The ABS, BREAK, and Traction Control, suddenly turned on when I was traveling on the freeway in mild traffic. I was traveling at about 60-65 mph. I attempted to use the breaks and could feel a substantial difference in how much and how hard I had to push in order to stop. The problem has not been confirmed by a dealer or service center. The vehicle was inspected by my brother in law who works on cars frequently. There were no warning symptoms prior to the problem. The problem first occurred on Saturday November 1st at about 12:30. According to my brother in law, it was the break pump that failed; you can hear air escaping when you push on the break pedal.

Emergency Brake, Traction and ABS warning came on and stayed on in addition to a five-second interval cycling on of a motor sound under the hood. O'Reilly Auto Parts and then Toyota dealership confirmed the same code that indicated brake booster assembly needs replacing (C1391) **This is a "known issue" but not yet a recall. Toyota did extend the part warranty to 10 years or 150,000 miles - my vehicle has only gone 85,500 and unfortunately is eight months into the 10th year and Toyota is refusing to cover any portion of the repair. I would like to be part of the effort to have this formally declared a recall because I got the quote today to replace the brake booster assembly and it's going to cost half the value of the vehicle! $4000 is the low estimate Bighorn Toyota gave me to do the repair/replacement. Thank you for your help!

Brake booster failed. ABS lights on. Brake light on. Traction control light on. Was sitting at stop with foot gently on brake when lights turned on. Attempted to restart car to clear lights, lights would not turn off after normal restart x3. Brake no longer function correctly, and the factory installed ABS are no longer functioning. Braking distance greatly increased, and no longer as safe to operate.

While driving my Prius on the highway, the ABS and traction control warning light suddenly came on. The brakes started feeling different right away, and it didn’t feel safe to drive. I took it to a Toyota dealership, and they found that the brake booster and pump had failed. I looked online and saw that many other Prius owners had the same issue, so it doesn’t seem like an isolated case.

2015 Toyota Prius ABS warning light came on . Went to car mechanic . Brake Actuator & Pump code leaking. It happen on 7/27/25 under 100,000 miles. Currently Anti Sway and ABS braking disable.

My small battery died and it is located on passenger side, inside, underneath tail light. There us no key entry. One must crawl to back if car and dismantle many pieces to access release. I had heavy 5 gallon jugs of water in back. There is no trunk release and no key entry. The only way to jump car is at fuse box under hood. Car needs a button inside, ideally up front.

The clear coat on the roof of my 2015 Prius began to peel little by little starting around 2021. What started out as small patches has now become large patches on various parts of the roof. The clearcoat is also beginning to peel right below the windows on the passenger side, close to where the door panel and the window meet. It's unsightly and I have not had this happen to any of my other vehicles in the past. I've done nothing different in the handling/washing/parking of the vehicle compared to my previous cars.

On August 10, 2021 my 2015 Prius was parked in a Strip Mall parking lot. When I turned on the Prius and shifted into reverse, after traveling only a foot or so, the shift selector popped into Neutral. When I shifted into Drive, it happened again. After several tries with the same results, I called Findlay Toyota Service. I was advised to have the car towed, at my expense, to Findlay Toyota Service. Note: the tow truck driver reported to me and the Findlay Service Representative that he could keep the shift selector in gear by holding it in place with his hand while driving it to the tow truck. The Prius was left at Findlay Toyota for 2 days. The Service Rep reported that 3 different mechanics plus himself could not duplicate the problem; the Prius stayed in gear for them. I researched online and found other Prius owners reported this problem ocurring intermittantly. The only solution seems to be replacing the Shift Selector Switch at a cost of $838.35 + tax. Even though this is clearly a safety hazard, there is no recall.

