BRAKES WERE APPLIED AT FULL FORCE DURING THE ACCELERATION WITH OUT EFFECT. VEHICLE CONTINUED DOWN THE DRIVEWAY, CROSSING OVER THE STREET, THEN JUMPED THE OPPOSING CURB, STRIKING A STONE WALL.

THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS 39,000. THE CURRENT MILEAGE WAS 49,000.

ON A FLAT SURFACE IN CRUISE CONTROL TRAVELING AT 40+MPH, BRAKES APPLIED, THEN RESUME IS ACTIVATED, AND VEHICLE SURGES AHEAD UNEXPECTEDLY. ON A HILL AT 50 MPH, WITH CRUISE CONTROL ON, THE VEHICLE SUDDENLY ACCELERATES EXCESSIVELY CAUSING THE VEHICLE TO VEER RIGHT OR LEFT. THIS CONDITION HAS EXISTED SINCE I PURCHASED THE VEHICLE NEW. AFTER COMPLYING NUMEROUS TIMES, THE AUTHORIZED TOYOTA DEALER COULD "DETECT NOTHING WRONG WITH THE CRUISE CONTROL". THEY DID NOT OFFER TO DO A TEST DRIVE. I HAVE PUT UP WITH THIS CONDITION FOR A LONG TIME, BUT THE PROBLEM IS NOW MUCH WORSE AND DANGEROUS TO DRIVE. *TR

WE SPOTTED DROPS OF OIL COMING FROM OUR TOYOTA AVALON 2006. A TOYOTA DEALERSHIP DIAGNOSED THE PROBLEM TO BE A SLOW OIL LEAK FROM THE FRONT TIMING COVER. THE DEALERSHIP ESTIMATES IT WILL COST OVER $2,200 TO RESEAL OR REPLACE THE COVER WHICH COSTS ABOUT $50. WE HAVE DECIDED, AS OF NOW, TO NOT SPEND THE MAJOR DOLLARS INVOLVED, AND KEEP A EYE ON THE OIL IN THE CAR. WE HAVE ALSO RESEARCHED THAT PARTICULAR PROBLEM IN THE 2006 AVALON, AND HAVE FOUND NHTSA TECHNICAL SERVICE BULLETIN, ITEM NUMBER 10021531, AND 2006 TOYOTA AVALON, SERVICE BULLETIN #: 01907. THE SUMMARY OF THIS BULLETIN READS: "ENGINE OIL LEAK FROM FRONT TIMING COVER." THE BULLETIN SEEMS TO BE DESCRIBING OUR PROBLEM EXACTLY. TL* THE CONTACT OWNS A 2006 TOYOTA AVALON. WHILE DRIVING AT APPROXIMATELY 5 MPH AN EXPLOSION OCCURRED IN THE ENGINE AND SMOKE BEGAN TO COME FROM UNDER THE HOOD. THE VEHICLE WAS PULLED OVER AND THE HIGHWAY PATROL APPEARED ON THE SCENE. A POLICE REPORT WAS FILED BUT NO INJURIES WERE REPORTED. IN JULY 2009 OUR 2006 TOYOTA AVALONXL HAD A VVT-I OIL HOSE EXPLODE CAUSING OIL DAMAGE AND PERSONAL REPAIR EXPENSE OF $277.67. I WAS NOTIFIED BY TOYOTA OF A RECALL TO REPLACE THIS HOSE. IT STATED IF ALREADY REPLACED THEN TO SEND IN THE REPAIR ORDER, PROOF OF PAYMENT AND PROOF OF OWNERSHIP FOR REIMBURSEMENT. THIS WAS SUBMITTED TO TOYOTA ON JANUARY 11, 2010. NO RESPONSE WAS RECEIVED UNTIL I CONTACTED THEM AFTER 71 DAYS. AFTER A LENGTHY DELAY, I WAS DENIED REIMBURSEMENT BECAUSE REPAIRS WERE NOT COMPLETED BY AN AUTHORIZED TOYOTA DEALER. TWO PREVIOUS AVALONS (97&98) AND THE ONE IDENTIFIED HERE, DID AND DOES ALLOW THE KEY TO BE REMOVED FROM THE IGNITION WITH THE CAR BEING IN PARK. THE DANGER IS THAT THE CAR WILL ROLL AND COULD RUN OVER SOMEONE OR INTO TRAFFIC AND CAUSE SERIOUS INJURY. MY HUSBAND AND I ALWAYS REMINDED EACH OTHER TO PUT THE CAR IN PARK, BUT HE IS NO LONGER WITH ME AND RECENTLY I STARTED TO GET OUT OF THE CAR AND IT WAS ON A SLIGHT INCLINE AND IT STARTED ROLLING WHILE I WAS GETTING OUT. I HAVE COMPLAINED IN THE PAST TO TOYOTA, BUT THEY DIDN'T BELIEVE ME. A RECENT COMPLAINT BROUGHT THE ANSWER THAT THE 36000 MILE WARRANTY DOESN'T APPLY TO THE REPAIR. I UNDERSTAND THAT, BUT THIS IS EVIDENTLY A SERIOUS ISSUE WITH AVALONS THAT TOYOTA HAS REFUSED TO ADDRESS, OR MY 2006 WOULDN'T HAVE THE SAME PROBLEM. WE HAVE ALMOST ALWAYS BOUGHT USED CARS AND TRUCKS AND HAVE NEVER HAD A VEHICLE THAT WOULD ALLOW THE KEY TO BE REMOVED WITHOUT BEING IN PARK. YOU ASK FOR A SPECIFIC DATE AT THE END OF THIS "STEP". IT IS ONGOING SO I HAVE JUST PUT YESTERDAYS DATE. THE CAR WAS A GIFT IN SEPTEMBER OF 2014.

