I bought my car used last year! I have almost been involved in many accidents with this car! There is only 1 small button for the horn mixed in with other buttons on the right side of the steering wheel! When stress hits because somebody is heading right for you and you are frantically looking for a small button to lay in the horn it’s almost too late! It’s a MAJOR safety issue and they are offering a new steering wheel for almost 1900.00!!!! I already replaced the yoke wheel it came with! This is a total safety hazard and people are going to end up dead because of it if they don’t fix it!

I received recall notice for 24V-554 on 9/22/2024. The recall notice informs that Tesla will resolve the recall safety defect by over-the-air update via the car's WIFI capabilities. I have informed Tesla that my car's WIFI capabilities are broken and I scheduled service for October 3rd. Tesla informed that parts for my vehicle are not yet ready and rescheduled my service for October 24th. I informed Tesla that I am missing a required safety recall update, and asked if they able to push the update to me via the car's internet or otherwise remedy the issue of my car not having capability to resolve the safety defect. Tesla responded "Due to contraints at the service center we unfortunately had to reschedule you" for October 24th. The Safety Recall notice informed that if the manufacter is unable to remedy the safety defect within a reasonable time, I may notify the NHTSA.

The horn is a button. As someone almost steered into me, I looked down to find the horn and almost ran into a child as my car swerved!!! This is incredibly unsafe to hide the horn button in an incredibly small area next to the wiper button and cruise control. How this was approved is beyond me!! Tesla needs to make all current cars correct by having the horn be accesible. Like every other car in the world!

The horn is a tiny button on the steering wheel. Several times when someone inadvertently started entering my lane I couldn't find the horn fast enough to warn them so had to veer away and brake suddenly to avoid them as I couldn't warn them.
Aug 03, 2023
I was in the middle of a turn when someone veered into my lane. I tried to honk my horn to warn the other driver, but I couldn’t find it. The button was on the other side of the steering because the yoke was turned. I couldn’t find the horn button in time to avert a collision. Fortunately I was able to steer away in time.

STEERING
17 complaints
I am the owner of a 2021 Tesla Model S Plaid (VIN# [XXX] ). At approximately 12,000 miles—well within the 4-year/50,000-mile Basic Vehicle Warranty—I began experiencing peeling on the yoke steering wheel. I reported this issue verbally at the time, but Tesla staff failed to document the concern, and the Tesla app provided no suitable method to submit the issue. Since then, I have made multiple good-faith attempts to schedule service for this defect. However, each time I have been met with out-of-pocket charge demands, including being told I must approve a $2,000 charge before Tesla would even address a basic light repair—effectively blocking my ability to have the steering wheel defect evaluated under warranty. When I explained that this issue is clearly a manufacturing defect and should be covered, I was told I did not complete the appointment. In reality, I was unable to proceed because of Tesla's unjust demand for payment for something that is not my fault. I have already reached out via email regarding this issue and received no meaningful resolution. I was also dismissed by Kennesaw store manager Hunter Moss during attempts to escalate. Public customer feedback suggests I am not alone—numerous Tesla owners have reported similar experiences with yoke steering defects and customer service dismissals. It’s frustrating and embarrassing to have friends and family see a visibly defective steering wheel in a premium vehicle like the Model S Plaid. Under the Magnuson-Moss Warranty Act (15 U.S. Code § 2301 et seq.), Tesla is legally obligated to honor express warranties and cannot deny warranty coverage due to internal miscommunication, technical limitations of the app, or improper handling of service requests. I am formally requesting a warranty-covered replacement of the yoke steering wheel without delay. Tesla has 7 days from receipt of this message to confirm a service appointment and warranty approval. If this matter is not resolved promptly, I will INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I have the yoke steering wheel. To activate the horn, you press on the right side of the steering wheel. I was driving at freeway speeds and a car did not see me and started to move into my lane. Due to the placement of the horn on the side next to windshield wipers, the wipers went on but not the horn. You have to take your eyes off the road to look down for the horn. Tesla is aware of the dangers of the placement of the horn and has a new yoke steering wheel with the horn in the traditional position, center of wheel. Unfortunately, Tesla has not on its own recalled the older model yoke although they have taken remedial steps to improve the design. I believe a formal recall should be initiated on this steering wheel.
Dec 04, 2024
Due to the yoke not having a traditional horn button it is hard to remember to push the tiny capacative touch button... very dangerous. Also the yoke feels more dangerous than a normal wheel.

Went to a Tesla dealership to get my car aligned. 4 days later they told me that my car couldn’t be aligned because my fron lower control arm was bent or broken. I purchased the car from Tesla with 5700 miles. The cad had about 19k miles when it went in for alignment. Seen owning the vehicle I never had an accident or damage to vehicle that would cause such damage. Like always they like to blame the customer to we can cover the defects of there vehicles.

Driving Southeast on [XXX] between Gallup, NM and Albuquerque, NM. I was traveling at 75 MPH on an open road, it was a sunny day. I had the adaptive cruise control engaged (ACC). Literally no traffic in front of me for as far as the eye can see. Nothing that would cause an alert or obstruction in front of the car. The Tesla was on ACC and suddenly braked, hard. I immediately disengaged the ACC and drove manually. It was a frightening incident. If there had been traffic behind me, it would have resulted in a crash, as I would have been rear ended. If the traffic had been a truck behind us, we would probably have sustained extreme injuries. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The adaptive cruise control has had phantom braking issues from day 1 of ownership (and through every software release from Tesla as I always update when a new one is available). Most of the time it's not possible to identify exactly what triggers the event. However, on July 27th 2023 I drove from Phoenix to Sacramento via I10, I210, I5 and had 6 phantom braking events. 5 were directly correlated with passing a semi with me in the #1 lane and the semi in #2. I had the car in adaptive cruise control mode, but I maintained steering control. The car approached the semis normally, but as soon as the car was completely alongside the truck it began braking aggressively. More than just allowing regenerative braking to slow the car, the brakes were applied. This did not happen with every passed truck, maybe 1 in 10. One of the 6 phantom events could not be correlated with any clearly identifiable cause. There is no warning when the car begins decelerating so it is a surprise to the driver. Further it is an extreme hazard to following vehicles who WILL be forced to react. I have not directly reported my specific events to Tesla. For one thing they have no reasonable way to do this (online access, phone access. The number they give to report problems hits an automated phone tree where there is no option for reporting issues). Clearly Tesla is well aware of their phantom braking issues. I'm reporting this to you now in order to apply more pressure to Tesla to finally resolve this. In general, considering some of the other phantom braking episode beyond this one specific trip, I have occasionally noticed the car displays a yellow signal light on the driver's display but only while phantom braking has already begun. I believe this is case where the car believed it saw a caution sign or signal light in it's cameras and was reacting to that. There were no signal lights, but the sun was either coming in that direction or reflected off of other vehicles or buildings.