When I parked my car and stopped the engine after driving for 90 min/62 miles on June 22, 2023, I found that I couldn't lock my car's door, through my remote, door handle or the switch inside the door. Upon further examination, I saw complete loss of the power: no light, no audio, no other accessory power, no start of engine. Foot press on the brake feels heavy/less responsive. One hr after the incident I went back (because I had some business to attend) to the car, everything looked normal (with full functions recovered) as if nothing happened before. 6 hrs later the car still behaved fine. I measured 12-v battery and it registered 12.3 v (I bought the car new 8 years ago and I haven't changed the battery yet). I noticed that my the clock in the car lost 10 mins (behind correct time) and I concluded the complete loss of battery power lasted about 10 mins. By the time of this report, I have started the engine 5 times without any problem but this loss of power gravely concerns me. I saw recalls for similar problems affecting similar prius models/years however my car is not on the list.

Window buttons on drivers side master power window switch failed. Felt warm on door. There have been recalls of other models of Toyota cars including Prius C.

During routine low speed suburban driving, a fire started under the hood in the right front of the car. The smoke was first noticed by a pedestrian so I pulled over, got out of the car, and called 911. I had no prior warning of any sort. The fire completely destroyed the car. This is being investigated by Toyota. I have photos and would be happy to provide more details.

My vehicle has an ongoing issue with the headlights not being bright enough to drive at night with. I have replaced the headlights more than 5 times now over the course of a year, each time with different brands including Toyota OEM headlights. Each time, they are still not sufficiently bright enough to see with at night. This has led to 3 collisions with curbs and objects which caused damage to my vehicle, because I could not see the objects while having to drive at night. I have brought the issue up to Toyota and they said they would not do anything about it. I brought it in for service to Toyota and they tried to charge me $200 to even look at it, money which I do not have. I am forced to continue driving at night for work and fear I will get into a more serious crash or hit a pedestrian as the headlights aren't bright enough to see safely at night. The photo I have included shows a gash in the tire and dent in the rim where it hit the curb. The issue seems to be the headlight wiring or connectors. When I replace the headlight bulbs, they are a bit brighter initially. Thus initial brightness only lasts about a week, and it's still not sufficiently bright to drive safely. They then dim even further to the point where it's like driving without headlights at all. Again this has led to 3 collisions at this point since I'm required to drive at night and don't have the money to fix it. Toyota refused a good faith repair and did not acknowledge there was an issue, however if you do a web search for "Prius headlights too dim" it seems to be a major issue with many other Prius owners. It should be noted that even the high beam headlights are not bright enough to sufficiently see with at night.

THE CONTACT STATED THAT TURNING OFF THE HEADLAMP SWITCH AND TURNING BACK ON TEMPORARILY REMEDIED THE FAILURE. THE CONTACT STATED THAT THE FAILURE WAS INTERMITTENT. THE VEHICLE WAS TAKEN TO MOLLE TOYOTA LOCATED AT 601 WEST 103RD STREET 64114 PHONE (866)-577-9913. THE CONTACT WAS INFORMED THAT THE HEADLAMPS OPERATED AS INTENDED WHILE BEING INSPECTED BY THE TECHNICIAN. THE CONTACT WAS ASKED TO HAVE THE VEHICLE TAKEN BACK TO THE DEALER WHILE THE FAILURE WAS PRESENT. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOT INFORMED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 100,300.

2015 FORD EDGE WITH 22,000 MILES. THE REAR CENTER BRAKE LIGHT ASSEMBLY HAS MALFUNCTIONED AND THE LIGHTS ARE NOT WORKING. THE LIGHTS CANNOT BE REPLACED - THE WHOLE UNIT HAS TO BE REPLACED, WHICH COSTS ALMOST $300. THE DEALER TOLD ME THERE IS WATER IN THE UNIT. THIS CAR IS GARAGED AT WORK AND AT HOME. I HAVE READ THAT OTHER PEOPLE HAVE HAD THE SAME ISSUE. I HAVE AN EXTENDED WARRANTY BUT LIGHTS AREN'T INCLUDED. SINCE THIS IS A WHOLE UNIT - NOT JUST A BULB - IT SHOULD BE INCLUDED.

