Vehicle Information: 2020 Tesla Model 3 AWD (Leased) Full Self-Driving (FSD) enabled Summary of Safety Issue: While Full Self-Driving (FSD) was engaged, the vehicle failed to detect roadside infrastructure and collided with multiple fixed objects, including a route sign board, a walkway sign board, and an underground electrical cable. Full Self Driving mode (with Hardware 3) consistently will enter the carpool lane when I am driving solo. In the vehicle navigation settings, I have, "use carpool lanes," unselected. It doesn't seem to use this input in it's decision to enter the carpool lane. Sometimes it is very quick to signal and then enter the carpool lane illegally that I can't respond quick enough to correct. However, it only seems to need 1 correction for it to ignore the carpool lane for the remainder of the trip. This then can/will repeat each new trip (it doesn't always occur for each new trip, sometimes it won't make an attempt to get into the carpool lane). I have sent Tesla dozens of recorded messages that it needs to be fixed. It will enter at an illegal point, crossing the solid line, this can sometimes be a dangerous maneuver as well as being illegal for a solo occupant. Also, I don't want to want to be ticketed. It's quite a stressful situation each time this occurs.

We were driving back from suffolk virginia along [XXX] - which has two lanes in both directions. It was a sunny day with clear skies. I had cruise control enabled. There were no other cars visible on the road in either direction in front or behind us. Suddenly and without warning the car slammed on the breaks - "phantom breaking". If somebody was behind us it could have easily been a crash. We continued our journey somewhat shaken, and 10 - 15 mins later it did the same thing in similar circumstances. We contacted Tesla who said we should press a button to record an event when it happened again. On a later day it did and we sent them the information. They contacted us back and said that nothing was wrong with car. Since then I feel the cruse control is basically dangerous, so I refuse to use it. I am reporting a critical safety incident involving my vehicle, which occurred during rush hour on a busy highway with no traffic lights or stop signs. While waiting in the left turn lane at a center divide, my emergency autopilot braking system activated unexpectedly, causing my car's left front side to encroach into oncoming traffic. Despite my attempts to move the car, it became immobile. I attempted to reset the car by turning it off and on, but the vehicle’s computer system malfunctioned entirely. With oncoming traffic traveling at speeds of 70 mph, I had no choice but to exit the vehicle for safety. I left the car in the center divide. This incident was extremely frightening, and I am deeply concerned about the potential dangers posed by the malfunctioning autobraking system. Thank you for your attention to this serious matter.

While driving on cruise control at highway speeds (75-80mph) on I-80 the car braked VERY hard and unexpectedly MANY times. There were no other cars around me, weather clear, road straight. No obstacles noticed to cause this reaction. If a car had been behind me I would have caused him to rear-end me because there was no apparent reason for the severe braking. I reported four incidents of this type to the Tesla during the drive which I believe were date/time stamped so they could diagnose (using the "report" function). I matched these with my own observations which I also provided to the service department. I have had the car diagnosed by Tesla and they claim they could not duplicate the problem and/or fix the problem.

I recently purchased a 2020 Tesla Model 3 Long Range AWD (VIN [XXX] internal systems and service records show that all recalls for this vehicle have been completed and closed. However, the NHTSA VIN lookup still shows seven open recalls. This is causing a major issue because I am participating in a California clean transportation incentive program (Replace Your Ride), which requires that all recalls be shown as closed specifically on the NHTSA website. Tesla states that the recalls are complete but has not updated the recall status with NHTSA. NHTSA customer service informed me that recall data can only be updated by the manufacturer. I am requesting that NHTSA notify Tesla’s regulatory or safety compliance team so they can update the recall reporting for this VIN. The outdated recall data is preventing my state incentive from being processed and is causing significant financial harm. Experienced sudden unintended acceleration (SUA) while parking the car. Was pulling into a parking spot at a very slow speed, approx 5 mph and the car accelerated to an extremely high speed on its own without any input from me. Tried to brake to stop the car but the car did not respond or stop or slow down. Due to the SUA event, the car crashed into the side of a building, all air bags in the car were deployed, and the car is a total loss per insurance.

While driving south bound on [XXX] in Torrance and intending to go [XXX] , the GPS suggests to take a left turn for the ramp. The left turn is illegal. The ramp is on the right side of the road. With FSD, I have to intervene and take control. This has happened multiple times in last 1 year, since the time the ramp on right side was opened and left side became illegal. GPS data is wrong. The Tesla windows have a window sensor in the door. It appears these fail. When they fail, the door does not shut securely. More specifically, it stays paritally ajar. I would like to report this a safety issue. More specifically, it is possible a passenger could fall out when the window does not shut properly. Additionally, it seem to impact the drivablity and safety. This should be considered a recall item.

Tesla's latest update pings you when you're sitting at a light and it turns green. It also shows you the green light on its screen rendering of your surroundings. The other day, Tesla incorrectly pinged me that a light was green when it was still red. It also displayed it as green on the screen. This is dangerous not only because it suggests the self-driving mode would have incorrectly registered the light and started forward (I was not in self-driving fortunately), but even without self-driving, drivers become primed to go when the ping happens, so this could lead a driver to run the light.

Incident #1 Vehicle sitting at a red light. After several seconds the vehicle operating under Tesla FSD suddenly started to drive as if the light had turned green, which it had not. I stopped it immediately. Incident #2 Vehicle was stopped a t a red light and started forward again as it the light had changed but had not. I noticed a flashing red light at the next intersection a few hundred feet away. So, I went around the block and tried again and it behaved the same. After a few seconds it started to go again. There was not another light near where the first incident occurred.

I've had the car since December 2019. The car will "phantom brake" when the autopilot adaptive cruise control is on. This happens intermittently, ~1 time per month on my normal commute. I will be going along, cruise set to 75 and out of nowhere it'll slam on the brakes and lose 10 MPH in a second before I can take over. This occurs with absolutely no traffic or reason, it'll be an empty highway and it does it. I drive with my foot over the accelerator so I can floor it to override the car. This is the exact opposite reaction I would think that someone should have to do with a safety feature. Tesla tells me the system is working fine, but random brake checks on the highway should never be normal. I don't understand how this system was approved to operated by the government.

On 9/3/23 we were traveling from Moab UT to Green River UT and the cruise control was engaged at highway speeds. We were not using the "Autopilot" functionality. Without warning, the car suddenly decelerated under what felt like hard braking. There was no warning immediately before the hard braking started and the car had no warning indicators of any kind either before or after the incidents. The hard deceleration happened at least four times. One of those times a raven flew up from the road, but the other times there was nothing in the road. At least three of the incidents occurred on highway 6 traveling north. The last time it occurred was on I-70 westbound after we stopped the cars and cleaned all the cameras and ensured there was no debris on the front bumper which could cause issues with the forward-looking radar. We lost confidence in the cruise control feature and did not enable it for the rest of the trip. This is a safety issue because the car does not just reduce speed, it feels like it is braking hard or perhaps full regeneration of the battery. Either way, on these 2-lane highways, there are often situations when other drivers are driving too close to the rear bumper and a sudden, inexplicable hard braking is very likely to cause collision. A collision at these speeds could push the car into oncoming traffic, leading to fatalities. We asked Tesla service to look into the issue, and said there is likely footage of at least one of the events (because I honked the horn to start recording). The Tesla service person gave several reasons why it deceleration may occur, but Tesla is neither reviewing the footage, nor taking action to remedy the issue. They said "Our remote diagnostics has confirmed the vehicle hardware is operating as expected and there are no repairs the service team can make in regards to this." With service unable to make a fix to the car, this problem remains unresolved and we have lost confidence in cruise control.

Sudden hard "phantom" braking while in adaptive cruise control. This put myself and other drivers in danger of a collision. The problems been reported to Tesla, but as of yet not fixed or inspected.The problem seems to be completely random with no advanced warning of any kind.

Phantom Braking occurred almost 40 times on our recent trip from Northern NV to California and return. We left on July 16 and returned on July 21. Our route was from Minden NV to Las Vegas on Highway 95 and then onto Temecula CA on Highway 15 and 215. The return trip was on Highway 15 to CA 138 to Highway 14 to Highway 395. All travel was during daylight hours. Sometimes phantom braking was so severe, the deceleration cause us to be thrown forward towards the dashboard. We wore seat belts to prevent hitting the dashboard. This also happened on a similar trip in 2022 and was reported to Tesla and were told that nothing could be done. The current software version is 2023.20.9

While driving my Tesla Model 3 with self-driving engaged in excellent conditions, the car suddenly braked to a full stop. There was no apparent reason to do so and the driver did nothing. The road ahead was free of traffic, and no obstacles, pedestrians or animals were visible. It was possible to resume driving by turning self-driving off. Tesla forums online show many instances of others reporting similar problems in various Tesla models over several years. A rear end collision will ultimately be the result of this flaw if it hasn’t yet occurred.

While driving on the highway from philadelphia to allentown using I-476 north i was driving using cruise control and the car suddenly applied brakes ( phantom braking) there was no one either in front or sides or behind me. No accident happened

Driving on the freeway with the adaptive cruise control set to 78 mph. Traveling in the #1 lane, medium traffic, no vehicles in front of us for a while (in our lane or the #2 lane next to us). Roadway was dry, sun was overhead at that time of the day. Weather said it was 72 outside, so not excessively hot. While nearly cresting the top of a slight incline, the car hard-braked, taking the speed from 78mph to 58mph for no obvious reason. (Thank heavens the car behind us was following at a safe distance and was able to brake, too.) There was no debris on the roadway or any other reason why the car would need to brake. Suffice to say that we deactivated the adaptive cruise control for the remainder of the journey.

While driving on the freeway I5 in central California in the middle of the day there were no cars in front of me and the vehicle went into sudden PHANTOM BRAKING mode. I was driving at 75 MPH and it suddenly went into hard braking. Extremely dangerous. The vehicle behind me had to swerve around us. We could have been killed. I was using the cruise control but NOT the Enhanced Autopilot. This needs to be fixed. We escaped severe injury or death by sheer luck

The contact owns a 2020 Tesla Model 3. The contact stated that on several occasions while driving with the cruise control feature activated at various speeds, the brakes applied inadvertently. The contact also stated that the cruise control feature failed to operate as intended after dark, after deactivating inadvertently. Additionally, the contact stated that the front and rear cameras displayed phantom images and failed to work as intended. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was unknown.

I was already situated in the left turn lane on a two way city street, slowing down, and about to come to a full stop before making a U turn when I felt a surge in power before the car picked up, accelerated uncontrollably, and launched forward. In front of the turn is a raised cemented median strip divider where the car hit and went on top of it. Instinctively, I just held on to the steering wheel firmly to keep the car straight so that it will stay on (top) and within the divider and not veering to the right or left and running onto incoming traffics on both sides of the street. The car came to a stop after coming off the other end of the divider (approximately 100 ft) and ended on one of the lane from the opposite traffic. Thankfully there were no cars on that lane at the time and I was able to sluggishly drive the car safely into an adjacent parking lot. I, as well as other could have been seriously injured or killed. All wheels and tires are visibly damaged and possibly the underside as well. The car is in a body repair shop and waiting for inspection at this time. I already reported the incident to my insurance and Tesla companies. All inspections and reports are pending at this time. There was no police report since there was no collision or bodily injuries. There were no warning signs, signals, messages, or symptoms from the car or from Tesla before the incident. The car 12V battery was changed 3 months prior by Tesla Service Center. This is my first encounter with this sort of problem.

The car had unintended acceleration. I was driving and suddenly it sped up really fast and I needed to slam on the brakes. This was on a residential street so it was really dangerous. This happened once before, on a highway. Both times this happened, I was touching the wipers controls. I have no idea if it’s connected or coincidence.

The contact owns a 2020 Tesla Model 3. The contact stated while driving 10 MPH and making a left turn, the steering wheel seized, and the front passenger's side of the vehicle lowered to the ground and made an abnormal grinding sound, before coming to a stop. The contact stated that upon exiting and inspecting the vehicle, it was noticed that the tie rod had detached from the front passenger's side wheel, and the front passenger's side wheel had turned inward and was pinned by the wheel well. The vehicle was towed to a Tesla Service Center, where it was diagnosed and determined that the front passenger's side steering assembly needed to be replaced. The contact was charged a fee for the repair. The manufacturer was notified of the failure. The failure mileage was approximately 36,000.

While driving I attempted a turn and heard many horribly loud clunking noises and was unable to steer car. I looked under the front of the car and saw the left front tire turned to the left (the right front tire was straight) and there was a piece on the ground which was determined to be the lower control arm. The two bolts were missing so it became disconnected. Thankfully I was in a parking lot at a slow speed so no injuries. The car was towed to a repair shop who analyzed the problem. There was no warning given on the car’s computer system, just sudden loud clunking sounds when the car became inoperable. One day later I located one of the two bolts in the driveway.

