The vehicle's tonneau cover was defective from the day of delivery and remained unusable for approximately the first 12 months of ownership. Factory over-application of adhesive forced the rear window against the tonneau rollers, preventing operation. I purchased this vehicle specifically for its cargo bed functionality for business use. The cover was finally repaired at Tesla Lincoln, NE when a senior technician was present — root cause identified as a manufacturing defect present from the factory. Component: Tonneau cover / rear bed cover system. Safety: Debris containment failure — unsecured cargo bed.

At approximately 6:30 PM on January 4, I experienced an incident where the passenger front door unlatched, completely by itself without pressing the unlatched button or pulling the manual release. It has since opened itself again in my driveway. And this can be a very serious issue if it worked to unlatch while driving and the door swings open and causes an accident or hits an object or person which can in terms caused serious injury, or property damage, and even in some cases if a passenger is in the seat in the door and latches without their seatbelt on, it can cause serious injury, and or death. In the event of a passenger falling out.

The contact owns a 2024 Tesla Cybertruck. The contact stated that while driving at a moderate speed on a windy day, a metal piece, approximately 2-3 ft. long and located on the side of the sunroof, detached from the vehicle. The contact managed to recover the piece that detached from the vehicle. The vehicle was taken to a Tesla service center, where the failure was acknowledged the failure, and the contact was provided an estimate for the repair. The contact stated that the failure was a manufacturer's defect and that the repair should be free of charge. The vehicle had yet to be repaired. The failure mileage was approximately 11,000.

Subject: Tesla Cybertruck Recall – Out-of-Pocket Wrap Costs I own a Tesla Cybertruck (VIN: [XXX] ) affected by the recall on Cant Rail Assemblies. Tesla requires the replacement and welding of new assemblies. My truck is custom wrapped by Rogue Films, a Tesla-recommended supplier. Rogue Films quoted $850 to re-wrap the affected areas after the recall repair. Tesla is only offering $300, leaving me to cover the remaining $550. This cost is a direct result of Tesla’s recall, not elective customization. In addition, Tesla does not provide a loaner vehicle during the lengthy repair process, adding further burden. I am filing this complaint because customers should not have to pay out-of-pocket expenses to remedy a manufacturer’s defect under recall. This is a clear case of recall cost shifting to the end customer. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

In July of 2025, I drive my vehicle on a 130 mile trip. About 25 miles into the drive, I started receiving errors about AC charging not available. I stopped at a Tesla supercharger Hannibal [XXX] but I couldn’t charge it there either. Drive it the rest of the way to my destination where I had an ac charger available. Tried there but I couldn’t charge there either. Tried resetting the car…still couldn’t charge. Opened service ticket. Tesla had to send a tow vehicle 110 miles to me and then tire the truck back to the service center in [XXX] . they had to keep the car 3 days and replaced the pcs system. I am submitting a safety complaint regarding the rear visibility system on my Tesla Cybertruck. The physical rear view mirror does not provide any usable visibility of the roadway behind the vehicle. The design of the rear window and cargo area blocks the view, which makes the mirror ineffective for its intended safety function. Because the physical mirror cannot be used, the driver must rely on the rear camera feed displayed on the center screen. This is the only way to see directly behind the vehicle during normal driving. However, the rear camera feed disappears every time the turn signal indicators are activated. When the indicators turn on, the screen automatically replaces the rear view with the side camera view. This removes all rear visibility at the exact moment when the driver needs full awareness for lane changes, merging, and turning. This behavior cannot be disabled or overridden. It is part of the default software design. As a result, the driver is left without any functional rear visibility method during critical maneuvers. This creates a clear safety risk and raises concerns regarding compliance with FMVSS 111, which requires reliable rear visibility. The loss of rear visibility during lane changes and turns increases the risk of collisions with vehicles approaching from behind. Since the physical mirror is unusable and the digital feed is removed by the software, the driver has no safe or compliant way to monitor rear traffic during these maneuvers. I request that NHTSA investigate this issue and require Tesla to provide a remedy. A potential remedy would include keeping the rear camera feed visible at all times unless the driver manually chooses to remove it, or adding a dedicated rear view camera feed that remains active during all driving conditions.

The PCs on earlier cybertrucks has been recalled. The same issue seems to persist. The issue will not allow you to charge T home and drastically cuts vehicle performance. In addition the only way you can charge is via a supercharger which makes one wonder if this whole thing was a cash grab for Tesla to force these trucks onto their costly charging network.