AUTO ADJUSTING LOW BEAMS NOT WORKING. LOW BEAM IS FIXED DOWN AS IF THERE WAS A HEAVY WEIGHT LOAD, WITH NO LOAD IN THE AUTO THE LOW BEAM LIGHT PATERN IS ONLY 25 FEET, THIS DISTANCE PUTS THE DRIVER AT RISK BEING UNABLE CLEARLY SEE THE ROAD AND DRIVE AT A REDUCE SPEED OR OVER DRIVING THE LIGHT. DRIVER EYES ARE STRAINED AS THEY TRY TO NAVIGATE. THERE IS NO PROBLEM WITH THE HIGH BEAMS. AFFECTS ONLY AT NIGHT ON HIGHWAYS AND. POORLY LIGHT WIEDY COUNTRY ROADS.

STARTED HEARING GRINDING AND SCREECHING NOISES FROM UNDERNEATH THE HOOD AFTER A 125 MILE DRIVE TO SAN FRANCISCO. THE FOLLOWING DAY VEHICAL STARTED BUT GRINDING NOISE PERSISTED WHILE VEHICAL WAS PARKED IDLING.DORVE TO THE STOE AND ON THE WAY BACK HOME THE CAR STALLED ON THE HIGHWAY. COULD NOT GET IT TO TURN BACK ON SO I PUSHED IT OFF THE HIGHWAY. ERROR MESSAGE ON DASH MENTIONED SOMETHING ABOUT AN ERROR WITH THE HYBRID SYSTEM.

THE PRIUS WAS STATIONARY, PARKED IN THE DRIVEWAY OF OUR HOUSE. MY TEENAGE DAUGHTER WAS IN THE CAR WASHING THE WINDSHIELD AND WINDOWS ON THE INSIDE AND ACCIDENTALLY BUMPED THE GEARBOX (SHIFTER). THE CAR WENT OUT OF PARK AND INTO NEUTRAL AND ROLLED BACKWARD. THE ENGINE WAS NOT ON. THE CAR QUICKLY ROLLED 20 FEET INTO A TREE AND CAUSED 6200$ DAMAGES TO THE REAR OF THE CAR. LUCKILY NO ONE WAS BEHIND THE CAR. I BELIEVE THAT THE PRIUS CAN BE ACCIDENTALLY PLACED IN NEUTRAL IF ELECTRICAL IS ON IN THE CAR (IE RADIO) AND SOMEONE HAS THEIR FOOT ON THE BRAKE AND HITS THE GEARBOX BY ACCIDENT. THERE IS NO SAFETY ON THE SHIFTER TO PREVENT UNINTENTIONAL SHIFTING. MANY OTHER CARS REQUIRE A MANUAL BUTTON ON THE SHIFTER TO BE DEPRESSED PRIOR TO SHIFTING. THIS IS NOT THE CASE ON THE PRIUS. WITHOUT THAT SAFETY FEATURE, ACCIDENTS LIKE THIS ONE CAN HAPPEN AND POTENTIALLY KILL SOMEONE. THE CAR CAN BE MADE TO BE IN NEUTRAL BY ACCIDENT RATHER EASILY.

I am having a hybrid propulsion system issue due to a part that was recalled according to this ID number for the NHTSA 17V718000 that was posted NOV 14, 2017 and my car does seem to be affected because it is facing the same issues as other cars reported from that batch. I want to make sure that I can have it serviced properly at my dealership

My Prius V has slow acceleration and the RPMs seem to go unusually high on acceleration. This makes it dangerous to pull out from a stop sign, etc. in certain situations that would be fine in another car of the same make and model. And recently it has started to throttle up when taking my foot off the gas and even when pressing on the brake. This happens intermittently and randomly. Fortunately no accidents have occurred yet and I'm trying to get the problems resolved with Toyota, but the dealer says it will be hard for them to diagnose when the problem is not occurring and there are no warning lights. Less than 60,000 miles on it.