Summary Complaint – 2020 Tesla Model 3 (VIN: [XXX] ) I am reporting a defect with the passenger side airbag system in my 2020 Tesla Model 3. Tesla Service identified the passenger airbag sensor as defective and quoted a $1,600 repair cost. This raises concern that the problem is not isolated, but may represent a broader safety defect affecting other Model 3 vehicles of the same year. Why this is a safety issue: - Airbags are federally required safety systems. A defective sensor risks non-deployment, delayed deployment, or incorrect deployment. - Tesla has already recalled 2020 Model 3 vehicles for airbag issues (twisted side curtain airbags – NHTSA Campaign ID 21V834000). - In 2024, a Model 3 recall was issued for a defective driver airbag module. - Other automakers (GM, Hyundai/Kia, Takata) have recalled vehicles for similar airbag sensor/module failures. Airbags did not deploy when vehicle hit 75 ft highway guardrail driving over 70 mph. The car was totaled, the driver's head hit the steering wheel, requiring stitches. EMS and police at the scene were surprised to find airbags not deployed. Vehicle has been inspected by insurance. Manufacturer has not yet been informed, to my knowledge. There were no warning lights or messages indicating a problem with airbags or anything else for that matter. Driver was wearing seatbelt.

I have to pay over $1444 to Tesla to change the airbag because it is not working properly. Tesla will: Update front passenger Airbag to Maintain compatibility with front passenger seat. Replace Occupant Classification System (OCS) of Front passenger Seat. Install OCS filter module.

When the seat moves forward I get a warning about the restraint error and turn the airbag off. When I slide the seat back the error goes away.

Front lower and upper control arms started generating noise around 100,000 miles. This was confirmed by an independent service center. So far, have spent $1400 on the lower control arms, only to find out that the noise is still there and the upper control arms need to be replaced as well.

The front suspension is failing. I have already replaced the upper control arms. Now I need to replace the bushings on compliance and lateral linksare tearing. There have been recalls for this model and year, for this issue. My VIN, is not included in the recall. I was notified of the aditional needed repairs when I had the upper control arms replaced.

THE FRONT CONTROL ARM IS DAMAGED, INTERESTINGLY ABOUT 6 MONTHS AFTER THE WARRANTY HAS EXPIRED. TESLA REPAIR TECH TELLS ME ITS SO COMMON THAT THEY DON'T CHARGE FOR THE REPAIR BUT DO CHARGE FOR THE PART. AS I SIT IN THE TESLA REPAIR CENTRE WAITING FOR IT TO BE FIXED, 3 OTHER PEOPLE WITH EXACTLY THE SAME PROBLEM JUST WALKED IN DESCRIBING EXACTLY THE SAME PROBLEM. I DON'T UNDERSTAND WHAT THIS PART DOES BUT GIVEN IT'S PART OF THE SUSPENSION, IT POSES A SAFETY RISK. THERE WERE NO WARNINGS EXCEPT FOR A VERY LOUD SQUEAKING NOISE EVERY TIME THE CAR TURNS OR ENCOUNTERS A BUMP

I am writing to express my concern regarding the recurring issue with the control arms and sway bar in the 2020 Tesla Model 3. It has come to my attention that Tesla acknowledges this issue, which manifests as creaking sounds during various driving conditions such as climbing into the car, traveling at low speeds, and navigating over speed bumps or uneven terrain. What's particularly troubling is that when I brought my car into them Tesla acknowledged that they are aware of the prevalence of this issue across many of their vehicles, yet they continue to charge consumers for repairs, even if their vehicle is not included in the recalled VINs. This policy unfairly burdens consumers who experience the same issue but are denied coverage under the recall. I urge the NHTSA to investigate this matter thoroughly to ensure consumer protection and fairness in addressing safety concerns in Tesla vehicles. It's essential that manufacturers uphold their responsibility to rectify known issues without imposing undue financial burdens on consumers.

My vehicle is 4 years old. 3 weeks after the expiration of my original warranty, the front suspension now squeaks loudly when driving at low speeds. This issue matches the known manufacturing defect described in Tesla’s 2021 service bulletin, “Reseal Front Upper Control Arm Ball Joints.” We never received this bulletin from Tesla, and we were never given the option of resealing the joints. As a consequence, the ball joints have now failed and both upper control arms must be replaced, at a cost to me of $330. I asked if Tesla can waive these repair costs due to the bulletin and widespread manufacturing defect, but Tesla declined, stating: “Your vehicle is not listed as an affected VIN for this bulletin. In order for a repair bulletin to be covered under the vehicle warranty the condition has to be reported within the warranty period. Once no longer under warranty, it is not covered.” Even though my VIN was not included, clearly it should have been since the ball joints have failed. Also, if this is a known car defect, why should it matter when the problem starts to appear in order for Tesla to take full responsibility and fix it? They are shifting costs to consumers for their own, known, widespread manufacturing defects. Additionally, if I had been informed about the bulletin in 2021, resealing the joints at that time would have been easier & far cheaper than replacing both arms now. It’s my understanding that Senators Markey (MA) and Blumenthal (CT) expressed concern that this defect could be a safety risk, and urged Tesla to issue a voluntary recall, which Tesla declined. I hope NHTSA will consider investigating this manufacturing defect further. Thank you very much for your time. Vehicle is available for inspection. Had to drive through water about 8” deep and while going through the water the rear bumper, and the skid plate underneath the car was yanked off breaking 2 brackets that hold the skid plate on and it broke the manufacturer welds on the other two brackets. This is a known defect with 2017-2019 but obviously it is also a defect with the 2020s.

Tesla Model 3 vehicles, including mine, manufactured before the 2024 "highland" vehicle refresh, do not have a mechanical latch mechanism for opening the rear doors, in the event vehicle power is lost.

The door handles on the chrome version of the Tesla Model 3 begin to delaminate after several years of use. They delaminate along the door handle seam which creates an extremely sharp edge that can cut fingers. I've had the rear handle on my own model 3 rear door cut my kids finger when they tried to get into the back seat. Because the car is technically out of warranty Tesla will not cover repair without significant cost. There is no warning sign or notification other than when you reach to pull the handle and the sharp chrome edge cuts you, the one door with this issue now has the handle partially protruding since the handle can no longer sit flush. There are several threads where similar chrome version model 3 owners have had the exact same issue occur with the exact same safety risks. The back trunk seal fails at the rear of the vehicle allowing water to spill into the trunk and deep trunk and cause flooding. I have had it repaired multiple times and the technician says that is a known issue but all they do is add more seal. This is known on model 3s before the most recent refresh. Also known as the Highland. This is cause deep trunk flooding and mold to grow on the Fabrics. This is just with casual rain living in Washington State. This is my second time dealing with the first happened within the warranty and the second is now without warranty however if this issue has been persistent since I have gotten the vehicle as originally they told me the tail light was missing seal but it was still leaking and different times that I inquiried they said to try to park in a dry spot to let the vehicle dry out to ensure that it's not water caused from opening the trunk.

My Tesla Model 3 was involved in a 'fender bender' almost 9 months ago. It took 3 months just to get an appointment at a "Tesla Certified Collision Center" in my town (and I live in a fairly major city - Portland, Oregon). After the vehicle went to the shop it has now been there for OVER 5 MONTHS! This is a result of parts being incorrectly sent from Tesla, parts being unavailable from Tesla, parts being mislabeled and/or 'hidden' in their Ordering system and so many other unbelievable problems that Tesla has had. I have now been without the vehicle for over 5 months and still waiting on some other part that was now ordered over 2 weeks ago. The whole process is completely unacceptable and Tesla has really dropped the ball. I will be selling the vehicle and never owning another Tesla again, the moment it is finally done being repaired. Do I have any recourse? Can I sue Tesla to cover the 5 months of insurance I just spent my money on? Thanks!

I am reporting a spontaneous windshield crack that developed on my 2020 Tesla Model 3 Performance under normal operating conditions. After exiting a routine drive-through car wash in Las Vegas on a hot day, I noticed a hairline crack in the windshield. It originates from the very bottom edge within the black ceramic frit area, curves upward, and spans across the glass. There are no impact points, chips, or pits of any kind. I brought the vehicle to Tesla Service, where the Service Manager visually confirmed it is a stress crack not caused by external damage. Despite this, they stated that Tesla does not offer goodwill or warranty repairs for windshields, regardless of the nature of the failure. This raises a serious safety concern. A windshield is a structural component of the vehicle and critical for driver visibility. A spontaneous failure like this without external cause compromises safety, especially at highway speeds. There are numerous reports of similar spontaneous cracks in Tesla Model 3 vehicles, often originating from the same area. This suggests a potential systemic defect in the design, installation, or bonding of the windshield. I am requesting that the NHTSA investigate this issue for safety implications and possible structural design flaws.

A crack appeared on the interior of the windshield, with no marks of any exterior impact and no external crack or other mark on either surface of the windshield. The crack originated directly from the windshield positioned camera/rear view mirror.

The windshield wipers do not have proper functional control. There is no adequate manual control over the windshield wipers without being distracting, further more the windshield wipers constantly put themselves in "auto" mode when you turn them off. This causes unnecessary distraction while driving and because the only feedback you have is either on the main touch screen or the bottom left touch screen this is a severe safety issue.

Tesla vehicles have an automatic wiper mode that is supposed to clear the windshield, without driver intervention, when it becomes obscured due to rain, snow, or water buildup. While driving on December 19, 2023, our Model 3 was passed by a large truck, throwing a large amount of water across the windshield. When this happened, the forward visibility was completely obscured momentarily. The automatic wipers did not clear the windshield as expected. The vision was obscured until the driver was able to manually enable the wipers. Obviously, driving at highway speeds on the freeway, without being able to see forwards, even momentarily, is a safety concern. Thinking the car had a failure, we tried to arrange for a repair from Tesla service. Their response was: "This issue has been identified as a known firmware issue, to be resolved on a future firmware release. For now, we recommend using the manual wipers as needed when auto wipers are not detecting rain. When a fix is available it will be sent to your vehicle automatically over the air. Unless you had any other questions, I will go ahead and close out this service request. Thanks!" Looking through Tesla owner forums, this appears to be a much discussed issue for the last 4-5 years. Many owners are having issues with functionality of the automatic wipers. We don't have any photos of the incident, but I recreated a similar situation in a local touchless car wash. With the car being completely deluged in water and soap, the automatic wipers never came on. The included pictures show the touchscreen with the Auto wiper mode and the windshield completely obscured, but no wiper activation. The last photo is a screenshot of part of the conversation with Tesla support about the repair request.

Currently occurring with FSD Beta firmware V.2022.27.12 but the same has occurred with previous firmware versions also. Normal operation of the vehicle with the ADAS engaged puts the windshield wipers into AUTO mode. At times the windshield wipers are activated when the windshield is dry. It isn't raining or misting and there's nothing on the surface of the windshield at the location of the front-facing cameras. "No rain" occurrences seem to correlate with times when visibility ahead has low contrast and there are no highlights and shadows to sharply define the scene, (ie, a little "muddy"). Observed when it is overcast but not raining, around sunrise when the sun is low in the sky and the light levels ahead are relatively low and headlights aren't helping illuminate the road ahead, and at times there are high-contrast light and shadows immediately ahead but farther down the road there is mist or fog, creating the same low contrast lighting conditions some distance away. It is as if the ADAS is aware that the scenery is not sharply defined and is "wiping its eyes" in an attempt to gain a more sharply-defined view. Occasionally, but rarely in my experience, the windshield washer is also engaged. Having the ADAS activate the wipers at these times is not a safety hazard, per se, but it is distracting and concerning as it gives the driver the impression that perhaps the ADAS is malfunctioning. I've read in user forums reports from a fair number of FSD Beta drivers complaining that the wipers go on when the windshield is dry and the ADAS is engaged. I haven't observed the same when the ADAS is not engaged.

My Tesla Model 3 is experiencing premature suspension failures involving the lower lateral links and stabilizer links. These components were already replaced once, and they are failing again in less than 30,000 miles. This is not normal wear for suspension parts on a modern vehicle. These failures cause clunking, instability, and looseness in the front end—conditions that affect the vehicle’s ability to track straight and maintain control, especially over uneven pavement or during emergency maneuvers. This isn’t an isolated case. There are extensive reports from other Tesla owners across multiple models (Model 3 and Model Y in particular) showing the same premature wear on these suspension links. Many owners have parts failing even earlier. Once the car is out of warranty, Tesla charges thousands of dollars to replace these components, despite clear indications that the suspension was under-engineered for the weight of the vehicle. Tesla knows this is a recurring issue but refuses to issue a recall or goodwill repairs. A repeated failure of critical suspension links in such low mileage suggests a defect. These parts should not be wearing out this fast, and the resulting instability poses a real safety risk. I’m requesting NHTSA investigate these premature failures.

