I was coming up the freeway off ramp and my car just turned off i had no braking i hit six other cars rolled over and was severly imjured in the hospital forr three days

2014 Prius V

Brakes completely failed and i rear ended a car. I was stopping normally on a city street. According to dashcam i was traveling at about 25mph. The car slowed to about 12mph when the brakes went down to the floor and the car accelerated.

2012 Prius V

8/2/22 My ABS light came on. Took the car to a dealer. They kept it for six weeks and returned it on 10/3/22. No charge and no explanation. 11/7/22 engine light came on. Took in to dealer, they returned it 11/17/22 indicated incorrect air/ fuel mixture. 10/9/23 Traveling 47 mph, came around a mountain and encountered stopped cars. Slammed on the brakes, car did not slow down, steering wheel locked to the right, car yawed to the right, and turned over, hit a curb and the car righted itself. 3 occupants, 2 injuries , 1 death. Researching the 2012 Toyota Prius, found many instances of a faulty ABS system, mimicking exactly my experience. I was never informed by Toyota or the local Toyota dealer.

Tl* the contact owned a 2017 toyota prius. While the contact was reversing the vehicle into a parking lot, it accelerated without warning. The contact attempted to depress the brake pedal, but it failed to engage and caused the vehicle to crash into a tree. All the air bags deployed. The contact did not engage the emergency brake. The contact and passenger sustained minor injuries and received medical attention. A police report was not filed. The contact's insurance company towed the vehicle to a lot and deemed it a total loss. The dealer and manufacturer were not contacted. The cause of the failures was not determined. The failure mileage was approximately 13,850. *ln*jb

2015 Prius V

My mom was involved in an accident in her 2015 Toyota Prius on April 19, 2022, which she advised lurched forward at a stop and when applying the brakes to stop the vehicle, she said was unresponsive, causing her to clip a vehicle and sending her vehicle into a ditch totaling her car and injuring her passenger and her. I would like to have this investigated and hold Toyota responsible. This could have been much worse and lives could have been lost. She is now without means of transportation, and she is now possibly responsible for damages to property and bodily injury because of this. Through my research I have discovered multiple class action lawsuits brought against Toyota for the same exact models. I asked the police officer to run a scan on the computer, but he advised it was only for serious accidents. There was no indication of the problem or messages.

2013 Prius V

On monday jan, 27, 2020, i was involved in the accident with my 2013 toyota prius i was driving on a freeway with regular traffic. The car in front of me suddenly came to a stop. I immediately slammed on my brakes very hard. I had plenty of safe distance between two cars. I ended up running into the rear end of front car without any noticeable slowdown. The brakes did not come on. There was not any feeling of grabbing or tire making any screeching noise. Also i did not see any tire marks on road. All of the air bags were deployed and there was a frontal impact. I also did not feel my seat belt tightening up.the brakes were never felt like being exerting at all. The car was badly damaged in front.

2013 Prius V

We rented a toyota prius from enterprise in shreveport la at the airport. We drove to kilgore tx for the shakespeare festival. In kilgore, the driver with one passenger attempted to enter a diagonal parking space in front of a strip mall. Upon approach, going approximately five miles per hour or less, the car's brake pedal went to the floor, with no braking happening. The car then surged into the parking space, up over the curb, and into the corner of a store, causing extensive injury to passenger side and passenger, minor injury to driver, and extensive injury to the building. Police records state that "at the scene, i checked the brakes of the vehicle both running and not running and fond there was little or no resistance while threatening pressure to the brake pedal. I observed the brake pedal go all the way to the floor and heard a metallic grinding. This lead me to believe the brakes did not function properly at the time of, or immediately prior to the accident." *tr

We purchased a 2017 toyota prius on january 29th 2017 and on march 22nd 2017 the car caught on fire out of no where. The car was parked and off when smoke began to come out from the around the shift changer and the touch screen area. After a few seconds the car was completely on fire.

