Car vents emit moldy smell when started , reported this to Tesla , they changed the filters but the smell still comes back, it seems other models besides S are facing it too, the technician from Tesla tells me to run high heat to kill any mold that might accumulate as condensation water from air conditioning is getting accumulated somewhere and not draining properly. This seems to me like a recall that should be done to fix it .

My AC heater stopped functioning when I was driving through sub zero temperatures in salt lake city, Utah. At that time Tesla said they will need to replace the entire system and estimated 3000 dollars. However the system started working on its own and when they inspected the vehicle, they said everything looks good with HVAC. Now after 3 weeks, AC heater has stopped working again and they are again quoting 3000 dollars to replace entire system. There are many Tesla owners who are complaining about the same issues across their models. My issues started happening again a day after a software update and I suspect that these are issues caused by their negligence that they are using to charge their customers who's warranty has expired. I have owned other cars but never had AC replacement needed after 3yrs 75K miles.

This vehicle has a well documented issue with tearing the inner side walls of the tires. I've gone through 4 sets in 30K miles, with no end in sight. What's worse it's 3 times the tire failed while I was driving at 60mph. Very dangerous

While driving suddenly in the middle of the road the car stopped with the warning Pull over immediately. Rear motor disabled. The biggest safety issue was the vehicle would not respond to any shift in gear drive , reverse or neutral. It wouldn’t move at all. I had to call 911 and cops also tried but the vehicle would not respond to any movement

This vehicle does not have a fully functional horn. To activate the horn, you must press a tiny area located in the upper right part of the yoke. The area to activate the horn is less than 1-inch wide. The horn is virtually worthless when you need it such as when someone is running a light or changing into your lane, etc. Because you can't find the horn activation area in an emergency, you are unable to warn others about dangers as you are required to do by Texas law. This has been corrected in later made Model S and they now have the horn sound by pressing anywhere in the large area in the center of the yoke. I've brought this to Tesla's attention but they refuse to repair or replace the yoke. They will do so at a cost of $2,300. Ironically, they will replace the yoke under warranty if the yoke's cover is peeling or cracking - a known defect. Cosmetic - they fix. Safety - they refuse to fix. Please consider issuing a notice to Tesla that they fix the horns on their 2022 Model S.

Vehicle: 2022 Tesla Model S (HW 3.0) System in Question: Full Self-Driving (FSD) Capability, initially purchased (Level 1 equivalent) for $6,000, and then upgraded (Level 2 equivalent) for an additional $3,000 (Total $9,000). I. Initial FSD Purchase and Unsafe Performance (Level 1) Upon initial purchase of the FSD package, the system (what Tesla later called FSD Beta, or its precursor) consistently demonstrated unsafe and aggressive behavior that necessitated immediate driver intervention to prevent a crash. Specific Safety Concern: The system would often execute turns, both in city and highway off-ramp scenarios, with excessive speed and aggressive lateral acceleration. This erratic behavior felt uncontrolled and risky, frequently causing the driver to feel unsafe and to rapidly override the system by taking manual control of the steering wheel. Resulting Risk: The system was virtually unusable for anything beyond basic highway cruising (a feature standard on many vehicles), yet its aggressive behavior posed a safety risk on the highway when attempting lane changes or taking exits. II. Upgrade and Deterioration of Practical Safety (Level 2) I upgraded to the top-tier FSD package with the hope of receiving a safer, more functional system that matched the "Full Self-Driving" name. Instead, the system introduced new safety and usability defects: Forced Disengagement/Driver Monitoring Defect: The system's driver monitoring is overly aggressive and interferes with safe, necessary driving actions. It delivers constant "nag" warnings and requires disengagement when the driver performs a brief but necessary safety check, such as looking at a side mirror, checking over the shoulder for a lane change, or briefly glancing at the car's screen to adjust climate/radio. Safety Implication: This defect forces the driver to prematurely disengage FSD in complex traffic situations to avoid a warning/lockout, which is counter-productive to safety. The contact owns a 2022 Tesla Model S. The contact stated while driving 35 MPH in inclement weather with no other vehicles nearby, the vehicle came to an abrupt stop. The contact stated that the brakes were applied without driver input. The contact stated that the emergency stop message was displayed during the failure. The contact stated that on another occasion, while driving 45 MPH, the vehicle suddenly stopped. The contact had since stopped using the Forward Collision Avoidance: Adaptive Cruise Control. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was approximately 50,320.

Tesla, in multiple cars that I own, has repeatedly changed user-preferences of user-selections, without warning, or update notes, for a variety of functions. This seems to be a consequence of sloppy software coding and seems to coincide with new software releases. In recent months, Tesla has attempted to inform people of new software update functionality with ‘release notes’. However, these notes seldom, if ever, explain that a user’s preference is getting discarded (though could be restored -- if the user were aware). One example, is the operation of ‘Traffic Aware Cruise Control’ was changed from a double click of a scroll wheel to a single click. No release nor or manual page was called out to indicate the change — at least prior to this driver’s noticing and diagnosing the problem. Nor was any pop-up message explaining the new, and surprising functionality of a ‘double-click’ — which was to momentary activate and deactivate TACC. Other functions that get changed, without notice: 1) operation of automatic windshield wipers is deactivated (and set to ‘manual) — from the user-preference of ‘auto’; 2) Auto park chimes sounds are turned off — despite a user-preference setting them to 'on'. These changes to the UI, necessitating guesswork using the large glass screen, prove to be highly distracting to a driver in the midst of driving. In addition, these undocumented and un-communicated changes lead the consumer to contact Tesla and even setup ranger service to make a repair for what ends up being a sloppy manual — to the detriment of Tesla’s effectiveness of their Service Department. If Tesla cannot make the user interface operate in a coherent manner day-to-day, there is no hope that any alleged full self driving will avoid reverting to some unknown configuration on unknown time scales. Perhaps Tesla should repurpose engineers that make video games for the car to the purpose of regression testing to avoid surprises like these.