TL* THE CONTACT OWNS A 2006 TOYOTA AVALON. THE CONTACT STATED THAT SHE HAD PROBLEMS WITH THE TRANSMISSION IN HER VEHICLE. THE STEERING COLUMN TELESCOPIC ADJUSTMENT LEVER HAS "SLIPPED OUT OF ITS LOCKED POSITION" FOR AT LEAST SEVERAL YEARS. The vehicles whole dash and lower part of dash has cracked everywhere. It’s gotten a sticky film on it which makes a glare on my windshield. Makes it difficult to see. If you are driving towards the sun it makes it very dangerous when you can’t see ahead of you.

DASHBOARD CRACKING IN TWO PLACES. OUR 2006 DASHBOARD IS CRACKING IN TWO PLACES, AREA BY DRIVERS DOOR AND IN THE PLACE WHERE THE PASSENGER AIR BAG IS LOCATED. OUR 2008 DID THE SAME THING AND TOYOTA REPLACED THE WHOLE DSAH BUT OUR 2006 IS TOO OLD TO BE COVERED. WE'RE CONCERNED THAT ONCE THE CRACK GOES ALL THE WAY THROUGH THE COMPARTMNT WHERE THE SIR BAG IS THAT IT WILL BUST OPEN HIT THE PASSENGER IN THE FACE.

TOYOTA COMPANY REFUSE TO HONOR MY COMPLAINT THAT I DID NOT RECEIVE ANY LETTERS REGARDING A RECALL ON MY 2006 TOYOTA AVALON. TOYOTA COMPANY STATES THAT THE RECALL ON MY 2006 AVALON HAS EXPIRED. TOYOTA COMPANY STATED THEY MAIL OUT A RECALL LETTER TO MY ADDRESS IN 2015 AND 2017. I DID NOT RECEIVE THE LETTER AT MY ADDRESS. I LIVE IN A MOBILE HOME PARK WHERE ITS ABOUT 30 MOBILE HOMES IS LOCATED WITH THE ADDRESS, EXCEPT THE LOT NUMBER IS DIFFERENT. I SEARCH THROUGH MY MAIL TO DOUBLE CHECK AND I DO NOT HAVE ANY LETTERS REGARDING A RECALL. I CALLED LAST YEAR (2018) BECAUSE I NOTICE SEVERAL SPLITTING LINE ACROSS THE DASH BOARD OF MY CAR DASHBOARD AND IT HAS A STICKY FEELING BY TOUCHING. I HAVE READ SEVERAL EMAILS STATING "ACCORDINGLY, PER WEBSITES ON RECALL THERE IS NO TIME LIMIT ON GETTING A RECALL FIXED, AND MOST DEALERS SHOULD HONOR THE RECALL AND FIX YOUR CAR FREE OF CHARGE. THE ONLY EXCEPTION IS IF YOUR VEHICLE IS OLDER THAN TEN YEARS AT THE TIME OF THE RECALL, THE DEALER MAY NOT FIX THE VEHICLE FOR FREE. REGARDLESS, YOU SHOULD STILL MAKE THE NECESSARY CHANGES OR CONTACT THE AUTOMAKER DIRECTLY AND LET THEM KNOW OF YOUR PREDICAMENT". MY PREDICAMENT IS I DO NOT WANT ANYTHING TO HAPPEN AND I CANNOT AFFORD TO PAY THE COST TO REPLACE THE DASHBOARD. ALL I WANT IS FOR THEM TO REALIZE THAT MY DEFECT FOR THE RECALL DID NOT SHOW UNTIL LAST YEAR. I GET MY CAR SERVICE EVERY YEAR AND THERE WAS NO VISIBLE DEFECT ON THE DASHBOARD.

