Spontaneous crack in the windshield without any external mechanixal force, such as rock, hale, or any other foreign body.

I bought this vehicle when new in 2021 and I started to see people complain about the quality of the windshield and how they were getting cracks from rocks on it. I’m [XXX] , and never had a windshield replaced in my life. After seeing people post this, I added windshield coverage to my car. Last year I had a rock hit my windshield and it cause a small star like crack. Right away I called the insurance company and was able to take care of it that day. Few days ago I hear another rock ding. Had nobody in front of me but think it was from the car on the side of me kicked it up. Some time passed and I saw the crack coming down the middle of the next morning it was even longer. From the top of the window down past my camera. I have an appointment for replacement this week. What I was reading seems to be true. Never in my life had I have this happen, had plenty of rocks hit my vehicles over the year. Never had one crack. This morning, I noticed a crack on the windshield of my 2021 Toyota Venza. The car was parked outside in the cold, and it seems like there’s some issue with the Toyota windshields, as it cracked for no apparent reason.

With no impact, our windshield cracked. I see many, many other owners of 2021 and later Toyota Venza have had this happen to them. This is clearly a large problem, so I'm hoping that by reporting our incident you will be encouraged to investigate further. Please let me know if you need any additional information, but the key points are that there has been no impact to the windshield that we are aware of, and the windshield cracked while driving. Thank you for your attention t this matter.

The rear tailgate is malfunctioning. I was told by dealer it is not covered under any warranty (even extended) this is something that has been reported by others and should be fixed. As well as having to repeatedly hit the lock or unlock buttons to actually work on the car before it works.

The rear tailgate squeaking is non stop. Dealer says not covered under warranty however MANY people online complaint of the same issue. This should be fixed by the manufacturer . Manu dealerships say it will cost upwards of 1 thousand dollars

I have an issues with the door on drivers side that won't unlock as much as I press the key fob it won't open its 2021 toyota Venza. I am physically disabled and I unable to stand for long. So that means I have to use my key to unlock my doors and I can't stand for too long I can tired and fall. And toyota knows of this issues and they refuse to fix and I don't think I should pay for something that they know has a defect. It's not fair.

Windshield cracked (apparently from the edge) during highway driving in normal conditions for no clear reason. The windshield has not yet been replaced but replacement is expected to be replaced in the week ending 1/11/25. Crack appears to start on the passenger side of middle, on the lower edge of the windshield. This area is behind the windshield wipers and is shielded by the hood. The crack extended upwards, then turned towards the drivers side. The Toyota dealer that sold the vehicle refused to inspect the damage because they do not work on glass. Toyota Brand Engagement, the name of the group that responded to a support request expressly stated that glass is not covered under warranty and there is no current recall on this vehicle. They finished their rejection of my request for help with a suggestion that my insurance should be engaged to repair the failure. I have not engaged my insurance carrier on this.

I just purchased as 2021 Toyota Venza Limited. The headlight on low beam cast a odd shaped shadow. It is very distracting and often makes you think something is in front of the car. The issue dissappears when headlight are switched to high beam. This is very unsafe and deserves a recall.

Head lights! Dangerous. They have blind-spots due to equipment blocking full light vision. This is my 2nd report to you. I have not gotten a response. I think this should be looked into immediately. I don't care to drive at night because of this blind-spot. Yes, I have reported this to Toyota and they could not do anything about it because they it from the new type of light bulbs. Please review, research and respond. Thank you.

HELP! I just purchased a pre-owned Toyota Venza 2021 Limited. I like the car but I went out tonight for the first time at dark and used the head lamps. Something's wrong! The low beams don't shine far enough down the road, there is a distinct cut off between dark and light, like you are looking under a shade. Plus there are dark spots in the light near the edge, especially on the left side and they follow the headlights while you are driving. It is so distracting and dangerous. I swerved a couple of times because I thought there was an animal in the road. The stark cut off between light and dark is like a straight line and the shadows are ridiculous! How could this design have passed safety tests! This makes me sea sick and scared bc I can't see more than 15 - 20' ahead of me! Very very dangerous! Please recall this stupid design!

You just cannot see with these headlights. I cannot drive the car at night. No crashes but we live in the country with no added light and these light don’t work. This is a proble since we bought the car a year ago.

Both left and right headlights on this car cast a dark "shadow" on the road in front of the car when driving at night. It only occurs when the headlights are on low beam, the high beam headlights are normal. Each headlight casts 2 shadows, for a total of 4, which can be seen about 100 feet in front of the car. They are very distracting at all times, but especially when the car is driven around a corner; it appears something is running across the road in front of the car. When I have pointed it out to passengers in the car, everyone remarks that it is very distracting and odd. I told the folks in the service department at the dealership I bought the car from about it, and they turned the headlights on and pulled the car up to a blank wall. I was with them and we could all clearly see the black shadows on the wall. It's like there are pieces of tape on the headlight lenses, but there isn't anything visible that would make these shadows. The service folks at the dealership just scratched their heads and said there was nothing they could do, and so the problem remains. This car is available for inspection at any time. There are no warning lamps or messages about the headlights. I bought this car new in May 2021 and noticed the problem right away. The dealership looked at it when we took it in for it's first oil change in February 2022, but they did not note it on the service receipt.