I am submitting this safety complaint as the vehicle owner of a Tesla that was involved in a collision on November 15, 2025. Although I am the policyholder and registered owner, I was not the driver at the time of the incident. My husband, [XXX] , was operating the vehicle, and Tesla Autopilot/FSD was engaged when the loss of control occurred. At the time of the incident, it was raining heavily, though visibility and road conditions were still within what Autopilot typically handles during normal operation. While driving under these conditions with Autopilot activated, the vehicle suddenly exhibited unexpected and erratic steering behavior, causing it to lose stability and veer out of control. The deviation happened rapidly and without sufficient warning or opportunity for my husband to intervene manually. The resulting collision appeared to be the result of an Autopilot system malfunction, not driver error. Key concerns include: Autopilot’s failure to maintain proper lane position Sudden abnormal steering input originating from the system Possible sensor, perception, or weather-related detection failure Lack of adequate warning or disengagement alert prior to the loss of control Tesla Insurance later determined the driver to be “principally at fault,” but this conclusion did not consider the possibility of Autopilot system failure under heavy rain conditions and does not reflect the sequence of events as experienced by the driver. As the vehicle owner, I am requesting that NHTSA review this incident as a potential safety defect involving Tesla’s Autopilot/FSD system. I am concerned that this malfunction—particularly under adverse weather—may also affect other Tesla owners and could indicate a broader systemic safety issue. I am willing to provide additional information, driver statements, documentation, or vehicle data upon request. We received a notice from Tesla about a recall involving the cant rail. We reached out to Tesla and they said we must go to a Tesla dealer and the mobile team will not be able to do this repair. We are in South Dakota and the nearest dealer is 6 hours away in Colorado. We asked Tesla to give us a name of a local shop who can do the repair and they said we must go to Tesla. They are unwilling to handle this recall.

My vehicle's windshield wipers were completely inoperable for an extended period. I drove this vehicle across multiple states without functional windshield wipers — confirmed on a recorded Tesla corporate support call dated May 21, 2026. This is also covered under NHTSA Recall 24V456000 (Windshield Wiper Motor failure), which Tesla Pensacola failed to identify or perform. Tesla Council Bluffs, IA identified this recall as requiring urgent attention. Safety risk: Operating a vehicle at highway speed without functional windshield wipers in any precipitation is extremely dangerous. I drove cross-country in this condition. Manufacturer response: Tesla Pensacola stated the vehicle had "nothing wrong." Tesla Council Bluffs identified the recall. Recall has still not been performed.

The adhesive that attaches the front windshield is separating from the front windshield also it appears the the actual windshield is delaminating. This is causing a hazzard to driving. Especially when the sun is coming right at the vehicle.

The wiper on the vehicle can not be controlled and set off manually. The auto wipe system repeatedly turns on and dry wipes the glass which reduces visibility for both the driver as well as the camera based safety systems. This is a software bug that has gone unfixed and unacknoledged by Tesla for over 6 months. This happens on every drive multiple times every day regardless of weather conditions.

Windshield wiper fell off, Tesla was unable to get me an appointment to repair for a month and stating it was wear and tear that made it fall off.

On [XXX], while driving highways for over 2 hours, the windshield became very dirty with dried debris from the road. I activated the window cleaning function on the steering wheel ( for the first time) and could not clear the dirt from the passenger side. I tried many times, but no result. The drivers side did clean acceptably. But not the passenger side! I could see arcs on the windshield where the wiper attempted to drop fluid. There were 8 or 9 such arcs left after the wiper withdrew. I did see cleaning solvent near the outside of the windshield. But I could not see out of the windshield. Reported same to Teslas customer svc and a service visit was scheduled for Jan. 8, 2025. By this time I had manually cleaned the window and verified the solvent well was full. Svc agent said, no one else complaining and system is operating 100% as designed. Experienced same situation again on [XXX]. Customer svc again scheduled an on site svc call for Feb 7 2025. I then saw on facebook, over 50 other people complaining of exactly the same problem. Most had tried svc and heard the same informational as I. Some included photos of the problem, some had their wiper or pump replaced, other e told that Tesla was planning an OTA update to cure the problem. I did a walk-in to svc on Jan 27,2025, and was told there is no guidance from corporate on this matter and that as far as they knew, corporate was not aware of the problem. I'll keep my appointment on Fri and hopefully I'll have some relief. I hate not seeing things coming from my right side! Apr 22, 2026
Tesla sent me a Recall notice on the Lightbar, but in the notice, Tesla says it will not connect the lightbar to the electrical system and make the lightbar operable when Tesla repairs the defective lightbar. I paid $300.00 to have the original lightbar connected to the electrical system, and now, due to Tesla's improper installation, I am going to have to pay to have the new lightbar connected as well. This is unfair, unjust, and allows Tesla to escape twice. This leaves a huge financial burden on consumers. Make Tesla repair and make the light bar operable, and return me to the position I was in before they improperly installed the lightbar.

When driving in a winter storm, headlights quickly become snow packed rendering them useless. This is extremely dangerous driving in any snowstorm at night. The light bar does not provide sufficient visibility at night and also can become ice packed.
Feb 02, 2025
When driving during a snow storm, the headlights of the Cyberteuck become covered in snow and ice. The main headlights that produce most of the light are just under the frunk in an inset groove. Because of the nature of the design it catches snow it collects in that location. Because of the LED headlights there’s not enough heat to melt away the snow. At night your lights slowly fade away until you can barely see anything. Photo is not my vehicle but my friend’s who reported same problem.
Dec 09, 2024
The headlight design, being behind and slightly above the front bumper, allows snow to accumulate in front of the headlights while parked and while driving. This prevents visibility in low-light conditions for both the vehicle driver and for other drivers.

