What's wrong. Forest River, Inc. (Forest River) is recalling certain 2026 Flagstaff, and Rockwood recreational trailers. The waste water holding tank ventilation pipe may not be terminated outside the vehicle, which can lead to sewer gas entering the vehicle.

Risk. Sewer gas that enters the vehicle increases the risk of injury.

(Forest River) is recalling certain 2019-2020 Flagstaff and Cherokee Arctic Wolf recreational trailers equipped with MORyde Orbital or You Turn Rubber Pin Box Wedge Kits. The pin boxes may have been equipped with bolts 1-3/4" in length when they should be equipped with 2" long bolts.

Risk. The incorrect bolts may allow the rotating pin box to rotate in two places, increasing the risk of a crash.

The Federal Placard may indicate an incorrect Gross Axle Weight Rating (GAWR) of 2,200 lbs when the correct GAWR actually is 3,000 lbs. As such, these vehicles fail to comply with the requirements of 49 CFR Part 567, "Certification."

Risk. Inaccurate GAWR information on the Federal placard can increase the risk of a crash.

What's wrong. Forest River, Inc. (Forest River) is recalling certain model year 2017 Rockwood and Flagstaff trailers manufactured July 8, 2016, to July 11, 2016. The affected vehicles are equipped with a Federal Placard that incorrectly indicates that the ST225/75R15D tires have a recommended pressure of 50 PSI. These tires should be maintained at 65PSI. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 110, "Tire Selection and Rims."

Risk. If the tires are run at a low pressure, they may overheat and fail, possibly causing a loss of vehicle control and increasing the risk of a crash.

What's wrong. Forest River, Inc. (Forest River) is recalling certain 2016 Rockwood RLT2604WS, WRLT2604WS, Flagstaff FLT26RLWS and WFLT26RLWS recreational trailers. Due to insufficient clearance between the front tire and slide out actuator, the tire may contact the slide out bracket and become damaged.

Risk. If the front tire becomes damaged, it may fail and cause a loss control of the vehicle, increasing the risk of a crash.

Fix. Forest River will notify owners, and dealers will install a lift kit that will provide the proper clearance between the tire and the slide out bracket. Damaged tires will be replaced. 2020 Flagstaff

Subject Castle Rock Radial ST 205/75/R14 tire was running 10 degrees F. hotter (around 110 F) than the three sibling tires after about 30 minutes of travel. This had been the case for an earlier trip of approximately 275 interstate miles. All of these tires are original equipment and similarly used and maintained. The tire eventually exploded (see uploaded pics). Subsequent inspection shows a bubble had formed between layers of the tread portion of the tire. The failed tire is available for inspection. Tire pressure before traveling in the morning was 65 p.s.i. (this is consistently maintained when in use). The load/speed ratings of these tires are D105/101L or 2039 lbs/75 m.p.h. Our running speed is typically 65 m.p.h. or less. The trailer weighs approximately 5600 pounds as loaded. Likely due to a combination of this safety margin in both weight and speed along with a weight distributing hitch with a stabilizer bar, we did not lose control of the vehicle when the tire failed. We were able to pull onto the emergency apron of the interstate immediately and replace the failed tire with a spare tire (not the original spare). The original matching spare with zero miles of use on the road suffered a similar fate (bubble/tire layer separation) in 2022 (NHTSA Reference No. 11490675). The failed tire was taken to a nearby tire center where it was inspected and replaced with a new tire. Subsequent to this trip, the remaining three (3) OEM Castle Rock tires have been replaced due to their high failure rate (40% to date for us).

2020 Flagstaff

The contact owns a 2020 Forest River Flagstaff. The contact stated that when he had inspected the RV spare tire prior to a trip, he noticed that the spare had a blister on the tread. The contact stated that the blister extends from the inner side of the tread to the outer and is approximately five inches wide. The contact stated the the tire DOT number is JUJTYMT 2819. The contact had taken the vehicle to a local dealer where it was not diagnosed the tire had been replaced. The vehicle had been repaired. The manufacturer had not been informed of the failure. The failure mileage was 0, the tire had never been used and was original equipment.