Adaptive Cruise Control. The vehicle in this mode suddenly brakes for no apparent reason which under the right circumstances could cause a collision. Tesla Service Center looked at it but could not reproduce. This has happened many times on various dates not just once. Sometimes it happens more than once on a trip, sometimes it does not happen. This happens with no warning. Tesla knows about this problem for years but has failed to correct.
Jul 27, 2023
Going north on New York State highway I-87, there is a bridge which results in Tesla Autopilot erroring EVERY TIME AND EVERY DAY and results in a "Full Self Driving Error" which causes Telsa car to brake along with putting on hazard lights. Issue was reported to Tesla, but they responded that it happens to everyone under this bridge so there is nothing they can do right now. The fault occurs on a busy highway at 55mph zone, and happens 100% of time going under the bridge, every single day. The bridge is wide enough that shadow results in error and car loses all understanding of what lane it is in. Today at the Tesla service center, the employees all reported the same issue as well and that there is no idea if it will be fixed. Happens only when using FSD/AP driving support; manual driving is unaffected.
Jul 18, 2023
When using adaptive cruise control vehicle randomly applies brakes while driving at highway speeds, for no reason. Sometimes braking is severe, sometimes moderate, sometimes light. In the instances of severe or moderate braking, a real danger exists that following vehicle will be unable to stop and will collide. This happens relatively frequently, particularly on some highways. Sometimes multiple incidents occur on one drive.

TIRES
9 complaints
Certified pre-owned 2021 Tesla Model S delivered on October 21, 2025 with worn Michelin Pilot Sport 4S summer tires (255/35ZR21 front, 295/30ZR21 rear). Tread depth measured approximately 2/32" on the outer tread shoulders of all four tires, below Tesla’s published 5/32" delivery standard. Within the first week of ownership, the vehicle hydroplaned multiple times in wet conditions at normal speeds. Tesla service declined to replace the tires, stating measurements were “taken in the wrong location” and that only the inner grooves count, despite Tesla’s public documentation and national tire safety standards defining tread depth across the full width of the tire. Vehicle was returned in same condition without correction, leaving a continued safety risk from inadequate tread and summer tires unsuited for Ohio’s cold and wet seasonal conditions.

I have a 2021 Tesla Model S Long Range with rear tire size 295 / 30 ZR 21. Both rear tires are worn so bad on the insides that the cords are showing. These tires were purchased and installed by Tesla along with a wheel alignment under 10,000 miles ago. The tires still look like new on the center and outside of the tire so I had no clue this was happening. The only way I became aware was because the low tire pressure warning kept coming on I started losing air slowly over the last month and thought maybe I had a puncture. The damage to the tire isn’t easily visible when just looking under the vehicle. I urge anyone with these tires to check them as it can easily result in a blow out.

295 / 30ZR 21 - My tire failed well before the tread life expired. You can see in the attached images that the tire failed at the meeting point between the tread and the sidewall.

Tires wear less than 1k miles. Deflate, side wall wear, separation, tread wear etc. Have purchased around 8 tires from January 2022 - Present FRONT - 265/35/ZR21 101Y Extra Load REAR - 295/30/ZR21 102Y Extra Load Michelin honored tire failure for side wall separation then denied all other claims. And this is the only tire Tesla has on hand since there is no spare.

Michelin Pilot Sport 4S 295/30 ZR 21 102Y extra load Purchased Oct 2021, estimated 12,000 miles on vehicle and tire. Tire split along inside corner, nearly all the way around the tire. No warning, instant loss of pressure. No significant wear on the outside corner or the tread. Previously happened earlier in the year on the passenger side rear tire.

The contact owns a 2021 Tesla Model S. The contact stated that while the vehicle was parked, the vehicle was connected to the charging port and independently went into Race mode. The contact stated that the charge increased from 0 to 60 instantly. The vehicle was taken to the dealer, where it was diagnosed with a software failure. The vehicle was repaired; however, the failure reoccurred. The contact stated that while the vehicle was charging, the charging stopped intermittently, and the vehicle completely stopped charging. The vehicle was taken back to the dealer, where it was diagnosed with battery corrosion and failure. The contact was informed that the battery contained water and was corroded. The contact questioned whether the vehicle had been in a flood. The contact was informed that the battery needed to be replaced. The vehicle was not repaired. The contact stated that the vehicle was taken to the dealer several times due to the persistent failure. Additionally, the contact received an unknown recall notification with a similar description of the failure; however, the contact was informed that the repair was not covered under the unknown recall due to indications that the vehicle had been in a flood. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 1,000.

The horn is a small button in a very small area of the steering wheel. The center of the steering wheel which acts as a horn on almost all other vehicles, does not work. In the event of an emergency, and the horn needs to be activated, it is difficult to find and press the small button, and the instinct would be to press the center of the steering wheel, which would not work and the horn would not activate.

The instrument panel (screen) goes blank for approximately 5 seconds while driving. This issue happens at random. It has happened while on city streets, the highway, and in parking lots. Because of this issue, I cannot see my speed, the visualization of the car and surroundings (blind spot or pedestrians), any warnings that may pop up (such as an issue with the Autopilot or Full Self Driving Beta), nor any lighting or wiper information (for auto high beams and auto wipers). Tesla says it is a known issue, and it is software related, but there is no timetable for a fix. They do not believe a replacement screen/components will have any effect, so they are not offering any type of fix.