Phantom braking continues to be a problem. I've never had it happen in the city or in traffic. I also owned a 2015 Tesla MS that NEVER had a phantom braking problem. For me PB has always occurred on a freeway with no traffic around at all. No overpass involved. Suddenly TACC thinks it must apply the brakes. Surely at least Tesla could log this kind of event and fix it. It's aggravating for me as a driver and terrifies passengers.

Dangerous Phantom Braking: this has occurred many times including very dangerous situations in high speed interstate hwy settings. The car will slam on the brakes for no apparent reason often with cars following closely behind. I have completely stopped using “full self driving” as it is a death trap and people will certainly die because of its use.

The yoke steering is not intuitive and makes it really hard to instinctively press the horn or use the directionals, without taking your eyes off the road. This presents a serious safety hazard especially in the case of an emergency, where milliseconds count to avert an accident. At a minimum the horn button should be placed on the center of the steering or be replaced with a traditional wheel. In fact, if the car were to spin out of control, the rectangular shape of the yoke would likely prevent me from regaining control of the car. When I purchased the car in 2022 a regular steering wheel was not option. Tesla is now allowing retro fits but at a significant cost to the owner. There have been several instances in the two years where i pressed the center of the yoke with no effect, which could have led to an accident. I am surprised this kind of serious hazard is still permitted to exist and I kindly ask for your immediate attention.

The car doesn’t have a conventional horn. Instead the horn is activated via a small button on the steering wheel. I don’t mean to be a pain, but I feel as though the horn is meant for emergency situations mostly. I’ve been in multiple situations now where someone has almost hit me and I tried to swerve and find the horn but given the fact I have to keep my eyes on the person swerving into me, was unable to find it in time. One of these times, the person continued to not see me and I nearly was pinned between the car and the wall of the freeway. Due to not being able to activate the horn, I had to swerve and slam on my brakes instead almost being rear ended by the car behind me. Ideally I’d love if Tesla has to recall this steering wheel for one with a mechanical horn. They have just started making the wheels with a mechanical horn again and it would be great if they received a recall so that us without them, who are put at risk, can receive them by visiting the service center and having the wheels swapped. The Tesla was driving normally as I was driving through the canyons of Utah at 65 MPH. the car gave an error about lane assist malfunction and immediately followed by steering assist malfunction. The car was very hard to turn and I nearly crashed into a concrete barrier as I forced the car to turn. I was not using auto drive

This vehicle came with a YOKE style steering wheel, I did not order it this way, it is how the new car arrived. It is TERRIBLE and I cannot believe TESLAS is allowed to put it in their cars. (This is my second Model S, my first is a 2015 and I love it). It has none of the standard stalks on the side (Directional signal, headlight flash, etc.). I heard that removing the stalks saved the company a pile of money too. It is a very uncomfortable shape to hold and control. I find myself holding it with just my THUMBS, draped over the top corners of the wheel. All the important features: left turn signal, right turn signal, horn, headlight flash, and wiper are controlled by SMALL icons illustrated on the front of the yoke. They are NOT user friendly and you have to LOOK at the wheel every time you want to use them, which takes your eyes off the road!! I have been driving this car for over a year now and I still cannot honk the horn or flash the lights in time to warn other drivers of impending trouble. Many times I end up doing the wrong thing: like flash the lights instead of honk! The turn signals are terrible, it is very hard to determine if I have even engaged the signal I want as it is a small spot to press, and it is easy to press the wrong direction signal. When backing up and having to look over your shoulder (sometimes you can not use the back-up camera), the flat top of the yoke wheel is extremely hard to hold on to while you try to move the wheel from left to right to back up safely: a round wheel is MUCH easier to handle. Also, the layout of the wheel and the monitor screen (it's now horizontal) is such that it blocks the entire lower left corner of the screen. I would also complain about the 2 scroll wheels on the yoke, they are too sensitive and control too many functions depending on how you push them, but that is mostly TESLA "high-tech" stuff which they still can't get right. GET RID OF THE YOKE STEERING WHEEL, and the small icons: it is dangerous to use.

Vehicle: 2022 Tesla Model S Tire: Goodyear ElectricDrive 2 255/45R19 104W XL Date of incident: October 15, 2025 Location: Chambers Rd., approximately 1.7 miles from Cosmopolitan Rd., Colorado Speed: Approximately 35 mph (speed limit 40 mph) Mileage on tire: Installed October 31, 2024 — approximately 10,000 miles of light local driving Description of the Problem At approximately 1:30 PM on October 15, 2025, while driving my Tesla Model S southbound on Chambers Road at around 35 mph, the driver-side front tire suddenly exploded and detached completely from the vehicle. I heard a loud thump, then saw a tire rolling ahead of me on the road. The car’s display immediately showed “tire pressure 0.” When I stopped and exited the vehicle, I found that the tire had either blown apart or peeled off the rim and separated entirely from the car. Weather was clear and around 70°F, road conditions were normal, and traffic was light. My tire pressures had been checked earlier that day and were all around 42 PSI. I was unable to retrieve the tire because I have limited mobility and use a scooter. My daughter arrived soon after but could not locate the missing tire either. The vehicle was towed by AAA to a tire shop. The technician stated that the tire appeared to have peeled off the wheel, but could not explain why. The tire was less than one year old and had very low mileage. I drive short local trips daily (around 10–15 miles) and had only taken one 700-mile trip earlier this year. Goodyear was contacted, but they declined to investigate because the tire could not be recovered. This incident could have caused a serious crash had it occurred minutes later on I-25, where I planned to drive at 75 mph. I am reporting this because I believe the tire may have suffered a manufacturing defect, and I am concerned other drivers could experience the same failure.

