My 2014 Toyota Prius (VIN [XXX] , 77,000 miles) displayed Brake, ABS, and Slip warning lights. A mechanic confirmed failure of the Brake Booster and Pump Assembly (DTC C1391). Toyota previously acknowledged this as a safety-related defect under Customer Support Program ZJB, but my coverage expired because I never received Toyota’s notice dated May 21, 2019. My car has been undrivable for weeks and Toyota refuses goodwill repair. This is a serious brake safety issue that should be reviewed for recall or extended coverage since many Prius owners have experienced the same failure. Upon inspection, our mechanic determined that the master cylinder assembly and the actuator assembly require replacement (error code C1391). It is currently available for inspection, but will need to be repaired soon. Our safety and the safety of others is at great risk, because there is impending brake failure if driven and the end result could be catastrophic. On 09/13/25, the BRAKE, TRACTION CONTROL, ABS and ((!)) lights came on about 10 minutes from home. We drove home, parked the car and did not drive it again until we took it to our mechanic on 09/15/25. During this incident, the brakes failed to respond properly, which could have led to a serious accident. We took the car to a local mechanic and repairs are currently pending. The estimated cost for the necessary parts is approx. $3,000 and the labor costs are estimated to be between $1,100 and $2,000. Our local Toyota dealership was also contacted and they confirmed the presence of code C1391. Please reference Toyota Service Bulletin POL18-03, which addresses issues related to the braking system in certain Toyota models, including the Prius V. This bulletin indicates that various brake system-related warning lamps may illuminate due to internal malfunctions within the brake system, and it outlines affected parts that may require replacement. Additionally, please reference Toyota Customer Support Program 20TE07, which provides extended coverage for certain brake system repairs. It is concerning that, despite this well-known, very serious problem, Toyota has not issued a recall. While our vehicle currently has 116,975 miles, we are aware that the warranty period ended 10 years from the first date of use or 150,000 miles. However, this malfunction not only endangers our safety, but also that of other drivers on the road. We urge the NHTSA to investigate this matter thoroughly and consider the implications for a potential recall for this model, because Toyota is well-aware of this issue.

I have a 2014 Prius V with 92,000 miles. My anti-skid, brake and ABS idiot lights went off approximately 1 week ago. (August 25, 2025) Scheduled an appointment at my Toyota dealer (September 3, 2025) and was told there was an abnormal leak in the accumulator and that the brake booster pump and brake pump assembly needed to be replaced. Cost $5715. Not related to the brakes, but they also said I had to replace the rear shock absorbers as they are leaking and needed to be replaced and then realigned. Cost $1363. This should have been a recall not a mileage or 10 year limit.

Toyota has a break issue that they extended the warranty for 10 years or 150000 miles and now a few months pass the 10 years I have a problem with the actuator giving warnings and my daughter went into a skid when applying the breaks. She was lucky and didn’t hit anyone. If Toyota knows there is a problem that is a safety concern they should fix it I did get a Toyota dealership to give an estimate to fix it and was told $4300.00 I think that’s too much for a defective part. It should be a safety recall.

While driving my 2014 Prius V it stalled. I turned it off and restarted it. The yellow car symbol with an exclamation point lit up. I turned the car on and off again. The light went away and I was able to drive home. I was unable to get any OBD 2 codes on my personal reader. I changed the 12 V battery and thought this had fixed the stalling. Then it stalled again and repeated the same process. Though we were able to drove the car I had it towed to a dealership for safety reasons. The dealership pulled codes, cleared them, drove the car 20 miles, and were unable to reproduce the codes. I was told to return when the codes returned. It stalled again and now I am afraid to drive this car.

Error code P0A94 Inverter/converter came and message to pull over. 9 yr 6 months.103365 miles. Toyota has this issue but never did a recall on my vehicle.

At a stop light, when the light turned green, I took my foot off the brake and pressed the gas pedal but the car did not advance forward. Instead it was as though the car was in neutral and it began to roll backwards because I was on a slight slope. I attempted to get the car to move forward by engaging in shifter into Drive, but nothing changed (still as though it was in neutral). I heard a whirring sound but couldn’t tell where it was coming from. The only way I could get it to drive was to turn the car off and restart. This happened twice before I could get where I was going. Fortunately it didn’t result in an accident but I’m afraid to drive the car now.

The contact stated that the check engine warning light was illuminated. The vehicle was taken to a local dealer where it was diagnosed with needing the head gasket replaced however, the vehicle was repaired. The manufacturer was not informed of the failure. SOMETIMES THE CAR NOTHING IN THE CAR WILL LIGHT UP NOR WILL IT START. WE REPLACED THE BATTERY IN THE REAR. WE REPLACED THE FOB BATTERY. SOMETIMES WE OPEN THE DRIVER'S DOOR AND SHUT IT AND IT WILL MAKE THE DASH LIGHTS GO ON AND THE CAR WILL THEN STAT WHEN THE START BUTTON IS PUSHED. THE CAR REMAINS DEAD SOMETIMES NO MATTER HOW MANY TIMES YOU OPEN AND CLOSE THE DRIVERS DOOR. COME BACK 15 MINUTES LATER AND THE LITTLE RED CAR LIGHT ON THE DASH IS BLINKING AND THE CAR THEN WILL START WHEN START BUTTON IS PUSHED. Airbags did not deploy when I got into an accident on I-91. The car has been totaled and is in Allstate's possession, and I believe it is available for inspection. My safety was put at risk because the airbags didn't deploy in a head-on collision with multiple objects (reflector pole, trees). The vehicle is in the process of being inspected by Allstate insurance representatives. There were no warning messages or symptoms prior to this incident.