My safety concern is with the 17 inch screen. Originally in the information about the car before it was purchased, the manufacturer said that the screen would tilt left or right to give the drive better a better view and access to the touch screen. I have talked to service about this issue and they have said they do not plan to retrofit our cars with the motors to make the screen move. This is now working on the same car that is one year newer. We were promised this and I believe this issue with the screen not moving is a serious concern. You cannot see the screen as well or touch it as well because it does not tilt towards the driver. Please investigate this problem, it is dangerous.

Vehicle braked without warning with cruise control engaged. Luckily there were no other vehicles near us when this phantom braking occurred.
Aug 25, 2023
The Traffic-Aware Cruise Control has been braking unexpectedly and for no valid reason. The most recent example was when we were driving along US Hwy 24 between Wilkerson Pass and Fairplay, CO. The car suddenly braked even though there was no hazard. This has been happening regularly on this drive, in both directions, multiple times per drive. When this happens, the road is generally straight, with little, if any, elevation change. This is a safety issue because if this happens when a car is driving closely behind us, the unexpected braking could result in a rear-end collision. I have not reported this to Tesla because I understand this is a widespread issue that many people encounter, so I believe Tesla is fully aware of the issue. This problem seems to have gotten significantly worse after Tesla deactivated RADAR in the vehicle (without my consent). My understanding is that they are now using only cameras to manage Traffic Aware Cruise Control. Regardless of the reason, this issue has been happening with much greater frequency since around December, 2022 or January, 2023.

SUSPENSION

I purchased my Tesla in September 2021. At 12000 miles, I had to replace both rear tires due to severe inside wear. I replaced them and continued driving, but at 22000 miles, the same issue occurred. Each time, I performed service exclusively at Tesla, where they checked the alignment and assured me everything was fine. At 39,000 miles, the issue repeated. Tesla performed another alignment and TOE adjustment, assuring me it was resolved. However, after another 10,000 miles, the same problem occurred, with the inner tread worn down to the cords, while the rest of the tire had adequate tread. The car is currently at the dealership again, and I’ve been advised to do another alignment and replace the tires, with an estimated cost of $3,300. Despite repeated services and assurances, the underlying issue remains unresolved, and I’m unsure how to proceed.

There is a repeated issue with my vehicle and I looked it up online and it seems all Tesla Model S vehicles have this issue. This is the 2nd time I've had this same issue. As I go over any bumps, accelerate, or decelerate there's a clicking sound coming from the front wheels. While driving there's a whistling sound. Based on websites and videos I've seen all over the internet, it's the result of a loose nut and bolt. It apparently continually occurs because of the very high amount of torque these cars have and the quality of the parts used for this torque doesn't suffice. Tesla needs to fix this on all their Model S vehicles. Instead apparently all they do at service center is apply grease to the affected area to temporarily make the noise go away but that grease melts away annually.

The suspension is very stiff and to the point when the smallest of pot holes, road bumps, or going over the road divider marker sends the shock to the body of the passenger. My family complaints of neck pain after a short ride. I live in San Jose, CA. I drive with the comfort mode with the soft ride setting for the suspension. This deep discomfort, pronounced for rear passengers is also increasingly deteriorating. Tesla inspected my car on Sept 8, 2023 - partially fixed the suspension noise. No change to the ride stiffness causing deep discomfort to passengers. Tesla inspected my car on Nov 8, 2023 for the same issue. No resolution. Tesla inspected my car on Dec 17, 2023 for the same issue. No resultion. I was told by Tesla technician who drove with me, that the ride is stiff, however, there are no fixes currently. A change to the setting and / or suspension will happen if there is a volume of complaints. Initially, the suspension had sound issues and frequent cranking. Tesla fixed it partially. However, this did not resolve the stiff suspension. I went to the Tesla dealership three times, each of the times, Tesla toggled with the settings but not avail. Tesla now claims that the suspension is what it is, any relief has to come from NHTSA or I have to sell my car.

In August 2022 I was driving my Tesla S. I was traveling 25 miles an hour or less. I unfortunately lost consciousness for a very short time. My tesla then crashed into a parked car. The last time, there was no car in front of me, but the vehicle braked so fast the the car behind me almost rear ended me. Front license plate holder is required in California. The bracket that holds the plate on the car is secured to the bumper with adhesive tape only. My bracket became loose, and thankfully we were able to retrieve the plate and bracket upon parking. This would have been an obvious hazard to us and other motorists if it fell off while on the street or highway. Tesla does not have any alternative solutions to fix or repair this issue and advised we purchase an aftermarket bracket. The front bracket on my car must be attached to meet NTSB safety standards.

I recently encountered a problem with my Tesla where the drivers door opened on its own while operating the vehicle. Tesla reported it was a faulty door handle. I am concerned that the software interlocks related to the door operation have anomalies that create a safety hazard fo car occupants.

Drivers door will not latch and car thinks the door is closed/latched. This allows the driver to drive away with the door unlatched. I've had to open and close the door while moving to get it to latch which is obviously unsafe. In attached picture you can see how open the door is but the car gives no indication this is the case. Have just learned (on routine check) that Front/Rear tire configuration has been backwards since delivery from the FACTORY; 24 mo, 24k miles. WHILE I WAS DRIVING MY NEW TESLA S (APPROXIMATELY 1000 MILES) ON A COUNTY ROAD, THE LEFT FRONT TIRE SUDDENLY CAME OFF MY CAR, WENT ACROSS A ROAD WITH ONCOMING TRAFFIC AND HIT A FENCE AT A LOCAL RESIDENCE. THERE WAS SUBSTANTIAL DAMAGE TO THE LEFT FRONT AREA OF THE CAR WHICH CAN BE SEEN FROM THE ATTACHED PHOTOS.FORTUNATELY, I HAD JUST EXITED THE GARDEN STATE PARKWAY WHERE I WAS DOING 65 MPH. I HAVE LITTLE DOUBT THAT IF THE ACCIDENT HAD OCCURRED 10 MINUTES EARLIER, THERE WOULD HAVE BEEN SEVERAL FATALITIES INCLUDING ME. ALTHOUGH TESLA CLAIMS TO PRODUCE LUXURY VEHICLES, IT LEASED ME A CAR THAT WAS INHERENTLY DANGEROUS AND COULD HAVE CAUSED A FATAL ACCIDENT.

