While driving my Tesla equipped with Autopilot/Traffic-Aware Cruise Control, I experienced an unexpected and abrupt braking event. A vehicle had entered the center turn lane and had already cleared my lane, posing no obstruction to my path. Despite this, my Tesla suddenly applied the brakes without warning, causing a rapid deceleration that could have led to a rear-end collision had another vehicle been following closely. There were no visible hazards, warning messages, or alerts prior to the braking. The incident occurred in clear daylight conditions on a dry road, and the vehicle in the center turn lane was stationary or turning away from my lane. This appears to be a case of phantom braking, possibly due to misclassification of the adjacent vehicle or overcautious object detection by the vehicle’s Advanced Driver Assistance Systems (ADAS). This issue has occurred more than once in similar scenarios, and I believe it poses a serious safety risk. I have not yet had the issue inspected by Tesla or a service center, but I am submitting this report to raise awareness and request investigation into the reliability of Tesla’s braking logic in these situations.

Autopilot (adaptive cruise control) will freak out and slam on the brakes when a motorcycle passes by splitting lanes. This is a serious safety issue as cars and motorcycles behind me may not be able to react in time and cause a rear end collision or into the wall or vehicles next to them. It is impossible to override the braking with the accelerator as it will be too late by the time the car accelerates. Flooring the accelerator is also not a safe option.

Description of Problem: While driving under normal highway conditions on July 2, 2025, my Tesla suddenly and violently phantom braked — meaning it rapidly and without cause applied the brakes forcefully. There were no obstacles, vehicles, or hazards present. The sudden deceleration caused a severe flare-up of a recent cervical disc replacement surgery I had undergone. I experienced extreme neck and spinal pain and had to seek emergency medical attention. I am now under medical supervision, on pain medication, and unable to work. I contacted Tesla requesting the logs from the incident and have received no response. I’m deeply concerned about the safety of this vehicle and the lack of accountability for this known and previously investigated issue. I am in the process of retaining legal counsel and intend to pursue this further as a product liability and injury case.

While driving with the Adaptive Cruise Control enabled, on California highway 5 on Aug 28, 2023, the vehicle applied the brakes very aggressively and suddenly, shy of making a complete stop, without any object or shadow requiring it to do so - known as the "Phantom Breaking". It was so dangerous that if it was a weekend with many cars behind me, I would've been violently rear-ended and perhaps not lived to report this incident. This happened twice in two hours, to the point that I have never engaged the cruise system again. The Adaptive Cruise Control (ACC) system (which is part of the Auto Pilot system) on this vehicle is dangerous and should be avoided. I wish the vehicle had the [old fashioned] non-adaptive cruise control but it doesn't. Answering the questions: Nothing failed but the ACC clearly malfunctioned since there was no object, shadow, road animal, change in asphalt color, etc. to cause it to abruptly break. It was a clear sunny day. Any accident at such highway speed could've been deadly to multiple drivers on the road. When I informed the dealer, they "verbally" acknowledged and informed me of the term "Phantom Breaking" that they had heard of the problem being reported by others but nothing to worry about and they didn't inspect the vehicle. There we no warning lamps, messages or symptoms prior to the problem. Again, this issue happened twice within a two hour period and I have not used the ACC system again. My spouse was with me in the car in both cases and witnessed the scary situation.

When traveling on an interstate highway with a speed limit of 65 mph, with navigation system operating and with TACC engaged, the system sometimes resets the maximum speed to that of the speed limit of the exit ramp (usually 40 mph) when I am NOT exiting the highway. This results in rapid deceleration significantly below the prevailing highway speed with the potential of causing a rear end collision from following traffic. This is consistent some certain exit ramps along I-93 in NH, and is prevalent when traveling in the right traffic lane.

The volume control buttons and vehicle setting buttons on my car’s steering wheel are made of metal. In high atmospheric temperatures the interior of the car heats up. The buttons also heat up and can cause burns if touching the button for an extended (more than 30 seconds) amount of time. This is very distracting while driving while also causing a mild burn.

The steering system appears to have malfunctioned, causing the vehicle to consistently pull to the right, even after multiple alignments with perfect specifications. This issue is ongoing, and the vehicle is available for inspection upon request. How was your safety or the safety of others put at risk? The pulling to the right affects the vehicle's handling and requires constant manual correction by holding the steering wheel tilted to the left. This compromises my ability to control the car safely, particularly during acceleration or regenerative braking when the issue becomes more pronounced. I am concerned that this could lead to an accident or further mechanical failure. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the issue has been demonstrated and reproduced during test drives with Tesla service technicians. Despite this, I was told the behavior is "normal" or attributed to road crown, which does not align with my driving experience. Multiple alignments have not resolved the issue. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle has been inspected multiple times by Tesla service centers. Technicians have confirmed the pulling during test drives, but no definitive resolution or explanation has been provided. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps or error messages have appeared. The problem began in September 2024 after I returned from a 4-month military deployment. During this time, the car was parked in my garage and not driven. The issue was first noticed immediately upon resuming use of the vehicle, which had approximately 12,000 miles on the odometer at the time.

While driving, the side mirrors closed and the doors locked by themselves. Then, the front trunk opened, the rear trunk opened, and when I opened the door, the headlights turned on and the front trunk opened again. When I opened the door another time, the side mirrors didn’t unfold, and the headlights started blinking. At high speeds (over 50 mph), I hear a ticking or metal-clanking sound, which is really scary. The car seems to be acting on its own, following some strange “manual” or behavior pattern. I feel unsafe driving it.

In a wreck the car does not have emergency door handles for the back doors if the power shuts off. This is a MAJOR safety issue.

I was horrified to read of the deaths of three beautiful California girls in a Tesla Cyber Truck, that was involved in a crash, was on fire, and the electrical system malfunctioned resulting in them not being able to get out of the car. Tragically, they burned to death.(Recall too [XXX] ' incident as well as other incidents). I went to search the Internet for Model 3 (2023) to see if there were manual overrides in the event of crash/fire/electrical malfunction (even if related to dead battery) and found a You tube video (NOT information provided by Tesla mind you) that showed that on the front seats, there should be a lever not far from the door handle that you can pull to activate a manual release of the door. In the video, they said 'models' are different as to the rear seats but there 'should be' a lever. I could not find any manual override for the back seat passengers. So, I drove to the Tesla Dealer/Service department to have them show me how to activate the manual override for back seat passengers. The technician admitted that in many Teslas, including mine, THERE WAS NO SUCH CAPABILITY!! He admitted Tesla was acutely aware, they do not prioritize safety and that the back seat passengers would be stuck. He agreed that it will take a major class actions and major fines, and/or jury verdicts for them to do anything about it. Appalling and not acceptable. I would request that NHTSA mandate that all Teslas (probably models before 2025) lacking the device necessary to allow passengers to escape death due to failed electrical system be recalled to modify the car at Tesla's expense to comply with the reasonable expectations of consumers. This is a major design defect, unreasonably dangerous and presents a very real threat to the consumers who have no knowledge of the defect, much less where to look for the override (if it exists) because that information is not provided when you purchase the car. Due to the manual locks being looked at as a safety issue, I went and looked at the REAR door manual release for our Model 3. We also have a 2023 Model Y and I know they have a rear door release, as stupid as the location is, it is there. Well to my surprise, there is NO MANUAL REAR DOOR RELEASE in the model 3!!! How is this even safe? You're supposed to crawl through the front seats and go out the front door? Or even worse, fold the seats down and exit through the trunk? What happens if you're stuck in the back, can't get your seatbelt off? What if you are incapacitated?? What if you get rear ended and then pushed into the car in front of you and it makes it so the front doors are unable to be opened? (this happens quite often) What if the car rolls over, is upside down and you can't get to the front? There are a million situations that make a rear manual door release necessary. How does this even pass safety inspection??? How do you (as a department) even allow this to be acceptable. How are children supposed to do this? This has to be one of the most ridiculous things I've ever heard of and here I am, driving my kids every day around in it. They should be required to either retrofit it with a release that's accessible or replace the rear doors with an accessible release. I'm just in awe as to the failures here.

My Tesla Model 3 disengaged cruise control while going 68 mph and braked hard for no apparent reason. I was driving northbound on I-405 at 3:45 a.m. going to LAX airport in the number 3 or 4 of six or seven lanes. Cruise control had been activated for at least 10 minutes before the incident at a speed of 68 mph. There was no traffic within half a mile before or after me due to the time of day. Neither my left or right foot were near either the brake or acceleration pedal, nor were they in motion. Both hands were on the steering wheel and no control was being activated. The only noticeable environmental condition is that the road surface had just changed from a dark gray asphalt to a bright white new concrete roadway. No emergency braking alert was seen or heard, so I do not think it was a false collision detection, and there was no car within a half mile. However, I do believe the severity of braking could have caused an accident if there had been someone behind me. Tesla Model 3 Software Version was v12 (2025.20.6 046c4575d120).

I was traveling at highway speeds on a highway with no vehicles or other obstacles in front of me. I had adaptive cruise control on. Then, the emergency brakes deployed for no reason (so called “phantom braking”), decelerating the car quickly. I turned off adaptive cruise control as fast as I could and nothing bad happened. But the situation was dangerous, as if a car was close behind me it could have rear ended my vehicle.

When using cruise control, my 2023 Tesla Model 3 will aggressively brake without warning or apparent cause. The braking is near maximal - as though the vehicle is attempting to prevent an imminent collision. This defect poses a hazard to occupants of vehicles behind mine as well as myself and my passengers as sudden braking could cause a rear-end collision and cascading loss of control. Inclement weather would have exacerbated the problem. This problem is documented. From my brief research it appears Phantom Braking has been the subject of a NHTSA investigation previously but for prior model years. The issue persists. When I questioned Tesla Service regarding this life and limb threatening problem their staff was aware and well versed at requesting the customer report the issue as though it were a minor software glitch. This is a significantly more serious problem necessitating an appropriate response.

The blinkers do not respond to the change in blinkers when I initiate the blinker (right or left) by using the stalk. There has been numerous times that I fully pull down or push up the blinker stall and it would not engage the blinker but today I am reporting it because now I almost changed the lane after engaging the blinker but it didn’t happen

The contact stated that while driving during rain at 60 MPH, the contact lost control of the vehicle as the vehicle spun in a circle at hit a highway wall barrier. The barrier wall was damaged as a result of the accident, and the contact was given a bill for $3,222.15. The air bags deployed upon impact. The contact did not seek medical attention but suffered from both back and head pains. A police report was filed. Due to the accident, the vehicle was towed to an independent mechanic, where the vehicle remained in their possession. The contact wrote a letter to the manufacturer about the failure, but had yet to receive a response. The vehicle was deemed a total loss as a result of the accident. The failure mileage was approximately 50,000.