FORWARD COLLISION AVOIDANCE

On December 23, 2025, my 2024 Tesla Cybertruck (Foundation Series) was operating with Full Self-Driving (FSD – Supervised) engaged. While FSD was active, the vehicle executed an unsafe driving trajectory. I attempted to intervene and regain control; however, the system did not disengage as expected and did not yield control appropriately to driver input. Despite driver intervention attempts, the vehicle continued along the unsafe trajectory, resulting in a loss of control and collision with roadside objects. There were no mechanical warnings or alerts prior to the incident. The issue appears related to system behavior, disengagement logic, and driver override response while FSD was active. The vehicle is currently preserved under a formal litigation hold. No inspection, teardown, or data access has occurred. Tesla has confirmed in writing that it does not modify, delete, or alter vehicle data it has received. I am submitting this complaint due to concerns regarding the safety behavior of Full Self-Driving (Supervised), including the system’s failure to disengage upon driver input and the human-machine interface governing control authority.

I am writing to report an incident that occurred on [XXX] [XXX] while my Cybertruck was operating in Full Self-Driving (FSD) mode. While driving on [XXX] Brooklyn NY with no other moving traffic present, my Cybertruck did not detect a vehicle that was parked improperly—sticking out too far into the lane. The FSD system failed to adjust its path accordingly and struck the side view mirror of my vehicle, causing significant damage to the mirror glass and housing. I understand that FSD is a driver-assist feature that requires supervision, and I was attentive during the drive. However, the clearance of side view mirrors from the driver’s perspective is extremely difficult to judge at street level, and I believe stationary obstacles of this nature should be reliably detected and avoided by the FSD system. This type of incident raises concerns about the performance of FSD in handling stationary objects, especially when the vehicle was otherwise properly supervised and operating in a straightforward environment. NHTSA has created a safety risk by forcing FSD to have extreme monitoring. While in FSD I have to go back to normal driving to change the station on the radio or look at my phone. This is something I do in my non fsd vehicles all the time. The reason for me purchasing FSD is to be safer. Your ridiculous overreach may actually kill me.The entire purpose is to reduce distracted driving, yet your ridiculous rules force an unsafe environment. I hope whoever pushed these restrictions realizes they are accountable for the crashes and deaths they create for forcing people out of FSD.

On 05/30/2026 at approximately 15H24, my Tesla Cybertruck was parked and stopped at approx. [XXX] of Target). No one was seated in the driver seat. A passenger entered the vehicle and sat in the front passenger seat to play music. Shortly after the passenger sat down, the vehicle suddenly and unexpectedly moved forward on its own and rear-ended the vehicle parked directly in front of it and causing damages. This occurred while the vehicle was supposed to be parked/stationary and while there was no driver in the driver seat. We do not understand what triggered the vehicle to move forward. This appears to be a serious safety issue because a parked, unattended vehicle should not move forward without driver input. The incident created a collision risk to nearby vehicles, pedestrians, and anyone standing in front of or around the vehicle. I am requesting that NHTSA review this as a potential unintended movement / unintended acceleration / park-to-drive activation / rollaway safety defect. Tesla should be required to investigate the vehicle logs and determine whether the movement was caused by a software issue, gear selection issue, Autopark/FSD/Summon/mobile app activation, brake hold/parking brake failure, sensor/occupancy issue, accelerator pedal issue, or any other defect. The new tesla update forces me to interact with the touch screen in order to operate my vehicle after self driving is disabled. There is no way to dismiss or disable this screen in the new update and no way for me to roll my vehicle back to the old software. After disengaging self driving a forced selector box forces you to make one of 4 choices in order to see your screen/navigation and perform other features in the car such as adjusting the windshield wipers. There is no way to disable this screen, it pops up every single time Full Self Driving is disengaged and there is no way to disable it. I was driving [XXX] going through Saint Paul and the truck sped up and became uncontrollable. I started braking as fast as I could and caused it to rear end into another vehicle. I’ve tried to contact Tesla about this, and they have not responded to me. I was told the engineering team would get back to me about the possible cause of the accident, but never heard back and they could not provide me any of the vehicle data or video which they told me in the service center. My Cybertruck’s Power Conversion System failed at 65,000 miles. I received a PCS alert, AC charging became unavailable, and the vehicle displayed charging faults. The repair required major disassembly of the tonneau, bed, and air suspension and cost 7000 because it occurred after warranty. Why This Is a Safety Defect: The PCS is a critical component that handles onboard AC charging, DC-DC conversion to the 48V system, and power delivery. Failure can strand drivers, prevent charging, and (as seen in related drive-inverter recalls) contribute to sudden loss of propulsion while driving. This Is a Widespread Issue: PCS failures are occurring frequently (“left and right”) in Cybertrucks, especially early production models. Owner forums and polls show dozens of identical failures, with some samples reporting 18–40% affected. Many owners face 2,000–7,000 repairs out of warranty. Tesla issued a limited recall for related drive inverters (NHTSA Campaign 24V-832) that can cause sudden power loss, but no recall covers the broader PCS defect despite the clear pattern. I strongly urge NHTSA to investigate this common defect immediately and require Tesla to issue a full recall for affected Cybertrucks, including free replacement of the Power Conversion System for all owners. Thank you for protecting public safety.