2016 Prius V

While parking my vehicle and slowing down to enter a parking space, i eased off the brake to pull into the space when i heard the engine rev loudly and then the car accelerated and continued to accelerate despite attempts to pump the brakes, release the brakes without touching any pedal and also trying to put the transmission into park. The vehicle would not stop at all. I was afraid for my life and tried everything to avoid other vehicles and pedestrians as the car accelerated greatly. I steered the car to try and avoid a slight embankment and curve but with the rapid speed of the car, it became hard to maneuver the car. The vehicle jumped the curb striking a tree and then a concrete post. The car came to rest on its side after impact. The car was a total loss.

On my way home i suddenly saw a sign on the screen "check the hybrid system. Stop in a safe place". I pulled over, smelled smoke and saw smoke in the rear. I opened the trunk ans frightened by flame. I pulled things out and covered the flame with two big towels and succeeded in putting it out, although i got burned on my fingers. I called geico. They sent a towing car and it took my car to my apartment. When geico took my car to a collision center, they said it is total loss after inspecting it. I claimed it to toyota. They inspected it, but said that there was no evidence that the car had electrical or mechanical defect. I did nothing but driving the car, but it caught fire. Whose responsibility is that? I believe the producer should take the responsibility but they do not. Where can i be compensated for the loss?

While driving on i-75 there seems to be a spark caused fuel to leak and burn back tire and the tire rolled off the freeway putting all the five passengers into a hazardous situation on the i-75 freeway. We smelled smell of tire and gas leak. Fortunately, we were able to pull off the freeway. It was reported to toyota and their dealers multiple times. No one even bothered or concerned about the issue or safety of passengers.

2015 Prius V

During routine low speed suburban driving, a fire started under the hood in the right front of the car. The smoke was first noticed by a pedestrian so I pulled over, got out of the car, and called 911. I had no prior warning of any sort. The fire completely destroyed the car. This is being investigated by Toyota. I have photos and would be happy to provide more details.

2015 Prius V

I was sitting in the passenger seat of the car while it was parallel parked. I pressed the push to start button to turn on the battery so i could charge my phone. There was no one in the driver seat. The car turned completely on when i reached over to press the button. Not only did the engine start, but without anyone pressing on the breaks the car released itself from being parked. The car started rolling backwards into the car parked behind me. I have no explanation about how this happened. I have been reading into it and apparently it's a defect of the 2015 and older models. When the car started rolling back i also started pressing both the park and push to start buttons to turn it off again. I've never felt so unsafe in a vehicle and goes to confirm that park and push to start should not be buttons!!! It makes the vehicle much more dangerous not being able to override those commands when it chooses to go on its own.

I was pulling out of a parking space where my front was up against a building with a car on either side of me. I reversed for about 25' where a telephone pole mandated that i make a k-turn to get out. As i switched from reverse to forward, i saw a spark as the car jet-propelled forward. I tried to break multiple times, but the car accelerated the 25' into the wall. All airbags deployed, and fluid gushed out of the front end. The car was declared a total loss. I was told by toyota and my insurance company that i must have accidentally accelerated instead of repeatedly breaking. I am grateful that i hit a wall instead of people. Someone could easily have died had i had switched from reverse gear to forward in less confined location. The entire accident lasted 2 seconds, with no opportunity to pull the emergency break or go into neutral. Ps: i am a happily married doctor with beautiful children and grandchildren, and not at all suicidal. This three month old 2015 prius 3 failed before the first oil change. I am not satisfied with the results.

