Mazda issued a safety recall (Mazda safety recall 4320A, NHTSA campaign number 20V-063) to address daytime running light flickering/failure. At the time, only the passenger side daytime running light was replaced under the recall by Mazda, with a gasket/gas absorption sheet being installed on the driver's side. The driver's side daytime running light is now flickering/dim. Mazda did not repair the safety defect properly on the driver's side for the daytime running lights under Mazda safety recall 4320A, NHTSA campaign number 20V-063, because Mazda only installed the gasket/gas absorption sheets that have since failed, causing the daytime running lights to flicker, increasing the risk of crash. Mazda has refused to cover the cost for replacement of the daytime running lights per safety recall notice 4320A / NHTSA 20V-063, even though replacement of these lights is covered by the recall. Both a Mazda dealer and Mazda corporate customer service have been contacted regarding this issue and both have refused to provide the remedy as outlined in the safety recall. Mazda has provided an estimate of $1,466 to replace the daytime running lights. Only one headlight was addressed by dealer in repair for 4320A | NHTSA #20V-063 and they're refusing to cover the other side. The other DRL led light is now out from seemingly the same cause. They stated they did not fully follow the procedure in the documents I found regarding this recall when they repaired the first one because they "pre authorized" the swap of one side on "good faith" so there is not a record I can cite around the serial or lot numbers of the new or old lights on my own.

The vehicle was taken to the dealer where it was diagnosed with front passenger's headlight failure. The contact was informed that the passenger's side front headlight needed to be replaced. The vehicle was not repaired. The contact received notification NHTSA Campaign Number: 20V063000 (Exterior Lighting) in 2020; however, only the front driver's side headlight was replaced at that time. The manufacturer was made aware of the failure but provided no assistance. The vehicle was not repaired. The contact was informed that NHTSA Campaign Number: 20V063000 (Exterior Lighting) had expired. The failure mileage was approximately 100,000.

The contact checked the lights and noticed that the lights would not display or activate properly. There was no warning light illuminated. The dealer was contacted but the vehicle was not diagnosed. The vehicle was not repaired. The manufacturer was notified of the failure and the contact was informed that the VIN was not associated with an unknown recall due to the date the vehicle was manufactured. from my service mechanic... "Rear brakes at 8mm/8mm on the right side, but 4mm/6mm on the left. These brakes were replaced 1500 miles ago and should not be showing this kind of wear. Examined brake pads, found evidence of overheating. Scanned vehicle, confirmed no codes from Electronic Parking Brake. Actuated parking brake and confirmed it is engaging and releasing as commanded. Found a Technical Service Bulletin related to parking brake drag on this vehicle. The brake pad damage pattern is consistent with what we observed today. Mazda recommends replacing the affect caliper(s) with an updated version to address this issue. Confirmed by lot number that current calipers are older version. TSB attached. Recommend replacing calipers and rear brake materials."

After having brakes serviced brake lights continue to come on. After research from many other complaints they are saying it’s the emergency parking that needs replaced? We shouldn’t have to replace this every time the brakes are replaced.

2nd time in 40k miles right rear caliper locked up mechanic suggest getting both left and right replaced figuring if one failed its only a matter of time, calipers should last 10 years and 2nd replacement in 4 years, this is a bad safety issue

My 2016 Mazda CX-5 has had ongoing issues with the infotainment. It freezes up in cold weather and the screen goes blank for long periods of time. This has been an ongoing issue with Mazda vehicles. I had to replace the SD Card in December 2024. The cost was $326.50. There is currently a class action lawsuit by Frederick Law Group 836 Philadelphia Street, Indiana, Pa 15701, Phone number 724-801-8555. When the infotainment goes out during phone calls or navigation, it is a distraction while driving. I called Mazda and chatted with Travius about this issue and forwarded by receipt for the part. He said Mazda would not reimburse me. Can the NHTSA help in getting the reimbursement do you have any suggestions?

My infotainment screen for my Mazda CX5 has bubbles on it. I contacted Mazda and they said this is an issue that many people have complained about, but there is no recall. The screen on the inside has a screen protector that is peeling off and eventually I will not be able to use my touchscreen. There should be a recall or an investigation into this issue. Please help or let me know where I can get more information. I should not have to pay $900 for a new screen kit.

Ghost Touch on infotainment system. The infotainment system will randomly make selections - make calls, change gps location destination, change settings, turn off screen, give verbal instructions, etc while driving. This is very distracting and can be a safety issue.

Front windshield wipers stopped working, front windshield washer still working. Fuse was checked, fuse is good.Vehicle is available for inspection upon request. My safety is put at risk because of lack of visibility when driving in rain. The problem has been confirmed by a Mazda dealership. The vehicle and the components have been inspected by a Mazda dealership. No warning lights or messages appeared before or after the front windshield wipers stopped working. So if a person is driving the car, there is no indication that the front windshield wipers will not work if it starts raining and the driver would need to use the front windshield wipers. This is a safety hazard because the driver will not be able to see, possibly causing an accident. This Mazda CX5 does not have automatic front windshield wipers (they do not come on automatically when it starts raining).

In September of 2023, while driving I attempted to use the windshield wipers, and they would not function. We live in a rainy part of the country, so having non working wipers is a major safety issue. Research for possible causes pointed to the body control module which had been recalled on Mazda 3s in recent years. I took the car to the dealership, and they traced the problem to an internal failure of the body control module. I do not have the component to inspect, and I do not know what the dealership did with it after removal. We filed a complaint with Mazda in October of 2023. They gave us case #001-463-33. They were not receptive to feedback, and simply stated many times that there was not a current recall for our vehicle. There were no warning signs/messages/ lights before the wipers stopped working, and they had worked normally the day before.