This error code (PCS2_a103_IsoTempSensorIrrational) is from a Tesla Cybertruck. It relates to the Power Conversion System (PCS2), which handles AC charging, PowerShare (vehicle-to-load/home), and other power conversion functions. My Cybertruck will likely be affected in the future by charging issues related to PCS. I understand this is a common issue with Cybertruck within my VIN range. I request this issue be investigated and a recall done prior to being affected.

One day my car would not charge. At all. Luckily, I was at home when this happened, but had it happened to me or worse, my wife it could've been a very scary or dangerous situation. Either one of us could've been stuck with no way of charging. It's like a gas car with no way of putting gas in. The car would not charge from home or other chargers, including Tesla superchargers. When I described the problem to the Tesla dealer, they were very aware of the problem and said that it was the Power Conversion System. There was no prior warning before this happened. Though they are aware of the problem, they are only fixing/replacing them on a warranty basis. Even though they know about the faulty part that they made, and acknowledge, they are not doing a recall or fixing the problem unless you pay for the parts and labor. I asked if this is normal for a car that is under 2yrs old and he said this part can go bad at anytime. The best thing that can happen is for it to happen early while still under warranty. But the part does not break due to wear and tear, or overuse, or abuse. It is due to a faulty design that needs to be replaced. That's why they have a new part that they replace it with now.

I am leasing a 2024 Tesla Cybertruck and have experienced multiple serious and unresolved safety issues. When driving at speeds of 50–60 mph or higher, the Cybertruck begins to shake aggressively, particularly in the front wheels. The vibration makes it difficult to maintain control. I took it to the Tesla dealership, and service staff confirmed there is a known malfunction with the Cybertruck tires. They informed me they are waiting on new tires to arrive, but offered no timeline and sent me home with the issue unresolved. No loaner vehicle was offered, no workaround was provided, and I am still driving a vehicle Tesla knows is defective. In addition, the rear windows malfunction frequently. When I close the rear doors, the windows often jam or freeze. This concerns me because the Cybertruck is built entirely with electronic door systems—there are no manual overrides. If power is lost during a crash, occupants may not be able to exit. This is even more alarming in light of recent reports of Cybertruck-related deaths. Multiple individuals have died in crashes where the battery was damaged or the vehicle caught fire. Power loss caused the doors and windows to become non-functional, and due to Tesla’s armored glass, bystanders couldn’t break the windows to rescue the victims. These people burned alive while trapped inside. This is not a theoretical issue—it has happened. Despite these risks, Tesla has not issued a recall or urgent fix. I’ve made multiple efforts to resolve these problems, but Tesla’s delays, denials, and lack of proactive safety measures are deeply negligent. This is a dangerous vehicle, and I am requesting that NHTSA immediately investigate both the front-end tire instability and the entrapment risk caused by electric-only doors and unbreakable windows. I no longer feel safe driving this Cybertruck and believe others are also at serious risk.

My vehicle achieves only 80-100 miles of real-world range per charge, requiring charging to 100% to reach even this reduced figure. On April 30, 2026 at 11:08 PM, I documented energy consumption of 460.8 Wh/mi — 37% worse than the specification required to achieve Tesla's advertised range of 318-340 miles. Component: High voltage battery / power distribution system. Safety risk: Severe undisclosed range reduction means the vehicle may reach 0% charge in locations and at times the driver does not anticipate based on advertised specifications, creating stranding and safety risks. Manufacturer response: Raised on May 21, 2026 recorded call with Tesla corporate. Tesla acknowledged the issue and said to discuss at service — but all service appointments have been cancelled since December 2025.

Normal night charging . Morning I get into my car I have alerts PCS2 saying I can't charge anymore at home . This is a huge safety concern as it's happened to many Cybertruck owners. If we go on a trip we can literally be stranded due to range and accessibility of chargers with kids. This needs to be fixed as a constant warranty even past 50,000 miles as the vehicle is not proven and in its early years. It's not a matter of if , it's when and creates unreliability and extremely heavy repair costs for an owner not due to personal wear and tear. Clearly a manufacture recall that should be monitored for a long long time.