The reverse lights are incredibly insufficient and pose a safety risk when backing up. The lights are so dim, they are virtually ineffective to the point that they illuminate nothing that may be behind the car. This is not a bulb issue as they came from the factory, new, in this condition and the service center said there is nothing they can do. Other owners I have spoken with report the same issue.

Passenger side headlight, daytime running light portion, is flickering, approximately 1-2 times a minute. I only noticed this as I parked in a dark garage with my lights on, seeing the light flickering against the wall. There is no warning from my car telling me this is happening. This is a safety issue because headlights are a safety feature. If my headlight is going on and off, it will affect my visibility, as well as the visibility of my car to other drivers/pedestrians on the road. I notified Tesla, they had mobile technician come to my house, he visually confirmed this issue, and he subsequently replaced the entire headlight. After the replacement, I noticed that this did not resolve the issue. I notified Tesla and they had me bring in my car to service center for inspection. Per tesla tech service summary: "identified customer concern as a known characteristic with no resolution available at this time. Please maintain regular WIFI connection to ensure timely downloads of future firmware updates. No repairs recommended at this time". I need this issue resolved, I do not feel safe driving my car. Tesla is not repairing the issue and has no clear timeframe as to when this issue will be fixed. I attached 2 photos from my video capture showing when the light is on and off.

The 2021 and beyond Tesla Model S comes installed with camber arms that cause the wheel to be tilted in and thus wears the tire on the inside to the point of wearing out within 6 months. This is a flaw in the building and pertains to all the model S's 2021 and greater. This is a safety issue and is not addressed by Tesla. I think Tesla should be forced to provide a recall for this.

On [XXX] at approximately [XXX] I was driving down a small back alley/street in Baltimore when my Tesla Model S Plaid sedan stopped operating. It would not restart, including with the emergency restart bar. Since I was unable to restart it, I left it there while I ran into an appointment. About maybe 20 minutes later it started up like nothing was wrong. I did not receive any warnings on the dash from the car that there was an issue, and I thought it must have been a computer glitch. Unfortunately, sometime between 10:30 and 11:30 am I was driving the Tesla on a major feeder multi lane feeder branch to a highway. I was on [XXX] merging onto [XXX] when the car just stopped with no warning. It is a miracle that I didn’t get hit, but it caused a truly dangerous situation with horns blaring, cars and trucks going around me. It was very unsafe. This time I was able to start the car with the emergency start bar Tesla has available. I was then able to drive the car to my office where the car was picked up and towed to the dealership. To make a long story short, apparently, they (service department) drove the car around the parking lot, trying to reproduce the problem but they said they were not able to. The only thing they were able to tell me about the car was that it had two high-voltage spikes and possibly that’s what caused the problem, but they didn’t know how to fix that problem. I did talk to the lead tech on my car, but at one point he said they were out of resources and would refer this to his general manager. The service manager told me that since they can’t reproduce the problem, that my car is drivable, I should pick it up and drive it. I refused. A car that stops dead with no notice in moving traffic is not a safe car. I was astounded that they would expect me to drive the Tesla. I told the manager that I would not drive it until it was fixed. It is still there. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The start up chime that dings when the car starts plays CONTINUOUSLY while the car is driven. This is not only distracting, but it prevents the driver from hearing the other dings that are associated with the safety systems (collision warning, lane departure, etc.). The horn button on the yoke steering wheel is a safety hazard. In a situation that requires quick responsiveness immediate reaction is to press the center of the steering wheel. Tesla has come out with a retro fit and the new yoke has it in the center. I am paying $1,000 for the retro fit for my safety and the safety of others. I expect the NHS to also look at this concern and address to assist with the prevention of accidents.

The position of the horn button on the steering yoke is problematic. The horn button on the 2021 Tesla model S is operated by the right thumb. This position causes activation problems in various situations. One example is where quick horn use is required, the natural muscle memory is to push the center of the yoke , (where the horn button is located on all manufactured vehicles). The initial reaction to push the center of the wheel, causes an UNSAFE delay in horn activation, as the user then struggles to find the small horn button with the right thumb. Most of the time, the horn button is missed and the windshield wipers are activated, which causes more confusion in a stressful situation. Another problem with the position of the horn button is that when making a tight turn and the wheel is spun 180 degrees, the horn button is impossible to find or activate in a timely manner. It is surprising to me that the horn button placement was allowed to be installed on a production vehicle. While driving the vehicle there have been numerous events where quick horn activation was needed. One of the events happened recently while exiting a public garage. While the yoke was turned 180 degrees to make a right turn, another vehicle almost hit our vehicle and when my wife tried to hit the horn, it was impossible to find the button since the wheel was upside down, ( spun 180 degrees). Only luck prevented an accident. Another event happened two days ago. While driving on a 4 lane highway, a vehicle aggressively changed lanes toward our car. The action of the other vehicle caused a startle response, when my wife tried to hit the horn, muscle memory caused her to push the center of the wheel, instead of the small button that is activated by the thumb. By the time she found the button, it was too late to prevent the other vehicle from swerving into our lane. Also, the windshield wipers were activated instead of the horn. The horn position needs to be changed to the center of the wheel.

During afternoon commute in a residential area, driving at 19 mph, a cat running after some type of lizard run in front of my car. I have been driving for the last 40 years, safe driver designation on my license, and have more than 1,000,000 miles on my back without major accident. I instinctively went for the horn in the center of the steering and tried to catch the top of the steering wheel (absent in my car) to change course. Finding a little button among other little buttons to sound the horn is impossible when the driver is under stress and needs to use the horn. The consequence, sadly, was a severely injured animal that did not survive. Without diminishing the importance of an animal life, if that was a child the consequence would be catastrophic. The steering yoke and horn control pauses a serious danger.