This report is filed to support NHTSA Investigation #PE24031. The four vehicles listed on the SGOs were manufactured when Tesla referred to the ADAS hardware suite as Hardware 3.0 (HW3). This March 2022 video recorded an incident facing into the sun in a 2020 Tesla Model 3 with HW3 installed. Tesla's ADAS is engaged and operating on FSD Beta V10.10.2 firmware. A video sensor reacts to bright sun and shade by raising and lowering its sensitivity, which may compromise exposure if it occurs once. In a series of pulsed, high-contrast lighting changes, the exposure may be less than optimal during much of the transient. The depth and duration of the exposure error will vary by the sensor's make and model. Next, See: [XXX] ) Only the video is relevant. The rear-facing camera view on the Tesla's display shows the shadow cast directly behind the vehicle, confirming direct alignment. At 0:51, the car enters a bridge section where cross-members cast shadows onto the windshield-mounted cameras, causing a pulsed lighting effect. If a video automatic gain adjustment (AGC) step is in the signal path downstream of the sensor, it may react adversely to the pulsed input. The signal path latency, processing time, and the attack and release settings of the AGC delay the output, making it out of sync with the sensor's direct output. Note: At 0:57, the driver requests a speed adjustment as the ADAS emerges from the strobed lighting. At 1:50, the visualization displays the VRUs as facing into the vehicle's path and the ultrasonics returning pings off the metal barrier to the right. Other: Social media reports indicate that interior condensation in front of the front-facing cameras can be resolved using the standard front-screen defrosting feature. An incident occurred in April of 2023 while running FSD Beta 11.3.6 firmware. I have been unable to retest this location on more recent software, so this concern may have been mitigated with a subsequent update. As seen in the video, the yellow lane marking on the left transitioned from the roadway up onto the movable center median. Once on the bridge, the left repeater camera and the center display visualization both illustrate the lane centering was definitely biased towards the left side of the lane, a possible indication the vision-based lane centering was miscued by the yellow line now appearing on the barrier instead of the road surface. This left bias brought the vehicle uncomfortably close to the barrier, which resulted in a manual takeover. It is difficult to judge based on the low resolution of the repeater camera view, but my estimation is that the tires were approximately 12 inches away from the protruding bottom lip of the barrier. [XXX] This same left lane marking concern also occurs at the entrance to the Oakland, California Caldecott tunnel eastbound bore. The Google maps link provided shows the yellow line which defines the left lane edge disappears at the tunnel entrance, at which time the vision-based lane centering seemed to be cuing off of the curb as the lane edge, rather than there being a marking that would delineate a lane edge further away from the curb. An FSD beta user (name and date unavailable) reported contacting the curb at this location at approximately the same date as the GGB recording. [XXX] The GGB matter was brought to the attention of the Bay Area Toll Authority with the suggestion they contact Tesla. I have been unable to verify that the Toll Authority followed up on that suggestion. The release notes for Beta 11.4.4 suggest the issue might have been addressed, to wit, "Improved offset consistency when controlling for static obstacles." I am writing to urgently express my concerns about the latest software update for the Tesla Model 3’s autopilot system, which, in my view, significantly compromises safety. As a Model 3 owner for over two years, I have generally found the vehicle and its features to be reliable and safe. However, the recent update has introduced an overly stringent hands-on-wheel detection mechanism that is not only inconvenient but also potentially hazardous. The new update requires frequent and often forceful interaction with the steering wheel to assure the system of driver presence. This change is drastically different from my previous experience, where I received only one hands-on-wheel violation in two years. The current sensitivity of the system disrupts the smooth operation of the autopilot, leading to frequent and abrupt disengagements. I have found myself struggling to maintain the system’s activation, inadvertently causing the vehicle to exit autopilot mode multiple times. This issue goes beyond mere inconvenience; it actively detracts from driving safety. The need for constant and sometimes aggressive adjustments to satisfy the system’s requirements is distracting and can lead to erratic vehicle behavior. The irony is stark: a system designed to enhance driving safety and ease is now a source of potential danger and stress. The unpredictability and over-sensitivity of the updated system could lead to dangerous situations, especially on highways or in heavy traffic, where sudden disengagement of the autopilot can be particularly risky. As a driver, I now find myself more focused on keeping the autopilot engaged than on the actual driving conditions and surroundings, which is surely contrary to the feature’s intended purpose. You need to investigate this issue as a matter of urgency. Adjustments are necessary to prevent potential accidents and ensure that the tech helps, not hinders.

My car is affected by a product defect where Tesla Model 3's which have blind spot monitoring cameras (all model 3s) during a certain time frame have extreme glare at night, preventing the driver from safely using the blind spot monitoring system. The camera is basically unusable, and this is a feature that drivers rely on to safely view their blind spots. I have done lots of research online, and Tesla repeatedly refuses to fix these product defects under warranty. There are tens of thousands (potentially hundreds of thousands) of affected vehicles, and I'm sure that Tesla is trying to avoid a costly recall, so they refuse to admit that this is a major safety issue and say that it is "within spec". If you search "tesla model 3 side camera glare" in Google, you'll find that there are thousands of complains on forums. Tesla fixes some under warranty, and refuses to fix others. I am aware that there was a recall on Model 3 vehicles already due to the rear view camera going out, which the NHTSA deemed a safety issue. I would argue that the defective blind spot cameras are a FAR more significant safety issue, because the rear-view camera is used almost exclusively during slow speed maneuvering into tight spaces, while the blind spot cameras are used on freeways for high-speed (60-70mph) lane changes. I have reached out to Tesla and requested that they fix this under warranty as it is a major safety issue, and they told me that newer production models have had the problem fixed, but that my model year needs to pay to have it fixed by Tesla. I have attached pictures of my conversation with Tesla to this complaint. Please take action against this company, which is putting its customers at risk because they don't want to bear the financial cost of fixing their product design defect.

I AM STILL WAITING FOR TESLA TO MAKE THIS REPAIR. I HAVE CONTACTED A SERVICE CENTER SEVERAL TIMES AND TRIED TO MAKE AN APPOINTMENT ONCE. TESLA CANCELLED THE APPOINTMENT AND KEEPS TELLING ME THEY HAVE NOT RECEIVED THE PARTS TO MAKE THE REPAIR. IT HAS BEEN OVER A YEAR AND A HALF SINCE TESLA ANNOUNCED THE RECALL AND IT HAS FAILED TO REPAIR MY CAR. MY UNDERSTANDING IS THAT THE HARNESS CAN SHORT OUT AND CAUSE A FIRE.

The contact owns a 2020 Tesla Model 3. The contact stated that while driving 75 MPH at night with self-driving assist mode, the auto high beams lights engaged three times within 20 seconds. The contact stated that the driver ahead of him then decelerated and veered to the side of the road because it appeared that the contact was sending him an alert through the activation of the high beams. Additionally, the contact was pulled over by a Police Officer. The contact stated that the auto high beam feature was distracting to other drivers. The contact stated that he was unable to disengage the feature. The manufacturer was notified of the failure and informed the contact that the driver did not have control over the feature. The vehicle was not diagnosed or repaired. The failure mileage was approximately 70,000.

The contact referenced his concerns about regulation Federal Motor Vehicle Safety Standard: 571-6; not allowing him to open or close the rear windows. The vehicle was not diagnosed or repaired. The manufacturer had been informed of the failure through the App. The failure mileage was approximately 44,300.

The car broke down completely saying that "Electrical system failure". It is immobile. This is dangerous as I was driving on the freeway and it suddenly started showing lot of warnings. Warning included "12v power system failure", "Electrical system failure". "Brakes might not work". Somehow I got in to the garage and the car is now immobile. It does not even lock. It is in the same condition as I parked. Cannot open other doors. This could have been life threatening if it had stopped on the freeway. With the price and safety rating, I never expected Tesla to have total failure.

I was traveling on autopilot in left lane with autopilot engaged on 06/13/2024 on HW 101 at around 9:40am my car all of the sudden slammed on brakes and dropped speed from around 65mph to 30mph before I was able to react and recover speed. Reached out to Tesla on their app about this and they are sending me estimate of $275 to check it out

My car cameras and audio will randomly stop working. Come to find out I took a recording with a timestamp of 1151 pm where I am on the highway and driving. Have witnesses of me driving and the issue. My model 3 logs, as stated by service technician, say my vehicle is "Parked and not being operated". This means I am driving with potentially dangerous and absolutely unreliable software in my car. It shows the car isn't being operated as I am going 60~ on the highway in traffic. My car cameras will go completely black and the audio warnings will go out including the backup sounds which are a required feature. Have contacted Tesla since 2020 and no action except a blanket statement of "a future update"

Was driving Tela on 237 Clavaras Blvd heading towads 880 South just before Abott ave in Mlipitas town California Tesla Model 3 acelrated suddenly hitting the car in fronts, emegency automatic braking did not kick n. Earlier it had autopilot set not sure if it tured off. We were surirsed, my wife was driving and I was i passenger seat none of the safety measure worked in the Tesla. My car is totaled.

When using the adaptive cruise control phantom breaking occurs. This is extremely dangerous because at times cruise is set to 60-75 mph and car will randomly hard brake. If someone was following me this could definitely cause an accident. It’s also so abrupt that it jolts your body forward. I have not taken the car to any service center to reproduce. There are no warnings, no indications of anything visually on the screen that the car would see to cause the breaking. There is also one spot on my route it consistently does this on. This issue has been occurring since early 2021. I take the same route daily for school drop off purposes. It’s a 45mi trip one way. The issue will consistently occur at least twice one way on the trip if I’m using the adaptive cruise system. I do not use auto pilot only the cruise.

We were pulling our car from 5th floor of a parking garage and as the car was coming down the ramp into the 4th floor, it accelerated itself to a high speed, hit another parked car, and came to a stop hitting concrete pillar in the garage. Our car was damaged severely and all the airbags got deployed, smoke came out it. We are still in shock from the incident and it was a traumatic experience for all 4 members of our family including two of our children. It would have been catastrophic had there been another person around or if something more had happened. Our car unexpectedly accelerated and none of the warnings worked. Tesla car usually gives us warning (a shrilling beeping noise) when an unexpected obstacle, be it another car or a trash can or a human, is within a short distance of the vehicle. This warning sound comes up even when no automatic driving feature is used. But on that day, the warning sound did not come and we think the vehicle may have malfunctioned. When we searched in the internet, we found many incidents of unexpected acceleration with Tesla Model 3.When we brought our concern to the insurance, we were asked to report to NHTSA. The car is now with insurance and the manufacturer has not looked into it as we haven’t informed them yet. The car was in excellent condition and there were no warning or failure lamps on before the incident. This warrants your attention and hence we are writing this to you. Please advise us the next steps and if we need to preserve the car to retrieve any data recorded in the vehicle. For the safety of other drivers and pedestrians, this needs to be thoroughly investigated. Kindly do the needful.

While operating TACC with lane keeping assist, I experienced multiple iterations of phantom braking when the road and weather was clear ahead. This phantom braking could potentially cause other drivers to collide with the vehicle that unexpectedly braked in the middle of the highway. This problem has been a common issue with multiple people taking their cars to the service center to get checked out with no problem identified. There are no warning lights or indicators that the Tesla is going to phantom brake on the highway. Recommend having Tesla provide information regarding this phantom braking.

While driving on the freeway, my 2020 Model 3 “phantom braked” multiple times while using the cruise control. No autopilot for lane keeping assistance. There were no cars in front of me or any hazards. The pavement was pretty new. I was using this feature earlier in the trip with no issues then it started all the sudden with multiple incidents in a short amount of time.

Had cruise control on and “phantom braking” occurred about 10 times on a 22hr trip from California to Oklahoma. Very scary. Car went from 80 to about 40mph in seconds. How is this even possible for Tesla to have these vehicles on the road? I’ve only had the car for 2 weeks.

When driving on a longer trip, I have repeated sudden deceleration and sometimes harsh braking when using cruise control. This has also happened in normal daytime hours with no traffic around me on rural interstates. On my last trip, I had to keep my foot on the accelerator to keep the car at a constant speed. It is unnerving to me and very scary for passengers. It has made it impossible to use cruise when I have passengers. Tesla has said there is no hardware issue and have declined to service the car.

Cars computer froze and I was stuck in cruise control. Hitting the brake would not disengage. Pressing up on the stalk would not disengage. I could manually slow/accelerate the car with the brake/accelerator but after letting off the car would continue on at that speed while everything else remains frozen. No turning signal function. I was afraid to attempt to stop the car fully as it would not disengage. Screen eventually went black and reset after what felt like forever. Functions returned to normal.

The tail light has stopped functioning, and I believe this failure is due to condensation build-up within the light assembly. When I previously reported the condensation to Tesla, I was informed that it was normal and not a cause for concern. However, it now seems that this persistent condensation has led to the failure of the tail light. FSD Beta 11.4.9 - A concern documented in NHTSA Complaint ID 11555478 and subsequently remedied with later releases has returned in the current FSD Beta build 2023.44.30.14. When engaging FSD ,the display screen enters an AUTO brightness mode and requires driver intervention to adjust brightness back to the driver's desired setting, as described by the following. When FSD is engaged, display AUTO brightness is activated and the screen adjusts up in brightness from the previous setting, which is often too high. The driver's attention is then taken away from road and traffic ahead while the screen is reset to the driver's preferred setting. In the current build the following anomalies also occur. When FSD is engaged and the display is set to AUTO brightness, if the driver opens the Display menu and adjusts brightness down, that driver setting is held even though the Display brightness is still shown to be in AUTO mode. However, if after activating FSD and the AUTO brightness is engaged, and the brightness is then adjusted down using the left scroll wheel, the display remains in AUTO mode and the screen is automatically adjusted back up to the AUTO set point. This is true when the driver's preferred setting is lower than the AUTO set point, but not true if the driver's preferred setting is higher than the AUTO set point. When the initial driver setting is higher than the AUTO set point, the screen remains at the driver's preferred setting. While these anomalies are not safety concerns, per se, they do require that the driver's attention is diverted to resetting the desired display brightness. If, as if often the case, FSD is disengaged and then reengaged, the process described above is repeated. In the course of a 30 minute drive this can require that the driver adjust screen brightness multiple times, sometimes repeating the adjustment process within minutes after prior adjustment.