I am submitting this complain regarding my 2021 Toyota Venza, which has developed a serious defect involving the fuel system. Although my vehicle is fairly new, I began experiencing problems with the fuel door not opening properly. Initially, I could not access the fuel cap, which left me unable refuel in a timely manner. Soon after, I noticed additional fueling issues: while attempting to refuel, the gas pump would repeatedly shut off and stop the fueling process. After taking the vehicle for inspection, it was determined that the actuator responsible for controlling the fuel door and fueling system required complete replacement. I am concerned this defect represents more than just an inconvenience. Inability to open the fuel door or complete a fueling cycle can create dangerous situations - such as being stranded without fuel or blocking a fueling station. Additionally, if the actuator fails while in use, it could interfere with the proper venting of the fuel system. I have since learned that other Toyota Venza owners are experiencing the same problem with the actuator, suggesting this is not an isolated case but a potential widespread defect. The miles do not seem accurate as the tank should hold 14.5 gallons but always register less than 11 gallons when filled. Total miles in that vehicle should be around 552 but has been 479 the last few times. Toyota dealers says not issue. Complaint in with Toyota motor co. I have pictures of this issues which has been going on for a little while but just documented the issue. This is an issue because I don’t know the true mileage I wait til the warning light comes on to fill but that has not been accurate lately. There are no warning lights or anything that there may be an issue with that car.

I was turning onto traffic and I tried pressing the gas pedal and nothing happened. I was in instant shock as I feared I was gonna be hit then I pressed the gas pedal again and I was able to accelerate properly. This just recently happened but hasn't happened again since.

There have been 2 occasions when I have been trying to make a left turn and the vehicle fails to accelerate. The pedal is pressed all the way down and nothing happens. Once I released the pedal and pressed it again, the vehicle accelerated. The first occurrence happened a couple of months ago. I was making a left turn, pressed the gas pedal and nothing happened. Vehicle did roll forward putting me in traffic lanes putting me in danger of being struck. When I depressed the gas pedal again, it worked. I believe I was in Normal driving mode. The second occurrence happened on 4/5/2022. I was making a left turn, pressed the gas pedal and nothing happened. Vehicle did roll forward putting me in the traffic lanes and in danger of being struck. When I depressed the gas pedal again, it worked. I was in Sport driving mode at the time.

Fuel tank filling issue. Could not fill anywhere close Tank Capacity, dealer claims it has several gallons reserves. Driver does not know what Distance to Empty shows vs what is real, how much gas left in the tank. Supposed 560 Mile range, only showing 420 miles. Even after driving DTE 0 miles, user can hardly fill 11 Gallons. Do not know the reasons why, whether gas is in the tank but can't use, or can't fill the gas. This causes confusion of real Distance to empty and car might suddenly shutdown on highway or even damage the hybrid system. Seems known issue by Toyota, not acknowledging or fixing the issue. It has been observed over several tanks of refueling.

Indeed, I recently encountered a concerning issue with my 2021 Toyota Venza. The problem manifested in mid-July, during the process of preparing to enter my vehicle. To my dismay, the door refused to open. Despite my repeated attempts to use the keyless entry system, the door stubbornly remained closed. Consequently, I was compelled to resort to using my key fob, which presented its own challenges. While I would not typically express such frustration, the safety implications of this issue are significant and have become quite inconvenient for me. As a physically disabled individual, I rely on a scooter lift in the rear of my vehicle to facilitate my mobility. This lift allows me to walk to the front of the car while holding onto the side for stability, preventing potential falls. On the day in question, I slipped my hand behind the handle of the door to open it and it didn't work. When I attempted to activate the unlock button on my key fob, it failed to respond. Despite my repeated attempts to press the button at least ten times, it remained unresponsive. By this point, my legs had become increasingly weak, to the extent that I felt a slight loss of balance when using the key. As a physically disabled individual and of an advanced age, I am unable to stand for extended periods. Therefore, this issue has caused me considerable stress and emotional distress. Upon visiting my local Toyota dealer, I was informed that the repair would incur a cost of approximately $800, which I unfortunately lack the financial means to cover. Consequently, I contacted Toyota’s 800 number to inquire about any special financial hardship programs that might assist me in paying for this repair. Regrettably, I was informed that such programs were not available. This situation has become increasingly frustrating for me. I find it difficult to reconcile the fact that Toyota has a history of encountering numerous issues with various of its models.

Windshield crack development possibly from an unnoticed very small impact. On August 13, 2022 we noticed a crack from center passenger side bottom edge arcing up and towards the driver side. No impact was ever noticed, heard, or seen. Upon close inspection there appears to possibly be a small/micro impact point that should not result in the development of a crack. This affects safety due to visibility issues. Insurance claim filed but not inspected or repaired.

I was involved in a no fault accident when an SUV slammed into the back of my Toyota Venza at full speed and pushed me into the left guardrail of the bridge we were driving on. Neither the front airbags or the driver (left) side airbags deployed.