CONSTRUCTION CREWS HAD REMOVED A PORTION OF THE ROADWAY, CREATING A DITCH, ABOUT 8 FEET LONG IN THE DIRECTION OF TRAVEL, ABOUT 10 TO 12 INCHES DEEP AND ABOUT TWO LANES WIDE. AFTER PASSING A CONSTRUCTION VEHICLE, SHE BEGAN HER TURN TO THE RIGHT AND RAN INTO THE DITCH. THE FRONT WHEELS OF THE PRIUS DROPPED INTO THE DITCH AND PROCEEDED ACROSS THE OPENING SOME 8 FEET, AT A SLIGHT ANGLE, AND IMPACTED THE CONCRETE ROADWAY ON THE OTHER SIDE, CAUSING A SUDDEN AND VIOLENT DECELERATION. MY WIFE SUFFERED SEVERE NECK INJURIES. TOYOTA SENT ME A LETTER SAYING THE AIRBAG SYSTEM OPERATED AS DESIGNED AND WAS NOT AT FAULT (MY SUMMARY.) I CLOSED THE TRUNK DOOR, AND THE BACK LIGHT GLASS (REAR WINDOW) COMPLETELY SHATTERED. WHEN THIS OCCURRED, THE GLASS WAS NOT CRACKED AND WE WERE NOT IN EXTREME TEMPERATURES (IT WAS ABOUT 65 DEGREES). I TOOK IT TO THE TOYOTA DEALERSHIP, AND THE SERVICE GUY SAID IT'S "IMPOSSIBLE" FOR THE GLASS TO BREAK AS IT DID. IT'S OBVIOUSLY NOT IMPOSSIBLE IF IT CAN HAPPEN WITH NORMAL CLOSING OF THE TRUNK DOOR. THERE IS A VISIBILITY ISSUE ON BOTH THE RIGHT HAND AND LEFT HAND SIDE OF THE WINDSHIELD. THE RIGHT HAND SIDE HAS A GLARE/BLURRY EFFECT WHEN MAKING TURNS. THE LEFT SIDE HAD SOME SORT OF CIRCULAR EFFECT THAT NEGATIVELY EFFECTS MY VISION. THE DEALER ATTEMPTED TO TREAT THE WINDSHIELD WITH RAIN X AS SOME SORT OF FIX. THIS DID NOT HELP. THIS WAS A VERY UNPROFESSIONAL FIX. IN FACT, I BELIEVE THE RAIN X MADE THE PROBLEM WORSE. I DON'T REMEMBER THE ISSUE OCCURRING UNTIL A FEW WEEKS AFTER THE INITIAL PURCHASE OF THE VEHICLE.

My wife opened the tailgate and the right side lift support exploded. A person standing across the parking lot thought it sounded like a gun shot. Small metal pieces flew about the cargo area in the car. When my wife then tried to close the tailgate the lift support bent. She was then able to disconnect it and close the tailgate. I included a picture of the remaining left side support plus the broken right side and where it was mounted.

I was parked in my driveway and put my 2014 Toyota Prius in reverse to back up and park in front of my late husband’s workshop about 15 ft. away,on Oct. 10, 2022. The car sped up so fast I could not brake fast enough (no warning).It stopped when it hit the large door of the workshop, and damaged it and the back of my car, costing about $2,000 to the car. I contacted my insurance agent right away, and the person above him at the company the next day, as well as the service manager at the Toyota dealer where we bought the car at the end of 2014. I expect an increase in my premium from my insurance company. I’ll take it to Toyota when it’s done at the auto body shop, but I don’t expect any flaw to be found b/c that is what happened when I had the same kind of accident with this car in Jan., 2022, when I put the car in reverse to back down my long driveway, (which I’d done many times before),and it sped up so fast (SUA) that I couldn’t stop it (no warning). It hit a maple tree in my yard, and it cost about $12,000 to fix the back of my car. I was not injured in either accident, but this flawed accelerator could have caused serious injury to me or others, as it has in many documented accidents in the last 20 years. From the reports I’ve read since my accident last week, thousands of people have experienced similar or worse accidents from a Sudden Unintended Acceleration in their Toyota vehicle. Also, I read in an excellent well-researched and independent report that NHTSA has not researched the accelerator problem thoroughly and has remained noncommittal as to its possible flaws. Toyota wanted to take the “black box” from my Prius in 2022 after my accident, but I didn’t trust them to take it, and tried hard to find an independent engineering group with advanced degrees to study it and identify the flaw in it. I PURCHASED THIS CAR USED, IN MAY 2017 WITH 14,500 MILES. I AM HAVING AN INTERMITTENT PROBLEM OF THE CAR SHIFTING BACK INTO REVERSE WHEN I AM TRYING TO SHIFT FROM REVERSE TO DRIVE. I was driving on the highway during rush hour when a loud beeping noice began. My 15 year old cousin was in the passenger seat and began to check for the source of the beeping. I was driving 55mph on a busy highway with no safe shoulder stop. As I was preparing for the exit, the brakes suddenly became dysfunctional. The pedal was extremely hard to press down and I found myself unable to slow down the car in enough time to avoid hitting upcoming traffic. I found myself in an urgent, life threatening situation and needed to pull over into the emergency lane located before the exit. I could not just consider my safety, but that of other drivers and my 15 year old passenger. She is traumatized. I was able to get the car to stop with my entire weight on the pedal. Once stopped, I scrambled to find the reason for the sudden brake problem, checking if anything rolled under my foot, if the floor mat was loose, if the gear shift was wrong, even checked if I accidentally hit the emergency brake. Nothing was out of place. No new dash lights. I looked in the manual. I had no information. I went to jiffy lube and they performed a diagnostic test and informed me they are unable see all the codes with hybrids because Toyota’s computer can only see Just in August I went to Toyota for my oil change, nothing was said about the brakes needing to be fixed and how repairing them now would prevent the failure of the accumulator pump system. If I had known paying $400-$500 for the brakes then would prevent having to pay $3000 to replace a pump. I would have made that sacrifice and went outside of my budget. This is paramount information that should be disclosed to all Toyota customers that take their car in for care and pay. After further research, I STRONGLY BELIEVE TOYOTA EMPLOYEES PURPOSELY WITHHOLD INFORMATION FROM THEIR CUSTOMERS NOT INFORMING CONSUMERS ABOUT AVOIDING PUMP FAILURE AND THE NEED REPAIR THEIR BRAKES AND IS COMPLICIT IN ANY INCIDENT CAUSED BY PUMP FAILURE MAY CAUSE.