Activating FSD Beta Version 2023.27.7 switches the Display brightness to AUTO and to a brightness that is higher then the level set by the driver. In a darkened vehicle at night the Auto setting was too bright and a distraction to safe driving at night. I then spent 2-3 seconds with my attention away from the road ahead as I reset the brightness on the display screen to the comfortable level. After disengaging FSD Beta the display remains at the brightness level set buy the driver. The next time FSD Beta is activated the display brightness is once again set to AUTO and the distracting process of taking the display out of AUTO and adjusting the brightness to a level comfortable to the driver must be repeated. Activating FSD Beta puts the High beams into AUTO and allows the system to use the high beams when the Vision system requires it. This is excellent and works well. The display brightness is a driver preference and adjusting the display brightness is not needed for the vision system to operate effectively and should remain set at the driver's preferred setting so as to no be too bright and so the driver does not need to repeatedly adjust the brightness each time FSD Beta is activated.

FORWARD COLLISION AVOIDANCE,LANE DEPARTURE

There have been recent reports of Tesla FSD accidents that are now under investigation by NHTSA relating to crashes that occurred under difficult visibility conditions. A video I recorded several years ago while driving into a fog bank in NW Washington, about an hour North of Seattle, is linked below. Seeing the fog bank ahead and with no traffic in front or close behind me, I let the car proceed into the fog until the system triggered a takeover alarm. Usually, at this point several years ago, when the driver wanted to disengage FSD, the driver did so by raising the gear selection stalk. Raising the gear stalk disengaged TACC ONLY in this incident, and Autosteering remained active. (see: [XXX] ) After disengaging TACC, the car was slowed by regenerative braking but still under Autosteering control. Turning the steering wheel finally disengaged Autosteering, but that occurred second and was delayed by the driver's confusion over why raising the gear stalk wasn't enough to stop the alarm. Applying the brakes would have disengaged FSD completely - but that wasn't wise with low visibility. I tried re-engaging FSD briefly while still in fog (not shown in this video), and the system refused. This brings to mind a previous documentation video where, with FSD engaged, turning the wheel disengaged *Autosteering, but *TACC remained active. Again, the driver was not initially cognizant that TACC was still active (see: [XXX] ). I believe a subsequent recall remedied this 2nd example, but perhaps the fog bank-type takeover alarm was not affected by that recall remedy. Finally, I have one more video documenting the ability of FSD to see under challenging conditions, in this case, light rain, heavy rain, and eventually hail. No alarms were triggered (see: [XXX] ). When driving in "Autopilot" mode which uses adaptive speed control, the car suddenly slows down without any warnings. This happens even when there is no vehicle in front of the car and therefore slowing down is not expected. This has happened several times while driving on freeways. If it weren't for my immediate reaction, serious multiple-car accidents would have happened. My reaction has been to quickly disengage the "Autopilot" system and manually accelerating to avoid rear collisions. The other issue is the car suddenly steer the wheel and drag the car to the side even when there is no reason to do so. In such cases, I have to forcefully steer the wheel to the opposite direction to avoid collision with other cars moving on either side, or parked cars on the streets.

I am writing to provide feedback on the recent mandated Tesla Full Self Driving software update (recall). When I keep both hands on the steering wheel, I now get constant warnings. Specifically, even when I drive with both hands firmly on the wheel at 9 and 3 o'clock and maintaining what I feel is adequate control, the system nags me with disruptive alerts. It tells me to apply pressure and flashes visuals that my hands are not properly positioned, despite both remaining clearly in contact with the wheel the whole time. I typically drive responsibly with proper grip, yet these frequent notifications are frustrating and stress-inducing. The sensors seem to be calibrated too sensitively if they are detecting grip issues when both my hands are correctly and securely holding the wheel. Prior to the mandated update, I didn't receive nearly as many warnings. The update was not helpful and makes driving more stressful and less safe if anything. I understand the goal is to enforce safe driving, but the current warnings are excessive even when drivers are maintaining proper hand positioning. Perhaps the regulations could be relaxed so alerts only activate when grip is legitimately inadequate, rather than routinely throughout normal driving. Please let me know if any further details on my experience with both hands on the wheel would be helpful. I would be happy to provide additional information to aid review of this system. I was driving on the Garden State Parkway in New Jersey in the right hand lane. I was using basic auto pilot and needed to pass a pick up truck in front of me. I disabled auto pilot and went into the left lane. I re-engaged auto pilot and the vehicle took a hard right and hit the truck. I heard the audible chime once it made contact with the truck indicating that auto pilot disengaged. My car went back into the left lane and I took control. We moved to the shoulder and waited for state police to arrive. Luckily no one was injured. A police report was filed.

On or about June 15, while driving my TeslaI received a notification from my Tesla that the automatic Emergency Braking was disabled. There were also notifications that Autosteer was unavailable, and so was cruise control. I made an appointment with a Tesla repair center for repairs to the car under warranty. My initial appointment was postponed to enable remote diagnosis. On July 6 I brought my car to a Tesla repair center. After looking at my car, Tesla informed me that the car had no then current alerts or faults. They "were able to confirm this is known firmware characteristic. Tesla is working to to resolve this in a future firmware update." It's not clear to me what they are saying and whether this was a false positive from the firmware error reporting system or a transient fault that they could not reproduce. In any event, the fault reporting and the failure of Tesla to repair its firmware for a 2 and 1/2 year old car is a concern to me.

The contact stated while her granddaughter was reversing, the rearview camera failed to turn on, causing the rear end of the vehicle to crash into a cement post. The contact was unsure whether there was a warning light illuminated. There were no injuries sustained. A police report was filed. The vehicle was towed to a Tesla-certified autobody mechanic but was not diagnosed or repaired. The vehicle was then towed to the contact's residence. The contact later received notification of NHTSA Campaign Number: 21V00D000 (Electrical System, Visibility, Back Over Prevention) however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the failure but did not provide any assistance. I took delivery of this Tesla Model 3 last month and have been using it often for my commute. Tesla’s “Traffic Aware Cruise Control” is dangerous and has put my life at risk multiple times now when driving on the highway at night. It slams on the brakes as if it is about to crash when nothing is ahead. It also slammed the brakes when passing a semi truck that was accelerating from being stopped on the shoulder. If anyone was behind me, they would have crashed into me. This “phantom braking” only happens at night. I would disable it immediately but it is not able to be disabled! This is the most frustrating part. I have a long commute on the interstate at night and really need cruise control, and now I am constantly worried when commuting that the car will slam the brakes. The fact that I can’t revert to an old fashioned cruise control with no sensors and no cameras is absurd and extremely dangerous. What component or system failed or malfunctioned, and is it available for inspection upon request? I don’t believe anything “failed”. I believe Tesla’s software is not ready for the real world and this “traffic aware cruise control” should be a beta feature that drivers can use at their own risk. How was your safety or the safety of others put at risk? Unexpected, random slamming of the brakes at highway specs on the interstate at night. In all situations that the “phantom braking” has happened, I would have been rear ended if someone was following behind me. Has the problem been reproduced or confirmed by a dealer or independent service center? No Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No, as there has been no crash yet Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warnings, no messages, nothing. Just sudden braking when using “traffic aware cruise control”.

on July 19,2022, when I was driving on driveway, at less than 5 miles, I tried to stop to drop my wife and applied brake. Instead of brake, due to computer mal function, it accelerated & hit the garage door, broke the garage door, door fell on the car parked in the garage, damaged that car,and tesla automatically reversed. while reversing brake wasn't working too so I tried to put in drive & brake still was not working so to stop the car for safety, I hit the curb in my front yard All airbags were deloyed, inuring both of us. All doors were locked & we couldn't open any of them. We could open windshield. We shouted HELP and my neighbor came & open the door from outside so we could get out otherwise we both could be dead inside the car

I WAS DRIVING STRAIGHT ON A HIGHWAY USING TRAFFIC AWARE CRUISE CONTROL WHEN THE CAR SUDDENLY BRAKED HARD. IF SOMEONE HAD BEEN DRIVING BEHIND ME TOO CLOSELY I WOULD HAVE BEEN REAR ENDED AT A HIGH SPEED. I'VE SEEN ONLINE THAT PEOPLE REFER TO THIS AS "PHANTOM BRAKING," AND IT SEEMS TO BE A COMMON ISSUE WITH TESLA CARS.

Dear NHTSA, I am contacting you regarding campaign #23V-838 and the recent Tesla software update, which I believe has compromised the safety features of my Tesla Model 3. For over two years, I’ve trusted my Tesla’s capabilities, notably its autopilot feature. However, the latest update has introduced a wheel detection system that’s overly sensitive and disruptive. Previously, my driving experience was smooth, responsible and safe, with hands on the wheel and autopilot assist for when I needed to make blind lane changes, allowing me to look over my shoulder briefly at the blind spot while the wheel kept straight without rear ending anyone. Now, with this mandated update, I’m forced to make frequent, forceful adjustments to the steering wheel. I can’t look away from the front of the car even briefly to check my surroundings. This constant intervention and forbidding drivers from looking around to check for safety is distracting and dangerous, leading to abrupt autopilot disengagements and brief rapid ways in which the car behaves as the car disengages and throws you into manual when you weren’t expecting it. This isn’t just about inconvenience; it’s a safety hazard. I’m now more focused on maintaining autopilot than on the road and surrounding conditions. This undermines the very purpose of the feature. As someone who’s always vigilant about my surroundings, especially at intersections, I find this update counterproductive. It forces me to turn off the feature, leaving me without the assistance that I’ve come to rely on for safer driving. I urge you to review this matter with utmost priority. The balance between driver engagement and the autopilot system’s safety has been significantly disturbed by this update. Necessary adjustments are needed to prevent potential accidents and ensure that this technology continues to enhance driving safety and efficiency. Thank you for addressing this critical issue.

The turn signal is attached to the left and right side blindspot camera. When using the blind spot camera at night, the turn signal light “bleeds” into the camera blocking your view. So it becomes a safety issue when switching lanes at night, especially on a dark freeway. I have contacted Tesla and stated that “the camera performance on older vehicles is neither a defect of materials nor workmanship… which is has been enhanced in newer vehicle production. “

The Tesla rear facing repeater cameras show the driver's blind spot when changing lanes. The cameras are also used during automatic lane changes when the car is in drive-assisted (FSD) mode on the highway and in city (FSD Beta). The cameras have a manufacturer defect. At night, light from the turn signal led is blinding a large part of the vision field of these cameras. This renders the blind spot vision for the driver but also the computer assisted automatic lane changes for the car partially obstructed. Tesla has acknowledged this defect by redesigning the rear facing cameras so the blinding is remedied. Tesla is refusing to fix this issue under warranty and requires owners to pay for this safety fix. Tesla's statement on this issue: "Hi Stephan! The blind spot camera performance on your vehicle is neither a defect of materials nor workmanship. It is a characteristic of the product. And it's a characteristic that has been design enhanced in newer vehicle production. If you wish to upgrade to newer camera technology, you have an option to purchase the upgraded cameras and have them installed." I am having my cameras replaced, and will pay for it. I believe Tesla should, however, seeing how this is driver safety related even if they are unwilling to acknowledge this.

After loading our two children ages 2 and 5 in to vehicle and securing in car seats, the car doors were closed and would not reopen. The children were unable to open the doors themselves. The trunk and other entries did not open. There was no obvious way to open the doors or the issue the car was experiencing. The battery cover would not open. Eventually after I searched the Internet I determined that the 12V battery died (no warning whatsoever, doors worked before loading children) and this caused all occupants to be detained in vehicle as the emergency release is the mechanical means of opening the door and this emergency release is not intuitive or obvious. Within 20 minutes I was able to use an arcane hot wiring method to unlatch a door with a spare 12 V source and a hidden 12 V port not immediately obvious either. Thankfully not in setting of an emergency, not a sunny or excessively warm day and had internet access to research this event. If any of the above were true, life limb and property loss would easily have occurred.

My Motor Control Unit failed. No reason given by Tesla Service Center. It simply just died. They cannot find a reason, and they also said I didn't do anything to cause the MCU to fail. Service Center agrees this is a rare event. Likely got a really faulty MCU to start with. I bought my car in 2020, so just over 4 years later my MCU died.

The problem is my car has been in the shop six times for an issue that Tesla claims it is unable to fix. I started smelling a burning smell (smoke coming from my air vents anytime the air is on) and brought it in. The first three times they were unable to find anything the fourth time they noticed debris That was embedded in the heating fins. They replaced that unit. The fifth/sixth time they actually took the HVAC unit out and noticed the entire plastic underneath was melted and warped. Since the smell has continued and I’m worried that it’s dangerous, I had the air quality tested. The formaldehyde levels were 50 times higher than the EPA recommends as a safe level. (HCH0 level of 6 ppm) I’ve submitted all this to Tesla and they said they are currently working with their legal team to figure out if a buy back as possible. The New York Attorney General told me that the car is not viable for a lemon law due to the fact they didn’t do three chances to fix it in 90 days. (The first 2 appointments were in the 90 days but the 3-6th were just past it) The car is not safe to breath air in and my lungs hurt from even spending a few minutes in the car. I’m worried about the effects it has on my health and I feel morally wrong to sell it to anyone else. Please help.