In the time of 2 years of ownership of this vehicle, I have had a blown out tire happen 3 separate times with these particular tires, whether it has been the rear left side or rear right side, leaving me stranded on the street.

No tires impacts and tire pressure maintained within recommended PSI (automatically remotely monitored by vehicle). Second rear tire to fail within last 500 miles of driving - failed in exact same way, and exact same place -delaminated above the DOT markings.

Rear tires have alarming wear on the inside, where the inside sidewall meets the tread. This is identical to the other complaints that are logged on NHTSA about Michelin Pilot Sport 4S rear tires on Tesla Model S Plaid cars. Tires are: Michelin Pilot Sport 4S 295/30 ZR21 102Y XL BSW. Specifically, the right rear tires loses pressure and needs to be reinflated. The left rear tire, according to inspection, actually looked worse, but is not yet losing pressure. This is impossible to see or know about without removing the tire or putting the car on a lift, since it is on the is inside sidewall/tread of the rear tire. There is high danger of a blowout because of some type of defect (camber/suspension or tire) which cannot be seen by the consumer. I would have never known about this if the tire technician had not alerted me, and probably would have had a high-speed blowout.

Michelin Pilot Sport 4S T2 295/30R/21 102Y. These tires are engineered by Michelin specifically for a Tesla Model S 2022+ hence the T2 mark. These are a second generation as the previous ones were a T1. Everything is supposed to be factored in by Michelin to cater to the vehicles specifications and needs. I bought a set of 4 of these mentioned tires brand new from Discount Tire. In less 10000 miles and 7 months the sidewalls have completely decoupled from the tread section/rest of the tire. The treadwear itself shows and even smooth normal wear pattern. Discount tire and Michelin are trying to claim this is an alignment issue citing too much camber. This is a false claim. The area of separation is not in the contact patch with the surface of the road. This is a defect in engineering and construction in the tire itself. This calls for a mass recall. I put the trust of my pregnant wife into these tires and she complained to me about a wallowing car ride with a decrease in handling, especially over bumps and on the highway. I could not see an issue. A few days later she informed me of a tire pressure indicator on the dashboard. It indicated tow low rear tires. Upon inspecting the tires I found both sides with a decoupled sidewall. Driver side being far worse than passenger. I immediately drove to Discount tire and the tire pressure sank from 26psi to 8psi in less than 4 miles. This could have proved fatal for my wife and unborn child on the highway. The tire company and the stores dont want to accept accountability. The stores just want to sell more tires. I am having them replaced at a cost of $610 per tire. Thats equaling to almost $3000 spent on tires in 7-8 months. Please investigate this matter. This is a defect. Not wear. The vehicle previously had Falken tires that lasted 24000 miles with smooth and even treadwear. No issues, no defects.

Air Suspension system throws an error. Car attempts to lift but it feels like a limit switch or some upper bound is not working properly, as the car lifts in the rear, then suddenly kicks back down before trying to lift again. This process repeats 5-6 times before stopping. Initially, I could hear a motor or compressor running for about 30 seconds while initially driving the car. Since then, I've not heard any sounds from the suspension and it consistently says that the suspension is either not available or not functional in the driver heads up screen. Tesla is forcing me to pay for this repair, it is uncertain or unclear whether this should be covered under a recall, especially considering the issues on Suspension faced in China. Tesla has ridiculous wait times at their service centers - it will take me at least until late July to get this fixed.

Severe inner tire wear on the rear tires of Tesla Model S and X. I have a brand new 2022 Model S Long Range model and started noticing heavy inner tread wear on the OEM tires at only 15k miles. The car defaults the suspension to "Low" which goes into negative camber and wears out the inside of the rear tires. This is a known problem and discussed often online. The contact owns a 2022 Tesla Model S equipped with Michelin tires, Tire Line: Pilot Sport 4S Tires, Tire Size: 295/30/ZR21, DOT Number: 4M5203DX4321. The contact stated that while driving at various speeds, TPMS warning light was illuminated. The contact took the vehicle to an independent mechanic where it was discovered that the tread inside both rear tires had severely worn to the extent that the wires inside both tires were exposed. The contact was informed that the failure could have led to a severe tire blowout. The mechanic also informed him that the failure was caused by a vehicle misalignment. The manufacturer was notified of the failure and the contact was informed that a diagnostic test was needed to determine the cause of the failure. The tires were not replaced. The failure mileage was approximately 10,000.

I was driving on the highway utilizing the full self-driving mode. There was traffic and I was behind an 18-wheeler. The 18-wheeler turned into the right lane, but because of the traffic, it couldn't completely clear the lane so the rear was slightly sticking. The Tesla stopped because it couldn't clear the lane while the 18-wheeler was sticking out. I didn't intervene because it had stopped. Then all of a sudden, it surged forward causing the Tesla's right side mirror to be hit by the 18-wheeler's back. The right side mirror's paint job was completely taken off. I didn't have time to intervene because of the sudden surge.

The hood/frunk open by itself. I was driving at around 25 MPH when hood/frunk suddenly opened. This is a serious saftey issue luckily I was able to avoid an accident as I could not see with the hood obstructing view. Vehicle will be taken to Tesla for inspection. No warning lamps or indications Also seems as if there is no secondary latch.