2020 Flagstaff

What component or system failed or malfunctioned, and is it available for inspection upon request -Tire. -I will keep the tire until 11/1/22. I will also have the undamaged spare available of the same make and model if needed. This will accompany a separate report with nearly identical language for the other trailer tire that failed about 60 miles later. How was your safety or the safety of others put at risk? -Tire failed on the interstate at Highway speed at 70MPH with a load pop sound, on an 86-degree, sunny, and dry day. The tire immediately delaminated all center tread between cap ply and center fabric belts. This sent debris, including wheel well components of the trailer, across multiple lanes of traffic. Thanks to high visibility and light traffic conditions surrounding vehicles slowed to avoid debris, tow vehicle, and trailer. An educated driver and a weight-distributing hitch recovered the trailer from the fishing condition in the adjacent lane. Has the problem been reproduced or confirmed by a dealer or independent service center? -No. However, many trailer owners have complained on various forums dubbing this tire a "china bomb" Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? -Since the original travel trailer purchase from the dealer in 2020. All tires including the spare did not hold air as well as a typical car tire. We would typically we would need to add air 10-20 PSI before embarking on a trip, and needed about 40 psi at the start of the camping season. No active monitoring systems are present on the trailer. No leak locations were found on tread using the soap and water method, and no handling issues existed prior to failure. Tires inflated to Manufacture recommended pressure 65PSI before all departures.

2020 Flagstaff

ST225/75R15 - had blowout on front driver side of RV that has dual axles. After putting spare tire on, I had gone less than 30 minutes down I-75 and had a second blowout on rear passenger tire as I pulled into a Camping World to get new spare. After 3rd tire was determined to be bad, ended up replacing all tires with Goodyear Endurance tires. Fortunately they had 4 in stock. I initially bought the RV brand new in July 2019 and tires only had about 3,000 miles on them. I am part of several Facebook RV groups and heard of this happening all the times re: “China bomb” tires. Never felt it would happen to me. I feel that with these castle rock tires having such a bad history, then RV manufacturers should be putting better tires on. And/or RV dealers should replace upon arrival to their sales lot.

2019 Flagstaff

The contact owns a 2019 Forest River Flagstaff Micro Lite equipped with Lionshead Tires, Tire Line: Castle Rock, Tire Size: 205/75/R14, DOT number: UJTYMT418. The contact stated that while driving 40-55 MPH, the driver's side rear tire went flat. The tire pressure monitoring system was beeping. The vehicle was taken to an independent mechanic and the contact was informed that the tread was separating. The tire was replaced however while driving at an undisclosed speed the TPMS was beeping and the driver heard an abnormal sound. The contact pulled over and noticed that the passenger’s side rear tire was flat. The contact stated that while the vehicle was parked, the passenger’s side front tire went flat. The manufacturer was not contacted. The tire failure mileage was approximately 700.

I noticed bulging of the spare tire cover mounted to the back of the travel trailer. When I removed the cover I saw that a chunk of the thread was missing and large gaps were in the thread next to it. It was a brand new tire that was never on the ground. The tire was inflated to the proper air pressure. The tire is a Castle Rock trailer tire size ST205/75 R14. ST 226.

ST 205 75/R14 June 17, 2022 Towing the trailer at 45 mph, the Right tire on the trailer began to wobble then exploded. Lug nuts were confirmed to be tight after the explosion. The trailer and tires were inspected at a PA State Inspection station the week prior. Tire is being replaced. No warning lamps are associated with the travel trailer tires.