The contact's husband owned a 2020 Tesla Model 3. According to a police report, while traveling at 70 MPH, the vehicle veered across three lanes of traffic and hit a light pole. The contact was wearing a seatbelt and the air bags deployed upon impact. The paramedics arrived at the scene; her husband was intubated and taken to the hospital. Her husband was pronounced brain dead due to injuries suffered in the accident and passed away two weeks later. According to the police report, witnesses stated that no effort was made by her husband to avoid or slow down prior to the collision. The contact disputes this claim and stated that her husband was not a reckless driver and would not have intentionally hit a light pole. The contact believed that the cause of the accident may have been related to a mechanical failure and would like an additional investigation conducted about the accident. The vehicle was towed to an independent tow yard and destroyed. The manufacturer had yet to be notified of the failure. The failure mileage was unknown.

I received a proactive service alert from Tesla car stating: Front Passenger Safety Restraint System Issue. Tesla wants to charge me to fix this and said there may be issues with the restraint system, such as seats belts or airbags if I don’t pay out of pocket to fix it. This seems ridiculous I have to pay to fix this safety concern.. please issue a recall for this… seems a lot of people are getting this after researching on google and Reddit. Thanks

My 2020 Tesla Model 3 has displayed a "Front Passenger Safety Restraint System Fault - Service is required" error since June 2023. This error has persisted for well over a year despite troubleshooting attempts, such as restarting the vehicle and checking for obstructions. Tesla service quoted over $400 to repair this issue, but I believe this is a basic safety-related defect that should be covered by the manufacturer, as it likely disables the front passenger airbag, posing an unreasonable risk of injury or death in a crash. This appears to be a widespread issue, as I’ve seen similar complaints from other Tesla owners online. I request that NHTSA investigate this as a potential safety defect for a recall.

In March 2023, my vehicle was in a major front end collision on the passenger side and the airbag did not deploy. In January 2024, a warning light appeared on my car’s screen that reads “Safety Restraint System Fault” I then took the vehicle to a Tesla service center who determined that the front passenger airbag and OCS needed to be replaced to maintain compatibility….

For the fourth time the seat sensor has failed in the driver seat. when the seat sensor fails the car no longer auto connects blue tooth media play from the phone but also no longer detects an occupant in the vehicle. this causes the vehicle to go in to park while driving at low speeds without a seat belt, i.e. moving the vehicle in the garage, and it also prevents the car from recognizing when the occupant has left the car and leaves it powered on the whole time. the first three times they repaired it under warranty and it has not failed for the fourth time in four years. I have asked if the sensor function can be deactivated and they said that it can not and needs to be continuously replaced. the service representative told me that the item is secured by adhesive and getting in to and out of the car can cause it to bend and kink which is what happened to mine - and he also said given the heat in Florida its understandable the adhesive wont stick.

The complaint was filed on behalf of the contact's daughter. The contact's daughter owned a 2020 Tesla Model 3. The contact stated that while the contact's daughter was driving at 35 MPH, the brake pedal was depressed; however, the vehicle failed to stop as intended. The contact stated that the contact's daughter stated that the brake pedal felt spongy and unusual. The contact's daughter stated that the brake pedal was depressed to the floor; however, the vehicle still did not stop. The contact's daughter enabled the emergency braking system; however, the vehicle still did not stop. The contact's daughter beeped the horn to alert the nearby driver, and the dash cam independently activated. The contact stated that because of the failure, the contact's daughter's vehicle rear-ended the vehicle in front of the contact's daughter's vehicle. The air bags did not deploy. The contact stated that after the crash, the emergency braking system failure warning light was illuminated. The vehicle was towed to a local tow yard. The vehicle was condemned as a total loss by the insurance company. A Police report was filed. The contact's daughter was treated at the local hospital on the following day. The contact's daughter sustained headaches, whiplash, and a concussion. The manufacturer was not made aware of the failure. The failure mileage was approximately 49,000.

Automatic breaking was initiated as I was pulling in a driveway as I got closer to a large trashcan. The braking was aggressive and I now have concerns that it might've given me minor whiplash as it was so sudden. I have pictures of where I parked. I was about 6-8 feet away from the trashcan when the car automatically braked. I've pulled into this spot numerous times and never had this reaction.

My car did a Phantom break while I was driving on President George bush Turnpike E at the Dallas North tollway exit . I was taking the exit with in the speed limit of 40 miles per hour and was keeping the distance from the car in front. Suddenly the car did an automatic break and car slow down significantly. I got hit from behind and had damage to the bumper and trunk. Emergency break feature went out and an alert came. It is very concerning that the car stops in the middle of road by itself and being hit from behind. I would like an answer from Tesla. I have the dash cam footage of the accident

The car has regenerative breaking where the car is supposed to slow down when you lift your feet off the accelerator. Since a new update, some times (10%), when you lift off the accelerator the car does NOT slow down and will maintain speed. This inconsistent behavior is extremely dangerous and you don't know if the car will slow down or not. Tesla refuses to acknowledge this is an issue.

I was driving my vehicle. It said there was an issue with code "RCM_a056 Front Passenger Safety Restraint System Fault." Many older model 3's are having this issue and with the quote from tesla, they would need to perform a retrofit for the electrical wiring system.

An alert stated: "RCM_a056 Front passenger safety restraint system fault." Upon bringing car to Tesla service center, Tesla stated: "Confirmed concern through vehicle logs. Found vehicle needing passenger ocs retrofit to resolve issue. Customer declined repair." They wanted over $1,200.00 for a faulty seat sensor because they claimed they couldn't replace the seat sensor without replacing the entire airbag system on the passenger side. They explained to me that they changed the airbag systems in their newer Model 3s and refused to stock or replace components in their older Model 3s.

I received a message on the screen saying there was a problem with the OCS and that it needed to be serviced immediately. I take it to Tesla service and they tell me they will need to do the following: 1. Replace MODULE - OCS FILTER 2. Retrofit SBR in OCS Equipped Front Passenger Seat - Remove and Replace 3. Replace FIRST ROW SEAT HARNESS 4. Replace FIRST ROW - SEAT BELT REMINDER SENSOR BOX CLIP 5. Replace FIRST ROWOCCUPANCY SENSOR 6. Replace FIRST ROWHARNESS ASSEMBLY -OCCUPANTCLASSIFICATION SYSTEM - RIGHTHAND 7. Replace Front PassengerAirbag And Update VehicleConfiguration - Remove and Replace To have all of this done, I paid $1,850. Nothing is covered under warranty. I have 64,000 miles on my car. It has never been in a wreck and I have taken good care of it. From what I understand, this is a common problem with Tesla’s.

I have an alert that says "Front passenger safety restraint system fault". I took the car in to Tesla Service in late April. Was told it would be about $270 to replace the sensor in the seat that detects it someone is sitting in it. They told me that when the parts people went in, that wasn't the issue and that to fix the sensor they thought it was they would have to tear the whole seat apart and that takes too long so instead I need to buy a whole new seat at the cost of $3000. I didn't have time to deal with it further at that point and the seat was back ordered anyway. I asked if the issue would affect the operation of the airbags or seat belts thus causing a safety concern and was told that it would not. That the car was still safe. I took the car back without repair. I have since noticed that when a passenger is in the seat, the symbol that states that the airbag is not engaged is still on. So it would appear that the airbag is not working properly. I called Tesla today to inquire and ask for clarification if this is a safety concern and if the airbag is not working. They told me a manager would call me back. They said there is not a Tesla wide recall- even though it would appear that this issue is causing the airbag not to work. From research online it seems like other customers have experienced the same or similar issues.

The car has message saying "Safety Restraint System issue" . There has been no event to cause this. No recent MVA or any stress or damaging incident to the seat belt or seat in general. Reviewing this on the internet, this is a common defect on Tesla's. The company won't fix it unless you pay them to do so which is crazy considering it is a defect in their safety system. And a well known one to them from my communication with their Tesla service centers which have a monopoly on fixing Tesla's. He said there would have to be a recall. So that is exactly what needs to happen from the NHTSA.

The passenger-side seatbelt system in my Tesla Model 3 (bought Dec 2020) has been malfunctioning repeatedly. The vehicle displays a "Front passenger safety restraint system fault” warning, even when no one is on the passenger side. This issue has persisted despite multiple repair attempts by Tesla during the warranty period, but they have been unable to resolve it. Safety Concern: A malfunctioning seatbelt system is a serious safety hazard, as it could result in the seatbelt failing to function properly in an accident, potentially putting the passenger at greater risk of injury or death. Additionally, the fault warning may prevent proper airbag deployment or create a distraction for the driver while operating the vehicle. Attempts to Fix & Verification: Tesla has attempted multiple repairs but has not been able to fix the issue. According to Tesla, this is a common issue with this model, which raises concerns about a wider systemic defect. The problem is persistent and ongoing, making the vehicle unreliable and unsafe for passengers. Warning & Symptoms: The seatbelt fault warning appears randomly but frequently while driving. Tesla attempted to fix this issue as early as June of 2023, and has remained unresolved despite 4 or more service visits. We might have asked Tesla to work on this issue even earlier and more times since we're unable to pull up repair history before this due to their system error. This issue requires immediate investigation to determine if it is a broader safety defect affecting other Tesla vehicles and if a recall or mandatory repair is necessary.

It seems that there might be a defect with the car, given the number of people impacted and the severity of not having proper safety restraint. I am very careful with my car, and it has not been involved in any accidents, so I think Tesla should do a recall to make sure all Front Passenger Safety Restraint System is functional

After the OTA software updated to 2021.44.25.2 there were several changes to the graphic user interface and software system. Two direct issues of the update are: 1) The rear and side cameras that assist during backup now take longer to activate and have very jumpy (laggy) video feeds. So much so that the driver can backup at least 10 feet at low speed before an updated image is shown. This causes a very unsafe condition when backup up as there are multiple blind spots in the car. 2) The simplification of touchscreen controls requires the driver to be more distracted by having to navigate additional menus to turn on wipers, climate control, the radio, and safety features like the dashcam. Waiting on an appointment with Tesla but that is at least 20 days away.

Subject: Urgent Complaint Regarding Power Steering Failure in 2019 Tesla Model 3 Hello NHTSA, I am writing to file a complaint regarding a very dangerous situation I recently experienced with my 2019 Tesla Model 3. While driving on the freeway at approximately 65 mph, the power steering failed unexpectedly. I struggled significantly to change lanes in order to exit the freeway, and it felt as though my vehicle was going to collide with other cars. After managing to get off the freeway, I nearly hit the guardrail while attempting to turn onto a side street, creating a hazardous situation. I am deeply concerned about the potential consequences; for instance, what if I had struck a pedestrian, or if the car had collided with another vehicle at high speed? I live on a winding mountain road, where I could have easily driven off a cliff. Turning the car off and back on temporarily resolved the issue. However, there are numerous complaints regarding steering rack failures with Tesla vehicles. Tesla has quoted me $2,700.00 to fix what appears to be a known manufacturing defect. They are aware of this potentially hazardous situation but are not taking responsibility for the repairs. This is a serious safety issue, and I urge Tesla to accept responsibility and cover the repair costs before someone is seriously injured or killed—if such an incident has not already occurred. My Model 3 intermittently has the steering wheel locked in position, which sometimes resolves temporarily after rebooting the car. I always get a steering assist reduced error on the car, regardless of rebooting. It also no longer has lane keep capabilities or Autopilot, which involve the car automatically moving the steering. I have not gotten this inspected by a dealer or service center. The issue first appeared yesterday, after I tried to pull out of a parking lot, but I had trouble since my wheel would not turn. I will be going to a service center next week. After the errors began, I have not meaningfully driven the car, since locking of the wheel while driving is very dangerous. A handful of errors showed up on the screen (Cruise Control disabled, Traction Control Disabled, Stability control disabled, Steering assist reduced), and I did not have any regenerative braking applying. I returned safely to my parking spot. After multiple reboots of the car (pressing both wheel buttons), I found that sometimes after putting my car into Drive, my steering wheel was locked. Other times, I could turn my steering wheel without issue, but I would always receive the Steering assist reduced error.