I WAS DRIVING ON THE INTERSTATE WHEN MY CAR'S HYBRID SYSTEM WARNING LIGHTS A CAME ON , AND THE CAR SLOWED TO A CRAWL. LUCKILY I WAS IN THE RIGHT LANE AND WAS ABLE TO PULL OFF ON THE SHOULDER OF THE ROAD. THE CAR COMPETELY SHUT DOWN. I'VE HAD IT TOWED TO TWO TOYOTA DEARLSHIPS, (THE SAME ONE TWICE),WHERE I WAS TOLD A NUMBER OF DIFFERENT THINGS. FIRST, I WAS TOLD THERE WAS WATER IN THE GAS TANK. THERE WAS NO WAY THAT WAS TRUE, SINCE I HAD ALREADY DRIVEN 3/4'S OF A TANK OF GAS, AND I ALWAYS FILL MY TANK UP. I REFUSED TO HAVE A NEW FUEL SYSTEM PUT IN TO THE TUNE OF $2500. I HAD IT TOWED TO AN INDEPENDENT REPAIR SHOP, AND WHERE HE GOT 5 GALLONS OF WATER OUT OF THE TANK. I ONLY HAD ABOUT 3 GALLONS OF GAS IN THE CAR TO BEGIN WITH, AND I KNOW I DIDN'T PUT WATER IN THE TANK.THE HYBRID SYSTEM SEALS HAD NEVER BEEN BROKEN, SO THE DEALERSHIP DIDN'T EVEN CHECK THAT OUT, EVEN THOUGH THE WARNING LIGHTS CAME ON REGARDING THE HYBRID SYSTEM. VERY FISHY...THE NEXT DEALERSHIP TOLD ME THERE WAS WATER IT AND IT NEEDED TO BE DRAINED AND FUEL REPLACED, EVEN THOUGHT THAT HAD JUST BEEN DONE. THEY SENT ME A PHOTO OF A COKE BOTTLE WITH ABOUT 1/4 CUP OF FUEL WATER MIX. I WAS ALSO TOLD THAT THE HYBRID BATTERY SYSTEM WAS NOT UNDER WARRANTY SINCE AN INDEPENDENT REPAIR SHOP DRAINED THE FUEL SYSTEM.THE CAR'S BATTERIES WERE STILL UNDER WARRANTY, SINCE IT WAS UNDER 100,000 MILES. WE SENT IT BACK TO THE FIRST DEALERSHIP WHERE THEY NOW SAID IT WAS A FAILED ENGINE WHICH WOULD COST $10,000 TO REPLACE. (MORE THAT THE WORTH OF THE CAR). THIS NIGHTMARE HAS LASTED OVER 8 MONTHS! IT BEGAN IN SEPT.2019. COMPLAINED TO TOYOTA CORPORATE TO NO SATISFACTION. BOTTOM LINE, TOYOTA DIDN'T KNOW WHAT WAS WRONG WITH THE CAR, AND WOULD NOT FIX IT UNLESS WE PAID TO HAVE IT FIXED EVEN THOUGH IT WAS UNDER WARRANTY. WHILE DRIVING ON THE FREEWAY, THE SUNROOF GLASS EXPLODED. FORTUNATELY, WE WERE NOT INJURED SINCE THE SUNROOF WAS CLOSED. I NOTED THAT THERE WERE MANY SIMILAR REPORTS. SURPRISED THIS WAS NOT A RECALL ACTION.

I WAS ON THE INTERSTATE GOING 70 MILES AN HOUR IN THE LEFT LANE. I WANTED TO CHANGE TO THE RIGHT LANE, PUT ON MY TURN SIGNAL, CHECKED BOTH MIRRORS, AND STARTED TO PULL OVER WHEN A HORN BLEW, I DID NOT SEE A CAR IN THE RIGHT LANE AS THE PRIUS HAS A TERRIBLE BLIND SPOT. THIS HAS HAPPENED A FEW TIMES. NOT GOOD I MISSED HITTING THE VEHICLE BE MERE INCHES.

