While using traffic aware cruise control or autosteer, the car occasionally detects a nonexistent person in the center of the lane I’m driving in, the proceeds to slam on the brakes when no hazard exists. It seems that tire markings in the road and certain lighting conditions cause the car to think there is a person in the road. Additionally, when driving on a highway with rolling hills, the car detects a forward collision alert and hits the brakes and disengages the autosteer. Both of these could cause a rear and collision at highway speeds. The dealer has claimed that the cause is functionally normally and I already tried to get it serviced but the car continues to have these issues. I do have video evidence of the car slamming on the brakes and detecting a forward collision.

I was driving east on [XXX] in Bourne MA using the Tesla adaptive cruise control system called "Autopilot" in very light traffic conditions. The sun was low on in the sky (behind and to the right). The system automatically engaged the brakes while crossing the bridge without obvious reason, decreasing the car speed about 10 MPH in less than a second before I overrode by pressing the accelerator. I believe the car vision system mistook a shadow cast by the vertical girders on the bridge onto the road as an obstacle. The sudden deceleration was a hazard to those behind that had to brake, but there was no collision or loss of control. The manufacturer seems acknowledge and accept that the defect exists, but has not been able to resolve the problem. The contact owns a 2021 Tesla Model 3. The contact stated while driving approximately 35 MPH, the vehicle experienced phantom braking, causing the contact to lose control of the vehicle. The contact was driving in a rainstorm during the failure and stated that the vehicle had not hydroplaned nor had the accelerator pedal been released. The contact stated there was no warning light illuminated. The contact stated that the failure occurred another time during a rainstorm. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 74,000.

While using the Full Self Driving Mode (FSD) the vehicle will randomly and without warning slam on the brakes at highway speeds and decelerate to 30-50mph on an active highway. If the highway is multilane, the car will slow and attempt to change lanes. It appears the vehicle thinks that something is in the road, even if the road is clear. This happens multiple times on my longer road trips, and is very concerning. It did this yesterday with a semi-truck closely behind me. The vehicle has gone into service and I did bring up the issue, but the service team was unable to assist on this issue (screenshots attached). The software currently installed in my vehicle is 2024.3.15, the latest available from Tesla. This issues has been ongoing from every software update I have had in the car however.

When using cruise control, auto lane assist and auto pilot, the vehicle will brake or slow down without apparent reason. This can happen suddenly and dramatically. This can happen when there are other cars nearby, but also when there is no other traffic on the road. It is distressing enough when there are no other cars nearby, but can be a hazard when there is other traffic behind me and they have no warning this is happening.

I drove into my neighbor’s driveway to drop her off. As I slowed down and applied the brakes to stop, the car failed to respond. It continued forward, running over the flower and plant pots at the edge of the driveway, crossing the backyard, and finally colliding with the trees and bushes at the rear of the property. All airbags deployed, and the vehicle was damaged beyond repair.

Major phantom Braking at 70mph on interstate 5, 4 times the brakes slamed on as if yo had done an emergency stop from 70mph to 20mph fortunately we were not rear ended as I had my son and grandchildren in the car. The car was in cruise control on each occurrence I finally had to stop using it for fear of a major accident

My Tesla Model 3 Phantom Brakes when using Cruise Control, particularly in the morning when the road is clear, no cars or obstacles in front or behind me. The NHTSA KNOWS THIS IS A PROBLEM AND I HAVE PREVIOUSLY MADE THIS COMPLAINT. I contacted Tesla to no avail. They did nothing. WHY HASN'T THE NHTSA DONE ANYTHING ABOUT THIS ISSUE. THIS IS DANGEROUS AS AN ACCIDENT IS WAITING TO HAPPEN.

“Phantom Braking”. Driving on the interstate approximately 75 mph using cruise control, auto steer, not auto pilot. Within a span of three hours today we experienced at least six episodes of the car emergency braking for apparently no reason. Immediately going from 75 to 40 mph. Thankfully no one was close behind us. We finally stopped using the cruise control. Now afraid to use it at all.

Ever since Tesla rolled out an update for everyone to beta test FSD, the adaptive cruise control has been changing speeds when near interstate type exits and entrances. I've had it change from 75 down to 40 mph which is dangerous in traffic. This change in speed some times is just a drop to the next 5mph increment, such as from 73 to 70. Sometimes it is more from 73 to 65 at the same place/exit. It is not consistent. The only thing consistent is it's near exits even when not in an exit lane or nearest lane to the exit. This "Feature" needs to be removed as it is unpredictable which makes it dangerous. This car, like many Teslas, is experiencing what has been come to be known as phantom braking. When in the autopilot or full self driving, this car will occasionally to frequently, depending on the day, suddenly break where it will decelerate from say 65 mph to 45 or 50 in a very short period of time. This can happen in very busy traffic conditions. It seems to happen when I'm going faster rather than slower. For example, I haven't experienced it so much on City Street. It seems much more prominent on highways. This is an issue that has many YouTube videos about. It has been reported by many Tesla owners as well as the media.

Driving on I-5 North from Los Angeles to San Francisco with adaptive cruise control activated. Suddenly and without warning car activates emergency braking as if it sees an obstruction on the road but obviously road was clear of any obstructions. This happened 4 times then I finally decided to no longer use cruise control. This happened on a clear sunny day around noon time 9/8/23.

I was driving west bound on I4 August 12th 2023 at 1236pm at mile marker 45-47. I was in the left hand lane with the cruise control set at 73 mph. all traffic was moving well. there was no car in front of me. there was a car passing me slowly on the right. there was a retaining wall to my left. My car for no reason changed the setting on my cruise control from 73 to 35mph. this caused my husband and I to be thrown forward and I had to jam on the gas to try to regain speed. This was the 4th time this happened and the second time I have reported to Tesla who insists that there is nothing wrong with the car. I am scared if someone is following me closely I will be rear-ended at a high rate of speed. They continue to quote information from the manual and in the invoices insinuating that any incident occurring from this would be my fault. This is a terrifying thing to happen when there is no reason for the car to slow for any reason.

Just completed a 2week trip and the car attempted to stop on its own over 37 times (stopped counting) on the highway. 2times I had a semi within a block of my rear of the car. Since this occurrence is not anticipated it worries me that it's going to cause an accident. Reported to Tesla and it's days the feature is beta, I don't believe that for a second . This my 3td car with adaptive cruise control. I was not using the auto guidence feature.

The rear doors in this vehicle have no manual open option in case of a power outage such as a fire or submersion. This is a huge safety risk for back seat passengers, who are more likely than average to be children who could not figure out what to do in the case of an emergency

I have a 2021 Tesla Model 3. The current owners manual shows how to open the rear doors in the event of loss of power using an emergency latch at the bottom of the door pocket. Our car doesn’t have that latch in the rear doors (though the front doors each have one). This concerns us and we feel it is unsafe for the back seat passengers.

It will read the speed limit sign on the off ramp and slow the vehicle down 20mps abruptly, it will see a car on an onramp and slam the brakes on thinking there is a collision coming. it is every single day using cruise control

While driving [XXX] on [XXX] in Texas, the Tesla autopilot abruptly and forcibly applied the brakes while I was driving approximately 77 mph. I was almost rear ended by a semi truck due to this issue. This happened again about 20 miles down the road and I disengaged the cruise control and autopilot altogether at that point. The vehicle was traveling in excess of 75 mph with only the autopilot system enabled. The vehicle brakes engaged by itself enough to squeal the tires and the rear end momentarily broke free from the pavement. I disengaged autopilot and drove for approximately 5 minutes. I reengaged autopilot and approximately 5 minutes later the brakes engaged again. I left autopilot disengaged for the remainder of the trip (~ 4 hrs) There were no other vehicles nearby but the breaking was severe enough that if another vehicle had been behind me, it could have caused a collision. The dealer acknowledges that this unintended breaking does occur on other vehicles. There was no warning that the vehicle would severely brake. The weather was clear with bright sun on route I25 south of Denver.

The car is available for inspection at Arrow towing service Oklahoma City. 2: After the car accident the car keep going can't stop got Tee boned and my passenger broke his legs and ankles, elbow. 3: No, the problem has not be discussed with the dealer or manufacturer yet. 4: No one, including police, insurance have been inspect it. 5: No warning lamp or any kind of warning.

Car was charging in garage overnight when an interior dash electrical fire started. Fire burnt dash and smoke damaged the interior or car. Fire smoldered itself out, and car was considered a total loss. Collision center that picked up vehicle stated that they believed the fire started from an electrical charging fault at the harness in the dash.

While charging Model 3 the amperage went from 32amps and was capped at 16amps. This is a wide spread issue with Tesla’s model 3 Power conversion system (PCS). There is 3 controllers in the PCS system. When all 3 are working correctly it will charge at 48amps. If one stops it will drop to 32 amps or drop to 16 amps if two fail or not even charge if 3 fail. This is a known issue and and costly repair as Tesla refuses to fix these under the battery warranty.

The interior rearview mirror exhibits tiny bubbles around the edges and streaking or cloudiness within the layered structure of the auto-dimming (electrochromic) mirror. The problem has occurred in multiple potential replacement mirrors, indicating a design or manufacturing defect. The issue first appeared as small bubbles around the outer edge, then progressed to visible streaking and cloudiness within the mirror surface. These symptoms impair rear visibility, particularly at night or in bright light conditions, when glare and distortion are most severe. This creates a safety hazard by: 1) Obstructing the driver’s clear view of approaching vehicles. 2.) Reducing reaction time Increasing the risk of rear-end collisions or unsafe lane changes. 3.) It compromises overall driver situational awareness, especially in high-speed or congested traffic environments. The defect is not isolated to a single part. Defective mirrors have been retained for documentation and are available for inspection. Mirror replaced with same part number under limited warranty, but the replacement option showed same failure, suggesting a broader defect not yet addressed by a permanent fix. Additional replacement parts offered show similar defects.

The defrost function on the computer went crazy and cracked the windshield in the morning, covered in snow. - It’s hard to see out of, and Tesla said it’s not covered even though it’s only 6 months old and to just drive it. - No, they want $1600 to look at it. I can’t get them to agree to look at it, but don’t give up. - no the defrost was slow and low, as winter is with no issues. So it snowed a lot in KC, MO. I keep my heat at 60 degrees and the fan on low. I unlocked my car with my phone to get some stuff out, but I started clearing off the back door stairs first to make a path. But only 10 minutes after unlocking it, I heard a loud pop. I thought it was electric, so I unplugged it and got in to leave and immediately was shocked to see and hear the heater on so high I didn’t know it went to that level. It had to have been 100 degrees in the car then before I even sat down… I saw the HUGE CRACK across the windshield from the bottom right passenger side, perfectly in the corner of the glass, going all the way to eye level in front of the driver. Diagonally, corner to corner. There isn’t a scratch or speck on this car. I hand wash it every week when it’s not freezing out. I keep the left scroll wheel programmed to control the fan speed, so I pushed it to lower the leaf blower setting it was on to find out it was still set to 60 and fan speed 2. I had to go up and down a few times to get it to go down to normal. I took a video and photos, and Tesla said repeatedly it’s going to cost $1600, even though I just bought it 6 months ago.

The contact stated that the front windshield wipers did not automatically turn on while it was raining and that the auto rain-sensing feature worked intermittently. The contact stated that the manufacturer was made aware of the unreliability of the auto rain-sensing feature, but stated it was a Beta feature. The contact stated that the risk was that there was no physical control for the front windshield wipers causing a safety issue because the contact could not engage the wipers safely when driving as it was not readily accessible via the entertainment panel when the auto rain-sensing was inoperable. No warning light was illuminated. The vehicle was diagnosed, but not repaired because there was no fix. The manufacturer was made aware of the issue. The failure mileage was unknown.

Latest update changed the cruise control to force inclusion of autopilot. Cruise control function is now unavailable with without this autonomous feature. It's not only distracting, but aggravating. What's worse, is there's no online ability to inform or complain directly to Tesla. This isn't the first safety-related software glitch. Please get them to fix these issues.

