What's wrong. Chrysler (FCA US, LLC) is recalling certain 2023 Chrysler Pacifica plug-in hybrid electric vehicles. An electrical short may occur due to an incorrectly installed 12-volt battery isolator post, resulting in a fire while parked or driving.

Risk. A fire while parked or driving can increase the risk of injury.

What's wrong. Chrysler (FCA US, LLC) is recalling certain 2020-2022 Pacifica Plug-In Hybrid Electric (PHEV) vehicles. The battery pack may cause a vehicle fire, even when the vehicle is parked with the ignition in the "Off" position.

Risk. A vehicle fire increases the risk of injury.

The second-row seat-to-floor mounting latch may bind and stick open, preventing the seat from properly securing to the floor. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."

Risk. An unsecured seat increases the risk of injury in a crash.

What's wrong. Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Risk. An unsecured wheelchair can move during transit, increasing the risk of injury.

What's wrong. Chrysler (FCA US LLC) is recalling certain 2018-2019 Pacifica vehicles equipped with an 8.4 inch radio display, 360 degree surround view camera system, and seat back video screens. A software error can cause the rearview camera image to remain displayed on the radio after the vehicle has been shifted out of reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rearview Mirrors."

Risk. The lingering rearview image can distract the driver, increasing the risk of a crash.

I pushed the pedal to the floor to accelerate to get out of his way but the vehicle did not respond, i pushed the pedal again on several occasions and the vehicle just bucked and did not accelerate. Unfortunately the vehicle hit me right in the door at a high rate of speed. Unbeknownst to me this vehicle had been in the shop on several occasions for this exact problem: This can all be verified due to the fact i had a Chrysler service manager on the phone as this horrific experience unfolded

We have had a few continuing problems since the original purchase of a 2017 chrysler pacifica braunability van. The car has had electrical issues and on 3 or 4 occasions has suddenly shut off or stopped working. One time may have been related to a car accident my husband was in. He collided with another car in 2018, immediately after the accident the car was not running, but he hadn't turned it off. It started back up again but this happened 2 or 3 times this year with different electrical features not working making the car inoperable at times. In addition, the air conditioning system has continuously been dysfunctional. We have had it to the dealership a few times for this problem. Most recently it was fixed a couple months ago and it is again not working.

Car kept shutting down regardless of speed. Chrysler sent specialist to deflect ship and replaced fuel pumps. Vehicle continued to shut down while driving. Attempted to drive to an independent mechanic, but crashed on the way. Car shut down on a curve, steering failed, brakes locked he hit a tree. This accident could've been a fatality. Car was totaled! 2021 Pacifica

The 2021 Chrysler Pacifica Hybrid will lose all power while driving. I have had it back to the Dealer for this Problem. I got a Recall for this in April. I took it in to the Dealer to do. They told me that Chrysler does not have a fix for this yet. Within 400 miles after that it done it again while my wife was driving and caused a crash.

2019 Pacifica

This van was purchased by a dealer with approximately 23,000 miles. It was sold to a retail customer and the following day it caught on fire in the customers garage. We had it towed to a chrysler dealership and chrysler said they would not accept any responsibility for fire, even though the vehicle is clearly under bumper to bumper warranty. After leaving it there while trying to get chrysler to fix it we finally sent a tow truck to pick it up. The tow truck driver got in the vehicle to put it in neutral and it caught on fire again. The entire front end of the vehicle is burner/melted and chrysler refuses to look at it or repair it under warranty.

The contact owns a 2017 Chrysler Pacifica. The contact stated that while his wife was attempting to park the vehicle with the brake pedal depressed, the engine unexpectedly began to rev. The vehicle then accelerated inadvertently, resulting in a collision with a pole in front of the parking space. There was no damage to the pole. The air bags did not deploy. The vehicle was towed to a collision center, and the insurance company declared it a total loss. No police report was filed. No injuries were sustained. A dealer was not contacted. The manufacturer was not notified of the failure. The failure mileage was 76,000. The VIN was not available.

While driving, the motor began to vibrate and lose power as if not firing on all cylinders, and acted as if it was going to stall in the lane of travel, causing me to have to pull off to the shoulder abruptly, striking a mailbox. The vehicle does not display any trouble lights or show any error codes on diagnostic equipment at an independent garage. Vehicle idles roughly.

30,390while cruising on interstate, (2/6/21, 3:17 pm, drizzling, 55 degrees f, at 70 mph in cruise control) instrument panel goes black, engine shuts off, power steering goes off, rt passenger window descends all the way, wipers go off, cruise goes off, gear indicator switches (on it's own from drive to neutral, to park. Coasting to side left of side of the (under construction) shoulder (rt shoulder is blocked by jersey barriers) instrument cluster comes back on. I shift from park to neutral, to drive, with no effect on drivability. I stopped off the roadway and shut off the ignition. Instrument cluster has battery and fcw warning lights on. There is a distinct smell of an electrical fire. I check under the hood to look for obvious fire sources. None are apparent. I checked the battery connection. This appears as normal for this vehicle, not tight ' but not loose either. I checked the fuse box. I did not have a fuse indicator tool with me and did not attempt to pull fuses. Seeing no obvious fire sources i shut the hood. Five minutes after stopping i tried the ignition again and the car started. The rt passenger window was to 3' short of closed, then it stopped. I pulled back on the highway and resumed the trip. The fcw warning light was still on. Cruise control was unavailable. The driver's side mirror warning indicator was on. On 2/7/21 i checked the car. All power devices on the rt front door were not active. I checked the fuses and determined that at least one had failed (f47). I attempted to replace the fuse. In doing so the rt passenger window immediately returned to the fully closed position, the fuse blew as i put it in, the smell of burning electrical wire was apparent inside the cab, but not under the hood.