To avoid collision on the highway with a deer crossing in front of me, I braked very hard. The left rear brake caliper mount broke as a result. I did not lose brake fluid pressure, thankfully but this could’ve been a catastrophic failure. I contacted Tesla who seemed indifferent and blaming the incident on my driving and the mileage of the car ( 48,000 ). I have never in my life heard of a caliper mount breaking on any car with any number of miles. I’m sure it happens, but it should not happen on a car with 1000 hp.

Im having total braking loss around freezing temps The issue occurs around freezing temps, after a car wash or if the roads are wet after driving on the highway for 20-30min without using the brakes. (Using one pedal driving) Last year I had the same issue and Tesla’s position was something to do with detergent in the car washes I use. Now they’re saying it’s because I put the “wrong” size rear tires on the car. They were sold to me and installed by Tesla as the factory recommended winter tires and rim package. They’re saying that the abs sensor is detecting the error and automatically applying ABS brakes because of the mismatch tire size. However, the issue is not an intermittent braking loss, it’s a complete braking loss. I’m also seeing abnormal front rotor wear. Tesla said this was an issue that was occurring on some other vehicles and they had a high level engineering team trying to figure it out. At the time mine was one of the first P2 refresh vehicles in Canada so I was guessing there weren’t that many being used in below freezing temps since they weren’t exporting to Europe at the time.

I have never received an audible alert for my blind spot warning. There is no light on the outside mirrors or anywhere else inside the vehicle to catch your attention. If I am looking at my display in front of my steering wheel I will see a car that is in my blindspot change from GRAY to RED when I activate my blinker; however, the car has never beeped or alerted me beyond passively changing the car color to red on my display screen. If I'm changing lanes I am not looking at my screen which is directly in front of me, I'm looking at the road and I'm looking left or right. I have almost hit many other vehicles multiple times because my car will not audibly notify me that theres a car in my blind spot or a car that is speeding up behind me which I might have missed. This is a feature which I paid for and I feel is defective in it's current state. The car is available upon request for inspection. The lack of an audible BSW alert puts myself and others on the road at an increased risk of collision. I have mentioned it to the dealer but after over two years no update has been made. I have not made a big attempt to correct this issue with the dealer since it's difficult to speak to someone at Tesla. No, the issue has not been inspected by anyone. I discussed it with a rep shortly after I purchased the vehicle but it seems like an issue that Tesla knows about if they only programed the vehicle color to change and didn't program an audible alert. No, there have not been any alerts before this issue as it appears to be a simple programing issue.

Autopilot cut off while driving on curved highway exit ramp, leading to driver's side of car grazing guard rail and rims of wheels grazing curb. Mostly paint damage. Repair estimate: $7,000.00 Car sustained minor damage, but I was told by collision center that other similar incidents had occurred, including one in which car had gone off the road into a ditch. I have confirmed this is a consistent software problem, cutting off on all highway exit ramps. Problem reported to local Tesla service, and they uploaded data from the car remotely. There were three dings as an alert, and a warning appeared on driver's display "autopilot guidance complete" and beneath it, in very small letters difficult to read, "Tap accelerator to continue." I consider this a software design flaw. Default on a curve should be to switch from autopilot guidance to lane keeping, not shut off altogether. Also, warning should be more visually an audibly prominent. For example, when guidance is not available on a city street, there is a distinctive alarm and obvious flashing on dashboard display. There should be a similar warning when autopilot is about to shut off.

The airbag module installed in the yoke has popped out while driving. It currently hangs by a wire. This makes the car completely inoperable safely. In fact had the airbag ever gone off in this condition it could cause serious injury and possibly death. Reached out to Tesla for repair and they are estimating 20 days to begin a repair. If cars with this problem get into an accident that activated the airbag I believe there could be very serious consequences for the driver.

While driving with a fully charged battery, the car dash warned “pull over to safe location. Battery cannot support all functions”. We were unable to fully pull over in the time before the car experienced a critical loss of functionality, as the car shut off in less than 1 minute from flashing the warning. The car lost all functionality, including loss of its flashers, putting us at significant risk since we were unable to alert other motorists to our presence on the side of the road as it was getting dark. The car is currently with the dealer and thus far they have been unable to identify the root cause of the issue but have acknowledged the existence of the problem. They’ve claimed the 12v battery died and the car does not have any safety redundancy built in to be able to access power from the main driving battery to power critical features such as the hazard lights. The biggest risk of safety is loss of indicators/flashers while on the side of the road. This feature in vehicles is meant to alert other drivers to the presence of a disabled vehicle. Consumer was told no physical problem was found with the car and a “firmware” problem is suspected. After repeated questioning, the Tesla dealer finally acknowledged that other 2021 Model S cars have experienced similar abrupt shut downs and the California engineers are aware of the problem. They cannot tell me when a firmware update will be available, but in the interim want to return the car to the consumer having done nothing to prevent the problem from recurring.

While using wildly falsely marketed $10k "full self driving" add-on across multiple software revisions and after reporting each and every bug to company I have experienced potentially fatal phantom breaking while at highway speeds and could have been killed by rear ending on at least 4 occasions. Have experienced 20+ erroneous forward collision errors with automatic breaking or lane assist corrections for obstacles such as hills, mailboxes, trees, absolutely nothing at all, etc. When driving around mildly banking left turn corners with 45mph speed limits whenever approaching a left turn lane within median the car will violently lurch into the turn lane causing panic for passengers and nearly causing a collision within resuming concrete median. While using autopilot the car failed to stop, correct or warn when a vehicle entered my lane and caused an accident with several thousand in damages. Car does not properly break for turns on highways requesting driver take over for what should be an easily navigable turn. Doesn't take apex or break early and will drift lanes making other surrounding drivers nervous. Moving the horn from muscle memory location to awkwardly placed touch button has prevented timely use of horn on multiple occasions which could easily result in fatal accidents. Overall this car is a literal death trap not at all even remotely ready for production use on public roads and neither I nor any of my passengers feel safe using the feature that was over priced, over promised and under-delivered. Tesla service center employees too worried about retaliation from management to inquirer about problems because of previous reparations from corporate offices. List of problems is shockingly egregious and should not be legally permitted on roads.