On 17th June 2023, while returning home on the WA-9/SR-9 NB, I stopped at the red light where WA-2 was crossing. My wife was sitting with me in the car. One Subaru rammed me from behind. This road has a speed limit of 55 Miles per hour and is a single-lane state highway. The rear was damaged heavily. I lost control of the vehicle, went into the SB lane, and saw a car approaching me from the opposite direction. I looked right and saw a little space between the car ahead of me and the vehicle ahead of it. I turned right without hitting those two cars and entered the roadside ditch damaging the front. In both cases, the airbag was not deployed, first the ramming from behind and then going into the ditch. Is this normal? Later my insurance declared my car as the "Total Loss."

My wife was in involved in a crash where a person ran a red light and she broadsided a truck which flipped over. The front end of our 2021 Toyota Venza was destroyed see photo which I uploaded. The airbags in our car did not deploy at all.

The contact owns a 2021 Toyota Venza. The contact stated that on two separate occasions the front windshield cracked. The contact stated that the first time he noticed the crack was ten days after the vehicle was purchased. The vehicle was taken to the dealer where the windshield was replaced however, the contact stated that the second time his wife was seated on the passenger side when she noticed that the windshield started to crack. The vehicle was taken to the dealer where the dealer had taken photos of the windshield and sent them to the manufacturer. The vehicle was not repaired. The manufacturer was notified of the failure and a case was opened and the contact was advised to contact the NHTSA Hotline. The failure mileage was approximately 11,500.

The contact owns a 2021 Toyota Venza. The contact stated while driving 30 MPH, an unidentified part fell from the vehicle ahead and struck and cracked the windshield. The vehicle was taken to the dealer where they replaced the front windshield. The contact stated that weeks later another similar event occurred. The windshield was replaced; however, the windshield was cracked by another foreign object. The dealer nor the manufacturer was not contacted or notified of the failure. The approximate failure mileage was 10.

Primary Control/Head Unit Failure - I can no longer access any of the electronic functions of the vehicle due to the computer screen failure. The back up camera works, but when the vehicle is in drive, it's nothing but a strobing light trying to connect. It's a safety concern most at night due to the constant strobe effect coming across the screen every 3 seconds. It draws your attention from the road and/or gives you the impression that something is coming to hit you from the passenger side. Yes, the vehicle has been inspected and diagnosed by Toyota. Toyota is unwilling to admit their head unit is faulty and want me, the consumer, to pay out of pocket for the parts and the labor. No, the failure happened without warning. One day it was working fine and the next it was frozen and then "dead" with this strobe effect.

The infotainment system has failed. It is the 12.5 screen-based system that enables the control of numerous features including air conditioning/heating, audio (all radio and satellite radio) and map/gps display. This system is the focus of a recall on this model car (newer versions) but the issue is the same for this year. Not having this display make navigation difficult, control of the cabin environment impossible. I wonder why this year vehicle was not included in the recall. I have ordered a replacement at a cost of $3,300. This began in May of 2025.

The Door lock actuators have all systematically failed on the car, so I can lock the doors manually, but anytime an electrical signal is sent to the lock (key fob, putting the car in drive, etc), the door locks do not operate. It is my understanding this is a known problem by Toyota across many SUV models. I have left my car unlocked on many occassions "thinking" it is locked, and I don't like driving with passengers in the car due to the safety issue of being able to open a door while the car is in motion when I "think" it is locked

VEHICLE SPEED CONTROL

As I attempted to park in a stall in the parking garage, I turned the steering wheel to the right and aligned the vehicle in preparation to back into the stall. In the split second that I took my foot off the accelerator to put it on the brake pedal in preparation to switch gears, the vehicle surged forward rapidly. I slammed on the brake as hard as I could, fearful that it would hit the car in the stall ahead. Maybe that's an indication that the Emergency Braking System failed, too. I don't know whether it stopped me or me slamming on the brake, but I was within 18 inches of the vehicle in the stall ahead of me. It truly scared me, which is what caused me to search for whether this was a known issue for 2021 Venzas and found that others have had this issue occur.

Seems like, This is common problems for all the Toyota Venza Car. After stopping at redlight, If we push the accelerator, It doesn't have a pickup speed as mentioned 7.1 sec from 0-60 miles. If, car is take on the 45/55 mile from the local road, it may hit by back side of the car. This is really concerning. Working with the dealer to solve the issue.

Intermittently my 2021 Toyota Venza will not accelerate. It feels like it loses power and will not accelerate. It started in March and has progressively gotten worse. It happens from a stop and from rolling acceleration. Toyota dealer Fred Haas Toyota said "they cannot duplicate the problem" even though I told them to go look online since this seems to be an ongoing problem. I do not feel safe in this vehicle since I cannot depend on it accelerating when I engage the fuel pedal.