We purchased the vehicle in sept 2017. About a month after we purchase the brand new van, we started having issue with the transmission slamming in to gear. The first time it happened, we literally thought the person behind us at rear ended up the jolt was that bad. Upon pulling over and inspecting, we discovered we werent. Over the last 23 months, we have had nothing but issues with the transmission. It shifts so at times it makes you dizzy. This takes place between 10-17 mph. In august 2018, our family was on our way to a fun day when at 10mph the van slammed so hard all 5 of us in the van felt our necks crack and we got dizzy instantly. Needless to say, we had to pull over because i couldnt see straight to drive and my elderly mother hurt for 3 days. I told her to go to the er for proof that the van is hurting the people driving it. We have called the dealership on several occasions and they wont do anything. Telling us they can reprogram the transmission. Give it time, the tranny needs to adjust how we drive never fixes the issue at all. We have an almost 2 year old toddler and we are petrified to turn her to forward facing in fear if the transmission slams, it would break her neck. Yes, the van had 32,000 miles or so on it but its our primary vehicle. We cant sit home and twiddle our thumbs. So, yes unfortunately we have to still drive this van, even if it kills us.

2021 Pacifica

This vehicle quit driving and you can move it. Confirmed bad transmission. I bought it this way from the insurance company because it bounced off a concrete barrier. I suspect this happened when the transmission failed. Unsure of a police report due to it happening to the previous owner.

2021 Pacifica

I stopped my 2021 Chrysler Pacifica, placed the transmission knob into park, opened the driver's door, leaving it opened with the engine running, and walked to the rear of the vehicle. I opened the tail gate and removed some boxes, threw the boxes into a dumpster, which was approximately 20 feet away and forward of the vehicle. I then returned to get two additional boxes from the rear of my vehicle. I retrieved the boxes and as I walked from the rear of the vehicle, it started to back up. I dropped the boxes, jumped out of the vehicle's path and tried to get into the driver's seat to stop the vehicle, but was unsuccessful as the vehicle picked up speed and the driver's door caught me and I fell. My vehicle was stopped after hitting a parked car in our church lot. Just a scratch on my vehicle's rear bumper, but frontend damage to the compact car it hit. No one else was in my vehicle. I then drove to get urgent care from cuts and bruises. (FYI - My insurance covers 100% of my care). I have aa service appointment to have the vehicle checked out tomorrow morning at San Antonio CDJ.

The following has happened twice. Once with me and once with my wife. The van is in reverse. My foot is on the brake. I am getting ready to reverse the van to get out of a parking space. Suddenly, the van lurches forward when I take my foot off the brake. The lurch is not soft of mild, it is shocking and frightening. In one case, we hit the car parked ahead of us. In both cases, had a person been in front of the car, he or she would have been surely injured.

While my wife was sitting in the car with the car running in park the car began to go in reverse and it started to accelerate and brakes didnt work and car crashed into a fence and shed. Multiple witnesses confirmed her account as she had window down and was screaming with her hands in the air. Once car came to a stop it was confirmed that car was in park. Very scary.

2017 Pacifica

The contact owns a 2017 Chrysler Pacifica. The contact stated that while his wife was attempting to park the vehicle with the brake pedal depressed, the engine unexpectedly began to rev. The vehicle then accelerated inadvertently, resulting in a collision with a pole in front of the parking space. There was no damage to the pole. The air bags did not deploy. The vehicle was towed to a collision center, and the insurance company declared it a total loss. No police report was filed. No injuries were sustained. A dealer was not contacted. The manufacturer was not notified of the failure. The failure mileage was 76,000. The VIN was not available.

While driving, the motor began to vibrate and lose power as if not firing on all cylinders, and acted as if it was going to stall in the lane of travel, causing me to have to pull off to the shoulder abruptly, striking a mailbox. 2019 Pacifica

While traveling in the left hand lane I was hit on the right side pushing me into a median. I applied the brakes and they didn’t stop the vehicle. When the van came into contact with the curb. The airbags deployed. I smelt smoke and told my 4 young children in the back seat we needed to get out now. Two of my children were in 5 pt harness car seats in the third row. We were able to run to a safe area before it went up in flames less than a few mins later. There were no witnesses and no one around. My children and I could of been seriously injured or worse. No warning lights were active. No signs of failure until I tried pressing the brakes. The vehicle was inspected by the fire department that arrived on scene when it was fully engulfed in flames at that point.

We purchased our vehicle on July 8, 2019 and it was 18,179 miles on August 9, 2021, is slightly over what would be considered eligible under the NY Lemon Law for new vehicles. We took our vehicle in to the Service Center for a routine inspection on August 9, 2021, because we realized our sticker expired only to learn that it would not pass inspection due to the check engine light coming on. After they diagnosed it, we were told the reason for the Check Engine light being on was some wires had supposedly been chewed by rodents. The service department repaired the wires and we paid $979.55 out of pocket for that repair. We were told to pick up the vehicle on August 11, 2021 (with an expired inspection) and drive it for approximately 100 miles and then bring it back in one week to see if they would be able to perform the inspection. After driving the vehicle for less than a week is when the nightmare begins! FIVE INCIDENTS LATER, the vehicle’s electrical system continues to dangerously malfunction WHILE WE ARE DRIVING. The vehicle still can not be inspected. The service department continues to give the vehicle back to us band-aiding every repair of the electrical system and the issue IS STILL NOT RESOLVED. The last incident, the 5th time, we took a trip to New Paltz NY and we received the dreaded Uconnect Service Alert about the Malfunction Indicator as soon as we arrived there. We immediately cut our trip short and attempted to go back to the Bronx to return the vehicle to the Dealer Service center. We called Eastchester Chrysler and left a voicemail that we were on our way to return the vehicle again. The vehicle started to malfunction severely as we were driving on I-87. Practically every system was indicating failure including the Air Bag, Brake system, Electronic Stability Control, Shifter, Steering, etc. The vehicle stalled near Exit 14A Chestnut Ridge. We had to pay $340 to be towed to our home in the Bronx, because it was too late to take it to the dealer.