I charged a 2021 Apple Macbook Pro with factory charging cable from the front USB-C charging port in a 2023 Tesla Model 3. The laptop charged up to around 70% but ended prematurely due to a melted charging cable and usb-c port (the connections from the usb-c port that the cable plugs into, and the part of the cable itself (male end) was melted and so hot that it burned my finger. I brought it to Tesla Service and they said it's not covered under warranty and are charging $377 + tax to replace it. The manual does not state any limitations as to what can/cannot be connected to the usb-c port. The contact owns a 2023 Tesla Model 3. The contact stated that while driving at an undisclosed speed, an alert chimed for several seconds before the infotainment screen went blank. The contact stated that the vehicle lost motive power, the A/C became inoperable, the windows failed to roll down, and the doors failed to open as needed. The contact attempted to reboot the vehicle, but the vehicle failed to respond immediately. The contact was very concerned about her and her grandson's safety while the vehicle was inoperable. The contact and her grandson were unable to exit the vehicle immediately in hot weather. Moments later, the vehicle inadvertently rebooted, and the contact was able to drive to the residence, where she and her grandson exited through the front driver's door. Upon checking the Tesla app, four error codes were displayed: VCFRONT_a592, VCFRONT_a191, DI_a174, and DI_a183. The dealer was made aware of the failure, and the vehicle was diagnosed through the Tesla App. The dealer determined that the high-voltage controller and relay needed to be replaced. The vehicle was not yet repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 2,725.

Brake Feature Malfunction Vehicle Details: •Model: Tesla Model 3 •Year: 2023 (approximately 1 year old) •Mileage at Incident: ~11,000 miles Incident Description: On 10/29, while manually driving my Tesla Model 3 on a public roadway, I experienced a critical braking failure. As I approached a vehicle in front of me that was decelerating, I initiated regenerative braking, then immediately applied the brake pedal forcefully. Despite pressing the brake pedal hard multiple times, the vehicle did not decelerate as expected and ultimately collided with the vehicle ahead. Throughout this incident, no collision warning, automatic braking, or other safety features activated, despite having all safety features enabled. This failure significantly impacted my confidence in the vehicle’s braking and safety systems. After the accident, I reviewed the Tesla incident report, which shows I attempted to brake but that acceleration was not reduced until the collision occurred. Despite the report documenting my attempts to brake, the system did not function properly, suggesting a potential mechanical or software defect in the braking and/or safety alert systems. Aftermath: •My vehicle sustained considerable damage. •I now feel unsafe driving the vehicle, particularly on highways, as I am concerned the brake failure could recur. Request for Investigation: Given the severity of this issue, I am seeking an investigation to determine: 1.Whether there is a defect in Tesla Model 3 braking systems that could result in brake failure under normal driving conditions. 2.If there is an issue with the vehicle’s safety features failing to activate appropriately. This incident has raised significant concerns regarding my safety and that of other drivers, especially given the car’s relatively new condition and low mileage. I would appreciate a prompt investigation into these safety concerns.

With the TACC system activated when traveling on a 2-lane road with a right-hand curve, if there is a semi-truck in the opposing lane, the emergency braking engages with a hard brake application. It can be overridden with accelerator pedal, but it endangers the car from collision with a following vehicle. There have been multiple incidents. I attempted to report the condition to Tesla customer support.

On [XXX], I was driving my Tesla model 3 from California to my new home in Idaho as part of my move there. During the day, I had 6 or 7 situations where the car would suddenly go into full regenerative braking, or outright apply the brakes for no apparent reason. Most of these events took place of freeways at freeways speeds, or on highways (not freeways) at the local speed limit. Every situation was when I had supervised self drive or traffic aware cruise control enabled. These situations were extremely startling and in my opinion potentially dangerous. I don't want to imagine what might have happened if one of these "phantom braking" events had occurred with a heavy 18 wheeler behind me in traffic. I was driving behind someone when a suv came over from left lane into the right lane in front of the cars that were in Front of me and made a sudden turn into a business. I was already applying the brake and was the 3 second rule behind the car in front of me, however the car did not stop as I slammed on the brakes as hard as I could. There have been other times that the car will automatically slammed brakes on because it feels that it need to…when it’s not necessary at all. But the other day when I needed the brakes to help me stop abruptly…they failed to do so. I hit the car in front of me and was cited as well. If I had been going faster..this could have been way worse. This definitely needs to be evaluated on Teslas. I have a dahcam video that I’m unable to upload.

As I was approaching my off ramp (which has a very tight turn), my automatic brakes failed which caused me to slam onto the brakes, and skid into one of the off ramp signs. All 3 of my children were in the back, the back driver side window shattered, I lost my driver side mirror, and there was damage done to the back door and wheel well (all on the driver's side). Although the damage looked minimal, no one was willing to work on the vehicle and I was strongly encouraged by my insurance company to claim it as a total loss. Granted, I had this vehicle for approximately 2 months, and had put at least $4,000 into it.

The vehicle’s brakes did not work, automatic emergency brakes and collision warning both did not work. I pressed on the brake for about 5 seconds but the brakes did not work, resulting in a collision with the car in front. Everything is available for inspection if needed. Both my safety and the driver of the vehicle in front of me was at risk. The problem has not been confirmed yet. The vehicle is currently at a shop, waiting inspection. There were no warning lamps, messages, or symptoms.

On [XXX], my 2023 Tesla Model 3 (VIN: [XXX] ) suddenly accelerated on its own and the brakes failed to respond despite repeated pressing. To avoid hitting other vehicles, I turned into a property driveway and struck a tree. The airbags deployed. This was a case of sudden unintended acceleration combined with complete brake failure, creating a serious safety hazard. Forward Collision Warning also did not prevent the impact. A police report was filed by the Dekalb County Police Department, GA, USA (Report #[XXX], Case #[XXX]), which specifically documented brake failure as the cause of the accident. The report also notes that the driver was injured by the airbag and that the vehicle had to be towed from the scene. A copy of the police report can be provided upon request. When cruise control is engaged, the vehicle suddenly decelerates, (at times dropping speed by more than 20 mph in less than 2 seconds). This occurs as often as 5-10 times in a single 60mi trip, and occurs at least once per trip on this same route (divided highway, sometimes with hills, often with no other cars ahead of or around the vehicle, speed 60-70mph) These events are unsafe because unexpected braking is a hazard for following cars and my passengers. My job as a driver is to be as predictable as possible, but when I use the cruise control the car behaves unpredictability Problem reported to dealer, confirmed by remote diagnostic, and car was serviced but the problem got worse. There are no warning lamps or error messages displayed in conjunction with these sudden braking events. First appeared when the car reached 2000miles on odometer

I was driving on I-35 from Kansas City to Des Moines on 9-11-23, and three times the car experienced phantom braking (i.e. the car suddenly braked for no apparent reason) while the adaptive cruise control was activated. During these instances the car would suddenly brake and slow down and on one occasion I had to accelerate immediately to avoid possibly being rear ended. On all occasions there were no obstacles on the road or cars within 1/4 of a mile ahead of me.

The contact was able to turn the switch for the windshield wipers and the wiper blades started operating as needed. There were no warning lights illuminated. The local dealer was contacted, and the contact was informed that the contact was unable to determine the cause of the failure. The contact stated that the failure began after an OTA update. The vehicle was not diagnosed or repaired. The manufacturer was contacted. The approximate failure mileage was 11,800.

While driving in rain, the windshield wipers will randomly stop. This will happen both in AUTO and in MANUAL modes, especially at night in poor visibility. I contacted Tesla and they came and replaced the wiper motor assembly but the problem was not resolved. I believe it is a software issue. I nearly had a crash as this happened to me first at night in a very busy and complex highway intersection and resulted in me being stuck in the middle and blocking traffic until I could navigate the touch screen and find the wiper option to get it restarted.

Automatic wipers are very erratic. Sometimes it works like crazy. Sometimes it never cleans the windshield even though it is soaking limiting visibility. Using it in manual mode is also very difficult as all adjustments are done on the screen forcing you to take your eyes off the road. There are no levers to adjust it.

Wiper control is limited requiring the driver focus on the touch screen. Auto wipers activation is slow and does not keep windshield clear from rain, forcing the driver take the eyes off the road and find the small area on the touchscreen to control the various wipers speed.

The contact lost control of the vehicle, coming to a complete stop after crashing into two poles and veering into a brick wall, where the vehicle briefly continued to accelerate. The air bags did not deploy. A Police report was filed. There were no injuries were reported. The vehicle was taken to the dealer, who was unable to determine the cause of the failure. The vehicle was not repaired. After investigating the failure, the contact related the failure to NHTSA Action Number: DP23002 (Speed Control). The manufacturer was notified of the failure, a case was opened, and the contact was referred to the NHTSA Hotline for assistance. The failure mileage was 100,000.

I have twice experienced a sudden acceleration when decreasing pressure on the accelerator pedal. It felt like the motor was producing full power. In both cases, I was able to quickly apply the brake which stopped the acceleration. Both episodes happened at slow speed, less than 20 mph, while either starting to slow for a stop or making a turn. Software on the car is fully up to date as of February 2024.

Ever since I purchased my 2023 Tesla model 3 I have periodically experienced a phenomenon widely referred to on the web as "phantom braking". Google it (though you probably already know about it). When it makes the biggest impression on me is when I am driving at over 50 MPH on an open highway and the car suddenly slows down, to the extent that it feels like someone is applying the brakes. Every time it happened in recent memory there were no cars in front of me for hundreds if not thousands of feet, which is why it's surprising because there has never been an apparent reason for the sudden deceleration. Instinctively, I look in the rear view mirror to see if anyone is behind me because if someone were relatively close behind me such a sudden slow down could, IMO, cause an accident - or at least alarm the other driver enough to slam on their brakes. Having read about phantom braking I believe that Tesla removed the traditional LIDAR (or equivalent) technology used by other car makers for adaptive speed control or a similar feature, and instead relies solely on cameras to make judgements. This decision, IMO, has led to a dangerous weakness in the reliability of Tesla's cruise control.

During a major car accident the airbags for my Tesla, Model 3, 2023 did not deploy airbags although other impacted cars had full airbags deployed. This posed a risk to my safety as a driver due to not having the proper equipment deployed during a major accident. This was noticed by the emergency personnel on the scene who stated that based on my impact the airbags should have been activated from my collision. According to the safety records associated with my car to date there are no safety reports regarding the airbags for my car. There was no warning of my airbags not being able to be fully functioning at the time of impact. This incident happened on November 1st and is in the process of being investigated by the Harris County sheriff's department.

It was inspected by an auto collision expert; his description, photos and estimate were forwarded to a claims specialist at the insurance company. The side of my head hit the window on the driver's side, but no airbags inflated. I feel this was a risk to my overall safety. There were no warning messages.

After a collision the screen flashed "Air bags deployed". The car then turned on the hazards, called 911 then vented the windows. The issue is the air bags did not deploy at all. I suffered a concussion and whiplash due to the accident. No warning lamps or messages ever appeared before while driving the vehicle.