I HAVE CALLED TOYOTA IN HOUSTON, TX SEVERAL TIMES ABOUT MY MELTING AND CRACKING DASHBOARD. I WAS TOLD THIS WAS NORMAL WEAR AND TEAR. MY CAR IS GARAGED, AND ONLY OUT FOR SHORT AMOUNTS OF DRIVING TIME. ON DECEMBER 31, 2014, I SAW ON THE NATIONAL NBC NEWS THAT THERE IS A RECALL FOR THE DASHBOARD PROBLEMS. I CALLED TOYOTA THIS MORNING, JAN. 2, 2015, AND WAS TOLD THEY KNOW NOTHING ABOUT A RECALL. TL* THE CONTACT OWNED A 2006 TOYOTA AVALON. WHILE DRIVING APPROXIMATELY 25 MPH, A VEHICLE MERGED INTO THE CONTACT'S LANE AND STRUCK THE FRONT DRIVER SIDE OF THE CONTACT'S VEHICLE. THE FRONTAL DRIVER AND FEET AIR BAGS DEPLOYED WITH GASES AND METAL FRAGMENTS, WHICH INJURED THE DRIVER. A POLICE REPORT WAS FILED. THE FIRE DEPARTMENT WAS ALSO PRESENT. WEATHER IS CLEAR, ROADS ARE CLEAR, HOWEVER, AREA HAD RECENT ICE AND SLEET BUT MOST HAS BEEN SNOW PLOWED TO THE ROAD SHOULDERS. TRAFFIC IS LIGHT, WITH VEHICLES AHEAD AND TO THE REAR. SPEED IS APPROXIMATELY 60 MPH AND I AM WATCHING THE ROAD CLOSELY. PASSENGER IN VEHICLE IS WITNESS. (2) I AM IMMEDIATELY UPON A CHUNK OF ICE ABOUT THE SIZE OF A BASKETBALL THAT APPEARED TO HAVE FALLEN FROM THE VEHICLE AHEAD. THERE WAS NO SWERVE BY THE VEHICLE AHEAD TO INDICATE AN ATTEMPT TO DODGE AN OBJECT IN THE ROAD. THE ICE IS SLIDING ON THE ROAD MOVING IN THE SAME DIRECTION AS THE TRAFFIC. I HIT THE ICE WITH MY VEHICLE JUST UNDER THE BUMPER AND INSIDE THE LEFT FRONT WHEEL. THE OBJECT WAS NOT ILLUMINATED BY MY VEHICLES HEADLIGHTS IN TIME FOR ME TO BRAKE OR MANEUVER AROUND IT. THE IMPACT BROKE MY FRONT BUMPER COWLING, UNDER ENGINE SHIELD AND RIGHT FRONT WHEEL MUD GUARD. (3) THE VEHICLE IS MOBILE AND NO PARTS LEFT THE VEHICLE AS A RESULT OF THE IMPACT. The contact mentioned waiting days later to start the vehicle and noticed the burnt odor coming from the vehicle. The dealer was contacted and informed the contact the price for a diagnosis for the harness wiring. The vehicle was not repaired. The manufacturer was not notified of the failure. WE ENDED UP HAVING AN INDEPENDENT MECHANIC OVERHAUL/CLEANUP THE TOP SIDE OF ENGINE FOR $4000. I HAVE THE ESTIMATES AND RECEIPTS FOR THE WORK IF NEEDED. I HAVE RESEARCHED ON-LINE AND FOUND MANY OTHER AVALON OWNERS WHO HAVE HAD THE SAME PROBLEM. THE VEHICLE FUNCTIONED FINE FOR A WHILE, BUT THEN THE SAME PROBLEMS QUICKLY RESURFACED. A TOYOTA REPRESENTATIVE STATED THE PROBLEM WAS NOT A DEFECT; RATHER IT WAS THE WAY THE VEHICLE WAS INTENDED TO HANDLE DUE TO ITS DRIVE-BY-WIRE SYSTEM. THE REPRESENTATIVE WENT ON TO SAY THE DRIVE-BY-WIRE SYSTEM DID NOT HAVE A TRADITIONAL ROD THAT CONNECTED THE PEDAL TO THE ENGINE; RATHER, THE PEDAL WENT TO AN ELECTRONIC SYSTEM THAT SENSED THE INTENDED ACCELERATION AND CAUSES THE ENGINE TO ACCELERATE AS DIRECTED BY THE DRIVER. THE CONSUMER WAS TOLD THE HESITATION HE COMPLAINED OF WAS NORMAL AS THE SYSTEM WAS DETERMINING HOW MUCH TO ACCELERATE. RE: TOYOTA 2006 AVALON I RECENTLY BOUGHT A TOYOTA AVALON. IT NOW HAS 3800 MILES ON IT. IN THE SECOND WEEK AFTER THE PURCHASE, I WAS DRIVING ON A RAMP TO ENTER A LIMITED ACCESS HIGH WAY. SPEED WAS ABOUT 50 MPH AS I BLENDED IN WITH THE FLOW OF TRAFFIC. THE LANE I WAS IN STARTED TO SLOW DOWN AND THE LANE TO MY LEFT HAD AN OPEN SPACE. I PRESSED ON THE ACCELERATOR TO GAIN SPEED AND TURNED THE STEERING WHEEL TOWARD THE LEFT LANE. THE CAR, INSTEAD OF ACCELERATING, APPEARED TO GO INTO