As I was attempting to merge onto Interstate 75, I noticed that steering became unexpectedly difficult. Consequently, I decided not to enter the highway. I then crossed the highway and pulled into a parking lot, where I turned off the vehicle. Upon restarting the vehicle, the issue seemed to have resolved itself.

To Whom It May Concern, I am writing to inform you of a recurring issue I am experiencing with the power steering in my 2022 Model X Plaid. This problem began on [XXX] and has progressively worsened. Initially, the power steering would function normally for approximately 30 minutes of driving. It would work fine when starting in the morning or when the car was cool. However, after driving about 15 miles, the power steering would completely fail, and a warning message would appear indicating that extra effort was required. Recently, the power steering now fails every time I drive. Based on my research, this appears to be a common problem with Tesla vehicles, and I understand a recall has been issued for some model years, although not for the year of my vehicle. I intend to have this issue addressed and repaired through Tesla service. However, I wanted to make you aware of this problem as it seems to be a known issue that is now affecting my 2022 X Plaid as well. Thank you for your time and attention to this matter. While driving my Model X 2022 I noticed the warning "Steering assist reduced, Steering may require increased effort". I was then at a red light and I started to make a left turn and I could BARELY turn the wheel it was like it was almost locked up. Luckily I had enough strength and I was driving into a parking lot where I needed to have my car towed. Tesla service just said there was moisture in the steering wheel connector and they just cleaned it up and connected it. They test drove it and said it's fine. 4 days later I was driving it on the highway and I got the message again and I had my daughter in the car and was terrified. Luckily the steering did not lock up and just drove it home. I have messaged Tesla to see next steps, but when I look at the forums this has been happening A LOT! This is a huge safety concern and I don't feel Tesla is properly handling it based on all the reviews I have read...

I was getting a message on my display saying: Steering assist reduced Steering may require increased effort. But in reality, it requires all my strength, to turn the wheel. Very dangerous. Tesla charge to fix $4,100. I’ll have to pay it.

We bought the Tesla Model X as a CPO vehicle from Tesla on [XXX]. Within the first 2 months of driving the car we saw an an alert for “Steering assist reduced” and shortly after the steering wheel locked up making the car inoperable. This has happened twice while driving: [XXX] [XXX] This puts the driver and passengers at huge risk because the car was unable to turn while blocking a major intersection on [XXX] . We have taken in the vehicle in to Telsa Santa Barbara service center and they were unable to reproduce the issues so they did not make any fixes and returned the vehicle back in the same condition. I am writing to express a safety concern regarding the Tesla Model X. I have found that the location of the horn is difficult to identify and activate, especially during emergency situations where quick reflexes are critical. In high-stress scenarios, the horn is a vital tool for alerting other drivers and avoiding potential accidents. However, the current design of the Tesla Model X makes it challenging to quickly locate and press the horn button. This could lead to delayed response times, increasing the risk of collisions or other hazardous situations. Given the importance of this issue, I strongly urge Tesla to redesign the horn button to make it more intuitive and easier to access under duress. Additionally, I recommend that any modifications or improvements related to this concern be made available to all Tesla Model X owners free of charge. Ensuring that this safety enhancement is accessible to everyone will help protect Tesla drivers, passengers, and other road users.

I was stopped at an intersection in the left turn lane heading southeast at a busy intersection. Left turn signal was on and within 3 minutes of getting to the intersection the vehicle experiences an uncommanded full power acceleration into a very busy intersection. To avoid oncoming traffic, I veered to the right while breaking which did not stop the vehicle. Avoiding a traffic pole and continuing into a business empty parking lot which usually is not empty due to loitering in the area, and avoiding the building the vehicle continued accelerating at a high rate of speed and was stopped only because it hit a metal pole which was hidden by a large bush on the side of the business. If the pole would have not stopped the vehicle it would have continued into oncoming traffic in the north bound lane. Air bags were deployed. The only visible injury I received was burns from the airbag on my right arm and sever busing on my abdomen which has taken 4 months to resolve. The police were on the scene immediately, my insurance company representatives have seen the videos produce by myself and Tesla sent to me via email. The insurance had put a hold on the vehicle which was towed to Carparts. Tesla has never contacted me or my insurance company that I know of besides my request by email for the additional data. I have driven a Tesla model S for 7 yrs prior to purchasing the Model X. The Model X had an incident once that it stopped on its own while I was driving on an interstate. I veered to the right to get off the highway and thankful no one was behind me. There wasn't any indication that anything was wrong and may have been something corrected by an automatic update which dealt with exiting right off a highway which I was not doing but this incident never happened again.

Center steering wheel horn is not included on these vehicles, which should be considered a significant safety issue as we are trained from a young age to hit the center of the steering wheel/yoke in the event of an imminent accident. I was recently driving when another vehicle ran a stop sign, and I was barely able to avoid them; however, when hitting the "horn" to alert the other driver, it didn't work as the horn button is not in a typical location. This should be considered a significant safety risk. Elon Musk himself indicated a software update would fix this issue; however, it has not. Tesla now indicates it requires a new wheel/yoke and has not yet made it available or covered under warranty.

I nearly was struck by another vehicle that was not paying attention. I tried to sound the horn but could not activate it in time because its a tiny button than can only be activated with right hand and there is no tactile feedback. (The center of the wheel does not have a horn which is where you naturally attempt to sound the horn) This is a known design flaw of the Tesla Model X and Model S. Tesla has been aware of issue for years. Huge safety issue. Thank you.