We have had a few continuing problems since the original purchase of a 2017 chrysler pacifica braunability van. The car has had electrical issues and on 3 or 4 occasions has suddenly shut off or stopped working. One time may have been related to a car accident my husband was in. He collided with another car in 2018, immediately after the accident the car was not running, but he hadn't turned it off. It started back up again but this happened 2 or 3 times this year with different electrical features not working making the car inoperable at times. In addition, the air conditioning system has continuously been dysfunctional. We have had it to the dealership a few times for this problem. Most recently it was fixed a couple months ago and it is again not working.

Was involved in a passenger side impact collision while in motion on a city street and the side air bags did not deploy. In fact as a result of the collision, all electronics when haywire. Windows would not roll up/down, display on radio said something like no service, lights were blinking, and the car would not turn off. Impact occurred just behind the passenger front wheel and all the way to the rear wheel. Seems like the electronics could be better protected in a side impact collision. Luckily, there were no passengers on that side of the van. I was actually on my way to pick up the kids from school.

2022 Pacifica

The contact's mother had rented a 2022 Chrysler Pacifica Hybrid. The contact stated that upon his mother entering a gas station at a low rate of speed, she hit a divot in the road which triggered the vehicle's phantom braking to engage without warning. As the vehicle came to a sudden halt, the air bags deployed without incident. Due to the failure, his mother was taken to the hospital where she was treated for a concussion as well as multiple injuries to her neck, hips, nose, and right thumb; she also sustained severe burns and bruising to both hands and arms. A police report was filed. The vehicle was towed to an independent mechanic; however, the whereabouts of the vehicle are unknown. The dealer nor the manufacturer had yet to be notified of the failure. The failure mileage was unknown.

I was backing out of the driveway. I had all the safety features on, and I was checking my surroundings. Typically, the blind spot and front sensors would alert me to dangers. However, on this and many other occasions the warnings didn't alert me to danger on my passenger side that I couldn't see. this resulted in damage to the front quarter panel of my van as well as damage to the bumper. The body shop said it would cost at least $5000 to repair the damage. I can't afford that; in turn I am driving around with the damage.

This incidet happened on August 30, 10:30am PST. 2021 Chrysler Pacifica Plugin Hybrid vehicle while parked in the garage catches a fire due to Plugin Battery. The car was not in charging when incident happened. The car was last used previous dat afternoon around afternoon for a short trip of couple of miles only, and then parked inside the garage. At 9 am in the next day morning I was in the garage, and no smoke identified. At around 10:15am we noticed burning smell inside the house, and when we checked entire house inside and then garage, found garage was filled with smoke, and smoke is coming out of Chrysler pacifica vehicle. We immediately called 911, and fire department came immediately with in couple of minutes and started working on controlling the smoke and fire inside the car. Fire fighters noticed smoke is coming out of plugin battery, and tried to put out the smoke, they were continuously running water entire day, and could not stop the smoke/fire coming out of the battery. If they stop the water for few minutes, smoke starts again. They have been working on the car to put of smoke entire day, even called chrysler to understand on how to control the smoke out of the battery, no luck, At around 8pm, they have setup 2 hoses running water from the home to the car continuously, and check the car often to make sure it is under control. I will be checking all the night to make sure it won't caught the fire and cause additional damage to the house. My garage is filled with smoke, and my entire house is also had smoke. It is a nightmare situation for me and my family to experience this, I want to make sure every other family purchased 2021 chrysler pacifica plugin vehicle are safe.If we were not home or sleeping in the night, it could have been a much worse situation of losing the family and entire house due to the fire.

The 2021 Chrysler Pacifica Hybrid will lose all power while driving. I have had it back to the Dealer for this Problem. I got a Recall for this in April. I took it in to the Dealer to do. They told me that Chrysler does not have a fix for this yet. Within 400 miles after that it done it again while my wife was driving and caused a crash.

Description: My 3 year old daughter was getting out of the rear of the vehicle on the drivers side. The door is automatic and closes via button in the passenger compartment or the remote keyfob. She pushed the button to close the door and the door closed on her wrist and hand, the door did not automatically reverse and the door latched with her hand still inside. She was injured and needed to go to the hospital. This issue has also happened to several other people as well. There is a Facebook page dedicated to complaints on this Van. The door has closed on adults and children both. There were no warning lights, the vehicle has had a recall done on the auto open sensors under the doors. This issue occurred on April 11th 2023. The manufacturer was contacted and they have opened a case. The case number is 85938082

2022 Pacifica

Traveling down a country road on a clear afternoon, the vehicle traveled approximately 35 mph while cresting a small hill. On the other side of the hill was a "T" shaped intersection where another vehicle stopped in the intersection from a stop sign several feet behind. The other driver failed to complete the turn when they saw oncoming traffic. The driver noticed the intersection had an obstructed path and attempted to move into the oncoming traffic lane since no oncoming traffic was visible. Just then, the onboard computer installed in the control panel of the drivers seat had acted upon either the obstruction or the switch in lanes and assumed complete control of the braking and steering systems. The driver was unable to retrieve control from the computer systems artificial intelligence before a direct crash occured with a telephone pole. Rendering the vehicle dangerous as live wires draped the saturated earth below the tires and feet of passengers exiting the vehicle. The manufacturer was contacted, but no follow up has been made. Neither the police or insurance representatives have been helpful or any investigation made into this account. There were several lights, sensors, noises, and messages that were flashing and beeping but none were able to be understood before the sudden impact.