NEUTRAL, THE TACHOMETER REACHED ALMOST 7000 RPM, THEN THE TRANSMISSION SHIFTED INTO THE PASSING GEAR AND LEAPED FORWARD. I ALMOST HIT THE REAR END OF THE CAR IN FRONT OF ME, BUT SINCE MY CAR HAD SLOWED DOWN A LITTLE I WAS ABLE TO STEER AROUND AND GET INTO THE LEFT LANE. I HAVE GONE INTO THIS DETAILED EXPLANATION TO DEMONSTRATE THE DANGER, AND POTENTIAL TERRIBLE ACCIDENT AT 50MPH THAT THE ??BUILT IN?? HESITATION OF THE 2006 TOYOTA AVALON COULD HAVE CAUSED. I WAS TRAVELING ON THE NY I-87 IN MY 2006 TOYOTA AVALON FOR APPROXIMATELY 30 MINUTES WHEN THE "CHECK ENGINE" LIGHT ILLUMINATED. WITHIN A MINUTE OR SO, THE "CHECK OIL" LIGHT STARTED BLINKING SLOWLY AT FIRST, THEN AFTER ABOUT 30 SECONDS IT WAS ON CONTINUOUSLY. BY THIS TIME I HAD PULLED OFF INTO A GAS STATION. AS I WAS PULLING INTO THE STATION THE ENGINE WAS MAKING A LOUD CLICKING NOISE. AFTER IT WAS PARKED, I CHECKED THE DIPSTICK AND THERE WAS NO OIL REMAINING IN THE ENGINE. THE UNDERSIDE OF THE CAR, INCLUDING INSIDE OF WHEELS AND BRAKES WAS COMPLETELY COATED WITH OIL AS WELL AS THE BACK OF THE TRUNK. PRESUMABLY FROM THE SPRAY UNDERNEATH WHILE TRAVELING @ 65-70MPH. I THEN PURCHASED 3 QUARTS OF OIL, PUT IT IN THE ENGINE AND IT DRAINED ONTO THE GROUND. THE MECHANIC STATED THERE IS A PIN HOLE IN THE RUBBER SECTION OF THE OIL LINE. THE USE OF A RUBBER HOSE FOR A PRESSURIZED FLAMMABLE LIQUID SYSTEM IN THIS LOCATION IS ABSOLUTELY RECKLESS. AT A MINIMUM, IT SHOULD BE SOME SORT OF ARMORED OR SHIELDED HOSE. I'M AMAZED THE CAR DIDN'T START ON FIRE FROM BEING DOUSED WITH FIVE QUARTS OF OIL LEAKING FROM THE RUPTURED HOSE. THE SERVICE PERSON AT THE DEALERSHIP INDICATED THIS IS A COMMON PROBLEM WITH 2006 AVALONS AND HE COULDN'T UNDERSTAND WHY THERE HASN'T BEEN A RECALL ISSUED. THE ENGINE REPAIRS CAUSED BY THIS DEFECT WILL COST ME UP TO $5000. TODAY I FOUND AT LEAST A 1000 COMPLAINTS REGARDING THIS SAME ISSUE ON CARCOMPLAINTS.COM AND ABOUTAUTOMOBILE.COM. AVALON LIMITED 3.2L V6 * AUTOMATIC TRANSMISSION * 72,900 MILES WELL, THIS IS MY 3RD TOYOTA, THE CAR WAS RUNNING FINE UP UNTIL THURSDAY NIGHT WHEN I STARTED THE CAR I HEARD A CRANKY FAN LIKE NOISE, I DID NOT KNOW WHAT IT WAS AFTER I DROVE SHOPPING AND CAME BACK AFTER 2-3 MILES AND PARKED THE CAR ON MY DRIVEWAY IN THE DARK. NEXT MORNING WHEN I PULLED MY CAR OUT OF THE DRIVEWAY THE NOISE CONTINUED AND I SAW A POOL OF ENGINE OIL ALL OVER, I CALLED THE DEALER IMMEDIATELY AND SURPRISINGLY THEY OFFERED TO REPAIR THE CAR THE SAME DAY (SAME DAY APPOINTMENTS-NEVER HAPPENED BEFORE EVEN FOR JUST OIL CHANGES) AFTER TAKING A LOOK THEY SAID THE OIL LINE NEEDS TO BE FIXED AND COST WOULD BE $450 FOR A HALF DAYS WORK. I CALLED BACK THE EVENING AND WAS TOLD NOW THEY CALLED TOYOTA AS THEY DON'T KNOW WHAT WAS WRONG BUT HAVE NOT HEARD FROM TOYOTA SO I WONT GET MY CAR UP UNTIL AFTER LABOR DAY WEEKEND- (HAD TO GET A RENTAL FOR A WEEK) NEXT WEEK, THEY TOLD BE THE GEAR SHAFT CRACKED AND GASKETS NEED REPLACEMENT AND A 16 HRS JOB AND WOULD COST APPROX $6000 TO REPAIR "BUT TOYOTA WILL COVER 50%" EVEN THOUGH I AM OUT OF 60,000 MILES WARRANTY. I DID SOME RESEARCH AND FOUND THIS IS A MANUFACTURER DEFECT. I WALKED IN TO DEALER TO SEE MY CAR AND THE MECHANIC SHOWED ME THE BROKEN PART AND TOLD ME TOYOTA HAS SINCE REDESIGNED THIS PART SO IT DOES NOT BREAK AND CAUSE SUDDEN OIL LEAKS- ONCE THE NEW PART IS REPLACED I SHOULD NOT EXPECT