The contact owns a 2001 Ford F-250 equipped with Toyo tires, Tire Line: HT Open Country, Tire Size: LT265/75/R16, DOT Number: CXLTCAK4815. The contact stated that while driving at an undisclosed speed, the vehicle was shaking and felt like the front tires were bouncing. The contact veered to the side of the road, where he noticed that the tires were deformed. The contact continued to drive. The vehicle was taken to the local dealer where it was diagnosed that the tread belt was separating. The front tires were replaced; however, the failure reoccurred with the rear tires. The vehicle was taken to an independent tire mechanic where the rear tires were replaced under an unknown recall. The contact stated that the failure reoccurred on the front tires. The dealer was notified of the failure. The manufacturer was notified of the failure but did not provide any assistance. The tires were not replaced. The tires were replacement equipment. The tire failure mileage was 30,000. The vehicle failure mileage was approximately 230,000.

TL* THE CONTACT OWNS A 2001 FORD F-250 EQUIPPED WITH MICKEY THOMPSON/BAJA ATZ TIRES, TIRE SIZES 35 * 12.50 R20. THE CONTACT STATED THAT WHILE TRAVELING 70 MPH, THE RIGHT FRONT PASSENGER TIRE BLEW OUT AND CAUSED THE CONTACT TO RUN OFF THE SIDE OF THE ROAD. THE CONTACT WAS NOT INJURED. THE CONTACT STATED THE BELT SEPARATED FROM THE TIRE. THE VEHICLE WAS TOWED TO A REPAIR SHOP. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE WHO DID NOT OFFER ANY ASSISTANCE SINCE THE VEHICLE WAS NOT INCLUDED IN NHTSA RECALL CAMPAIGN ID NUMBER:06T024000(TIRES:TREAD/BELT) THE FAILURE AND CURRENT MILEAGE WAS 2360,000. UPDATED 11/19/12 *BF THE CONSUMER STATED THE DISCOUNT TIRE MANAGER REFUSED TO REPLACE THE TIRE OR GIVE A DOCUMENT STATING THERE WAS NO FAILURE WITH THE TIRE. THE MANAGER DID BALANCE THE TIRE BUT WOULD NOT PRINT A RECEIPT FOR THE CUSTOMER. ON 10/15/2012 THE CONSUMER FILED A REPORT WITH COOPER TIRE OVER THE PHONE AND WAS TOLD A PACKAGE WOULD BE SENT OUT. MY THIRD UNIROYAL LIBERATOR TIRE HAS FAILED IN THE SAME MANNER AS THE TWO BEFORE. THIS IS MY THIRD COMPLAINT CONCERNING THESE DEFECTIVE TIRES. THESE TIRES ALL FAILED IN THE SAME EXACT MANNER (TREAD/TIRE SEPARATION) AND ALL WITHIN WEEKS OR MONTHS OF EACH OTHER. THE TREAD PULLED LOOSE FROM THE CARCASS,CAUSING A BUMP-IMBALANCE. I RECOGNIZED THE FAILURE AND WAS ABLE TO DRIVE HOME WHERE I CHANGED TO A SPARE. I LATER REPLACED THIS TIRE. THIS TIRE ONLY HAVE ABOUT 37,000 MILES. WALMART,WHERE I PURCHASED THE TIRES,REFUSES TO GIVE ANY TYPE OF WARRANTY OR TIRE CREDIT AND NO LONGER CARRY THIS TIRE. THIS TIRE HAD PLENTY OF TREAD AND WAS WELL WITHIN ITS LIFE SPAN. THIS LIBERATOR TIRE SHOULD BE RECALLED AND PULLED OFF THE MARKET BECAUSE OF ITS CONSISTENT FAILURE RATE. I HAVE SENT COMPLAINTS TO UNIROYAL WITH NO RESPONSE. CONCERNING TREAD SEPARATION ON UNIROYAL LIBERATOR A/T LT 265/75R16 TIRE PURCHASED FROM WALMART ON 092007,APPROX. 36,000 MILES ON TIRE. USED ON 2001 FORD F-250,DIESEL, REAR WHEEL DRIVE. THE TIRE HAS DOT MARKING AS FOLLOWS: DOT BFW8 C62U 0807, E RANGE RATED. ON MARCH 23 2011 WHILE DRIVING AT 65 MPH I FELT BAD VIBRATION AND WHEN SAFE, PULLED OFF ROADWAY TO FIND THE RIGHT REAR TIRE TO HAVE A BULGE ON TREAD AND SIDE WALL DISTORTION. I HAVE HAD THIS SAME FAILURE FROM COOPER TIRES WHICH CAUSED LOSS OF CONTROL AND DESTRUCTION TO QUARTER PANEL. I CHANGED THE TIRE AND LATER (03302011) CONTACTED UNIROYAL WHO OPENED A FILE BASED ON MY PHONE NUMBER([XXX]). UNIROYAL CLAIMS NO RECALL NOR DO THEY ACKNOWLEDGE ANY SIMILAR COMPLAINTS ON THIS TIRE. I HAVE 3 OF THESE TIRES AND HAVE SAFETY CONCERNS ON THE 2-REMAINING TIRES IN USE. POWER TRAIN