On the latest software, Tesla cars automatically turn on air recirculation. This deprives the driver from fresh air and quickly increases the level of CO2 in the cabin. High concentrations of CO2 in a cabin can be bad for safety: 1. Cognitive Impairment: High levels of CO2 can adversely affect cognitive function, reducing decision-making abilities, focus, and alertness. 2. Health Symptoms: Symptoms like headaches, dizziness, and fatigue can be caused or exacerbated by high CO2 levels.

It have come to my attention by Tesla that NHTSA instructed Tesla to disabled auto steering for a week time, unlike in the past that it would be disabled for the rest of the drive, we have a 2023 Mercedes eqs with same system and had other cars in the past with this system, this does not exist in any other car manufacturer and you did not instruct any other manufacturers to suspend the system for a whole week if unattended, this is a gross discrimination against Tesla and grounds for a lawsuit for discrimination

at night, vehicle incorrectly identified a dog running alongside the vehicle as a human and engaged the automatic breaks causing 5 occupants to suffer minor injuries including whip lash. One occupant was a baby in a car seat. two weeks later also at night, the same event occurred. tesla has refused to provide a means to permanently disable the automatic breaking feature when a human is in control. The software incorrectly identifies a dog as a human, this is a serious software bug that has caused harm to my family. If a human is in control, I am responsible, if Tesla is in control then they should be responsible.

The auto-speed (cruise control) associated with the latest software release, 2023.27.12, is experiencing a problem that had occurred in Tesla cars a couple of years ago, namely "phantom braking". In the previous situation, the "phantom braking" was random. In this software release it is reproducible and, in my case, occurs at the same location every time I drive in that location. The car slows in a few seconds from 50 mph to 23 mph or less until I take intervention. This is dangerous in traffic. It occurs regardless of the level of autonomy selected or whether it is in Auto-Speed only or Auto-Pilot.

Driving using Full Self Driving mode, I've had at least 3 instances where, when merging onto a limited access highway, the car does not merge into the through lane at the earliest possibility, but rather goes to the end of the on-ramp before moving into traffic. At this point, I've experienced the situation where I'm steering in front of a vehicle that is overtaking me in the through lane. Furthermore, the car does not activate any turn signals; a traffic violation in New York State. (Previous versions of FSD did not exhibit any of these characteristics.) Current vehicle software is 2022.45.15, FSD version v11.3.6. (I have been unable to contact Tesla by email about this matter.)

The latest update of the Full Self Driving software, version 11.3.3, significantly decreased the safety of the car and increased violations of the driving rules. 1. The car frequently turns on turn signals near intersections without making any turns. This misleads other drivers and may lead to an accident. The problem is extremely frequent in the specified version of software. 2. The car ignores electronic speed signs installed in high-risk segments of the freeways (e.g, I-5 in Seattle, WA). Then, it breaks late approaching stopped traffic. This creates a significant risk of an accident - especially, on the back, when the car breaks aggressively. 3. The car turns to the HOV line when on Autopilot even though the “Use HOV Line” setting is switched off is the settings (e.g., on SR-167 in Washington). This has never happened on the previous versions of software. 4. The car often reads speed signs for trucks as applicable to a passenger car (e.g. on I-5 at night or in rain). This was also reproducible on the previous versions of software.

Suspension Design and Negative Camber: Rear tires have huge inner wear when wires are visible after few thousands miles. Changed tires 3 times already within 14k miles. Current miles is 18,200, but 1st time it happened in Aug 2022 when miles were 4200.

This vehicle suffered a flat tire on the left rear at 20,000 miles. It was discovered that the tire had worn out on the inside prematurely due to a faulty negative camber alignment. The other tire on the right rear was examined and found to be worn down to the treads on the inside as well. I informed Tesla and they said this is normal for the make and model. You cannot see the inside of these tires during a normal walk around inspection. Tesla says that the problem cannot be fixed. I could have suffered a tire failure at high speed which could have been catastrophic. Tesla just says its unfortunate and you have to live with this issue. i consider this to be a very unsafe issue that Tesla should address at their expense. This car is under warranty and Tesla refused to repair the car.Tesla did not divulge this issue to me prior to purchasing the vehicle.

My car is been vibrating and shaking for over a year now, and everytime I took it to certified Tesla service, they brought it back to me telling me they can't feel anything, and that there is noting to fix. After the fifth time, they suddenly tells me that I got into a hole in the road and that is why it's shaking, also, the car had a message on the screen saying that the air pressure can't lift the vehicle, and it made a huge noise, I had to call a tow truck to take it to Tesla center. NOW, they are telling me that a mouse ate the hose, and that will cost me $9000. I can't pay that much money, and it's not OK. Many newer models of Teslas do not include a horn in the middle of the steering wheel. Instead there is a small button on the left side which is impossible to find to warn others of a possible collision! The driver gets distracted trying to find the horn and makes the situation even worse by not reacting quick enough. They released a new version of the wheel with a horn but those who are out of warranty have to pay a large amount for a basic yet hugs safety oversight. They should be replaced on with recall free of charge.

On Tesla's site, it says to report safety concerns to you vs to Tesla so that's what I'm doing since Tesla doesn't have any options to write to them about these problems. 1) I got into a parking lot accident on 7/7/2023 because of the "slide bar" on the Tesla Model X. I have had problems with it and complained to people about it before. If your hand has lotion on it, when you "scroll" up with your finger, it doesn't always "take". That's what happened earlier on 7/7 as well as in the parking lot. I scrolled to go forward, but it didn't scroll so when I hit the "gas" (its electric), it went backwards into the car in back of me. What if the car behind me was a person instead - that would have been awful. Tesla should fix this. 2) Tesla's horn is a small button on the right side of the 1/2 steering wheel. You can't even reach it with your right fingers when you are driving. If your right hand is not available, it is almost impossible to use your left finger to push exactly that small button for the horn. I also complained to Tesla about this safety issue when I had another problem with the car. Previously, someone cut me off and I couldn't even warn them because my right finger couldn't push exactly that small button. Another time, my right hand was occupied and I was just jabbing at the right side of the steering wheel to try to get the horn to sound, which it didn't. Thank you for listening.