Rear Video Camera / Back-up Camera Display Malfunction: The display in the dashboard continuously resets, causing the view from the back-up camera to disappear completely or dim to to the point where it is not viewable. This causes a safety hazard when backing up and often occurs once the vehicle is already in motion. Mazda dealer's service center reports Mazda is aware that this is a new, widespread problem, but to check for updates in 4 weeks.

2016 Mazda CX-5 windshield wipers - Correctly operating windshield wipers have four modes: single wipe, intermittent wipe, slow continuous wipe, high speed continuous wipe. One morning (September, 2021) used the single wipe mode to clear overnight accumulated moisture (car parked outside) but wiper made five passes instead of one and then stopped in an 'up' or 'non-parked' position. Tried the other three modes and all three behaved the same way, five wiper passes and then the wipers stopped, sometimes upright blocking forward vision, sometimes at an angle, but never in the 'parked' position. Driving in rain or snow requires activating the wipers every 30 seconds or so (approximate time of 5 wipes). Have had the wiper motor (which includes a circuit board) replaced twice to no avail. Today had wiper switch replaced, again to no avail. This problem seems to be similar to windshield wiper problem reported regarding 2016-2018 Mazda CX-3's.

About a month ago, started hearing a whining noise from the automatic transaxle when driving at different speeds. The more speed, the more noise you hear inside the vehicle. It occurs with all types of driving, acceleration, deceleration and steady speed. Took my Mazda CX-5 into auto experts and found there is a Service Bulletin regarding this issue in 2013-2016 Mazda CX-5's. There is a damaged bearing in the transaxle due to improper configuration of the transaxle case. There needs to be a recall for this issue, not just a service bulletin. I cannot afford a new transmission and replacing the transmission with a like for like or used transmission, there is no guarantee the issue won't come back.

I was driving on the freeway, going 60-65mph for about 10 minutes. All of a sudden, the engine went from ~2k RPM to ~4 RPM. The speed stayed the same. And then it happened again a few minutes later. A few days after this incident, while driving on the freeway, going 60mph, the car refused to go past 60mph. This also happened ~2 weeks prior. This would have caused an accident had I needed to speed up to avoid an issue but my car refused to go any faster on a flat road.

WHILE GETTING OFF A FREEWAY EXIT SUDDENLY THERE WAS SOME NOISE IN THE ENGINE AREA FOLLOWED BY MAJOR RATTLING EVERY TIME THE CAR SLOWED DOWN OR SPED UP, MAKING IT UNDRIVABLE. THE PROFESSIONAL DIAGNOSTIC WAS THAT SOMETHING WITHIN THE TRANSMISSION WAS BROKEN/COMING APART. THE CAR HAD ONLY AROUND 27000 MILES ON IT AND THIS HAPPENED WITHIN MONTHS OF DRIVE-TRAIN WARRANTY EXPIRING.

WHEN THE CAR IS COLD AND PARKED THE TRANSMISSION WILL LURCH INTO DRIVE SOMETIMES AND EVERYTIME WHEN THE CAR IS COLD THE TRANSMISSION WILL GET STUCK IN THIRD OR FOURTH GEAR AND THE CAR WILL NOT ACCELERATE UNTIL I FLOOR IT UNTIL THEN THE CAR WON'T SHIFT OUT OF GEAR FOR A FEW HUNDRED FEET AND CONTINUE TO BIG DOWN THE ENGINE, THEN AFTER THAT THE TRANSMISSION WILL DOWNSHIFT AND THE CAR WILL START TO ACCELERATE. IF I MANUALLY SHIFT THE CAR INTO FIRST THE PROBLEM WON'T OCCURRE AS BADLY. THIS SOFAR ONLY HAPPENS ONCE WHEN THE TRANSMISSION AND ENGINE ARE COLD. BUT EVEN IF I LET THE CAR SIT AND IDLE FOR A FEW MINUTES IT'LL STILL HAPPEN.

We had another CX-5 in the back that we used to test whether or not it was either the throttle body or the BCM ( Body Control Module ) or the PCM ( Power Control Module ) not working that is causing your vehicle to not wanting to turn on The throttle body passed we transferred it over from vehicle A ( Being the donor ) to vehicle B (Being your vehicle ) and it was still doing the same thing which it would crank but not fully turn on and that is how we ruled that part out The BCM we also transferred over from vehicle A to vehicle B and same thing it would crank but not fully turn on This is what the mechanic at Mazda Collision Center of Irving stated: The PCM has power and is communicating with everything else in the vehicle. If it was the PCM then nothing would work and we wouldn't be able to even cycle the power as if we were going to turn on the vehicle We brought this over to our Mazda Foreman and we showed him the codes it was throwing and what was going on and explained everything and he did tell us that it could be the main harness that is causing all of this he is not for sure just because he is not the mechanic involved in this but he said that is a good start. I am uncertain exactly what is wrong with the vehicle. When I took it in to be serviced, it worked. The mechanic then said he couldn't work on it anymore after trying to fix the body work because the electrical system had failed. I then had it towed to Mazda to see if they could fix the issue. I had originally took it in because of a minor fender bender. I am unclear as to why it has taken four months and my car still doesn't work and I do not know how to afford buying a car to replace it when I cannot trade it in since we cannot resolve the electronic portion through insurance.