On July 5, 2023 my front passenger seat assembly was replaced due to a passenger safety restraint system fault. Two days ago I discovered two black self tapping bolts sitting behind the front passenger seat on the floor mat. The front passenger seat assembly was only connected to the car chassis by the two front mounting bolts. It was possible to tip the seat forward by pushing on the headrest. I am now able to screw these two bolts almost completely back into the chassis using fingertip pressure only. Either these bolts were never installed during seat replacement or they simply rattled loose and fell out. This attachment hardware needs to be designed for repeated installation removal cycles over the life of the vehicle and should require considerable torque for installation and removal. Something is seriously wrong!

I just purchased this new 2023 Tesla Model 3. I am not reporting a failed or malfunctioning component but I have a safety concern. After taking delivery I noticed that there are no emergency door latch releases on the rear seats like there are on the front seats. This could make exiting the back seats very difficult or even impossible in the event of an emergency with loss of power to the vehicle. I cannot believe that this feature was omitted. I worry that people will die because they will not be able to escape a burning vehicle quickly enough. This seems like the door could be easily modified to correct this issue and make this vehicle much safer.

Brand new Tesla Model 3 delivered a week ago. Passenger seat, front inside mounting bolt is not tight. Corner of the seat rail can move up/down 1/8-1/4". Can't tell how well attached it is. Potential safety concern. Passenger side back seat cushion also exhibits up/down movement 1/8-1/4" but likely not a safety issue.

Regen braking sporadically not working when going down mountain or steep roads. Sometimes they work and sometimes they won’t. This is after extensive driving on warm days while battery is between 70-80%. Support could not help me and they don’t have any technicians available. I’ve sent them videos and time stamps and they are ignoring the issue and unable to help due to lack of knowledgable representatives This is a serious safety concern

Three times while driving at low speed and attempting to reduce speed—even while pressing the brake pedal—my 2023 Tesla Model 3 unexpectedly accelerated. During the first incident, the Dynamics screen did not allow me to change the Deceleration Mode. After rebooting the touchscreen, the options reappeared briefly, but later that same day they became unavailable again. I was turning into a parking garage when my car suddenly slammed on the brakes very hard to a sudden stop. Like there was an imagined hazard. I did not have the car on auto Drive as I do not trust the technology, but Tesla had just offered everyone full self driving automation that month. I did not elect to use that, but I feel that technology had something to do with this. Not only the incident scary. The stop was so hard. I was in pain after.

Curvature Assist feature agressively brakes while driving at highway speeds. This is supposed to slow down for curves, but frequently intervenes for straight, clearly marked lanes. Feature cannot be turned off. Drivers behind me have gotten agitated several times for the sudden, harsh braking, and I do not feel comfortable driving this car.

Tesla Motors has two self-driving features, a mode they call enhanced autopilot and Full Self-Driving. Enhanced Autopilot is used primarily on freeways, and allows the car to switch lanes without driver's assistance, while maintaining the speed of traffic. Full Self Driving is a separate system in and of itself that relies on far more sensors, and is far more complex, as it needs to be in order to handle all the nuances of allowing a car to drive itself on and around city streets. During the month of April ,Tesla previewed the supposed improvements in its full self driving mode by releasing it to everyone for free for that month. I was exiting the 101 freeway in Los Angeles, utilizing the enhanced autopilot feature to navigate myself to my destination. At this exit, the car makes a right on Main St, drives downabout 500 feet and then makes another right so that you're briefly traveling underneath the freeway from you'd just exited. As the car made that second right, while on what should have full self-driving mode, the car failed to navigate the angle of that turn and ran right into a steep curb at a fairly fast speed, destroying the right drive train, the right tire, the entire front bumper, and the cars wheel chassis and suspsnesion system all still have to be fixed. I believe the malfunction was caused by the car failing to properly switch vbetween enhancd autopilot and full self-driving if they utilize GPS to mmake that determination, because it felt almost as if the car was trying to switch into the lane to the right (which didnt exist there was the curb there) and not like it was attempting to negotiate or preparing to make a wider turn maneuver. I don't believe it even decelerated as it hit the curb. I broke my toe but otherwise am all right. Tesla won't honor their warranty and is threatening to repo the car they won't fix.

Autopilot now requires so much force to confirm you have your hands on the wheel it’s disengages causing rapid deceleration and steering into the other lane or shoulder. Also requires confirmation at times when the driver should be looking at mirrors when passing etc instead of watching the display.

I took deliver of the Model 3 Performance in late October 2022 and have experienced phantom braking at least a dozen times over the last few months on the highway/interstate when using cruise control and Tesla’s standard autopilot, which keeps the vehicle centered in the lane. The phantom braking has occurred both in times with just the cruise control enabled and also when both cruise and autopilot have been enabled. The vehicle has slowed from 70 to 60 and even 50 miles per hour very quickly when there was no object nearby that would cause a collision. This is both frightening and upsetting, as such sudden braking could cause an accident, especially with any car following me from behind. I am fearful to use the cruise or autopilot as a result of this defect.

I am writing to file a claim concerning a recall affecting my Tesla vehicle, identified by NHTSA Recall Number 24V376. The recall involves a defect in the seat belt reminder system software logic affecting certain Tesla models, including my own. The defect causes a failure to provide audible and visual seat belt reminders when the driver’s seat belt is not fastened after the ignition is turned to the “on” or “start” position. This issue does not comply with FMVSS No. 208, S7.3(a)(1). Safety Concerns: While this defect has not led to any injuries, it has caused considerable anxiety regarding the safety and compliance of my vehicle. The absence of seat belt reminders can increase the risk of driving without a fastened seat belt, potentially leading to serious injury in the event of a collision. Remedy and Actions Taken: I understand that Tesla is providing an over-the-air (OTA) software remedy, firmware 2024.20.3, to address this issue. The remedy involves updating the seat belt reminder logic to rely solely on the seat belt buckle and ignition status, independent of the driver seat occupancy switch. Impact and Request: The ongoing concern about the functionality of the seat belt reminder system has affected my confidence in the safety of my vehicle.

After Tesla upgraded their insurance driving behavior to raise rate when the driver side seat belt is not engaged; the car doesn’t warn the driver through visual or audio alert about driver side seat belt not engaged. The car clearly indicates if any of the passenger seat is not engaged; but not for driver side. I had driven for many miles until I realized seat belt was not engaged and car never alerted me. I have already created a service request with Tesla but I feel this is just not an isolated incident.

Trunk struts snap/come off. Causing trunk to slam shut. Damaging body. Tesla states vehicle is safe to operate, however vehicle trunk does not latch down anymore and opens every time I hit a bump. The trunk allignment is also well off. The vibration of the trunk causes several electrical issues including Automatic Emergency Braking. Original problem occured september 15th 2023. Issue was fixed and persists. Current issue has been going on since february 18th 2025. Issue is not yet resolved or fixed. Tesla is not taking any action to repair issue at all. Vehicle is unsafe to operate.

While driving down the highway at less than 60 mph the rear drivers side window spontaneously exploded. There was no one next to me at the time. Nothing hit the window. It exploded and most of the glass blew out of the car onto the highway. I have video of the incident but can't upload that.

The Tesla model 3 rear bumper came off completely while driving in rain when I drove into a very light puddle. Having a rear bumper detaching completely off could be a serious hazard for someone that driving a motorcycle on the road it could result in a deadly accident. this problem has been addressed to Tesla and they do not have a service bulletin or a recall for 2023 model only 2018 model through 2021 models. The vehicle has been inspected by Tesla. No warning signs where indicated when the rear bumper detached completely while driving

The car has issue with suspension, i know it is sporty suspension, but I drove so many sport cars. But this is the only one that has jarring issue, my whole upper half is shaking back an forward on bumpy wavy roads causing me to have lower back pain and if i put my back head on head rest on these road my back head will be bouncing on it non stop. I went to service center and they say the car drive normally ,I told them mode 3 2024 does. Ot have that issue they say it is completely redesigned suspension. I told them from the sporty cars I drove non have that issue. They say model 3 is different than other models. It is so frustrating buying 2023 and having this issue and tesla is not will ing to fix it and have to suffer back pain .

The front left suspension arm failed and fell out as I was parking my car. If this would have happened at highway speeds it would have caused an accident because the car would have run into incoming traffic, potentially resulting in death. This occurred after a collision repair. The collision center that did the initial repair also fixed the issue. The car has had issues with pulling to the right since this repair. It has been taken to them several times but they are unable to address my concern.

I leased a Tesla 3 Performance Package on Oct 24, 2023. It was delivered to Connecticut, with the cold weather package, and unbeknownst to me, summer tires. Due to the summer tires and driving with them in temperatures unacceptable for their compound, I damaged the passenger rear sidewall. But again, I did not know this was the reason. I brought it to the Milford, CT service center on [XXX] where they replaced the tire. Yet, replacing the tire was unsafe because it was, of course, also a summer tire. Nobody told me what was going on at any point in the process. A week after, I damaged 2 additional tires (photos attached) and at that point, I investigated and was frustrated to find that they were summer tires. I purchased 4 Continental All Season tires and had them installed on my vehicle. While the tires were replaced, the technician showed me that the 3 rims involved with the 3 tire malfunctions were slightly bent as the tires afforded inadequate protection. Nowhere on the purchase order or in the purchase agreement did it state summer tires were equipped, I had no idea to even check, and I'm guessing there are a LOT of people driving unsafely on inappropriate tires because of the lack of disclosure. This seems like a big hazard and that's why I'm reporting this. I believe Tesla should be made to proactively convey this information and should be responsible for repairs and damages that occurred as a result. On 3/11/23, around noon time, I drove on I-280 N bound, just past Edgewood Road. It was nice weather, no wind, nothing extra loaded in the car, smooth clean road surface. All of a sudden I saw black pieces flying at my left side of the car through the window, I heard some noise and the car started unbalanced. I pulled the car slowly to the shoulder ... It was a left rear tire blowout! This car was only for 4 months, and drove ~2700 miles. NO warning sign beforehand and no damage on the tire, the rim and the tire cover after carefully examining the Tesla Test Center at 680 El. Camino Real, Santa Clara, CA. Attached are four photos taken on 3/11/23. New tire was replaced on 3/14/23 at SC site, CA. It was charged $441.67 (receipt is not included here)

The system failed to detect the driver having a seizure and had no way for the passenger to break the vehicle. People in the vehicle and Others were put at the risk of a seizing person accelerating in a car that could not be prevented by the car or by the passenger. No dealer/service center has confirmed the issues. The vehicle is currently being inspected by police. No known warning signs/messages before the incident.