On a trip out of town, after cruising for 3 hours on I-65, The drive power suddenly stopped and stuttered. The announcement screen started flashing a jumble of messages which only added to the hazard. First message was " Your feet are on the brake and accelerator." My feet were on the floor. We were on cruise control. I put on the emergency flashers to warn traffic not to run into us. The highway ran through a water filled swamp with the shoulder too narrow to be safe. I eased off on the power and pulled off the next exit which was a half mile away and coasted to a stop in a parking lot. It appeared to be overheating in the motor drive so we let it cool while locating a dealer. We struggled to get to the dealer, half mile at a time with pauses to cool. Dealer tech found the coolant for Motor drive was gone. Said there was a pinhole in the radiator. Said they did not have a replacement. Called all over. No replacement in US. Discovered there was a national back order for this part. A number of these radiators had failed. We were stranded, our trip aborted and 200 miles from home. The tech resorted to a JBWeld repair of the radiator & supplied us with 2 gallons of Toyota pink coolant and we made it back home. The local dealer reported the national back order had increased. We have had no further problem until today. Checking under the hood, we found that the motor drive coolant was down near the danger point. We filled it back up and will be checking it daily until it is repaired. This incident brought to light the following engineering deficiencies: No warning of loss of coolant. This is a critical system point of failure which could cause a fatal incident which would be attributed to "Loss of Control." False and distracting messages flashed on announcement screen, compounding the danger of sudden total and erratic loss of drive power in highway traffic. The Vehicle Diagnostic Report stated "Coolant Pump"A" Component Internal Failure." Obviously false also.

The contact owns a 2021 Toyota Venza. The contact stated that upon entering her garage at a low rate of speed, her brake pedal failed to operate as needed. The vehicle independently accelerated and crashed through the garage drywall and struck a water line. The air bags did not deploy upon impact. There were two unknown warning lights illuminated. The contact suffered back, head, and neck pain and sought medical attention from her doctor. A police report was filed. The contact's dry wall and water line were damaged as a result of the failure. The vehicle was towed to an independent mechanic where it remained in their possession and had yet to be repaired. The manufacturer was notified of the failure and an inspection was performed on the vehicle; however, the results of the inspection were unknown. The failure mileage was approximately 18,000.

I was at an intersection. Right turn lane, waiting for a green or a hole in traffic to turn right. The light turned green and I lifted up on the brake, turned the wheel to the right, and tried to accelerate into the turn. The car started rolling, and I heard an electric noise (hybrid car), but the car did not accelerate normally. I applied more 'gas' on the pedal, and nothing happened; the car continued to lightly roll. Usually if you press the gas pedal past a point, it will kick in the gas engine with the electric drivetrain, but this did not occur. After approximately 2 seconds, the pedal felt 'normal' and the electric drivetrain accelerated normally. I was OK because traffic had turned green, but if I had tried to find an opening in traffic and turned on the red, after a complete stop, having the acceleration pedal not correctly functioning for 2 seconds would have been highly problematic and not safe at all. No warning light or beeps. The only noise was an odd electrical sound which was 'not' the normal sound that I hear from the electric drivetrain. I have not told the dealership yet, but I will during my next appointment. Sometimes when I'm driving the Toyota Venza 2021 downhill and press the brakes they freeze and sound weird without control to stop the car. I think is very dangerous. I reported two times in the dealership but nothing happened.

After Traction Control recall firmware update was completed, my vehicle now turns on the parking break randomly when coming to a stop while still in drive. The vehicle says time for maitenance with aprox 5,500 miles before service required. Remote services no longer connecting. The firmware update to correct traction control not turning on automatically at vehicle startup has caused other electrical features to fail/malfunction.

Pulling into a parking spot (under 5 mpg), brakes did not engage. Car drove up onto the sidewalk. Had to put car into reverse to avoid hitting the building. While in reverse, brakes would not engage as well had to turn off the vehicle to stop it after it hit the curb 20ft behind the vehicle. Had vehicle towed to the dealership. They could see where the brakes were engaged 3x, but could not replicate the issue. Although there was snow, ground was not icy or slippery.

On three separate occassions, when applying the gas pedal, the car did not immediately accelerate. One time I was pulling out across two lanes of traffic and the accelerator was to the floor and nothing happened for about 5 seconds. The last time happened today, 11/3/23, leaving a gas station entering traffic. I hit the gas pedal and it was 3/4 the way down before the car moved which, once again, could have caused a accident pulling out into traffic. I took the car into Jones Junction with the complaint and they could not find any issues after allegedly checking the fuel pump. Online other individuals are having the same problem.

Twice in the (almost a)year I’ve had the Venza I have gone to accelerate from a stop sign and the car started to go then the gas pedal went all the way to floor and nothing happened - then when I tried again the car accelerated as expected. The first time (@ approx 4000 miles) I was turning left with oncoming traffic (of semi truck plus others) coming and had it happened a few seconds later - I would have been in a serious accident. Second time was just over 10000 miles on 1/12/2021

SERVICE BRAKES,FORWARD COLLISION AVOIDANCE

I was on the highway, no ice on the road, in a construction zone, going about 50mph. The cars in front of me stopped completely, and the light to warn me to "Brake" came on the dashboard to engage the brakes, but the emergency braking system failed to engage the brakes and slow down the car. I also pushed on the brakes, but nothing happened, the car did not seem to slow down at all, like I had no brakes. I kept going and crashed into the vehicle in front of me. It's like the brakes did not work and the automatic braking did not work either. There was a complete system brake failure. Also, the air bags did not deploy, which I'm surprised with so much front end damage to my car. The brakes also did this last year, and I collided with a vehicle the same way, but there was ice on the road, so at that time, I attributed it to the icy road. Now since these two accidents are so similar, I am concerned about a defect in the brakes and automatic emergency braking system. I am also worried to own a vehicle with the Automatic Emergency Braking System. I am having the vehicle towed to the dealership to have the brakes looked at.