The contact owned a 2021 Tesla Model 3. The contact stated that after reversing out of a parking space, then shifting to the drive position and lightly pressing the accelerator pedal the vehicle suddenly experienced unintended acceleration and drove forward at a very high rate of speed. The vehicle drove across a street and crashed into a building. During the crash, the driver sustained an upper torso and neck injury and was transported to the hospital. One person inside the building was also injured during the incident. No further information was available. The vehicle was destroyed and towed away. A police report was taken at the scene. The manufacturer was notified of the incident. The failure mileage was 12,400.

This accident involved a high speed collision on the 2nd floor of a parking garage at a hospital in a suburb of Houston, Tx. Both the driver and the passenger were killed. A question has arisen as to how the vehicle could have accelerated at such a high rate, in such a short period of time, especially inside a parking garage? When we found out NHSTA was re-opening, the Tesla sudden acceleration issue, we thought it important to include the facts of our accident. Sudden acceleration in this case, raises some concern, taking into account the facts and circumstances surrounding this accident. We have photos of the vehicle if needed.

2021 Tesla Model 3 RWD; 21,000 miles I pulled into a parking spot creeping in like I am about to park the car. When I tapped on the break to stop the car, it suddenly accelerated forward. It jerked so quickly forward that the back of my head was pushed into the headrest like a whiplash. Luckily, there was not a car parked in front of me because I generally park away from other cars. I quickly hit the break and the car stopped. I am 100% positive I did not accidentally hit the gas pedal. If I did hit the gas petal, I would not have hit it hard enough to make the car jerk forward and accelerate like it did especially when in a slow creeping speed to come to a stop to park the car. In 36 yrs of driving I have not had one accident. This was quite a scary.

Unexpected acceleration while driving my car at about 20 mph and making a right turn the car suddenly accelerated. Using the brake immediately worked. This has happened a few times with my car. The sudden acceleration surprised my wife and she asked what happened. The car does not have Full Self Driving installed and Autopilot had been off for several miles. On [XXX] at approximately [XXX] on [XXX] in Oakland, CA, I was intending to park the car on the street at the curb. As I started to pull to the side of the road to park, the brake suddenly fully engaged, bringing the car to an immediate complete stop. My foot was not on the brake at the time. There were no obstructions of any kind in the road or in front of the vehicle. No warning sounds, messages or lights were activated before, during or after the incident. The incident caused no injuries, because of the low speed at the time, just a sudden jerk forward. However, if the event had occurred at a higher speed, some kind of injury would have been likely. I am also reporting the incident directly to Tesla. There has been no testing or inspection of the vehicle. The vehicle will be made available for inspection or testing if requested. My Tesla slowed from 70 to 50 on the freeway in under two seconds for no apparent reason while on autopilot. There was a concrete barrier a few feet away on the shoulder, which I had been driving next to for about a quarter mile before the incident. There was no other traffic within 50 yards of me.

On at least 4 occasions, I have been driving at freeway speeds of 70mph with cruise control engaged - typically while driving on I-35 between Duluth and Minneapolis in Minnesota - and the vehicle has applied the brakes very hard for no known reason, without human input to the brake pedal. The most recent occurrence was on May 27th, 2022.

My Tesla has full self driving supervised. When I use this feature, since about three updates ago, my Car has started braking for no reason. It hasn’t happened on a freeway, but, but on secondary roads, it will brake and try to come to a stop. It seems to do this, when there is a shadow across the road, or when there is a puddle in or near the road. Sometimes it will brake for no reason at all. I I have come close to being hit from behind. I think this is a dangerous situation that needs to be looked into.

Location: 35.084609,-92.414194 While driving through the roundabout north to south on Amity road, the car has twice now applied automatic emergency braking while exiting the roundabout. There is a billboard 200ft from the road with an image of a person on it. The car is confusing that as a person in the road. This has caused my dog to be thrown (thankfully buckled in) and us to almost be rear-ended both times. This is extremely dangerous issue.

On March 31st, Tesla alerted me that I had been "granted a 30-day complimentary trial of Full Self-Driving" capabilities. They didn't make it clear that the service was automatically enabled (vs. allowing me to opt-in) and did not provide any information about how it operates. Because I have no interest in the service, I didn't click the "Learn More" button in the email. Since the service was turned on, the car has unexpectedly "grabbed" my steering wheel twice while I was driving. I now understand that it's possible that I turned it on without knowing it but, again, I wasn't given any instructions. I was terrified. When I pulled over and tried to find an "off" button, I discovered there is none. When I spoke to Tesla today, I was told that there is no "off" for the service and that all I can do set it to the least invasive setting and "live with it" until the trial period expires on April 30th. I can still feel the car self-adjusting my speed and steering. I remain terrified that it's going to cause an accident. The bottom line: because I've had the car for two years and did not purchase AutoPilot, I KNOW there's a way to disable it entirely. Tesla is simply refusing to do it and using me as a lab rat with no regard for my safety.

Problem: As the vehicle is driven and the Adaptive Cruise Control, AutoSteer, FSD, …. is activated. As the system detects an unexpected outside event, the vehicle would slow down for a few seconds, then it would continue driving and re-accelerate to about the same speed that it was before and/or to the maximum set speed. Opinion: This is can be dangerous if the driver is not observant, alert, and does not control the vehicle which can be dangerous and cause death, destruction, and/or damage to the driver, passengers, pedestrians, vehicle, surrounding vehicles, fixed objects, etc. Possible Solution: As the vehicle is driven and the Adaptive Cruise Control, AutoSteer, FSD, …. is activated. Once the system detects an unexpected outside event the system should: * Temporarily Disable Adaptive Cruise Control, AutoSteer, FSD, …. * Interior Cabin LCD would flash Yellow/Orange and beep quickly 3 times in a row - Showing a message on the screen “For the safety of you and others _____ (Adaptive Cruise Control, AutoSteer, FSD, …. ) has been temporarily disabled due to an unexpected outside event. Please re-enable ____ (Adaptive Cruise Control, AutoSteer, FSD, …. ) when it is safe.” Possible Solution Reason: * Temporarily Disabling Adaptive Cruise Control, AutoSteer, FSD, …. disables the drivers dependence on the system, slows the vehicle, and forces the driver to take control of the vehicle, situation, and avoid the unexpected outside event. * Alerting/notifying the driver alerts the driver to a dangerous unexpected outside event that the driver needs to assess and know the best course of action.

You guys have ruined auto pilot and made it less safe. Quit trying to get a promotion or recognition by going after Tesla. My model 3 was the safest car to drive hands down. I've had 17 different cars in the last 20 years, Tesla's are like anything else I've ever driven. The problem you caused by forcing stricter safety for autopilot, actually makes it way less safe. Before it assisted on highways so I could briefly do things like change temperature, music, or navigation without accidently running off the road or colliding with another vehicle. Now it's so sensitive with warnings to pay attention, it's actually a distraction. Look at my speed, warning to pay attention. Check my surroundings in mirror, warning. Change temperature, warning. It's to the point where it's distracting me more then anything I would do in a traditional car. Seriously, NHTSA can go [XXX] itself. Someone in the government always trying to make a name for themselves acting like a savior for the people. Why don't you make yourself useful and go after those [XXX] traditional company's like Ford. Ford always rolls out vehicles with serious safety defects, but weigh the cost of recalls/lawsuits against profit. Ford shouldn't be allowed to sell vehicles in the US, yet you go after Tesla, who in my opinion makes the safest car ever created. There is no manual door release for the rear doors. Thus rear passengers are at risk should the electrical system fail in a crash. Tesla should be required to provide a modification that allows rear passengers to manually open the rear doors.

We were not able to open the door after the 12 V battery died; fortunately we were not inside the car else this could have been catastrophic. There was no way for anyone trapped inside to come out.

Tesla doors are opened from the inside of the vehicle by pressing a button. In the event of a power failure, the buttons will not open the doors. The front doors have an additional manual release to open the door. However, the rear doors do not. In a power failure mode, rear seat passengers will be unable to open their doors to exit the vehicle. Newer model Teslas contain the manual release for the rear doors. However, I have contacted Tesla and they have no plans to address my vehicle. I consider this a life threatening issue. Tesla should be required to retrofit any of their vehicles that have no way to manually open the rear doors.

The car, with no known or evident driver, stopped the lane awareness, cruise control, etc. All of those just stopped working and per Tesla that's expected which is surprising since it's all part of the safety systems. The car's visual cues stopped identifying lanes and as a byproduct of that, none of the dependent features work.

Component/System involved: Advanced Driver Assistance Systems — FSD (Supervised)/Autosteer (Lane Keeping Assistance) and Adaptive Cruise Control. Possible failure to warn/stop (FCW/AEB). Vehicle and data are available for inspection upon request. I preserved dashcam files and requested Tesla to preserve engineering logs and EDR. What happened & safety risk: On [XXX] at ~[XXX] PDT on [XXX] near East Palo Alto, CA, with FSD (Supervised) engaged, the system appeared to misinterpret an exit ramp/gore area as a continuing lane at a highway fork and maintained ~60 mph (posted ~70). As soon as I saw the trajectory was unsafe, I braked and began manual steering takeover, but the vehicle contacted a roadside sign near the gore/shoulder before I could complete the maneuver. I then stopped safely. No other vehicles were struck. Airbags did not deploy. This posed a serious collision risk to me, my passenger, and nearby traffic. Reproduction/confirmation: NOT REPRODUCED. I have not attempted to reproduce the event. UNKNOWN whether the issue has been reproduced by Tesla or a service center yet. Inspection to date: Police responded (report pending). My insurer has opened a claim. I opened a Tesla Service request asking to PRESERVE Autopilot/FSD engineering logs for the incident window and to coordinate EDR extraction; engineering review pending. Vehicle remains drivable. Warnings or symptoms before failure: No audible/visual forward-collision warning was perceived by me and I did not observe automatic emergency braking. No prior warning lamps/messages were noticed before the departure toward the gore. UNKNOWN whether any internal/partial interventions were recorded in logs. Autonomous Driving Problem [XXX] Driving from Cornelia, GA to Commerce, GA on [XXX] at 60 mph driving on the right lane with very lite traffic. 6 different times the FSD (Full Self Driving) (Supervised) 2021 Tesla Model 3 starts signaling left and changes lanes with no traffic or vehicles in front. The 5th time FSD (Supervised) changed lane to the left was a near car accident because a car was speeding at 70-80 mph from the rear and the vehicle changed lanes in front of the speeding car nearly causing an accident. The autonomous cabin driver detection sensor does continuously give false positive warning messages to the driver to "Pay attention to the road" when I am wearing my glasses, so I have to drive without my glasses. Pillar cameras constantly complain blocked or blinded. Services refused to even look into the issue blaming glare from the sun, even when happening to both on opposite sides of the car at the same time. Report it even happens at night I am told it might be too dark for the camera. Did not occur like this on my 2019 Tesla Model 3. This seems to be causing random unexpected lane departure alerts or failures in lane keeping on cruise control or with lane assist. Message are extremely frequent and distracting. Message placement is blocked by arm while holding the steering wheel requiring me to take my eyes off the road to read. Been complaining about this for months, one of the two cameras (drivers side) was replaced which improved the issues with that side complaining but passenger side complains extremely often. Drivers side issue not 100% resolved but improved after replacement, possible cabling or MCU/ECU issue. Instead of looking at the issue Tesla insists the cameras must be dirty, moisture, too bright, too dark, etc. And fails to even examine components. Recent appointment canceled by Tesla after the scheduled date and time without notice. Camera doesn't appear to detect or recognize people or objects in that cameras visions creating what appears to be a blind spot when viewing in vehicle display. Have repeatedly seen person walking around my car will disappear from the screen around the passenger front quarter but reappear when in front of the vehicle or by the rear quarter.

The Traffic Aware Cruise Control and Auto-Steer are easily confused so that the vehicle does unexpected things like suddenly slowing down, even heavy breaking, or disengage steering control while in a corner next to semi-truck. This commonly happens when driving at higher speeds, 70 MPH, on the freeway. Suddenly slowing down at such a high speed is dangerous when there are cars following me as they don’t expect a sudden slow down when they can clearly see there is no reason for our car slamming on the brakes. In my observation this problems occur most frequently at higher speeds and when approaching a semi truck or overpass. It is especially worse when going through a corner. Sometimes the center barrier seems to cause sudden slowing down. Also, driving in a corner on the freeway while driving next to a semi truck has caused the auto steer to suddenly disengage which in itself was not dangerous since I had both hands on the steering wheel, it certainly startled me which could potentially be dangerous.