The used 2016 Mazda CX5 I just bought apparently has the "ghost touch screen" where the Infotainment system will randomly pick controls, place phone calls, change navigation. How can this not be a safety recall? I'm afraid to drive this car and not have full control of the navigation system or controls. This is DANGEROUS to have this happen when driving!! Mazda should be held accountable for this clear safety problem.

The contact owns a 2016 Mazda Mazda CX5. The contact stated that on two separate occasions, the vehicle unintentionally accelerated without warning in an attempt to park the vehicle. On the first occasion, the vehicle drove in between two trees with the contact's foot on the brake pedal before the vehicle came to a stop. The air bags did not deploy upon impact and no injuries were reported. The vehicle was towed to an independent body shop where the vehicle was repaired for minor damage. The contact was also informed that no computer codes indicated any signs of a mechanical failure. On the second occasion, the vehicle crashed into a parked vehicle while again attempting to park the vehicle in a parking lot. The force of the collision caused the already hit vehicle to crash into yet another parked vehicle. The air bags did not deploy upon impact and no injuries were reported. A police report was filed. The vehicle was towed back to the same independent mechanic where the code reader indicated that the vehicle had a low battery; however, the battery did not need to be replaced. The dealer nor the manufacturer had yet to be notified of the failure. The vehicle remained in the possession of the independent mechanic and had yet to be repaired. The failure mileage was approximately 70,000

The car engine is eating oil. There were no warning lights on the dash to indicate this. i happened to check the oil and it was 3 quarts low. This could have caused the engine to malfunction while driving if not caught in time. This started around 70K miles. Coffmans Service Center in Warminster, Pa has confirmed the car was spraying oil on [XXX] and performed a clean combustion chamber with iridium spark plugs, replaced intake manifold gaskets and PCV valve, and Ow-20 oil and new filter. I contacted the manufacturer on [XXX] case # [XXX] and it was dismissed. I did read online that this Mazda car has had a problem with eating oil around the 70K mile mark. The problem is still occurring. I read that there was a class action lawsuit against Mazda. [XXX] et al v. oil Mazda motor of America. However, the class action lawsuit doesn't include the 2016. I think it starts around 2021. I uploaded the receipt for the repair on an older complaint. the complaint number for that one is: 11627409. Currently there are over 450 complaints on the 2016 Mazda CX5 on the NHTSA website. Can we get a recall on this? Catastrophic engine failure. Needs a total engine replacement, regular oil changes. We went to get the oil changed within 1,000 miles of the suggested mileage range and it was “out of oil”. The dealership told us we needed a new engine. It hasn’t been driven and sounds like a helicopter when turned on. We have had regular maintenance on the vehicle, one owner. It started acting up and was trying to stall out on the highway. Then seized up and wouldn’t go over 10mph. It’s totaled and we feel strongly that this is an extension of the current lawsuit covering many years of Mazda models.

My daughter was driving her 2016 CX5 when the coolant temperature warning message displayed on the instrument panel, advising her to pull over immediately. She had just driven on to the expressway, but luckily was still in the right lane and was able to pull over safely. If she had been in any other lane of the busy highway when this occurred it could have been a serious safety issue for my daughter and the other drivers on the highway. I contacted AAA to tow the car to my local mechanic. The service tech reported that they began by plugging their diagnostic tool into the car and they received two engine codes: P111A-00 and P011A, and that a software update must be performed first. I had the car towed to Turnersville Mazda. They said that there would be a $215 per hour diagnostic fee. They found that the thermostat needed to be replaced as it failed to open during engine operation. This service incurred a $397.55 cost. After replacing it they noticed that a milky substance was produced, indicating motor oil mixing with coolant. They recommended replacing the engine due to a crack in the cylinder head at a cost of $10,300. For some reason the service tech elected to replace the spark plugs and update the software AFTER the catastrophic engine issue was discovered, incurring an additional $330 in charges. Prior to this incident there were no issues with the vehicle, no warning lamps or symptoms of any kind.

THE DRIVER'S DOOR ON MY 2016 MAZDA CX-5 HAS A MALFUNCTIONING SENSOR THAT SOMETIMES INDICATES THE DOOR IS CLOSED EVEN WHEN IT IS OPEN. SINCE THE DOOR COULD BE UNLATCHED WITHOUT AN AUDIBLE CHIME SOUNDING, IT'S POSSIBLE THAT AN OWNER COULD BE UNAWARE THEY ARE DRIVING WITH THE DOOR OPEN. THUS, THIS VEHICLE FAILS TO CONFORM TO FEDERAL MOTOR VEHICLE SAFETY STANDARD (FMVSS) NO. 114, "THEFT PROTECTION AND ROLLAWAY PREVENTION."

THE PASSENGER SIDE MIRROR FOLDED IN BY ITSELF WHILE I WAS DRIVING MY MAZDA 2016 CX-5 ON A LOCAL CITY STREET AT ABOUT 15 MPH. THE PLASTIC OUTSIDE HOUSING OF THE MIRROR POPPED OUT AND FELL ONTO THE ROAD. I RETRIEVED THE TRIM PIECES OF THE HOUSING BUT ONE PIECE OF TRIM WAS CRACKED. I REINSERTED THE TRIM AND TAPED UP THE CRACKED PIECE. MAZDA SAYS IT DOES NOT SELL THE REPLACEMENT PIECE OF TRIM AND WANTS TO REPLACE THE WHOLE MIRROR FOR A COST OF $500.