STEERING,VEHICLE SPEED CONTROL,LANE DEPARTURE
2 complaints
On December 23, 2025, at approximately 9:45 AM, I was operating my Tesla Cybertruck with Autopilot engaged under normal driving conditions. There was no inclement weather, the roadway was straight, and visibility was clear. While driving, the vehicle suddenly veered off the main roadway without warning and traveled off-road, ultimately colliding with a tree. The movement was abrupt and not consistent with normal lane departure. I immediately attempted to correct the direction of the vehicle; however, the vehicle did not respond appropriately to my steering input and felt unresponsive during the event. At no point did I intentionally steer off the roadway. The loss of control was unexpected given the road conditions and environment. This incident raises concerns about the reliability and safety of the Autopilot/driver assistance system and its ability to maintain lane control and allow driver override when needed.

While operating on a public roadway with Tesla Full Self-Driving (Supervised) engaged, the vehicle unexpectedly departed its intended roadway trajectory. Upon recognizing unsafe vehicle behavior, I attempted to manually override and disengage the system using normal driver control inputs. Despite these attempts, the system did not disengage or yield control in a timely or effective manner. The vehicle continued on an unsafe path, left the roadway, and collided with a fence and a tree before meaningful driver control could be restored. This incident raises concerns regarding the reliability of driver override authority and the system’s ability to relinquish control during supervised operation.

WHEELS
2 complaints
During my last wash of my cybertruck I noticed the cast aluminum wheel has a spoke that was completely cracked through. There was no violent pothole incident or collision to cause the crack. As an engineer I researched the method of manufacture and material of the rims. It is my opinion that the proper knock down factors were not used when designing these wheels for the 7,000 pound vehicle. Castings contain defects as a normal outcome of each pour. Knock down factors allow for castings to be used in design responsibly to preclude brittle failures at the defect locations. The cross sectional area of each spoke is minimal too for aesthetic purposes increasing the stress field in the spoke. This is a cast aluminum wheel taking the shape of a forged or wrought product. Furthermore the wheel covers make it difficult to identify cracks in the wheel since there is no visibility allowing a driver to unknowingly drive on a wheel that has had one or more spokes fail. This wheel is part of the all terrain wheel upgrade. Marketed for off road capability.

I am filing this complaint regarding Tesla's failure to include the advertised Autosteer feature in my new Cybertruck, which I believe raises serious safety concerns. At the time I ordered my vehicle, Tesla's website and the official Cybertruck Owner’s Manual (archived October 15, 2024) clearly listed Autosteer as a feature included with Autopilot. Tesla also markets Autopilot as a system that “enhances safety and convenience,” which formed a core part of my purchasing decision. I previously owned both a Model 3 and Model Y, both of which had Autosteer included in the base Autopilot, reinforcing that expectation. Upon delivery of my Cybertruck, I learned that Autosteer was not actually included, and Tesla now requires customers to pay for Full Self-Driving (Supervised) to access this core safety function. This change was not communicated prior to purchase or delivery and contradicts both historical Tesla offerings and the documentation available at the time of sale. Tesla’s decision to restrict a safety-related feature—one that assists with lane keeping and crash avoidance—may put my family and others at greater risk. Drivers may assume Autosteer is included based on Tesla’s marketing, which could lead to misuse or over-reliance on the limited Base Autopilot. I contacted Tesla directly and requested a resolution. Despite multiple follow-ups, I received no meaningful response. I believe this matter represents a pattern of deceptive practices and raises legitimate safety concerns regarding what features are expected and what is actually delivered. I respectfully request that NHTSA investigate Tesla’s removal of Autosteer from Base Autopilot and its potential implications for consumer safety and transparency in driver-assistance system advertising.

Also, the lighting on the vehicle is very dim because the vehicle pretty much only operates off of two tiny fog lights, and in a low visibility area there’s not enough lighting to actually see the road and bicycle is biking on the roadways even in a dark situations, it’s the same thing not enough lighting to actually see anything on the road.

On May 15, 2025, at around 11:30 AM, my Tesla was involved in a collision while Full Self-Driving (FSD) was engaged. The incident occurred at the Tesla West Covina Dealership. The vehicle struck a barrier wall without any collision warning, suggesting both a failure of the sensors and FSD system. After the incident, Tesla staff directed me to their West Covina Service Center. A service advisor reviewed the dashcam footage with me and acknowledged the crash appeared to result from FSD malfunction. I was told Tesla will cover the repair. However, when I returned on May 26, 2025, I was informed that the dashcam footage was no longer available. I did not delete it and suspect it may have been removed by Tesla personnel. They also then claimed they will not cover any repairs. I have written to their legal department to formally request the video footage and any other data they have regarding the crash. Despite follow-up, Tesla has not responded or released any data. I am reporting this as a possible safety defect and mishandling of evidence. I request NHTSA investigate: The failure of Tesla’s FSD and collision avoidance system. The disappearance of crash footage after review by Tesla staff. Tesla’s lack of response to requests for data and documentation. Thank you for your attention.