Needs whole suspensions system fixed and is only 4 years old. Was told by the service center employee that other customers with the same year and model have had the same issue. Suspension has been noticeably affected for the past year (uneven tire wear so I had to get new tires and a bumpier ride), but just took it in today and had it officially diagnosed.

While getting a routine tire change, my mechanic alerted me to the fact that nuts were missing from the rear suspension on both sides. On one of them, the bolt had nearly slid out as a result, and could have been very dangerous as it did so. The tires had been rotated at this shop prior to this, and they had not noticed it. So the nuts must have loosened enough to fall out between 21,496 miles on 2/15/2023 and 26,278 miles on 6/16/2023. We never noticed anything out of the ordinary with regards to noise, vibration, or handling.

Without any warning from feel unstable, wobbly, or noisy, my car began pulling hard to the right as I was going straight on a small street. I was going about 10 m-15 miles an hour. My steering wheel did not respond to my efforts to straighten the vehicle and my brakes did not slow the car’s path. I was only stopped by two parked cars on the side of the road that I bounced off of. When I got out, bystanders pointed to the sheared tie rod on the front passenger side. The witnesses stated it was clear from their view that if I had been going faster on a wider street there would have been severe injury Instead of only three cars being damaged. My car was three years old and had passed its annual inspection one week before. The police were called twice but did not come within three hours so the car was towed without a police report.

To Whom It May Concern, I am submitting this complaint to report a serious and potentially dangerous safety defect affecting my 2021 Tesla Model 3 Performance. After exposure to heavy rain or wet conditions, the vehicle’s horn completely stops functioning. The failure occurs regardless of the activation method—neither pressing the steering wheel horn nor using the horn via the mobile app produces any sound. The horn remains nonfunctional even though the vehicle is otherwise operable. This issue has occurred after the vehicle becomes wet due to rain and appears to be related to moisture intrusion or an electrical/design flaw. Once the failure occurs, the driver fully loses the ability to use the horn for warnings or emergency signaling. This is a critical safety concern. The horn is a federally required safety device and is essential for alerting pedestrians, cyclists, and other drivers to avoid collisions. Losing horn functionality significantly increases the risk of accidents, especially in urban driving, highway merging, and emergency situations where immediate warning signals are necessary. This issue is not isolated to my vehicle. Numerous Tesla owners have reported the same horn failure after rain or wet conditions on online forums and owner communities, indicating a widespread and systemic defect rather than isolated wear or misuse. Given the safety implications and the volume of similar reports, I respectfully request that NHTSA investigate this issue to determine whether a design or manufacturing defect exists and whether corrective action or a recall is warranted. Vehicle Info: Make: Tesla Model: Model 3 Year: 2021 Incident Date: April 7, 2025 Mileage: 49,000 miles Component: Electrical System / Software / Powertrain While driving on a public road, my Tesla Model 3 experienced a total system lag and shut down unexpectedly. The screen and vehicle controls froze, and the vehicle became unresponsive, leaving me stranded in the middle of the road. Tesla service later stated that the car computer had “slow FPS” (frames per second), which they claim caused the issue. They cleared memory, updated firmware, and said no further diagnosis was needed. However, no root cause was provided, and I am concerned that this issue could reoccur. This kind of unexpected shutdown while driving is a serious safety concern. I was lucky to avoid a collision, but if this had happened on a freeway or near traffic, the outcome could have been worse. I am filing this report in hopes that NHTSA will investigate whether this is an isolated issue or part of a broader safety defect related to Tesla's onboard software or power management systems.

I am writing to file a formal safety complaint regarding a critical malfunction that occurred with my Tesla Model 3 on [XXX], in West Palm Beach, Florida. This incident raises significant concerns about the safety and reliability of Tesla vehicles, particularly following software updates. Incident Overview: On [XXX], around 5:00 PM, I placed my French Bulldog in the back seat of my Tesla Model 3 and closed the passenger side rear door. As I walked around to the driver’s side, all the doors locked automatically, and I was unable to open any of them. Despite multiple attempts to unlock the car using both my iPhone and key fob, the vehicle remained locked. My dog was trapped inside the car, and I became increasingly panicked, especially considering the extreme summer heat in Florida. I immediately contacted Tesla customer service, but after a 15-minute conversation, they were unable to unlock the car remotely. I was forced to call 911, and the fire department had to break the passenger side window to retrieve my dog. Even after the window was broken, the doors still wouldn’t unlock, and I had to pull my dog out through the shattered glass. Additional Details: The car was charged to 80% just a couple of days before this incident. After this incident, the car is completely dead. It is non-functional, and I cannot power it on or access any of its systems. The day before this incident, I completed a software update on my Model 3, which I believe may have caused or contributed to the malfunction. There is CCTV evidence of the entire incident, which I can provide upon request. My dog was severely traumatized by this experience and is now fearful of being in the car, even when it is parked in the garage. I was also extremely distressed during the incident, and the situation could have resulted in a much worse outcome. Please advise on the next steps for an investigation, and let me know if you require any further information or evidence. Driver side window fails to lock completely upon locking the car manually and/or using auto walk away function. This leaves the car very much unsecure and open to being vandalized. Additionally, the driver sides window fails to close fully when auto close button is pressed. The driver side window button has to be fiddled with few times to get a complete window seel.

I was driving my car on the highway I275 approximatley 5 miles North of St Petersburg Florida. When without reason the car spun to the left very quickly, I did not have any steering control at that moment. If the car had hyro plained it would not have spun out of control so quickly. The road was slightly wet as there was a drizzle but not to any extent to allow the car to spin out of control due to heavy water. My vehicle hit the highway guard rail and was badly damaged.

When performing a u-turn, at low speed, the steering wheel locked in position, only allowing the car to go straight. I was able to regain the ability to steer, after approximately 5 seconds. I didn't notice any alerts or warnings, as it was happening. It was in the middle of a busy intersection and I was fortunate I managed to regain control of the vehicle, in time. I have scheduled a service appointment with Tesla. the earliest available appointment is the 20th, in my area. I have since driven the vehicle, approximately 40 to 50 miles, without incident, but I am concerned that this might happen, again.

The steering system malfunctioned. Yes it is available for inspection. The car turned across oncoming traffic turning the vehicle 180 degrees leaving me facing oncoming traffic during rush hour. I could have died in a horrific accident. The service center for the vehicle sulfate they will not evaluate vehicle until body damage to vehicle are repaired. The vehicle had no notifications or warnings prior to the incident.

My vehicle is 3 years old and has roughly 52000 on it. After a recent software update (within the past month or so) all of the automatic safety features such as automatic braking, blind spot monitoring, cruise control stopped working on the vehicle. I brought it to Tesla and they confirmed the issue and said the entire ECU needs to be replaced, a $2700 expense on a 3 year old car. There is a warning message every time I start the car that says the automatic breaking is not working.

While the car was parked in a parking lot, I get two notification stating “Sentry mode has turned off due to a sentry system error.” When I checked out the car, the car has completely failed to do any of the computer related task, leaving only the gear, AC, and Bluetooth was working. None of the cameras, GPS, sensors, brightness mode and many others were completely stuck. This continued until the next day. This happened without prior symptoms or warnings. I would have been in deep trouble if I have been driving at the time of the incident. When I took the car to the shop, tesla didn’t update the status for a week. After a week, Tesla told us that the Turbo A, Autopilot main processor chip failed. This caused the autopilot to go on loop. Therefore the whole motherboard needed to be replaced. This happened to a brand new purchased Tesla 2021 standard range. Which cost me $2404.00.

AC/Heat pump total failure at 18,317 miles. Due to cold weather in Michigan and simply running the car, the heat pump went out entirely which creates the climate control in the vehicle. This caused dangerous driving conditions since the windows could not be defogged and the vehicle was below freezing temperatures inside. The service agents I spoke with seemed to understand this was a common occurrence in cold weather climates. The reason I am reporting this, is although mine was replaced at no cost to me under warranty, this is a manufacturing oversight and needs to be addressed by the company. This can cause very unsafe driving conditions for many people, and cost thousands of dollars in repairs all because they sell a vehicle that has parts not suitable for the climates they operate in.

The heat pump failed in 12/2022. The defroster would not work and heating was an issue. I encountered 2 dangerous situations in snowstorms where I could not see out the window and the heating was inconsistent and difficult to control.This was fixed by Tesla. The heat pump failed again (less than a month later). I was told today that there was an ice buildup in the coils possibly due to car wash (?). This problem, I am afraid, is going to continue.

My vehicle has a VCFRONT_A447 error code and will not heat up. With temperatures as low as -20 and highs of 25 not only is just the temperature potentially deadly but im unable to defog my windshield without freezing. I feel my car is entirely unsafe to drive.

The heat stops working at cold conditions. Many other users have reported this problem, after going into service I was told there is a sensor that is out in the open behind the bumper that when exposed to snow and rain can freeze and causes heat to stop working. I have experienced this myself when going on a trip back home from upstate NY to NC , I had no heat with temps outside being at 3 degrees F , not only was the car extremely cold causing me and my wife to get sick , but you can not use the defrost because there is no heat. So there is absolutely no visibility since there’s frost on all windows that you cant do anything about.

I believe that Tesla's autosteer and Full self driving features are improperly designed. I will speak to them as if they were one and the same. I noticed that you have a check box for Lane Keeping Assistance above, but there is no checkbox for coercive auto steering, which is Tesla's product. The only ability it offers as far as steering the car is to forcibly disengage the autosteering feature. This is not assistance and this is the problem. A properly designed Lane Keeping Assistance feature continuously monitors driver steering action, lane position, and traffic conditions. If the Lane Keeping Assistance system finds itself 'alone' with little or no measurable input from the driver, action is called for. With the Tesla, the feature has an embarrassingly stupid 'driver attention check' which, until recently, could be easily defeated by the simple addition of a weight to one side of the steering wheel or the other. In a misguided attempt to correct this ridiculously poor design, they have recently made matters much worse. Now they distract the driver's attention from driving by forcing the playing of some sort of 'patty cake' type game, which does prove alertness of the driver, but detracts from the real and necessary work of circumventing the actual threats encountered in everyday driving. In short, a proper Lane Keeping Assistance feature should not be totally ignorant of the driver's subtle driving moves (it's an excellent feature as long as autosteering isn't engaged) and should honor requests to move into different parts of the lane long before disengaging, and no attention measuring system should come at the expense of driver awareness of actual issues. I have tried through email and phone to convince Tesla that something needs to be done, but have received no response from them.

This defect affects the lane departure warning system of my vehicle. In certain situations, the lane departure warning system incorrectly activates when the vehicle is turning (vs. leaving a lane). On one occasion, the system activated while turning from my driveway onto the adjacent roadway. On another occasion, the system activated while leaving a parking spot. On both occasions, the vehicle was traveling less than 5 MPH and was not departing from a lane. When the lane departure warning activates, a loud noise is triggered, along with an alert on the center display. A driver's natural response in this scenario is to slow down mid-turn, potentially resulting in unsafe conditions for both driver and nearby motorists.

On 1/3/21, approximately 2pm. Driving south on I-77 in the city of Broadview Heights. In center lane of highway with Autopilot (speed and steering only) on. A Hummer in my left lane drifts into my lane and hits my car (I have video). No alarms or alerts went off to warn me. No automatic steering occurred. My car continued in lane and at speed (about 64mph), until my driver mirror broke off and adjusted my position to the left. Broadview Height police report # [XXX]. I have a service request into Tesla. I am awaiting the at-fault party insurance to contact me to begin repair process. To above questions: -I am suspicious about the ultrasonic sensors and/or camera and it failure to warn me I was about to be hit. -I was hit on my driver's side by a car that was not detected by my Tesla -I have a service request made to Tesla, awaiting response -No inspections or diagnostics have been performed yet by Tesla -No warning occurred. I am very familiar with the warning my car can produce: things like drifting out of my lane; someone in front of me braking hard; or Autopilot applying brakes when it thinks a car is in front of me (when it's actually on a curved road). I was driving on FSD Beta but when an alien landed on the road in front of me. The FSD Beta software did not detect the alien’s vehicle and almost hit her as she was stepping out. System initiated beeps and I tried to take control but it forced control away from me and tried to steer towards the alien. I had to kick the car to get control back. The rear middle seatbelt has permanently locked and is unusable. It occurred after having the rear seats folded down and was observed when replacing the seat-backs to their normal position. Tesla has refused to repair due to vehicle being out of warranty despite the vehicle not having been involved in any collision.