On 27-apr-2018 i purchased a new flagstaff 832okbs. The tires on this travel trailer were castle rock (lions head tires) st22575r15 load range e 10 ply tires. On the first day of a recent trip, about 28 miles from home, the right rear tire on the travel trailer exploded. I pulled off the road to assess the problem and could not believe the condition of the tire and the damage to the fender area of the trailer. I inspected the other tires and did not see any other damage so i installed my spare to resume our trip we got back on the road and i decided to stop at the next rest area (24 miles from the first blowout) to check everything out and re-torque the lug nuts on the wheel i changed. As i was pulling into a parking spot the left front trailer tire blew. These tires all had 2017 date stamps and had approximately 4235 miles. I checked all the tire pressures before we left and they were properly inflated to 80 psi. I had recently weighted the trailer loaded at a pilot truck stop and it weighted 9080lbs, which is within the safety rating of the tires. After my tire failure i did some research online and found numerous accounts of the castle rock st22575r15 failing catastrophically. I filed a claim with lions head and they agreed to reimburse me for all five tires but would not cover the damage to my trailer. Vehicle damage is not covered under there warranty. I believe these tires pose a significant risk to life and property and should be recalled and replaced.

2021 Flagstaff

On the morning May 22nd, just 45 minutes after we left our home in Portland, our trailer burst into flames on 1-84 off exit 44 near Cascade Locks. It was inexplicable as to why this happened. It was a brand new trailer with less than 2k miles on it and purchased in December 2020. As you can see from the pictures, it was a total loss. We are filing this report to help protect others. We are a part of the forest river group and quality seems to be a concern. Demand for these vehicles have skyrocketed and many feel that QA has been downgraded in order to get these trailers to market.

The contact stated while driving 65 MPH and towing the trailer, two full 7.5 propane tanks detached from the trailer and both were being dragged behind the RV. The contact was notified by another driver that the tanks were being dragged behind the RV. The contact pulled over safely and placed both propane tanks into the trailer. The contact stated that the backing plates on the tanks were missing. Additionally, the tanks were not installed correctly. The contact diagnosed the failure and repaired the trailer. The manufacturer was notified of the failure. The approximate failure mileage was 69,000.

2019 Flagstaff

Two of the Frame Bolts underneath the front of our E-Pro 19FBS camper are partly rusted and missing a nut (one on driver side and one on passenger side). The bolt on the passenger side is out of the metal frame hole it should be in, and the metal frame is slightly bent. Both bolts are at a slight angle. The other 10 Frame Bolts are not visibly rusted and there is a nut on each one but a few are loose enough to twist off by hand. The others seem tight enough but we can’t be sure without knowing what torque number they should be tightened to. We double checked the Forest River owner’s manual but there is no mention of Frame Bolts in the 'maintenance' list or anywhere else. Also, there are no lock washers on the frame bolts to prevent self-loosening. This is surprising because it seems these frame bolts are the only thing holding the body of our camper to its frame. We have owned our camper a little over 3 years and we use it 3 to 5 times a year to travel short distances, all on main roads and major highways for camping only on paved or gravel leveled lots. We have never taken the camper off road or towed a heavy load. Our frame bolts should not be so loose that two bolt nuts fell off and a few can be twisted off by hand. We are reporting this Frame Bolt failure because it is something that can cause separation of the RV box from its frame while in motion. We routinely check everything on the camper but not everyone does unless there is a reason to. Maybe what we found is not a common issue but it is worth reporting because paying special attention to Frame Bolts during production (Forest River) , at point of sale (Dealership) and as part of routine maintenance (RV Owner) could save a life. We sent an email to Forest River yesterday describing the problem and requesting they cover the cost of repair. Nobody has inspected our camper except for us because we just discovered the issue last weekend during routine camper maintenance.