This was a Autodrive malfunction event on a brand new Tesla Model S. I was in standstill traffic when my car, which had been engaged in Autodrive, lurched forward and suddenly accelerated at a high rate of speed into the car in front of us, without any driver prompts. The problem was immediately reported to Tesla who has been delaying in providing the data logs and engineering analysis for over two months. Tesla did what they called a "safety inspection" after the crash and says the car is fine. They have refused to try and replicate the malfunction because they maintain the car is fine. Luckily, no one was injured, and there was only property damage to my car and the car in front of me, but there is something wrong with the software, the cameras, and/or the front end sensors and their calibration for the car to have done that.

Vehicle: 2021 Tesla Model S Long Range VIN: [XXX] I am reporting multiple safety-related defects that existed at delivery or developed shortly thereafter, and Tesla has refused to diagnose or repair them under warranty. These defects create an unsafe driving condition and risk of loss of control or collision. Defect Summary: 1.Alignment Defect at Delivery: On the day I bought the car, the steering wheel pulled to the left and felt unstable. I reported this immediately to Tesla. They dismissed it as “normal.” 2.Premature Tire Wear: Within months, the left tire wore down to the steel belts on the left side while the right side had normal wear. An independent shop confirmed this was caused by suspension/bushing issues, not road damage. 3.Suspension/Bushing Failure: A third-party Tesla-certified shop confirmed that the suspension components are bent/tweaked and require replacement for safe driving. Tesla refused to inspect these components properly. 4.Unlatched Rear Seat at Delivery: The rear seat was delivered unlatched, which is a major safety hazard during braking or collision. 5.Broken Glass Under Rear Seat: Broken glass was found under the rear seat cushion at delivery, raising concerns about vehicle history and delivery inspection standards. 6.New Defect While in Tesla’s Custody: On Nov 17, 2025, moisture accumulated inside the left taillight while the vehicle was parked outdoors at Tesla’s Stockton facility. This can cause electrical shorting or lighting malfunction. Tesla’s Response: Tesla has repeatedly refused to inspect the suspension properly, dismissed my complaints, and produced an inaccurate service invoice that did not reflect what was discussed. They told me we are “at an impasse.” These defects pose a serious safety risk (tire blowout, loss of control, lighting malfunction). I request NHTSA investigate these issues as they may indicate broader problems with Tesla’s suspension, alignment, and delivery inspection processes. FORWARD COLLISION AVOIDANCE,BACK OVER PREVENTION
1 complaint
I engaged Tesla's Autopark to park my car in my usual spot. The car kept going after reaching the back of the parking space, crashing into a pole located around one foot behind the parking spot. The time between when it should have braked and the post was of such short duration that I had no time to apply the brakes and prevent the accident. The car lacks both radar and lidar, either of which would have detected the post. Instead it relies on a single video camera. What made this parking different from previous safe parking attempt in the same space was that the car was perfectly lined up. That meant that the camera could not make use of parallax to enable the AI to notice that the post was moving against the background and must, therefore by close behind instead of 40 feet away. For Tesla Autopark to function safely, it needs to "know" how far back possible obstructions are. It could do so by never backing up with perfect alignment. Instead, the car should move enough from side to side to generate enough parallax effect for it to be able to determine if it is about the crash. The service manager who called to tell me that Tesla decided the accident was my fault absolutely refused to even hear my opinion on what Tesla could do to prevent such accidents in the future. He told me he had be instructed to deliver Tesla's disclaimer and to not listen to anything I might have to say. As an engineer who has patents in the area of both aviation and automotive safety, I have an opinion that might have been worthy of Tesla's hearing. I now suggest you folks warn people that Autopark should not be used to complete the parking procedure unless and until Tesla decides to address this problem. In the meantime, drivers should be cautioned to either not use the feature or to tap the brakes lightly to disengage the feature several feet before reaching the back of a parking space. The driver can then complete the maneuver manually.

Front suspension has a popping and creaking sound when driving and turning steering wheel. Safety issue as sounds something is loose but Tesla says no issue it’s the body flexing which is even scarier to hear the panels rattling if this is in fact true. Tesla will not do anything about it. Additionally the brakes and rotors squeal when pressed at slow speeds coming to full hard stop. Both front and back. Both brakes and rotors have 90% life and according to Tesla shouldn’t need replacing until 90k miles due to the regen braking. The braking system does not engage as it should or how it is designed and car has 40k miles. Tesla uses the excuse that that they don’t cover this as it’s a wear and tear component. Which another issue because they don’t know the reason of why it’s doing this. They want customers to pay and replace the brakes and rotors. On 10Jul2023, approximately 1:00pm, while traveling southbound on Hwy 69, south of Toll Road 49 in Lindale, Texas, while driving in the right lane and speed set at 75mph, when all of a sudden the car activates the right blinker and - without slowing - pulls into a fairly short, dedicated right turn lane that turns into a business. At the end of the turn lane is a drainage culvert. The route of travel did NOT include a turn and the car should have been continuing straight down the highway. The car enters the turn lane and never slows from 75mph. I immediately hit the turn signal button to use the camera to get a look to my left to see if a car is there and also look over my left shoulder when there was no response (no camera) to activating the left turn signal. This all occurs as my hand is grabbing the yoke and my foot moving to the brake. I see the blue "path indicating line" wavering left and right and then lock into a right turn into the business parking lot at the end of this dedicated turn lane. Again, there is a culvert at the end of the turn lane also. As soon as I see over my shoulder that the lane to my left is clear the car attempts to make a 90 degree right turn at 75mph. At that exact moment I am able to yank the yoke left and tap the brake to disengage the Full Self Driving Beta as the car is starting the right hand turn. The rear end of the car breaks loose and slides as I get the yoke steered into the right hand lane of the highway. I allow the speed to bleed off to get the rear tires behind me and continue on. My dog has been thrown around the back of the car, my wife banged her head on the passenger side window. This whole event occurred in mere seconds.