Tesla has moved the horn button to be a small touch area within the steering wheel that can be used with the thumb in the right location. I was making a left turn, and a car ran the red light, I was trying to hit the horn and was not able to locate it with my thumb in time. The horn needs to be either made bigger or moved back to the center under the airbag, like all other cars.

When installing front-facing car seats for children in the 2nd row seats of the 2022 Tesla Model X, there is not a safe way to attach the tether strap. While there are LATCH anchors, and a place to attach the tether clip, the strap for the tether cannot pass under or over the fixed headrest, it must pass over the “shoulder” of the seat. This is not effective as no matter how tight a person cinches the tether strap, it slides off the edge of the seat, leaving no way to safely attach the required tether for child safety seats. The owner’s manual displays the seat as if there is a detent that would prevent the strap from sliding off, but this does not exist in my vehicle. When I asked at the local Tesla sales/service center, they said that is simply how it is designed.

The contact owns a 2022 Tesla Model X. The contact stated that the car seat sensor was not functioning properly. The vehicle was taken to the dealer for a diagnostic test. The contact was informed that the vehicle failed to detect aftermarket car seats. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was 9,000.

Was driving on the freeway and drivers seat started moving backwards. Feet slipped off the pedal. Turns out the seat adjust button behind the 2nd row seat controls also moves the drivers seat. NOTHING should move the drivers seat WHILE DRIVING except driver himself.

The contact owns a 2022 Tesla Model X. The contact received notification of NHTSA Campaign Number: 23V085000 (Steering, Electrical System) however, the part to do the recall repair was unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure. Parts distribution disconnect.

In all late 2021 and 2022 Model S and X cars from Tesla, the manufacturer removed the left and right stalks for a) directional signals b) wiper controls c) forward/reverse/park controls and also made it nearly impossible to use the horn, with only a tiny button to sound the horn. The driver is forced to remove their eyes from the road to a) turn on left or right directional signals b) choose forward or reverse c) turn on window washers d) turn on wipers when they fail to turn on automatically and e) press the horn. AT NO TIME SHOULD A DRIVER HAVE TO REMOVE THEIR EYES FROM THE ROAD TO DO THESE FUNCTIONS. Traditionally, any driver could use a left or right hand without removing eyes from the road and use any such control. This is not possible.

The 2021-2022 model X “yoke” steering wheel is poorly conceived and a safety hazard. 1) horn location is tiny, and moves as wheel turns, impossible to quickly activate horn in emergency situations 2) high Beam lights are on a tiny button which also moves and cannot be activated when wheel turns or in an emergency situation, or even when choosing to cross an intersection during a yellow light because a car behind you is tailgating. 3) wheel shape: when used at lower speeds in autopilot Beta mode, the electronics literally rip the wheel out of your hands when turning right on residential streets. It is impossible to maintain hands on the wheel without risk of personal injury during some autopilot beta turns. Safety of the vehicle would be returned if the prior 2017-2020 round wheel with turn signal, wiper control, and gear selector stalks returned. The inability for the driver to control following distance in the ADAS (cruise control) systems without dropping 3 levels down into on screen menus is also unsafe.

The wing-style door of the car closed while my 1-year-old daughter's leg was sticking out. It dragged her leg sideways as it closed, and he femur broke. None of the sensors worked, the door should have detected motion or pressure, but they did not and it resulted in my daughter's leg breaking.

The front cover (frunk) keep opening even when the car moving , which is easily can block the ability to see cars infringe of you , I tried to reach the dealer services and they told me that they are so busy and it will take time to fix my car , the problem that I have to drive the car to go work and the dealer would not give a me a sooner time to fix this issue , it is a serious problem and can easily cause car accidents , I try to minimize the use of my car but I still have to show up for work . The front cover keep opening 8-12 times a day even if the car is closed and locked !!! But she I saw that it opens while your driving I think this is a serious issue and it has to be reported and the dealer need to take car if this issue ASAP, it is a brand new car and it should not have these problems , this car spent More than 30 days at the service for different problems ( paint, seat belt , falcon wing that doest close ) and now this issue!!!

The passenger side back door does not open consistently leading to loss of egress. Although the other doors do function regularly, removing a child from a car seat is nearly impossible when accessing from another door. The door also struggles to close consistently, and has closed on my son in the past. The inconsistency of a functioning falcon wing door is a safety risk and it continues to worsen despite many attempts to repair the sensors. The Tesla service center has attempted to repair the door and sensors 4 times without success. The most recent service appointment they stated the door was not opening correctly because it was getting stuck in “indoor mode” sensing something over the top of the car. They replaced the sensor but the problem has not improved. I have notified Tesla of the ongoing issue multiple times without any response, even via certified mail with proof of delivery. There are no warning lamps, beeps or any notifications showing an error in door function.