MORE SIMILAR PROBLEMS- I AM KEEPING MY FINGERS CROSSED. WHEN ENTERING TRAFFIC FROM THE ON RAMP AND WITH PLENTY OF TIME UNDER NORMAL CONDITIONS MY TOYOTA AVALON, UPON ACCELERATION THE ENGINE ACTUALLY CUT OUT I MEAN WITH A SERIOUS HESITATION THAT I COULD NOT RESPOND TO IN TIME. THE AUTO QUICKLY APPROACHING MY REAR VEERED INTO THE NEXT LANE CAUSING THE THIRD CAR TO RUN OFF ON THE SHOULDER OF THE ROAD. THIS IS INDEED A HIGHLY SAFETY PROBLEM. I OWNED A 2006 TOYOTA AVALON. WHILE DRIVING ON MOUNTAIN ROAD, AS I ROUNDED A CURVE, MY CAR SEEMED TO SURGE. A DOG CROSSED IN FRONT OF ME AS I CAME INTO THE CURVE. NOTICED THAT THE DASHBOARD SECTION AIR BAG (CONSOLE) ON PASSENGER SIDE HAS STARTED "CRACKING". WHEN FIRST NOTICED IT WAS APPROXIMATE ONE INCH IN LENGTH, & IS PROGRESSING CURRENTLY APPROXIMATE 2" INCHES IN LENGTH. I HAVE A SET OF 215/55-17 LIFELINER SLE TIRES BY COOPER TIRES. THEY WERE INSTALLED ON MY 2006 TOYOTA AVALON AT 19000 MILES. I HAVE 33000 ON THEM NOW. THEY WERE BALANCED AND ROTATED AT 10000 MILES. I HAVE MADE 3 COMPLAINTS TO DEALER ABOUT TIRES. THEY HAVE BEEN VERY ROUGH RIDING LIKE THEY HAVE KNOTS IN THEM SINCE THE DAY THEY WERE INSTALLED. I HAVE HAD ALIGNMENT CHECKED BY TOYOTA AND IT IS FINE. ON A TRIP FROM FLORIDA TO OHIO THERE WAS A HUMMING THAT STARTED BETWEEN 40 AN 60 MILES PER HOUR. WHEN IT WAS TAKEN TO THE DEALER THEY IDENTIFIED IT AS A RIGHT FRONT WHEEL BEARING THAT WAS DEFECTIVE. THE VEHICLE IS A 2006 TOYOTA AVALON WITH 44,500 MILES ON IT. TL* THE CONTACT OWNS A 2006 TOYOTA AVALON. THE CONTACT STATED THE ELECTRONIC STABILITY CONTROL GOES OUT ON HILLS AND LEAVES THE VEHICLE STRANDED. THE CONTACT STATED IT SHUTS THE VEHICLE DOWN AND THEY HAVE TO BACK DOWN AND AVOID HILLS WHEN DRIVING THE VEHICLE. THE CONTACT STATES THIS ONLY HAPPENS WHEN THE VEHICLE HAS TO BE STOPPED WHEN GOING UP A HILL. THE ENGINE WILL RUN FINE WHEN PLACED IN NEUTRAL OR REVERSE, BUT THE ENGINE SHUTS DOWN WHEN IN DRIVE. THE DEALER STATED THIS IS A FEATURE OF THE VEHICLE AND THERE IS NO REPAIR FOR THE FAILURE. THE FAILURE MILEAGE WAS 1,200 AND THE CURRENT MILEAGE WAS 25,900. UNINTENDED ACCELLERATION OF MY 2006 TOYOTA AVALON WITH ONLY 25,000 MILES ON IT. FEB 24, 2010. I WSA DRIVING 35 MPH, HAVING SLOWED FOR TRAFFIC, THEN CHANGED LANES, BARELY TOUCHED THE ACCELERATOR AND THE VEHICLE TOOK OFF LIKE A DRAGSTER. I WAS ABLE TO PLACE IN NETURAL AND BRAKE THE VEHICLE TO A STOP WHILE THE MOTOR WAS AT HIGH END REV. I CHECKED TO SEE IF THE FLOOR MATS WERE OBSTRUCTING THE GAS PEDEL. THEY WERE NOT. I TURNED OFF THE VEHICLE AND STARTED IT TWICE MORE, BOTH TIMES THE ENGINE WAS IN EXTREME HIGH REV MODE. AS I SAT ON THE HIGHWAY, THE ACCELERATOR "SPRANG" BACK ON IT'S OWN. I THEN STARTED THE ENGINE AGAIN AND IT WAS OPERATING NORMALLY. I CALLED TOYOTA IMMEDIATELY AND WAS DIRECTED TO A CUSTOMER SERVICE REP IN CALIFORNIA. I EVENTUALLY MADE IT TO A TOYOTA DEALER IN TULSA, OK, WHERE THE ON BOARD COMPUTER WAS "FLASHED" AND PEDEL RECALL WAS PERFORMED. SERVICE TECH WROTE UP A FLOORMAT ISSUE AND STATED THE "SERVICE ENGINE" LIGHT WAS REPORTED BY ME TO BE ON, WHICH IT WAS NOT. THIS SHOULD BE VERIFIED BY THE INCIDENT REPORT #1002144206 OF 2-24-2010 AS TAKEN OVER THE TELEPHONE BY TOYOTA CSR DON LOMBARDO; PHONE # 800-225-2769. I WOULD BET MY LIFE THIS WAS NOT A MECHANICAL RELATED CAUSE, BUT A PROGRAMMING ISSUE WHICH CAUSED THE THROTTLE BODY