1) Lower drivers knee airbag panel improperly installed. Photos available. 2) This could cause malfunction of the airbag in an accident 3) No response from Tesla service when reported in the Tesla App, and in person. 4) No, but I have photos and the EPC identifies the parts in question. 5) Loose trim panel made the problem apparent, noted at delivery. Noted loose and broken lower dash trim panel under the steering Yoke. Upon investigation, noted the panel was designed to be held in place by 4 circular magnets, two on the panel and two on the lower dash support. However, in this case, a 10mm high strength steel bolt with a torque stripe was inserted into the mating magnet on the lower dash structure. The strength of the magnet was such that the bolt was apparently fully seated. The problem was, where did this bolt belong? It was found the adjacent lower panel was the knee airbag panel was designed to be secured by two 10mm bolts, and one was missing! The errant bolt was placed into the proper hole and torqued per the SM. The passenger side panels and bolts were found to be properly installed, and torque striped. I believe the placement of the Yoke makes it difficult for assembly personnel to ensure the airbag bolt is placed into the proper hole, and the magnet keeps the bolt in position when it is incorrectly placed. The problem is this leaves the lower knee airbag secured by a single bolt, and that is a potential safety concern. Please contact me for photos and supporting documents.

Windshield cracked after preheating the car and pulling out of driveway in cold weather. Saw and heard the crack happen. Found hundreds of other complaints online of owners reporting similar issues. Brought into Tesla dealer and they claim it was from an external windshield chip that then spread although no external chip was present. They seemed very knowledgeable about this issue and trained to try to not cover this under warranty.

We had the adaptive cruise control, turned on and the car braked violently on I 90 in Washington state heading east. The brakes slammed on so hard that the tires screeched. We were in the fast lane and it was fortunate that no one was directly behind us or there would’ve been a serious accident. It happened again about 5 miles later, but not as violently.

Recurring uncommanded FIRM phantom braking for no apparent reason. At risk of being hit from the rear. Driver & passengers frightened & panicked. It has occurred a number of times but cannot be reproduced at will. Vehicle inspection has not been performed. Dealer says it's a software issue. No warning lamps, messages or any other symptom prior to event. Event has recurred multiple times.

Driving midday on interstate 90 at 78 mph. The car was in cruise control. Four passengers. No other cars in front of me. Luckily there were no cars directly behind me. The car violently slammed on the brakes. My passengers thought I had hit a dog. Very scary. I am glad no one was hurt. But it is dangerous. I think the car had a recent software update. I have heard that the phantom braking issue can be worse right after a software update.

I was driving my Model S from Oregon to Texas, 14-16 Sep. The car was new, picked up on 31 Aug. I had the car in Autopilot when I was on the highway on the drive to Texas. In the first two days I had four incidents of "phantom braking" at highway speeds, as high as 80 mph. There was not a lot of traffic those first two days and the conditions were dry so there was little risk to others and the car stayed straight during the braking. However it's not hard to imagine scenarios where unexpected braking at high speeds could be a problem, such as during wet or dusty road conditions, driving into a high speed turn with other vehicles nearby, etc. The vehicle has not been inspected but the incident has been reported to the dealer and I am awaiting guidance.

The contact owns a 2022 Tesla Model S. The contact stated while the vehicle was parked and occupied, the message “Electrical System Power Reduce - Vehicle will Shut down” was displayed. The contact stated the vehicle had stopped charging and turned off once removed from the charger. The contact stated that the message “Climate Keeper Unavailable Due to System Fault” was displayed. The vehicle was towed to the dealer where it was diagnosed and determined that the battery needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The approximate failure mileage was 47,000.

The Tesla center console screen fails to reliably respond to user-touches for simple inputs concerning the HVAC system. This system failure is highly repeatable in my car, and was first noticed driving on a normal Houston street at posted limits. A typical failure pattern, is the car is being driven on normal roads with their occasional imperfections. This can lead to finger-tips touching the display screen accurately, but only very briefly, to trigger an intended input. https://youtu.be/IcQ-mAuQ4cY The screen provides active areas for input in the form of buttons, and slider user-interface elements. In the video, I tap the slider that controls fan speed. Some 7 of 12 touches to the screen simply do not produce a corresponding effect: a) changing the slider positions; nor b) changing the rate of the HVAC fan. This is a safety risk, since a second attempt to touch the screen is required, demanding additional driver focus on the display, and away from watching for traffic and correct driving. This Tesla failure is a pattern observed across several user interfaces, including the voice command system, which is highly unreliable, even in quiet settings with normal N. American speech patterns, for which the car was presumably designed. Faulty car responsiveness across multiple systems is not a coincidence. It is a sign of cultural failure at the highest levels of Tesla to make their cars safe. No warnings, in the displays, or in the Tesla manuals, suggest that this would be a problem under any condition. No inspection has been made by Tesla of this car. Apple does not have this problem, even with their earliest phones. It is unknown what positive purpose there can be to have the display software reject taps, even brief ones. Or for that matter, why Tesla would fail to indicate that user-contact is insufficient and treated as error.

The vehicle was taken to the local dealer where it was diagnosed that the control bar needed to be replaced. The vehicle was not repaired. The manufacturer was contacted but no further assistance was provided. The approximate failure mileage was 1,500.

The horn is a small button on the steering wheel which isn't easily located in an emergency. Tesla has said a software update was coming to move the horn to the center as with all other vehicles but it still hasn't done so. This could cause accidents while a driver is searching for the horn.