Four warning lights related to the braking system illuminate, followed by what seems to be the cessation of the ABS. After 5 minutes of driving, the lights went off and abs returned, although brakes are shaky. The abs seems to be a known faulty park and is covered under warranty but only up to 150,000 miles. When taken to a dealer, the repair shop recommended I call corporate. Citing the arbitrary mileage limit, the Toyota mechanic seemed convinced the corporate office would cover the cost, saying he’s seen it done on a case by case basis. This coverage should be made unilaterally for all car owners with this defect, not simply those willing to fight for it. Class action lawsuits over what seems to be a similar issue have been privately settled.

Code C1391 - Abnormal Leak in Accumulator. Accumulator for the brake pump is not holding pressure so the pump has to keep turning on to refill. Brake pump and ABS unit have to be replaced. In emergency situations, a malfunctioning ABS module leads to increased stopping distances and loss of steering control during hard braking. Lastly, tires can be worn more easily, which could lead to tire blowouts. This puts myself and others at risk for deadly consequences due to an increased chance for collision if I am unable to control or stop my car. This problem has been confirmed by Toyota and multiple years of Toyota vehicles have had their brake actuator assembly warranty enhanced. The warranty enhancement program has secondary coverage, which is applicable for 10 years from the first date of use, or 150,000 miles, whichever occurs first. Currently I meet the 10 year requirement but have 162,000 miles on my prius, which is completely frustrating. Many have posted with the same problem of being just outside the warranty. The vehicle has been inspected at a Toyota dealership. A symptom was noticeable starting from over 1 ½ years ago when I was under 150,000 miles. The car produced clicking sounds when the brake pedal was held down at traffic lights. Once released, the clicking sound would stop. I spoke with a Toyota mechanic and he told me it was an issue with the brake pump and ABS unit, but Toyota would not repair it until it completely failed. He also mentioned that there was coverage available for the ABS unit once it failed as it was a common issue for the Toyota Prius. If Toyota warranty allowed for the brake pump and ABS unit to be replaced when the symptoms appear instead of when it completely fails, I would have qualified with my mileage. Not only is the repair costly, but for those who cannot afford it and need to continue to use their car despite the brake pump and ABS unit failure, risk their own and others safety when continuing to operate their vehicle.

2014 Toyota Prius: While driving at high speed the engine stalled unexpectedly, steering and braking were not effected. Was able to safely coast to a stop in the breakdown lane and after a few minutes, restart the engine and proceed driving. Another event occurred on the entrance ramp onto an interstate highway. I was able to pull over safely and the car restarted after a short delay. This seems to be a very dangerous condition. Toyota dealer service said they could not determine the cause and they know of no recall. I found online a reference to NHTSA Recall # 20V369000 that referred to "unexpected hybrid power system shut down." That is what happened to me.

Hi my name is [XXX]. Toyota headquarter case # [XXX] Incident day July 6, 2018 I had a big technical problem with my 2014 Toyota Prius V where it caught on fire because one of the valve lifters fell into the engine head and broke the engine block while driving. When that happened, the oil began to leak into the muffler and the engine and hybrid wiring immediately caught on fire. Many of my colleagues who also bought the same vehicle for Taxi business use in San Francisco all have a similar issue as mine. A few of theirs caught on fire, and a few others mentioned to me that the head gasket for their same Toyota vehicle blew out and each of them had to replace it at around 120,000 miles. I believe that’s why they don’t manufacture these same vehicles anymore because of the defectiveness in the engine. I opened a case with Toyota Manufacturer back in July 2018 and they sent a specialist to inspect the vehicle after the fire. He found the piece that fell inside the engine and that damaged the engine. The Toyota specialist admit to me that the engine of my 2014 Toyota vehicle is very defective and must be covered under warranty. My mechanic replaced the engine for me with a used one with 28,000 miles. This entire case was documented under my name with Toyota Motors Manufacture. I contacted the highway safety and I tried to file a claim against Toyota at the time, but I never got a follow up on my case after I left various messages. All they said was that they will investigate but never gave me any information regarding the engine fire situation or honor any warranty. I hope this incident will be investigated by your department and have this vehicle under recall. Thank you. The contact towed the vehicle to the local dealer, where it was diagnosed with needing the inverter assembly to be replaced. The vehicle was not repaired. Upon investigation, the contact associated the failure with NHTSA Campaign Number: 20V369000 (Hybrid Propulsion System). The manufacturer was informed of the failure and the contact was informed that the recall repair had been performed on August 20, 2020. The failure mileage was approximately 86,500.

TURNING THE STEERING WHEEL MORE THAN QUARTER TURN. RATTLING, CLICKING NOISE IS APPARENT OUT OF CENTER STEERING CONSOLE. TURNING THE STEERING WHEEL MORE THAN 360 DEGREES. THERE ARE SEVERAL RATTLES AND CLICKS IN THE WHEEL. DRIVING IS NOT AFFECTED. IT FEELS AS IF SOMETHING IS LOOSE INSIDE THE AIRBAG. THE MAIN CONCERN IS A FEELING OF LOOSENESS NEAR THE AIRBAG CENTER. THE VEHICLE CAN BE STATIONARY IN MOTION. IT DOES NOT MATTER IF THE VEHICLES IN MOTION. THE ONLY THING IN MOTION IS THE STEERING WHEEL.

AS I TRIED TO MAKE A RIGHT TURN AT A VERY BUSY INTERSECTION, I HEARD A STRANGE NOISE AND TOTALLY LOST CONTROL OF THE STEERING WHEELING. I TURNED THE STEERING WHEEL 360 DEGREES ALL THE WAY, AROUND AND AROUND, BUT THE CAR KEPT GOING STRAIGHT... AFTER A MINUTE OR SO, THE COORDINATION BETWEEN THE STEERING WHEEL AND THE WHEELS RE-ESTABLISHED AND I COULD MANAGE TO DRIVE TO A NEARBY PARKING LOT. THE SITUATION HAPPENED AGAIN IN THE PARKING LOT BEFORE IT CAME BACK TO NORMAL. WE TOOK THE CAR TO A LOCAL TOYOTA DEALERSHIP AND LEFT IT THERE FOR 2 DAYS. HOWEVER, TECHNICIANS SAID THEY COULD NOT DUPLICATION THE ISSUE MEANING THEY COULD NOT FIND ANY PROBLEM WITH IT. THEY TOLD US THAT IT IS PROBABLY THE ELECTRICITY PART! The car stalls randomly and does not enter fail-safe mode. The dealer says it is the transmission but the transmission has never shuddered or hesitated. I am reading that these vehicles have intelligent power modules in the hybrid inverter that fail and can cause stalling. I feel like the dealership is working with Toyota to avoid addressing the actual problem. In the meantime I was sent home in a vehicle that continues to stall randomly and does not enter fail-safe mode.

my wife's 2014 Prius V failed to run. after towing to service diagnosed as failed Inverter. service records indicate this was serviced as a recall 2/2019. now toyota refuses to recognize problem as a warranty issue for repair. even though Recall notice indicates repair or replacement should be free of charge. breakdown occurred 5/14/2022. incident started as she was getting on highway. drove home at 25mph.