TO GO WIDE OPEN. The contact owned a 2006 Toyota Avalon. The contact stated that while at a complete stop, the vehicle accelerated independently without warning. The contact heard an abnormal noise as he pressed on the brake pedal; the vehicle eventually came to a stop. The contact was able to restart the vehicle and resume normal driving operation. The contact took the vehicle to a dealer and was informed that a mechanical fault was found with the vehicle. The manufacturer was also informed of the failure and a complaint was filed. The contact continued to experience multiple failures after the vehicle was inspected. On the latest incident, the contact stated that he had entered a car wash on the left lane and attempted to enter the right lane by reversing the vehicle. Once the vehicle was placed into drive, the vehicle made an abnormal, squealing noise and independently accelerated without warning into a concrete wall. The airbags did not deploy upon impact. A police report was filed with Apex City, NC police department. The contact sought medical attention from his doctor for the soreness to his right hand and right knee. The contact had the vehicle initial towed to a dealer where it was deemed a total loss. The vehicle was then towed to an independent tow yard. The manufacturer was notified of the failure and a report was filed. The vehicle was destroyed. The failure mileage was 155,835

ON APRIL 17, 2012 BAYSIDE TOYOTA REPLACED THE BRAKE MASTER CYLINDER SEAL ON THE AVALON UNDER RECALL# 10V499000. ALMOST EXACTLY 3 YEARS LATER ON APRIL 18, 2015 THE BRAKE MASTER CYLINDER SEAL FAILED AS I WAS BACKING THE CAR OUT OF MY GARAGE. I WAS ABLE TO USE MY PARKING BRAKE TO STOP THE VEHICLE. TOYOTA CLAIMS THAT ALTHOUGH THE ORIGINAL SEAL NEVER FAILED AFTER 6 YEARS OF SERVICE IT WAS REPLACED BECAUSE IT WAS A SAFETY CONCERN. WE DID FURTHER DIAGNOSTICS AND DISCOVERED THE 3 BOLTS THAT HOLD THE VARIABLE-VALVE TIMING (VVT) GEAR TO THE INTAKE CAMSHAFT CAME LOOSE, THESE BOLTS ARE SUPPOSED TO BE TEMPER-RESISTANT AND NEVER BE REMOVED WHICH LED ME TO BELIEVE THE RESULT OF ENGINE DYING IS CAUSED BY SERIOUS MANUFACTURING AND DESIGN DEFECTS. PLEASE REFER TO ATTACHED THREE PICTURES SHOWING 3 BOLTS LAYING NEXT TO VVT GEAR, VVT GEAR LOOSELY HELD ON AND THE VIBRATION OF BOLT LEFT CUTTING MARK ON THE GEAR, AND ALL INTAKE CAM ROCKERS FALLING OUT. IN JUST A LITTLE OVER A MONTH, LESS THAN 100 MILES, AND WITHOUT ANY TRAUMA (NO POTHOLES, CURBS, ETC), THE REAR PASSENGER TIRE HAD A MAJOR, SIDEWALL BLOWOUT ON THE HIGHWAY (70 MPH) WITH NEAR CATASTROPHIC CONSEQUENCES. CONTINENTAL WILL NOT ALLOW A REFUND BUT WILL ONLY PERMIT ME TO PURCHASE TIRES IN THEIR LINEUP ACCORDING TO THEIR 60 DAY POLICY. HOWEVER, THIS SHOULD BE AN EXCEPTION DUE TO THE NEAR MISS THAT COULD HAVE CAUSED SERIOUS BODILY HARM OR DEATH. TL* THE CONTACT OWNS A 2006 TOYOTA AVALON. WHILE DRIVING LESS THAN 5 MPH TO PULL INTO AN UNDERGROUND GARAGE AREA, THE ACCELERATOR PEDAL BECAME STUCK AND FAILED TO RELEASE. THE BRAKE PEDAL WAS APPLIED WITH FORCE, BUT THE VEHICLE LUNGED FORWARD AND CRASHED INTO THE GARAGE DOOR. UPON IMPACT, A YELLOW PIECE ASSOCIATED WITH THE FRONT PASSENGER SIDE AIR BAG FRACTURED. NONE OF THE AIR BAGS DEPLOYED. THE DRIVER SUSTAINED INJURIES TO THE HEAD AND NECK AND SUSTAINED BODILY SCRATCHES AND BRUISING, WHICH DID NOT REQUIRE MEDICAL