2021 Toyota Venza while being driven while merging onto the highway, the vehicle lost all braking power causing my wife to almost cash into the car in front of her. The vehicle was moving between 50 to 55 miles per hour. Once the vehicle was pulled over, the dash was displaying these error messages: Pre-Collision System Malfunction; Braking Power Low/Visit your Dealer; Check Engine; and Secondary Collision Brake System Malfunction. This caused my wife to be stranded on the side of the highway. The vehicle has 24,811 miles on it. The vehicle had to be towed to the Toyota dealership. This is the second time this happened. The first time this happened was about 20 months ago when I had just bought the vehicle new and put about 400 miles on it. The first time the dealership said they could not find anything wrong with it.

I was pulling out from a stop sign to cross a four lane intersection. I attempted to pass between two cars, but the emergency brake system activated, presumably because the first car was still in front of me, even though it would have well passed by the time I got there. Because of the emergency braking, I was left stuck across two lanes of traffic with cars coming toward me and no way to get out of the way. This has happened twice in 800 miles on the car. I live on a very busy intersection and it took me a couple of months to get comfortable driving in this congestion. I think the car simply isn't designed for that level of traffic, but the way that it's reacting is terrifying. Consumer stated issue continues to occur even when the pre-collision system is disabled.

multiple times PCS (pre-collision warning system) would go off only in city driving when i was passing a car that had turned right (and just completing turn)-approximately 45 degrees to from right of my car. The dealer said my car needed front wheel alignment and said that had been the problem. Using the computer analysis system and since i was not getting a warning light saying PCS was not working, i was told it was fine. On a recently completed road trip out of town, at 70 miles an hour, PCS through us into hard break due to a car that had just entered exit ramp-45 degrees to my car. Almost caused multicar incident. Can not get further help from dealership since computer analysis not showing issue and no response from Toyoto NA. Have since just turned system fully off.

This report covers 2 events for the same problem. The first occurred in the spring of 2025 and I did not document the date or time. The second one occurred on November 10, 2025 @ about 10:15 a.m. (1) Last spring my wife was driving the Venza up the street to our residence going a slow rate of speed. As she approached the house, she took her foot off on the gas and coasted toward the left curb so she could back into our driveway. At that point, car suddenly surged forward and accelerated toward the curb. My wife rapidly applied the brake and averted any problem. (2) The second event occurred on November 10, 2025 @ 10:15 a.m. My wife was starting to park at a parking lot in the Tanasbourne Town Center. She was going to back into a parking spot so she drove forward going slightly left. Then, with the car still in drive, the car surged forward. She quickly stepped on the brake and the car stopped abruptly. (She was angling the car and was about ready to stop and put the car in reverse when this happened) Again, with her quick response braking, she was able to avert hitting person or thing. As to which component or system failed, there appears to be a random malfunction of sensors or perhaps the CPU...something that is calling for full throttle when the gas petal is not pushed. The safety risk is possible harm to the driver, outside property, or the worse situation would be harm to pedestrians that might be in the way of the car. We have not been able to reproduce the problem. The same maneuvers have been repeated 100's of times without incident. As of this writing, the vehicle is being taken to the Toyota dealer to have the oil changed and at that time they will be asked to do a thorough inspection of the systems that might cause this, check for any recorded malfunctions, etc. To the best of our knowledge, there were no warning lamps or messages that appeared.

The infotainment system is not consistently working. It is the 12.5 screen-based system that enables the control of numerous features including air conditioning/heating, audio (all radio and satellite radio), backup camera feed, and map/gps display. I see that there is a recall for this system for some Toyota models, however I have not yet received a notification, and I am assuming that it is because my car is a 2021 model. Not having this display makes navigation difficult (as the screen will just turn off suddenly) and control of the cabin environment impossible (i.e. I struggle turning on the AC/Heat as I don't know what the temperature is set to). Why would my vehicle not be included in the recall? What can be done to get this fixed? I view this as a safey concern as all vehicle safety notifications are posted on the screen (which won't work), all of the systems that are controlled from the screen are not available, and the back up camera display won't work at times.

The contact owns a 2021 Toyota Venza. The contact stated while attempting to start the vehicle, several unknown warning lights illuminated. A dealer was contacted. The contact stated that during the first failure, the vehicle was towed to the dealer to be diagnosed, however the diagnosis was unknown. The contact stated that during the second failure, the steering wheel warning light illuminated, the steering wheel became difficult to turn, and the accelerator and brake pedals seized. The contact turned off and restarted the vehicle, and thirty seconds later, the vehicle operated as intended. The contact stated that during the third failure, the contact became aware that the fuel gauge was not operating properly. After refueling the vehicle, the fuel gauge warning light illuminated. The vehicle was taken back to the dealer to be diagnosed, and the contact was informed that the battery needed to be replaced. The battery was replaced, and the vehicle was repaired. The contact stated that the buttons on her steering wheel and the infotainment system also failed to function as designed. The manufacturer was notified of the failure but offered no assistance. The approximate failure mileage was 57,346.