Essentially, I find the lack of emergency, unpowered door release for the back doors a little concerning. There is no way to open the door in the event that all power to the door latch fails. Even in newer models (at least of 3/Y/Cybertruck, that i know of) the emergency release is buried under the cupholder liner in the rear seat. Here is a youtube video showing a homebrew retrofit. It seems like something like this should probably be provided to owners of this model of car. The internals of the door seem to make this feasible. [XXX] The front seats, on the other hand, have an easily accessed and intuitive emergency door release. I imagine that the rear seats differ from this model because of child safety locks. Just removing all emergency handles from the door seems like a pretty bad solution. I further understand that there are situations in which even an emergency door release will fail to open the door, and a glass break device is probably the ultimate recourse to this situation. But rear seat emergency egress really seems like a blind spot in NHTSA requirements at least. I shouldn't have to do an airline style safety briefing to my rear passengers in a consumer vehicle. I am writing to report a repeated and potentially dangerous defect in my 2020 Tesla Model 3. On four separate occasions, the front trunk (frunk) latch has opened on its own, without any input from me. This has occurred while parked in my garage and poses a serious safety risk, especially at highway speeds where the hood could flip up and obstruct the driver’s view. I am aware that Tesla has issued recalls for this exact problem involving the frunk latch mechanism on certain Model 3 vehicles. However, I have been told by Tesla that my VIN is not included in the recall, despite my car exhibiting the same malfunction. I brought my vehicle to the Carlsbad CA Tesla service center, but they found no issue and have since told me they consider the matter closed. I also want to note that I previously experienced a failure of the rear camera cable, which Tesla also initially claimed was not part of a recall—only to later repair it under warranty when the problem became undeniable. I believe this hood latch issue poses a critical safety hazard that may be more widespread than Tesla currently acknowledges. If the frunk opens while driving, it could cause a catastrophic accident. Tesla’s dismissal of this issue leaves me driving a car that could endanger myself and others on the road. I respectfully request that NHTSA investigate this defect and consider whether the existing recall should be expanded to include additional vehicles like mine. I would also like NHTSA to contact Tesla and direct them to address the situation in my vehicle…to do more than say “we could not reproduce the reported issue." The contact owns a 2020 Tesla Model 3. The contact stated that while driving 25-35 MPH, all four windows slightly went down independently, wiggled and made clicking sounds. There was no warning light illuminated. The contact stated that the sound had become unbearable. Additionally, the contact stated that the front driver's side and passenger's side doors and the rear driver's side door failed to close securely. The door chime reminder engaged. The contact also stated the vehicle would shift into park (P) and would jerk when attempting to drive due to the failure. The contact stated that the doors required to be slammed in order to close securely. Additionally, the contact stated that the vehicle shifted into park (P) and stopped abruptly while driving. The contact stated that the failure also occurred while driving downhill and while at a traffic light. The vehicle was taken to a Tesla Service Center several times, and the windows and doors were readjusted; however, the failure persisted. The vehicle was taken back to the Tesla Service Center, where the window trim panel and the door trim panel were readjusted; however, the failure increased. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 14,000.

I am experiencing persistent front suspension issues with my Tesla Model 3 Performance. The vehicle has been taken to a Tesla Service Center three times, yet the problem has not been resolved. The issue includes: Unusual noises (clunking, knocking, or creaking) from the front suspension. Instability or poor handling at various speeds. Repeated failures of suspension components despite repairs. Each time, Tesla service has attempted repairs, but the issue continues to persist, raising serious concerns about the safety and durability of the suspension system.The vehicle is currently at Tesla service center as of March 4th 2025 and they are working on the same problem for the 3rd time.

To Whom It May Concern at the National Highway Traffic Safety Administration (NHTSA), I am writing to urgently express my concerns about the latest software update for the Tesla Model 3’s autopilot system, which, in my view, significantly compromises safety. As a Model 3 owner for over two years, I have generally found the vehicle and its features to be reliable and safe. However, the recent update has introduced an overly stringent hands-on-wheel detection mechanism that is not only inconvenient but also potentially hazardous. The new update requires frequent and often forceful interaction with the steering wheel to assure the system of driver presence. This change is drastically different from my previous experience, where I received only one hands-on-wheel violation in two years. The current sensitivity of the system disrupts the smooth operation of the autopilot, leading to frequent and abrupt disengagements. I have found myself struggling to maintain the system’s activation, inadvertently causing the vehicle to exit autopilot mode multiple times. This issue goes beyond mere inconvenience; it actively detracts from driving safety. The need for constant and sometimes aggressive adjustments to satisfy the system’s requirements is distracting and can lead to erratic vehicle behavior. The irony is stark: a system designed to enhance driving safety and ease is now a source of potential danger and stress. The unpredictability and over-sensitivity of the updated system could lead to dangerous situations, especially on highways or in heavy traffic, where sudden disengagement of the autopilot can be particularly risky. As a driver, I now find myself more focused on keeping the autopilot engaged than on the actual driving conditions and surroundings, which is surely contrary to the feature’s intended purpose. I urge the NHTSA to investigate this issue as a matter of urgency and safety.

THE AUTO STEER IN MY TESLA MODEL 3 FAILED TODAY WHEN THE CAR WAS TURNING ON A CURVY DOWNHILL ROAD. I BELIEVE IT'S BECAUSE THE LANE MARKINGS ON THE RIGHT SIDE ENDED JUST BEFORE THE CURBING STARTED. THE CAR SUDDENLY TURNED RIGHT AND HIT THE CURB BRUSHING THE RIGHT FRONT WHEEL. IT WOULD HAVE BEEN A FATAL ACCIDENT IF I HADN'T TAKEN OVER. AUTO STEER TECHNOLOGY IS NOT YET READY FOR USE ON ALL THE ROADS AND IT SHOULD NOT BE INCLUDED BY DEFAULT IN ALL THE TESLA MODELS. MY CAR DID NOT BRAKE BECAUSE THE CRUISE CONTROL WAS STILL ON. Rear windshield developed a stress crack. No incident was involved, it just suddenly appeared. It ran about 1/3 of the rear window. I was concerned the glass would fall in on infant passenger seated in a car seat in the back seat. This appears to be a common glass problem for Tesla.

This is regarding the computer and the cameras on this car. From the first day that I got this car, I have had the issue that when I use the cameras or the backup camera on this it laggs and this is part of safety issue because I was going to ran over some people when I was reversing. The lagging is very noticibale and I took the car to Tesla multiple times and said is ok but when they send technician to my house, we went for drive and I showed him the cameras and he duplicated the problem. The problem is getting worst and last month the computer was blacked out while I was driving. I need you guys to help me out to either put a recall on this car or return and get my full money back for this junk.

Tesla software version 2022.24.8 that I currently have installed has switched off the vehicles equipped forward facing radar. I can't find any method to undo their software update, I'm unable to find a way to contact a person via their website. I've lost control of my lights and wipers when the vehicle is using Autopilot. Both of which are set to automatic leaving me the driver with absolutely no way to turn them off short of turning off Autopilot which I've paid money for. I'm unhappy that a vital safety feature that I'd paid for (radar) has been switched off by the manufacturer without any means whatsoever to turn it back on. I do not want their software overriding my decisions as the driver, radar needs to be on and I the driver need to have full/complete control over lights and wipers.

WHEN THE VHEICLE IS GOING AROUND A LEFT CORNER, IT VEER TO THE RIGHT AND DROVE ON THE SHOULD LANE. THE SAME THING HAPPEN GOING TO THE RIGHT CORNER. WHEN TAKE OFF FROM THE GREEN LIGHT, SOMETIME IT SLOWLY,SLOWLY,SLOWLY INCREASE SPEED EVEN THOUGH THERE IS NO CAR IN FRONT OF IT. WHEN SLOW DOWN, IT WAIT THE LAST MINUTE AND TURN THE CAR. MY WIFE AND I LIVE ON A TWO-LANE STATE ROUTE IN THE COUNTRY. WE WERE HEADED HOME FROM THE FARMERS' MARKET THIS MORNING AROUND 10 A.M., HEADED DOWN A HILL WITH THREE CARS BEHIND US. THERE WAS AN 18-WHEELER HEADED UP THE HILL. I HAD THE CRUISE CONTROL ON AND TRAVELING 55 MPH IN A STRAIGHT LINE WITH NO CURVES IN SIGHT. WHEN THE 18-WHEELER COMING UP THE HILL WAS 7 CAR LENGTHS AHEAD OF US IN THE OPPOSITE LANE, MY CAR AUTOMATICALLY BRAKED HARD ENOUGH TO BRING OUR SPEED DOWN TO 35 MPH BEFORE I COULD OVERRIDE AND ACCELERATE. MY SPOUSE SAID, "I HATE THIS CAR. IT'S GOING TO GET US KILLED ONE DAY. WE COULD HAVE BEEN REARENDED." I HAVE USED CONVENTIONAL CRUISE CONTROL TO DRIVE FOR MORE THAN 20 YEARS, EVEN IN TOWN, TO KEEP FROM INADVERTINLY SPEEDING. NORMAL CRUISE CONTROL DOESN'T BEHAVE IN THIS MANNER. FIRST OF ALL, I WAS PULLING THE TESLA OUT OF OUR GARAGE. I WAS COMPLETELY OUT MANEUVERING THE CAR TO MAKE IT PARALLEL TO THE RIGHT SIDE OF THE STREET WHEN SUDDENLY, I FELT THE CAR MOVED EXTRA BACKWARD. I have been getting an alert: Front passenger safety restraint system issue/Service is required. It has come on when there is no one in the seat and goes away shortly after I start to drive. I contacted Tesla and they sent an estimate to repair it which includes a bunch of different bolts, Passenger Airbag, Airbag Harmiss Retaining Clip. Also replacing Occupant Classification System (OCS) of Front Passenger Seat while Servicing Supcomponents. List of parts for that is First Row Seat Harness Assembly-Right Hand, First Row - Seat Belt Reminder Sensor Box Clip, First Row Occupancy. All of this will cost over $700 . Aren't these items required safety equipment that they should responsible to maintain? I have seen that there are a lot of other people having the same issue, I think that there should be a recall on this that they shoud have to pay for.

I contacted Tesla and they informed me that my airbag sensor needs to be recalibrated and may jeopardize a child sitting in the seat if the airbag is deployed. I understand that this is a common problem with the Model of Tesla.

The Rear Drive Unit of our Tesla failed on our Tesla Model 3 2020 (2 years at the time of incident). It is no longer available for inspection as it was replaced by Tesla. We had our drive unit fail for our 2020 M3 driving in the middle of what is considered one of the most dangerous freeways in [XXX] . After all the warnings came online, we had 30 seconds before the entire car stopped. Of the warnings (Power Reduced, Acceleration and Top Speed Reduced, Automatic Emergency Breaking is Disabled) there was no indication that the car would shut down. We made it to the exit ramp, but couldn’t make it to the shoulder so we blocked the entire exit. [XXX] was able to come in and help tow our car to a safer location. We took it to Tesla to be serviced and when asking the service rep if there’s any way we could have seen this coming, she just brushed it off as “it happens” and that there was no way to tell. 12v battery failed without warning. Car was parked. Was unresponsive. Could not unlock the car. Able to open trunk and passenger side door only. Drivers side would not open. Touchscreen black no power to Do anything car essentially dead with no warning. There should have been on screen display informing driver of the need to replace 12v battery. There was none. Car needed to be towed. If this has happened while we were in the car we would have been locked in. Tow operator jumped the battery to open hood. Touchscreen restarted but car still unresponsive. Multiple warnings on screen. Tow mode would not work so car needed to be pulled with the rear wheels locked onto tow truck.

There is no manual door opener from the rear seats. In case of a power loss doors would not open. Only way the rear passengers can get out is climb to the front seats and get out through front doors. This is a serious situation in case of accident when power loss can happen.

I am writing to formally report a critical safety incident involving my 2020 Tesla Model 3, where the bumper detached while driving through a puddle of water on Oct 9th, 2025. This occurred on a busy street, and fortunately, another driver alerted me just in time, averting a potentially serious accident. Following the incident, I arranged for my vehicle to be towed to a Tesla-authorized collision center and opened a claim with my insurance company. I have since learned that this issue is recognized internally by Tesla, as they have created a service incident regarding it, but have failed to communicate this to customers effectively. Tesla's lack of accountability is concerning. Despite repeated attempts to address this with their support and service teams, they have deflected responsibility and continue to downplay the seriousness of this matter, insisting I must rely on insurance to cover the costs. This situation raises significant safety concerns, not just for myself but for all Tesla Model 3 drivers. I urge NHTSA to take the following actions: 1. Investigate this safety issue regarding the bumper detachment on Tesla Model 3 vehicles. 2. Pressure Tesla to publicly acknowledge this known issue and take immediate corrective action to prevent future occurrences. 3. Ensure that affected customers are compensated for damages and repairs due to this defect. I appreciate your attention to this serious safety issue and look forward to seeing NHTSA hold Tesla accountable to protect all consumers and ensure vehicle safety. Vehicle must have thought a shadow line in the road from a bridge indicated a wall I was about to hit, as it slammed on the breaks in the middle of the freeway. Car behind had to swerve to avoid. On the same trip, several nags of 'keep eyes on the road' were incorrect, as my eyes were directly on the road, but phone was in hand functioning as a microphone taking voice memos. Car proceeded to shut itself down while in the middle of the freeway, causing me to go slower than traffic and put me in danger as cars behind me had to swerve or lane change to avoid me as I tried to pull off to the shoulder. These forced shutdowns from incorrect 'nag fails' are causing safety issues. Nag failures should not forcibly shut the car down. Not sure if this is being enforced by NHTSA, or why Tesla is choosing to disable their auto steer or FSD when drivers are detected to be failing nags - nags should be there to help drivers, and potentially the nag data/cab video can be used after the fact should a user failing nags be involved in an accident for insurance dispute resolution, but shutting the cars down themselves is causing issues.

Software update was pushed onto my car, after the update the car autopilot ECU no longer works. When the ECU finally failed these features were stuck or are completely broken/not working: - Auto dim rear view mirrors (these were stuck in dim mode, preventing me from seeing in rain or darkened environments such as at night or under shade, 0% visibility under these conditions when changing lanes or reversing) - Rear view camera completely stopped functioning, as did all cameras on the car - Forward collision warning no longer works - Autopilot (ADAS features in general) no longer functioning including lane keep assist - On screen navigation is locked in place where the car failed (screen shows location where the car ECU presumably failed). I am further unsure if the following features function as required: - Airbag deployment - AEB (Automatic emergency braking)

This happened on 2/18/2023. After I charged my car at a local supercharging station, I drove on the local roads for about 2 miles when the car started shutting down. I could barely steer, and the computer monitor stated that the brakes were turning off, and other parts, like cameras, were losing functionality. I pulled over into the local park. I was frightened that my car was no longer functional and that I could be in grave danger driving again. I tried to call Tesla but no one answered. I had to go through the app. My dad came and we found a Youtube video that said to walk out of the car to let it restart. Looks like it happened to another owner. We did that and the car started functioning again. Not sure why it did this after a charge. But it happened again 3 mos later when I was on the freeway. Luckily, I knew what to do from prior experience. But I am mindful of this whenever I supercharge my car. I wait after charging to drive. This shouldn't be the case and Tesla should tell people of this issue. I tried to tell Tesla service when I went in for another repair but they ignored my comments.