LOUIS(55-70 MPH, HIGH WIND, 60-70 DEGREES AND RAIN, WIPERS ON CONSTANT HIGH BETWEEN ONE HOUR TO ONE AND QUARTER HOURS OF DRIVING, WIPERS STOPPED WORKING, BLADES POINTING STRAIGHT UP. CHECKED FOR OBSTRUCTION AND TRIED TO MOVE BLADE/ARMS NO CHANGE. AFTER ABOUT 15-30 MINUTES THEY STARTED WORKING AGAIN ON THERE OWN, (VERY UPSET HARD TO PINPOINT TIME). USED INTERMITTENT MOSTLY AND LOW AS LITTLE AS POSSIBLE FOR NEXT THREE HOURS OF DRIVING WITH NO PROBLEM, (SWITCHED TO HIGH WIPERS ONCE OR TWICE FOR LESS THAN 30 SECONDS EACH TIME). REAR WASHER/WIPER WAS FINE AND FRONT WASHER WORKED WHEN PROBLEM WAS PRESENT. VERY SCARY DRIVING EVEN AFTER THEY STARTED WORKING NOT KNOWING IF THEY WOULD STOP AGAIN. TOOK VEHICLE TO DEALER TWO DAYS LATER AND THEY COULD NOT DUPLICATE CONDITION AND RETURNED VEHICLE.

THE DRIVER CANNOT SEE ANY PART OF THE FRONT OF THE CAR. IN MOST CARS, THE DRIVER CAN SEE ENOUGH OF THE HOOD AND FRONT FENDERS TO JUDGE HOW CLOSE THE CAR IS OTHER OBJECTS (CARS, WALLS, PEDESTRIANS, ETC.), BUT IN THE PRIUS, EVEN THOUGH I AM OVER 6 FEET TALL, I CANNOT SEE ANY PART OF THE HOOD OR FRONT FENDERS. WITH THAT LACK OF PERSPECTIVE, I SCRAPED THE LEFT FENDER AGAINST A PARKING LOT WALL. BUT THAT SAME LACK OF PERSPECTIVE COULD RESULT IN HITTING A PEDESTRIAN.

I WAS DRIVING ON I-10 W ABOUT 10 MILES OUTSIDE OF NEW ORLEANS WHEN MY CLOSED, LOCKED SUNROOF EXPLODED UPWARDS, SHATTERING COMPLETELY THE CENTER SUNROOF PANEL AND SHOWERING ME WITH GLASS. IT WAS A CLEAR SUNNY DAY, ABOUT 70 DEGREES, THE ROAD WAS CLEAR WITH NO TRAFFIC OR DEBRIS AND I WAS TRAVELING AT APPROXIMATELY 65 MPH.

I bought this vehicle on July 3, 2015. When I took it in for an oil change in Dec. 2015, I was informed that there was a mice infestation and that the electrical wires and other systems were damaged, resulting in about $2,800 in repairs due to the mice chewing the wiring. I just took this vehicle back in on 29 Nov. 2021 because some of the systems weren't working properly, and they confirmed that a mouse has again chewed up the electrical system, again costing about $5,000 in repairs. I've just been informed by my exterminator that Toyota uses a wiring harness coated with a soybean based coating which is causing reports of mice damage in a lot of Toyota vehicles. We have 2 other vehicles which are not Toyota vehicles and have never had a single problem with mice chewing the wiring harnesses of those vehicles, although all are parked in the same area. The vehicle is currently pending repairs at Fred Haas Toyota Country in Houston, TX and will be there for at least a couple of weeks pending availability of parts. Electrical issues of this type can cause serious failure of safety features. Toyota has never told me that they use soy based elements in their vehicles that would entice rodents to chew the wiring or otherwise entice them into the vehicle. I do not have a report from insurance yet, but I have filed a claim.