Hi my name is [XXX]. Toyota headquarter case # [XXX] Incident day July 6, 2018 I had a big technical problem with my 2014 Toyota Prius V where it caught on fire because one of the valve lifters fell into the engine head and broke the engine block while driving. When that happened, the oil began to leak into the muffler and the engine and hybrid wiring immediately caught on fire. Many of my colleagues who also bought the same vehicle for Taxi business use in San Francisco all have a similar issue as mine. A few of theirs caught on fire, and a few others mentioned to me that the head gasket for their same Toyota vehicle blew out and each of them had to replace it at around 120,000 miles. I believe that’s why they don’t manufacture these same vehicles anymore because of the defectiveness in the engine. I opened a case with Toyota Manufacturer back in July 2018 and they sent a specialist to inspect the vehicle after the fire. He found the piece that fell inside the engine and that damaged the engine. The Toyota specialist admit to me that the engine of my 2014 Toyota vehicle is very defective and must be covered under warranty. My mechanic replaced the engine for me with a used one with 28,000 miles. This entire case was documented under my name with Toyota Motors Manufacture. I contacted the highway safety and I tried to file a claim against Toyota at the time, but I never got a follow up on my case after I left various messages. All they said was that they will investigate but never gave me any information regarding the engine fire situation or honor any warranty. I hope this incident will be investigated by your department and have this vehicle under recall. Thank you. PARTS OF THIS DOCUMENT HAVE BEEN REDACTED TO PROTECT PERSONALLY IDENTIFIABLE INFORMATION PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).

Unknown. I was basically driving on the freeway, when my car's engine suddenly stopped running. The car was automatically put into neutral and was not able to control the speed of the car, so i had to move to the side of the freeway. As i was sitting in the car, i started smelling burned wire and interior started to fill with smoke. I immediately turned the car off and got oit. As i was standing far away from the vehicle, fluid started leaking and it had caught into fire. The front bumper was completely burned off, as well as some of the engine and electrical parts were affected as well.

Pulled out of the driveway drove about a 100 ft heard large clunking sound front end dropped down as if there was a flat tire pulled off the highway drove through the grass 100 ft back to my home the car started smoking from the engine compartment and burst into flames. Tried to extinguish with fire extinguishers and water hose while calling 911. Fire Department extinguished the fire. Car is a complete loss. There was an oil leak trail from where the car first drop down through the grass. The car did not have a previous oil leak. The car had 300,000 miles on it. No one was injured.

I was driving down a city road at 25-30 mph at dusk (5:15pm), and with my headlights on. The first indication of a problem was the smell of burning rubber when i turned on the heater. Assuming the problem was with another vehicle ahead of me, i turned off the heater and kept driving. A mile later, as i was nearing my destination, i noticed several warning lights turn on (skid light and exclamation mark light) and the steering became less responsive. I then saw smoke emitting from under my hood. A few seconds later, the front passenger-side corner of the car burst into flames. I managed to pull over, woke up my two young children from their car seats, and got everyone out. Within a minute, the entire hood was ablaze. Given where the fire started, i suspect the headlight. I replaced it the day before. The light seemed to work fine for 1 day, but it is suspicious that is also the area where the fire started.

The rented 2013 toyota prius with one passenger and the driver, pulled into a diagonal parking space in front of a strip mall store. Traveling at maybe 5 mph to park, the car's brake pedal gave way and hit the floor flat, providing no braking at all. The car kept rolling toward the corner of the brick building. The car then suddenly surged ahead, smashing into the corner of the entry to the brick building at such a sudden force that the doors to the building were wedged shut. After emergency vehicle took the driver and passenger to the hospital, the policeman on the job, as well as the wrecker service that came to extract the vehicle, could not move the car because there were no brakes, as stated in the police report. The wrecker driver could not load the vehicle to tow using the car's power because there were no brakes. No airbags deployed, but the condition of the corner shows damage that a vehicle moving faster than 5 mph would incur. The policeman came to the hospital to inform the driver that there indeed were no brakes on the car and the driver was not at fault - the vehicle's braking and acceleration systems had failed. *tr

The engine caught fire while sitting in my garage. I had driven it for a few hours doing deliveries for a store and parked it in my garage around 2:30pm. About 1.5 hours later, my wife was coming home and opened the garage door and saw smoke above the hood of my car and then a few seconds later it burst into flames. I believe that it was smouldering for a while and the gust of wind when the big garage door opened gave it the extra push to ignite. The car was bought used and had a front end accident prior to my purchasing it so the electrical system could have melted and started the fire but the inspector that Toyota sent did not give me any copy of his report. It is a 2012 model. I bought the car in 2014, and had it for about 5 years until this happened and completed burned up the car and all the contents of my garage. I had a T-Mobile Sync-up module in the OBD port and it recorded a drop in voltage from around 12 to 5 volts at the time of the fire which I believe was transferred to my phone when I was near it.