Failure of seat belt forces no occupancy of front passenger seat, as the only alternative would be to not wear a belt. This raises the following concern: 1. Will the belt fail to work properly in case of a crash event? 2. Will the belt disengage when normal driving. I am the owner of a 2021 Tesla Model 3 and am filing this complaint due to a significant safety concern: my vehicle does not have manual or mechanical releases for the rear doors; only electronic mechanisms are present. In the event of a total power failure, accident, fire, or submersion, rear seat passengers would be unable to exit the vehicle through the rear doors, as there is no physical lever or alternative escape route directly available inside the rear seats. This design poses a serious risk to occupant safety in emergencies, especially for families with children or passengers unable to access the front seat manual releases or trunk escape features. The lack of a rear mechanical release is inconsistent with standard automotive safety practices and could lead to injury or loss of life if electronic door access is disabled. It’s important to note that newer Tesla models now include manual rear door release mechanisms, demonstrating that Tesla recognizes the safety benefit and necessity of this feature. Owners of earlier models, like mine, are left without this critical safety upgrade, and I believe this should be addressed for all affected vehicles. I strongly urge NHTSA to investigate this issue and consider whether a recall or mandatory modification is warranted for the affected Tesla Model 3 vehicles. While traveling over 230 miles from home on a road trip, my 2021 Tesla Model 3 suffered a complete and catastrophic failure. While in North Carolina, the vehicle became entirely unresponsive—doors would not unlock, windows couldn’t be operated, and I was completely unable to access the charging port, trunk, or frunk. The vehicle was effectively bricked without warning. This wasn’t a minor inconvenience—it was a serious safety issue. I was stranded out of state from Saturday to Tuesday, completely dependent on others for transportation while waiting for the vehicle to be towed 58 miles to a service center. I was eventually informed of the cause, auxiliary battery failure, but the fact remains: this happened without warning, in the middle of a long-distance trip, and could have left me in an extremely dangerous or isolated situation. Tesla's systems failed without notice; an apparent design flaw left me with no way to access or secure the vehicle. The inability to open the doors or charge the car poses obvious and unacceptable safety risks. The windows were secured with plastic to prevent rain from coming in. This incident is deeply concerning and should be taken seriously. I am urging NHTSA to investigate this issue and hold Tesla accountable to ensure this does not happen to other drivers under even more hazardous circumstances.

The Tesla model 3 has a manual release lever for each door in the front. The passenger side lever doesn't work, creating a safety issue if the car were to lose power then that door would not be able to be owned in case of an emergency. It was never used before and just doesn't work. I've heard this is a common issue on the 2021 model as it was the first refresh or re design.

The contact stated while his wife was driving at approximately 29 MPH, the vehicle independently accelerated and the front of the vehicle crashed into a tree, where it came to a stop. No warning lights were illuminated. The front driver's side air bag was deployed. A police report was filed. Medical attention was sought as a precaution; however, there were no injuries sustained. The vehicle remained at the residence. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 80,340.

While drive after dark on a four-lane city road with no breakdown lane my vehicle alerted me of a problem and I should slow down. About five second later the car came to a stop and placed itself in park in the right-hand lane with traffic coming up behind me at 50 MPH. I was unable to locate the emergency flasher so I turned on the turn signal (I later found out the emergency flasher was located on the ceiling with white marking which were not visible in the dark). With the right turn blinker on I attempted to exit the vehicle but when I did, the turn signal turned off. This left me with a choice to leave a dark vehicle in the right-hand lane with moderate traffic or sit inside keeping the lights and signals on. No accident resulted but this was a very dangerous situation. The failure was a power converter which led to the disabling of both the front and rear drive motors. I was also unable to put the car in neutral and move it as far right as possible. I would have expected this type of failure to at least turn on the emergency flashers and give the driver the option of placing the car in neutral so it could be moved off the road. When i bought the car I specifically ordered the one with front an rear drive motors so failure of one would not completely shut down the vehicle. Seem like this power converter is a single point failure which leave the operator and other vehicles in a very dangerous situation.

While driving, a small warning appeared on the screen, directing us to pull over. Roughly 10-15 seconds later, the car died in the middle of the road. The car was towed to a Tesla service center. The rear motor power inverter was replaced. My other complaint was that the user experience telling us to pull over was not sufficient. If the problem was that dire, we need much more dramatic audible and visible alerts, as well as an indication of how long we have to go. We could have pulled off into a safer spot if we knew we only had a few seconds.

While parked, as I selected "R" (reverse), the car lurched and displayed two warning messages: "Rear motor disabled - safe to drive" and "Vehicle shutting down - PULL OVER SAFELY". The drive train was locked up and I fear that if the car had been moving when this happened, it would stopped suddenly possibly causing an accident. The car was towed away.

Vehicle's turn signal is not hardware based, but instead relies on the software registering signal use. The turn signal can be "fully" pressed, and "partially" pressed, and the behavior is supposed to be: Full press: Blinks until you manually disengage 1/2 press: Blinks 3 times, then automatically disengages. Anything "between" a full press and a half-press, ie beyond the level which is supposed to trigger a 3-time blink, but not "bottoming out" the signal, regularly fails to trigger the signal at all. This leads to scenarios in which the driver "thinks" they have signaled, and creates unsafe situations for surrounding vehicles.

The passenger side headlight has stopped functioning on repeated occasions, both the high and low beams stop functioning simultaneously each time. I have been on long distance road trips now each time that this has occurred. Tesla service techs insist that it's software related in both instances and provide a software update. This alleviates the problem temporarily. I am currently insisting that they replace the unit completely, as these are not standard headlights that a consumer is able to purchase and replace at a parts retailer or any auto service center. Today, I got another service update, no replacement yet. The inherent danger in this practice is the amount of days and weeks required to schedule a service appointment, while operating the vehicle with one headlight and a complete inability to source an alternative resolution.

The model 3 no longer turns on the headlights automatically when the windshield wipers are on. It requires the operator to search through menus to turn the lights on manually. This is required even if the headlights are in automatic mode. Voice Command doesn't turn the headlights on either. I believe it is dangerous to be diverting eyes to a menu screen to turn the headlights on while driving in the rain. In addition, failure to turn on the headlights in my state is a moving violation.

Front right headlight on a brand new car no longer functions, safety was put at risk by the fact that it is illegal to drive on 1 working light in the dark and rain; and this happened thankfully before leaving home on a dark morning so it was detectable and actionable. Problem was confirmed by remote diagnostic from dealer. No warnings or messages before failure which was confirmed by Tesla representative on the phone. They have not inspected the component and will not return calls / have not responded to service scheduling for 3 days. There is no service possible when they have a monopoly on their supply chain and will not schedule service. My car is unsafe to drive and is basically a lemon at this point until they fix it.

Tesla full self driving software malfunctioned and turned into oncoming traffic lanes instead of crossing over to the correct opposite side of the roadway. This occurred after getting off a on ramp in Ukiah, California around 2am Nov 15, 2025. This could have put others in serious danger if there were cars coming in the oncoming lanes heading in the different direction. The problem has not been reproduced by a dealer or other entity. Yet This has happened to other tesla drivers as well using FSD as you are probably already aware. No inspection and it was sudden and unexpected.

Tesla allows owners to enroll as FSD beta testers however, the owner cannot unenroll when they request. The FSD beta is extremely dangerous and makes the use of even ACC impossible to use. I have requested to leave the beta via emailing [email protected], which an automatic response is received stating to unenroll respond to that email with your VIN and they will process the request. However, they do not process the request; I have sent dozens of email requests with no response and am still stuck in the beta software queue. I have attempted to submit service requests but the app recognizes the verbiage and blocks the submissions, stating to send the email instead. There is no other method to contact Tesla.

I have been very cautious on charging my Tesla Model 3 and have followed all of the manufacturer's recommendations. The HV battery recently failed at 105,050 miles. South Korea has noted a battery reliability issue for this year car and model, so I am reporting this to you so if a recall is needed, you can track this. I believe my battery is a Panasonic NCA.

I was driving home from work on a two-lane highway when my car suddenly (and without any warning) lost all driving power and shut down while there was still battery left on the vehicle (roughly 10%). I could no longer maneuver the vehicle due to lack of propulsion, so my car died in the middle of the road. It was towed directly to Tesla service center where diagnostics were supposedly run. Their conclusion was the car died due to insufficient charge (even though there was about 10% battery left at the time of the incident). About a week later, the same EXACT thing happened! The car lost all power and shut down mid-drive without any warning with about 7% remaining. Both times, I had my home address in the car GPS (so it knew the exact distance I was traveling) and it never indicated once that I needed to stop and charge the battery. Again, I had the car towed to the service center where they reached the same "conclusion" that the car died due to simply running out of battery. When I showed them proof (pictures and videos) that the car died with charge left on the battery, they did not investigate the issue further. They simply stuck to their original conclusion and no longer wanted to help remedy my defective battery. I have video and pictures proving that my car dies without warning (on multiple occasions) while there is still charge left on the battery, but Tesla refuses to fix/warranty my car (let alone try to investigate or replicate the issue). They tell me that my car is "operating as intended" and is safe to drive, all the while knowing it is definitely not safe to drive. I'm especially concerned due to the fact that I drive my two daughters in my car. They both are very young and use car seats. What happens in the event that I am driving on the interstate with them in the backseat and my car randomly dies without warning? There would be no way to safely remove them from the vehicle (let along myself) before being involved in a serious high-speed accident.

Steering rack failure. Steering assist, auto pilot and lane departure prevention failed and would not work properly so this is a safety issue. Manual steering worked fine but the safety and auto pilot functions would no longer function. Paid for Tesla service over $3,000 to replace steering rack and now is working properly. Reporting this as this may happen to others and probably should be a proactive recall on the steering racks so others don't lose these safety features. Maybe related to computer as it failed, worked again and then failed again and was then repaired by Tesla service center.

On [Wednesday, 07/09/2025], I was driving my Tesla Model 3 in rainy conditions when the vehicle suddenly swerved and crashed into a sidewall. The road was wet but not flooded, and I was traveling at a moderate speed. As I lifted my foot off the accelerator to slow down, the car's regenerative braking system engaged abruptly, which I believe shifted weight forward and caused the rear tires to lose traction. This led to hydroplaning and complete loss of control. There were no warning lights or alerts prior to the incident. The road surface did not have visible hazards or standing water that would typically explain a spinout. The tires were moderately worn but within usable limits. As a result of the crash, the car was declared a total loss. The insurance company now has possession of the vehicle. Thankfully, no one was injured, but the crash was severe. After the incident, I researched similar cases and found multiple reports and articles (e.g., AutoEvolution) linking Tesla’s regenerative braking to increased risk of hydroplaning or loss of control in wet or slippery conditions. The regen system eliminates coasting and applies braking force immediately upon throttle lift-off, which appears to be a safety risk in reduced-traction environments like rain, snow, or ice. I believe this issue deserves investigation, as it may affect the safety of other Tesla drivers in similar weather conditions.

Worst occurrence was this morning driving home from work on cruise control, max speed cruise 73mph, suddenly my car braked on its own down to 59mph in 2 seconds. Very scary. My wrist hurts from that incident & my forehead almost hit the steering. Nothing in front & thank God no tailgating car behind me. This is not safe. It happens to me all the time but this was the scariest of all.