The car began to jerk, made knocking noises, miss fires on cylinder 1, 2 and 4, coolant leakages, oil leakages, compression issues and the check engine light keeps turning on and off. My safety has been put into play because, the car would jerk many of times as it would want to stop as I was driving. I would then have to pull over and call for assistance. Per the Toyota service advisor he recommended that I needed a new rebuilt engine kit. The extended warranty company stated that I was negligent with my car and denied my claim and refused to pay for the 10 grand motor and I had to tow my car to another shop. However, the car at this time is still experiencing the same malfunctions. The warning lamps are still showing at this present time and prior to the failure of problem. This car meets the "Lemon law guidelines, However, I have been informed otherwise that I don't meet the criteria for lemon law due it being expired sometime last year in Oct of 2024. As stated this is a lemon car.

My car stalled while I was driving it on 2 separate occasions. It is a 2014 w less than 70K miles: 1/2022 Within seconds of 2 or more warning lights going on, the engine stalled. I was able to steer, brake and drive on the electric motor towards home. The electric motor drove another .5 mile before it also died, leaving me with no power steering or brakes. I was on a slight incline and coasted most of the way off road. The car was towed to our local dealer, which said the intake manifold had failed. Dealer replaced IM and related hoses and gaskets. 6/2022 I was driving on a busy interstate at high speed in the passing lane. The car started to lose power and decelerate. I maneuvered to the side of the road and UP! the on ramp. Made it to roadside business on remaining battery power.

I started noticing the ghosting or tripling of light issue in my windshield. I mostly noticed this during night times, I initially thought I was just tired and my eyes were tired, but I’ve been able to photograph it and verify my experience with other people. This effect is horrible for my eyesight and makes all lights at night so blurry I cannot drive safely.

I was driving home from work on 10/14/22. The traffic was moving very slowly because there was a wreck ahead and emergency crews were on the scene. I was having to break often while moving at a slow speed. All of the sudden multiple dash warning lights came on the vehicle and the vehicle no longer responded to my breaking in a normal manner. I pulled off of the hwy and drove to my local dealership on a side road at a slower than normal speed. I told the dealership to check my car for the recall I remember getting in the mail a while back that mentioned the car trouble I was experiencing. I was told that the recall did not apply to me because my car was over 150,000 miles. The local dealership indicated they could not assist me with having Toyota honoring the recall past the specified mileage. I contacted the national Toyota number and was told the problem was included in a "customer support program" not an actual recall from Toyota and that Toyota would not pay to fix my vehicle since it was over 150,000 miles. I see that some people have filed a class action lawsuit against Toyota in this matter. I was told the Brake Booster needed to be replaced and also a break pump assembly would be required with an estimated cost of $2,500. The car is not safe to drive as it is right now. Toyota is aware of the problem and I guess put the "support program" in place to try and get around a forced government recall and also in turn put a cap on the mileage out of the reach of many vehicle owners. I would like the government to make Toyota actually recall the vehicles that have this problem and pay to fix my vehicle with this issue.

I DROVE MY CAR TO CLOSEST DEALERSHIP THE 2ND TIME THIS HAPPENED (NOV. 21, 2105; WALDORF MD TOYOTA), WHO TOLD ME THEY RECEIVED AN EMAIL (BUT NO RECALL) FROM TOYOTA CORPORATE ABOUT THIS ISSUE. DEALERSHIP COULD NOT DETECT THE PROBLEM OR REPRODUCE IT SO, AS INSTRUCTED BY DEALERSHIP, I CALLED TOYOTA CORPORATE NOV. 23, 2015 TO GET CASE NUMBER ASSIGNED. AT ONE POINT, TOYOTA REPRESENTATIVE DEMANDED "WHAT DO YOU WANT FROM TOYOTA?" CASE WAS 'ESCALATED' BUT NO CASE WORKER WAS ASSIGNED UNTIL NOV. 30, 2015. TOYOTA CORPORATE STATED THAT THE TECHNICIAN HAD TO REPRODUCE THE PROBLEM AND IF THEY CANNOT, THERE WAS NO PROBLEM AND DEALERSHIP WOULD RETURN MY CAR TO ME WITHOUT REPAIR. THE DEALERSHIP PROTESTED AND SAID THEY OT REQUIRED TO REPRODUCE THE PROBLEM AS IT WAS AN EPISODIC AND RARE EVENT. FURTHER, THEY WOULD NOT RETURN MY CAR TO ME UNTIL THE PROBLEM WAS FIXED BECAUSE IT WAS A SAFETY ISSUE. WALDORF TOYOTA STILL HAS MY CAR (OVER TWO WEEKS NOW AS TODAY IS DEC. 9, 2015). THEY HAVE NOT RECEIVED ANY FURTHER GUIDANCE BUT ARE WAITING FOR THE FIELD TECHNICAL SPECIALIST TO EXAMINE THE CAR. I FINALLY RECEIVED A CALL FROM CUSTOMER RELATIONS AT THE REGIONAL OFFICE (CENTRAL ATLANTIC TOYOTA) TO AUTHORIZE ACCESS TO THE EDR (ELECTRONIC DATA RECORDER). THIS MEANS OVER TWO WEEKS HAVE PASSED BEFORE EVEN THE FIRST STEP WAS TAKEN BY TOYOTA TO DIAGNOSE THE PROBLEM.