Tesla Model X all doors are 100% automatic which means that they cannot be manually opened from the outside if the car software is not working. I have been facing a lot of issues with the Falcon doors and to my surprise a lot of other Tesla X users on Reddit face the same issue. So, when you press the open button on your app to open the rear falcon doors it might not open at all and just make a beep sound. It's hard to replicate this issue but I shot a video showing how it happens. This is not the first time that it had happened with my car. I have had this happen multiple times to my car. Every time the service center would simply change the door sensor which would fix the issue but temporarily. I think it a big safety issue as I have 2 toddlers who occupy the rear row in their car seats. I cannot get them out of the car when such issue arises. I have done a workaround to make it work which takes at least 2 minutes to work properly. A lot can go wrong in 2 minutes with kids. This issue happens very randomly and there is no prior warning. Earlier tesla told me that it had to do something with the climate. If it's too hot outside then the door sensors might get fried up, which I think is not completely true. I have heard different explanations from their different technician. Well yesterday when it happened, it was 34F and windy. I was outside of the car in this cold weather trying to get my daughter out of the car and figuring out how to open these damn falcon doors.

As I was trying to make a turn into the parking lot of a convenient store, my Tesla Model X started accelerating suddenly and unintendedly without my foot being on the accelerator. I kept pressing on the brake to stop the car, but it would not stop. I had no control and while it accelerated for 15-20 yards eventually it hit two other cars that were parked there. This happened so quickly. We all were terrified and shocked as I could not stop it from accelerating. I kept yelling "it is not stopping" to the person seating on the passenger side. I am confident that I did NOT press the accelerator pedal. The car was not on autopilot. My initial speed was probably 5 miles/hour, but it accelerated so suddenly that four airbags deployed, and the car is not drivable. Insurance company is evaluating whether the car is repairable, but I am afraid of driving with this car again until the problem is found and fixed. This could have turned to a major tragedy.

The new stockless button controls on the Tesla are entirely unsafe. The buttons themselves are extremely unreliable and don't always respond, and the placement on the steering wheel requires you to take your eyes off the road in order to locate the correct button. I was recently driving on the freeway with my wife and kids in the car, it had been raining earlier and the roads were wet. A car in the opposite fast lane hit a puddle which sent wave of water over the center divider. It covered our windshield to the point where I couldn't see. With previous cars, including previous Teslas, it was easy to quickly tap the stalk and engage the wipers, but now with this new button system I had to look down at the steering wheel to find the button to quickly engage the wipers. From the surprisingly blinding wave and the time it took to clear the windshield, I veered into the next lane and almost hit the car next to me. It was an eye-opening experience with a car I've really enjoyed owning. The buttons are terrible, and a deviation from a universal, internationally accepted system that was not broke, did not require fixing, and has created a safety hazard.

Critical Safety Issue: Malfunctioning Wipers in Both Auto and Manual Modes. Experiencing nearly catastrophic situations due to faulty wipers during heavy fog and rain at nighttime while driving at high speeds is a severe safety concern. Immediate attention is required, especially with the impending rainy season. Tesla Service's previous attempts to resolve the issue have been unsuccessful. 1. Auto Wipers Malfunction. - Historical Performance: The auto-wiper functionality has consistently performed inadequately. - Current Issue: As of the latest software update (2023.7.30), the auto wipers frequently fail to activate throughout the duration of trips in heavy rain. - Case Instances: This issue was observed on August 29, 2023, at 8:04 pm, 8:08 pm, and 8:09 pm, resulting in zero visibility through the windshield as the wipers did not activate even once when set to Auto. 2. Manual Wipers Deficiency. - Current Issue: Occasionally, pressing the wiper button on the yoke yields neither action nor haptic feedback, sometimes lasting for a few minutes. - Case Instances: This problem was replicated on multiple occasions, including January 8, 2023, at 10:39:33 pm, and July 25, 2023, around 6:57 pm when other buttons on the yoke also failed to respond. Despite yoke replacement on January 6, 2023, the issue persists. 3. Wiper Speed Options. - Having just wiper speed settings (Off, I, II, and III) is insufficient for safely navigating varying rain intensities. This issue is further compounded by a faulty yoke button responsible for initiating the wiper speed change. Conclusion. Simultaneous failure of both manual and automatic wiper systems results in zero visibility, posing an extremely high risk of vehicular accidents. Immediate action is imperative.

I was driving on the road send out of no where, While driving the wheel started to shake and felt I was losing control. I I started to Press down on the brake in my car would not slow down as fast as it normally would and I felt that I was pretty much driving on metal. Because I had no control due to the wheel on the passenger front side my car was going in and out of the gravel on the road where I almost flew into a ditch. I got the car to stop And noticed the full rim split completely in half. My safety was at risk because I had a toddler in the car with me as well and something like this should never happen to a brand new car rim and tire

I was a passenger in the vehicle. The car was moving forward at 10-15 mph and the auto braking system was activated appropriately when a large truck parked on the side of the street driver's door opened unexpectedly and was in the path of our vehicle. At around 9:10 p.m. on 7/26/2023, I was driving 2 guests on the I-5 highway from Seattle to the airport. The car’s automatic driving assistance system suddenly failed to work. Tip 1: The steering assistance effect has been reduced 2: Lane departure avoidance function is not available; At this time, the steering wheel suddenly couldn't turn, and I felt locked tightly, unable to change lanes, so I had to slowly reduce the speed, turn very, very hard to change lanes, from the innermost carpool lane to the outermost exit lane, slowly The driver controlled the steering wheel hard, and it was impossible to drive on the deceleration road when getting off the high speed. When the car automatically started to "precondition" for supercharging while driving to the supercharger location, the binnacle lit up like a Christmas tree. Warnings included, autopilot unavailable, brakes faulty, navigation unavailable and low tire pressure. In addition, all camera related functions such as back up camera and blind spot warning froze. The behavior has repeated itself without warning shortly after I supercharge the car each and every time.

Traveling south on I 5in CA on 2/27, weather and road were clear, speed 70 MPH, the car suddenly ceased to maintain it's set speed and decelerated rapidly. at 70 MPH with vehicles right behind me closely following, this is safety hazard. Same problem on return trip home northbound on CA 101 - destination Brentwood, CA. Third occurrence was on CA 246 - same problems but at 60 MPH.