For the second time (first in 2019) I was seconds and maybe two feet from stopping the car to park when, with my foot on the brake, the car suddenly and without warning lunged forward at what seemed like full throttle. The first "crash" was in my driveway. The force was shocking and took off the front of the car. I insisted Toyota investigate and they ultimately denied any liability. After the car was allegedly repaired, I started noticing another phenomenon: every time I would slow down to brake--approaching an intersection or stop sign, etc.--the car would briefly lunge forward before the brakes engaged to stop the car. I reported this to the dealer 2-3 times and each time I was assured that this was "normal" for a hybrid transitioning from gas to electric at a lower speed. Two weeks ago, with a friend in the car, I was pulling into a parking space, foot on the brake and about to stop, when the same thing occurred: without warning, the car lunged forward, at what seemed like full throttle, and crashed into a low concrete wall. Had it been seconds later, we would have had our seatbelts off and gone into the windshield. There were injuries. FYI, I have a pretty much perfect driving record and log thousands of miles each year. I had ZERO problems with the Prius I owned prior to this one. I will never drive this car again.

After researching replacement windshields, i discovered that they are back ordered because so many prius owners are having to replace windshields. In fact, a class action lawsuit related to these defective windshields was filed this march. In early or mid may 2018, when my new 2017 toyota prius prime had less than or approx. 10,000 miles on it and when i was getting into my car one sunny morning in my gated parking lot, i first noticed a very thin vertical crack (circa 2-3 inches) on my windshield - starting at the very bottom of the windshield below my wipers. The crack was located in the middle of the glass, i.e. Between driver & passenger. The car was stationary, and had sat in my parking spot for the last day or two, and then on this may morning, there was a crack. No chip mark had existed prior to this, and no rock had hit it when i drove it last. There was no large tree near my parking spot, plus nothing was on the windshield when i got to my car that morning, so nothing fell on it. In less than a week, the damage on the glass grew larger (6 inches) and was spreading. I took my car in for 10,000 mile service per toyotacare, but toyota denied warranty coverage for the repair or replacement of its fragile prius prime's windshield, despite it being within the warranty period and the defect manifesting itself when the car was stationary. Within the next few days, the crack grew and spread larger, eventually turning 90 degrees and expanding horizontally. In late may or early june 2018, i replaced the windshield via safelite auto. The total balance of the first replacement was about $800 - i.e. The replacement cost was about $400, but i was told more money was needed to properly fix camera (adas) problems caused by replacing the windshield. In june 2018, the new windshield cracked, almost 1 or 2 weeks after i just had it replaced. While driving on the highway, i noticed a tiny object hit my windshield. I contacted the dealer. In august 2018, the dealership said it knew of no problems with the 2017 toyota prius windshields.

Crack has continued to expand to points almost exactly 5 inches beyond each end of defrost vent, then stopped expanding. Passenger side (right side) crack is aligned exactly with centerline of defrost vent. At point 5" outboard of vent, passenger side crack turns down and extends to bottom edge of windshield. Driver (left side) crack is parallel to vent on line 3" below vent, then turns up, crosses vent, and parallels vent 3" above vent to termination of crack at 5" outboard of vent. Nothing impacted windshield during trip and windshield was not cracked at end of trip. Engineering defect- inadequate glass strength- unable to use defroster for extended periods. Crack created by stress from localized thermal expansion- aligned with heat source of defroster vent at lower middle region of windshield. This is first extended use of defroster in car- purchased may 2018. Currently has 21,800 miles.