THE RADIO/NAVIGATION/TELEPHONE SYSTEM ON THE TOYOTA PRIUS V 2015 STARTED FREEZING A FEW WEEKS AFTER I PURCHASED THE VEHICLE. THE GPS, RADIO, TELEPHONE ETC. ARE ALL CONTROLLED BY A CENTRAL TOUCH SCREEN PANEL ON THE DASHBOARD OF THE VEHICLE. THE GPS BECOMES UN-RESPONSIVE TO TOUCH AND CAN SUDDENLY NO LONGER BE USED! THIS HAS HAPPENED REPEATEDLY BUT RANDOMLY ON MY VEHICLE, AND MANY OWNERS ARE SEEING THE SAME ISSUE ON THEIR PRIUS V 2015 TOO. AFTER A FEW MILES OF DRIVING, THE SCREEN BECOMES UNRESPONSIVE, AND REMAINS UN-RESPONSIVE UNTIL THE CAR IS TURNED OFF AND STARTED AGAIN, ESPECIALLY ON HOT DAYS. THE ONLY WAY CERTAIN OWNERS HAVE BEEN ABLE TO GET A PERMANENT FIX FOR THE ISSUE IS BY A REPLACEMENT OF THE NAVIGATION SYSTEM. IT APPEARS THERE IS A SERVICE BULLETIN FROM TOYOTA T-TT-0336-15 THAT MENTIONS THE ISSUE. HOWEVER, I HAVE ALREADY BEEN TO MY TOYOTA SERVICE CENTER 3 TIMES FOR THIS ISSUE, AND THEY KEEP SAYING THAT THEY CANNOT "REPRODUCE" THE ISSUE, AND NEED TO ANALYZE FURTHER. I HAVE TAKEN MULTIPLE VIDEOS OF THE ISSUE BETWEEN MAR 25 AND AUG 8 AND SHARED WITH THE SERVICE CENTER. BUT THEY REFUSE TO ACKNOWLEDGE AND TAKE ACTION. When I looked up my car on Kelly Blue Book for pricing, it said that it has been recalled as of 2020. I was not aware of this at all and could have got into a serious accident. I recently purchased a 2015 Toyota Prius Hybrid in February and not even my saleswoman told me that it has been recalled. It has a sleuth of issues that came with it.

THE SOUND OF THE ENGINE WAS LOUD ENOUGH THAT PEOPLE IN NEARBY HOUSES HEARD IT. MY CAR WOULD NOT SLOW WITH MY EFFORTS TO BRAKE. I TRIED TO SHIFT INTO NEUTRAL AND PARK BUT THE CAR DID NOT SLOW DOWN. Failure of brake pump and brake cylinder assembly system, resulting in reduced braking ability and stiff brake pedal. Per Toyota's diagnostic: trouble code C1391 is present, abnormal leak in accumulator, replaced brake ABS actuator assembly, along with brake master cylinder, and bleed the brake hydraulic system. No warning lamps or safety indicators prior to failure. Upon failure (while on freeway going 50+ MPH) the ABS, Brake, and traction control warning lamps illuminated. The failure resulted in no accident or injury, however it did occur while on the freeway going freeway speeds, and resulted in difficulty in braking, which was dangerous. Took Prius into Toyota dealer for diagnostic and repair, was told that part was known to fail in this model of Prius, and had a manufacturer designated extended warranty for 10 years or 150,000 miles. However, despite prius being under 150,000 miles, it passed ten years since purchase in march 2025, meaning I had to pay in full for the repair ($3,631.96). The car gave no indication of needing the repair until the day of the failure, which was on June 18th 2025. If the vehicle had given some prior indication of brake pump system failure or fault I would have had it repaired prior to the warranty expiration, but it did not, leaving me to pay out of pocket for a failed part that Toyota is at fault for making prone to failure.

The headlight bulbs keep going out. I have replaced the bulbs at least 2-3 times a year for the past few years. I have asked dealers about this and they tell me there is no recall or tsb. This is a hazard to night driving because of decreased illumination, especially in dark roads with not enough lighting. Please recall/fix this issue!!