Travelling at highway speed with cruise control on, the brakes came on hard as if there was an upcoming collision, slowing the car by 25-30 mph. But there was no car or obstacle ahead of mine. I was able to immediately accelerate back to speed without further incident. And fortunately, the cars behind me were able to slow down without hitting me. This is the third (3rd) time this has happened. The previous was less than two weeks ago on city streets without cruise control and I don't remember the first which happened shortly after getting the car in March 2022. I noticed that others have had a similar experience, which led me to submit this report.

Vehicle repeatedly applied the brakes while on cruise control. This sometimes seemed to be a response to other vehicles approaching in the oncoming traffic lane, but sometimes seemed totally unexpected. Once, the vehicle even sounded the collision alarm when no collision danger was present. If someone had been following me closely, the inexplicable braking could have caused an accident. A search of Tesla forums shows this is a common problem with these vehicles.

My 2022 Tesla Model X has two recurring safety-critical defects that remain unresolved despite multiple repair attempts. 1. Airbag / Restraint System Failure (RCM2 / OCS Defect): The vehicle frequently misclassifies the front passenger seat. It shows a passenger when the seat is empty and activates passenger seatbelt warnings incorrectly. Recently the vehicle displayed: “Front passenger safety restraint system issue: Service required (RCM2_a064).” Tesla attempted to repair an electrical harness in the seat, but the problem returned. A Tesla technician documented that the seat may still need a new occupancy sensor. Because this affects the Occupant Classification System and airbag deployment logic, it raises potential FMVSS 208 compliance concerns. 2. FSD / GPS Safety Hazard (5 Unsuccessful Repair Attempts): Across five service visits, Tesla attempted to fix GPS drift and unsafe FSD routing. The vehicle still attempts incorrect turns, performs premature lane changes, and misinterprets intersections. One recent incident involved FSD attempting a left turn one intersection early, nearly causing a collision. Tesla replaced the GPS antenna, recalibrated cameras, and replaced the computer, but the unsafe behavior continues. These two defects—incorrect airbag/OCS behavior and hazardous automated-driving/navigation behavior—create a significant safety risk to occupants and other drivers. The vehicle presents a compounded safety defect creating a risk of passenger entrapment and high-voltage fire. 1. High Voltage Manufacturing Defect: On September 3, 2024, authorized Tesla Service technicians diagnosed a critical safety fault. The service record states technicians 'found A/C compressor HV harness was incorrectly routed, rubbing against the steering column' and 'found sand inside the connector'. Tesla repaired this defects under the 'Basic Vehicle Limited Warranty,' confirming it was a factory assembly error. This defect created an imminent risk of a high-voltage short to the steering column, which could result in driver electrocution or total vehicle power loss at speed. 2. Passenger Entrapment: The vehicle has a documented history of Rear Falcon Wing Door failure (7 repair attempts from April 2023 to May 2025). The doors frequently fail to latch or open. On July 8, 2025, the Left Rear Door failed to open due to a sensor fault. 3. Combined Hazard: If the incorrectly routed High Voltage harness causes a vehicle power failure, the rear doors' electronic latches will fail. Because the doors have a history of mechanical jamming, passengers in the rear seats would be trapped in a vehicle with a compromised High Voltage system.

I had my two Teslas parked in the garage. The model X was charging at about 1:30 AM. We heard explosions coming out of the garage. Went to check only to see my model X in flames. We evacuated and called 911 eventually a fire investigation report was made Showing the fire started within the vehicle outward. The battery pack was on fire. We had lithium contamination all over the house and this fire endangered five lives. This incident has been two years ago and until now Tesla refuses to acknowledge the faulty battery and is refusing to do anything about it

On September, 21 2024 I scheduled an appointment with the Tesla Service Center for the “Front Row Curtain Airbag” recall to be performed. October 30th, I took my 2022 Model X Plaid to the Tesla Service Center for the recall work to be performed. During the repair, their service technician BROKE-OFF an anchor nut that secures the airbag to the A-pillar. The advisor said in order to “fix” it they would need to take the car to a body shop to have a “new” anchor nut welded onto the A-pillar. I was told this repair would require cutting open the A-pillar to access the location where the nut should be welded. Cutting the A-pillar will compromise the structural integrity of the vehicle's roof. And retrofitting a new mounting point for the side curtain airbag will compromise the effectiveness of the airbag in the event of an accident. The manufacturer's airbag recall "fix" does not involve cutting open the A-pillar to weld in a new mounting point for the vehicle's airbag. Performing the airbag recall in this manner creates a greater safety issue for my vehicle. Therefore, this "recall repair" should not be allowed to be performed on my vehicle.

My Tesla stopped in the middle of a 4 way intersection without notice. As I was turning left, the car started to shake for about 2 seconds and then stopped. No warning or anything. The notification stated "electrical system unable to support functions." My safety was put at risk and others. I was traveling with my 11 month old and other cars could have hit my car if they were not paying attention when coming through the intersection. The car was towed to the Tesla dealership. No warnings prior to the failure. Warnings appeared after the car stopped.

when applying the brakes the steering wheel vibrates and car has reduced stopping power. when I took it to Tesla service center they said the "rotor runout is out of spec" the car has 25k miles and new "performance pads" were installed by Tesla as an upgrade in December 2023. the shaking developed after this. Tesla says the rotors are a wearable part and not covered under warranty.