We were traveling at approximately 70miles an hour on a divided highway. The cruise control was activated. My husband was driving, i was the passenger. We both were wearing our seat belts. My husband suffered some sort of medical incident causing him to lose control of the vehicle. We crossed through the median, became airborne, crossed two lanes of opposing traffic and headed up a steep incline. The car came to rest on a pile of debris and hung up on a wire fence. My husband suffered a subarachnoid hemorrhage from striking something on the dash of the car. When the car stopped he was slumped over the wheel bleeding.. He also suffered a mild l4 compression fracture. At the time of the accident i had ligament damage in the chest area. As a result he was taken by ambulance to the regional hospital and then air-evacuated to las vegas. Our side curtain airbags deployed but not the frontal bags. I called toyota and was told that the crash was not severe enough to cause them to nflate but my husband's injuries would tend not to support that. An engineer did remove the "black box" from the vehicle to test it. He promised he would get back to us with the results. When i did not hear from him i called and he told me that all results have been sent to toyota. He was reluctant to say more. I called toyota and was told that several people had looked at the case and that the results had been sent to legal, the attiitude of the people there has gone from helpful to abrupt. My husband is still under the care of a neurologist because of the injury and to determine what cased the incident. They have not been able to determine a cause for his blackout but we have been assured via ct scan that the bleeding has stopped into his brain. If he hit something in the car hard enough to cause this type of injury it seems to me that the airbag should have deployed. .

The prius was stationary, parked in the driveway of our house. My teenage daughter was in the car washing the windshield and windows on the inside and accidentally bumped the gearbox (shifter). The car went out of park and into neutral and rolled backward. The engine was not on. The car quickly rolled 20 feet into a tree and caused 6200$ damages to the rear of the car. Luckily no one was behind the car. I believe that the prius can be accidentally placed in neutral if electrical is on in the car (ie radio) and someone has their foot on the brake and hits the gearbox by accident. There is no safety on the shifter to prevent unintentional shifting. Many other cars require a manual button on the shifter to be depressed prior to shifting. This is not the case on the prius. Without that safety feature, accidents like this one can happen and potentially kill someone. The car can be made to be in neutral by accident rather easily.

Toyota issued safety recalls e0e and f0r for the failure of the hybrid inverter. If the inverter had not failed yet, the toyota's remedy is a 'software re-flash'. The software reflash 'remedy' is not sufficient. There have been an excessive number of inverter failures after the software re-flash 'remedy' for these recalls has been completed. This is very dangerous to drivers, as it leads to unintended deceleration ' the prius spontaneously entering 'limp-home mode' or even a 'hybrid system shut down'. This is dangerous as in results in drastic reductions, if not the complete loss, of power. These unintended deceleration occurrences predominantly happen while the vehicles are in motion ' many times at high speeds. Between only two dealerships, we have documented approximately 100 inverter failures after the e0e and f0r software reflash 'remedy' has been completed. During one inverter failure, which took place after receiving the e0e software re-flash, excessive heat was generated within the inverter that melted steel bolts and created an electrical arc that blasted two holes through the aluminum case of the inverter assembly. The driver stated if she had experienced this failure while on the freeway, 'she wouldn't be here anymore.' the vehicle was a 2012 model year and at the time of the failure it had only 31,222 miles on it. Instances of unintended deceleration will continue to grow daily until toyota implements a sufficient remedy that will prevent these losses of power from occurring. Attached please find a report detailing, for only two dealerships, the inverter failures we've experienced after the e0e and f0r software re-flash remedies have been completed. Also attached, please find the photographs of the inverter which experienced the electrical arc. Please consider that "date this happened" as the filing date of this report. *js

The air bags did not deploy. A police report was not filed and there were no injuries. The vehicle was taken to the dealer where it was not diagnosed, but the structure of the vehicle was repaired. The failure recurred when the brake pedal was depressed, and the vehicle failed to stop. The manufacturer was not made aware of the failure. The approximate failure mileage was 20,000.

Since we had purchase our car prius v four with drl (day running lights)(vertical led unit in bumper) they never worked properly-hence not visible in our accident.. Unlike other earlier models the user could adjust the sensor and turn this feature on or off-but now the feature can only be performed by a toyota service mechanic. The feature also operates off a day light sensor that also cannot be adjusted by the owner of the vehicle. Hence since this is a safety feature noted in faq on toyotas site it should be considered for a recall.