On [XXX] around [XXX], my daughter drove her Tesla Model 3 on [XXX] from school. Two strange/scary accidents happened to two different Tesla cars (Model 3 and Y). It was raining with low/medium visibility. One of the cars was driven by my daughter, and luckily, both drivers were safe with very minor injuries. The cars had significant damage but were drivable, and there were no airbags out. These two accidents occurred within 3 to 5 minutes at the “SAME LOCATION.” When I asked them what happened, my daughter and other Model Y drivers described a sudden loss of control, with their cars spinning and becoming unresponsive to steering and braking. They then hit the guardrail on the center line. I checked the video recordings of our car multiple times; no vehicle-to-vehicle collision was involved. I also checked the road to see if there was any oil spillage, but the lane was clean. She didn't use Auto Pilot, FSD, and Cruse Control. The office recorded it as Hydro Plane in his report, but my concern was incidents that happened only to Teslas. Also, when I posted this incident on a Tesla WhatsApp group, multiple Tesla owners reported that they had a similar issue when it was raining. It seems like Sensors malfunctioning in the rain. Also, my daughter was driving below the posted speed (68/70), and she gave a written note to the officer too, but the officer recorded it as 70/70. Recent recall update makes auto pilot more dangerous and distracting. I am getting alerts while my hands are on the steering wheel and paying attention to the road, which then causes me to take my eyes off the road to see the alert. Which I then get kicked out of autopilot. Was using autopilot and got an alert the max speed was reduced due to road conditions and when I looked at the alert got another alert to pay attention to the road, then was immediately kicked out of autopilot. This is extremely dangerous especially if road conditions are deteriorating and to have to constantly be looking at all the new and frequent alert. The recent recall has made me constantly have to take my eyes off the road. I can even check my side view mirrors without getting a pay attention alert.

I bought a new Tesla Model 3 in December of 2022. In mid March 2023 we had our first road trip in this vehicle. I had at least 4 major phantom braking incidences while traveling an average of 75 mph on I-80 with the cruise control on. Two were experienced while traveling eastbound in Nebraska and two were while traveling westbound a week later in Wyoming. Each experiece was separated in time by at least two hours. The last one scared me and my wife so much that she won't ride in the Tesla on the freeway until it is fixed. We also had several minor phantom braking experiences which did not alarm us. What happened is that the car slammed on the brakes while going 75 and reduced the speed to about 55 in seconds. It was over before I could react. The tires did not skid and fortunately no one was following us. I fear that if someone was tailgating me that they would have rear ended our car. I had experienced a couple of phantom braking events in Utah in previous months but they weren't severe enough for me to report it.

Vehicle started displaying warning messages as I pulled out of garage - "steering assist reduced" and "lane departure avoidance features unavailable." Afterwards, I noticed vibration and clicking sensation while turning steering wheel and driving at low speeds. I performed a software reset but messages were still there. Then I performed controls, safety, power off procedure. Messages disappeared, but steering still felt jerky. This happened when the vehicle had approximately 2,300 miles on it. A few days later, when exiting the garage, steering wheel was initially locked. It then "popped" when I applied gentle pressure to turn the wheel and the warning messages returned. Steering felt stuttery afterwards. I scheduled service appointment with Tesla. They replaced steering rack. I have had no problems since. Currently at 3,700 miles.

On August 7th, 2025 at around 8 AM in the morning, my vehicles mobile phone car key was in-operable and I was unable to get in the car which led me to trouble shoot how to fix this issue with the car. Upon researching I was directed by Tesla to directly complete a soft reset of the vehicle holding down both buttons on the vehicle once I was inside, and pushing on the brake pedal. Upon completion, my vehicle booted up and I realized that all camera systems were down, my navigation systems were down, my safety features were down, my auto pilot, lane keeping assist and auto brake were down, sensors were down and I was no longer able to pre heat my battery to charge my vehicle due to issues with the navigation. I then took the vehicle to Tesla Service Center Irvine where a service advisor quoted me 2900 for a new ECU and Bolts and told me that I needed to replace my whole unit and that I was to cover the cost of the repair even though my vehicle is only 2 years of age and this failure is due to the quality control and software of the vehicle. I have been driving around a vehicle with absolutely no safety features whatsoever such as no cameras, sensors, auto braking, or full self driving due to this ECU failure and I fear for my safety and well being while driving this car.

During a road trip, autopilot started pulling to the left and right because the autopilot was struggling to center the car in the lane. Driving without autopilot showed that the car was pulling to one side. A wheel alignment by Firestone showed alignment specs consistent with a strike, however the car was not actually struck during the trip. The rims and wheel look intact and brand new, without cracks or bubbles. A visit to the Tesla service center showed that the right hand rear control arm has an enlarged hole, indicating abnormal wear and tear on the suspension elements for a 4-month car with 7,000 miles. The car's steering wheel still feels loose even though the car drives straight.

To reiterate, in this issue the suspension components failed even though the rim and wheel were pristine, and the sidewall did NOT have bubbles even a week after steering issues were observed. Yes, the vehicle and its components are available for inspection upon request. -The sudden brake failure and steering wheel lock resulted in a complete loss of control over the vehicle. The inability to stop or steer the car properly led to it veering over the curb/grass area, posing a severe risk of collision. Additionally, the failure of the airbags to deploy during the impact further compromised my safety. This incident not only endangered my life but also posed a significant threat to the safety of other road users on Highway I676. -At this time, the problem has not been reproduced or confirmed by a dealer or independent service center. Further investigation and inspection are necessary to determine the root cause of these critical system failures. -There were no warning lamps, messages, or other symptoms indicating any issues with the braking system, steering system, or airbags prior to the sudden failure. The malfunction occurred without any prior warning, which exacerbates the concern regarding the vehicle’s safety systems.

Within one day of taking ownership of a new 2023 tesla model 3, the car suddenly stopped running, slowing rapidly to a crawl on the highway, causing a potential hazardous situation. Error code DI_a137 appeared onscreen, and a message that the vehicle was not drivable. After several minutes on the shoulder, the car behaved normally. I cancelled my trip and attempted to return home. The fault recurred, forcing me to pull off the road again. After several minutes the car allowed me to return home. There was never any prior warning. Tesla roadservice picked up the car and today notified me repairs have been made. Tesla Lynwood WA confirmed the intermittent high voltage inverter problem. The rear motor inverter was replaced. I have heard of many such failures in the U.S. and a recall in China for this engine failure problem.

While driving to work on a local road, I approached a red light where there were no vehicles immediately ahead. However, my vehicle unexpectedly steered off the road in front of a construction sign, entering a ditch. Despite my attempts to regain control of the steering and braking systems, I was unsuccessful. The vehicle subsequently stopped automatically on an incline. Fortunately, I avoided a collision with a gas station and intersecting roadway, but sustained damage to my car’s front bumper. Law enforcement and a towing service were contacted to remove the vehicle. I tried calling the dealership almost half day without an answer, then I went to the dealership where they told me that the hardware is fine but they can’t do anything about the possible software issues which leaves me in a situation where I’m nervous to drive the car. I reached out to the company about the accident and didn’t hear back yet from them. I request a thorough investigation into this incident to determine the cause of the unexpected, highly dangerous, autopilot behavior. I am disturbed mentally and emotionally due to the accident. I also require confirmation of the vehicle's roadworthiness and assurance that it is safe to operate before resuming driving.

On 09/11/2023 at approx. 6AM CST, my wife went to a nearby Walmart store for groceries. As she was parking the car in the parking space, the car started to accelerate by itself and went around the parking lot in random directions. She couldn't control the steering and brake despite the continuous tries. It was such a traumatic experience, and the car went on to hit couple of other parked cars and the cart return stations in the parking lot. After few seconds of random self-drive in multiple directions of parking lot, it finally came to a halt as it got on a curb that flattened the tires. By God's grace, the car stopped right before entering the main busy road due to flat tires and also no pedestrians at that time. Otherwise, the situation could have been worse as the car was going in an uncontrollable manner. The incident was reported to 911 and they got the car towed to a towing garage. We got the incident report from the police department. They also have the CCTV footage of this incident. We also have the dashcam videos recorded by TESLA and pictures taken after the incident. I will provide any supporting documentation you might need to understand this incident and address the issue. We are trying to come out of the trauma from this incident and would like to bring this issue to the attention of all concerned parties for next steps.

Incident Summary: On [XXX], I was in an accident and dropped my Tesla Model 3 at Tesla’s North Phoenix Collision Center in Nov 2024. I received the car back in Dec/Jan. Immediately, I noticed dangerous swaying, bouncing, and veering when hitting dips—especially on the freeway. The car was unstable and unsafe to drive at high speeds. In Feb/Mar, Tanner (assistant manager at the collision center) drove the vehicle with me in the passenger seat. After hitting a dip on the freeway, he panicked, exited, and agreed something was wrong. I was later told they did a wheel alignment, but the issue remained. This was the second time they returned the vehicle in an unsafe condition. I nearly crashed due to how unstable it felt. In May 2025, I brought the car back again. Tanner suspected the steering column but said no diagnosis was done. He gave me a loaner vehicle, which had a flat tire. I had to drive my own unsafe car for nearly a month while waiting for another loaner and parts—no resolution during that time. On 6/13/2025, I contacted the Tesla Glendale Service Center. The advisor expressed frustration, saying this collision center often offloads unresolved issues. I also contacted Tesla Support (877-798-3752), who advised me to file this NHTSA complaint. As of June 2025, the issue is still unresolved. My car continues to sway and veer dangerously on the freeway, and Tesla North Phoenix Collision has failed to properly diagnose or fix it despite multiple returns. Documenting a sudden unintended acceleration experience. Today while driving and trying to stop for a red light, I lightly applied pressure on my brake pedal to brake for the red light. Midway through the braking, my car lunged forward for about half a second. Went through the dash cam footage, and I noticed that the footage skipped half a second as well. Seems like something electrical that caused the car to lunge. This is a huge safety issue.

After backing out of a parking place I could not switch into "drive". The car froze and had to be put into "tow mode" to be pushed aside and later taken to a dealer. Before being collected the car was partially blocking a car park, but there was no other safety risk. Warnings appeared when the event occurred, but not prior to. The rear drive unit was replaced under warranty.

Tesla autopilot drifted over two lanes and into the side of a semi-truck on the freeway. It was night time but with plenty of light, no rain, and clearly marked lines separating the lanes. Not only did autopilot fail, but the features to help avoid drifting out of a lane even when not using autopilot, also failed. Those features are supposed to auto-steer back into the lane and beep to alert the driver. None of this occurred. 1. Yes vehicle is available for inspection upon request 2. Could have easily led to extensive damage, injuries or fatalities. Everyone got lucky 3. Problem has not been reproduced by dealer, but I have video of the event 4. On several occasions, the last being July 19,2024, auto correct on steering took control of my vehicle when no correction was needed. I’ve been taken by surprise and struggled on all occasions to gain full control of the vehicle. In addition, my car is not able to receive the over-air update to address the hood latch recall which leaves me more vulnerable to a catastrophic event. I can not reach a person in Service as Tesla pushes every communication through the app. My appointment was pushed out over three weeks and I still have not heard from Tesla to confirm this appointment nor reach out with any concern whatsoever regarding this horrific safety concern.