All of a sudden my 2 driver side windows wouldn't fully close yesterday. I reported it to Tesla via the app and they thought it was their existing safety recall due to software issue, but service rep said it wasn't. Instead the window trim off the vehicle is very fragile and typically they noticed it needs to be fixed within 12 months of ownership. This is an obvious defect that can cause many issues with flooding, theft, and more if left as is. Getting a service appointment typically takes a week currently but I stressed the issue and that I don't have covered parking so they moved up my appointment. The contact stated while driving approximately 100 MPH, as he took his foot off of the accelerator, with the vehicle brake control set to "aggressive slowdown," as the vehicle slowed the contact looked into the rearview mirror and observed white smoke issuing from the rear of the vehicle. The contact then depressed the brake pedal, and instead of slowing the vehicle accelerated at a dramatic rate and the contact lost control of the vehicle and left the roadway. The contact stated he was not aware of any warning lights being illuminated. The vehicle went into a ditch and continued straight following the ditch. The contact stated that the vehicle had not lost any speed, rather it seemed to be going faster. The vehicle crashed through a fence, drove up a slight berm, and went airborne. The contact's vehicle crashed into the ground, front end first, after flying 130 feet in the air, and began to roll end over end two to three times. The contact and his two passengers were unconscious after impact with the ground, the vehicle had come to a stop and caught on fire. The contact had attempted to exit the vehicle once he regained consciousness, however, he had difficulty since the driver's side door was jammed and his left ankle was dislocated and his right ankle was broken. The contact had managed to crawl out of the vehicle through the driver's side window which had completely shattered. The contact stated that the passenger in the front seat was not seriously injured, although he was transported to a hospital and released that same day. The passenger in the rear was transported and hospitalized with internal bleeding and a broken rib, the contact was not told which rib was broken by the passenger. The vehicle had been engulfed in fire and the contact had rolled into a ditch, a passerby was on the scene and called emergency services. The contact was transported from the scene by a rescue helicopter and flown to a trauma center. At the trauma center, the contact was treated for internal bleeding, the contact stated that 30 percent of his colon was removed, and his spleen had been ruptured. The contact had fractures of both orbits of his eyes, his nose was broken, and he had a severe concussion, with two broken ribs on the right side and one broken rib on the left of his chest. The contact stated that according to a police report and insurance investigation all the air bags had been deployed. The vehicle had burned completely and had to be lifted onto a flatbed truck and was driven to an impound lot. The fire had been so intense that the vehicle's black box was destroyed. The manufacturer had been informed of the failure. The failure mileage was approximately 8,000.

UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE
1 complaint
Today was at least the 3rd time "phantom braking" has occurred. While traveling on I-64 in Virginia, my speed was about 75mph with adaptive cruise control on. Suddenly, the car braked hard, slowing to about 40mph. I thought the car was going to stop. Fortunately, there were no vehicles behind me or there could have been a rear end collision. I did not use the automatic feature again for the rest of that drive.

UNKNOWN OR OTHER,LANE DEPARTURE,FORWARD COLLISION AVOIDANCE
1 complaint
On at least nine occasions the car has very suddenly and with no input from driver braked hard during passing another vehicle, when assuming the original lane after passing a vehicle, and, on one occasion when no other vehicle was close by. I have brought all incidents to Tesla's attention. They tell me the problem resides in the software of the car and that they "are working on it". This is a very serious problem, which has been going on for over a year in my case. A totally unacceptable and extremely dangerous situation! Occurred in December 21, 2020; Several times during the summer of 2021, other sporadic times, and most recently on September 6 and 8, 2022

SERVICE BRAKES,UNKNOWN OR OTHER
1 complaint
It is still unknown, but I believe the brakes failed. My wife was driving and making a left turn into a residential neighborhood when she applied the brake and it didn't respond, even when pushing the pedal to the floor. All that resulted was a clicking noise. The brakes failing to respond, and the emergency braking not engaging did cause an impact with a curb and sidewalk. At the same time, the seatbelt did not lock on impact or when pushing the brake pedal to the floor. The safety of the my wife was put at risk as she could not stop the car. With the cars weight and the amount of power that it has, it creates a dangerous situation for anyone around, especially in a residential area. The vehicle is available for inspection and is currently at the Tesla Service Center. There was a warning light that said something regarding the "Hold" not working and another for "Regenerative Braking." I do have the four angle dash cam videos from the Tesla of both times that it did occur.

experiencing phantom braking. On a recent trip I had several episodes of phantom braking where the vehicle slammed on the brakes without any obstacles in front of the car. Just an open road. While this has happened on two lane roads with oncoming traffic on this recent trip it happened on a freeway with nothing in front of it. There were a lot of bugs in the air and I'm wondering if a bug hitting a camera was the cause. Also, the car at a stop light gave me a heads up that the light had changed. Without looking at the lights I started to accelerate only to find out that it picked up the left and turn light not the light controlling my lane. Almost crashed into oncoming traffic.

Tesla Sudden Acceleration Accident: I ordered a Tesla Model S on June 21, 2021 and received a Tesla Model S on November 10, 2021. On Monday, February 7th, 2022, at approximately 5pm EST, my Tesla Model S failed to stop in a parking lot. I was pulling into a parking lot at Fawcett Hospital in Port Charlotte, Florida. As I was turning into the parking spot, my Tesla accelerated and drove over the parking stop, crashed into a chain-link fence, and grazed the Honda SUV of a colleague. I tried to brake, but the Tesla did not respond. When the car finally came to a stop, I looked down and saw my foot on the brake pedal. The police reviewed the situation but did not issue any citations because, luckily, no one was injured, and the Honda SUV suffered almost no damages. I had the Tesla towed to the Tesla Service Department in Fort Myers, Florida, on the following day, Tuesday, February 8th, 2022, to check the vehicle. The Tesla Service Department charged me $175 for the service. It noted that "100% acceleration was recorded at approximately 4:55pm ET on February 7, 2022 and brakes were applied within three seconds: "Time stamp Monday 2/7 between 4:30-5:00pm Customer would like inspection he statedthe car wouldn't stop after he ran over a parking block and went through a fence. Requested vehicle log data for time stamp provided by customer to investigate collision event. Uponreviewing data, on February 7 2022, @ 16:55:59 / 100% acceleration recorded and on February 72022 @ 16:56:02 Brakes applied recorded. Upon reviewing of recorded data, 100% acceleration iscause of unwanted collision." The report does not indicate why the acceleration occurred.