My vehicle has an ongoing issue with the headlights not being bright enough to drive at night with. I have replaced the headlights more than 5 times now over the course of a year, each time with different brands including Toyota OEM headlights. Each time, they are still not sufficiently bright enough to see with at night. This has led to 3 collisions with curbs and objects which caused damage to my vehicle, because I could not see the objects while having to drive at night. I have brought the issue up to Toyota and they said they would not do anything about it. I brought it in for service to Toyota and they tried to charge me $200 to even look at it, money which I do not have. I am forced to continue driving at night for work and fear I will get into a more serious crash or hit a pedestrian as the headlights aren't bright enough to see safely at night. The photo I have included shows a gash in the tire and dent in the rim where it hit the curb. The issue seems to be the headlight wiring or connectors. When I replace the headlight bulbs, they are a bit brighter initially. Thus initial brightness only lasts about a week, and it's still not sufficiently bright to drive safely. They then dim even further to the point where it's like driving without headlights at all. Again this has led to 3 collisions at this point since I'm required to drive at night and don't have the money to fix it. Toyota refused a good faith repair and did not acknowledge there was an issue, however if you do a web search for "Prius headlights too dim" it seems to be a major issue with many other Prius owners. It should be noted that even the high beam headlights are not bright enough to sufficiently see with at night.

My 2012 prius was hit in back on passenger side at approximately 80-85 miles/hour while my speed was 62 m/h. With impact from hit in the back, my car went flying head on into the median wall (those temporary cement walls used during reconstruction). Two things happened were: 1. After the first impact my prius had total power loss, not even emergency flasher light would turn on; 2. Non of airbags deployed. I am wondering if all this is normal for this car or there is some sort of malfunction or manufacturing defect? *tr

I purchased my 2017 prius v ii in may 2017. Approximately 9 months or so after purchase my headlights burnt out, first the driver's side then passengers side. I am now replacing, purchasing new headlights every 3-4 because they continue to burn out. I've tried various brands including buying directly from toyota but they still burn out. This is dangerous especially for night driving. Luckily the headlights do not burn out together at the same time. Toyota dealer won't give me an answer except to purchase a new lighting system for my headlights

The prius v 2017 vehicle 4 series does not come with 'fog lighting' which i had in my prius v 2012 and used all the time-the car i have does come with led day running lights-i really never knew they were lights because i never saw them on. At the dealers-the did reset them and the lights worked but now toyota wants to charge me $300-400 to trace the power lines-the relay to these lights is $31-but for safety reasons-especially in texas and the bright sun-i wish they were working and should be looked at by the dealers for all owners. I drive professionally so my passenger's safety is also my main concern in the matter.

Under certain daytime conditions the dashboard display becomes temporarily unreadable due to pupillary delay adjusting to unilluminated dashboard display of speedometer.the dashboard, by design, is not illuminated during daylight. Because of this, under bright sunny conditions, when the driver glances at the speedometer display, his eyes cannot immediately adjust to read the vehicle without taking his eyes off the road ahead to know his speed.

The problem appears at night, and is very dangerous on curving mountain roads which i drive often. It is a four lane road, often hwy 17. As the ambient light reduces, as there are fewer cars around me, the windshield darkens from top to bottom, and leaves me with perhaps only 10 inches at the bottom i can see through. I cannot see very far in front of me, to see where a curve goes, what is on the road, or how much room i have around me. I have been told it is the headlights that provide reduced lighting. The dealer told me there is nothing they can do, that i should turn on my brights, regardless of oncoming traffic. It is very dangerous.

The headlights are either too bright or aimed poorly that it blinds oncoming traffic, who frequently flash their high beams at me. This occurs without any load in back and without me using the high beams. Can Toyota either adjust the bulbs or place a film on the headlights to prevent blinding oncoming traffic? Thank you!