When im in traffic, starting out driving or merging between intersections my truck decides to either hard shift or not shift at all. Several times ive had to lay on my horn in an attempt to warn oncoming vehicles that i have no power and cannot accelerate and several times it has nearly caused an accident. I purchased the truck used and have yet to take it into the dealership to have them inspect the issue though from what i’ve read thus far is that ford calls this problem “normal operation” and has been less than understanding when people attempt to have this obvious defect resolved. There are no warning lights that come on when this happens. I can have my truck inspected by whomever, whenever need be. I will also be returning to the dealer to use my warranty to help deal with this issue. The date this most recently happened causing a near miss was today, though there has been an occasional but recurring situation almost weekly.

PARKED VEHICLE AT MAIN POST OFFICE , REMOVED KEYS, I KNOW THIS BECAUSE IT HAS MY POBOX KEY ON KEY RING. I HOPPED OUT AND STATED TO WALK OFF, THEN I GLANCED A VIEW TO THE RIGHT, AND NOTICED OR THOUGHT I NOTICED, MY TRUCK ROLLING BACKWARDS. IT WAS !!! I CHASED MY TRUCK AND CAUGHT UP TO IT, AND ATTEMPTED HOP IN AND ENGAGE FOOT BRAKE. I AM TO FAT AND SLOW TO DO THAT, SO I THOUGHT QUICKLY, , AH HA, THE EMERGENCY BRAKE . I REACHED DOWN AND LET THE TRUCK ROLL THE ER BRAKE TO MY HAND, AND ENGAGE IT I CALLED THE FORD DEALER, NO KNOW ISSUES LIKE THIS HAS HAPPENED, SO I WAS TOLD. DT*: THE CONTACT STATED THE TRANSMISSION BECAME STUCK IN 1ST GEAR AT TIMES AND WOULD NOT UPSHIFT UNLESS THE ACCELERATOR PEDAL WAS RELEASE. THIS OCCURRED UNDER NORMAL ROAD CONDITIONS. THE NATURE OF THE FAILURE WAS PERMANENT GRADUALLY WORSENED. THE SERVICE DEALER WAS NOT ABLE TO DETERMINE THE NATURE OF THE FAILURE.

THE CLUTCH CYLINDER FAILED AND ENGAGED THE CLUTCH. THIS MADE THE VEHICLE TAKE OFF, SOMETIMES SENDING THE CONSUMER INTO THE INTERSECTION OF TRAFFIC. THE CYLINDER HAS BEEN REPLACED TWICE AND IT CONTINUES TO PERFORM THE SAME WAY. *JB