STEERING,LANE DEPARTURE,FORWARD COLLISION AVOIDANCE

Beta Autopilot was engaged, driver had hand on steering wheel, vehicle was traveling less than 35MPH (speed limit) and coming around a corner when it misjudged the road curve. The road curved to the left and as the car took the turn it took too wide of a turn and veered slightly off the road. Unfort the front right side of car went up and over the begining of a rock incline. The front right tire blew out and ONLY the side AIrBags deployed (both sides). The car traveled about 500 yards along the road and then turned itself off. I could not get the horn to work, nor put it in neutral. There were no warning signs before incident. In fact the I belong to a beta driving scoring rating and for that day Tesla rated me a 91/100. I called Tesla roadside assistance and they said they would not provide any help as it was viewed as an accident and not a self-driving issue. It was reported to Tesla and I have called several times to try and understand why the car didn't stay in the lane and why only the side airbags deployed. I reported this to Tesla Customer support and was now told they would not provide any information based upon their analysis, although initially I was told that it would take 5 weeks for engineering to review and get back to me. Vehicle is at repair facility and was told damage to car will be almost $28-30K. Car previously had issues with Forward Collision Warning system going off, although I was not aware there was a recall regarding this exact problem, back in October 2021

SUSPENSION,ELECTRICAL SYSTEM
1 complaint
The rear passenger side Adaptive Suspension wiring harness was either installed improperly or routed improperly and rubbed against the inside of the tire. After many miles, this harness was worn down to the point of breaking electrical contact, which puts the vehicle into a fallback state. When the dampers are disabled, the car becomes floaty and disconnected and is dangerous to drive. There were warnings in the system immediately when this broke. It is available for inspection, as Tesla has stated they cannot fix it for 6 weeks. The problem has been confirmed by their service dept via remote diagnostics. There has been no inspection by anyone other than myself, however I have extensive photographic evidence. The issue is very obvious when you know to look.

On Nov 14 at 09:01 AM while driving on the freeway with clear weather and autopilot is on for at least 15 min and and i am fully attentive and no traffic the tesla auto pilot decided to turn left and hit the cement barrier of the freeway damaging my left rear wheel then it tried to correct it fault by turning right to the right lane abruptly luckily there was no traffic coming on my right side too otherwise it would have been fatal I have the incident recorded by the sentry mode cameras and exact time and sec and the logs of the car black box proves that the tesla auto pilot was engaged at that time when it hit the cement barrier I reported this to tesla so they would take responsibility of their risky autopilot software but they dont care and told me i can report it to NHTSA it doesnt matter to them even with all the documented proof i have

STEERING,WHEELS
1 complaint
I’ve reported a continually present rack and pinion issue.. FOUR times. Tesla techs claim they don’t experience it or that it is ‘normal’ for this car. The car emits an audible grinding sound plus a visible wheel skipping when turned hard left or right. Today, the right front wheel began to emit a loud whirring sound in addition to the steering issue. Tesla refuses to investigate the issue beyond test drives. The situation could deteriorate quickly resulting in my inability to control the car while driving. It is a very SERIOUS matter.

STEERING,ELECTRICAL SYSTEM,EXTERIOR LIGHTING

My car has a « yoke » steering wheel that feels unsafe and should be illegal along with the touch turn signals and horn. I almost crashed today when I had to swerve to avoid a mother crossing the road with her child. I tapped on the honk, but unfortunately, it’s not in the middle of the steering wheel as one one would expect, but it’s a small touch button right next to the cruise control touch button. each time i need to engage it, I have to take my arms off the road and find this button. It’s very distracting, especially in emergency situations. The turn signals are also « touch » only and there’s no way to really know if you have them or not. When turning in a roundabout, the yoke steering wheel is almost upside down and trying to signal is nearly impossible. I think the yoke steering wheel and touch buttons are marketing gimmicks used to get people to talk about Tesla at the expense of the safety of the owners and other innocent users of the road. This is extremely dangerous in my opinion and I’m now officially scared to be driving my car. Please make Tesla recall these cars and retrofit a normal steering wheel with a regular horn and stalks for the turn signals. Thank you for reading you are our only hope. DRIVING ON HIGHWAY AT 65-70 MPH WITH AUTO PILOT AND SPEED CONTROL ENGAGED. WENT TO CHANGE LANES FROM LEFT TO RIGHT. PUT ON TURN SIGNAL, CANCELLED AUTO PILOT BY TURNING STEERING WHEEL TO RIGHT AND MOVED TO OPEN RIGHT LANE. THERE WERE NO VEHICLES AROUND US AT ALL. THE CAR SUDDENLY, UNEXPECTEDLY, AND VERY SHOCKINGLY SLAMMED ON THE BRAKES CAUSING ITEMS TO BE TOSSED AROUND IN THE CAR, ENGAGING ALL OF OUR SEAT BELTS WITH SOME DISCOMFORT, AND SENDING SOME LOUD ALARM OR WARNING SOUND TO GO OFF IN THE CAR. IT SCARED ME, MY WIFE AND DAUGHTER VERY MUCH. IF THERE HAD BEEN A VEHICLE IN THE LAND BEHIND US, THERE'S NO WAY THEY COULD HAVE AVOIDED HITTING US VERY VIOLENTLY AND CAUSING INJURY AND PROPERTY DAMAGE. THIS HAPPENED 2 OR 3 TIMES IN MY FORMER 2016 MODEL S, BUT I NEVER EXPECTED IT IN THIS 2021 MODEL. A GOOGLE SEARCH WILL REVEAL THE VAST NUMBER OF TESLA DRIVERS WHO HAVE HAD THE SAME OR SIMILAR EXPERIENCES.