The air temperature was probably about 50 or 60 F. On a route I routinely drive , I accelerated up hill normally from 0mph to about 45 to 55 mph. I did not accelerate aggressively. Just after climbing the short hill, The engine suddenly loses all power. I had to pull over. I look under the engine to find five pieces of metal blown out of the bottom of the engine block above the oil pan and oil spilling out onto the ground. The car has about 98,000 miles on it and was maintenance regularly including on schedule oil changes, coolant change, state inspections. There were no check engine lights or electronic error codes to indicate anything was wrong with the vehicle which I would have been able to diagnose and possibly fix. The car had been maintenanced in August/September for winter by myself. On the last maintenance, oil was discovered inside the engine air manifold and throttle body that had to be cleaned out and (The EGR was also moderately carbon clogged). I'm very dissatisfied with how Toyota handled my case. They called twice to ask when I wanted to schedule my next service appointment when they had the car sitting on their parking lot for three months. I also wrote them a detailed email similar to this report asking how Toyota could help and that I could not afford to buy another car. The engine failed and it can be inspected upon request. The vehicle had complete loss of power. The vehicle was towed to the nearest dealer. I have seen videos on youtube.com that show other people replacing blown prius engines and head gaskets. You would see chunks of metal missing from the engine block. The vehicle was not inspected by the manufacturer police or insurance. I could have been hit by another car from behind especially if the hill were longer like on a mountain side or some other situation. Separate issue: Mice enter the two rear ventilation openings to nest in the car. Mice chewed on of the oxygen sensor wires to cause an electrical fault.

HEADLIGHTS AND CABIN LIGHTS FAILED TO TURN OFF WHEN REMOTE LOCK WAS ACTIVATED. LIGHTS WERE THEN TURNED OFF MANUALLY. FIVE HOURS LATER, IN THE PARKING LOT, THE CAR WAS NEARLY COMPLETELY DEAD. LAST TIME IT HAPPENED, WE LEFT IT AND IT RECHARGED ITSELF. THIS TIME, IT DID NOT RECHARGE ITSELF; IT WAS COMPLETELY DEAD THE NEXT DAY. WE HAD TO HAVE IT TOWED TO THE DEALERSHIP, WHO "JUMPED" IT AND LET IT SIT OVERNIGHT. THEY SAY IT NOW HAS NO PROBLEM.

I PURCHASED A 2014 TOYOTA PRIUS ON MAY 31, 2014. TODAY 6/18/14 I FOUND OUT THERE WAS A RECALL ON THIS MODEL VEHICLE. I CALLED TOYOTA AND I WAS INFORMED THE RECALL SERVICE HAD NOT BEEN PERFORMED. YOKOHAMA AVID ENVIGOR 215/50/R17 TREAD SEPARATION: REAR DRIVER SIDE TIRE DEVELOPED A CRACK ON THE INSIDE OF THE TIRE. TIRES HAVE 18,000 MILES ON THEM. INSTALLED 12/27/2017. CRACK RESULTED IN AIR LOSS TRIGGERING THE TPMS SYSTEM. DISCOVERED WHILE DRIVING ON CITY STREET. REMOVED WHEEL AND INSPECTED VIGOROUSLY BEFORE FINDING SAID CRACK. WE PURCHASED A NEW TOYOTA PRIUS THAT CAME WITH YOKOHAMA ALL SEASON RADIALS. BECAUSE THE TIRE LIGHT CAME ON, I CHECKED THE AIR PRESSURE IN THE TIRES AND SET THEM TO THE PSI AS RECOMMENDED AT ABOUT 1000 MILES. THERE WAS A MINOR IMBALANCE OF 3-4 PSI IN SOME TIRES, BUT NO SERIOUS UNDER INFLATION. WE NOTICED SOME MINOR ROAD NOISE AND VIBRATION. WE HAD A MECHANIC INSTALL MUD FLAPS AND INSPECT THE TIRES, WHEELS, AND BRAKES AT ABOUT 1500 MILES TO SEE IF THERE WAS A VISIBLE REASON FOR VIBRATION. THERE WAS NOTHING OBVIOUS. WE WERE CONCERNED ABOUT THE ROAD NOISE AND ABOUT 2000 MILES WE TOOK THE CAR BACK TO THE TOYOTA DEALER WHO INSPECTED THE TIRES, CHECKED THE AIR PRESSURE, AND DECLARED THAT THEY DIDN'T SEE ANYTHING WRONG. AT ABOUT 3000 MILES DRIVING AT A SLOW SPEED (35 MPH OR LESS) IN A SCHOOL ZONE, WE HAD A COMPLETE BLOW OUT OF THE LEFT REAR TIRE. FORTUNATELY, THERE WAS A GRASSY SHOULDER AND NO TRAFFIC, SO WE HAD NO INJURIES OR OTHER DAMAGE. THE TREAD APPEARED TO SEPARATE AND THERE WAS A SIDEWALL TEAR STARTING AT THE TREAD. WE RETURNED TO THE TOYOTA DEALER AND ALSO SHOWED THE TIRE TO THREE DIFFERENT TIRE SHOPS (ALL THREE SELL YOKOHAMA TIRES). THE TOYOTA DEALER DID NOT OFFER AN OPINION OTHER THAN TIRE DEFECT - SO WE NEEDED TO DEAL WITH THE MANUFACTURER. TWO TIRE SHOPS THOUGHT IT WAS A DEFECT (EVEN THOUGH THEY SAID IT WAS A HASSLE TO GET A REPLACEMENT PAID FOR BY YOKOHAMA). ONE SHOP REPORTED IT WAS A DEFECT UNLESS THE TIRE WAS UNDER INFLATED, BUT THEY DIDN'T SEE ANY WEAR PATTERN THAT INDICATED UNDER INFLATION. The contact owns a 2014 Toyota Prius V. The contact stated that while driving approximately 35 MPH, the brake light, tire pressure, traction control, battery, and other warning lights were illuminated. While attempting to depress the brake pedal, there was more resistance than normal, making it difficult to slow the vehicle. The contact was able to drive the vehicle to the nearest mechanic; however, the vehicle was not diagnosed. The vehicle was then taken to the residence. The vehicle was taken to another certified mechanic, who diagnosed and determined that the brake master cylinder and actuator assemblies had failed. The brake master cylinder and brake actuator assemblies were in the process of being repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 220,000.