Vehicle at operating temperature, running, in gear and at a stop. Making a left turn into traffic. I release the brake and apply the accelerator, the car rolls out into traffic but no power, I press more and more until it is 100% throttle, all the way to the floor. Then after about 5 seconds the vehicle moves forward. Complete loss of response/power for 5 seconds. Thankfully no traffic coming. This happens a second time with the same circumstances, different location. Press the accelerator, no response, sudden loss of all power, then after 5 seconds it responds. This time there is traffic coming to my driver's side at 50 MPH, I was trying to press harder but it wouldn't respond. There were no warning lights, MIL, sounds or messages. Toyota wants $169.99/hr plus tax to investigate.

When accelerating from a stopped position, the car fails to go forward. This incident has occurred six times and often seems to be at the worst times, such as pulling into a busy road from a stop sign. The car will start to accelerate and stop with no response from the gas pedal for 3-5 seconds. There have almost been a couple of accidents with this vehicle because of this issue. No check engine light or any light at all came on in the car when this incident occurred. This vehicle has been taking to the dealership and the problem has been mentioned to them, however with no codes to read for the OBDII they claim there is nothing they can do about it.

The crash occurred at 2:50 PM on January 10, 2024 while pulling our 2021 Toyota Venza into a diagonal parking space on a slight upgrade. The vehicle surged over the curb, across the sidewalk and into a brick wall with gas pipelines attached to it. As this was happening, the driver yelled, “I don’t know what’s happening.” The car then rebounded onto the sidewalk and was turned off quickly because of a gas smell. We exited the vehicle and tried to keep people away from the scene because of the smell. The passenger had great difficulty getting out of the car because the passenger side door was blocked by a tree in the median. The Fire Department cleared the area, the police interviewed both the driver and the passenger and the gas company arrived to repair the meters and pipes. No citation was issued. During a call on January 12, 2024 to cancel a pre-paid service agreement, we were referred to the Toyota Brand Engagement Center who interviewed us before forwarding information on to the Toyota Liability Team. Toyota sent a contractual employee to download the information from the vehicle’s computer modules for further analysis by Toyota. On February 20, 2024, Toyota issued a report that stated the vehicle had no defects related to the accident and that the accelerator pedal had been fully depressed prior to impact. The report did not address other information that was provided to individuals investigating the incident, including information about the incident itself, information about a delayed acceleration event several days prior to the incident and notification from the local Toyota dealer the next day that a check engine light had come on around 2:50 the day of the incident followed a minute later by alarms indicating “airbag failure to deploy” and “hybrid system failure.” The report also did not address the possibility of a problem with the throttle system, specifically the accelerator sensor.

It's its been about 4 times for myself and twice for my husband. When pulling out of parking lot onto busy street making a right hand turn, the car failed to accelerate so that I can merge onto the street. I almost got reareded twice because of this not being able to get up to speed. I have taken it to the dealer twice. The first time they told me there was a computer recall so he would do the update and it if it happened after that to bring it in. We'll it's happened many more times. And the dealer can't tell me what it is.

I was parking my car in a parking lot very slowly (1 mile/hr, or less). My foot was on the break paddle, and I just turned into a parking spot. Suddenly, I heard huge acceleration noise and the car surged forward so rapidly, so powerfully on its own, and totally lost control. Then the break did not work at all. There are bushes in front of the parking spot and it is on top of a retaining wall with side walk road below. The crazy lost controlled car run through the bushes and jumped down from the top of the retaining way onto the side walk road. It hit the railing of the side road and then slowed down. I then gained the control of the car as the break back to work. The car is badly damaged and several parts falling down when it jump down the retaining wall. It was so terrified and I though I will die. And if someone was walk in the side road at that moment, they will be killed. I am still shaking when think about what was happen. Luckily, no one was there. I bought the car in Jan. 2022, brand new, only 8 miles used. I saw a "MALFUNCTION" on the dashboard and the air bags did not deploy. I injured my back, neck and right arm. My car that only has 9K miles on it, was completely totaled. Toyota's Product Liability Dept - 3rd party engineers inspected the electronic data recorder. and car I'm awaiting the final report from Toyota.

A small crack appeared on the windshield driver side of vehicle. I have no recollection of any items or debris hitting the car and damaging it. I then searched online and found there have been many registered instances of this specific car model windshield (2021 and newer) either easily cracking or seemingly cracking under no impact.

- Vehicle was operating as normal. Slowed down to a complete stop as normal. When attempting to accelerate from stop, vehicle had no throttle response and slowly idled into the intersection. Would press pedal down and nothing would happen. About 5 seconds later, vehicle gained throttle response and took off quickly. Unable to duplicate issue at this time, appears to be intermittent. - Safety was risked due to inability to take off from a stop and being stuck in the intersection for a brief period of time. - Unable to duplicate problem for dealer to confirm. - Called the dealer and they stated they needed to do a diagnostic, but it needed to be in a failed state. It is not currently in a failed state. If it was in a failed state, I’m unsure how I would get it to the dealer for inspection as the issue is lack of throttle. - No warning lamps or other symptoms prior to the failure.

On May 30, 2023 at approx. 11:30am I was leaving the car wash located at 2330 Olympic Hwy. N. Shelton Washington. The automatic bar pushed me through the car wash to the end where it left you. There was a slight grade downwards to the highway (Olympic Hwy. N.) I touched the gas peddle slightly and the car surged forward across the SB lane where a car traveling SB had to slam on its brakes to avoid a collision. My car continued across to the NB lane where it struck a vehicle in the drivers side rear quarter panel. I had slammed on the brakes but not quick enough to avoid the collision. The brake was slow to respond when I applied them. I'm thinking what may have happened as this is a gas electric hybrid that the gas engine kicked in instead of the battery when I gently applied the gas peddle which caused it to surge forward.