The horn "button" on the 2022 Model S steering "yoke" is impossible to engage consistently when the wheel is in any orientation other than straight. I have been in 5 near accidents that have required lane departures and roadway departures to avoid collisions with other vehicles because it has been impossible to find and engage the horn button without removing attention from the road and impending accident. This design essentially leaves the Model S with no operative horn in an emergency situation. There are no tactile keys to find the button in the dark, it is not illuminated, and there is no way to find it when the wheel is in a non-centered orientation. In many cases, I have inadvertently engaged wipers/washer fluid in the midst of an accident scenario, further obstructing my view and causing additional distractions from safely operating the vehicle. Please make them fix this!!!

This is regarding the turn signal controls on the 2022 Tesla Model S requiring users to take their focus off the road at best and being difficult/impossible to use in many situations at worst. In order to turn on a blinker, instead of physically actuating a stalk like on virtually all other cars, the user has to find and press at a certain place on the steering wheel. There is no physical button to feel for, so one has to look down at the steering wheel to find the button. If one tries to feel around for it, they risk the chance of initiating the wrong signal, since they're flat, capacitance buttons. I've had several incidents where I was unable to turn on my blinkers because the wheel had been rotated and the arrows were not only way out of their normal location, but they were also confusingly reversed in direction. Instead of focusing on looking for pedestrians, watching other traffic, and properly starting the turn and/or exiting the roundabout, I have to focus on finding the turn buttons and figure out which one I need to use (since they're both facing the opposite direction in that wheel orientation). Other incidents include turning on the wrong indicator when the wheel is rotation half way around, and not being able to find the indicators quickly enough to activate them in emergency situations (like when an ambulance appears and I need to quickly turn them on to pull to the side of the road). Finally, unlike physical stalks, they give no physical indication of when they turn off, so they are often left on when the computer isn't smart enough to turn them off automatically, or when I don't think they're turning off properly, I press the button again to toggle it off only to have it turn off an instant earlier, causing me to turn on the signal again and possibly confusing other drivers. I've never had any of these issues with regular turn signal stalks in other cars, and this new design does not offer any visible benefits.

Incident Description: While driving through a parking lot at 20 mi/hr, my vehicle's AEB system activated in response to another vehicle approaching from a perpendicular direction. However, the system's response was dangerously delayed and inappropriate stopping the car right in front of the other car: The system failed to provide any advance warning or alert before applying the brakes The braking occurred too late to create adequate separation from the approaching vehicle The vehicle stopped directly in the path of the oncoming car, increasing rather than decreasing collision risk The abrupt stop left my vehicle in a position where I was at greater risk of being T-boned Only the other driver's quick reaction prevented a collision. Had that driver not stopped in time, the AEB system would have contributed to, rather than prevented, a serious accident.

We took our first major road trip in our new 2022 Tesla Model S on Friday 10/14, leaving at 10:00 am for a 9 hour trip. During the trip the car experienced multiple severe incidents of the car braking hard while at highway speeds and two of the incidents almost caused an accident. The car drops 10 to 15 mph instantly for no reason. We drove the car home on Monday 10/17 without AEB, Auto Steer, FSD Beta, Emergency Lane Departure Avoidance and Obstacle-Aware Acceleration and used TACC occasionally and still had the issue but wasn't as severe. Even on the day we purchased the car (Sept 30) and drove it home (a short drive on highway 95) we had a similar incident but it was so new we weren't sure if it was us or the car but now we know it is a fault of the car and creates a very dangerous driving situation which could result in an accident or death to a passenger in our car or in the car behind us when our car decides to slam on the brakes for no apparent reason. There were no warning lamps, messages or symptoms of the problem prior to the failure. We turned in a service request to Tesla and their response was to drive the car and have it happen again and create a Bug report. So basically Tesla is asking us to put our lives on the line to help them diagnose the problem because they don't want to take the time to review the diagnostic data that is readily available to them. In our opinion, this is a very serious and dangerous problem and the car needs to be taken off the road until Tesla resolves the problem.

While driving with Traffic Aware Cruise Control engaged and set at 55 MPH the car did a hard break to an almost complete stop in the middle of the lane for no apparent reason. This occurred at least two times and both times there was no traffic in front of me. The road was a two lane highway. There was oncoming traffic and cars behind me but no cars in front of me. The breaking was so hard on both occasions and it startled me and caused me to veer off the road in the first instance . The car behind me had to break suddenly as well. While driving at approximately 5 MPH, the front passenger side window spontaneously shattered without any external or internal impact. I was alone in the vehicle and no one was seated in the front passenger seat. There were no objects in the car that could have made contact with the glass, and there was no indication of any road debris or other outside force. The window failure caused microshards of glass to scatter into the cabin, creating a potential safety hazard to the driver and any possible occupants. There were no warning lights, alerts, or symptoms before the failure occurred. The vehicle was inspected by a manufacturer service center, but the request for an engineering-level inspection was denied. The service center did not identify any external cause for the failure but still refused warranty coverage and did not escalate the issue for further analysis. The issue has not been reproduced but has not been properly investigated by the manufacturer either. Given the complete absence of impact and the sudden nature of the failure, this appears to be a glass defect or an installation issue. The broken component is available for inspection.

Car without obstacle will without warning execute a hard slowdown/stop. We have received the latest OTA update and have disabled auto steering and fsd. These unplanned stops are incredibly dangerous when driving in traffic. They charged a substantial fee to enable FSD and we are afraid to use it which sucks but the fact that basic cruise control is so dangerous is criminal.