2022 Pacifica

The contact owned a 2022 Chrysler Pacifica. The contact stated that while driving approximately 55 MPH, a very loud snapping noise had suddenly occurred, and the message "Stability System Activated" displayed before the steering wheel suddenly turned to the right, causing the driver to lose control of the vehicle to crash and roll over multiple times. The police and first responders were called to the scene, and the occupants were transported to the hospital. During the crash, the driver sustained multiple neck and back fractures and a right leg injury from the seat belt strap. The front passenger sustained multiple rib fractures. The vehicle was destroyed and later towed away. The cause of the failure was not determined. The manufacturer and local dealer were not notified of the failure. The contact also indicated that due to the injuries sustained in the crash, his right hand is unusable. The VIN was not available. The failure mileage was 5,000.

My girlfriend was driving my pacifica. She experienced the car jump and she described it as being hit from behind. Then all of the sudden the vehicle forced itself to turn hard left and cause her to hit a box truck. She then bounced off that car and hit another vehicle. She said the brakes weren't working to good either. This car only had 11,186 miles on it. I have reported a similar issue of my vehicle jumping like it was pushed from behind or a jolt of power like the trans was slipping, only months into owning the vehicle. It was taken in and looked at more than once by my dealership, but they could not reproduce the problem, there were no codes, and therefore they couldn't find or fix the problem. Now the problem caused a car accident and luckily my kids were not in the vehicle. The car air bags also failed to deploy even though it experienced impact on both sides at 30-35 mph. The front end was destroyed pretty good, so i am in shock the air bags were not deployed. Now the dash indicates the air bags need service? After the accident the car would not shut down and 3 officers felt the car was malfunctioning. Upon looking into my service records i noticed something odd. My "chrysler app" shows nhsta recall # 18v-049 as being completed according to the vin. But my last visit to my dealer in feb of 2018 indicated there was no fix available at this time. How can it show completed then? I realized that the issues she experienced, and my self, are similar to the symptoms in this recall. There may be another similar...serious issue, that has to be looked into. Some type of malfunction caused this car to jump and drive right into another vehicle. Car kept shutting down regardless of speed. Chrysler sent specialist to deflect ship and replaced fuel pumps. Vehicle continued to shut down while driving. Attempted to drive to an independent mechanic, but crashed on the way. Car shut down on a curve, steering failed, brakes locked he hit a tree. This accident could've been a fatality. Car was totaled! On December 28, 2024 my family and I were in a crash with our Pacifica van. In a gradual left turn at approximately 40 mph, the van slowly drifted to the right. I slowly corrected to the left with no affect. After slightly more left correct, the van cut hard left and crashed head into a concrete wall on the opposite side of the roadway. The steering correction inputs should not have resulted in such a hard left turn. The drift to the right should not have occurred and I was not able to correct for some reason. The vehicle was inspected by insurance and totaled. No other inspections were completed to my knowledge. There were no issues or warnings of a potential problem. We did suspect the lane keep assist system could have caused an over correction but have no way to confirm that.

While driving above referenced vehicle, I was driving southbound on Hwy 515 when i noticed a vehicle driving northbound began to lose control, so i slowed down and then began to watch the vehicle cross the median and headed straight for me. I pushed the pedal to the floor to accelerate to get out of his way but the vehicle did not respond, i pushed the pedal again on several occasions and the vehicle just bucked and did not accelerate. Unfortunately the vehicle hit me right in the door at a high rate of speed. Unbeknownst to me this vehicle had been in the shop on several occasions for this exact problem: This can all be verified due to the fact i had a Chrysler service manager on the phone as this horrific experience unfolded

We were stopped for fuel. Prior to fueling we turned off the engine and opened the fuel door, inserting the gas nozzle and then began to pump fuel. While the nozzle was running, a passenger opened the sliding door by pressing the auto open button - and the door slid into the hand of the individual holding the fuel nozzle, crushing his hand between the door and the nozzle - causing the nozzle to dislodge from the filling neck , releasing gasoline all over the driver and j hiring his hand. There is no interlock preventing the door from auto opening while the fuel tank is open - which is a critical design flaw, that can result in injury to the individual fueling and hazardous material spills/ hazardous conditions where the pump attendant is directly drenched in fuel. A simple sensor on the door with a software interlock would resolve this issue and prevent future instances. In this case the injury was minor/bruise and discomfort, but it could have been more severe had the driver not reacted quickly, or had the driver had underlying medical conditions such as brittle bones or was on blood thinners.