On [XXX], at approximately around [XXX], an incident occurred involving the Auto Park feature of a Tesla vehicle in a quiet parking lot. The conditions were clear, with no adverse weather, obstacles, or surrounding vehicles. The Auto Park function was engaged, and the vehicle reversed and moved forward as expected. However, it failed to stop and collided with a yellow pole directly in front. The front camera feed was visible on the screen, but the vehicle did not detect or recognize the pole, and no collision alert was issued. The incident occurred too quickly to allow manual intervention via the brake pedal. The collision resulted in minor front bumper damage, with repair costs estimated at over $1,500. All incident details and vehicle data have been provided to Tesla for investigation. And they rejected to provide front plastic bumper replacement. The contact owned a 2024 Tesla Cybertruck equipped with Pirelli Tires, Tire Line: Scorpion ATR, Tire Size: 285/65/R20, DOT Number: N/A. The contact stated that while driving at 70 MPH, he heard a loud bang, and the vehicle veered to the left. No warning light was illuminated. The contact attempted to steer right, but the vehicle failed to respond. The vehicle struck another vehicle in the left lane before both vehicles crashed into the median barrier wall. The contact's vehicle then spun into three right lanes and came to a complete stop on the right shoulder. The air bags were deployed. Upon inspection, the contact discovered that the rear driver's side rim was fractured. The contact and three passengers were taken to the hospital via ambulance. The contact sustained third-degree burns on his arms as a result of the air bag deployment. The second passenger had cuts on the head from the seat belt. The third passenger had cuts and bruises on the face from the seat belt. The fourth passenger sustained no injuries. A police report was filed and stated that the failure was caused by the rear driver's side tire blowout. The vehicle was towed to a tow yard and was deemed totaled by the insurance company. The manufacturer was not made aware of the failure, but the contact obtained the event data report and summary. The failure mileage was approximately 8,504.

I purchased 2024 Tesla cybertruck and got the delivery of the truck on May 11th 2024. One the very first day I experienced a critical warning that stated " Critical Steering issue detected" and it started beeping loudly and I was unable to steer it. Now I am unable to drive the car as I have a whole lust of critical alerts that pop up on the screen. I am concerned about my safety and others on the road. One of the warning says "loss of system redundancy detected. Unable to shift out of park because vehicle has detected loss of redundancy. The vehicle may suddenly lose electrical power, steering and propulsion, and maybe unable to apply the parking break." I tried contacting Tesla emergency roadside support and they just keep me on hold and I am unable to reach a real person. I tried scheduling a service appointment but these warnings are critical and is a potential safety concern.
May 12, 2024
While driving vehicle a prompt flashed on the screen saying “power reduced, steering performance reduced due to overheating”. Then the next screen said “critical steering issue detected pull over”. Then I got a loss of system redundancy detected. This has happened twice in one week and I’ve towed it to Tesla twice.

My Tesla Cybertruck has experienced a failure of the Power Conversion System (PCS), which prevents the vehicle from charging at home using the installed residential charging equipment. Tesla diagnosed the issue and advised that the required replacement part is unavailable, with no estimated date for repair. As a result, I have been forced to rely exclusively on public Superchargers for several months. While Tesla has provided free Supercharging, this does not restore the vehicle’s intended functionality and requires significant additional travel, waiting time, and disruption to my daily activities and business operations. The vehicle was purchased and represented as capable of convenient home charging, and the prolonged inability to repair a critical charging system due to parts unavailability substantially impairs its use, value, and practicality. I am submitting this complaint because Tesla has been unable to provide a repair timeline, and I am concerned that other owners may be experiencing similar failures and extended delays in obtaining repairs. The offered loaner vehicle is a Tesla Model Y, which is not comparable to a Cybertruck and does not meet the hauling and cargo needs associated with my business operations.

The failed components include the Tesla Vision system (cameras), sensors, Active Safety Control Module (AEB), and Gear Selection UI. Symptoms first appeared on April 30, 2026, including black screens and "Autopilot Unavailable" alerts. Internal vehicle logs from Apr 30 to May 02 confirm persistent "SNA" (Signal Not Available) and "UNAVAILABLE" states for these critical safety modules well before the collision. On the day of the accident (May 02), the logs recorded 25 instances of Gear UI signal loss, which completely disabled the rearview camera and proximity alerts while reversing. This total "blind" state put my safety at extreme risk as I had zero visibility of blind spots and received no audible warnings. The Automatic Emergency Braking (AEB) was also recorded as "UNAVAILABLE," failing to prevent the collision. The problem is scientifically confirmed by the vehicle's internal diagnostic logs, which I have retrieved and am ready to provide for inspection. The accident has been reported to the local police and Progressive Insurance. I am currently coordinating with Tesla Service for a formal inspection of these software and hardware defects.

This incident happens when driving home from work, it happened half a mile away from where I live on a local road. All of a sudden car lost all power, steering wheel and brake completely failed, drive train completely failed while the car was still moving. there was no way the car could be controlled to move to the side of the road or to break to completely stop it.. thankfully it coasted to a complete stop. As it was the middle of the day, there were no cars around the local road I was on. Once the car stopped everything shut off completely. There was no way to turn on the hazard lights or get out of the car either I had to use emergency exit.. car wash towed to Tesla service center where it stayed with them for a few weeks and I was told that it was PCS an ancillary auxiliary part that needed to be replaced. While searching the forums, I found out that this is becoming a problem where multiple cyber trucks have this issue going on which I feel is a safety issue as it can cause a major accident or fatalities if somebody was driving on the highway on highway speeds and this happened. I am opening this ticket to help others so hopefully a recall is done on this.