The contact stated that after five minutes the seat belt was released. The contact then stated that while driving at approximately 20 MPH, with the front passenger and the driver’s side rear seats occupied, the restraint system fault for each passenger was displayed. Additionally, the contact stated that on several occasions while driving at various speeds, the messages appeared intermittently while both seats were unoccupied. The vehicle was taken to a dealer where it was diagnosed that a seat belt module needed to be replaced. The vehicle was repaired but the failure persisted. The manufacturer was notified of the failure. The failure mileage was approximately 2,000.

A couple of days ago, when I was dropping off my child at school, I parked in a spot that required parallel parking to save time as I was in a rush. After reversing into the spot, I just needed to move forward a bit by 1-2 feet. I naturally slide the gear shift up on the screen. Unfortunately, it didn't slide into place, and the car was still in reverse (R), but I didn't realize it. I was already very close to the car behind me. When I pressed the accelerator, the car continued to back up. Luckily, I slammed the brake, avoiding a collision. Just wanted to share that even after a year and a half of driving a Model X, mistakes can still happen. After reading what happened to [XXX] , I posted what happened to me on a social platform and someone else shared they had a similar experience that needed up with a crash. So I decided to report mine and encouraged them to do the same. I like the minimalist design but I need to stay alive to enjoy it. As Tesla is moving away from stalkless design in more models, I hope they can do something to make the current shift more noticeable. The horn is a small button on the steering wheel instead of the larger pad in the center of the steering wheel. When making turns or complex maneuvers the horn is not easily found since the steering device is turning. This is a safety issue that needs to be addressed.

I continue to experience accelerated severe rear tire wear since my original purchase of this car new in February 2022 on both rear tires (275/45R20). The car has had the alignment specifications confirmed by Tesla. Yet I have had to replace two sets of rear tires in less than 36,000 miles due to the inside edge of both tires being worn through the rubber and into the steel cords of the tires. This last set of tires went ~14,000 miles and still had 5/32" tread depth remaining across all treads. This substantial wear is not readily observed by the vehicle operator without crawling under the car. The wear on these tires is indicative of excessive negative camber settings of the suspension. However, TESLA states the rear suspension alignment is within camber specifications at -2.13 to -2.06 degrees. The tires appear road worthy during a casual walk around of the vehicle. I have been fortunate these tires have not experienced a blow out, which at highway speed could have resulted in loss of control of the vehicle. I have retained both removed rear tires. Repair orders, alignment details and picture of excessively worn tire are attached.

Ever since a recent software update, when turn signal is activated, right fender camera feed appears on screen but is completely black instead of showing a visual of the right blind spot. Due to this other features such as Autopilot, Automatic Emergency Braking, and likely most of the ADAS functions are also disabled. Due to the vehicle's poor natural visibility, there is a high reliance on the camera systems functioning properly. Not having a blind spot camera endangers turns and lane changes, especially if one has become accustomed to their usage. With most if not all the ADAS features disabled, we are returning to a 1990's level of auto safety. The problem remains unresolved despite a recent 2 week stint in Tesla service. Since this is most likely a software issue (possibly a regression introduced with the recent rear camera recall fix) there is no recourse except for Tesla to issue a bug fix. The car was returned to me without resolution. I'm not the only party experiencing this problem. It seems the only constant is it's a problem with the fender cameras however whether it's the right or left side is variable. My vehicle does not have a horn in the center. It is a button. Elon Musk told the public over and over they all had a center horn that just wasn’t activated yet, that would enable it in a future software update and that was a lie. This button horn on the right hand side never functions when needed. There is no “cover the entire right side” with your hand to make it honk when you’re quickly reacting to a safety hazard. It sets off the wipers usually without even honking. I don’t mind the weird turn signals, but this needs to be fixed. Now that they’ve announced a new model with a regular horn and lied to us about our cars actually having them but being “disabled” I think we are entitled to a retrofit of a brand new one with a center horn like every other vehicle on the road. Rear cross traffic alert? What real cross traffic alert? After I bought this car I was absolutely floored to find that when I back up, it doesn’t alert or beep or make any sound if a car is coming or a human is walking behind it. I was inches from hitting a neighbors child on a bike once and I am still shaken from what could have happened had I not heard someone yell. The car will scream and beep and flip out at every little other issue but doesn’t have this? Every car I’ve had since 2008 has had this feature. It is absolutely unacceptable that this noisy beeping computer car doesn’t have any audible rear cross traffic alerts. Please force them to fix this. It is DANGEROUS. The rear window is so high up it’s impossible to see beneath it. If I’d hit that child, I never would have forgiven myself. Please force them to add an audible warning.

Bran new Tesla Model X 2022 model went up in flames in my garage on it is own, burning my garage, and destroying my other tesla. the car melted completely and Tesla Warranty is no where to be found, they are not even talking to me after 8 weeks of the incident and as of this date filing this complaint. The Car was purchased directly from Tesla, Brand New, owned it less than 14 months and had less than 30,000 miles on it.

I sometimes hear a creaking noise from under my car when driving and steering at low speed and believe there is a potential defect that poses a risk to motor vehicle safety. I took my car into Tesla but they said they would not look at the car unless they could first hear a noise which is I told them is intermittent. They could nto hear a noise at the time, so did nto look at it. I have on occasion heard the noise again. My car is a year old and had around 5.1k miles when I took it in. It now has around 7,500 miles. I have never had a dealer refuse to put the car on a lift, especially a new car that is under warranty, to examine it based on a complaint from a customer. My concern is that I had read articles of reports to to NHTSA of Tesla suspension breakdown and wheels failing off of Tesla's on highways that have caused serious injuries to passengers and other vehicles and I am concerned something like that could happen to my car.