So thanks to YOUR agencies interference and forcing Tesla to levy this bogus "recall fix" on us I almost got rear-ended...in the car I work so hard to pay for, the car that I love because it keeps me and my family safe because guess what we are humans and not robots. Just as YOU do and everyone in your agency we ALL have to look at our phones briefly while driving because we live lives where we have to make sure those we love are ok or make sure our phone is charged for when we reach our destination. You are going to do exactly the opposite of your agencies supposed mission of keeping us safe. You will kill people with this stupid "recall". Design defect that caused the cooling system to fail and potentially not cooling the battery system. Per Tesla service center, leaves and debris which are common in everyday use got on top of the radiator, the fan on the back of the radiator sucked the debris and caused the damage to the radiator to leak. the radiator’s design may be inadequate, leading to the issue and caused potential damage to the battery pack or computer as a result of overheating which might leads to fatal accidents.

I just exited freeway and pulled my foot off the pedal and it started to decelerate then there was a chime sound and the car suddenly shot forward. I slammed the brakes as quick as i could. Luckily no one was around. The driving conditions were terrible, it was raining hard and cold. I also just cancelled autopilot by signalling right before exiting. On December 24th 2023, I was reversing into a parking spot with a protruding wall that was only 4 feet high, but the Tesla Model 3 reverse camera was unable to detect the wall. From the angle of the mirrors, it was difficult for me to tell there was a protruding wall, so I was relying on the reverse camera to warn me about any obstacles. However, there was no warning sound as I backed up the car, even as it collided with the protruding wall and shattered the rear roof glass. This ended up costing me almost $700 in repairs out-of-pocket to replace the rear roof glass.

Summary: FSD malfunctioned, failed to disengage, causing loss of steering control, thus causing car to steer into wrong lane and resulting in an accident. Tesla Service Center refused to inspect unless car is fixed. Car was sent to Crash Champions ( autobody shop) and was quoted $35k to fix. Car was deemed as totaled by Tesla insurance. Weather condition: Clear night, well lit. Low traffic. What happened:
 My car has been driving in FSD on Lawrence Expressway and it was doing everything correctly. Approaching that upcoming tight bend, the FSD slowed down the car, and I took over by steering the steering wheel to the right to deactivate FSD and taking the tight turn, right before the white rows of triangles on the street. At this moment, the steering wheel forcibly turned to the opposite direction (i.e. left). I tried to correct the steering wheel by turning right even more, but the steering wheel wasn’t responding to my input for a split second. At this moment, my car is now on the wrong lane of Wildwood Ave and my car finally responded to my steering input. I had to swerve in the opposite direction to avoid the incoming traffic and ended up brushing against an SUV, followed by an impact on the curb, before finally slowing down. Seeing that there was incoming traffic from the back, I drove down to a residential street to park my car for safety reason. The SUV driver called the police to handle the situation, we had our information and testimonies recorded down. I have already followed the safe practices of driving with FSD according to the manual, but in this situation, I am not certain which technology is responsible for causing my car to override my manual steering input. On [XXX], around 1:00, I was driving normally on the highway. I remember using autopilot, and the speed limit was 55 mph, but I'm not very sure. At that time, it was raining heavily, and I was following the car in front, not doing anything else. Suddenly, I noticed that the Tesla car turned sharply to the left by itself! I couldn't control it. Let me repeat: the Tesla car suddenly turned left by itself. I instinctively grabbed the steering wheel, turned in the opposite direction to correct it, and pressed the brakes, but it was too late. I collided with a car in the adjacent lane, and the car later turned about 180 degrees and stopped! The left side airbags in the front and back all deployed! However, I were not injured. I got out of the car, immediately called the police, and contacted the owner of the collided vehicle. Later, the police arrived to document the incident. I believe it was a malfunction of the Tesla that caused the accident! I did not make a left turn manually! It was in normal driving mode. What component or system failed or malfunctioned, and is it available for inspection upon request? I have no idea, should check Tesla's driving records and the recorded video from the incident. Has the problem been reproduced or confirmed by a dealer or independent service center? I decided to take my first road trip in the new Telsa Model 3. The trip was from Seattle to Portland down [XXX] which is a pretty straight and easy road. Because of this I decided to use the Autopilot feature (I purchased the FSD package) to help take the stress out of driving. (Note, this is my first Electric Vehicle AND even my first vehicle with a screen, so I stayed very aware and present the whole time driving with hands on wheel). After a while of driving I noticed the 'Arrow' icon on the maps, which indicates my car, wasn't centered on the road and was in fact about quarter to half an inch to the right of the road. I didn't think much of it as I've seen my Google maps do this before and after a little bit it readjust and fixes it. As the car was driving, it performed a SUDDEN deceleration and then put on the blinkers to turn left (which I'm on the interstate so there was no left turn). What I noticed was that because the icon on the map wasn't centered on the road, it thought I was off the side of it. When it came up to a side street, it then thought I was on the side street. Thus it rapidly slowed me down to the side street speed limit (35 mph vs the 70 on the interstate), and then thought it needed to turn left to get on to an on-ramp to get me back on the interstate highway. Luckily I was able to catch it in time, but I had to fight with the blinkers for almost a full minute to get them to turn off. After this happened again several more miles down the road, I decided to manually drive the rest of the way. Once I got to a charging station and powered the car off and on, the icon then was centered on the road again. I have noticed a few times since, that it 'gets confused' on what road I'm on and it is concerning that this has a lot of control over the car. It would be great to have a manual way to tell the map that I am currently on which road so it can then note that. While traveling on a busy interstate highway, my Tesla 3 presented four errors on the control screen: Automatic Emergency Braking disabled, Power reduced, Vehicle shutting down and Unable to drive. The left hand side of the screen flashed red and instructed me to pull over safely. The car was immediately slowing down, and as I was in heavy traffic, I was unable to pull over to the shoulder safely. I turned on my hazard lights and performed a soft reset of the car - which caused my hazard lights to turn off, all while cars were honking and whizzing around me. After the car reset, I was able to drive a short distance, with enough speed, to cross several lanes of traffic and come to a stop on the shoulder emergency lane of an exit ramp. The car is only 6 months old, with 4000 miles, but had to be towed to the Tesla service center. The problem has been diagnosed as a problem with the rear drive unit inverter and will be replaced under warranty. While I appreciate the resolution and that it will be fixed, it could have been a very serious situation and caused an accident. Thankfully, I was able to get to safety and wait to be towed.

I am writing to report several safety issues that I have experienced with my Tesla Model 3. These issues pose a significant risk to my safety and the safety of other road users. I kindly request that you investigate these matters thoroughly and take appropriate action to address them. 1. Steering Wheel Failure: On three separate occasions, the steering wheel of my Tesla Model 3 suddenly stopped working. Two of these incidents occurred while driving on the highway, which made it extremely difficult for me to maintain control of the vehicle. This sudden loss of steering functionality is a serious safety concern and could potentially lead to accidents and injuries. 2. False Forward Collision Warnings: The car's forward collision warning system has been providing false warnings, which has had a negative impact on my insurance premiums. Despite the fact that my insurance company is aware of the issue and acknowledges that it is a system failure, they have been automatically increasing my insurance rates on the leased vehicle. This has resulted in a significant financial burden, with my insurance premium increasing from $120 to almost $400 per month. Furthermore, I have been informed that next month, my premium will be raised to $600. I would like to emphasize that I have a clean driving record with no previous accidents or traffic violations. 3. Slow Charging Speed: Another issue I have encountered is the extremely slow charging speed of the vehicle. Despite the fact that the Tesla Model 3 is rated at 160 kW/h, it consistently charges at a maximum rate of 50 kW/h, regardless of the charging location or time. This significantly limits the usability. Took the vehicle to the Tesla service center twice. However, they said that they have been unable to replicate the steering wheel failure which would be crazy if they could replicate it every time the car is driven. and they have dismissed the slow charging speed as a normal variation and it's not. Disregarding safety!

As part of transitioning from radar to a camera based system (this affects model years '22 onward), Tesla, has "temporarily" disabled key features such as lane departure, blind spot alerts and emergency braking. Though disabled, these features are still selectable (not greyed out) when the user chooses safety options. These features, which are available on most modern cars, have been disabled for many months with no committed date to restore functionality ... this poses a major safety risk. This should impact Tesla's NHTSA safety rating which will pressure the company to re-enable these basic safety features ASAP. Thank you.

Tesla has serious safety issues automatic emergency, braking Blindspot warning, Lane, departure warning, and other basic safety features which are very common in the cars these days. I purchased Tesla for my teenage child. In past three months we have encountered multiple instances where Tesla was not able to perform as promised this morning I was driving tesla loaner Vehicle when I was heading to pick up my car and someone drifted into my lane without warning and Tesla did not apply emergency brakes or generated any warning signals. I have notified Tesla service about the incident and past incidents to tesla service representative. Tesla representative at Costa Mesa office opened the service request to get the software diagnosis completed so that they can get to the bottom of it, but [XXX] from Irvine office called me and he is in denial that it is a car issue and blame it on me that it is between the car and the car owners manual, I have requested an escalation from Tesla service Center. In the meantime, I am filing this complaint so that NHTSA can start their investigation before. It is too late and some life is lost because of irresponsible behavior by Tesla and send in selling the features which do not work I have Lexus RX 350, 2017 model and all these features work seamlessly in the car , I am shocked Tesla is falsifying and and selling inaccurate features to the consumers which cannot be discovered until it is too late. Car accelerated forward unexpectedly while making a turn and hit a telephone pole. Brakes did not engage nor did the forward collision warning sound or engage. Was manually driving. Car is non-driveable and repairs from Tesla are backlogged so we waiting 8+ weeks to get our car back.

Compliance arm bolt securing it to the subframe is fractured. Tesla has dismissed my ongoing concerns of vibration and instability while driving. Stating they performed an in depth inspection of all suspension and subframe components and found nothing of concern during last service appointment. I did my own inspection and found the fractured bolt. Sent photo to service and they are trying to play it off as normal and safe to drive. Contacted several manufacturers and mechanics stating otherwise. Told this will end in catastrophic failure and loss of vehicle control but apparently Tesla wants me to drive it to the service department for next service

The contact stated that while driving at an undisclosed speed on the third floor of a parking garage, the vehicle suddenly accelerated to full speed unintendedly, and the steering wheel and the brake pedal were inoperable, causing the contact to lose control of the vehicle. The vehicle crashed into a wall and crashed into the vehicle in front, and the vehicle stopped. No further information was available. No warning lights were illuminated. The front driver's side air bag deployed. The vehicle was towed to a tow lot and was deemed a total loss by the insurance company. A police report was filed. There was no injury sustained. The manufacturer was not notified of the failure. The failure mileage was 66,000.