. The stabilizing jacks are definitely just for stabilizing. We thought they would take the load of the rv. 4 jacks to support 1 rv. The warning on the jacks is located on the topside of the jack, so a clueless, new rv owner would not see them until the jacks are down. Also the documentation does not say the jacks are not load bearing. My only experience with a screw jack is from my car and changing a tire. Long story short, we bent 2 of the 4 jacks b/c we thought they could take the rv load to level the rv on un-even ground. Luckily no one was hurt and no other damage. *tr

My 9 month old infant son was playing on the bed at the front end of the camper with supervision when he slid back and almost completely out from under the canvas where it attaches. Thankfully i was right there to catch him but this could have been very bad! The vehicle was stationary, we were set up for camping for the weekend. Incident occurred 10/82016. I will be happy to take a photograph and send it in. The front canvas is not secured to the camper in the front bottom. It can be tucked in, but any pressure pushes it right back out. There only appears to be glue in the corners and nothing in the front to secure this in place.

Dt: 2005 forest river flag staff 831bhss. The wiring caught on fire, causing the electrical brakes to lock. The trailer and the vehicle 2004 gmc, yukon flipped over. The brake wires were exposed because there was no protective covering on them. Consumer was driving at 60 mph when this occurred. There were no other vehicles damaged. There was a report to the police and witness to this incident. Insurance agency said that it was due to faulty wiring. *ak the elecrical wiring for the brake system rubbed on the metal crossbar, cut through the wiring and shorting out the braking system which then locked up. 2019 Flagstaff

I setup my camper and turn the hot water heater on using the propane setting. I left the camper and came back and could smell something hot/burning inside the camper. I looked at the rubber propane line connected to the hot water heater was on fire. I opened the cover to the hot water heater and blew the flame out. Propane was leaking from the rubber line that was cracked. I turned the gas off to prevent additional fire. The gas line appeared to by dry rotted. The camper would have burned if I didnt catch the line on fire. The line was replaced, but should have been a better line. I made the repair myself and did not report it to the manufacturer or anyone. There was no warning of the fire until I bought the fire outside.

On 6/19/13 we left from burlington, wi - clinton, ny with our trailer. On our drive back to wi, we had stopped at a truck stop in indiana around 10:30 pm on 6/28 to get fuel & snacks from the trailer. Once we arrived home we realized that the outside latch door that covered the outdoor refrigerator had been ripped off, the refrigerator door itself had been ripped off, & both sets of steps were bent by the doors coming off & holes & dents down the bottom/lower side of the camper. The tires on the camper had worn through the bottom side of the camper & there was less than 1? Of clearance between the tires & trailer bottom. We contacted scenic rv & were told to bring the trailer back. The manger contacted us mon. Morning & stated that there was clearly a problem with the axle & the clearance on the trailer. We believe that because the tires were bouncing off the bottom of the trailer & with all of the vibration, it caused the latch door to bounce out of the lock catch ( ?deep) & fly off. We contacted forest river & they told us that there were no manufacturing issues & the axles were correct for that trailer. After a lot of correspondence to the dealership, forest river & dexter axle we found out that the axles spec?d for the trailer were not the axles that were installed on our trailer. We talked with a woman in the warranty dept. & after i read her the serial numbers from the axles, she stated that those were not the axles spec?d for our trailer. I have also found more forest river trailer owners that have had the same problem. Some of them have had to have the axles replaced while others have had to have the dealerships install & dexter lift kit under the trailer to raise it up. There are multiple stories on the forest river forum that detail this issue. We were lucky that we were traveling late at night when those doors flew off; someone could have been killed. *jb

Consumer is requesting that 2007 forest river flagstaff tent camper model 206st be recalled for failure of the lift system. *nj the consumer didn't understand why a battery was required to operate an electric winch. The battery on the camper was in a vinyl box which sat on the tongue of the camper and not bolted down. After going over road construction on a camping trip, the battery in the box fell off and was dragged under the camper, which caused it not to function. Because of what happened, when the consumer plugged the electric cord into the outlet, the electric winch did not work. The consumer was told to override the electric winch. . The consumer stated the gear broke when the override feature was cranked beyond its point which was why the top fell. The consumer received a letter from the manufacturer which stated to take the camper to the dealer because the top may free fall if the electric winch was in a stalled position. *jb