While traveling on a dry highway, with good tires/new tires, and decelerating from 45 mph and approaching a red light, and a moderate, but firm pressure on the brake to bring the van to a full stop, the speed decreased to 15 mph, with the tachometer stable at 800 rpm and then to 10 mph, with both hands on the wheel, left foot on the dead pedal, and right foot on the middle of the brake pedal. As the speedometer made the transition at 10 MPH, the tachometer surged to 1500 RPM and then to 3000 in under 3 seconds, which cause me to initiate an emergency action drill of stabbing the brake pedal hard three times and rotating the transmission control to the lowest gear. The transmission refused to shift lower, and the control of the vehicle turned on oa wrestling match and the tachometer surged from 1500 RPM, them 3000 RPM, then 4500 RPM, and then 6000 RPM, each time the vehicle acting like it was repeatedly being rear-ended at a speed exceeding 15 MPH. The engine could not be disengaged and aceelaretd uncontrollably to over 40 MPH, through a busy intersection, and required over 24 hard break stages over a 10 second period to actually stop the car, and the engine refused to disengage until stopped. Once the car was restarted it immediately surged forward trying to fight through the hard applied brakes. The cruise control was never on the entire time. I then stut down the engine once I was able to wretle it to a stop with the engine climbing to over 4500 RPM. I then took this vehicle to a safe area, and repeated the conditions, and again as the vehilce speed dropped below 15 MPH, and certainly below 10 MPH, the egine tachometer increased in steps of 1500 MPH, 3000, 4500, 6000 RPM, and no control could be achieved other then violently "stabbing" the brake pedal. Under controlled conditions the behavior was repeatable over 20 times. This is a life threatenign defect, and it appears to be software in origin. I have CAN data streams from the events (23) and video.

While driving on interstate rt 80 i experienced a loud noise and smoke and flames were coming from the engine. Pulled over and got myself and my granddaughter out of her car seat. The engine continued to burn until the fire dept come. *tr

2023 Pacifica

While driving on the highway, the dashboard alerted that the engine would lose power, then the engine cut off and I had to coast the van to the shoulder of the highway. I was able to avoid hitting other cars, but there was no power steering or engine during this time. The vehicle has since been towed to our driveway, but it does not run. There was no warning lamp or messaging more than a few seconds before the engine quit.

2023 Pacifica

I was slowing down to turn left and felt a stutter, like when antilock brakes activate. Then a red triangle appeared on the display on the dash that said something like "pull over safely vehicle will turn off soon." I came to a stop to wait for oncoming traffic. When I hit the gas again it had barely any power and the power steering had disengaged. I was able to get through the left turn at about 3-5 mph. Shortly after I was out of the intersection (maybe 100 feet) the accelerator stopped giving any power. The vehicle was still able to turn on and I could use things like power windows, radio, and gear shifter (to go into neutral to push it to a better place to wait for a tow) but no thrust since. The mechanic said it seemed to be a problem with the hybrid battery and we towed it to the dealership since that was still under warranty. The dealership says it's the transmission and is in the process of figuring out which version of repairing it works for the warranty coverage.

2023 Pacifica

I purchased this car new on December 31, 2024. I have owned it for less than 16 months, and in that time, we have had an issue occur at least 5 times where we get an alert on the screen that says, "Stop Safely Vehicle Will Shut Off Soon". Less than 20 seconds later, the car completely dies. On each of the 5 instances, we were lucky to be going relatively slow and were in a safe place with a shoulder to pull over on. The vehicle was able to be turned back on within a few minutes. The check engine light came on momentarily, but would shut off before we were able to get the codes read. We brought this to the attention of Herb Chambers Chrysler Dodge Jeep RAM FIAT of Danvers (not where we bought the car, but the closest Chrysler dealership for maintenance) and they were unable to provide any help. They said they have never seen this, and since they couldn't get any codes, they didn't know what was going on. My research informs me that this is a known issue, so I'm disappointed that they didn't know about this. I am also terrified to drive my young children in this car, in fear that this will happen again when we aren't able to safety stop. I will provide details for the most recent time that this occurred, but I haven't previously reported the other occurrences.

We have a continuing issue with the Pacifica of showing a dashboard warning of "STOP SAFELY VEHICLE WILL SHUT OFF SOON''. The power becomes diminished and with a few minutes, all power is lost, including lights, engine, all dash warnings are gone. After sitting on the side of the road for a few minutes, the vehicle will restart like nothing happened. Occurrences shown in order by date, location and mileage. [XXX] in Gatlinburg, TN with approx 1800 miles. Dealer found error codes relating to hybrid system but could not repair for at least 4 weeks. Advised to continue trip and make an appointment when we return to Georgia. [XXX] on [XXX] enroute to Myrtle Beach the issue reoccurred [XXX] in for repairs at Chrysler 3206 miles. April 18, back to dealer same issue at 14,145 miles. Extensive repairs performed on hybrid system. April 24 with 14,303, same issue. Received vehicle back on June 20, 2025. [XXX], another occurrence with 15,645. This time the vehicle would not shut down by the FOB but would not function at all, though the dash stated put vehicle in park, but it was in park.. Ackerman Towing of Macon, GA attempted to tow vehicle, but the parking brake would not release nor would the transmission shift into neutral. After 3 hours, vehicle finally shut down and restarted. Back to Chrysler. Received vehicle back Aug 13. They could not find a problem. [XXX] enroute to VA Beach. Same issue at 17,750 miles on [XXX] outside of Lumberton, NC. Restarted after a few minutes. Continued trip. I feel this problem of shutting down is a death trap. Losing all power including outside lights could easily become catastrophic, especially if it happened after dark. Chrysler is aware and seems to be unable to find a remedy. I have more info, but out of characters to relay more. The rear brake at 4MM at 32K miles at the time of OIL change at dealer and front BRAKE still have 7MM, as a mechanical engineer, I know this should not happen. also while driving, there is high freq metal sound, I have notified the dealer every time I went to oil change, they confirmed it is fine but I do not feel safe. 2nd issue is the powertrain system feels no power while climbing hills and the car almost stops and sliding back even I put on gas pedal and almost hit the car behind me one time. 3rd issue is that gas door seal is broken within first year of use, it could cause gas leaking.