On numerous equations, when I press on the gas pedal the car does not react from either the hybrid battery or from the engine. I will depress the pedal and the car pauses for a few seconds before it moves forward. This has happened multiple times at different mileages but until this report, I did not think much of it since it was not in a near collision situation. On two occasions as I am pulling into traffic I will put my foot on the gas and since I get no reaction, the cars coming towards me have been very close to hitting me. One would have been a T bone incident where I would have been hit and another would have been a rear ended situation. Both times, the cars coming towards me had to brake rapidly. I have taken the issue to the dealer to examine. There have been no warning lamps and it does not happen consistently.

The car has a new type of sunroof (Stargaze) that turns clear or dark. It is operated by the electrical system. Mine shorted out and makes a buzzing sound if turned on. This same problem has been reported by many others with this sunroof. It is a design defect that is prevalent in the Venza. The Toyota Venza is a low volume vehicle and a very small percentage are equipped with this sunroof, but l have read about many others experiencing this issue in Toyota forums online. The pictures show the roof in clear mode and frosted mode with the shorted out burnt portion.

Forward collision avoidance; automatic emergency braking; adaptive cruise control; I pulled into a straight-ahead handicapped parking slot to park, going about 1-3mph . There was a handicapped sign in front of me on a newly installed steel pole. I pulled in within inches of the pole. When I tried to engage the brakes, the braking system failed to engage the brakes and slow down the car. The car instead accelerated and would have broken through a store front had the pole not stopped it. I also pushed on the brakes, but nothing happened, the car did not seem to slow down at all, like I had no brakes. It's like the brakes did not work and the automatic braking did not work either. There was a complete system brake failure. Also, the air bags did not deploy, which I'm surprised with so much front end damage to my car. So, in a panic to stop the forward acceleration, I put the car in reverse; after which the vehicle accelerated in reverse out of my control and crashed into a 3-foot concrete wall and another vehicle next to the wall. This crash stopped the vehicle. I had no control over speed of the acceleration, steering of the vehicle, or stopping. I am concerned about a defect in the brakes and the PKBS braking system. I could have been killed or backed into another person or car killing someone else. The car has been repaired. I did receive a msg about the raider cruise system being on days prior; I didn't know what it meant; it went away and I did not pursue why cruise control was on because I didn't engage it. I am seeking inspection by Toyota and my AAA Insurance rep.

Extremely loud seatbelt warning chimes created a hazardous situation by causing a severe distraction, preventing ability to hear traffic, and having no way to correct the situation. I had a near miss accident while trying to figure out what was wrong, and was forced to make a very dangerous stop on the side of a highway to correct the problem. The seatbelt warning chime was improperly triggered by a fault in Toyotas programming. A child briefly plugged their seatbelt into the 'wrong' receptacle in the rear seat. They quickly realized it was the wrong one and plugged it into the correct receptacle. After visually and verbally checking with my child that they were properly restrained, we left a parking spot at a shopping center and got almost immediately onto a highway. The car improperly deduced there was a child in the middle seat because of the brief failed attempt of buckling into the wrong buckle. And since the buckle was not buckled, it assumed there was an unbuckled child now in the middle rear seat. After some testing, it seems the only way to clear this alarm is to fully stop the vehicle and open one of the rear doors. This is how Toyota decided to 'clear' the status of someone leaving the car. The seatbelt warning system does not begin it's chimes until the vehicle has reached a certain speed, which did not occur until I was on the highway. The extremely loud chime with no obvious reason created a panic situation. The child was unable to help in the rear because they did not understand, and would not have been able to safely reach over and buckle the unoccupied seat anyway. While focused on resolving the intrusive noise, we had a near miss situation. I was unable to hear any of the safety system warnings over the chimes. Ultimately I had to perform a dangerous stop on a 65mph highway to get out, and figure out why the car thought there was someone in the rear seat that wasn't there.

The contact stated that while she was driving in traffic and slowing down to 10 MPH with her foot on the brake pedal, the vehicle unintendedly accelerated with a jolt. The contact stated no warning light was illuminated. The unintended acceleration had occurred on several occasions. The contact had taken the vehicle to a local dealer who diagnosed the vehicle and stated that the failure was caused by driver error. The dealer stated that the vehicle information showed that both the brake pedal and accelerator pedal had been depressed simultaneously. The vehicle was not repaired. The manufacturer had been informed of the failure. The failure mileage was approximately 15,000.