I purchased a 2016 flagstaff v-lite wrliks30 in oct, 2015. It came with 5 castle rock st225/75r15 load range d tires--4 installed on two axles and one spare. The tires came inflated with nitrogen; i regularly checked and refilled them with nitrogen at crawford tire in ocoee, fl. While driving westbound on interstate 10 on oct 17, 2016 with the trailer lightly loaded and at a speed of 62 mph, the right front tire on the trailer suffered catastrophic tread separation. I was able to control the trailer and come to a safe stop, but the trailer was damaged (repairs totaled approximately $800). The tire was still under warranty from lions head specialty tire and wheel, 827 e. Lincoln avenue, goshen, in 46528 and i made a claim. They sent me a new tire identical to the originals, $30 for mounting, and paid for damage repairs. On feb 3, 2017, i retrieved the trailer from giant outdoor recreation after the repairs were made and drove it approximately 15 miles to another service facility. On feb 9, i was informed by that facility's manager that a tread separation had occurred on the right rear tire. The tire, though still inflated, showed severe warping and distortion of the tread and at least one steel thread penetrating the sidewall. This certainly happened on the 15 mile drive to the facility because the tires were inspected at giant recreation world. I spoke with jake carter at flagstaff customer service. He offered me a new, identical tire or $75. I told him that with two of four tires having catastrophically failed, i could no longer continue to drive on this type of tire. He told me they were fine tires with a failure rate less than 2%, i had just had "bad luck," and denied my request for alternative tires. These tires are clearly a safety hazard. Flagstaff and lions head should be required to replace all of them with tires that are proven safe!

I just noticed that my fifth wheel trailer came with c load rated tires. Rated at 2150# at 50 psi for a total of 8600#. My trailer dry weight is 8273 # and cargo max of 1241 # (this data is on official plaque on side of trailer) for 9514 # or 914# over the tire max rating. I will be replacing these before my next trip.

I took possession of my brand new e-pro coach on march 12, 2021 and on march 31,2021 the axle detached from the frame and cause extensive damage. Luckily i was travel at approximately 5 miles per hour. I could have been severely injured as well as any passengers, other travelers, bystanders etc. If this occurred at a higher rate of speed or in any other area. The bolts that should of attached the coach to the frame where know where to be found in the mile and a half i walked searching for them on the roadway which was recently traveled. When forced to drive over a canvas tent bag, Due to traffic and road conditions. The tent bag wrapped around the drive shaft, ripped out the brake lines and the emergency brake lines damaged the master cylinder and damage the emergency brake mechanism in the cab of the RV. There should be protection under the vehicle so that does not happen! The loss of both types of brake systems could be catastrophic.

2017 Flagstaff

Checking wiring in electrical box at front of trailer i discovered that the wire from the brake breakaway switch was not connected. The tow brake wire, the trailer brakes wire and the switch are all supposed to be crimped into a single connector. The switch wire was twisted around the other two - but not into the crimp. Thus the switch would not have function and was not likely tested. A family member owns a similar trailer, made at the same factory that had the same problem, except it was the main brake wire that wasn't in the crimp.

2008 Flagstaff

Purchased new 2008 forestriver flagstaff hw camper june 20, 2008 from tom johnson's rv in marion, nc. I left home with my grandchildren for a 3 week roadtrip on june 23, 2008 pulling this camper. On our route home on i-29, just outside of council bluffs, iowa, my car and camper swerved violently. I was able to keep it under control thinking it was caused by the wind. The next morning when leaving the motel we stayed in - i heard a grinding noise. I drove across the parking lot into the camping world lot. There they discovered that the brake on the camper had "burned up" and was melted to the axle. I attempted, as did they, to contact tom johnson's in marion, nc. My 4 grandchildren and i waited in the lobby all day while they repaired the camper. When the repair was complete we again tried to reach tom johnson's but received a recording that they were closed. We also contacted forest river. Neither of the two companies offered to pay for the extra expenses i incurred. I had to pay out of pocket - money i needed to cover expenses on the way home. Later i did receive a check for part of the cost from the manufacturer of the brakes or wheel. The repairmen at camping world said that they had seen this happen with trailors like the one that my camper was built on. They said it was a boat type and that frequently mechanics will put too much axle grease in the axle. Tom johnson's added axle grease just before i left their dealership. *tr