2023 Pacifica

On August 2, while traveling about 75 MPH, the check engine light came on along with the warning "Stop Safely – Vehicle Will Shut Off Soon". The vehicle started immediately losing power. We were driving in the fast lane in traffic so pulling over was difficult and dangerous. After we were to the side of the road, we turned the van off. After 10 minutes or so, we continued on and pulled over at a rest stop. We have an OBD2 and checked the codes. At first, there were no codes even though the check engine light was on. After checking a few times, there were 2 codes. They were P1C65 PD and P0A43. I took the van into the dealership on August 4. They saw one of the codes saved. On August 7 the dealership preformed a software update. They were not able to reproduce the issue. Today, September 19, while driving, the check engine light came on with the warning "Stop Safely – Vehicle Will Shut Off Soon". The vehicle again started immediately losing power. This time it happened when traveling about 45 MPH.

Just then, the onboard computer installed in the control panel of the drivers seat had acted upon either the obstruction or the switch in lanes and assumed complete control of the braking and steering systems. The driver was unable to retrieve control from the computer systems artificial intelligence before a direct crash occured with a telephone pole. Rendering the vehicle dangerous as live wires draped the saturated earth below the tires and feet of passengers exiting the vehicle. The manufacturer was contacted, but no follow up has been made. Neither the police or insurance representatives have been helpful or any investigation made into this account. There were several lights, sensors, noises, and messages that were flashing and beeping but none were able to be understood before the sudden impact.

2021 Pacifica

The contact owned a 2021 Chrysler Pacifica. The contact stated that occasionally, while driving at various speeds, upon depressing the brake pedal, the vehicle would surge forward 3 to 4 feet before proceeding to slow down or come to a complete stop. No warning lights were illuminated. The vehicle was taken to a local dealer, where it was diagnosed, and the contact was advised that the failure was not duplicatable and nothing was found to be wrong with the vehicle. The local dealer proceeded to replace the spark plugs, the brake fluid, field spark plug weld, and gasket cylinder head cover, however the contact stated that after the repair the failure increasingly persisted leading them to be involved in a crash where the contact stated that while driving on a highway at 55 MPH, there were two vehicles stopped ahead, the contact depressed the brake pedal and the vehicle proceeded to surge forward and crash into the vehicle Infront of them with the entire front end of their vehicle. The impact caused the vehicle in front of the contact to crash into the rear end of the vehicle in front. That vehicle sustained damage to the entire rear and front end of the vehicle as the impact caused that vehicle to crash into the rear end of the vehicle in front of it. No property damage was reported. The vehicles came to a stop upon rebound after the impact. The air bags were deployed. The vehicle was towed to an auto body center, and no warning lights were illuminated. A police report was filed. The vehicle was destroyed. The contact was injured while seated in the driver's seat; the passenger in their vehicle was not injured. An individual in the vehicle in front of the contact was injured. The contact sustained injuries to their ribs and sternum. Medical treatment was needed for the injuries, and the other individual also required medical treatment for unknown injuries. The individual at the front of the line in this collision was not injured. The local dealer was contacted however, the vehicle was not diagnosed as it was declared a total loss. The approximate failure mileage was 82,500.

In the past month i had experienced on two occasions, (while in motion and on a city street, specifically on the cross streets of dysart and camelback and a light at dysart and w. Sonoma dr.) that when i engaged the breaks the vehicle failed to slow and i felt as if the breaks were not working and the vehicle was not going to come to a stop. In the same month, approximately a week and a half after the last incident, on december 18th, 2019, my chrysler, pacifica was involved in a motor vehicle accident. Two vehicles in front of me came to an immediate stop. I applied the break but my vehicle began sliding and the abs system did not seem to engage. The accident occurred on litchfield and camelback. My insurance company is conducting an investigation on the accident currently. I feel that it is important that i disclose the problems i had noticed and was ultimately too late to discuss with chrysler. I do not want this issue to occur to anyone else.

While traveling in the left hand lane I was hit on the right side pushing me into a median. I applied the brakes and they didn’t stop the vehicle. When the van came into contact with the curb. The airbags deployed. I smelt smoke and told my 4 young children in the back seat we needed to get out now. Two of my children were in 5 pt harness car seats in the third row. We were able to run to a safe area before it went up in flames less than a few mins later. There were no witnesses and no one around. My children and I could of been seriously injured or worse. No warning lights were active. No signs of failure until I tried pressing the brakes. The vehicle was inspected by the fire department that arrived on scene when it was fully engulfed in flames at that point.