On a recent trip, we started experiencing extreme issues with our car's cruise control and autopilot features. Specifically, when in cruise control or auto pilot, the car would randomly disengage the cruise control or auto pilot and start braking. Taking the car out of cruise control or auto pilot, restarting the car, etc did not fix the problem. We noticed the problem was much worse at night and when we were traveling on a two lane highway. We would be in cruise control/auto pilot and often when another vehicle would approach us from the opposite direction the vehicle would suddenly brake. While this happened more often at night, it continued to happen at any point during the day and when we were on many different roads/interstates. This seems extremely unsafe to have our vehicle hard braking randomly when we are traveling at high speeds (60mph+). We scheduled a service appointment with Tesla and were told that this is a software issue and we would have to wait for an update.

Front passenger safety restraint system fault alert popping up every few minutes while driving. It is the most irritating thing and there is no way for me to fix it without spending over $500 dollars. This is seemingly a common problem for this make and model through reddit posts and car forums. I was also told by a employee at the tesla service center that they see this very often. It is shocking that Tesla hasn’t taken responsibility in fixing this, when there is nothing any of us could’ve done to have this happen. I’ve seen people get quoted anywhere from $500-2,000 for this exact problem. It is so beyond distracting while driving.

This same event has happened to me approximately 3-4 times since owning the vehicle. It occurs randomly without warning. The vehicle screen will randomly and suddenly freeze up and render itself inoperable. Eventually, the screen will reset itself or I will have to manually force a reset. After about 5-10 minutes, the screen will reset and the problem goes away. During the 5-10 minutes of the screen not operating, I cannot see or change any of the vehicles controls; loss of HVAC control, cannot turn on windshield wipers, cannot see vehicle speed, don't know if turn signals are on/off (also lose audible sounds when turn signals are on) are the major safety related losses; you essentially lose all control over the vehicle settings/controls. Vehicle continues to drive as normal, and all functions are restored after reset is complete.

The supermanifold failed, causing the front defroster to not work properly, resulting in loss of visibility in freezing temperatures. This is a common problem with 2021/2022 model 3/model y with the new heat pump that was supposed to be fixed with a software update, but was not. I now have a $3900 repair bill on a 1 year old car with 56,000 miles. The warranty expired at 50,000 miles. The failure was not a result of negligence.

I'm getting a notification that says " cabin climate control system requires service cabin heating/cooling limited or unavailable". As a result of this failure the defroster is not working properly. The windshield defroster will only blow cold air very slowly. This causes the windshield to fog up very quickly in cold temperatures. I cannot see out of the windshield properly.

However, today 10/6/22 during an hour and a half time frame my car has had phantom braking episodes at least 5 times in the middle of the highway where it's speed suddenly drops by 20mph.

Description of Problem: On or around [XXX], Tesla pushed a mandatory over-the-air software update to my vehicle. Immediately after, multiple safety-critical systems failed: all cameras (including backup) became intermittent or non-functional, auto steer/autopilot stopped working, and the car emitted constant distracting beeping alerts due to a critical motherboard failure. This made driving hazardous—impaired visibility for parking/reversing, loss of collision avoidance features, and constant audio distractions that could lead to accidents. I had to drive in this unsafe condition for weeks while waiting for service. Tesla attempted a remote fix but it failed. I scheduled service on November 19 for December 3 (19-day delay). Diagnostics confirmed the update induced the hardware failure, a known issue reported by other 2021 Model 3 owners on forums like Reddit (e.g., software bricking motherboards post-update). Despite this, Tesla refused warranty/goodwill coverage, quoting $2,576.83 for repairs. Service advisors Josh and Andy were unprofessional: Josh was rude/condescending; Andy was aggressive, sarcastic, and retaliatory. Vehicle does not have interior rear manual door release to allow rear seat occupants to escape in case(s) of emergency or vehicle electrical power failure 2. Vehicle does not have an exterior manual release for all doors in order to allow first responders the ability to assist or rescue vehicle occupants and/or driver 3. Vehicle doe not provide regenerative charging of 12V batter used as the only primary source of power to operate all Onboard Computer and Diagnostics, as well as electronics 4. Vehicle does not provide redundancy to operate Onboard Computer and Diagnostics or to maintain the electronics in order to allow vehicle to keep satellite contact or radio beacon 5. Vehicle does not have a mechanical locking system and is unable to be opened in the event of failure of the 12V battery 6. Vehicle does not have any audible seatbelt warning 7. Vehicle does not shift into Park or Neutral when drivers door or passenger doors are open 8. Vehicle interior parts and exterior parts are not securely fastened or reasonably installed to a standard that provides protection to occupants in cases of side impact, rear impact, and frontal collisions 9. Vehicle does not adequately provide secure installation or covering of Side-curtain Airbags, as well as a poorly designed tether that can easily be pulled triggering the Airbags gas canister 10. Vehicle does not have any audible seatbelt warning 11. Vehicle does not shift into Park or Neutral when drivers door or passenger doors are open 12. Materials used in vehicle are both flammable and gaseous, as well as produce a toxic smell when ignited 13. Vehicle "Panoramic" glass roof provides UV protection and actually absorbs heat 14. On [XXX], I was given a Tesla Model 3 loaner vehicle (VIN: [XXX] ) from the Knoxville, TN Tesla Service Center while my own Tesla was in for warranty repair. At pickup, I told the service advisor that the passenger-side tires looked unsafe. No inspection or warning was provided. After driving only 7.7 miles at normal speed on smooth roads, both passenger-side tires failed simultaneously. There were no potholes, curbs, or debris impacts. Tesla’s own service invoice states the cause was “not specified” and only speculates possible curb or debris damage without proof. Two tires failing on the same side so soon after hand-off suggests a pre-existing structural defect or safety issue. This incident posed a serious safety risk to myself and other motorists and could have caused loss of control at highway speeds. Pirelli PZero 235/35/R20 tires on the entire car, was driving when front left tire blew out after 4500 miles, had the car towed back to Tesla dealer and tire replaced. Now after 2 months, rear left tire has bubble on the sidewall after only 5400 miles with no indication of previous damage. I believe this to be a defective tire and an immense safety hazard.

The 2017-2020 Tesla model 3's have a recall for the rear camera wiring harness. The recall ends with cars built until September of 2020. My car was built in December of 2020 and my 2021 tesla model 3 was just repaired and had the exact same issue. The coaxial cable wore out and my rear camera didn't work. Because my car wasn't included in this recall, i had to pay for it. This recall needs to be expanded to other year models of the tesla model 3. The problem isn't isolated to vehicles prior to 2020.

While backing up, the rearview camera feed occasionally displays a noticeable delay, which prevents objects and pedestrians from appearing in real time. This lag creates a significant safety risk, as the visual information on the screen does not accurately reflect the vehicle's current surroundings. Additionally, the display on the left side (vehicle proximity visualization) does not match the camera view or actual situation, further contributing to confusion and potential danger when reversing. This issue persists despite software updates and appears to be a systemic flaw that compromises driver awareness and vehicle safety. Cross-traffic alert appears impacted.

Passenger SRS system fault. Initial error is due to a fault with the occupancy sensor. Presumably disabling or degrading seatbelt pre accident tensioning and proper airbag deployment. Also causes repeated & distracting beeps throughout drive.

This "self driving" Model 3, with FL License Plate [XXX], attempted to merge onto I95 south from an on-ramp, and very nearly caused a major accident. The vehicle had a bumper sticker on the back that said something like "please be patient, I am a self driving vehicle." The vehicle was merging from a left side on-ramp, at a VERY LOW SPEED, and crossed two lanes of interstate traffic, very erratically accelerating, then braking, then accelerating again, and then braking. My vehicle was directly in its path, and I had to brake hard, and take to the shoulder to avoid the collision. If this vehicle's trips are logged, it needs to be investigated. If it weren't for the humans paying attention, the actions of the Tesla car/driver would have caused a major accident, and most likely bodily injury or death. The contact stated while driving at various speeds, the hood opened unintentionally, the A/C unit and the radio intermittently turned on and off, the infotainment system screen intermittently turned black and randomly turned back on, along with several other failures that occurred. No warning lights was illuminated. The failures occurred on several different occasions. The contact was notified that the VIN was included in NHTSA Campaign Number: 24V554000 (Latches/Locks/Linkages, Electrical System) however, while attempting to update the vehicle software, the vehicle failed to complete the software update. The vehicle was drivable and was taken to the place of residence; the contact attempted to get in contact with Tesla Roadside Assistance however, once the contact reached the "confirm pickup spot" screen, the screen froze and would not confirm the request. The vehicle was not repaired. The contact attempted to notify the manufacturer however, the contact received no response in return. The failure mileage was unknown.

[XXX] I have taken stills from that video that show the Tesla wheel lying on the roadway well before the crash site and marks on the road that show the car slewed sideways after the wheel fell off and then crashed and burned incinerating the young driver. The contact owns a 2021 Tesla Model 3. The contact stated that while attempting to park at 5 MPH, the brake pedal was depressed however the vehicle accelerated independently. As a result, the contact crashed into the two parked vehicles on the side of the parking space she was attempting to park. No air bags were deployed. One parked vehicle had an occupant and the other was unoccupied. There were no injuries or medical attention required. A police report was filed. The manufacturer was made aware of the failure and the vehicle was taken to an unknown dealer where it was unable to be diagnosed due to no available appointment. The dealer then suggested the vehicle be towed to the contact's residence where it currently remains in the garage. Tesla informed the contact that the brake system would not be inspected until she repaired the crash cosmetic damage to the vehicle. The approximate failure mileage was 33,000.

Tesla is claiming it is driver error. That is false my foot was pressed fully on the brake. Scared to go back in the car. I don’t want that car on the road. I hit the curb went airborne missing a car, sign, brick wall, trees onto an embankment. I have a wittiness too.

On certain sections of highway unsuccessfully attempted to activate auto-steer (two down clicks) multiple times in a row. Did this multiple times in a row but was unsuccessful to activate auto-steer. Instead a supposed "easter egg" (which activates with 4 down clicks) very loudly plays loud jarring music (called "rainbow road")...this is NOT FUCKING FUNNY--I nearly ran my car off the road and into oncoming traffic. Giant 2000 pound hunks of metal going at 60 mph should NOT FUCKING COME with special surprises. This is incredibly dangerous and jarring to drivers.

This is a complaint about Tesla using unqualified drivers to test Atopilot Beta on the public roads. Please note that the VIN of a random Model 3 Tesla has been used to enable this submission to be completed. Here is a video of a driver testing Autopilot on what is in his own words the most dangerous road in Virginia. https://www.youtube.com/watch?v=7VBhdrj1CqM He is driving hands free around sharp bends and against oncoming traffic entirely hands free but just giving the wheel a little nudge with his finger every 30 seconds to defeat the nag. At one point the car does swerve towards an oncoming car but corrects itself just in time. The driver is alert and monitoring the situation but because he does not have his hands on the wheel he is playing russian roulette with many lives. Please do not allow this madness to continue for one day longer. Elon Musk thinks this is all a joke which is why he has called it release 69. Smutty and unamusing. I will be posting a copy of this report on social media where it will be widely read. If you fail to take action there will be consequences. Thank you. The Tesla uses “auto high beam” when in autopilot. The pros of this can allow drivers to utilize the light so the cameras can detect anything to avoid any possible wrecks, but it does not have an option to be turned off. Along with other people online, my brights have flashed at people on the interstate and highways or haven’t turned off at all. This is irritating and limits the way I am comfortable with using autopilot. Please inform Tesla to make the auto high beam feature OPTIONAL.

Car brakes while using adaptive speed control. In one incident the car was traveling at 71 miles per hour and braked to 25 mph. In other situations the car braked by at least 5 mph, on some situations this happened while speed control was engaged and I was traveling at 50 mph or less. In all these situations, there was not another vehicle in front of me that posed an immediate threat of collision. The adaptive self dimming headlights do not work properly while driving at night. Lights do not always dim for approaching vehicles or when approaching another vehicle from behind. On some occasions the bright lights engage when approaching other vehicles OR dim when not approaching another vehicle. Other times the light repeatedly switch from bright to dim when following other vehicles. I reported these issues to TESLA. The response was that their engineers are aware of the problenms and it is related to a firmware update and that will be coming. I worry about being struck from behind when the brake suddenly engages. I also worry that the headlight situation can cause a crash by blinding drivers.