PHANTOM BRAKING: I was driving northbound on the I-5 highway south of Sacramento, California using cruise control and traveling at the 70 MPH speed limit on September 14, 2022. At approximately 2:50 p.m., I moved from the right lane into the left lane by using the turn signal to execute the lane change. Once in the left lane, the car immediately began violently applying the brakes. There was a car behind me that appeared as if it was going to rear end me. I pressed on the accelerator pedal which allowed me to regain control over the vehicle and avoid being hit by the vehicle behind me. The weather was clear and visibility was excellent. There was no apparent reason for the emergency braking system to become operative.

Since purchase, I have experienced nine tire-related failures, including four separate blowouts at highway speed, leaving the vehicle disabled on major roads multiple times. The most recent occurred on [Insert Date], when the car suffered another blowout and is now disabled at my residence. This pattern of failure has occurred despite tire replacements and service visits by Tesla. The issue began on the day of delivery, when a tire failed within 70 miles, and service staff admitted the car had not been safety inspected prior to delivery. Tesla has replaced nearly all tires, reimbursed a third-party shop once, and confirmed on a recorded call that the level of tire failure is abnormal. Still, no root cause has been identified. The failures have occurred under normal driving conditions and pose a serious safety hazard to me, my family (including children), and others on the road. My family now refuses to ride in the car. This issue is ongoing, unresolved, and I believe there is a systemic defect involving the vehicle’s suspension, alignment, or structural components that is contributing to abnormal tire wear and catastrophic blowouts. I am requesting that NHTSA investigate this vehicle and Tesla’s CPO inspection process, and that appropriate safety action be taken before someone is seriously injured or killed.

Tonight, shortly after dusk, on a lightly traveled road near my home, the high-beams failed to function as described in the manual, or according to any alternative way to activate them. Traffic is 30 MPH with a single lanes divided by a double yellow center line with cat-eyes. This area frequently has deer and other wildlife. Accordingly, whenever possible, high beams should be activated. Conditions were dry, and clear, without oncoming traffic -- while driving at or near the posted limit. Pushing and persistently holding the high-beam light maintained high-beam only while pressing the high-beam marked part of the steering wheel. Typical operation, in the past, operated the high beam if the button (capacitative sensitive surface on the steering wheel) was held in for about a full second and then released. This is no longer how it operated tonight. Driver pushed the high-beam for a full second. Upon release the high beams reverted to normal low beams. Driver repeated this 2 more times. After doing this twice, the driver is already distracted from wildlife and starting to focus on the car -- looking for error messages of some kind. After three times, its clear that it is Tesla software that is broken. Although the car is in good repair, no messages were presented. Again, conditions were clear and dry. Failure to operate visibility mechanisms is highly contrary to safe driving. Some features which should always work: 1. Wind shield wipers; 2. Defogger 3. Defroster 4. head lights 5. high beams Hopefully, Tesla has not abandoned functional high-beams like they have with Full Self Driving. On the other hand, if FSD is no longer among the executive compensation goals, perhaps providing a visible landscape to the all-camera FSD algorithm is also superfluous.

All tesla models have excessively bright headlamps, blinding oncoming traffic. There are many teslas where I live and commute. I had many instances where I could not see what was in front of me on a two lane road due to a tesla coming in the opposite direction at night. It is seriously dangerous to have these cars with such bright lights on the road. They create a hazard for every driver in the oncoming lane. I strongly believe that these bright headlamps ought to be illegal, as they jeopardize safety of drivers.

The manual door release on the front passenger side of my car was released automatically without anyone in the seat. I was the only one in the car. I was making a turn off of an exit and went over the seam between the bridge and the street. After I went over the seam, the front passenger door manual release was triggered and opened the door. I was able to pull over the close the door immediately after. This is a route I take daily for work and haven't had this problem before. This is a safety issue should someone been sitting in that seat or if there was a car to the right of me and the door fully opened outward. This could result in someone potentially falling out of the car or an accident if someone hit the door if it had swung out. I have tried to reproduce the problem and it has not done it since 3/13/2025 at 4:27pm when the original incident occurred. I did receive an error on the screen stating the manual release button was triggered when I went over the bump. I have tried to book an appointment with Tesla to investigate, but the earliest appointment they had available was April 1, 2025.

My Tesla Model S 2022 unexpectedly shut down while parked in a covered garage on [XXX], after I had driven for an hour. This occurred after I returned from a family trip to Lima, Peru, where I did not use the car. Tesla service center diagnosed it as water damage requiring a $20,578.50 battery pack replacement, which was covered by my insurance. There were no warning signs prior to failure. Within 20 minutes of the tow truck dropping off my car at Tesla, a representative informed me it was water damage, citing a system error and claiming over 20 vehicles had the same issue. This suggests a widespread problem and Tesla's awareness of it. This incident raises serious safety concerns about a potential design flaw and the lack of a water intrusion warning system. If this occurred while driving, it could lead to a sudden loss of power and an accident. I urge the NHTSA to: - Investigate if a design flaw exists in the Model S 2022. - Investigate Tesla's awareness of this issue and their failure to implement a warning system. - Require Tesla, and all EV manufacturers, to submit water mitigation plans. - Mandate water intrusion detection and warning systems in all EVs. This incident highlights a critical safety risk for Tesla - and perhaps all EV - drivers, and I urge the NHTSA to take immediate action. The update that you required for Auto Steer has made the car undrivable. I get a written and audible warning IMMEDIATELY when I engage Auto Steer. Literally within 2 seconds. I always keep my hands on the wheel, lightly, while I drive on the highway. Anything short of a death grip on the wheel causes the warning to go off. However, the nag feature (after the changes you required) goes off constantly. Honestly, it's very distracting. If I look at the radio to change the channel, the warning goes off. You need to undo the changes you required. It makes Auto Steer less safe, not more.