The contact sustained a sore neck, hip, and back that did not require medical attention. A police report was filed. The contact stated no warning light was illuminated before the crash. The vehicle was towed to brad wreckers service and then taken to a & l rv sales (4503 bristol hwy, johnson city, tn 37601) where a failure was not diagnosed. The contact insisted that there was an electrical issue and an issue with the brakes so they installed bluetooth brake controllers to help with brakes, but the failure still occurred. The manufacturer had not been informed of the failure. The failure mileage was approximately 70,000.*dt*jb*dt

2005 Flagstaff

Dt: 2005 forest river flag staff 831bhss. The wiring caught on fire, causing the electrical brakes to lock. The trailer and the vehicle 2004 gmc, yukon flipped over. The brake wires were exposed because there was no protective covering on them. Consumer was driving at 60 mph when this occurred. There were no other vehicles damaged. There was a report to the police and witness to this incident. Insurance agency said that it was due to faulty wiring. *ak the elecrical wiring for the brake system rubbed on the metal crossbar, cut through the wiring and shorting out the braking system which then locked up. As a result the trailer swayed out of control. *nm

On the morning May 22nd, just 45 minutes after we left our home in Portland, our trailer burst into flames on 1-84 off exit 44 near Cascade Locks. It was inexplicable as to why this happened. It was a brand new trailer with less than 2k miles on it and purchased in December 2020. As you can see from the pictures, it was a total loss. We are filing this report to help protect others. We are a part of the forest river group and quality seems to be a concern. 2008 Flagstaff

Purchased new forest river flagstaff 2008 model on june 20, 2008. I was not told of a recall on the winch and did not know of it until 12/01/2011. The recall was 11/2007. On the first night of my first trip june 24, 2008 i had to have assist from the campground host to raise my camper - the electric lift would not work. Almost everytime i have used the camper since then, i have had problems with the lift. I informed the dealer (tom johnson's rv, marion, nc) of the problem with the lift (winch) in 2009, when i took the camper back for service following another problem (see complaint odi # 10438096). They did not tell me then that the winch had been recalled. On june 13, 2011 @ 1:30am while camping at cades cove in the great smokies this camper started setting itself down with me and three of my grandchildren sleeping inside. We were awakened by the noise and were able to escape through the emergency exit only because we had the emergency braces in place. It was very traumatic to us all. I spent 2 days attempting to resolve the problem. I spoke to a representative at forest river several times asking for repair references. That representative never told me that the winch on this camper may have been recalled. Finally on day 3, 2 repairmen came from knoxville and lowered the camper so i could take it home. The cost over $500.00 + lodging and food for 3 days. I was not aware of the (probable) recall of the winch on this camper at anytime by the manufacturer or the dealer. I did not become aware of the recall until 12/01/2011. *tr

I took possession of my brand new e-pro coach on march 12, 2021 and on march 31,2021 the axle detached from the frame and cause extensive damage. Luckily i was travel at approximately 5 miles per hour. I could have been severely injured as well as any passengers, other travelers, bystanders etc. If this occurred at a higher rate of speed or in any other area. The bolts that should of attached the coach to the frame where know where to be found in the mile and a half i walked searching for them on the roadway which was recently traveled. I purchased the coach from western skies rv in reno, nv. I've been in contact with them about this matter. Please advise me on the next steps i should take to resolve this vehicle matter.