My girlfriend was driving my pacifica. She experienced the car jump and she described it as being hit from behind. Then all of the sudden the vehicle forced itself to turn hard left and cause her to hit a box truck. She then bounced off that car and hit another vehicle. She said the brakes weren't working to good either. This car only had 11,186 miles on it. I have reported a similar issue of my vehicle jumping like it was pushed from behind or a jolt of power like the trans was slipping, only months into owning the vehicle. It was taken in and looked at more than once by my dealership, but they could not reproduce the problem, there were no codes, and therefore they couldn't find or fix the problem. Now the problem caused a car accident and luckily my kids were not in the vehicle. The car air bags also failed to deploy even though it experienced impact on both sides at 30-35 mph. The front end was destroyed pretty good, so i am in shock the air bags were not deployed. Now the dash indicates the air bags need service? After the accident the car would not shut down and 3 officers felt the car was malfunctioning. Upon looking into my service records i noticed something odd. My "chrysler app" shows nhsta recall # 18v-049 as being completed according to the vin. But my last visit to my dealer in feb of 2018 indicated there was no fix available at this time. How can it show completed then? I realized that the issues she experienced, and my self, are similar to the symptoms in this recall. There may be another similar...serious issue, that has to be looked into. Some type of malfunction caused this car to jump and drive right into another vehicle. Car kept shutting down regardless of speed. Chrysler sent specialist to deflect ship and replaced fuel pumps. Vehicle continued to shut down while driving. Attempted to drive to an independent mechanic, but crashed on the way. Car shut down on a curve, steering failed, brakes locked he hit a tree. This accident could've been a fatality. Car was totaled! The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving at 80 MPH with the cruise control activated, the vehicle experienced phantom braking. The following day, while driving at an undisclosed speed, the vehicle was vibrating and the brakes failed to immediately stop the vehicle. The contact stated that the brakes caught fire, and there were flames coming from the brake calipers. The contact was able to put out the fire with a hose. The vehicle was towed to the dealer, where it remained for twenty-one days; however, no cause of failure was found. The failure recurred, and the vehicle was taken to an independent mechanic, who recommended the contact stop driving the vehicle as it was unsafe on the roadway. The vehicle was not repaired. The 2021 Chrysler Pacifica Hybrid will lose all power while driving. I have had it back to the Dealer for this Problem. I got a Recall for this in April. I took it in to the Dealer to do. They told me that Chrysler does not have a fix for this yet. Within 400 miles after that it done it again while my wife was driving and caused a crash.

I was on the highway traveling at 60mph, with the adaptive cruise control on. The semi in front of me began to brake, I tapped off the cruise control and began braking. The semi came to a complete stop and I pushed my brake pedal to the floor to stop my vehicle. It did not stop. My Pinnacle has emergency braking and forward collision warning. My driver's instrument panel did not give a forward collision warning indication in a timely manner and my emergency braking did not stop my vehicle. At the time of the accident vehicle had less than 10,000 miles, dry weather conditions, and vehicle was used for leisure - in town driving. A month previously, I took my Pinnacle into the dealership because I didn't feel my brakes were working properly. They felt mushy, little resistance, and were making a noise. An inspection was performed by the dealership but were unable to detect a noise or visual issue. My safety was put at risk when a system I depended upon, that was advertised as, "Full speed forward collision warning with active braking assists you by providing both visual and audible warnings as well as applying emergency braking to help bring the vehicle to a full stop, if needed" (chrysler.com/pacifica/safey-security.html) failed. The brakes didn't engage properly to stop us before hitting the vehicle in front of us. The forward collision warning indication didn't happen in a timely manner, it was delayed causing us to lose precious seconds. If this had been a passenger vehicle in front of us we could have injured the occupants and we ourselves could have been gravely injured had we been traveling at a higher rate of speed. The problem has not been confirmed by a dealership, but was taken there prior to the incident less than a month before. I reported the problem to Chrysler the day of the accident. The vehicle is pending inspection by insurance and I have asked that the Electronic Data Recorder be removed and the information on it to be evaluated.

While operating the vehicle - car unexpectedly accelerated! I quickly reacted and brought a car to a complete stop. Gas pedal broke off. Police and auto-insurer were notified. Police officer stated that broken gas pedal while driving = severe manufacturing defect; and that i am very lucky to come out alive. No one wants to bother with the repair ' now returning this defective product to a loan-lender requesting terminate lease due to a car defect. Leasing agent'notified, doesn't respond. Dealership'refused to accept, each time i call ' requests to deal with manufacturer. Chrysler ' manufacturer-was notified; case was opened = in 2 hrs-matter escalated to the highest department supposedly, and that i would be contacted. My numerous and continues attempts for 2 months = customer care said repeatedly that, as to my particular case, they are instructed to only take my messages; would not transfer to any of the supervisors and declined providing any info. I've been repeatedly told that my case is with a special investigation unit; as i must wait for their call. At each call i'm denied any information or help as to that special investigation unit's address, name of the department, last name of the assigned agent, his title, phone number, contact info, everything literally. Lately, they've been simply hanging up on me. One time one rep said that legal department is holding the process. They reconfirmed-all my messages were delivered and accepted by my assigned case manager, who called our home once - may 9th w/out leaving a message. When i called displayed 586-274-8000 as chrystler grp, it said 'old chrysler non-working number'. Auto-insurer requires full coverage regardless of a defect. Auto-policy adjuster ' can not repair for it's manufacturing defect. Loan-lender demands payments, knows defect, says 'doesn't care' and not helping to get through to chrysler.