Tire separation on the inside rear tires. I purchased my car new and had continued slight loss of air pressure at 12,000 miles. I inspected the rear tires and found one of the tires and begun to split open (at the seam where tread adheres to the tire), the other tire once sprayed with soap water had tiny pin hole leaks all the way around the same seam. I had tires replaced and filed a tire warranty claim. The original tires were Michelin Pilot Sport 4S and the tires I replaced them with were the same. Now at 30,343 miles the same problem occurred, both tires show separation around the inside seam with the tread still remaining. If left uncorrected/addressed the tires would eventually blow out driving.

The driver crashed into a metal gate on her property. The front bumper was damaged. There was no beeping sound while approaching the object. There were no warning lights illuminated. There were no reported injuries. The vehicle was taken to a body shop, where it was diagnosed that the bumper needed to be replaced. The rear bumper was damaged while at the body shop. The mechanic stated that the clips were missing. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The contact removed the USB and was unable to locate the incident. The manufacturer was contacted, who referred her to an approved body shop. The approximate mileage was 11,544.

The "Yoke" steering wheel does not have the horn in the center as traditional across all other brand vehicles. It's a touch-capacitive area to the right and only about the size of a thumb tack. Yesterday while my car was supercharging. Loud Arcing and electrical sounds were heard. I was inside the car with my seatbelt on as I was only planning on supercharging for about 10 minutes. Approximately 2 minutes after charging started I heard loud electrical sounds similar like when a transfer explodes. I was getting ready to get out of the car and it stop. Within 30 seconds or so my HVAC stop working and "cabin climate control system requires service-Cabin heating/cooling limited or unavailable" caution message was displayed. The cabin heating stop working completely and it was about 29 degrees f in Chicago. I went a little deeper into settings and more than 10 faults were showing relating to high voltage condition detected. I call roadside assistance for a tow and I was told I could still drive the car home which I did. Car sat overnight and faults remained and did not cleared. I had to drive in below freezing temperatures this morning to work and I had to wipe my windshield clean and it resulted in me NOT BEING ABLE TO SEE OUTSIDE DUE TO BEING COMPLETLY FOG UP. It looks like my heat pump stop working completely which shutdown the HVAC in the car. This is a safety hazard due to visibility outside car getting possibly blocked due to no A/C.

The gear shifter is unreliable without visually checking the gears on screen. It may or may not register the shift during selection. When merging from residential traffic to a main road turning right, the driver has to pull forward to check the incoming traffic. In ideal situation, if the other vehicles are approaching at speed, the driver would want shift in to reverse and backup from hanging over main road and back to the residential alley. However the inconsistencies of the gear selector make it dangerous during those occasions. The car could fail to register the shift, or cannot shift fast enough to reverse. It poses significant risks to the driver and other vehicles on the road. A front/rear cross traffic alert or a better mechanism to shift into reverse should be implemented. The mounted secondary gear selector in the center console does not light up during this cases, it it still require a visual check when defeat the purpose during emergency.

On November 1, I terminated my 3 year lease on a 2019 Tesla S for a new lease on a 2022 Tesla Model S. I was very happy with my original lease. During this week of driving I learned of the many changes that were made for this new model. So far most of them are only a matter of adjusting to these new/modified features. But some of them are definitely a safety hazard. This is the deletion of the left stalk on the steering wheel to indicate a planned change of direction and blink the appropriate turn signals. It is replaced by two “touch” buttons on the left side of the steering wheel, one for a left turn indicated by a left arrow and one below it for a right turn. Possibly one may memorize their location but until they are memorized the driver will have to take his/her eyes off the road for the few seconds to it takes to find the button. Neither of these buttons feel different by touch compared to the nearby surface. They can only be selected by looking at them. Of course if the steering wheel is not in its default position such as when the driver is making a turn, he/she has to take the eyes off the road for a few seconds. At 60 mph the car travels about 440 feet in 5 seconds. I looked up the Massachusetts Drivers Manual, on the web, about the use of the directional signals. I copied those sections below. The area of the touchscreen that one must swipe is also often occluded by the steering yoke. This has happened multiple times, where the lack of strong enough feedback (or a mechanism that allows for less error-prone input) as to D vs R allows for an unsafe movement in the opposite direction of what was intended. In one case, there was traffic approaching, and the car ended up further in the path of that traffic. In another case The car should provide instantaneous tactile, visual, and/or audio output to clearly indicate that the car has shifted from D->R or R->D. A mechanical stalk or button, as was present in previous model years, would certainly be adequate.

My new Tesla Model S, delivered 10/1/22 has frequent emergency breaking events every trip to town, approximately 25 miles on a 2 lane State highway. These breaking events involve severe emergency breaking and happen in cruse control or auto steering, (this car does not have auto driving feature). My concern is that these breaking events could cause an accident if a vehicle following me should rear-end my car because my vehicle breaks hard for no reason, without warning. I requested a service appointment from Tesla but my scheduled appointment was soon canceled by Tesla and instead, I was referred to a long written company explanation on why the breaking might be happening but nothing about when the issue might be corrected. I have a copy available if requested, and the vehicle is available for inspection upon request. Basically, the Tesla shop won’t look at the vehicle. To my knowledge, there is no authority, insurance representative, or independent service in New Mexico that would be interested in inspecting the vehicle, but I would make the vehicle available for such inspection within 100 miles of my residence. The emergency breaking event happened the first time I put the car in auto steering and again in cruse control, every time I drive the vehicle. There are no warnings lights or messages before, during, or after these events. A trip of 25 miles will have about 10 or more of these events. If cruse control or automatic steering is off and the driver has control, these events do not happen.