ATTENTION. THE INSURANCE COMPANY WAS NOTIFIED OF THE FAILURE AND PLANNED TO INVESTIGATE THE CAUSE OF THE AIR BAG AND ACCELERATION FAILURES. A DEALER WAS NOT CONTACTED. THE VEHICLE WAS NOT INCLUDED IN NHTSA CAMPAIGN NUMBERS: 10V017000 (VEHICLE SPEED CONTROL) AND 09V388000 (VEHICLE SPEED CONTROL). THE MANUFACTURER WAS NOTIFIED OF THE FAILURES. CAR HAS 32000 MILES AND HAS BEEN KEPT IN A GARAGE SINCE NEW. DASHBOARD IS SO SHINY, WE CAN NO LONGER DRIVE IT. TOYOTA HAS DELAYED GETTING THE REPLACEMENT DASHBOARD FOR MOST OF 2015, AND NOW SAYS IT COULD BE ANOTHER YEAR TO YEAR AND A HALF. WE CANNOT DRIVE THIS CAR BECAUSE THE SUN SHINES OFF OF THE DASHBOARD, MAKING IT IMPOSSIBLE TO SEE. THE REFLECTION IN THE INSIDE OF THE WINDSHIELD IS SO INTENSE, THAT WE HAVE ELECTED NOT TO DRIVE IT (IT WAS OUR PRIMARY CAR), SO WE WENT OUT AND BOUGHT A NEW HONDA. I'M A DISABLED VETERAN, AND THIS REALLY HURT OUR BUDGET - BUT WE HAD TO HAVE A DEPENDABLE CAR. WHEN DRIVING IN HEAVY CONDITIONS AT SPEEDS UNDER 35 THE CAR HESISTATES BADLY WHEN YOU TRY TO ACCELERATE AND THEN "JUMPS" FORWARD. SAME THING HAPPENS ON THE DOWNSIDE TO THE POINT THAT IT FEELS LIKE ITS GOING TO STALL. THE SERVICE ADVISOR ALSO SAID THERE HAD BEEN OTHER COMPLAINTS ABOUT THIS CONDITION. ON 03/22/2010 TOYOTA REPLACED A VVTI OIL HOSE UNDER RECALL # 90K BECAUSE THE RUBBER PORTION OF THE HOSE WAS SUBJECT TO FAILURE THAT WOULD CAUSE THE ENGINE OIL TO SPAY OUT. 1. I WAS AT A STOPLIGHT, FOOT FIRMLY ON THE BRAKE, DOING NOTHING BUT WAITING FOR THE LIGHT TO CHANGE. SUDDENLY I REALIZED MY CAR WAS MOVING FORWARD, SO I PUSHED THE BRAKES HARDER. THE PEDAL WENT DOWN FURTHER BUT THE CAR KEPT MOVING FORWARD. ALL OF THIS TOOK ONLY A FEW SECONDS. THEN I HEARD AND FELT THE ENGINE ACCELERATING VERY LOUDLY. JUST AT THAT MOMENT THE LIGHT CHANGED, AND TRAFFIC SPED AHEAD. WITHOUT THINKING, I PUT MY FOOT ON THE ACCELERATOR AND THE ENGINE STOPPED ROARING, THE CAR ACCELERATED NORMALLY AND THE BRAKES WERE RESPONSIVE. 2. THIS WAS THE ONLY INCIDENT. 3. THIS IS NOT A COMPLAINT, BUT A FOLLOW UP TO COMPLAINTS REGARDING THE TOYOTA AUTOMOBILE THAT THEIR THROTTLE STICKS OPEN CAUSING UNWANTED ACCELERATION. MARCH 2006 I BOUGHT A NEW TOYOTA AVALON XLS FROM DAVID BRUCE, BOURBONNAIS, IL. ABOUT SIX MONTHS FOLLOWING THIS PURCHASE, I WAS PULLING AWAY FROM A RED LIGHT ACCELERATING RATHER QUICKLY. WHEN I LET UP ON THE ACCELERATOR, THE CAR CONTINUED TO ACCELERATE. WHILE APPLYING THE BRAKE WITH THE LEFT FOOT, I PUT MY RIGHT FOOT UNDER THE ACCELERATOR PETAL IN AN EFFORT TO RAISE IT. IT WAS NOT STUCK IN A DOWNWARD POSITION AND I WAS ABLE TO RAISE IT UP AND DOWN WITH MY FOOT WITH THE CAR STILL ACCELERATING. TL*THE CONTACT OWNS A 2006 TOYOTA AVALON. ON SEPTEMBER 4, 2009, HE STATED THAT THERE WAS A RETURN FUEL LINE IN THE FUEL PUMP AND WAS AT RISK OF CAUSING A FIRE IF THE LINE WERE TO BREAK. HE ALSO OBSERVED THAT THERE WAS AN OIL LEAK ON THE FLOOR AND CONFIRMED THAT THERE WAS NO OIL ON THE DIPSTICK AND A LARGE AMOUNT OF OIL LEAKED ONTO THE EXHAUST PIPE. THE VEHICLE WAS TOWED TO THE DEALER. THE DEALER STATED THAT THEY WOULD REPAIR THE VEHICLE PER THE EXTENDED WARRANTY AND THAT THERE WERE NO RELATED RECALLS. THE PART NECESSARY TO REPAIR THE VEHICLE IS ON BACK ORDER. THE CONTACT IS CONCERNED ABOUT THE SAFETY RISK AND THE POSSIBILITY THAT A FIRE COULD HAVE OCCURRED. THE FAILURE AND CURRENT MILEAGES WERE 77000.