On thursday feb. 15, 2018 in harlingen, tx i entered the expressway then planned to immediately get off as this bypasses a twisty frontage road. When we (my wife and another couple) got to the top of the ramp and on the exit lane, we could see that down the expressway there was an accident and all three lanes were filled with stopped traffic. As we approached the exit, the car suddenly started to accelerate at a rapid rate. I applied the brake and it would not work. I tried several times to try to stop the car, but couldn't. It continued to rapidly accelerate. We went from 20 mph to very fast in a matter of feet. We had nowhere to go. We hit a tan car on the left back side which in turn flipped our car. It rolled several times then came to a stop on its left side up against the cement barrier. The car filled with a toxic smoke. It turned out to be what was in the air bags. We could hear people outside trying to break the windshield to get us out, but it wouldn't break. Several men reached up and held the door open which was on top now. They reached in and pulled my wife out. She was ok. The men opened the back hatch and got the rest out that way. We were taken to the hospital by ambulance. I refused treatment as i was fine. My wife was in a head brace as she said my neck was a bit sore. It turned out that she had a cervical strain and some bruising on her leg, arm and stomach. Our friend had some chest contusions, but was ok. His wife however, was in a lot of pain. Her stomach was swelling and she had bruising from the seat belt. She also had a compressed # l12 vertebrae. She was admitted and spent the night in the icu. The next day her husband said she was still in a lot of pain, but was out of the icu. She was released from the hospital wed. 09/17/23 Was driving 65mph, car was slowing Down in front of me and turned cruise control off. Immediately car started stalling. Was in middle lane with cars on both sides. Turned on flashers and no power in gas pedal. At about 30mph, was struck from behind by Jeep! Third car behind struck Jeep and Jeep struck me again. There were injuries and a death. My car was towed. 10/02/23; Now car is in body shop. Contacted Chrysler, they inspected car, but did not drive it. 10/12/23 Just received email stating that they found no problem. Description: My 3 year old daughter was getting out of the rear of the vehicle on the drivers side. The door is automatic and closes via button in the passenger compartment or the remote keyfob. She pushed the button to close the door and the door closed on her wrist and hand, the door did not automatically reverse and the door latched with her hand still inside. She was injured and needed to go to the hospital. This issue has also happened to several other people as well. There is a Facebook page dedicated to complaints on this Van. The door has closed on adults and children both. There were no warning lights, the vehicle has had a recall done on the auto open sensors under the doors. This issue occurred on April 11th 2023. The manufacturer was contacted and they have opened a case. The case number is 85938082

2019 Pacifica

This van was purchased by a dealer with approximately 23,000 miles. It was sold to a retail customer and the following day it caught on fire in the customers garage. We had it towed to a chrysler dealership and chrysler said they would not accept any responsibility for fire, even though the vehicle is clearly under bumper to bumper warranty. After leaving it there while trying to get chrysler to fix it we finally sent a tow truck to pick it up. The tow truck driver got in the vehicle to put it in neutral and it caught on fire again. The entire front end of the vehicle is burner/melted and chrysler refuses to look at it or repair it under warranty.

Description of Incident: On February 15, 2026, my 2017 Chrysler Pacifica Hybrid experienced three distinct and severe safety system failures involving the electrical system, power seats, and the power sliding door. Power Sliding Door Sensor Failure: While my minor son was entering the vehicle, the power sliding door closed on his leg. The pinch sensor failed to detect the obstruction and did not reverse. This resulted in physical bruising and marks on the limb (photographic evidence maintained). Unintended Passenger Seat Motion: While the vehicle was stationary, the front passenger power seat moved forward to its maximum limit without user input. I was seated in the chair at the time and was pinned against the dashboard before I could intervene. Electrical Shock: My minor daughter received a perceptible electric shock upon touching the exterior door-handle button to open the sliding door. Components Involved: Electrical System Seats (Power) Sliding Door (Power) Injuries: Yes (Soft tissue bruising/marks to minor’s leg). Risk to Safety: Had the driver's seat moved forward without input while the vehicle was in motion, this absolutely could result in loss of control of the vehicle and an accident. Had the sliding side door continued to close on my son's leg, it could have caused severe injury. Had the electrical shock been worse, it could have resulted in burn or injury. The vehicle is available for inspection. It has not yet been inspected by the manufacturer, police, or insurance. The issues have not repeated at this point, although the vehicle was not used the last 12 days. I do not have confidence in its electrical components. There were no warning lamps, messages, or other indications of any problem with the vehicle.

This car was bought used in 2020. It had less than 15000 miles on it at that time and was still under original warrantee. I took it to a Chrysler dealership for some minor repairs which they resolved. I informed them that there was a rapid acceleration issue which they couldn't resolve. For the last year it has continued. It only happens at low speeds an is definitely not a pedal issue I recently happened while pulling into a handicap spot and I hit a pole. It was fortunate that it wasn't a crosswalk or some on could have been seriously injured.

The van lost control on its own and would not allow me to break whatsoever, until i got into a very dangerous accident. I was told by fiat chrysler automobiles that there was nothing wrong with the car, but there certainly was something wrong with the breaks. There was no issue with my driving, nor was there any issue with the weather. I was on my way to my son's doctor appointment, pulled into the hospital/clinic parking lot, parked my vehicle and turned it off. I heard a pop after i turned it off and when i stepped outside, i smelled smoke. Dark smoke was coming up both sides of the hood so i got my son out of his car seat, ran away from the vehicle and called 911. In a matter of minutes the front of the vehicle was in flames. The fire department was quick on the scene and put out the fire, but it was gas so it took some time to put out. Vehicle is total loss. I contacted chrysler and they have not called me. An inspector they hired has and is looking at the vehicle this week.

As a result, the latch would tighten when an object touched the lock, even as the door was locked. I purchased my 2007 chrysler pacifica at a dealer ship in february 2017. I have just had the vehicle for 5 months. The car had everything running and when i say everything running; i mean a/c was working just fine as well as the heater and all windows rolled up and down. I had everything updated on this vehicle. I am a single mother of 3 that works a double shift just to make a way in life. I am very disappointed in this experience and fear for my life from here on out. I am scared to drive or get in any car because mind you my car had been driving perfectly fine. I am disgusted at the fact that there are cruel people out here with license dealerships that are able to sell vehicles with vehicles that can cost someone's life. This is not acceptable and i am requesting an explanation to why this happened and i would like some kind of reimbursement for my vehicle.