My mom was involved in an accident in her 2015 Toyota Prius on April 19, 2022, which she advised lurched forward at a stop and when applying the brakes to stop the vehicle, she said was unresponsive, causing her to clip a vehicle and sending her vehicle into a ditch totaling her car and injuring her passenger and her. I would like to have this investigated and hold Toyota responsible. This could have been much worse and lives could have been lost. She is now without means of transportation, and she is now possibly responsible for damages to property and bodily injury because of this. Through my research I have discovered multiple class action lawsuits brought against Toyota for the same exact models. I asked the police officer to run a scan on the computer, but he advised it was only for serious accidents. There was no indication of the problem or messages.

As stated from 5 months after we bought it, and the first prius, the car won't start. No matter if parked flat, keys in or out over night, or the temp, when the break is pushed it is squishy and makes draining noise and won't start. As a dr on call i have LOST patients because I could not leave my house. i carry and use the charger daily ever since. Taken in lots. they say, "you don't drive enough..or some other bogus excuse, like don't leave keys in car, which is how priuses start and run!. the car is driven in all ways a lot. also we buy headlights and headlamps on both about every 2 months. also it looses power and has shut off while driving. the worst is when it would start on its own in the locked garage. at that point my wife just quit driving all together. nothing but nightmares. She almsot crashed on fw when power went out.

While attempting to accelerate from a stop light, the Prius stalled. It would not accelerate although the engine revved. I restarted the car by powering completely off and back on. I tried to accelerate again multiple times until it slowly came up to speed the third attempt. The car was in drive and has never happened before. This is a safety concern for multiple reasons but the main being it can cause harm to those in the vehicle as well as other drivers on the road when the car stalls unexpectedly. There were no warning signals on the dash. Toyota dealer looked at it right away, ran diagnostics, and could not find an issue. Nothing was “fixed” or updated.

The contact owns a 2015 Toyota Prius. The contact stated while attempting to start the vehicle, there were several unknown warning lights illuminated. Additionally, the Hybrid System failure message was displayed on the instrument panel. The vehicle was not drivable. The contact towed the vehicle to the local dealer, where it was diagnosed with needing the inverter to be replaced. The vehicle was not repaired but was pending the repair. Upon investigation, the contact associated the failure with NHTSA Campaign Number: 20V369000 (Hybrid Propulsion System) however, the Vin was not included. The manufacturer had not been informed of the failure. The failure mileage was approximately 110,000.

BEFORE WE COULD FIND A SPOT AT THE SIDE OF THE ROAD THE ENGINE SHUT OFF AND WOULDN'T TURN BACK ON, BLOCKING TRAFFIC. VEHICLE TOWED TO LOCAL TOYOTA DEALER. ENGINE TURNED ON AT DEALERSHIP. DEALER RAN DIAGNOSTIC TESTS AND SAID NOTHING WAS WRONG WITH CAR. SENT US HOME SAYING CAR WAS FINE. NOVEMBER 24, 2020 RECEIVED NOTICE FROM TOYOTA ABOUT CUSTOMER SUPPORT PROGRAM FOR FAILURE OF "INTELLIGENT POWER MODULE (IPM) LOCATED INSIDE THE INVERTER ASSEMBLY OF THE HYBRID SYSTEM". CONDITION DESCRIBED AS "HYBRID SYSTEM WARNING" SHOWING ON INSTRUMENT PANEL AND VEHICLE GOING INTO "FAIL-SAFE DRIVING MODE". CALLED PHONE NUMBER ON THE NOTICE AND WAS TOLD THE PROBLEM WE HAD WITH THE CAR WAS THE CONDITION DESCRIBED IN NOTICE, BUT NOTHING COULD BE DONE UNTIL IT FAILS AGAIN AND THE DIAGNOSTIC TESTS ARE POSITIVE. IF THEY KNOW THIS IS A SAFETY PROBLEM, AND IT ALREADY HAPPENED ONCE IN OUR CAR, THEY SHOULD REPLACE THE FAULTY PARTS, NOT WAIT FOR US TO GET INTO A WRECK ON THE FREEWAY.