I STARTED TO NOTICE A CLUNK WHEN I TURNED THE WHEEL TO THE LEFT WHICH I THOUGH T WAS UNUSUAL. IT CONCERNED ME THAT IT SOUNDED LIKE A WORN LINKAGE OR BALL JOINT AND THE CAR HAD ONLY 21,000 MILE ON IT. WHO INFORMED ME THAT THERE HAD BEEN A BAD LOT OF A STEERING LINKAGE PART PUT ON CERTAIN AVALONS THAT THEY COULDN'T IDENTIFY. THEREFORE, NO RECALL WHICH DISTURBED ME. I THOUGH TOYOTA WAS KNOWN FOR OUTSTANDING QUALITY! THE CAR NOW HAS 26,300 MILES AND THERE HAVE BEEN NO FURTHER ISSUES. *TR

TL*THE CONTACT OWNS A 2006 TOYOTA AVALON. WHILE ATTEMPTING TO ACCELERATE FROM A STOP, THE TIRE WOULD SPIN AND THE BRAKES WOULD AUTOMATICALLY APPLY. THE CONTACT COULD NOT FEEL THE VEHICLE SPIN, BUT THE WARNING INDICATOR LIGHT ILLUMINATED TO INDICATE THAT THE TRACTION CONTROL WAS APPLIED. THE DEALER STATED THAT THE FUNCTION WAS NORMAL AND THE TRACTION CONTROL COULD NOT BE DEACTIVATED. THE DESIGN FLAW OCCURRED ONLY IN THE SNOW. CAR DID NOT START FOR SEVERAL TRIES. ON ABOUT THE 5TH TRY, THE MOMENT I SHIFTED INTO REVERSE, THE THROTTLE STUCK AND THE CAR HURTLED OUT OF THE GARAGE. IT VEERED TO THE LEFT (AS YOU FACE THE GARAGE) IMMEDIATELY AFTER CLEARING THE GARAGE, CUT UNDER THE BRANCHES OF A BIG SPRUCE TREE ON THE CORNER OF MY NEIGHBOR'S LOT, CROSSED HIS DRIVEWAY, ACROSS A SMALL DRAINAGE DITCH AND SLAMMED INTO THE TRUNK OF A HUGE SPRUCE TREE. CAR STOPPED THEN, WITH EXTENSIVE DAMAGE TO REAR, AND ENGINE DIED. CAR WAS DRIVABLE AND I DROVE IT TO THE DEALER, WHO CHOSE TO NOT EXAMINE ANYTHING. DURING THE EVENT, I HAD MY FOOT ON THE BRAKE PEDAL BUT THE ANTI-LOCK BRAKES KEPT IT FROM LOCKING THE WHEELS AND I TRIED TO KILL THE ENGINE BY HITTING THE START BUTTON (THERE IS NO KEY ON THIS MODEL CAR). THAT DID NOT WORK AND I HIT THE TREE BEFORE I COULD THINK OF ANYTHING ELSE TO DO. I WAS NOT HURT. *JB

LTR FM (FL) RE THE DEFECTIVE TRANSMISSION IN HER 2006 TOYOTA AVALON. *TS THE VEHICLE WAS TAKEN TO THE DEALER OVER FIVE TIMES WITHIN IN A SIX WEEK PERIOD. THE CONSUMER STATED THE VEHICLE WOULD HESITATE AND THEN SURGE FORWARD WHEN GENTLY DEPRESSING THE ACCELERATOR. THE REAR SEAT BELT WAS DEFECTIVE AND THE FRONT PASSENGER AIR BAG SENSOR DIDN'T COME ON AT THE PROPER WEIGHT AS DESCRIBED BY THE SALESMAN. *JB