WHILE DRIVING 55 MPH, HEARD A LOAD BANG AND THEN A STEADY POPPING SOUND. AFTER PULLING OFF ROAD TO CHECK PROBLEM OPEN THE HOOD TO SEE THE COIL EXPLODED AND ARCHING AGAINST FUEL RAIL, AND SPARK PLUG ON THE VALVE COVER. GOT THE TRUCK HOME STARTED RESEARCHING THIS ISSUE AND I SEE THIS IS A COMMON PROBLEM YET FORD SAYS IT ISN'T. MY BEST EDUCATED REPAIR SOLUTION IS CAL VAN 38900 FORD 5.4L SPARK PLUG PORT REPAIR KIT AT A COST OF 250 . IT TOOK A DAY AND A HALF TO DO A COMPLETE REPAIR. SEEMS TO BE WORKING BUT FORD HAS A PROBLEM AND SHOULD DO SOMETHING FOR THE OWNERS SHOULD IT HAPPEN. THEY CANT SAY THIS IS A TORQUE ISSUE THIS ARE THE FACTORY PLUGS AND COIL. DT: THE CONTACT STATED THE POWER STEERING PULLEY FELL OFF THE ENGINE WHILE DRIVING. THIS CAUSED THE CONTACT TO LOSE CONTROL OF THE VEHICLE AND RESULTED IN LOSS OF POWER STEERING AND BRAKES. UPON INSPECTION, THE PULLEY APPEARED TO BE GLUED ON INSTEAD OF BOLTED. THE CONTACT BELIEVED THERE WAS A DEFECT IN THE WAY THE PULLEY WAS ATTACHED TO THE SHAFT. NHTSA CAMPAIGN ID NUMBER 06V286000 (VEHICLE SPEED CONTROL) WAS DISCOVERED; HOWEVER, THE MANUFACTURER WOULD NOT DISCLOSE WHETHER OR NOT THE VEHICLE WAS INCLUDED. THE OWNER WAS INFORMED THAT AN INVESTIGATOR MAY INSPECT THE VEHICLE. THE CURRENT AND FAILURE MILEAGES WERE 90,000.

FRONT PASSENGER SEAT BELT WILL DISENGAGE WITH OUT TOUCHING THE RELEASE BUTTON. I HAVE TRIED USING HIGH PRESSURE AIR THINKING IT WOULD DISLODGE SOMETHING. EVEN TRIED A LITTLE WD-40 TO NO AVAIL. THE CYLINDER HEAD GASKETS ARE A MANUFACTURER DEFECT. THEY USED INFERIOR QUALITY GASKETS WHICH RESULT IN ENGINE MALFUNCTION. THIS MANUFACTURER DEFECT PERTAINS TO A 2001 FORD F-250 SUPER DUTY 5.4L V8. I AM AWARE THIS ISSUE BECAME PREVALENT IN 1999 FORD F-250 MODELS AND THIS ISSUE CARRIED FORWARD TO 2001. THERE WAS A RECALL FOR THIS ISSUE FOR 1999 MODELS - $4,500 PER UNIT TO REPLACE COMPLETE ENGINE ASSEMBLIES WHERE THIS MANUFACTURER DEFECT WAS MOST PREVALENT WITH CYLINDER HEAD GASKETS. Was T-Boned in the passenger side of the truck which changed my direction of travel resulting in hitting a tree head on. Neither frontal air bags deployed. The frontal impact was hard enough to push the tree 18 to 20 inches into the engine compartment bending the frame where the front bumper was attached. The accident resulted in a fatality. I'm just confused why neither air bag deployed. I have had this truck since it was new so I know it's history. There was not any warnings lights on, No air bag warning lights, No modifications were ever done to the truck. All stock. There was never any indication that the air bags were not operating normal. So Why did they malfunction?