The TPMS alerted me that the front driver tire was low at 36 psi. Pulled into gas station within a mile from initial alert and when I unscrewed the valve cap the valve stem broke and shot out of the tire and the tire deflated. The components were found and are available for inspection. The car has not been repaired yet and is currently at the Tesla Service Center in Agoura Hills at 28721 Bldg A Canwood Street Agoura Hills, CA. I drive two children under 5 and am lucky this did not occur at high speeds or while the vehicle was in operation at any speed. There is no reason this should have happened. There was no prior damage to the wheel or tire.

The tire fell off the car while driving. The lateral link bolts have come unfastened and cause the link to come out of place and consequently damage several suspension components. As the bolts unfastened, the wheel was separated from the car and fell off, causing the car to be undrivable.

The adaptive cruise control (Tesla "autopilot") system has unexpectedly disengaged and applied light to heavy braking numerous times. The system frequently mistakes a particular highway sign for a stopped vehicle and applies moderate braking. The system has also applied light to heavy braking in circumstances where I simply cannot figure out what the camera is interpreting as a hazard. This puts me and other drivers around me at risk as it is unnecessary and unpredictable braking at highway speeds in traffic. Tesla has not been able to remedy this problem and I do not consider the cruise control system to be safe to use. This has happened more than 10 times.

While driving the vehicle using autopilot or adaptive cruise control, the vehicle "phantom brakes" - meaning the vehicle abruptly applies the brakes to slow down even though there are no obstacles in the road. This happens almost every time either one of these systems is used.

This is a complaint about the unacceptable fire risk that affects all Teslas due to the dangerous battery chemistry and the vulnerability to impact damage that can trigger an instantaneous fire . The VIN of a random Tesla has been used. This problem affects every Tesla. Here is a video of a test conducted by a European testing authority which shows the Tesla igniting almost immediately after a deliberate crash that only caused a minor strike to the underside. https://www.youtube.com/watch?v=QRDCKfCdh90 When NHTSA and many other testing authorities perform crash tests on Teslas they should do so with a fully charge battery that is warm from charging or from having being driven. This is a much more realistic scenario than using cold depleted batteries that are less prone to instant ignition. By comparison with a Tesla the Pinto was just a damp squib. And yet it was recalled and grounded. Please do the right thing before even more people get burned alive. Thank you

Vehicle displayed "no capable drive units" error at 106 miles on 10/08/2021. System shut down and stranded me in downtown Tampa traffic less than a mile from my home and fewer than five minutes after I started the car. All propulsion was lost and I did not have sufficient warning or time to process the situation and safely handle the vehicle. Tesla Tampa Service Center diagnosed a failed drive unit and took approximately 27 days to repair the vehicle. The service center invoice described the drive unit replacement as a goodwill repair and not a warranty failure. This seemed odd to me as goodwill repairs are generally special accommodations to satisfy customers. I was told there were parts delays. Tesla seems to be managing its supply chain to get these cars turned around to avoid lemon law exposure (my wait was just under one month) and to suppress the number of warranty failures by calling them goodwill repairs. All at the expense of customers. The service center was generally arrogant and noncommunicative in resolving the issue. Happy to gather records and discuss further.

Phantom braking. The Tesla brakes without warning or provocation when on cruise control. On two lane roads brakes deploy forcefully every time a semi truck is traveling in the on coming lane. It does not have to be on a curve. It is every time a large truck is in the opposing lane. On 4 lane divided highways, it will forcefully brake (from 75 mph down to 40 mph in seconds) climbing small hills, patches of tar on the roads, shadows, off ramps, side roads and for no apparent reason. I have almost been rear ended when a car is following closely and it happens. I have recorded these incidents , reported to Tesla, taken the vehicle for diagnostics and they have no answer and imply that occasionally it may happen however, it should not be an issue. IT IS A DANGEROUS PROBLEM and an accident waiting to cause serious injury. I drive daily with my job and it happens every day.

I was pulling up next to a curb inside a neighborhood to park my car. I released my foot on the accelerator pedal to slow and stop my car. I slowly stepped on the brake pedal to switch the gear into reverse and my vehicle sped up very fast and hit the parked car in front of me and ran over the curb, my steering wheel also shifted to the right as if it was trying to dodge an obstacle on the road. It happpend so fast that I could not avoid the impact. I could have ran into a pedestrian and put their life at risk, or injured the person in the car that was parked, and even hurt myself badly. I've been getting neck pain and headaches. This incident was very terrifying and traumatizing. I tried to reach out to Tesla Service Center, but my tesla app was not working after the incident. The Tesla app was frozen for 10-15 mins after the incident. I kept trying to reach out to Tesla Service center and kept getting an automated response to utilize the Tesla App. Finally, the app started working again, I left them a message regarding the accident and photos, and they responded two days after, but did not address the issue about unintended acceleration until I kept asking why are they ignoring my concerns about the uninteded accerlation, and then I received a phone call from a technician. He said that I do not have to bring my car for inspection, instead they will just check the logs and could take up to 8 weeks to see what happened. Estimated cost for exterior damages according to Geico Insurance is 5344.74. Tesla Service Center denied having the car inspected physcially for any sensor issues and malfunction issues, states that I don't have to bring my car in at all, they will submit a report for engineering team to review the driving logs at the time of the accident only. My car is currently towed to a 3rd party body shop that is Tesla certified. No warnings from my car screen alerts or anything prior to the accident.

WHEN I TEST DROVE THE TESLA MODEL 3, THERE WAS AN OPTION TO TURN REGENERATIVE BRAKING TO LOW. THIS ALLOWED THE CAR TO HAVE MORE OF A COASTING EFFECT WHEN ONE LIFTS THE FOOT OFF OF THE ACCELERATOR. WHEN I RECEIVED DELIVERY OF THE CAR, THE ABILITY TO REDUCE THE REGENERATIVE BRAKING EFFECT WAS NOT AVAILABLE. THE REMOVAL OF THIS FEATURE IS BEING DISCUSSED AT LENGTH IN ONLINE FORUMS. THE LATEST VERSION OF THE OWNERS' MANUAL STATES THAT REGENERATIVE BRAKING IN THE STANDARD SETTING COULD CAUSE THE CARE TO LOSE TRACTION IN WINTER CONDITIONS AND THAT DRIVERS SHOULD SELECT THE LOW SETTING. BY REMOVING THIS ABILITY, TESLA, BY ITS OWN ADMISSION, HAS DECREASED THE CAR'S SAFETY IN WINTER CONDITIONS. A RAPID DECELERATION OF A CAR ON SLIPPERY ROADS IS ALMOST GUARANTEED TO INDUCE A SKID AND MAKE THE CAR DIFFICULT OR IMPOSSIBLE TO CONTROL. I FIND THIS EXTREMELY CONCERNING. TESLA NEEDS TO BRING THE FEATURE BACK, OR PUBLICIZE OTHER COMPENSATIONS THAT MAY HAVE BEEN MADE UNBEKNOWNST TO OWNERS. I TOOK DELIVERY OF THE CAR ON 12/24/20, BUT ACCORDING TO ONLINE FORUMS THIS SEEMS TO HAVE HAPPENED STARTING IN OCTOBER OF 2020. A SIDE COMPLAINT WOULD BE THAT TESLA DOES NOT UPDATE THE OWNERS MANUAL IN A TIMELY MANNER TO COINCIDE WITH THE MANY SOFTWARE UPDATES. THE ABOVE EXAMPLE IS JJUST ONE OF MANY, SINCE THE CURRENT MANUAL MAKES A RECOMMENDATION THAT IS NOT POSSIBLE TO DO IN THE 2021 MODEL 3 REFRESH.

After parking and leaving the car, I realize later the dashboard did not shut down. The dashboard was still illuminated, and I was also able to hear the sound from a tuned FM radio. I re-open and closed both the driver and the passenger doors in an attempt to shutoff the dashboard. Since I did not get the expected result, I decided to get back into the car. I sat down in the driver chair to investigate the problem when the car was instantly activated and switched to a sudden and uncontrolled acceleration. It crashed into a facing wall after a run of about 30 feet.

After Tesla replaced my high voltage battery in May of 2024 all of the following started to happen. Prior to the replacement none of these issues were present. While driving under normal conditions, my 2021 Tesla suddenly and violently steered into an oncoming lane or toward the roadside without any driver input. The steering wheel physically turned on its own with force. Full Self-Driving was NOT engaged at the time of the incident. This was not lane assist correction and occurred without warning. Importantly, the vehicle did NOT issue a warning at the time of the steering event. A “Take Control Immediately” message appeared several minutes AFTER the incident, not before or during, indicating a delayed or failed system response. This defect began immediately after a High-Voltage Battery replacement performed by Tesla Service in May 2024 and has occurred multiple times since. The incident presents a serious safety risk, including loss of vehicle control, roadway departure, or head-on collision. In addition to the steering defect, the vehicle began exhibiting multiple electronic and control anomalies immediately after the High-Voltage Battery replacement. These issues did not exist prior to the battery replacement and support the presence of an underlying electrical or control system instability related to the steering defect.

This car has 69k miles and I'm informed this is out of warranty and has a cost of $696.75 to repair on a car that is less than 4 years old. Saying the sensor failed is not accurate as the error does not exist unless there is someone in the passenger seat which tells me the sensor is working somewhat. Tesla will not inform me of what will and will not work in the event of a collision but that these items need to be replaced to ensure safety of occupant's. I am skeptical since safety is your number one concern and should be the concern of every auto manufacturer. Unless damage by the owner specifically, how and why is this not covered for the life of the vehicle?

My Tesla was operating in Full-Self-Driving Beta when it experienced a sudden uncontrolled accelration event. I was driving on [XXX] at approximatley [XXX] this morning. The road speed limit was 40 mph. The car display was correctly showing that the road speed limit was 40mph. The speed offset in the cars setting was limited to 5 mph max above the speed limit. While the car was driving it suddenly accelerated very hard from approximately 40 mph to over 60mph before I was able to stab the brake pedal and kick the car out of full self driving mode. There were no errors, warnings, or signs that anything was wrong with the car. I have also notified Tesla via the service request function in the Tesla App under the section for "software performance". While driving, I started to hear a creaking sound while backing up and turning the wheel left. The sound became worse over the course of a few weeks. The steering also felt a little loose. I reached out to the manufacturer for a service appointment, and could only be scheduled about a month out. After a few weeks, while trying to parallel park, the car felt like a tire was stuck, or had a flat. I stopped the car to look at all of the tires and they looked fine. When I returned home, I tried to recreate the issue with my wife moving the car slowly from the driver seat while I looked at the wheels. As she turned and backed up, the creaking sound was very loud, but the tires looked fine. When she put the car in drive and began to pull back into the driveway, I could see the front passenger tire was turned abnormally far to the left, looked like it was leaning, and it was locked (not rolling). Once we had the car in a safe position, I jacked up the car and looked at the passenger control arm to see a bolt completely missing from the front suspension lateral link. The other bolt was 1/2 of the way out and barely holding on. If this had come apart at highway speeds, I am afraid it could have killed or seriously injured my family.

I started receiving messages that the cameras were occluded and that there was an error and ALL of my system safety features were disabled. No front forward collision warnings, no emergency braking available, nothing available and the system computer that showed all of the traffic and everything completely disappeared from the screen, the car has literally gone blind. All of this started happening on with the camera issues in May, but I thought that it was a software issue and would be addressed in the next update, although the critical disengagements were supposed to be sent to Tesla. I took the car to the service center on 9/4/24 at 9am, and they stated that the did a system test and saw that the triple camera was showing a fault and so they replaced the camera and gave it back to me the same day. Everything worked for the rest of that day for a service that took at least 3hours to figure out. Today after I get off of work at 1pm and I go to initiate FSD and I get 3miles from my residence( I live <8mi from my job) and the system disengages again asking me to take over and all of my critical systems have yet again been disabled. My whole safety is compromised at this point yet again. I immediately bring the car back and they tell me that it's going to take them an additional 30-45 mins to "diagnose" the issue although it's the same thing that's been happening that was supposedly fixed yesterday. After that wait time the service consultant comes back and tell me that the car is throwing all kinds of codes and that, "It's still safe to drive." When I told that it cannot be safe to drive if all of the critical systems were disabled he suggested that I take it and they can do a remote log pull so they can look more into the issue or I can leave it as the part that's needed needs to be ordered anyway. I don't know how they can order a part if they're claiming to not know what's wrong with it. Something else is going on that they don't want to take accountability for!