LIFTGATE RECALL BY MAZDA IN OCTOBER 2016 WAS NEVER COMPLETED AS "PARTS NOT AVAILABLE", BUT THEIR WEB SITE SHOWS THIS VIN AS HAVING COMPLETED THE REPAIR, WHICH IS ERRONEOUS. THIS INDICATES THAT SOMEONE AT MAZDA CHANGED THE DATA TO SHOW IT AS COMPLETED WHEN IT WASN'T COMPLETED.

The front passenger seat warmer caught fire as i was driving the vehicle. No fuse tripped or anything when it over heated. The seat not just melted but actually caught fire and filled the vehicle with smoke.

Vehicle was t-boned by another vehicle that was traveling 40mph. Both front seats are now loose, and have been for some time. Insurance refused to cover seat replacement during repairs in fear of going above the “totaled” threshold and it being a total loss. Now we are concerned about the safety of our vehicle, and we are scared to drive it on major highways where the risk of accidents are higher.

MIGHT HAPPEN TODAY AND NOT AGAIN FOR SEVERAL WEEKS OR MORE THAN A MONTH BUT COULD HAPPEN AGAIN TOMORROW. SEAT ADJUSTMENT SWITCH WORKS NORMALLY OTHERWISE. DEALER UNABLE TO DIAGNOSE OR DUPLICATE PROBLEM. NO FURTHER ASSISTANCE OFFERED OTHER THAN WE MUST BE RUBBING AGAINST THE ADJUSTMENT SWITCH AS WE EXIT THE VEHICLE. THIS WOULD BE IMPOSSIBLE SINCE THE SEAT ADJUSTMENT SWITCH REQUIRES CONTINUOUS ACTIVATION RATHER THAN A MOMENTARY INPUT WHILE EXITING THE VEHICLE.

Denso Fuel Pump should be recalled!!! It is not sending fuel to the my engine properly it's running rough at even 0-30 miles, becomes difficult to start (cold or mild engine), sputters and idel, and suddenly stalled out in the middle of the road several times, put us in danger from other motorist. My check engine warning indicator is also on, it's not shifting correctly and is lacking rpm power. Mechanic put a code scanner on it and found P0192 (low fuel rail pressure) and P0171 (System too lean)... I have the pictures. Several auto makers have issued recalls, including Mazda. But Mazda only went back as far as 2018. Toyota recalled cars as far back as 2013. Mazda needs to do the right thing or be forced to do so!

Fuel system failure at only 60k miles on car. Engine light came on dash & my car just randomly started failing as if it wasn't getting gas, trying to cut off while driving down the road!! Huge hazard!! This could have caused an accident but i luckily going in a 25mph zone. Local shop diagnosed the fuel pumps failed (this car has 2 of them) and they also were able to reproduce the same issue when going to start the car to move in and out of the bay. I have called mazda and done the research- they did not put a recall on this year but it appears other years gave been recalled with fuel system related issues. This should be looked into as I've found NUMEROUS complaints on this year and model!

The low pressure fuel pump on my Cx-5 failed on my drive back home, while in a remote area. I was able to limp the vehicle home with a check engine light that illuminated (at the time of the incident) and erratic vehicle performance (uneven power deliver). This occurred on level ground at highway speed - no incident precipitated the part failure. Once home the following day, the vehicle would not restart. Upon diagnostic review, the vehicle had thrown two codes, one being a low fuel rail pressure (P0192 and P0171). This is a know issue for these defective (Denso) fuel pumps. The manufacturer should extend the recall to include vehicles manufactured prior to 2018.

My son was driving the car when it suddenly lost power and would not shift correctly. This occurred without warning while in moderately fast traffic that was moving at 40-50 mph. There were no warning lights illuminated prior to the problem occurring. Upon reviewing trouble codes, it is likely that the low pressure fuel pump located in the fuel tank has begun to fail. When the tank gets low on fuel, about 1/4 tank, the low pressure fuel pump is unable to supply enough fuel to the high pressure fuel pump. This results in sudden loss of power, and also hard starting. I believe the fuel pump is the problem because it was manufactured by Denso, who recalled 2,000,000 similar fuel pumps two years ago. Those pumps were recalled because they created the exact problem we experienced. Toyota recalled vehicles using these pumps as far back as 2013. However Mazda and other OEM's were allowed to only recall vehicles going back to 2018. EVEN THOUGH IT IS THE SAME PUMP. When the manufacturing defect that causes a safety problem is well known, why are some manufacturers allowed to limit their recall? Why aren't all cars using the pump forced to be recalled? If a recall is excessive, why aren't manufacturers required to provide an extended warranty? NHTSA, you dropped the ball on this, and you need to fix it.

FUEL FILLER PIPE MELTED CAUSING EXTENSIVE FIRE AND DAMAGE. I HAVEN'T HAD USE OF MY VEHICLE SINCE 5/14/19. MAZDA IS AWARD AND DOESN'T CARE. NO ACCIDENT INVOLVED JUST HAPPENED. The airbag warning light is on. I’m concerned the airbag may not deploy in an accident. There was a recall on the airbag, but the dealership only reset the light and did not replace the airbag. I have taken my car to two different Mazda dealerships: One told me I would have to pay for the airbag replacement. The other said the airbag is functioning properly. I’m not sure which one is correct, but the warning light continues to come on. Sometimes it stays on for a couple of days; other times, it flashes on and off intermittently. The airbag light is clearly visible on the dashboard and continues to behave abnormally.