Vehicle Safety Complaint - Tesla Cybertruck Steering System Failure During FSD While driving my Tesla Cybertruck with Full Self-Driving (FSD) engaged, the rear steering system failed without any prior warning. FSD immediately disengaged, and a red steering system ERROR appeared on the instrument cluster. The rear axle steering was completely disabled, turning radius became severely limited, and vehicle speed was restricted. This occurred while actively driving in traffic and was extremely dangerous. I accessed the vehicle’s Service Mode and documented three fault codes that triggered simultaneously at 17:48:14 on March 4, 2026: 1.RSAM_a260_monitorTripDetected ∙Monitor trip detected, system degraded ∙Potential Impact: Steering system performance degraded, vehicle speed limited ∙Only clears on next power-up 2.RSA_a212_activeFault ∙Rear Steer Actuator (RSA) stopped at current position ∙Internal condition detected, defaulted to safe state ∙Fault Reason: CONTROLLER_IRRATIONAL ∙Previous State: RSASTATE_ACTIVE_STEERING (was actively steering when failure occurred) ∙rsamControllerRational = false 3.PEPS_a182_rearSteerDegraded ∙Primary Electric Power Steering (PEPS) detected RSA not functioning as expected ∙Reason: RSA_UNHEALTHY ∙rsaUnhealthy = true ∙rearSteerCommand = -0.59 All three faults were set at 17:48:14 and cleared at 17:50:55. Additionally, an earlier alert — EGGRIGHT3_a203_airwaveLeftLateralUnavailable — was logged on March 2, 2026 at 21:11:08, which may indicate a related or recurring issue. The simultaneous failure of the rear steering actuator and disengagement of FSD while driving at speed represents a serious safety hazard. The RSA controller was flagged as “IRRATIONAL” while the system was in active steering mode, meaning the steering hardware failed during operation — not at startup or in a parked state. While driving on 2-22-2026 at approximately 3:30 pm on the 5 freeway near Santa Ana, the appx 4 ft long light bar flew off and ricocheted violently on vehicle, while still attached by the power cord. This damaged many parts of my vehicle, specifically the glass roof, front windshield, and various body components which were shattered or scratched. I cut my hands. Your database says there are 0 open recalls and that is a lie by Tesla, because they sent me a notice months ago saying they are awaiting mechanical attachment methods. Since the release of V14, the Full Self-Driving (FSD) system automatically defaults to operating approximately 7 MPH above the posted speed limit. This occurs on all road types, including city streets and highways. When FSD is engaged, the vehicle sets the cruising speed above the detected speed limit without driver input. There is no setting that allows the driver to enforce strict compliance with the posted limit. Available speed settings only permit equal or greater deviation above the speed limit. The driver cannot manually lock or command the system to remain at or below the posted limit while FSD is active. The only way to prevent speeding is to fully disengage FSD. This is not a malfunction. The vehicle is operating as designed under V14. This creates a safety concern because: 1.The system is programmed to exceed legal speed limits by default. 2.The driver does not retain meaningful authority to ensure lawful operation while automation is engaged. 3.Automatic speeding increases crash risk in construction zones, school zones, and changing speed areas. 4.Drivers may receive citations despite not intentionally selecting an unlawful speed. Because this behavior was introduced via V14 software, it likely affects all Tesla vehicles running FSD V14, not just this vehicle.

SEAT BELTS
1 complaint
The rear passenger-side seat belt retractor is malfunctioning. The seat belt does not properly retract or maintain tension during normal use. The belt remains loose and does not snugly secure the passenger unless manually adjusted multiple times, reducing the effectiveness of the restraint system. This seat is regularly used by a child passenger. Because the seat belt does not function as designed, the safety of a child occupant is compromised, particularly in the event of sudden braking or a collision. This involves a primary safety restraint system. The issue was discovered during normal driving conditions and is repeatable and consistent. The seat belt assembly is available for inspection upon request. No warning lights, alerts, or messages appeared prior to or during the malfunction. The issue was not caused by an accident, impact, or misuse. The vehicle is otherwise operable. The manufacturer was contacted for service. Initially, a service charge of approximately $55 was proposed to inspect or repair the seat belt assembly. After questioning whether a safety restraint system should be subject to a service charge under warranty, the charge was removed. At the time of this report, the issue has not been inspected, repaired, or addressed, and guidance on safe continued use—especially with a child occupying the seat—was delayed by several days.