Tesla provides a back-up input form to its touch screen -- namely voice commands. Voice commands operate, among other things, map positioning, HVAC, lighting. Unfortunately, as quiet as the car is, the voice commands are a roulette wheel of results. 80% accuracy in detecting the specific command is the best Tesla manages. This would be a problem, by itself. However, even when the voice commands are accurately 'read back', and even when the voice command is one that Tesla explicitly has among the commands that Tesla reports (in its manual, etc.) to be an active function that produces an active result to the car's operation -- even then -- the commands/car still are unable to perform the documented function. 1) I commanded the car to 'turn on dome light'; 2) the car recognized 'turn on dome light'; 3) the car acknowledged 'dome lights on'; 4) but the car did not turn on the dome lights. The entire user interface operates in a random way at random times in random traffic. This is a recipe for distraction. Tesla needs to encourage eyes-up, on the road. Instead the multiple stages of failure, result in many unnecessary risks. The car was/is made available to Tesla's service center, and they are aware of the contents of this report. I expect zero result from their 'diagnosis'. SW release is 2022.20.8.

Steering Yoke Safety Issue: Tesla Model S Plaid cars come with a steering yoke instead of steering wheel. The steering yoke’s horn, turn signal, high beam, voice assistant, windshield wiper, and cruise control LED indicators that identify these buttons become invisible due to sun glare when the sun is behind or to the left of the driver. The glossy surface of the yoke combined with the weak LED lights of the indicators make the yoke unusable for honking the horn, make it impossible to find the horn, turn signal, windshield wiper, and other buttons, effectively making these features unusable. At a minimum, these critical safety buttons being invisible and having no tactile way to finding them during sunny days causes a significant distraction away from looking at the road. In the attached photo, you will see the effect of sun glare on the yoke while driving. This design flaw in the steering yoke/wheel puts my safety and the safety of others at risk because of inability to use the horn, windshield wipers, voice assistant, high beams, and turn signals when the sun or any other bright light causes yoke glare. Furthermore, the lack of tactile indication of where the buttons are, because they are only indicated/separated by lighting (LEDs) causes the driver to have to divert attention away from the road and onto the steering yoke to find the controls, or push parts of the yoke randomly until the correct function button is found. This is a serious safety issue which can be resolved by using a different material for the steering yoke indicator buttons, by having tactile separators/notches where the yoke buttons are so the user can find where to press during sunny days.

I have used the self driving feature of my 2022 Tesla Model S on several highway trips, most recently a round trip between Seattle and Palm Springs. The following problems with the Tesla self driving feature were noted. (My car has only the basic self driving feature, not the full self driving upgrade) 1. Random “phantom braking” where the car, without apparent reason, suddenly braked. 2. On several occasions the car abruptly swerved off toward an off ramp. I think this occurred when the white line marking the side of the road was interrupted by the off ramp where there was no painted line. 3. On several occasions when the turn signal was activated in order to change lines the car started to move over into the adjacent lane but abruptly swerved back to the original lane. I was able to maintain control but it was a frightening event. Due to the unpredictable nature of these events I greatly reduced my use of the fsd feature.

The contact owns a 2022 Tesla Model S. The contact stated while pulling out of the garage without the seat belt securely fastened, the audible seat belt reminder failed to engage. The contact stated that the vehicle was taken to a Tesla service center twice, where it was diagnosed that the software needed to be updated; however, the service center advised the contact that they were unable to update the software on his vehicle for unknown reasons. Additionally, a Tesla mobile technician came to the contact's residence to repair the failure but was unable to update the software. The vehicle was not repaired. The contact stated upon contacting the Tesla service center he had previously taken the vehicle to, to inquire whether a recall was associated to the failure, he was referred to the NHTSA.gov website. The contact became aware that the VIN was included in NHTSA Campaign Numbers: 22V045000 (Electrical System, Seat Belts) and 22V063000 (Electrical System, Forward Collision Avoidance). The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The failure mileage was approximately 400. Parts distribution disconnect.

The 3 rear-looking cameras display 3 images to the center display, when the car is in reverse. I rely on them as I pull my car into the narrow opening of my garage stall at home. The opening is for a single car, & has pillars on either side, allowing 8-9", total, clearance with mirrors folded. Even with mirrors extended, I have limited visibility to the clearance of the sides of the car, as they pass the pillars. The repeater cameras, in the front fenders, are a great support to see the garage as I back into my garage. However, the UI, immediately upon selecting 'Reverse' while near the home garage, conceals 50% of these side images from view with a pop-up rectangular user interface (UI) -- ruining any advantage I have to see the rear as I pass these obstacles. As a result, I have to dismiss the pop-up UI & avoid triggering the controls within it, by making a sliding motion on the center display, with my fingers applied to the UI. This is a problem because: 1) the UI displays the option to trigger one of my 3 garage doors, each of which have an associated homelink rectangular button. This is useless when the garage is already open. 2) I'm in reverse, so I'm trying to back-in to the garage in one motion. Adding an extra steps, by clearing this obscuring UI distracts. 3) With tight clearances added problems are: trash might lie outside the garage; items can be left in my garage stall; people could be nearby. Distractions mess this up. Tesla likely fails: a) that the garage could already be activated to be open (or even opening) -- & therefore, tends to prompt the user multiple times, while parking; b) that 3 homelink garages are at the home. They didn't budget screen real-estate to show the homelink buttons without obscuring the backup display-screens assigned to each of the 3 rear-ward facing cameras; c) that a driver prefers to back-in to the parking space, relying on views of the 3 cameras to avoid obstacles. Tesla counted on people to forward to park.