I couldn't stop the car and I crashed head on into a two story garage. I saw a "MALFUNCTION" on the dashboard and the air bags did not deploy. I injured my back, neck and right arm. My new car that only has 9K miles on it was completely totaled. Vehicle is scheduled to be inspected by Toyota's Product Liability Department - 3rd Party Engineer. The only other time I was concerned about the vehicle was about 5 months ago, I was at a stop sign and when I pushed on the gas, the car felt like it lost acceleration power, but then it was fine after that so I didn't think anything of it. Vehicle is available for inspection for a very limited time (probably within 1-2 weeks of this post), after Toyota inspects it, it will be going to the salvage yard. It was raining, I was driving in the far left lane next to the center divider. I noticed the car in front of me drive through a significant collection of water that was the width of the entire lane. I was unable to switch lanes, so I slowed down and went through the water as well. As soon as my Venza came to the collection of water, my steering wheel locked up. I was unable to direct the car in the opposite direction of the pull. I was not able to move the steering wheel at all. The car continued to go in it’s own direction, despite me pumping the brakes. The Venza spun out, hit another car before it crashed into the concrete center divider. As a result I smashed my head on the driver’s side window. My driver’s side air bags NEVER deployed. No air bags in the entire car deployed. After hitting the center divider my car ended up in the car right lane. My car was towed from the scene of the accident, it was not drivable. The insurance adjuster originally totaled the car, then they changed their minds and said the car can be fixed. It’s been 3 months and my car is still in the shop. There were no warning lights, it was a brand new car, I had it less than 6 months. I could have died, other people could have died. If the steering wheel didn’t lock up I could have easily maneuvered through the water, like the car did I front of me and like I have done many times in the past with my previous vehicles. If the driver’s side air bag deployed I wouldn’t have suffered such a bad concussion requiring medical attention, neurologist, loss of work and loss of pay.

The contact stated the bottom of the driver seat was not aligned properly with the steering column. There were no warning lights illuminated. There were pains in the driver's body that caused it to be pinned to the right while driving. The contact received medical treatments for the pains. The vehicle was not diagnosed and it was not repaired. The manufacturer was notified of the failure. The approximate failure mileage was 40,000.

as I attempted to turn left in the parking lot of a strip mall with several restaurants, my engine suddenly revved and accelerated and despite pumping the brakes several times I could not slow it down or stop it. As a result I slammed into a concrete berm, kept going and crashed through the glass doors of a coffee shop/restaurant, continued on and the car did not stop until it hit a metal counter. Fortunately I was not hurt, nor did I kill anyone, although I was told two workers went to the hospital with bruises and lacerations on their legs. Since I was unable to stop the car, not only was my life at risk, but anyone who crossed my path or who was in the coffee shop could have been crushed. The police came, made a report and had my car towed to their impound lot. The problem has not been reproduced or confirmed by a dealer or independent service center as of yet. . My insurer will have the car towed to a preferred repairer and they will inspect the car to determine whether it's repairable. I have asked them to check what made the car suddenly accelerate. I was informed at the police lot that they expect the car will be towed and inspected on Monday, 7/18/22. The accident occurred on 7/14/22. The vehicle has not yet been inspected by the manufacturer, police, insurance representative or others. There were no warning lamps, messages or other symptoms of the problem prior to the failure. The vehicle is available for inspection upon request.

While steadily accelerating on a freeway onramp, the vehicle suddenly accelerated, surged and gained speed rapidly without my control. I took action by braking and slowing the vehicle to a safe freeway speed. Thankfully no one in my vehicle was hurt and no other vehicle was near during the incident. There were no warning messages. The first time I experienced this unintended acceleration was the first month when this brand new vehicle was purchased from a local Toyota dealer. I was turning into my driveway from a residential street , the car suddenly accelerated while I was in the middle of turn. I immediately/emergently stopped by stepping on the brake, I could have crashed into the power pole if I did not stop. Because this was a new car, I convinced myself I was unfamiliar with its controls. But since the second occurrence, I am greatly concerned about this random "unintended acceleration". The Toyota Venza had been dropped off at our local Toyota dealer for inspection and hopefully repair.

2021 Toyota Venza Steering assist feature creates a safety hazard. The steering assist works with the cruise control and lane departure. When on, it appears that the steering assist picks up its position or centers the vehicle in the lane from the nearest white line on the right side. It positions the vehicle approximately 12 to 18 inches from the right line position thus positioning the vehicle slightly right of the center of the lane. One is forced to fight the pull of the wheel to maintain a center lane position. This Steering Assist function creates the following problems: 1.Off-center to the right can make it too close for vehicles traveling to the vehicles right. Also, it does not adjust for vehicles traveling too close or possibly swerve on your right side. It will maintain its position regardless of events alongside of you. It receives its bearings from only the white line. This condition is more severe when traveling in a left lane on a multiple lane road with a dotted line. 2.Because the steering assist picks up its orientation from a right-side line; when the line is out of position or not available, the car will automatically move right until it finds something to coordinate the cars position. Without something fixed like a line or curb it will take the car off of the road. When approaching an off-ramp, the right solid road barrier line, usually veers right off the highway and will follow the direction of the off-ramp. Under this condition I have discovered that the steering assist feature will actually take me off of the highway onto the off-ramp and alongside the right-most solid line wherever it goes. I feel that NHTSA still should examine any issue on a car that could make its operation unsafe. Furthermore, it appears that NHTSA has not performed any crash-worthiness testing including weight and roll-over. Why has safety testing and crash worthiness not been performed on this vehicle? Lastly; why doesn’t it have a standard NHTSA door label?

The dealer informed the contact that the failure was related to a TSB. The vehicle was repaired. The manufacturer was made aware of the failure and informed the contact that the owner needed to report the failure to the dealer for the TSB to be accessed. The approximate failure mileage was 5,500.