During driving, Tesla car spontaneously stated multiple active safety features and regenerative braking were no longer available and stopped working during driving with multiple warnings appearing. Driver safety at risk during this as well as others safety due to the lack of functioning. The car was taken to Tesla service center and was explained that front lower lateral suspension links were damaged by "impact" although the car was never in an impact or accident preventing proper alignment and functioning of the car. The part has been since repaired and currently undergoing further investigation for warranty dispute.

The contact stated that the failure had recurred several times upon shifting to drive(D), which required the contact to exit the vehicle and manually close the trunk. The vehicle was taken to a Tesla service center and to Tesla Service Sunnyvale(680 E El Camino Real, Sunnyvale, CA 94087), where unknown repairs were performed several times; however, the failure persisted. Additionally, the contact stated that while attempting to reverse out of the garage, the contact was unaware that the garage door was closed, and the vehicle hit the garage door. The contact did not recall whether the rearview camera activated; however, the contact stated that the rearview camera pedestrian warning alert failed to engage prior to the incident. The contact stated that the garage door and the rear end of the vehicle were damaged. The manufacturer was contacted through the Tesla mobile app and advised the contact to take the vehicle to a Tesla service center for service. The vehicle was not diagnosed or repaired. Vehicle Information: 2023 Tesla Model 3 VIN: [XXX] Incident Date: [XXX] Location: [XXX] Summary of the Problem: On [XXX], while Autopilot was active, my 2023 Tesla Model 3 suddenly accelerated without driver input. I was pressing the brake pedal, but the vehicle surged forward uncontrollably, struck a fence and a tree, and caused major property damage. Both I and my wife (passenger) required urgent care treatment after the crash. Details / Safety Defect Evidence: •Tesla’s own raw logs show the accelerator spiked to 100% immediately before impact. •Brake pressure was recorded prior to impact, proving the brake pedal was applied. •Despite this, Tesla’s official Vehicle Data Report falsely states “manual brake not applied.” •Autopilot did not disengage when the brake was pressed, contrary to Tesla’s design claims. •The vehicle logged a “Near-Deploy Collision” at ~27 mph, yet the airbags did not deploy. Why This Is a Safety Defect: This incident reflects multiple critical failures: 1.Sudden unintended acceleration. 2.Failure of brake-override safety design. 3.Airbag non-deployment in a crash severe enough to trigger “Near-Deploy.” 4.Inaccurate Tesla reporting that conceals braking input. On June 4th I was in a hurry to get to work, pushed open my car door and started to slide in at which point the door unexpectedly to me swung back shut, clamping my head behind my ear pressing it against the top of the door frame. Everything stuck for about 1 second and then the door released my head. I am concerned that the door shouldn’t close so forcefully and should not bounce back closed when pushed open; that perhaps this contributed to the dangerous situation? There were other factors such as my car parking pad is slightly sloped. It seems this is an issue with the design of the car as opposed to a defect as there are no warning lights or recalls. I am still trying to figure out the effects of this injury. I have been to two doctors- not immediately because at first there were no immediate effects except for the discomfort of the pressure at the time. But less than 24 hours following the incident I developed a puffed up patch behind the ear where the door clamped, and now have developed a very swollen painless lymph node behind my other ear as well as headaches and some soreness that comes and goes. I have not contacted Tesla about the problem yet but am extremely cautious when I use the door and I don’t drive people in the car. I have included some photos but also just took some videos illustrating how the door swings back shut pretty forcefully when pushed open.

My pant leg got caught on the plastic seat trim under the seat. Kept my leg trapped so I fell out of the car on the ground in the parking lot. Happened again the same day, but I caught myself before I fell. The incident caused a hole and rip in my clothing, but the rest of my body went to the ground leaving my knee and other leg caught until the end. Had a different outfit on today and it caught again! I did not fall this time either. The time I did fall I hyperextended my wrist and landed on my hip and leg, creating bruising and a sore pelvis, knee and back. Went to massage therapist soon after. Currently, following up with doctor

My dad was driving to work in the morning in the Tesla in the left lane of the freeway. Then the car started swerving on its own. He tried to regain control with the steering wheel but he couldn’t control it. The car wouldn’t turn when using the steering wheel and he was not using autopilot or lane assistance. He said it felt like the steering wheel and the car were not talking to each other. And the brakes weren’t stopping the car at all and because he couldn’t control the car it turned left into the divider and hit the divider and the car turn again to the right and automatically stopped in the middle of the second lane. The airbags deployed for the driver side. Thankfully there weren’t any other cars on the road at the time so there aren’t any injuries. But this crash could’ve caused serious injuries to him or other people. There were no warning signs, alerts, or noises before this happened. He has filed a claim with the insurance company. And a crash report was filed by the police.

Any time I get into the car and immediately start driving before the car has time to catch up with what I am doing, the NHTSA mandated safety sounds played when the car is in reverse or drive does not play until the cars computer can catch up and begin playing the sounds. Most times this can take 3 seconds or so but today I had an incident where it took over 30 seconds before it would start playing. The same issue can be detected when restarting the infotainment screen by holding down both scroll wheels on the steering wheel. When doing this the car will cease playing the sound for around 5 seconds while the computer reboots.

Software updates caused failure notifications (seat restraint system) and disabled cameras/emergency brakes, happened immediately right after software updates. All hardware performed and properly functioned before the software updates. 1. First failure noticed on the begin of March 2024, component affected : " Rear center safety restraint system fault". Second failure noticed on [XXX] component affected: " Autopilot cameras unavailable", "Automatic emergency braking is unavailable" 2. I feel extremely unsafe now when using the vehicle with failure notification of the car's components, especially while driving my children to school. I am unsure if the system failure that could causes accidents and put my kids at risk or danger. 3. First issue was notified Tesla representative immediately right after the software update (happened over night). They scheduled an appointment, but I saw a charge bill that I have to approve before taking the car in for an inspection. It happened when my car is just out of warranty. I called Tesla dealership in Buena Park, CA to discuss about my safety concerns and got told it might be just a software glitch and I should be ok to drive. (??). Second issue was also notified Tesla and got a schedule for an inspection/service on December 10th, 2024, and again with an approval of $650 that I might got charged for ahead of the service ???? (Why do I get charged for something that wasn't my false until Tesla pushed a bad software to make us pay for their bad scripts?) 4. There was no failure notification "at all" before the last 2 or 3 software updates. Tesla offered some kind of 1 month free FSD then software updates to remove the FSD then everything started to fall apart. I am very concerned with Tesla software updates now and failures it might cause that put our family at risk because of their bad scripts or batches. If a Tesla software update fails it causes an APB version mismatch causing all safety features to become disabled until you schedule service for repairs. It is not covered outside of warranty even though it’s an issue caused by their update. If you google APB Version Mismatch, a lot of people have experienced it. Out of warranty repair is $2-3k for a computer replacement even though it was working fine before the update.

Component or System Failed: The critical safety issue involves Tesla's door locking system, specifically the lack of a readily accessible and obvious manual door release in the rear seat. This design flaw could prevent any child or passenger from knowing how to open the door or escape in an emergency. Safety Risk: The absence of a clearly identifiable manual release mechanism for the rear doors creates a severe safety risk. In an emergency, such as a power failure or accident, a child or any rear-seat passenger may be trapped without an obvious way to exit, increasing the potential for injury or death. This is particularly concerning given two prior instances where the battery failed to charge, leaving the vehicle completely without power even while connected to a charger. Reproduction and Confirmation: Tesla refuses to address the issue, This is a critical fire hazard. Purchased 2023 Tesla Model 3 Performance in January 2024. On [XXX] at approximately 2 pm, I got a flat tire drivers side front on a remote highway outside of Reno Nv. At that time, I found out that I didn't have a spare tire or jack supplied by Tesla. I called Tesla Roadside Service and was told before they would send out assistance, I had to agree to purchase a new tire at 540.00 per tire, the towing company couldn't be to my location till after 6pm. I declined Tesla service. The Tesla had less than 18000 miles on it. My son came and assisted me in removing the flat and we took it to Les Schwab Tire center where I purchased a new tire for 375.00 dollars. Les Schwab stated that the wear on the tire was caused by alignment. I contacted Tesla made a service appointment for alignment for August 26, 2024. I got another flat on the drivers rear on [XXX], which cost 283.00 from Les Schwab with the same disposition as caused by alignment. I took the car in the next morning and had the remaining two tires changed as it was no longer safe to drive 766.00 cost, and alignment was to blame mileage was documented by Les Schwab at 18792. When I went for the service at Tesla, I was told they do not warranty tire or alignment and when I told them that I would let small claims decide if that the tire wear was excessive and caused by improper alignment, they threw me out of the dealership and have refused to work on my car. The main reason for filing the complaint is none of this was disclosed when I purchased the car, every car I have ever bought has come with a spare tire, jack, and tools needed to change it. There are no other tire repair shops that can do Tesla alignments, Tesla offers no warranty on their tires and Les Schwab stated even their lowest cost tires come with a 20k mile warranty. Tesla is price gouging for tires 540.00 each and alignment 275.00. Any of the Tesla forums state that this is happening thru all models and tire sizes to most owners. Since I purchased my car in July 2023, I’ve had multiple false forward collision warning resulting in my insurance premium increasing by sizeable amounts and has caused me undue stress. I’ve reported this problem to Tesla but to no avail. Today 8/9/2024, between 5:17 and 5:21 pm, while driving my cars forward collision warning was activated while no vehicle was in front of my but instead to a parked car on the pavement. This has happened multiple times and Tesla denies being aware of any issues.

This is a duplicate of the letter I sent to Tesla on June 25, 2024. “For the past two weeks I have traveled to New Jersey and back to North Carolina. I’m writing to you because of my experience with TESLAS FSD. On one occasion I was traveling on [XXX] (NJ) heading to [XXX] -with my navigation and FSD engaged, I approached north and south exit ramps, my Tesla could not make a decision - I had to bring my car to a full stop before the apex of the exit. Cars wizzing by me on both sides. On [XXX] heading home on [XXX] somewhere in Virginia- with NO ONE around me doing about 65 mph, my car almost came to an abrupt stop ON THE HIGHWAY - once again, FSD FULLY ENGAGED - MY FOOT OFF THE BRAKE AND MY HANDS LIGHTLY ON THE STEERING WHEEL. WHY? I have NO CLUE - fortunately it reversed itself back into normal. Of course, both of the above incidents frightened me. I’m writing to you because I want a record of these events. I am writing to file a claim regarding a safety issue with my Tesla vehicle, which is subject to the recall identified as NHTSA Recall Number 24V554. The recall involves a potential defect in the hood latch assembly, which may not detect an open condition. As a result, this defect can cause the front hood to unintentionally open while driving, potentially obstructing the driver’s view and increasing the risk of a collision. Safety Concerns: Although no crashes or injuries have been reported related to this issue, I experienced significant safety concerns while driving my vehicle. The possibility of the hood unexpectedly opening and obstructing my view caused considerable anxiety and distraction while operating the vehicle. Remedy and Actions Taken: Tesla has issued an over-the-air software update (firmware release 2024.20.3) to address this issue. However, despite the update, I remain concerned about the reliability of the latch system and the potential risk it poses to my safety and the safety of others on the road.