THE VEHICLE WAS TAKEN ROGER BEASLEY MAZDA SOUTH (4506 S IH 35 FRONTAGE RD, AUSTIN, TX 78745) WHERE THEY STATED THAT WAS A COMMON ISSUE; HOWEVER, HER AIR BAG FAILURE WAS MORE VISIBLE AND PROMINENT THAN OTHERS THEY HAD SEEN. THE CONTACT WAS THEN INFORMED THAT THERE WERE NO RECALLS ON THE VEHICLE AND THAT THE REPAIR WOULD BE OUT OF POCKET. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND THE CONTACT WAS GIVEN THE SAME INFORMATION AS THE DEALER. THE VEHICLE HAD YET TO BE REPAIRED. THE FAILURE MILEAGE WAS 43,589.

SUSPENSION

With no prior warning, just after turning into driveway, my car got stuck and would not move. It gave clicking sounds and would not move no matter which gear it was in. The towing service person needed a dolly in order to safely lift the vehicle onto the towing bed because the front, driver’s side tire would not move and was pushing into the fender. So far, the control arm and axel are completely separated from the car. Further investigation is ongoing to determine the extent of the spontaneous injury. This all happened without warning.

The contact owns a 2016 Mazda CX-5. The contact stated while driving 20 MPH, there was an abnormally loud chirping and squeaking noise coming from the driver’s side front wheel. The contact took the vehicle to a local dealer to be inspected however, the dealer could not duplicate or diagnose the cause for the failure. The contact was concerned that the failure was related to the suspension. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 45,000.

THERE WAS A THUMPING SOUND COMING FROM THE RIGHT FRONT WHEEL, AT SLOW SPEEDS ON A DIRT ROAD. WHEN I TOOK THE CAR TO THE REPAIR SHOP I WAS TOLD THAT THE BUSHINGS, LOWER CONTROL ARMS AND STRUTS NEEDED TO BE REPLACED. I KNOW THERE IS A RECALL ON SOME OF THE CARS LIKE MINE, BUT APPARENTLY MINE IS NOT PART OF THE RECALL. HOWEVER, I DO NOT THINK IT IS A COINCIDENCE TO HAVE SIMILAR PROBLEMS.

This vehicle has rear Electric Parking Brakes. Some time it's works sometimes it doesn't release the rear passenger side breaks. The rear passenger side brakes make to much noise and brakes pads destroy rotor earlier then others. Mazda dealers doesn't says to much on that and ask for above $1000 repair to change faulty EPB clipper. But my research says many people have same complaints with this model by mazda built-in around 2016. This is safety issue, if I replace brake pads every 4 or 5 months it cost me money. If I don't I cost me my safety.

Last month, my vehicle suddenly applied the electronic parking brake (EPB) while I was driving. The EPB & related warning lights illuminated & would not release until the vehicle was turned off. After turning off the vehicle, the EPB motor could be heard applying the brake to the rear wheels. When the car was restarted, the EPB would remain off for a period of time but would then suddenly reapply. I took my vehicle to a Mazda dealer & they determined the calipers & EPB motor had failed due to my vehicle not being placed in service mode by the brake servicer. This explanation makes no sense because the brakes were replaced over a year ago (& not since) with thousands of miles being driven since then without this issue arising. I also conducted my own research & found that Mazda has issued at least 3 separate technical service bulletins (TSB) on this issue. The first two recommended replacement of the EPB switch & the third determined that defective calipers were the cause and should be replaced. I asked the dealer to save my calipers & confirmed that they are part of the defective batch noted in this TSB. I have contacted Mazda (including their legal counsel) & they are unwilling to provide any explanation or engage in any sort of investigation. As former legal counsel for an automotive company, I believe this presents a serious safety issue for consumers based upon the fact that a parking brake could randomly engage while driving & cause sudden loss of speed/control. Thankfully, this did not result in harm to my family, myself or others. I am certain that this issue is affecting others based on chatboard discussions I have found online related to this topic (which helped me find the TSB). I am concerned that Mazda may have attempted to undertake a silent recall via this TSB where in warranty customers are covered & out of warranty customers are given the 'service mode' explanation to justify their responsibility. This should have been handled as a recall by Mazda.

ELECTRIC PARKING BREAK (EPB) WARNING LIGHT START BLINKING AFTER THE RELEASE OF THE BRAKE. I LIVED IN A WARM TROPICAL WEATHER. IN MY UNDERSTANDING THIS FUNCTION WORKS ON COLD WEATHER.PROBLEM PERSISTS INTERMITTENTLY. IF VEHICLE TURNED OFF, PROBLEM "DISAPPEARS" BUT APPEARS AGAIN WHEN THE BREAK BUTTON WAS RELEASED. WEEKS LATER THE CAR STARTED TO ACTIVATE THE SMART CITY BREAK ALARM WHEN MOVING IN MY NEIGHBORHOOD AND IN A CITY STREET WITH NO REASON, SUDDENLY, AND BRINGING A SAFETY ISSUE. ALSO STARTED TO MAKE STRANGE NOISES RELATED TO THE BRAKE, SIMILAR WHEN THE BRAKES OR PADS GET WET AND MAKING THE CAR FORCED TO BE MOVED WHEN PUSHING THE ACCELERATOR PEDAL, MAKING THE VEHICLE DIFFICULT TO MOVE, NO MATTER BREAK IS RELEASED AND TURNING IT SLOW AND VAGUE. AN ELECTRONIC DIAGNOSTIC REPORT WAS MADE AND SHOWS THE FOLLOWING CODES : CODE C2005 (ELECTRIC PARK BRAKE MOTOR GEAR UNIT (RH) CIRCUIT MALFUNCTION CODE C112A (INCORRECT ELECTRIC PARKING BRAKE MOTOR GEAR UNIT POSITION) CODE C2006 (ELECTRIC PARKING BRAKE MOTOR GEAR UNIT (LH) CIRCUIT MALFUNCTION.