Component/System Failed: The primary component failing is the Front Overhead Console Assembly (P-R-N-D manual gear selector/shifter and hazard light switch). The assembly detaches, rattles, and fails to remain securely affixed to the headliner. The defect has been present since delivery (Aug 2024, 208 miles). Safety Risk: The overhead console contains the crucial backup gear selector and the hazard light button. Should the vehicle’s primary touchscreen fail (rendering the main gear selector unusable), reliance on this repeatedly detaching or faulty manual shifter compromises the driver's ability to safely operate the vehicle (Park, Reverse, Neutral, Drive) or activate emergency hazard lights. Problem Confirmation/Inspection: The failure is confirmed by the manufacturer (Tesla Service Centers). Tesla attempted to fix the overhead console defect at least four times. The defect was confirmed again on October 22, 2025 (11936 miles), where Tesla noted only a "slight adjustment... as a temporary fix" and stated a "Permanent fix will be performed on the upcoming visit". The vehicle and its components have been inspected repeatedly by Tesla technicians. Warning Lamps/Symptoms: The primary symptom is the physical rattling, looseness, and detachment of the overhead console/gear selector. The center console can come off at ant time that the windshield visor is detached from the console. This has happened to me several times while I was driving and it caused the cybertruck to shift out of gear and has changed the gear to neutral and park on different occasions. The first time this happened while I drove, I was driving down the street and the car came to a hault when the gear shift cover came loose. This could have caused a serious accident, but fortunately it didnt. This problem existed from the time of delivery & we had to make our 1st service apt on the day we took possession of the truck. This happened several times afterwards and tesla was unable to fix this in 7 attemp

On [XXX], [XXX] sat in the left rear-seat of a Cybertruck that was involved in a solo-vehicle collision. [XXX]'s impact injuries were minor. The truck caught fire. The right rear door remained mechanically openable, but its ordinary operation depended on low-voltage electronic buttons located inside the cabin and on the exterior pillar. Those buttons failed after the crash. A man tried to rescue the occupants, but he could not open the doors because the buttons failed and the truck lacks exterior handles. The "armor" glass made it difficult for the rescuer to break open the windows. The mechanical backup releases for the rear doors are concealed beneath the map pocket liner at the bottom of the doors — not feasible to find or use in the smoke, heat and chaos of a post-crash fire. This obscure release left the rear seat occupants with no practical means of egress. [XXX] died of smoke inhalation and burn injuries because he was trapped inside the Cybertruck. I have NEVER had this issue where FSD Autopilot disengaged after a trip and returning. Once the settings are placed in FSD Autopilot they were never changed unless done manually. There were no alerts/notification telling me FSD/Autopilot was disengaged. While driving with Tesla's Full Self-Driving (FSD) system engaged, my vehicle struck two standard traffic cones in a clearly marked construction zone. The system failed to detect the cones or make any evasive action. I have video evidence of the incident. The situation was extremely dangerous. The cones could easily have been construction workers, pedestrians, or something more severe. I hesitated to intervene because it happened in a split second, and I was trusting the system to respond — which it completely failed to do. In that moment, I was also faced with a difficult choice: swerve and potentially cause a more serious accident, or trust that FSD would navigate correctly. It didn’t. The impact caused damage to my vehicle. When I brought it to Tesla, the technician dismissed my concern, laughed, and sarcastically told me, “There’s no way we’re paying for it.” He said it was my responsibility to avoid the cones — even while FSD was fully engaged. That kind of response is unacceptable, especially when Tesla markets this system as being capable of handling real-world driving environments like construction zones. This incident shows a significant failure in Tesla’s FSD recognition and decision-making systems. There were no warnings or alerts prior to the impact. The event raises major safety concerns, not only for the driver but for anyone on or near the road. I’m reporting this because I believe Tesla's current version of FSD has critical deficiencies that could lead to injury or worse. The system should never ignore basic road hazards like cones, and the company's dismissive response after the fact is alarming.

STEERING,ELECTRICAL SYSTEM,SERVICE BRAKES
1 complaint
On Saturday, [XXX], vehicle was at a T-intersection, in normal drive mode (not FSD) waiting for traffic to pass to enter roadway from side road. Vehicle suddenly started to go backwards. Had to press on the brake to stop. Rebooted the car and the error happened again. Tried to reverse from the intersection to be able to stop the vehicle in a safe location, but steering (drive by wire) was not working. Had to drive with one foot on the brake and one on the electronic throttle to go backwards, even though vehicle was in drive model. Display read 'Traction Control Disabled', 'Adaptive Rider Control Degraded', 'Stability Control disabled', and 'Emergency braking not available'. Tesla service was requested and vehicle was transported to Tesla facility in Tampa. I am very concerned that had this failure occurred while on interstate could have resulted in a serious accident. SUSPENSION,ELECTRICAL SYSTEM
1 complaint
MoThe air suspension is stuck saying it cannot adjust. The car states that there is an electrical issue. As an electric car electrical issues seems to be a concerning error message to receive. The service center has stated a part was replaced two times but the error message remains. The error is in reference to exhausting from the valve block. This removes the ability to utilize any of the adjustments and potential water wade mode. The service center said a redesign is in the works, but no such redesign is in the works.