The Head Gasket failed on my vehicle, and began to burn coolant and overheat my engine. Upon description of the issue, a dealership mechanic was able to diagnose the issue. The sudden failure of my vehicle due to overheating while driving was extremely risky, and I had no indication that the issue was a head gasket until a dealership mechanic was able to look at my vehicle after being towed. I had taken my vehicle to a third-party mechanic, and they could not tell me why my vehicle was burning coolant, but provided a temporary Stop-Leak fix that did not hold. The first appearance of the Check Engine light was about 9 months ago, was assumed to be the spark plugs, which I had replaced, but then my coolant light started coming on with the check engine light for about a week before the engine overheat incident. The vehicle has been inspected by a Toyota Hybird Mechanic. No other inspections have been done. My safety and other's safety was put at-risk as my steering, speed control, and speed monitoring systems immediately shut off once the system detected an overheat, making it difficult to pull over safely and slowly to address the issue/call a tow truck. I required physical assistance from another motorist to move my vehicle off the road to allow others to pass.

Car was diagnosed at a Toyota dealership with a brake booster issue reading code 20TE07 after my vehicles brakes suddenly would not work and the car continued to jolt forward. The brakes wouldn’t stop the car. According to Toyota, my vehicle is three months outside of the warranty period but under the covered mileage so we are left to repair this defect ourselves. This was a very dangerous situation and could have cost us and our children our lives instead of just the $4800 repair cost. This has happened to both of our Prius’s so this is not a rare issue. Toyota needs to have a plan for these parts failing suddenly and they should all be recalled for safety in my opinion. The experience was very scary and we had zero control over the vehicle when the brakes suddenly went out. Where do I join the class action lawsuit?

I was dropping my daughter off at school and the car completely shut down on me and turned off in the middle of an intersection. I have just had brakes and oil changed but ever since I got the car codes keep popping up and when checked it’s all good. Front lights keep glitching and even with new brakes there is still a bad sound. My dash computer thing keeps going in and out. It happens a lot more when it rains and makes a sizzling sound I’m afraid to drive it in the rain. I’m besides myself how the car turned off on me and my child we couldn’t move I could accelerate and we almost got hit then another light came on and I couldn’t even turn my wheel. Very dangerous

Good Evening, I'm writing to request a callback in regards to my 2014 Toyota Prius which is subject to a recall. "The involved vehicles were designed to enter a failsafe driving mode in response to certain hybrid system faults. Toyota has found that in rare situations, the vehicle may not enter a failsafe driving mode as intended. If this occurs, the vehicle could lose power and stall. While power steering and braking would remain operational, a vehicle stall while driving at higher speeds could increase the risk of a crash." I recently just moved back to South Florida, I was living in NJ for a few years and had the Inverter malfunction earlier this February, 2023, forcing the car to enter the fail safe mode described giving me the warning to "Check Hybrid System, Stop the Vehicle in a Safe Place". The car was at Toyota Universe, Little Falls, NJ for about 45 days, since this was going to take the complete replacing of the inverter and an update of some sort of software. After receiving the car back around March 17th, I moved back to Florida about 2 weeks later. This evening around 5:30 returning from work, the car stalled again while I was driving surrounded by dense traffic, wouldn't accelerate with all lights flashing. Thankfully I was able to pull over slowly into a parking lot after which my steering wheel locked, and my car was no longer able to be put into Drive or Reverse. Car was only able to shift into Neutral and Park. This was the most terrifying experience of my life had something worse happen in the middle of traffic. I have dropped off my car to the dealership parking lot and will be checking it in first thing in the morning. I'd like to ascertain what I should expect from Toyota Manufacturing should this issue be unfixable? I am more than happy to let them attempt fixing it, but I also need to live a normal life and don't feel the need to be responsible for a manufacturing defect putting my life in jeopardy every time I go drive.

Lost steering on car while driving. Shop noted right control arm completely corroded through and broken axle. Entire undercarriage rusted and corroded. Available for inspection. Sudden loss of steering is a risk factor for serious injury or death. Problem has been looked at by two independent service centers who have agreed on problem. Has not been inspected by manufacturer, whom I have contacted over 8 times regarding this issue, or by any other party. There were no warning signs or symptoms prior to failure.

Blizzard Pearl White paint started peeling off the car's body around the top areas of the 4 side doors (passenger & driver sides). There seems to be an active recall on this from Toyota for the same paint color and type, but the Prius model is not included in the active recall for this same condition. It has become a hazard such that such paint chips can be ingested by children. I took it to the Santa Monica, CA Toyota dealership and it was verified by the dealership representative there that this was a condition caused by a defect in the original paint with exposure to sunlight.

Originally, Toyota issued a safety recall in 2014 to address the inverter defect in model years 2010 to 2014, an attempt to fix the problem on about 800,000 cars in the U.S. by modifying software that controls the hybrid electrical system. It was intended to fix the overheating problems. The inverter controls the power flowing between the battery and electrical motors. The inverter boosts the battery from 200 - 500 volts. They inspected my car saying it was in excellent condition. After, the software upgrade i had major problems. My original Prius battery with around 65,000 miles stopped holding a charge. My 12 volt battery also died shortly after. And my power outlets in my got super hot and spiratually worked off and on. And my driver seat gets hot when i turn on car even though seat button is off. I am sweating all the time. Plus, i have skin conditions and asma. I talked to dimitri, and they tried to charge me 1200for unnecessary repair. Also, failed to call NHTSA as promised

UNKNOWN. I WAS BASICALLY DRIVING ON THE FREEWAY, WHEN MY CAR'S ENGINE SUDDENLY STOPPED RUNNING. THE CAR WOULD NOT ACCELERATE AS EXPECTED. IT BEHAVED AS IF THE FUEL INJECTOR WAS NOT WORKING PROPERLY. WE WERE NOT ABLE TO KEEP A CONSISTENT SPEED AND THE MOTOR STARTED TO HAVE A ROUGH IDLE SOUND. THE HYBRID SYSTEM STOPPED WORKING, THE ICON OF THE BATTERIES TURNED PURPLE AND WOULD NOT RECHARGE WHEN STOPPING. WE TOOK IT INTO THE DEALERSHIP, THE SERVICE PERSON THEY SAID IT WAS A SOFTWARE AND EMISSIONS MISS-FIRE ISSUE. IT WILL TAKE A PART TO BE REPLACED AND SOFTWARE UPDATED. TOYOTA SENT OUT A MEMO TO THEM STATING THIS MIGHT HAPPEN AND THIS IS HOW TO FIX IT. IT SEEMS SINCE THEY ARE WARNING SERVICE STAFF TO BE AWARE OF THIS THAT MAYBE THEY SHOULD TAKE CARE OF IT BEFORE IT HAPPENS AND NOT HAVE ME PAY OVER $900 AND THE STRESS OF IT HAPPENING ON THE FREEWAY AND THEN HAVING ME GET IT FIXED.