My 2022 Tesla Model X is equipped with yoke steering wheel with capacitive turn signal buttons integrated, and there's no turn signal stalk. While I like the simplicity, the turn signal buttons sometimes do not respond. It happened about 10x in the past 12 months / 10k miles of ownership. I took the vehicle to Tesla service center twice in the last a few weeks and they couldn't do anything other than a customer paid diagnostics. The most recent occurrence was 9/17. Here's what I put in my service request. "I was about to move to the right lane, but right signal button did not respond for 10-15 seconds. In the meantime, left signal button worked and I was able to activate it twice just to confirm it wasn't my thumb being wet of something else, but the right signal button would not respond at all. I wanted to record a video but it wasn't safe to do so. I captured a dashcam recording in case that helps." Here's the service result on my invoice "Unable to duplicate concerns present on steering yoke. Checked vehicle logs and no hardware related concerns or abnormalities identified. Performed inspection in-house and verified proper operation." (I was charged $70.5 for the diagnoses) The same also happened on 9/4. I put in the service note "I pressed on the left turn buttons about 5 times when the first 4 times it didn’t respond. I’ve capture a dashcam video to record the exact time in case that helps." Here's the service result on my invoice "Unable to verify the customer concern, diagnosed and pulled car logs and customers time stamp. Reviewed data with the lead technician, and found no issues or cracked alerts during the time of when the button was operated. unable to replicate concern. Buttons are working as designed. No repairs needed at this time." This imposes road safety issues for me and other drivers, especially when I have to change lanes on busy roads.

While driving on an 80 MPH speed limit, 2-lane interstate in a rural area, the vehicle's "traffic aware" cruise control was enabled. The driver attempted to pass a semi-truck on left while the road was curving to the left. The vehicle's emergency lane keep assist appeared to engage causing the vehicle to swerve toward the semi-truck. The driver corrected back to the left, when doing so the power steering system failed causing the driver to apply significant force to the steering to avoid a collision. For the next 12 miles of driving the power steering never engaged and turning required significant force. While charging at a supercharger the error on the dashboard stating that power steering was not available persisted. Upon departure the error message went away and the power steering worked again. The manufacturer was contacted as the vehicle was under warranty. It was determined that no repair was needed after the power steering once again began to work, but the failure created a significant risk to the five vehicle occupants at the time of failure.

I was driving my Tesla model X in interstate I35 with FSD turned on. After 45 minutes of FSD, I felt the car slowed down. I immediately push the accelerator to speed it up but it didn't help and the car quickly fully stopped. I again pushed accelerator but the car won't move at all and my car block one of the two lanes of north bound I35 in Gainesville area. I was forced to reboot the car as the car won't move. Several minutes later, the car finally rebooted successfully and I immediately drove to exit I35. As I had no idea why the car stopped itself so I didn't feel to continue driving it and had the car towed back. From the trip data on my phone, it shows "hard breaking" prior to all this abnormal happened. I am sure that no break panel was pushed as my foot is on the floor mat as the car was driven by the FSD. The most serious problem is I couldn't speed it up after the car "hard breaking" itself and I had to reboot the car in order to make the car moving. I have the car service by Tesla but they don't have any explanation for me yet. It will be difficult to reproduce the problem. It can only be found by Tesla by looking into the stored driving data.

In April, 10 2023 approximately 6:10 PM I had an traffic accident. My vehicle was parked at paid parking spot next to the pedestrian road (street parking) and I was approached from the back of the car and I recognized the bus approached from my backside of the road. I have no idea somehow, the front driver side door suddenly opened when I approached to my car about 4-5 feet away from the trunk, even though I did not pressed any door open button in my Tesla smartphone app. As a result, the opened door had collided to the LA Metro bus backdoor. I would like to NHTSA will investigate this faulty safety issue. Thank you.

Several times, while traveling north on the I15 express lanes in Rancho Bernardo, CA, with the full self driving beta software engaged, the Tesla unexpectedly brakes when going under the overpass located at the George Cook Expressway exit. I must keep my foot over the accelerator pedal so that I can override the braking, if necessary. It appears to happen about 50% of the time.

The contact stated that after his wife had entered the parked vehicle and the driver's door was still open, the vehicle experienced unintended acceleration, causing the vehicle to drive over a curb, into a ravine, and then crash into a tree. No warning lights were illuminated. A police report was not filed due to the contact's wife not sustaining injuries. The vehicle was towed to a local dealer; however, the vehicle was not diagnosed or repaired and was then towed to an independent mechanic, where it remained awaiting diagnosis and repairs. The manufacturer was made aware of the failure. The approximate failure mileage was 200. The contact noted that the actual failure mileage was 1,646.

This happened with my new Tesla Model X Plaid with installed Self Drive software. The AUTOPILOT or SELF DRIVE mode was NOT enabled when the incident happened. Turning into a parking spot at a Target superstore, (next to a damaged at the rear end car, parked to the left of the spot I was pulling into), the car went out of control with unintended acceleration as if the car's Emergency Evasion system got activated unintentionally. It went over the curb and made a turn on it's own on the grassed divider and came to a stop on it's own, while I was trying to apply breaks all along. It caused damage to other cars but fortunately no people were harmed. This incident happened end of July of 2022 and I have since repeatedly tried to get answers from Tesla. There have been multiple other incidents of unintended acceleration reported by other drivers of Tesla with two incidents resulting in loss of lives. My car finally got repaired and has been released to me saying they have tested and found it safe to drive although I was told with regards to my incident "the engineering case is still open with no resolution at this point". I had requested a master technician to check my car but that has not been arranged yet. I have repeatedly contacted the local Tesla service center manager who has not been able to get an answer from her team with regards to the cause of the problem but at the same time was told that the current issues do not warrant repurchase of the car like I had requested.