The vehicle, a 2021 Tesla Model 3, had an episode of sudden unintended acceleration. I was almost stationary waiting to make a left when the car accelerated by itself, left the road, crossed a grass field at high speed and crashed into bushes. The vehicle was not on auto pilot, it resulted in damage of almost $ 20.000 as estimated by insurance. The vehicle is available for inspection, it will be checked by Tesla service after the damage is fixed. There were no previous warnings, this is the first incident of this kind. It is not clear what component malfunctioned, probably some electric/electronic device in charge of acceleration. It was obviously a very dangerous event that could have jeopardized my safety and/or some others.

Tesla ADAS detects non existent objects and issues collision alerts and phantom brakes randomly if there is no cars in front of the Tesla, braking abruptly and almost getting me into rear end accidents. Tesla ADAS freaks out when making lane changes randomly, shaking violently and giving the driver and its passengers a fishtailing simulation. Auto wiper triggers randomly while it's sunny, distracting the driver and forcing an interaction on the center console to stop it. Auto wiper cannot be turned off as it reengages upon using Tesla ADAS. The rear view camera lags. It stutters and shows me a delayed view. Compounded by the Model 3's very small rear view mirrors and side mirrors, this is incredibly dangerous as I have no way of gaining sufficient visibility of the back of the car. Tesla service told me this is all normal.

My 2021 Tesla Model 3 has various issues with the auto pilot feature. When engaged the car makes sudden and abrupt stops while driving. This has happened various times while driving on the freeway when there were no obstructions present. The autopilot feature also prompts to change to the right lane, when dismissed the prompt appears again repeatedly. This is repeated throughout the drive and poses a major distraction. There was supposed to be an “over-the-air” correction but my vehicle has never received the software update.

My car computer touchscreen has been rebooting with every entry since/start since software update 2023.44.30.5.1. in mid December, 2023. Software update 2023.44.30.8 did not resolve the issue. During the reboot process I am able to move my vehicle but the safety systems (parking sensors, back-up camera, wipers, etc) are inoperable for up 3 minutes during the reboot process. I have reported the issue to Tesla and brought my vehicle in for service 1/29/24. The service center claims that reboot issue is due to a software bug and is a “known characteristic” with this release. They also claim that the lack of the ability to use cameras/sensors while operating the vehicle is convenience related, not safety. Tesla can give me no ETA on when I can expect a software update to remedy the issue which has been outstanding for 5 weeks now.

The cruse control is unable to keep its speed and sometimes the vehicule breaks suddently on the highway, especially when the road is wet. The cameras are obstructed by mist or other dirt and the system does not work properly. Related to this problem, the wipers are dangerous to operate. When the cruse control is engaged, then the wipers are forced to automatic mode, which does not work properly. It could start to swipe the dirty mist and actually reduce visibility. Operating the wipers with the touch screen is a hazard, forcing the driver to leave the eyes from the road. I contacted Tesla about these problems last year and the company did nothing. Even worst, on their new cars they removed the stalks completely, which is a non sens. The NHTSA has to intervene.

The rear seat doesn’t detect kids under 50lbs. Putting the seat in car seat mode only disables the seatbelt alert and doesn’t keep the rear fans on. You can turn them on manually, but the car forgets that they should stay on if you precondition the cabin from the mobile app. Meaning that my child cooks in the back seat due to lack of rear cabin conditioning unless I remember to turn it back on in a buried menu every time.

Ever since Tesla pushed a software update to my vehicle on or around 12/20/23, the vehicle screen crashes and reboots every time I enter enter my vehicle. The software version is 2023.44.30.5.1. The vehicles systems; backup cameras, parking sensors, etc. are in operable for approximately 2 minutes during the reboot process rendering the vehicle unsafe to operate. I have reported this issue to Tesla on multiple occasions with no resolution offered other than a note that this is “considered” normal with the current software version. I was pushed software update 2023.44.30.8 on 1/8/24 and the issue persists. I have scheduled a service center appointment for 1/29/24.

My 2021 Tesla Model 3 shuts/powers down WITHOUT warning while driving with a battery charge >0%. This unsafe event has occurred twice now! While the battery gauge displayed 10%, I was driving about 60mph when all of these warning messages appeared prompting me to pull over. As soon as these messages displayed, my car lost all propulsion and I was left stranded in the middle of the road. Both times I narrowly avoided being hit by other vehicles traveling at high speeds since my car died in the roadway. The car was towed to Tesla on both occasions, and they weren't concerned about my safety issue in the slightest. Instead, they said the battery just ran out; however my car displayed that I had about 6-10% charge remaining. So, if the battery were completely depleted (like Tesla says), then the meter should have displayed a 0% reading at the time of the breakdowns or, there is something defective with the battery/computer. Tesla service has refused to investigate the cause of the safety issue and, thus, refused to replace the EV battery (even though my car is still under the manufacturer's warranty). Electric vehicles should not die/run out of battery at 10% charge. However, my EV has done so TWICE! Tesla says that I just ran the car out of battery but has no response when I show them the pictures and video proving that the car died when there was, in fact, a decent amount of charge remaining on the vehicle (PLEASE SEE ATTACHED VIDEO AND PICTURES). If your phone died at 10%, then you would take it back to the manufacturer and demand that they fix the defective product. However, Tesla refuses to help fix my defective vehicle and just tells me that it is "operating as intended". EVs should not just lose all propulsion mid-drive. This creates an unsafe vehicle for me to drive, for my two young daughters in their car seats, and for other drivers who might hit me. I can provide other Tesla owners who have had a similar experience. Please force Tesla to take action!

I was driving to work that all of sudden heard warning sound and the monitor displayed the "Shutdown" sign as well. The car lost its power and came to complete stop in the middle of the street. Luckily there was no car near me. I was not able to drive the car and had to tow it to the dealership. According to mechanic “the rear drive unit has failed internally”. This was extremely scary and dangerous and may have caused a fatal accident. The problem has been fixed.

The contact owns a 2021 Tesla Model 3. The contact stated while driving out of a parking space, the steering wheel seized. The message "Front Motor Disabled. Continue Driving" was displayed. The manufacturer was contacted and provided remote assistance, but the failure persisted. The contact stated that the vehicle regained power steering functionality after an hour. The contact stated that the failure recurred while driving out of a parking space on another occasion. The vehicle was towed to a Tesla Service Center however, the failure could not be duplicated; however, the circuit board was replaced. The contact stated that the failure recurred while pulling out of a parking space on a third occasion. The vehicle was towed to a second Tesla service center, Tesla Sales (750 E El Camino Real, Sunnyvale, CA 94087) who informed the contact that an unknown software update was needed. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 17,000.

The driver's seat automatically adjusts when I get into the car. Normally this is a good thing, right? But when my wife and I both get in the car, and I'm going to drive, the car often assumes my wife is the driver and adjusts the seats to her settings as I'm getting into the car. This is a serious problem that has the potential to injure me. Since I'm tall, and my wife is short, the seat starts moving forward as I get in, and it starts to crush me. It's too hard to get back out, and I'm not always able to select my seat configuration quickly enough. If I happen to be carrying something as I get into the car, then that object usually is compressed between me and the steering wheel. There have been times where I didn't have my legs in fully, and they started to get crushed by the seat pushing them forward into the steering wheel. I've talked to Tesla about this several times. Tesla employees have told me that they hear about this problem regularly, but they don't have a way to fix it. And when I ask how to contact Tesla, they tell me use Twitter. I think Tesla's service department should have a method to communicate customer complaints directly. No one has given me any feedback indicating that they intend to do anything, even though they acknowledge that this is dangerous. I also wonder if it is possible that this might happen while I'm driving! For example, what would happen if the battery on my phone drained? The bluetooth connection would be lost. Would the car then pair to my wife's phone and readjust the seats? Of course, there is a simple fix. Instead of doing the automatic but stupid and dangerous adjustment, it should just ask for my confirmation. And never automatically go into a mode of "let's crush the driver"! I only want a computer to think for me if it's going to reason properly, in every situation, every time. Clearly Tesla didn't think this one through.

On May 22, 2023 driving south on I-580 through Reno with the Tesla adaptive cruise control, there's a 55mph construction zone. As a result, I use the right scroll wheel to adjust the speed from 65 to 55mph. The scroll wheel didn't work. As a result, I turned off the cruise control with the right steering wheel stalk. At the same time, I noticed the vehicle speed indicator on the upper left of the CRT display was malfunctioning; both failing to respond to changes in the car's speed, and then indicating erroneous speeds (at one point indicating 34mph when I was going around 60mph. These issues continued. I took the exit on Double R Blvd to turn right. As I got into the right lane on the freeway to exit I attempted to turn off the cruise control and it didn't work. Approaching the exit, I then applied the brake. The regenerative braking function did not apply. As a result, it felt like the cruise control didn't turn off. However, I braked to a stop at the intersection in the right lane with no issues. When the traffic cleared, I attempted to turn right and proceed east on Double R Blvd. The car didn't move. Using the right stalk, I tried shifting into drive, reverse, and back. All this time, the drive (D) and reverse (R) indications on the CRT did not show. I let off the brake and the car inched forward. I turned right immediately into the Maverick gas station entrance to get off RR. The CRT display now showed the back-up camera displays (a normal indication of the car being in reverse). I cautiously let off the brake and applied the accelerator pedal to see if I backed up, but the car was in drive and proceeded normally. I continued to drive and the CRT screen went completely black. I soon realized the car's computer was rebooting with the faint Tesla logo on the black screen. Within a mile or so, everything came back on the screen and worked normally. I then reached my destination. When I returned to the car, it was as if nothing had happened.

The contact stated while driving approximately 25 MPH as he was exiting a highway and slowing for a traffic signal at the bottom of the off-ramp his brakes failed to engage. The contact stated the brake pedal was stiff and he had attempted to use full leg force to depress the brake pedal. The contact's vehicle ran the traffic signal and the contact stated he hit the right front end of a vehicle that was across traffic from him. The contact stated no warning light was illuminated. The contact stated that the crash stopped his vehicle and the other vehicle. The contact said that the air bags had been deployed and that the left front end of his vehicle was crushed into the left wheel well. The contact stated that the other vehicle received only light damage to the right end of the bumper due to it being higher than his vehicle. The contact stated that the police were on the scene and filed a report. The other vehicle was able to drive away, and the contact's vehicle was towed to an impound lot. The contact stated that his insurance provider declared the contact's vehicle a total loss. The vehicle had not been diagnosed or repaired. The contact stated that prior to the crash he had attempted to engage his cruise control while on the highway and it would not work. The contact stated that when he engaged the cruise control the windshield wipers would activate and he stopped attempting to use cruise control. The manufacturer had been informed of the failure. The failure mileage was approximately 18,000.

Fortunately it was late at night and there were no cars in the immediate vicinity. In addition to this the vehicle has had multiple episode of phantom breaking. I do not feel safe driving the car.

Random "phantom braking" while using autopilot or cruise control. Causes the car to slam on brakes and has lost control once almost causing me to go off the road. Also, has brake-checked many cars that follow behind me while I'm using autopilot or cruise due to the phantom braking. Also Tesla Repair center in Chicago replaced my front bumper with a discolored front bumper that had the same issue of peeling on the inner edges of the bumper as the prior front bumper. When I brought up the issue to Tesla they said that was a manufacturing process issue due to the molding process of the bumpers and that is how they all are. If that is the case why change the bumper? Could have just told me that's how all the cars are and saved a lot of my time. Also, have had many repairs from Tesla that don't seem to end. These cars arent being produced in a consistent manner. What was promised during the purchase of the car doesn't seem to hold up.