1) Driving at highway speed on a highway, the windshield wipers stopped and would not restart in the middle of a pouring rainstorm. We could barely see, but we pulled to the roadside and stopped the car. We put the car in park. Once we put the car in drive again, the wipers would work again. We took the car to the Tesla service center and they replaced the wiper module. 2) When stopped at a stoplight the car would not drive forward when the light turned green. The car would not go again until I put it in park and then back into drive. When exiting a parking lot and pulling out into traffic, making a right turn, the car just stopped and would not go. The car would not go forward in drive and would not go backward in reverse, it just sat there. I put the car in park. Then I put the car in drive again and it went forward as usual. I took the car to the Tesla service center and here is their report: "Repair Notes: Technician verified customer concern through review of vehicle alerts at customer timestamp and tow. Technician performed a very exhaustive circuit integrity check on the vehicle communication network and found no faults. Technician then investigated closer at the customer timestamp and found the vehicle Car Computer restarted itself due to an over memory condition upon wake, resulting in the experienced symptom. This is a know firmware characteristic that Tesla Engineering is aware of and actively working on a permanent fix to be released in future firmware. Technician also removed and replaced the faulty wiper motor.

On 5/28/24, while pulling up the driveway, the car suddenly accelerated and it’s so fast that we couldn’t react. The car crashed into the garage, knocked down one wall along with the main electrical panel. It stopped because it got stuck on 10 in foundation’s of garage structure. The collision mitigation feature did not get triggered and didn’t give any alert or helped to stop the car from crashing into the garage. Airbags on both driver and passenger sides did not deploy. Prior to this accident, the car already had issue with the driver’s profiles and the seat just moved back and stuck at the easy entry mode. The car became inoperable because the driver’s seat can’t be moved forward to a position where I could reach the gas and brake pedal. I contacted Tesla service and was told to delete the driver’s profile and create new one , power off the screen, power cycled the car. I’ve done all of that but the problem kept happening on and off. The car was scheduled to be looked at by Tesla on 6-20-24 but the accident happened on 5-28-24. We’d like to find out the root cause of the accident whether the car has any malfunction that could’ve triggered the sudden unintended acceleration. We’ve reported the accident to both Tesla and my insurance company. I was turning onto a busy street and the car suddenly stopped and would not respond to the gas pedal. A message said "shift into drive". I had been driving for about a mile already and had not touched the shifter. I then tried to shift into drive and it would not change. I tried multiple times. I then opened the driver door, shut the door, pressed the break to put in my pin, and then it would shift into drive. This was very dangerous as it stopped with the car partially in the road. Luckily there were no cars around.

when I was driving my Tesla Model 3 with FSD exiting [XXX] , the FSD directed me to a wrong routing. I corrected the direction by taking over the FSD, the car then lost its full braking system. I was not aware of it until my car slipped backward at a stop and hit a car behind me. The other driver is good enough to let me go, but my Tesla back bumper was damaged. When I got home and read the message on the screen, I was told that the braking hold was disabled, autopilot was disabled, braking charge was disabled, and wheel alignment was disabled, and more. I contacted Tesla and make an appointment half a month later. I was also told that the Tesla recommends to have alignment adjusted every 6000 miles or every 6 months- while I was not aware of this, I thought Tesla is maintenance free. This was what I told Tesla representative then: If this disfunction is safety related, it should be mandatory, not recommended! I feel I was lucky that I only have a small accident, it could be worse. I just dropped my car to Tesla for repair yesterday, the estimated costs was originally $275, then it went up to ~$1500, then ~$1800, When I was talking to a representative, the costs increased to $2167 in a few minutes! I asked why the prices go up, they said the previous prices are mistakes, and if I don't want to pay, I can drive my car home! I believe this happens to many other Tesla users as well, it needs to be addressed before more accidents and damages. The fragile safety operation and the way Tesla treat its customer are not acceptable, at least to my case. 1. My 90 year-old mother-in law got trapped inside the car in very bad weather because my phone key wouldn't open the door after I got out and walked her elderly friend to the door of her house about 20 feet away, and then walked back to the Tesla. I've been able to reproduce the problem repeatedly, of course without my mother-in-law in the car. 2. Several near-misses because, unlike most other cars, the Tesla's blind-spot warning system, and many other basic control functions such as adjusting the windshield wipers, opening the glove compartment, etc, require you to take your eyes off the road. For example changing lanes requires you to look away from the lane you're about to move into to check the blind spot display on the touch screen. 3. Several near misses because easing up on the accelerator slows down the car forcefully 98% of the time, but does nothing 2% of the time. Specifically, accidentally flicking down on the right-hand control stick, which turns on the windshield wipers in most cars, activates cruise control and gives minimal feedback to the driver that cruise control has indeed been activated. In this circumstance, easing up on the accelerator pedal doesn't slow down the car at all. This is very frightening and unexpected and could easily cause an accident.

The car went forward when in reverse mode. After going to other modes (drive/neutral/park) and back to reverse, the car kept going forward. Reset the system and the issue persisted. Finally resolved after third restart. The car bumped up against the driveway gate because it wouldn’t reverse despite the dashboard clearly indicating being in reverse. Contacted Tesla, who examined the system remotely and said no issue was identified. This was the second time this issue occurred, and Tesla offered no fix.

This incident is reported by a law office. The car being driven by our client was a TESLA Model 3 which made a SUDDEN UNEXPECTED ACCELERATION when our client was slowly looking for a parking space at a parking lot. The TESLA car suddenly accelerated at a high rate of speed without client pressing on the accelerator. On the contrary, our client was stepping on the brakes in an attempt to stop it but the TESLA did not stop but still continued to accelerate. This accident caused property damages and injuries to our client, his passenger, and others.

I turned into a parking lot across a gutter filled with ice from a snow plow and drug a block of ice under the car about 50 feet. When I stopped there was a blue fluid leaking out from under the car. I had the car towed to a collision repair shop. Estimated cost to repair $21,591.79. Damage seems excessive for a low speed collision.

I was driving my daughter to school on the freeway when we heard a loud bang toward the back of the vehicle and the vehicle alarms started going off. It said the car was powering down and would not restart. The car immediately reduced speed from 65 to 35 mph and I had to maneuver across 4 lanes of rush hour traffic to safely park on the side of the freeway. Thankfully this did not cause a collision but we were lucky. Even though the car was charged at 55% within 10 minutes the car was completely dead and powered off. I was able to take a picture of some of the error messages on the screen quickly before it died. VCFRONT a191 Electrical system power reduced vehicle shutting down, VCFRONT 192 Electrical System is unable to support all features switching off features to conserve energy, BMSa 059 Vehicle May not restart, BMS a170 acceleration and top speed reduced, HVP w028 Vehicle May not restart. There was no visible damage to the outside or undercarriage of the car. At the dealership when I had the car towed there I was specifically asked if I had washed the car or gotten it wet, which I had not. It has been a week and I have not had a response from Tesla as to the cause of this terrifying event. I have only been told that it has been escalated to the “higher ups” and they are still diagnosing the problem and will not tell me what defect they have found in the car or any date as to when they might have answers. The life and safety of myself and my child were put in danger due to this total shut down of my car on the freeway. Thankfully I was able to safely navigate to the side of the freeway at reduced speed but I do not want this to happen to anyone else. I hope I can get some answers to prevent this from happening in any other 2023 Model 3 .

As a Model 3 owner, I have generally found the vehicle and its features to be reliable and safe. However, the recent update has introduced an overly stringent hands-on-wheel detection mechanism that is not only inconvenient but also potentially hazardous. The new update requires frequent and often forceful interaction with the steering wheel to assure the system of driver presence. This change is drastically different from my previous experience, where I received only one hands-on-wheel violation in two years. The current sensitivity of the system disrupts the smooth operation of the autopilot, leading to frequent and abrupt disengagements. I have found myself struggling to maintain the system’s activation, inadvertently causing the vehicle to exit autopilot mode multiple times. This issue goes beyond mere inconvenience; it actively detracts from driving safety. The need for constant and sometimes aggressive adjustments to satisfy the system’s requirements is distracting and can lead to erratic vehicle behavior. The irony is stark: a system designed to enhance driving safety and ease is now a source of potential danger and stress. I am writing to express my deep concern regarding the recent rear drive unit internal part failure in my new Tesla Model 3. The incident has left me fearful that this issue may occur again, and I am genuinely worried for my safety. I implore you and all the employees at Tesla to be honest and transparent about this defect or failure. The gravity of the situation became apparent when I realized how fortunate I was that the car did not come to a halt on the train tracks. The potential consequences could have been catastrophic, not only for myself but also for the passengers on the train. It is alarming to think that such a dangerous situation could arise in a vehicle that is merely five months old. This level of negligence is unacceptable. The rear drive unit failed and the car stopped. I was instructed to reset. I did it twice but still not working. I am lucky it happened on a not busy street and was able to steer the car to the side of the road before it became disabled.

Bought a 2023 Tesla model 3 performance that vibrates from the day I bought it. After 12 service visits, new wheels tires and even a new brake rotor the car is still vibrating. Tesla seems to say it’s normal and that all teslas drive like this but no other vehicle manufacturer has ever produced a car that vibrates. It’s unsafe and scary to drive this car the way it is. They tried fixing it at first hence why so many parts got replaced but since they don’t know how to fix it they wanna just push the “it’s normal and within spec” speech to me. Please get a recall going to address the ongoing steering wheel vibration problem faced by numerous owners as shown on numerous threads and forums. This is unacceptable.