WHILE DRIVING BACK HOME FROM SHOPPING MY BRAKES (ANTI-LOCK STYLE) LOCKED UP AND VIOLENTLY SHOOK MY VEHICLE ALL OVER THE ROAD I WAS DRIVING ON. I HAD LIMITED OR NO CONTROL AT ALL OF THIS INCIDENT. WAS DOING ABOUT 40 MPH. TRUCK ACTED AS IF BRAKES WERE APPLIED ON AND OFF 50/60 TIMES SO FAST THAT THERE WAS NOTHING TO DO BUT HANG ON TO THE STEERING WHEEL. AT ONE POINT I THOUGHT I WAS GOING TO FLIP OVER AS THE TRUCK WENT SIDEWAYS. STEERING WENT FROM SIDE TO SIDE AT FIRST, THEN A CONSTANT PULL TO THE RIGHT. I THOUGHT ONE OF MY FRONT TIRES WAS BLOWN OUT. SLOWED DOWN ENOUGH TO GAIN SOME CONTROL BUT IT TOOK MY TRUCK OVER A CURB AND STOPPED ON A SIDEWALK AND SOME GRASS. WHEN I DID APPLY THE BRAKES NOTHING HAPPENED UNTIL I WAS ON THE SIDEWALK AND GOING VERY SLOW BY THIS TIME. GOT OUT AND INSPECTED MY TRUCK FOR DAMAGE BUT SAW NOTHING. WAS ONLY ABOUT .3 MILES FROM HOME SO I STARTED MY VEHICLE AND LIMED IT HOME AT WALKING SPEED. WHEN I TRIED TO GO A LITTLE FASTER IT WOULD START JUMPING AND SHAKING AGAIN. GOT IT HOME AND THAT IS WHEN I NOTICED THE ABS LIGHT WAS ON. IT WAS NOT ON WHEN I STARTED HOME FROM THE STORE. THIS HAPPENED SO FAST THAT I AM GLAD NO ONE WAS IN ANOTHER LANE. THE TRUCK BURST INTO FLAMES WHILE PARKED. IT HAD BEEN SITTING FOR OVER 13 HOURS AT THE TIME OF THE FIRE. IT DESTROYED ONE CAR ON EITHER SIDE OF IT. THE FIRE INVESTIGATOR COULD NOT SAY EXACTLY WHAT CAUSED THE FIRE AT THE TIME, BUT MADE ME AWARE OF A RECALL ON THAT YEAR OF FORD VEHICLES INVOLVING THE SPEED CONTROL SAFETY DEACTIVATION SWITCH. INFORMATION ON THE RECALL SAYS THAT THE WIRE COULD HEAT UP AND CAUSE A FIRE. THE TRUCK ALONG WITH TWO OTHER VEHICLES WERE COMPLETELY DESTROYED IN THE FIRE.

BRAKELINE RUPTURED, MAYBE RUSTED THROUGH IN RONT OF LEFT REAR WHEEL WELL, PEDAL WENTT O THE FLOOR & ONLY HAD SLIGHT AMOUNT OF BRAKES. I CALLED FORD CUSTOMER SATISFACTION, THEY WERE LESS THAN CONCERNED. SEE ON LINE OTHERS HAVE HAD SIMILAR ISSUE. SEEMS MOR OF A PROBLEM THAN THEY WILL ADMIT.

WHILE I WAS DRIVING ALONG A HILLY, WINDING ROAD THE ENGINE OF MY 2001 F-250 DIESEL 4X4 TRUCK UNEXPECTEDLY SHUT OFF. I LOST STEERING AND BRAKING. THERE WERE NO LIGHTS ON THE DASH. THERE WAS NO PRIOR INDICATION OF ANY PROBLEM WITH THE ENGINE OR ELECTRICAL SYSTEM. I'VE HAD THE TRUCK FOR 12 YEARS AT THE TIME AND NEVER EXPERIENCED ANY PROBLEM LIKE THIS. I WAS VERY FORTUNATE TO HAVE BEEN ABLE TO GET THE TRUCK TO THE ROAD'S SHOULDER. AFTER THE TRUCK STOPPED I PUT THE SHIFT INTO PARK. THE KEY WAS STILL IN THE RUN POSITION. I SIMPLY TURNED IT TO THE START POSITION AND THE ENGINE STARTED RIGHT UP. I TOOK IT IMMEDIATELY TO THE LOCAL FORD DEALER WHO RAN DIAGNOSTICS ON THE TRUCK. THEY FOUND NO INDICATION OF A PROBLEM OTHER THAN ONE BATTERY'S VOLTAGE WAS SLIGHTLY LOW, BUT NOT OUT OF ACCEPTABLE RANGE. I REPLACED BOTH BATTERIES AS THEY WERE ALMOST 3 YEARS OLD. SO FAR, I'VE NOT EXPERIENCED THIS PROBLEM AGAIN. *JS