In researching this I see it is a common issue with hundreds of thousands of cars affected. And the NHTSA has an open investigation into it. Tesla as usual is denying the problem exists.

In a private parking lot, within line of sight and ample time to respond to problem I used Tesla Smart Summon. Instead of taking an obvious route through the parking lot, it narrowly hit a parked car, then proceeded to drive over a parking barrier and into a wooded field towards me and a crowded food truck area. I was unable to cancel the feature in time to stop the car from the collision as it accelerated too fast and without any initial signs of an issue. Even after canceling through the app, the car tried to continue for a brief period. If anyone was in the area or we were closer we could have been hit. Tesla replaced the cameras but claimed no responsibility for damages, citing I was "in control". There were no warning lamps or indicators of any defects before the incident. I also have the original dashcam footage portraying the incident, which clearly shows numerous safety hazards to the smart summon feature.

Unknown issue with front passanger safety restraint system. Car suddenly notified me of an issue with the restraint system. Before the fault the car was simply just parked in my driveway, then I went into the car and the safety issue appeared. The seat was adjusted for my mom's height the day prior, this might have caused a wire to become pinched and caused the fault due to how the wiring under the seat is set up from manufacturing. Online search results show this to be a common occurrence for cars from 2019-2021 in Tesla.

I am filing a complaint regarding a serious safety issue with my 2020 Tesla Model 3 Standard Range Plus. While driving on the freeway, the car displayed a “Power Limited - OK to Drive” message, followed by “Power Reduced,” and then automatically limited my speed to 60 mph. This happened unexpectedly on a busy freeway, nearly causing an accident with a car behind me. The message advised that “exiting and re-entering the vehicle may restore operations.” I took the freeway exit and exited the car, re-entered, and resumed driving, but the error returned, reducing my speed further to 58 mph, and at one point, down to 7 mph. This behavior makes the car unsafe for freeway driving, as the speed can drop suddenly, putting me at risk of a collision. This issue persists on city roads as well, with speed reductions happening within a mile of driving, restricting the speed anywhere between 7-60 mph. I scheduled a service appointment with Tesla on 10/12/2024, but the earliest available date is 11/1/2024. I contacted Tesla’s customer service to prioritize my appointment or perform a remote diagnosis, but they refused to accommodate my request. During my own online research, I found that the car log indicates a “RearDrive Inverter Sensor” detecting high temperatures, likely causing the speed restrictions, along with a non-functional powertrain pump. Although my car is under an 8-year warranty for the battery and drive unit, Tesla has sent me a pre-approval bill for the upcoming service, making me concerned they may try to classify these issues as non-warranty and charge me hefty amounts for repairs. Currently, I cannot safely use my vehicle, especially on freeways, and I have no other repair options outside of Tesla. This ongoing issue is urgent and impacts my safety and ability to drive the car reliably.

Received several "Steering Assist Reduced" alerts when stopped in traffic last week, followed immediately by the failure of the power steering after each alert. Power steering resumed after a soft restart. Tried to make appointment at Tesla Service Center last Friday to get this addressed, but they said they had no appointments available and would not be able to diagnose my vehicle until today, August 12, 2024.. In the interim, the power steering went out again while on the freeway, forcing me to exit and restart the vehicle again. This is a clear danger to myself and my passengers. After receiving the results of the diagnosis today, they informed me that both the steering rack and harness would need to be replaced at the cost of nearly $5,000. The technician showed me some corrosion in one of the electrical connections where there had been water intrusion. I told him this was surprising since the car is relatively new and it would seem that Tesla would have sealed this connections to resist the relatively dry rainy seasons we experience here in Southern California.

I have taken my vehicle into Tesla to have the issue fixed twice. They don't know what's happening or how to fix it. My car is randomly taking the wheel and steering me into other cars / off the road. It happens once a month or so, I have a newborn baby I drive in the car and I don't feel safe driving. I've recorded the incident and Tesla Service said "that should not be happening unless I'm using auto pilot" which I never do. I've looked into lemon law but I did not purchase the car from tesla directly, I bought it from a used car dealership. I'm not sure what to do, I can't sell the car because I would still owe $10,000 after the sale. Please help anyway you can, I'm desperate for anything.

Braking and Stopping - Braking system - Improperly functioning braking systems. I experienced a problem with the brake pedal, brake calipers, and/or any component of a Model 3 braking system. “Model 3 has an anti-lock braking system (ABS) that prevents the wheels from locking when you apply maximum brake pressure. This improves steering control during heavy braking in most road conditions…Forward Collision Warning and Automatic Emergency Braking do not operate when driving at very low speeds (see Collision Avoidance Assist on page 128). Do not rely on these features to warn you, or to prevent or reduce-the impact of a collision. (Tesla owners Manuel71-73) I stop on a downward slope with my foot on the brake to wait for a gate to open in my apartments. I let go of the brake and the vehicle remained steady until it started rolling. When it started rolling, I jam my foot on the brake but the brake is engaged (locked). The vehicle accelerates, the brakes and steering wheel is locked giving no control over the vehicle. The vehicle drives through the gate and into a wall. The airbags deploy and I get out immediately for caution of explosion and shock. I call a tow truck and made a claim with USAA. I got a concussion and multiple injuries on my body. I’ve been getting medical treatment for 6-8 months. USAA didn’t help as they deemed the accident was my fault and totaled the vehicle. I bought the vehicle for more than what’s it’s worth. I have loan payment that I can NOT pay. This was my only asset and this accident changed my work life. Tesla gave me the run around on how to make a claim and no compensation. Directed me to insurance company. This demonstrates failure of an Braking System, operating electric vehicles, company responsibility, safety and transportation regulations and policies. These vehicles are highly dangerous for my safety and others as there’ve been multiple accidents and deaths.

I am writing to bring to your attention a concerning issue regarding the recent update related to recall alerts in my vehicle, which has inadvertently led to an increase in distraction while driving. I received a software update for my vehicle, which included new features aimed at enhancing autopilot alerts. However, instead of improving safety, these updates have unintentionally resulted in an increased level of distraction while driving. Issues Identified: Frequency of Alerts: The frequency of recall alerts has become excessive, leading to a constant stream of notifications while driving. This has proven to be highly distracting and has compromised my ability to focus on the road. Alert Presentation: The visual and auditory presentation of the alerts is intrusive and often occurs excessively during driving. The alerts are not sufficiently streamlined to ensure minimal disruption, causing a significant distraction. Impact on Driving Behavior: The continuous alerts have forced me to divert attention away from the road, leading to an increased risk of accidents and compromising overall road safety. Recommendations: As per most other vehicle brands that benefit from self-driving technologies, like Cadillac and Ford, they are not affected by this recall, and do not make the driver continually look at the display for information regarding to the alert. Intelligent Alert Timing: Incorporate a smart algorithm that considers the driving context before issuing an alert. This can prevent alerts during critical driving moments, reducing distraction. Comprehensive Testing: Conduct thorough testing of software updates before release to identify and address any unintended consequences, such as increased distraction. Before this update, it was a lot less distracting to drive this vehicle, due to the limited notifications. Since this update, I've found myself removing my eyes from the road because of the increased notifications. A reversal of this recall is in need.

We had Model 3 with instance of unintended acceleration at the gate of a parking lot. This car was bought in May 2020. On December 6, 2020, she was on the way out of the parking gate and suddenly the car accelerated, hitting the side wall of the gate. The car flew over a two-way street, resulting in a compound fracture of her right hip requiring internal fixation and transfusion. The vehicle was inspected by the police. There were no warning lamps, messages, or other symptoms prior to failure.

FSD Beta Firmware Version 2023.27.7. -- When operating with FSD Beta in control of the vehicle, the turn signal is activated and the appropriate left or right side blind spot video is brought upon the driver's display automatically. This works perfectly for city street lane changes and turns at intersections or driveways, etc. When operating on a controlled access highway the systems works in the same manner, activating the turn signal and displaying the blind spot video on the driver's display when changing lanes and exiting the highway. However, the turn signal is not activated and the blind spot video is not displayed on the driver's instrumentation when first entering a controlled access highway. This is the one exception where the driver is required to activate the turn signal while FSD Beta is in control, and seems like an oversight or 'bug'. However, it might be by design, in which case the intent that the driver is required to activate the signal indicating the vehicle's merge has not, to my knowledge, been communicated to the driver as the one exception to the otherwise automatic operation of the turn signal and blind spot video display while the vehicle is under FSD Beta's control.

On August 2, 2023 at 0400, the Tesla M3 was charging in the garage using a home charger. Smoke indicative of an electrical fire likely originated from the vehicle and eventually triggered a smoke alarm in the kitchen. The driver was unaware the source of the smoke was the Tesla, disconnected the Tesla and moved it out of the house to protect the vehicleb assuming a fire was in the house, and called firefighters who searched the home without success for a source of the fire. The driver now believes the Tesla was the source as the garage access door was cracked open to feed the charging cable from the 240v outlet in the laundry room. The nearest smoke detector from the Tesla is in the kitchen, where the driver smelled smoke from an electrical fire. At roughly 0745, the driver was about to complete a one hour commute to work in this Tesla unaware that the incident at 0400 was related to the Tesla. The driver experienced a sudden vector and velocity change in the vehicle while traveling at highway speeds. This is the driver's last memory of the accident that followed. A witness driving behind the Tesla described the vehicle traveling in clear lanes when it abruptly jerked left, then right into the side of the highway into concrete barricades. The Tesla was destroyed, airbags deployed, and the driver was rescued by first responders,firefighters, and paramedics. The driver was evacuated to the nearest hospital with serious injuries. The driver suffered broken ribs, a pneumothorax, several lacerations to head and chest, several contusions across the shoulder, legs, and body, and a mild traumatic brain injury.

The contact owned a 2020 Tesla Model 3. The contact stated while pulling into the driveway and depressing the brake pedal, the vehicle failed to respond, accelerated unintendedly, broke through the garage door, and crashed into the vehicle parked inside the garage. The other vehicle moved away and damaged the garage. There was no warning light illuminated. The air bags did not deploy. The contact shifted into reverse (R), pulled out of the driveway and shifted into drive (D). The brake pedal was depressed but the vehicle failed to respond. The contact stated that he hit the curb to stop the vehicle, and the vehicle stopped. The contact stated that all the air bags then deployed. The vehicle failed to unlock. The contact rolled down the windows and cried for help until the neighbors arrived and opened the vehicle from the outside. The contact sustained bruises on both arms, and the contact's wife sustained bruises on the right shoulder and on the left leg. Both the contact and his wife received medical assistance. There was no police report filed. The vehicle was towed to a third party it was sold to. The other damaged vehicle was totaled. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was 9,600.

The automatic windshield wipers came on and would not shut off. The car was clean, the day was sunny, and the camera lenses were free of dust or anything else that could cover them. While driving along, the wipers started and would not shut off. I went into the main monitor/screen that has the wiper settings but I was not able to activate the wiper/off button, so the wipers kept working. Pressing on that button would not activate it. I was in autopilot at the time, i.e., cruise control + lane keep assistance. I have FSD/full self driving, but have never used it since it is still in beta testing. I’m using Tesla software # 2023.7.10 I checked the various Tesla websites, and you can turn off the wipers only if you disable the ADAS features. After I turned off the ADAS, I was able to activate the wiper/off button. But this is a terrible situation, the ADAS needs to be activated, but if I activate, it wipers will begin working whenever they feel like it, regardless of the weather outside. This is clearly a software problem with the Tesla ADASN needs to be corrected. I don’t want to be driving along with auto pilot on, and then the wipers will simply turn on. Any help you could provide would be greatly appreciated to correct this dangerous situation. On October 29, 2022 the Michelin tire on my 2020 Tesla Model 3 that had 16,000 miles on it, suffered catastrophic tread loss while traveling at 70 miles per hour on California Route 110. The tire was the original equipment tire supplied with this vehicle when my lease began in January, 2021. The DOT number on the tire was B9 EL 04 EX 0419 and the tire size was 235/45R18 98W. The tread did not wear out, the tread delaminated from the tire and was lost on the side of California Route 110.

Recall 21v-00d was registered DEC 2021, notice said waiting for parts. It's been OVER 12 months and no parts yet? Also, the phone number Tesla has listed on the recall notice and their recall info on your website goes to a self-service site that states to use your Tesla app to schedule a repair so there is no way of contacting the company directly to find out about the status of the recall.

The driver seat occupancy weight sensor has to be replaced twice now. The car does not recognize someone is in the driver seat. It is unclear whether the airbags would deploy if I was in an accident as the car is unsure if someone is even inside. When I get out of the car it stays on. When I put it into reverse without having the seatbelt on it slams on brakes.