Tesla Model 3 main computer system failure. Failure was not cause by any negligence by the operator. The vehicle was parked over night and upon attempting to operate the vehicle the following day there was no power to the screen. There were no warnings prior. The screen which displays vehicle speed, vehicle battery level, seatbelt warning indicators was inaccessible. Also as a result of the computer failure the vehicle did not emit the required pedestrian safety sound, the front and rear collision warnings were disabled, charging was not possible, the ability to disable passenger airbags in case of a child needing to be transported in the front was inaccessible, if the vehicle was operated during winter the ability to defrost the windshield would be impossible and all safety and security features were disabled. The screen/computer has been an issue for several owners as easily viewed by a simple google search. The vehicle is only a year and a half old and was purchased new. The issues that occurred from the defective computer could of easily have caused of an accident or a caused disabled pedestrian to have been hit. The computer was deemed the cause by Tesla’s service professionals.

The contact's son owned a 2021 Tesla Model 3. The contact stated that his son was involved in a fatal collision while driving at an undisclosed speed with the autopilot activated. The contact was informed by his son's friend, who was a passenger in the vehicle, that the autopilot malfunctioned, causing the vehicle to inadvertently drive off the road, hit a tree, and then catch fire. An unknown bright orange light was seen on the windshield right before the failure occurred. The minutes long clips show what happens when the mainframe screen goes black, dies out or powers down while driving. Listed from memory are the following concerns associated with the Tesla ‘Black Screen of Death’: 1. No energy/distance shown 2. Glove box is inaccessible 3. Front trunk ‘Frunk’ cannot be opened 4. Proof of signalling is gone 5. Proof of low vs high beam is gone 6. Changing of wiper speed is gone 7. Access to heating/Air-Con is gone 8. Back up camera is obsolete 9. Cameras used when signalling are missing 10. Speedometer is not visible 11. Odometer is missing (have never found it on screen when running anyways!) 12 Air pressure indication is gone 13 GPS is obsolete From memory that is all I have to add. Imagine everything that was/is available to the driver in every other cars dashboard and then take those parts away sporadically for minutes at a time and you’ll have an understanding of how precarious driving my Tesla can be. As an Uber driver I had the opportunity to speak with a corporate agent who was my passenger. She said Tesla is aware of this issue. Having spoken with a Tesla rep at a Tesla garage I was given the same advice found on Internet forums: “Find a place to park, park, press the steering volume & station finder buttons for 10 seconds until display starts up” This very rarely works, and is impossible, even dangerous in many situations. I’ve learned to drive with this continually faltering equipment, and I know every Tesla driver I’ve spoken with must do the same to varying degrees. While all Tesla drivers experience this, none are as repetitive as mine and none seem as proactive to inform government agency of this. We were driving on the freeway at 69 mph and suddenly with no warning the center console main screen froze. All electronic functions of the car stopped and the car was stuck at 69 mph. Speedometer And turn signals did not work. We managed to get to the side of the road (mechanical brakes and steering were unaffected). The car could not be put in Park so a manual reboot was not possible. The car rebooted itself (screen went black and then came back online) after about 10 minutes or so and then the car functioned normally.

Tesla offers a blind spot camera, standard on the vehicle, which activates automatically when the turn-signal is active on the appropriate side. The blind-spot camera feed appears on the center screen. The feed comes from each of the side cameras mounted to the external portion of the car. Since the side turn-signal is mounted right above the same blind-spot cameras, the light from the turn-signal often 'bleeds' into the camera feed used by the driver to determine if the appropriate lane is clear. At night, this causes the blind-spot camera video difficult to view since the video feed alternates between 'normal' video and a bright, flushed out video, matching the turn-signal timer. The end result is often times I, the driver having difficulty observing cars on the side of my vehicle at night. More specifically, several times I have missed the headlights of cars in the lane next to me because the turn signal light, bleeding into the camera feed, obscure or hides the car and/or headlights (since they driver's eyes have to chateaubriand adjust between a bright andrark video feed flashing). This causes me to start merging into a lane which I believe is clear, only to have to quickly swerve back into my original lane when I realize the lane is not clear. Both the driver in the new lane and myself are put at risk, unnecessarily when this occurs. I have reached out to the manufacturer for this complaint. In writing, they have admitted this is a problem and even though my car is only 10 months into a 4 year warranty, they will only fix this safety issue if I pay for a replacement part. This seems unacceptable to me to force customers to pay for a safety fix. The failed component is available for inspection. I have sample video available to demonstrate the problem. There are no warning lights for any components. The manufacturer does not need to inspect the component since they already know, and admit to the shortcomings, per my written conversation with them.

The car was on FSD jerked right toward a semi truck then jerked left toward the posts in the median as it was accelerating and FSD would not turn off. Was trying to control the car but it kept pushing and accelerating causing us to crash in to the median posts. Normally FSD would turn off if it was unsure but this day it would not. Plus other times it would phantom brake if we were on 2 lane road and met on oncoming large truck. We also had other times of phantom braking going over a hill. We have reported this to Tesla but no response yet. We owned this car 11 days when our wreck happened.

Since the V11 user interface update, when you put the car into reverse, the rear camera sometimes does not show up, or often shows up only after a delay. Sometimes the delay is just a second or two, other times it is a minute or seems to hang forever. Today I was parked in front of my kid's school dropoff zone, and needed to back up after they got out to leave. When I went into reverse the cameras were just black and stayed that way. This was especially worrying in this scenario since small children could be walking behind me that I wouldn't be able to see without the camera. This was never a problem until the V11 user interface update, which also causes other safety problems, by burying important functions behind menus. For instance you now have to dig into a menu and reach all the way to the right side of the screen to operate the wipers, you have to dig into menus to operate basic climate control functions that used to be immediately available, etc.

Hello, Driving on a snowy road around 20 MPH. Attempted to slow down in a straight line, used the brakes to try and slow vehicle to go into a driveway. Vehicle would not respond to any inputs. The brake pedal seemed frozen, coming off and back on the brake pedal did not change anything. There sounded like an ABS pulsing but it was not working. Post accident pictures showed it looked like the car was dragging one axle like it was locked in the snow. Car ending up grazing a mailbox post and afterword said 'take control'. I believe the car tried to do an automated braking maneuver which did not work. The road was covered in 1-2" of snow and the mailbox would have been a reference point in the distance. When turning the steering wheel away from the object that was hit, the car continued to go straight like the wheels were not attached. The car never veered left or right during this whole event even though i tried to turn.

I was driving north on Wisconsin route 14 at about 60 mph in my Tesla model 3 using the cruise control.. When a large transportation truck came from the opposite direction, my Tesla suddenly braked sharply. A Ford model F150 truck was following behind me and almost crashed into the back of my car. Fortunately the driver was alert and by sharply apply his brakes on driving into the ditch he avoided colliding with me. This could have been (and nearly was) a lethal accident. I would urge that you put a recall on a Tesla vehicles until this problem is corrected. I have contacted other Tesla owners and they have experienced similar problems. I contacted my local Tesla service station and they seemed unconcerned about the problem. I urge you to issue a recall before someone is seriously hurt or killed.

I want to report an issue I have with Tesla concerning the damage that occurred to my vehicle as a result of a failure of Tesla FSD version 10.3 firmware release 2021.36.5.2 recalled by Tesla on 10/29/21 NHTSA recall number 21V-846. At noon on 10-24-21, I was testing the latest release of FSD as part of the beta program. My Tesla was in self-driving mode on a city street at 35 mph. This was the same route it had already successfully done earlier in the day, when suddenly the car jumped over the curb, causing damage to the bumper, to the wheel and a flat tire. This appeared to have been caused by a discolored patch in the road that gave the FSD a false perception of an obstacle which it tried to avoid. Immediately after the accident, the car was towed to the service center. The repair was finished on 10/27/21 at which time I was asked to pay $2,332.37 to take possession of the car. I explained that the damage was caused by the recalled version of FSD NHTSA recall number 21V-846 and that Tesla should be liable for the damages. I was advised that the engineering team would conduct an investigation and based on the results the money would be reimbursed. However, I did not agree to pay hoping that after the investigation Tesla would honor the recall. After two months of waiting, there is still no communication on the results of the investigation even though the case is quite simple. The car was going straight at a very low speed in self-driving mode. There is nothing except the recalled version of FSD that could have changed its trajectory. Therefore, it is obvious that nobody looked at this issue and there is no commitment to doing so anytime soon. I communicated using the Tesla app chat that I am not willing to wait indefinitely and gave notice that I intend to file a petition with the Court. I gave e-mail notice to Tesla’s resolution team as well to make sure that my notice reaches Tesla’s legal department. I have paid the invoice on 12/22/21 to get my car back.

break backing plate screw loosened and go into the break disk. it create noise when driving and breaking. the loosened screw might the damage the break rotor. the screw was removed from the break disk and tighten again by independent service center The manufacturer have inspected no warning lamps but it create noise. so it is not hard to detect this issue. it first appear on 19 Oct and I have the service center fix it right the way.

On September 1st, I was perpendicular back parking on the street in front of my home. Spaces are fairly tight and the street fairly narrow, so it is not uncommon to shift between reverse and drive a couple times to adjust one's fit before parking. That said, a quarter of the way into my parking spot, I shifted from drive to reverse after finishing adjusting. After doing so, I pushed on the accelerator and was sent forward as if I were in drive. I toggled between drive and reverse multiple times over the course of 15 seconds. Pushing on the accelerator continued to send me in the opposite direction, causing me to hit the sidewalk curb in front of me multiple times. The situation resolved itself after 20 or so seconds. Then, the same exact situation occurred on Sept 7 and again on Sept 13, and those times, I nearly hit the car parked adjacent to me. All of this occurred while hold mode was on, meaning the car was not simply swaying with gravity. There could be a host of defects involved in the manifestation of such a problem - perhaps a bug in the software which controls the power train. The swapping of reverse and drive for no apparent reason presents a huge safety concern/liability. I could have hit pedestrians on the sidewalks in front and behind or severely damaged mine and someone else's vehicle. My street is one of two which allow access to a monument. That said, there are always a ton of pedestrians, many of which are young children. I scheduled a Tesla service appointment after the first occurrence. Timestamps and video of each of the incidents was captured and provided to Tesla. They claim there is no issue and that the car was simply rolling due to gravity - despite hold mode. Furthermore, I drive on the same street every day and the aforementioned issue does not happen every time. Even given my three scary experiences, Tesla has refused to take any further inaction, other than to reassure me nothing is wrong with my vehicle.

When I was driving on highway the suddenly I lost power and if I press gas pedal it would not go. I applied brake to slow down and pull over but as soon as I applied the brake it corrected on its own and continued to go since I did had similar issue like this previously I called Tesla and asked them to tow it to the shop. This is the 3nd time that I'm having an issues with drivetrain that my car was towed to shop.

There was engine power loss while I was driving on the highway. So due to power loss I parked the car on the side and then put it back in to drive and it would go for approximately 10min then I lost power again, it started to do like these few times. Since it was unsafe I stopped the car and had the factory tow it to the shop.

HEAT NO LONGER WORKS IN THE CAR'S CLIMATE CONTROL. THIS APPARENTLY IS TIED TO FAULTY HUMIDITY AND TEMPERATURE SENSORS THAT MUST WORK PROPERLY FOR THE HEATING SYSTEM TO WORK, AND TESLA SERVICE CENTERS ARE AWARE OF THIS YET ARE NOT PROACTIVELY REPLACING THEM. THIS IS A SAFETY ISSUE IN COLD CLIMATES SINCE THE WINDSHIELD DEFROST NO LONGER WORKS, AND THE CAR IS UNSAFE TO DRIVE OVERALL IN COLD TEMPERATURES WITHOUT FUNCTIONAL HEAT. THIS ISSUE APPEARS TO BE RESTRICTED TO THE 2020/2021 MODEL 3/YS WITH THE HEAT PUMP THAT REQUIRE THESE SENSORS TO FUNCTION PROPERLY. ANOTHER SYMPTOM IS THE CLIMATE SCREEN DISPLAYS AN ERROR MESSAGE OF "CLIMATE KEEPER IS UNAVAILABLE DUE TO A SYSTEM FAULT."