2022 Model X plaid is exhibiting “ Phantom Braking” while in autopilot mode using navigation. Vehicle abruptly brakes while driving on highway as navigation appears to become confused and thinks it is on a side street. Vehicle goes from high speeds of 70-80 mph, down to 30-40 mph. If was being followed closely, car behind Tesla would have collided with rear of vehicle. This has occurred 10+ times since purchased vehicle in February. Vehicle has just under 6000 mi. as of today. I have times/dates to include a video of one incident. I have brought it to Tesla for service on 2 separate occasions, to no avail. They say it needs the latest upgrade, which has not resolved issue.

Warning alerts that car "unable to charge", "acceleration and top speed reduced", "vehicle may not restart", "regenerative braking features temporarily reduced" appeared on less than 2 week old car with less than 700 miles. Car has been in service center for 3 weeks now. Tesla claims they need to change the high voltage battery pack but have not yet been able to locate a replacement. Service advisor at local Tesla Svc center told me there are several other Tesla cars on the service lot with similar issues with high voltage battery packs and are all awaiting replacement packs.

Trying to secure a backless child safety booster seat which has latch tether devices. The Tesla model X 2022 3rd row does not have the latch tether bars. I believe the brand of seat is GoFit Chicco. My 7 year old granddaughter was only restrained with the shoulder lap belt. During a forced rapid deceleration emergency stop she was choked by the seatbelt when the booster seat slid off the seat during rapid deceleration. She was not injured other than a red mark, sorry no photos, and was significantly traumatized from the incident. This booster seat has a clear warning stating the latch system is required and does not have a built in seatbelt channel since it’s backless. The 2nd row seats were in use with two other child safety seats using the latch system which provided ample protection.

While driving, our tire lost pressure resulting in the tread and side wall of the tire to separate. This failure that occurred is the same defect reported on the Continental CrossContact LX Sport recalls. It was a huge safety issue as we were driving on interstate and had to safely get to other side and pull over. Tire size - 265/45 R20

I ordered a new Tesla X on 01/10/2021 and I just received it on 06/03/2022. I can’t believe that the car passed quality inspection (I found that the passenger falcon door can’t close unless I override the door to close from the driver seat and non of the closing bottoms works in addition to safety seat built is not even attached to it seat!! These issues should be fixed on the spot (actually I should not receive $110000 car in with a safety issues and I should get it serviced on the spot but they kept telling me that I have to make an appointment through the tesla App and I have to wait weeks before the fix the safety issues. This is my second tesla and I found that the tesla X quality is much worse than the tesla Y. Malfunctioned components and systems: 1) Outdated feed from the backup camera. No signal from the side cameras. Reproduced multiple times. 2) False input from physical controls. Reproduced multiple times for the left wheel button on the yoke (scroll up and press) and the honk button on the yoke. 3) False signal from proximity sensors. 4) False signal from open door sensors. 5) Spontaneous activation of autonomous parking (when disabled in the settings). 6) Spontaneous activation of the service mode. The vehicle shows: "Do not drive. Improper use may cause damage or injury." 7) Brake fault. Reproduced multiple times. 8) Parking brake fault. Reproduced multiple times. 9) Steering assist fault. 10) High-voltage system fault. 11) Spontaneous computer restart. Available for inspection before 5/5/2022. Safety concerns: 1) The outdated feed from the backup camera almost led to an accident on 03/26/2022 at ~3:09 pm when another vehicle was shown at a distance, but it was actually right behind the vehicle. 2) False signal from proximity sensors on 04/17/2022 at 10:35 pm likely caused a false accident avoidance maneuver toward leaving the drivable surface at a high speed with no vehicles in proximity (video is available by request). 3) Spontaneously increased volume on 04/17/2022 at 12:00 pm distracted the driver and could damage the ears. It couldn't be counteracted by the driver's input due to the fast false input and the persistence of the problem (the volume increased and the playback resumed). 4) False input from physical controls may potentially lead to an uncontrollable vehicle. 5) Spontaneous activation of autonomous parking may potentially lead to an uncontrollable vehicle. The problem was reported to Tesla on 3/26/2022 (two days after the delivery of the vehicle). The vehicle was inspected and the problem was confirmed by Tesla Service on 4/5/2022. They recommended installing a software update that didn't resolve any of the reported issues. Tesla has issued a safety recall with NHTSA Recall No. 22V-296 and Manufacturer Recall No. SB-22-00-009. The recall addresses the same issue as described in this complaint that occurs after preheating the battery before supercharging and after supercharging. The safety recall states that the issue increases the risk of a collision.

Car is displaying a warning reading “Rear right safety restraint system fault. Service is required.” Same for the left side. I took the vehicle for service and was told that this is a “Known Characteristic of Current Firmware”. Tesla sent me a message stating that “all the hardware components are OK and the alert is caused by a firmware bug at the moment which will be resolved in a future update”. They did not provide any estimates of when this update will be available. They returned me the car without making any statement on whether it is safe to drive, and whether rear passengers would be safe.

While driving my 2022 model x in full self driving beta the vehicle suddenly took a sharp right into another vehicle causing damage to both no one was severely injured. At the time of the incident I was traveling at 40mph which was the designated speed for that area. I had a passenger in the vehicle that can verify the incident.

While driving the electronic systems that monitor the brakes as well as traction control malfunction. It happened multiple times. At one point, the steering wheel was not operable. The brakes became inoperable. I had it towed to the Tesla service center where they said they corrected it. However, upon getting the car back it happened again. Car is very unsafe to drive when all systems shut down.