I AUTOMATICALLY AND IMMEDIATELY PUT MY FOOT BACK ON THE BRAKE WHICH STOPPED THE CAR. MY VEHICLE SHOT FORWARD AND IN A SECOND HAD CRASHED OVER A CEMENT CURB, THROUGH A FLOWER BED, AND WAS STOPPED - DESPITE MY DESPERATELY JAMMING THE BRAKE - ONLY BY SMASHING INTO A BRICK WALL. THE SUSPENSION AND STEERING ON THE CAR IS NOW SEVERELY DAMAGED, AND THE CAR CANNOT BE DRIVEN. I AM HORRIFIED: I COULD HAVE BEEN SERIOUSLY INJURED AND WORSE, HIT/HURT ANOTHER PERSON (OR AT THE VERY LEAST SEVERELY DAMAGED PROPERTY). WHAT IF I HAD CHOSEN A PARKING SPOT IN FRONT OF A GROUP OF CHILDREN?!! THEY CLOUD HAVE BEEN KILLED! THE CAR IS NOW AT A TOYOTA SERVICE CENTER WAITING FOR TOYOTA CORPORATE TO "DECIDE" IF A CAR ACCELERATING INTO A BRICK WALL OF ITS ACCORD IS A "DESIGN FLAW" THAT IT WILL PAY TO FIX. I BELIEVE THAT WHAT MAY HAVE HAPPENED IS THAT THE COMPUTER SYSTEM THAT CONTROLS THE THROTTLE/ACCELERATION EXPERIENCED A SOFTWARE BUG WHICH THREW THE CAR FORWARD. IN FACT, TOYOTA WAS FORCED PAY OUT OVER ONE BILLION DOLLARS IN FINES LAST YEAR (2014) BY THE US GOVERNMENT FOR THIS EXACT ISSUE - BUT APPARENTLY IT DIDN'T FIX ITS 2015 LINE B/F LETTING IT GO TO MARKET. I PURCHASED THE TOYOTA PRIUS BECAUSE IT WAS RECEIVED TOP SAFETY AND RELIABILITY RATINGS. ONLY NOW DO I LEARN TOYOTA HAS A HISTORY OF HIDING UNINTENDED ACCELERATION BUGS IN ITS CAR DESIGNS FOR YEARS - AND BLAMING OWNERS FOR ITS RECKLESSLY UNSAFE AND THEREBY UNETHICAL AUTO DESIGNS (NO, THIS WASN'T CAUSED BY A STUCK FLOOR MAT). I AM FURIOUS. NO ONE CAN AFFORD TO BE BLAMED FOR VEHICULAR MANSLAUGHTER DUE TO MANUFACTURER NEGLIGENCE. I TRIED TO BREAK MULTIPLE TIMES, BUT THE CAR ACCELERATED THE 25' INTO THE WALL. ALL AIRBAGS DEPLOYED, AND FLUID GUSHED OUT OF THE FRONT END. THE CAR WAS DECLARED A TOTAL LOSS. I WAS TOLD BY TOYOTA AND MY INSURANCE COMPANY THAT I MUST HAVE ACCIDENTALLY ACCELERATED INSTEAD OF REPEATEDLY BREAKING. I AM GRATEFUL THAT I HIT A WALL INSTEAD OF PEOPLE. SOMEONE COULD EASILY HAVE DIED HAD I HAD SWITCHED FROM REVERSE GEAR TO FORWARD IN LESS CONFINED LOCATION. THE ENTIRE ACCIDENT LASTED 2 SECONDS, WITH NO OPPORTUNITY TO PULL THE EMERGENCY BREAK OR GO INTO NEUTRAL.