EBRAKE WOULD NOT DISENGAGE ON THE MAZDA CX-5 2016 GT WITH CITY BRAKE AND ELECTRONIC E-BRAKE AT 36,100 MILES DURING VARIOUS PARKING LOCATIONS SCENARIOS. VEHICLE WOULD START NORMALLY AT PUSH OF START BUTTON BUT THE E-BRAKE LIGHT WOULD STAY ON IN THE FLASHING MODE. TURN ENGINE OFF/RESTARTING WOULD RESOLVE THE PROBLEM TEMPORARILY JUST TO RETURN TO THE SAME ISSUE AT RANDOM PARKING LOCATIONS. DEALERSHIP SUGGESTED A REPLACEMENT BATTERY MAIN 12 VOLT BATTERY FOR $185+TAX+INSTALLATION. BATTERY REPLACEMENT SUGGESTED BASED ON A CLAIM OF LOW VOLTAGE MEASURED AT THE E-BRAKE SWITCH LOCATED ON THE CENTER CONSOLE. THERE WAS NO OTHER SOLUTIONS PROVIDED BY THE DEALERSHIP FOR FURTHER TESTING/ISOLATION OF ISSUE PRIIR MAKING A PAYMENT, TO INCLUDE CONFIRMATION THAT IN FACT THE BATTERY IS THE CULPRIT. UTILIZING AN ALTERNATE/TEST BATTERY COULD BE A TEMPORARY TESTING SOLUTION. PAYMENTS WERE MADE AS REQUESTED AND THE ISSUE OF THE FLASHING E-BRAKE LIGHT FLASHING/E-BRAKE NOT DISENGAGING IS LESS APPARENT WITH 2 INCIDENTS 1 MONTH AFTER BATTERY REPLACEMENT. INCIDENTS OBSERVED WHILE IN COVERED/UNCOVERED PARKING SCENARIOS, OUTSIDE TEMPERATURE OSCILLATE FROM 75 TO 92 F. VEHICLE REAR BRAKES WERE REPLACED UNDER 5000 MILES BY DEALERSHIP DUE TO NOISES AND PREMATURE WEAR. VEHICLE ALSO HAS A MAZDA REMOTE START SYSTEM INSTALLED BY DEALERSHIP WITH A HISTORY OF ELECTRICAL FAILURE OF THE A/C COMPRESSOR. DOCUMENTATION AVAILABLE FOR REVIEW.

THE LED DAYTIME RUNNING LIGHTS ON BOTH SIDES OF MY 2016 MAZDA CX-5 HAVE BURNED OUT/STOPPED LIGHTING AFTER 39K MILES AND OUT OF THE WARRANTY PERIOD. THE REPAIR REQUIRES THE REPLACEMENT OF THE ENTIRE HEADLIGHT ASSEMBLY WITH AN ESTIMATED COST OF $1200 PER ASSEMBLY. ($2400 TOTAL). BECAUSE THIS IS A SAFETY ISSUE, AND ONE APPARENTLY KNOWN TO MAZDA, THIS SHOULD BE A RECALL.

LED DAYTIME RUNNING LIGHT ON PASSENGER-SIDE HEADLIGHT FLICKERS AND IS VERY DIM. ON A FEW OCCASIONS, THE DRIVER OF VEHICLE IN FRONT STATED THAT IT LOOKED AS IF MY HEADLIGHT WAS OUT AS WELL. VEHICLE IS ONLY 3-YEARS OLD. MILEAGE SHOULD NOT AFFECT ELECTIRAL LED COMPONENTS. DRL'S OR HEADLIGHTS ARE REQUIRED DURING ADVERSE WEATHER CONDITIONS OR WHEN OPERATING THE WINDSHIELD WIPERS.