THE EDR REPORT CLEARLY SHOWED THAT THE BRAKE OIL PRESSURE CHANGES DID NOT CORRELATE WITH THE PROGRESSIVELY INCREASING VEHICLE DECELERATION. THE ISSUE IS OCCASIONAL, RARE AND CANNOT BE REPRODUCED EVERY SINGLE TIME. THIS LEADS TO DISPARITY IN ASSUMED BRAKING DISTANCE FROM ROUTINE EXPERIENCE BY DRIVING THE VEHICLE AND ACTUAL BRAKING DISTANCE AT THE TIME OF FAILED BRAKING WHICH SUPPOSEDLY MAY BE AS A RESULT OF INVERTER ISSUES THAT ARE RESPONSIBLE FOR EXCLUSIVELY REGENERATIVE BRAKING AT LOW (<50 MPH) SPEEDS.

THE CAR BRAKES FAILED AT LOW SPEEDS (10-15 MPH) AND BUMPED INTO THE CAR IN FRONT IN A STOP-AND-GO TRAFFIC ON THE WET FREEWAY AFTER A RECENT RAIN. THE BRAKE PEDAL WENT ALL THE WAY DOWN TO TOUCH THE FLOOR, AS IF THE BRAKE PEDAL WAS DISENGAGED FROM THE BRAKING SYSTEM AND THE ABS LIGHT DID NOT TURN ON AT THE TIME OF BRAKING. THE CAR WAS DRIVING IN THE STRAIGHT LINE AND IMMEDIATELY BEFORE THIS I WAS ABLE TO SUCCESSFULLY BRAKE THE CAR ON THE VERY SAME TRIP. SO THE PROBLEM IS INTERMITTENT. THIS IS THE SECOND TIME WITH THIS CAR, AS MY SPOUSE ALSO EXPERIENCED THE SIMILAR BRAKE FAILURE AT EVEN LOWER SPEEDS (<10 MPH) RESULTING IN TOUCHING THE CAR IN FRONT AND MINOR SCRATCH ON THE BUMPER. DURING ONE INVERTER FAILURE, WHICH TOOK PLACE AFTER RECEIVING THE E0E SOFTWARE RE-FLASH, EXCESSIVE HEAT WAS GENERATED WITHIN THE INVERTER THAT MELTED STEEL BOLTS AND CREATED AN ELECTRICAL ARC THAT BLASTED TWO HOLES THROUGH THE ALUMINUM CASE OF THE INVERTER ASSEMBLY. THE DRIVER STATED IF SHE HAD EXPERIENCED THIS FAILURE WHILE ON THE FREEWAY, 'SHE WOULDN'T BE HERE ANYMORE.' THE VEHICLE WAS A 2012 MODEL YEAR AND AT THE TIME OF THE FAILURE IT HAD ONLY 31,222 MILES ON IT. SINCE THE VEHICLE WAS PURCHASED, THE ODOMETER HAS BEEN OFF BY ABOUT 2 MILES PER HOUR AT 65, AND 3-4 MILES PER HOUR AT 95. THIS MEANS THAT THE OBOE GET IS RECORDING 4% HIGHER MILEAGE THAN WHAT IS ACTUALLY BEING DRIVEN. MY CAR IS NOT ALONE, MANY PRIUS V MODELS HAVE THE SAME PROBLEM. DEALERSHIP DID NOT HAVE A SOLUTION. THEY KEPT THE CAR FOR AN EXTRA DAY, RAN THE DIAGS OVERNIGHT JUST TO SEE IF THEY CAN REPRODUCE BUT, WAS TOLD THAT THEY WERE NOT ABLE TO REPRODUCE THE PROBLEM. NO IDEA WHAT MAY HAVE CAUSED THE ISSUE. THE SMART KEY WAS SITTING ON THE CENTER CONSOLE. TL* THE CONTACT OWNS A 2014 TOYOTA PRIUS. WHILE DRIVING APPROXIMATELY 25 MPH, THE CONTACT ATTEMPTED TO DEPRESS THE BRAKE PEDAL, BUT THE VEHICLE DID NOT STOP. AS A RESULT, A CRASH OCCURRED AND THE AIR BAGS DID NOT DEPLOY. THERE WERE NO WARNING INDICATORS ILLUMINATED. THE CONTACT WAS NOT INJURED AND A POLICE REPORT WAS FILED. THE MANUFACTURER WAS MADE AWARE OF THE ISSUE. THE APPROXIMATE FAILURE MILEAGE WAS 3,200.

I WAS IN A PARKING LOT AND PUT THE CAR IN REVERSE. THEN I HEARD A "BOOM" AND I THOUGHT THAT I BACKED INTO SOMETHING, SO I QUICKLY PUT THE CAR INTO DRIVE. I THEN FELT THE CAR EXCELERATING FAST AND I WAS PUSHING THE BRAKE AS HARD AS I COULD AND COULD NOT STOP THE CAR FROM MOVING FORWARD. THE CAR PROCEEDED TO EXCELLERATE AND WHEN OVER A CURB UP ONTO PAVEMENT AND THROUGH A METAL FENCE AND BENT A 5 INCH ROUND METAL FENCING DOWN TO THE GROUND AND THE CAR KEPT GOING ANOTHER 30 FEET TILL IT FINALLY STOPPED. YOU CAN SEE THE SKID MARKS IN THE THE PARKING LOT.

