehicle occasionally feels like it surges forward and I'm accelerating instead of breaking. This causes me to push harder on the brakes, which then seem to mechanically engage and then stop the van. I've had the van for a year now and it happens a few times a month, especially if I breaking on an uphill incline.

Today in the morning while driving on normal road, suddenly Rear Cross Traffic alert and Blind spot Monitor became unavailable according to a message on the dash. This message persisted after restarting. In the afternoon another message gave these two company. The vehicle informed me, that the parking support brake is now unavailable as well. Funny is, that these messages are not documented in the alert history of the vehicle, although they still persist. Mileage of first occurrence was 9800. Another user in a sienna forum reported a similar problem and said this might be related to the poor design of the rear bumper that allows ingress and accumulation of dust, dirt and mud around the rear radar sensor. The vehicle was driven the week before the incident and about 700 mi before the incident on a dirt road (County road) in Michigan. This was pretty dusty and a bit muddy, but nothing a normal non-off-road vehicle couldn't stand.

I my Sienna had its 12 volt battery depleted over night with no reason. After it was jump started, literally every single piece of equipment was malfunctioning and producing an alert (hybrid system, brake pressure, RCTA, BSM, AWD,tire pressure and so on). After several restarts of the engine (about 15 min after running engine), all systems went to normal except hybrid drive system, that still asked for maintenance and the tire pressure sensor was still on alert. However I drove the vehicle to a dealership in Maine after the parking brake finally decided to get loose again. These guys told me, they had just the same problem with another 2022 sienna a week before. However, they reseted the errors, and everything worked fine. They had no explanation why two sienna in one week come to their lot with a depleted battery and as a consequence with a malfunctioning hybrid systen. That's very unsatisfying.

SRS Airbag warning light is in the dashboard. It is confirmed by the Toyota dealer a faulty radio head unit can not register the SRS airbag. In the event of collision, the SRS airbag will not be deployed, causing passengers at risk.

The padding in headrest is unglued and coming loose. My son has grabbed the head supports at times but I noticed it detached one day and not sitting in the correct position. I have tried to reattach but would require heavy duty glue and assistance with assembly. Car seat is available for inspection and I will include pictures. The car seat has not been inspected by police, manufacturer of others.

I got the software recall fix last week. Today is the first time I have had any issues. 2 time today the emergency braking activated. First time I was leaving McDonald’s parking lot. I waited for a car to pass then I took off to get on the road. The emergency braking system activated leaving me halfway in the parking lot and part way in the road. I thought that was weird. The a few minutes later I arrived at home and started backing into the driveway like I always do. As I was backing into the driveway a car started to pass on the road in front of me. The car emergency braking system activated leaving me part way in the road and driveway was the car passed. I have saved the ring camera footage of me arriving home in case it is needed.