WHILE ON A RECENT HIGHWAY ROAD TRIP, MY 2016 MAZDA CX-5'S DRIVER SIDE DAYTIME RUNNING LED LIGHTS, WHICH COME EQUIPPED WITH THE TECHNOLOGY PACKAGE, STOPPED ILLUMINATING PROPERLY. THE DAYTIME RUNNING LAMP LED LIGHTS IN QUESTION WILL FLICKER OR NOT ILLUMINATE AT ALL. I FOUND THIS TO BE A SAFETY ISSUE AS DAYTIME RUNNING LIGHTS ARE EQUIPPED ON VEHICLES TO ENHANCE VISIBILITY AND SAFETY. A FLICKERING DAYTIME RUNNING LAMP CAN CAUSE SERIOUS SAFETY CONCERNS RESULTING IN DISTRACTED DRIVING RELATED COLLISIONS. MAZDA ADVERTISED THESE SPECIFIC HEADLAMPS AS BEING A LONG LASTING AND PROVIDING ENHANCED SAFETY AS THEY ADAPT TO CHANGING ROADWAY CONDITIONS. UPON CONDUCTING INDEPENDENT RESEARCH INTO THE DRL (DAYTIME RUNNING LAMP), IT WAS LEARNED THAT MANY OTHER OWNERS OF THE 2016 MAZDA CX-5, WITH THE TECHNOLOGY PACKAGE / LED HEADLAMPS, HAVE BEEN EXPERIENCING THIS ISSUE ALMOST AS SOON AS THE BASIC FACTORY WARRANTY EXPIRES. IT WAS THEN LEARNED THE AVERAGE COST TO GET THIS SITUATION REMEDIED WOULD RESULT IN HAVING TO PAY ALMOST $1500 - $2000 TO GET THE WHOLE HEADLAMP UNIT REPLACED. RESEARCH ON THE NHTSA WEBSITE REVEALED A FAULTY GASKET AT THE BASE OF THE HEADLAMP UNIT WHICH CAUSED MOISTURE/GAS TO DEVELOP ON THE INTERIOR OF THE HEADLAMP LENS, THIS IS ALSO PRESENT ON THIS VEHICLE. I FEEL THIS COULD POTENTIALLY BE CAUSING THE HEADLAMP DRL TO MALFUNCTION DUE TO A MOISTURE BUILD UP, OR THE UNIT IS ALL TOGETHER DEFECTIVE FOR A CERTAIN GROUPING OF CX-5'S WITH THIS HEADLAMP ASSEMBLY I FEEL A RECALL INVESTIGATION SHOULD BE CONDUCTED TO DETERMINE THE SOURCE OF THE MALFUNCTION. I FEEL THIS IS A SIGNIFICANT ISSUE AS IT WAS LEARNED THERE WAS A RECENT NATIONAL BACK ORDER FOR THESE HEADLAMPS, I FIND THAT SUSPICIOUS IF THEY WERE TO BE IN PROPER WORKING ORDER.

PASSANGER SIDE DAY TIME RUNNING HEADLIGHTS BEGAN FLICKERING FOR ABOUT A MONTH AND THEN SIMPLY WENT OUT. THIS CAR IS BRAND NEW WITHIN THE PAST YEAR AND A HALF AND IS ABSOLUTELY RIDICULOUS. THE DEALERSHIP HAD TO REPLACE THE FULL HEADLAMP UNIT AS IT IS LED AND MAZDA KNOWINGLY INSTALLED THESE FAULTY HEADLAMPS. I HAVE SEEN HUNDREDS OF OTHER DOCUMENTED CASES OF THIS ISSUE ON THE INTERNET.

THE FUMES MADE ME AND MY DOG NAUSEOUS.I HAD TO SWERVE ACROSS THE HIGHWAY AND STOP ON THE SHOULDER TO EXTINGUISH THE SEAT FIRE/BURN. THE FUSE WAS PULLED FOR SAFETY. THE SEAT IS IN STORAGE FOR INSPECTION. MAZDA WILL BE CONTACTED.

While driving on a remote highway the seat heating was on on both the driver and passenger seat. After a while it felt way ti hot and smelled like something burning and found the passenger jacket (that was taken off and left behind his back to be extensively burnt. We stopped and checked the seat. We found a big hole and the seat was damaged as well as the 300 dls coat. Plus the risk of having burns I. The passenger back, which was only protected by the cost and other layers of clothing

The contact owns a 2016 Mazda CX-5. The contact stated that upon approaching the vehicle he observed a small crack on the top left corner of the windshield. Over time the crack proceeded to progress across the windshield. The contact stated that the windshield was not impacted by an object or projectile. No warning lights were illuminated. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was repaired, but the failure reoccurred. The manufacturer was not notified of the failure. The failure mileage was 40,000.

THE CONTACT REFERENCED MANUFACTURER COMMUNICATION NUMBERS: 10091444 AND SA218, WHICH STATED THAT AN OVERHEATED HEATER CORE WAS DUE TO CLOGGING. WHILE DRIVING ON THE HIGHWAY, WE NOTICED A CRACK THAT BEGAN AT THE BOTTOM CENTER OF OUR NEW CX-5'S WINDSHIELD. THERE HAD BEEN NO IMPACT BY A ROCK OR ANYTHING ELSE. IT BEGAN AT THE VERY EDGE OF THE WINDSHIELD, AND WENT UPWARDS. WE WERE ON A WEEKEND TRIP, AND THERE WAS NO NEARBY MAZDA DEALER. OVER THE MILES THE STRAIGHT CRACK GREW INTO QUESTION MARK SHAPE ABOUT 10" HIGH. ARRIVING AT OUR DESTINATION, THE DEALER CLAIMED THERE WAS A TINY IMPACT MARK EXACTLY UNDERNEATH THE WIPER BLADE. I WAS NOT ABLE TO SEE ANYTHING EXCEPT THE CRACK, AND I KNOW THERE WAS NO IMPACT OF ANY TYPE. The vehicle's air conditioner expansion valve is defective/malfunctioning. While driving the vehicle with the air conditioning operating a loud whooting noise fills the interior of the vehicle. The noise sounds like the siren/hooting sound the police make when they want you to pull over or get out of their way. But the sound is loud and comes from inside the car jarring the driver and passengers. I purchased the car in early 2017 new. I first noticed the AC issue in the summer of 2018. I brought it up to Herzog-meir mazda (where i bought the car) when scheduling an oil change. they said they could not replicate it and so there was nothing they could do. I brought it to them at least 4 times with the same issue. Each time they sent me on my way and said they couldn't replicate it. I brought it up to Foothills Mazda in Spokane in 2023 and again they said that they couldn't replicate it and sent me on my way. This past august i had to schedule the car for an update covered by a class action lawsuit and had them look at the noise issue again. Prior to scheduling I has research online if others have had the issue and low and behold lots of mazda owners have. It is a known issue that mazda has created a "helpline" to deal with it. So I brought it up while scheduling it and asked to make sure they researched the AC expansion valve prior to my appointment (more than a month out). The Mazda lady that scheduled my appointment heard the noise several times with I was idiling the car and finishing scheduling the appointment. The first available appointment wasn't until Oct 1st so of course it wasn't hot enough outside to replicate the situation that causes the noise. 11/19/2023 - About 8 pm. on November 17, 2023, I was parking at my condo parking lot and slowly drove the car into the parking space when all of a sudden the car accelerated on its own. I immediately pressed on the brake pedal and it stopped. I felt fortunate that nobody was on the sidewalk. The same thing happened about a month ago but I thought it was just something unusual but now that it happened again I am very concerned and I am going to call the Maxda dealership to see what they say.