AFTER ABOUT 6 MONTHS A/C AND CABIN FAN SYSTEM SMELLED MOLDY, MILDEWY AND VERY MUSTY. TOYOTA SERVICE DEPT SAID ALL NEW MODELS HAVE THE PROBLEM (!) AND TOLD ME TO JUST TRY TO RUN THE FAN WITHOUT A/C. PROBLEM DID NOT GO AWAY! AT MY 12 MONTH CHECK, I REPORTED AIR QUALITY MOLDLY/MILDEW PERSISTS, AND SERVICE REP REPORTED THAT ALL NEW 2014 MODELS DO HAVE THE MOLDY/MILDEW CABIN AIR QUALITY PROBLEM AND TOLD ME TO ALWAYS RUN THE A/C AND FAN SYSTEM WITH "OUTSIDE AIR" COMING IN TOTRY TO CLEAN IT OUT. PROBLEM DID NOT GO AWAY. I LOOKED ONLINE AT "TOYOTANATION.COM" FORUM AND NOTE THAT LONG TERM TOYOTA OWNERS, WHO NOW HAVE NEW 2014 MODELS, ARE EXPERIENCING THE PROBLEM FOR THE 1ST TIME! MUSTY/MILDEWY/ MOLDY CABIN AIR IS DEFINITELY A HEALTH HAZARD, AND TOYOTA SHOULD FIX IT OR AT LEAST OFFER ROUTINE A/C AND AIR SYSTEM FLUSHING SERVICE. THIS IS THE WAY THE NEW DESIGN IS, THE AIR SYSTEM IS CLOSED OFF AND THE MOISTURE LAYS IN THERE GROWING MOLD." HE SAID "YOU HAVE TO OPEN THE WINDOWS, IF YOU ARE SENSITIVE, OR HAVE IT CLEANED EVERY WEEK TO KEEP THE SMELL OUT." HE SAID " THAT IS A CHARGE NOT COVERED BY YOUR LEASE, I'LL TAKE THE DASH OFF AND PUT A NEW FILTER AND FLUSH THE AIR HANDLING SYSTEM OUT FOR YOU, THIS TIME. IT USUALLY COST OVER $250.DOLLARS. YOU CAN DO IT YOURSELF BY SPRAYING A WHOLE CAN OF LYSOL BEHIND THE CABIN FILTER EVERY WEEK". MOLD IS KNOWN TO BE DEADLY AND THIS SHOULD NOT BE AN ALLOWED DESIGN. SHORTLY AFTER PURCHASING MY 2014 TOYOTA PRIUS IN JUNE 2014, I STARTED NOTICING A MOLDY SMELL COMING FROM THE AIR CONDITIONER. THE HOTTER OUTSIDE IT GOT, THE WORSE THE SMELL INSIDE GOT. I TOOK THE CAR TO THE DEALER TO DISCUSS THE PROBLEM WITH THEM. THEY INFORMED ME THAT TOYOTA SAYS THIS IS AN "ENVIRONMENTAL ISSUE" AND CAN HAPPEN IN NEW CARS AND IS THEREFORE NOT WARRANTIED. THE SERVICE MANAGER INSTALLED A CHARCOAL AIR FILTER IN THE CABIN AIR FILTER AND SAID THAT IT WOULD HELP ELIMINATE THE ODOR. IT DID NOT. IN FACT, THE CHARCOAL AIR FILTER REDUCED THE AIR FLOW INTO THE CABIN WHICH CAUSED THE AC TO NOT COOL. I HAD THE SERVICE DEPARTMENT REPLACE THE CHARCOAL WITH AN ORIGINAL AIR FILTER IN SEPTEMBER AFTER COMPLAINING THAT THE AC WASN'T COOL. THE SMELL IN THE AC PERSISTS. IT'S NOT JUST A BIT WHEN THE CAR IS TURNED ON. NO MATTER WHAT SETTING I PUT THE AC ON, IT DOES NOT HELP (FRESH AIR OR RECIRC). WHEN IT RAINS OR WHEN IT IS REALLY HUMID OUTSIDE, THE CAR SMELLS LIKE A SWAMP (EVEN BEFORE IT IS TURNED ON). THERE IS ALSO HUMIDITY INSIDE THE CAR WHEN OPENING THE DOOR. I'VE ASKED MULTIPLE TIMES TO HAVE THE EVAPORATOR REPLACED BECAUSE I FEAR THAT IT IS NOT DRAINING AND CAUSING MOLD AND MILDEW TO FORM. BREATHING IN THE MOLD IS A SERIOUS HEALTH CONCERN AND TOYOTA WILL NOT AGREE TO ANY KIND OF FIX. THE CONSUMER STATED HE TOOK THE VEHICLE TO THE DEALER, FOR ERRATIC STEERING WHEN GOING AROUND CURVES AT MODERATE SPEEDS. THE SAME MORNING, WHILE DRIVING, THE VEHICLE WENT OVER A DIP IN THE PAVEMENT. THE REAR OF THE VEHICLE WENT AIRBORNE AS IF, IT WAS ON A HYDRAULIC LIFT. THE CONSUMER HAD NO CONTROL OF THE STEERING OR BRAKING. WHAT SAVED HIM WAS THE LEFT REAR BRAKE AND THE TIRE JERKED ON THE PAVEMENT AND PULLED THE VEHICLE BACK STRAIGHT. HE EXPERIENCED THE FAILURE FOR A SECOND TIME. ACCELERATION DRIVING EAST ON US RTE. 50 AT ABOUT 60 MPH: VEHICLE ACCELERATED FOR NO APPARENT REASON. I NEARLY RAN INTO THE CAR AHEAD OF ME. THIS CAR IS EQUIPPED WITH RADAR ADAPTIVE CRUISE CONTROL, WHICH WAS ENGAGED AT THE TIME. I TURNED OFF THE CRUISE CONTROL IN TIME TO AVOID A COLLISION. UNINTENDED ACCELERATION HAS SPORADICALLY OCCURRED, BUT IT IS USUALLY FAIRLY MILD. THIS TIME IT WAS SO POWERFUL THAT IT FELT LIKE I HAD STOMPED ON THE ACCELERATOR; THE ENGINE ROARED AND THE CAR TOOK OFF AT FULL THROTTLE. THANKFULLY I WAS TRAVELING AT THE MAX DISTANCE OF THE CRUISE CONTROL. ACCELERATION WAS NOT CAUSED BY FLOOR MATS. I WATCHED WHILE THE DEALER SERVICE MANAGER CHECKED.