I am writing to report an incident involving my Tesla Model 3 (Blue) and to initiate the process for an insurance claim. The incident occurred on [XXX], at approximately 2:15 PM. My fiancée was the driver of the vehicle at the time of the incident. Incident Details: - Date: [XXX] - Time: Approximately [XXX] - Vehicle: Tesla Model 3 (Blue) Location: The incident took place on the freeway merge from [XXX], where it merges onto [XXX]. Heading home. Circumstances: My fiancée was operating the vehicle in Autopilot mode while maintaining her hands securely on the steering wheel. As she approached the freeway merge from ramp [XXX], heading onto [XXX], she experienced an unexpected jolt. Despite her continuous control of the steering wheel, the Autopilot system seemed to be accelerating without her input. Simultaneously, my fiancée felt a sudden tire malfunction, which resulted in a significant loss of control over the vehicle. Despite her best efforts to maintain the car's position within the lane, the Tesla Model 3 veered off the freeway and entered the adjacent Ravine area. Impact and Airbags: Upon entering the Ravine area, the vehicle experienced a collision. multiple plants/highbrush/uneven terrain as well as having to maneuver around 2 large palm trees which resulted damage to side mirror. Notably, none of the vehicle's airbags were deployed during the incident, and there were no sensor alerts, except for the tire pressure warning light. Also there wasn't any record of dashcam recording when we checked the car. We are seriously concerned that the airbags did not deploy , attached are photos concerning the damage. I would like to emphasize that my fiancée was diligently following safety guidelines and had her hands on the steering wheel while using Autopilot. This incident occurred suddenly and unexpectedly. While driving on a two lane highway I experience daily ghost braking incidents. While doing 75, in both cruise and autopilot mode, the car will slam on the brakes when approaching a semi in the right lane. Today it even swerved to the right without the semi moving out of its lane. I also experience panic braking if a car crosses the lanes in front of me even if it is 500m away and the crossing car even left the road before braking occurs.

The vehicle jumped the curb, crashing into two different vehicles to eventually be stopped after crashing into a tree. The air bags did not deploy. There was damage to the other vehicles and to the tree. The driver received injuries to their neck and back. Medical attention was not needed. A police report was filed. The vehicle had been towed. The vehicle was taken to a certified mechanic who had not diagnosed or repaired the vehicle as of yet. The manufacturer was made aware of the failure. The approximate failure mileage was 500.

On August 5, 2023, my car charge got too low for me to make it to a charging station in Lafayette Louisiana. I had the vehicle towed on a flat bed. I put the car in Tow Transportation mode. While we were towing the car, I opened the car to check the battery level and somehow the frunk was activated (perhaps I accidentally hit it, I'm not sure). The frunk opened while we were going about 60 mph down the highway, the wind lifted the trunk up and it crushed my front windshield and the frunk and opening mechanics were damaged. Had we not immediately pulled over the frunk hood could have been ripped off the car by the wind and caused a major accident. This seems to be a flaw in the Tesla electronics/app system - you should not be able to open the Frunk once it is put into Tow Transportation mode.

The rear door jammed shut. It would not open and we discovered there is no mechanical way for a passenger to open the back doors of a Tesla model 3. The car’s electrical system has to be on and functioning properly in order to exit from the back. This is terrifying.

soon after purchasing the vehicle (mile 43) while driving, the propulsion system failed completely and the screen instructed me to pull over but I could only drift as, again, pressing the gas pedal did nothing. This was a very dangerous incident and one that makes me fearful of putting my family back in the vehicle. I have repeatedly asked to speak with anyone at tesla about this issue but have had no email or phone calls returned or been given phone numbers that do not work. I received a message today that i am required to take back possession of the "repaired" car or be charged 100 dollar per day fee to keep their loaner. I do not wish to be behind the wheel when the other manufacturing defects are discovered but Tesla has given me no alternatives. A quick online survey of online forums indicates this is a recurring issue and one that tesla seems to treat with a "lets fix and hope it doesn't happen again" attitude. again, this is not a "failure to start" issue. this was a dangerous incident Tesla offers zero avenue to address with their corporate staff.

Software Version: 2023.12.10 Over the past month, I have experienced a recurring problem with my vehicle's auditory notification system. The issue first began a couple of software updates ago and has persisted through the most recent update. The problem manifests sporadically, having occurred three times within the past month. When this issue arises, all system sounds, excluding radio and media, cease to function for the duration of my ride and persist across all future rides until the vehicle is hard rebooted. There are no visible warning signs or error messages on the dashboard to indicate that the system sounds are malfunctioning. Seatbelt Warning Sounds 3. Blinker Indicator Sounds 4. EV Reverse Sounds 5. System Notifications 6. All alerts and warnings Attempts to Resolve the Issue: To temporarily rectify the issue, I performed a hard reboot on the vehicle's system. This measure restored the sounds, but the problem consistently returns. **Safety Concerns:** This issue poses a significant safety concern as the lack of audible notifications increases the risk of accidents, particularly due to the absence of forward collision warning sounds, seatbelt warning sounds, and blinker indicator sounds. Additional Observations: It is important to note that this issue began a few software updates ago and has persisted despite subsequent updates. There are no error messages or warnings on the dashboard when the auditory system malfunctions, making the problem harder to detect until an auditory notification is expected but not heard.

While crossing the rump and side walk to garage the car, as usual slowly proceed to enter the Garage but immediately the car went into extremely fast speed by itself and hit the side of the Garage, damaged the Garage wall, The garage opener, the cabinet, then hit the water softener and our other car in the front. It also smashed the bike against the wall , damaged the side wall cause a bulge to outside of the structure. The garage frame and right side of the Garage door fell on top of the car almost killed our daughter who was at the front passenger seat. The car brake did not stop the car, the alarm did not go off, the air bags did no inflate.

I picked up my Tesla Model 3 Performance less than a month ago and from day 1 it has been having vibrations from 40mph all the way up to 80-85 mph. I haven't driven it faster than that due to safety concern. I took it to Tesla and they said they needed to balance the wheels (why wasn't that done on a brand new car already?) Anyways, I take the car back and I was still having the same issue. Tesla has now booked another service and said that my Wheels might have been bent by me! The audacity! I gave them a piece of my mind so they will not charge me for service and will inspect my vehicle soon. Meanwhile I have driven the car 2500 miles and the vibrations have not gone away. I know this problem exists in Model S Plaids as well as with some other Model 3 Performance owners as I reached out on the forums. Tesla has given up and accepted that they cannot repair these issues. Lets see what happens with my car. I plan on taking them to the court for lemon law

The contact owns a 2023 Tesla Model 3 equipped with Pirelli Tires, Tire line: P Zero, Tire Size: 235/35/R20, DOT Number: (Not available). The contact stated while driving 65 MPH an hour after purchasing the vehicle brand new, the rear passenger's side tire experienced a blowout, and the rim was severely damaged. The vehicle was towed to a Tesla Service Center, where the rear passenger's side wheel and tire were replaced; however, the rear driver's side tire experienced a blowout a week later. The vehicle was towed to the Tesla Service Center. The manufacturer was notified of the failure and informed the contact that he would be charged for the repair. The tire was not replaced. The tire and vehicle failure mileage was approximately 80.

The undercarriage shield of my car disconnected and started dragging while I was driving in the rain. I did not collide into anything or hit a bump. I inspected my wheel well to discover a set of screws that were bunched up and not installed during manufacturing. I had to pull the shield from under my car so that I can continue to drive the vehicle. the shield is connected to the forward warning and pedestrian warning collision system which became inoperative. The faulty manufacturing that caused the shield to disconnect caused a dangerous condition while operating. After I removed it, a second dangerous condition was created since all of the safety senors were disconnected.

Tesla promised in Nov 2022 that software update enabling parking assist will be available within "next couple of weeks". 3 months passd since then and there is still no parking asist.

My Tesla Model 3 was delivered on 14 December 2022. I first drove it on 16 December 2022 and that day I had an accident. I was about to park my car and I was already on the designated parking slot when the car made a sudden Un- commanded acceleration. I first hit the handicap sign, but the car did not stop and apparently the collision avoidance safety feature did not work. The continue to accelerate and I hit a restaurant establishment. The police came and I reported the incident to my insurance (Geico). The police (Mr. McClure) gave me the case number 2238293. Geico is taking care of the repair which would cost over $10K and the property damage, which added up to 10K. I went to the Tesla service center in Carlsbad and Mr. Hyuma said that I can request for the log. I guess it is the EDR. They gave me a link to request for the EDR but the start date is 23 January 2023 way after the accident date of 16 December 2022. I have not received any log or EDR from Tesla to date. I am uploading the pictures of the car before the repair. Please help me find out what exactly happened and why the car suddenly accelerated without pressing the gas pedal. I am a retired US Navy (20 years) and I bought a Tesla to save money because i have solar in my house. I have a fixed income and cannot afford to spend money on the EDR. I believe that here is a defect on the Tesla Model 3 that I bought and I will appreciate all the help you can provide.

The contact owns a 2023 Tesla Model 3. The contact stated that while driving at 50 MPH another vehicle suddenly entered the lane that the contact was driving on, causing the contact to strike the other vehicle. No air bags deploy, and the brake assist feature failed to activate. There were no reported injuries. A police report was filed. The vehicle was towed. The manufacturer was notified of the failure and filed a case. The failure mileage was 8,728.

This model has misleading user manual about park assist chime button, park assist front camera on button. Apparently they have removed the ultrasonic sensor and did not enable any user interface to do with Park assist in the control menu. This is misleading safety feature and total lie to the customer. Please flag this as a safety flag so the user will be aware of it. There is no option for the user to enable the front camera for whatever the little useful it can offer. They need to immediately send us the software update to view any camera at will.

I was driving a turn on a major intersection Stirling and I-95 Holly wood Florida to the on ramp to enter I-95 southbound when my car began shaking violently during the turn and completely shut down. The car was dead and I could not go forward or reverse. The center information panel was still lit up but the car would not move in any direction. I was blocking two lanes of the major intersection for about an hour till 2 Road Rangers arrived and dragged my car off the road. If I had made it around the turn and this catastrophic shut down occurred when I was on I-95 I could have experience severe injury or loss of life if I was struck by traffic on the highway. The car was brand new with 108 miles on it at the time. I purchased the car on 11/20/22 and the shutdown occurred on 11/23/22 after only 3 days of driving it. This Tesla Model 3 was very unsafe and the cause of what happened to me should be investigated so other people do not get injured or lose their life.

I was pulling into a park spot , coming to a stop and the car within seconds accelerated into a building. There was no warning at all. Also, after the collision a warning popped on the screen saying that the emergency break was disabled. This was the first time seeing this alert, and I did not disable anything. I picked up the car on 11/28.

Picked up car from the dealership same day and had only 40 miles total on odometer. The car was running smooth. I was driving with my daughter in the passenger seat (12 yrs old) and got 2 error codes and my car went limp in the carpool lane. Basically went into neutral. Deceleration from 75mph .I had to avoid cars next to me to get to the shoulder and was driving at most 30-40 mph hoping a car would not slam into my rear. By the grace of God I made it to the shoulder safely. “Acceleration and Top Speed Reduced” and “Automatic Emergency Braking is Disabled” were the two errors. I’d never been so stressed. That was highly dangerous and I’ve been reading online of several other Tesla owners suffering the same fate. Please don’t wait until someone gets injured or killed before making Tesla recall their cars. I’ve been hearing its the Rear Drive Inverter (Motor) that is faulty according to others online. The service center has not diagnosed the problem with my car yet but I think that is the same issue. I feel that Tesla is negligent and should be penalized for allowing these cars to be driven off the lot by unsuspecting customers.