TL* THE CONTACT OWNS A 2001 FORD F-250. THE CONTACT STATED THAT DURING RAINY WEATHER, THE GENERAL ELECTRIC MODULE WOULD BECOME WET AND CAUSE THE VEHICLE TO MALFUNCTION. THE CONTACT STATED THAT THE FAILURE WOULD ALSO CAUSE THE VEHICLE TO START INDEPENDENTLY, THE LIGHTS WOULD FLASH AND THE TURN SIGNALS WOULD ILLUMINATE INDEPENDENTLY. IN ADDITION, THE AIR BAG WARNING LIGHT WAS ILLUMINATED. THE DEALER STATED THAT THEY WERE AWARE OF THE FAILURE AND MADE A COVER FOR THE PART. THE CONTACT TOOK THE VEHICLE TO THE DEALER AND WAS TOLD THERE WAS A LEAK COMING FROM THE WINDSHIELD THAT WAS ALLOWING WATER TO ENTER THE GENERAL ELECTRONIC MODULE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS 180,000 AND THE CURRENT MILEAGE WAS 240,000.

DT*: THE CONTACT STATED WHILE THE VEHICLE WAS IDLE, A FIRE ERUPTED WHICH WAS CAUSED BY A FAULTY ELECTRICAL SYSTEM. THE FIRE ORIGINATED FROM THE ENGINE COMPARTMENT FRONT UNDERHOOD. THERE WERE NO WARNING SIGNALS PRIOR TO THE INCIDENT. UPON INSPECTION, THE CRUISE CONTROL DEACTIVATION SWITCH AND WIRING FOR THE ELECTRICAL SYSTEM WAS OBSERVED TO BE COMPLETELY BURNT TO THE GROUND. A FIRE REPORT WAS TAKEN# 06-017205. ALSO, THE MANUFACTURER WAS NOTIFIED, BUT WOULD NOT OFFER ANY RESOLUTION WHILE THIS CASE WAS UNDER INVESTIGATION.

MY HUSBAND AND I HAVE A FORD F250 TRUCK. IT'S A 2001 WHICH WE PURCHASE IN 3/2002. MY HUSBAND WAS DRIVING THE TRUCK WHEN HE HEARD A BANG THEN THE TRUCK DIED. WE HAD THE TRUCK TOWED TO A LOCAL AUTO REPAIR SHOP. THE REPAIR SHOP STATED THAT THEY WOULD NOT TOUCH THE TRUCK BECAUSE THE SPARK PLUG BLEW OUT CAUSING THE HEAD TO BLOW. THEY STATED THAT THIS IS AN ONGOING THING WITH FORD TRUCKS AND FORD WILL NOT ADMIT THAT THEY HAVE AN ENGINEERING PROBLEM WITH THE MOTOR. I BROUGHT THIS MATTER TO THE DEALERSHIP AND THE FORD CUSTOMER SERVICE DEPARTMENTS ATTENTION. NEITHER ONE OF THEM WOULD ASSIST ME. THEY STATED THAT WE WOULD BE RESPONSIBLE FOR THE REPAIRS. AFTER THESE PHONE CALLS I CALL A FEW OTHER AUTO REPAIR SHOPS. THEY ALSO STATED THAT THAT WAS UNUSUAL THAT A SPARK PLUG WOULD BACK OUT AND EVEN IF THE SPARK PLUG DID BACK OUT THAT IT WAS UNUSUAL THAT IT WOULD BLOW THE HEAD GASKET. AT THIS POINT WE HAVE TO PAY FOR THE REPAIRS WHICH IS 3,500.00. I WOULD LIKE THIS MATTER LOOKED INTO.

WE WERE NOTIFIED BY POLICE AND FIRE DEPARTMENT OFFICIALS THAT THERE WAS A FIRE IN OUR YARD. OUR FORD 250 WAS ON FIRE AND HAD NOT BEEN DRIVEN FOR AROUND 4 HOURS. UPON INVESTIGATION THE FIRE MARSHALL INDICATED THE LOCATION OF WHERE THE FIRE BEGAN AND PRESUMED IT WAS THE BRAKE PRESSURE SWITCH BASED ON ITS LOCATION. THE CAR HAS BEEN TOTALED AND CAUGHT ANOTHER VEHICLE ON FIRE NEXT TO IT.