Ever since a recent software update was downloaded and installed, my Model 3's incidence of "phantom braking" events while using traffic aware cruise control/autopilot has gone up by a very noticeable degree. It has always been a known issue, however since the redundant safety systems (radar and ultrasonic sensors) on my model year vehicle were disabled by one of the recent updates, phantom breaking events that can possibly CAUSE a collision event or loss of control of the vehicle happens far more frequently. Instead of occurring once every 1000 or so miles, it now occurs two or three times every 250 or so miles, judging from performance on recent cross-state road trips. My car does not use the "Full Self Driving" package that Tesla offers.

Someone's gonna shoot my ass for blasting them repeatedly with highbeams. Because autosteer is connected to auto highbeams, I can't turn it off unless I turn autosteer off and drive like a 1995 peasant.

Sporadically when driving at highway speeds, car will experience an error. Steering assist IMMEDIATELY decreases (wheel harder to turn), cruise control (if enabled) will disengage, and following Error codes appear on flatscreen : - Vehicle Hold Feature Unavailable - Tire Pressure Monitor System Fault - Automatic Emergency Braking Disabled - ABS (Anti-lock brakes) disabled - Regenerative Brakes Disabled - Steering assist reduced - Traction control disabled Unsure of which component is at fault. Tesla Service center (Norfolk VA) has pushed off appointment for more than 30 days but has acknowledged that there is an issue. SAFETY: Loss of ABS is a major safety risk to occupants. Loss of steering assist also puts driver and others at risk due to increased steering effort. Loss loss of regenerative braking also a safety risk due to the instantaneous change of vehicle braking behavior when regenerative braking switches off.

I recently brought my car in for routine servicing and informed the Tesla service technician that I’ve been experiencing significant lag with the rear (reverse) camera video feed since roughly early-December 2021. The rear (reverse) camera lag makes reversing hazardous, as any reliance by the driver on the the rear (reverse) camera feed can cause serious (potentially fatal) injury and/or property damage. I asked the technician to look into and address the issue; I told him I was aware there had been a recall and that I received a notice. He informed that they were unable to replicate the issue—which I find hard to believe, given it happens constantly—and that rear (reverse) camera lag is common in Model 3s as they age. My car is less than two years old—hardly classifying as “aged”. In looking at Internet forums, this appears to be a very common issue with Model 3s, and one that has yet to be resolved by the manufacturer through either hardware or software update(s).

Tesla model 3 has side repeater cameras that work as blind spot monitor cameras and provide visuals for safe lane changes. However, my model 3 suffers from a manufacturing defect in the camera assembly which causes the visuals to wash out at night due to light bleed from the turn signal. This caused an unsafe condition where i almost ran into a car in my blindspot being unable to see them on the camera visual. I raised the issue with service and they do not consider it a manufacturing defect. The contact stated that while driving on the right side of the road at 35-45 MPH, on autopilot mode with a passenger in the front passenger seat, the vehicle drove off the road independently and crashed into a tree. The air bags failed to deploy. There were no warning lights illuminated. The contact's face hit the steering wheel. The contact sustained a black eye with laceration, bruised ribs, head, neck and back pain, sprained left ankle, bruises on her thigh, may have sustained nerve damage on the right side of the face that hit the steering wheel and sciatic nerve damage on the right side of her body. The occupant of the front passenger seat sustained neck, back and clavicle injuries. The contact sought medical assistance the next day. The vehicle was towed to a tow lot, then it was towed to a Tesla certified specialist body shop. A police report was filed. The contact has not been able to get a hold of the manufacturer. The vehicle was not diagnosed or repaired. The failure mileage was 55,000.

There was a recent software update and Tesla rearranged items on the touchscreen. Passenger was trying to pair his phone with the vehicle, and touched his name as displayed on the screen. This caused the driver seat to slide back quickly to fit the pre-set for his height. He is considerably taller than the driver, and this caused the driver to be unable to reach the brake pedal or accelerator. Therefore, the driver had almost no control of the vehicle. This occurred on a quiet side street going uphill, so driver could steer the vehicle to the curb which caused it to stop, even though she could not reach the brake pedal. However, if this had happened on a busy street or highway, there would have been a terrible accident. Remedy is simple. It should NOT be possible to activate the driver side seat position pre-sets when the vehicle is moving.

Tesla pushed an update (v. 2021.44.25.2) to my Model 3 a couple of days ago that has dramatically altered the usability of the car, and potentially has safety implications. There was no warning prior to the update being applied that such changes to the car's user interface were included, nor is there any option to revert to the prior user interface. Major issues with the changes are outlined below: - 1-button, single-click shortcuts to seat heating, defrost/defog functionality have been removed; when driving in bad weather where the screen may fog up rapidly, you're now required to take your eyes off the road, and click twice on a small target on the touch screen to activate the required functionality. - 1-button, single-click access to the wipers has been removed, now requiring one to either rely on the auto-sensing functionality (which works properly about half the time), or click through the HVAC menu to adjust the wiper speed. - The quick access driver profile selection dropdown in the previous interface has been removed. My spouse and I differ significantly in height, and about half the time, the car selects the wrong profile when we're both getting into the car (despite our phone keys being linked to our specific profiles). With the earlier UI, it took one tap on the screen to switch to the correct profile. Now, I have to dig through menus to select the correct profile, while my knees are being slowly ground into the steering column/dash. - When driving, the speedometer, gear selection and navigation look significantly different than in the earlier UI. This is likely to lead to more distracted driving while we get used to the new interface. I filed a service request with Tesla, requesting that they revert our car back to the prior version of the interface; this request was denied and the appointment was automatically canceled with a generic message about firmware updates being rolled out on a schedule.

While turning into a parking spot in a covered parking garage the car seemed to speed up and lurch. It hit the cement wall and pushed the car back with such force the air bags deployed. Thank God it was just a wall. The car is now at the repair garage and have not heard back regarding how much damage and if the car is going to be "totaled". When I mentioned this at a party we were at, a friend of mine said she had the same problem. There were no collision warnings. It happened in a split second. The parking garage was at my Doctors office so I was able to have my wrist x-ryed right then. No serious injury. Just a little leary of driving this model car if not any electric vehicle.

I BOUGHT A TESLA MODEL 3 ON 9/19/20. ON 9/23/20 I WAS WAITING IN MY CAR IN A LINE OF CARS AT A FAST FOOD DELIVERY WINDOW, STOPPED 8-10 FT BEHIND A LARGE VEHICLE WITH MY FOOT ON THE BRAKE AS WELL AS IN THE PARK MODE BECAUSE OF THE SLOW SERVICE AHEAD. AFTER A WHILE I REALIZED THAT MY TESLA HAD STARTED MOVING SLOWLY TOWARDS THE VEHICLE IN FRONT. PUMPING THE BRAKE, TRYING TO CHANGE THE GEAR TO REVERSE DID NOT STOP THE CAR. THE SCREEN DATA DISAPPEARED AND A MESSAGE FLASHED SAYING THAT ALL SAFETY FEATURES WERE OFF. I OPENED THE DOOR WHICH IS SUPPOSED TO STOP THE CAR. IT DID NOT EVEN AFTER I GOT OUT OF IT. IT CONTINUED TO MOVE EVEN AFTER CONTACT WAS MADE WITH THE CAR IN FRONT. THE TESLA WAS COMPLETELY CRUSHED. IT WAS A TOTAL LOSS ACCORDING TO A REPAIR SHOP WHERE IT WAS TOWED ON TESLA'S RECOMMENDATION. FRIDAY JUNE 26TH, 6:00 P.M. PUBLIC ROADS. SUNNY, DRY. SOMETHING SNAPPED WHILE DRIVING 5 MPH AT THE MOMENT I STARTED A SLOW LEFT TURN. THE CAR LURCHED TO A HALT. THE RIGHT FRONT WHEEL WAS ANGLED LEFT AT ABOUT A 35 DEGREES, WHILE THE LEFT WHEEL POINTED STRAIGHT. TESLA SENT A FLAT BED. THE TRUCK TECHNICIAN REPORTED BY PHONE THAT THE WHEEL WAS SEPARATED FROM THE SUSPENSION AND A TESLA BOLT WAS AVAILABLE FOR HIM TO TEMPORARILY HOLD ALIGNMENT FOR SAFE LOADING. I UNDERSTAND HE LEFT A NOTE AND THE BOLT INSIDE THE CAR. I BELIEVE THE BOLT WAS PART OF THE BROKEN SUSPENSION. THE CAR HAD 3,300 MILES. DURING MY OWNERSHIP, I NOTICED A LIGHT CLICKING (OR CLUNKING) WHEN THE CAR PASSED OVER A 3 INCH CHANGE IN ROAD ELEVATION, SUCH AS OCCURS DURING REPAVING WHEN STRIKING A MAN HOLE COVER OR NEW LAYER OF ASPHALT. I TOOK THIS TO BE A PECULIARITY OF THE TESLA SUSPENSION COMBINED WITH THE 18 INCH WHEELS. THE SOUND WAS RARE, AS MOST ROADS ARE QUITE SMOOTH AND I MADE IT A POINT TO DRIVE SLOWLY OVER SUDDEN BUMPS. MORE RECENTLY, AS OUR TOWN IS REPAVING, I HEARD THE CLICK MORE FREQUENTLY. ON FATHER'S DAY, WHILE A FRIEND WAS TEST DRIVING THE CAR, HE BEING A CAR GEEK, HEARD THE CLICK, CALLED IT A "CLUNK," AND TOLD ME TO GET IT LOOKED AT. I HAD NOT YET MADE THE APPOINTMENT WHEN SOMETHING SNAPPED ON JUNE 26TH. OVER THE WEEKEND I LEFT VOICE MAILS AND SENT TO TESLA SERVICE AN EMAIL STATING TESLA SHOULD CONSIDER GIVING ME A REPLACEMENT VEHICLE, BUT I WISHED FIRST TO HEAR THEIR REPORT ON WHAT HAD OCCURRED. IN ONE INCIDENT, JUST AS AUTOPILOT WAS ENGAGED, THE TESLA BRAKED SUDDENLY FROM 65 MPH TO 50 MPH. OTHER DRIVERS WERE FORCED TO BRAKE HARD TO AVOID COLLISION. ON THE SAME DAY, WHILE IN ADAPTIVE CRUISE CONTROL, THE TESLA SUDDENLY BRAKED HARD AT A SPEED OF 78 MPH, DOWN TO 60 MPH. TESLA HAS BEEN NOTIFIED BY OTHERS WHO HAVE HAD SIMILAR EXPERIENCES, SO THEY ARE AWARE OF THIS HAZARDOUS PROBLEM. THE ISSUE HAS BEEN REPORTED BY MANY ON TESLA SOCIAL MEDIA SITES SINCE AT LEAST 2018. I MADE A RIGHT HAND TURN FROM A FULL STOP OUT OF THE PARKING LOT INTO THE FARTHEST LEFT LANE INTENDING TO TURN LEFT. THERE WERE A LINE OF CARS AHEAD OF ME. AS I WAS TURNING INTO THE LANE AND LET MY FOOT OF THE ACCELERATOR PEDAL THE CAR ACCELERATED RAPIDLY INTO THE BACK OF THE VEHICLE AHEAD OF ME. IT HAPPENED VERY QUICKLY. IT FELT AS THOUGH THE VEHICLE TOOK OVER ON IT'S OWN. THE CAR HIT THE VEHICLE ONCE AND THEN CONTINUED MOVING AND IT THE VEHICLE AGAIN - SO TWICE IN ALL. *TR

SO REGARDING THE CAR BRAKES THEY ARE VERY HARD TO FULLY DEPRESS AND I BELIEVE ITS BECAUSE OF THE REGENERATIVE BRAKING. FOR EXAMPLE WHEN GOING EAT HIGHWAY SPEED AND YOU NEED TO PRESS THE BREAK PEDAL HARD IT LITERALLY FEELS LIKE THE CAR IS REFUSING TO LET YOU USE THE FULL CAPACITY OF THE BREAKS. ITS VERY SCARY IT HAPPENED TO ME AND I COULD NOT PRESS THE BREAK PEDAL COMPLETELY DOWN. AND REGARDING THE LIGHTS MY CAR HAS FOG LIGHTS INSTALLED BUT I'M UNABLE TO TURN THEM ON BECAUSE ACCORDING TO TESLA SUPPOSEDLY I DID NOT PAY FOR THEM BUT THEY ARE INSTALLED ON THE CAR AND THEY DON'T WANT TO ALLOW ME TO TURN THEM ON BECAUSE ITS CONTROLLED VIA SOFTWARE. THIS IS AN UNNECESSARY THING BECAUSE IF THEY ARE INSTALLED ON THE CAR I SHOULD HAVE ACCESS TO THEM BECAUSE I BOUGHT THE CAR. THIS COULD PUT US THE DRIVERS IN A HAZARDOUS CONDITION IN FOG OR OTHER LOW VISIBLY CONDITIONS UNNECESSARILY.

WHILE NAVIGATING ON AUTOPILOT ON THE FREEWAY THE CAR TOOK THE INTENDED EXIT RAMP, HOWEVER NEVER SLOWED DOWN UNTIL I HAD TO INTERVENE. REMINISCENT OF THE CHARACTERISTICS WHICH LED TO THE RECENT FATAL CRASH REPORTED OVER THE LAST WEEK.