My EPB or Electronic Parking Brake started to Malfunction. It will lock up during parking, and driving and stay locked. It will flash Malfunction light, and the Parking Brake light on the dash will come on, and off. Sometimes If I turn off the vehicle for a while, it will again function. I have not been driving this vehicle, because of the issue I am very scarred my brakes will lock up when it is icy or wet, or slick, and I will injure myself or others Unintentionally. when I first had this issue, I decided to do research, and it seems to be a very common problem across all 2016 model Mazda Cx-5 with EBP. I suspect the EBP module is defective, and I cannot afford to pay the dealer 1500 to replace this part to correct the Safety Hazard Mazda originally created. This should never happen to me or the countless others whom own the 2016 model with EBP. A quick google search will result in countless claims of this same issue with the 2016 model.

Car: 2018 Mazda CX-5 Touring with approximately 39,000 miles. Issue: Burning smell coming from engine compartment- Oil leaking from engine. Problem: Cracked cylinder head as diagnosed by dealer and to be replaced under Mazda Power Train Warranty. Safety was a concern not knowing the issue. Concerned of engine failure, fire, and overall safety. Warnings: No warnings or check engine light was also concerning. Time frame: 10/1/22 evening- noticed slight burning smell which lasted only a few minutes. No evidence of oil leaking. - 10/2/22: Slight burning smell occurred briefly on and off through out late afternoon. No evidence of oil leaking. Smell dissipated. - 10/3/22: Stronger presence of burning smell. Started to notice oil leaking from under car. - 10/4/22: Car inspected by local mechanic and advised to bring to dealer for further assessment. Mechanic observed increased oil leakage and stated oil was coming from within engine. Car towed to dealer, evaluated with a cracked cylinder head. Repair to be covered under Mazda Power Train Warranty. Additional concern: Short and Long Term residual effects on other engine components. Other reports: none. Awareness: I’ve come to understand that there seems to be similar issues with certain model years/production runs that might not be reported due to being covered under warranty. The consumer provided the correct VIN for the vehicle.

Dealer identified drive belt tensioner as leaking and in need of replacement on a Mazda CX-5 with less than 30k miles on it. Many owners have reported identical failures at or around 30k. An insufficient part that is failing so frequently, this early on with low mileage is a significant safety issue.

THE VEHICLE FINALLY CAME TO A STOP AFTER BECOMING WEDGED UNDER THE HAND RAIL AND BUMPING A PARKED CAR. DAMAGE WAS TO THE R FENDER, FRONT BUMPER AND HOOD A TOTAL OF $5,100 WORTH OF DAMAGE. VEHICLE WAS TOWED TO DEALER. MAZDA WOULD NO LOOK INTO WHAT OR WHY IT HAPPENED, OUR INSURANCE COMPANY SENT OUT A MEMBER OF THEIR FORENSIC GROUP TO INVESTIGATE THE PROBLEM. I HAVE A FULLY LOADED CX5 GRAND TOURING 2016 ---WHICH AFTER DRIVING 500 MILES, THE TRANSMISSION FAILED. WHEN PUTTING THE GEAR IN DRIVE, THE CAR DID NOT MOVE--AND ACTED AS IF IT WAS IN NEUTRAL. AFTER A BIT, IT WOULD LURCH FORWARD INTO DRIVE--WHICH WAS SCARY AND DANGEROUS. MY EXPERIENCE WITH THE TRANSMISSION IS VERY SIMILAR TO OTHER REPORTS ON THIS SITE AND SHOULD BE INVESTIGATED. I BECAME STRANDED IN A PARKING LOT UNTIL I WAS ABLE TO GET THE CAR TO MOVE. I HAD TO HAVE IT TOWED FROM MY DRIVEWAY SINCE IT WAS NOT SAFE TO DRIVE. A NEW TRANSMISSION WAS ORDERED FROM MAZDA AND INSTALLED. I RECEIVED MY VEHICLE BACK AFTER 2 WEEKS. AUTOMOBILE WAS ALMOST BRAND NEW, DIDN'T EVEN HAVE LICENSE PLATES ON IT YET, WHEN I PROCEEDED TO BACK OUT OF A PARKING SPOT. THE VEHICLE BACKED OUT BUT WOULD NOT GO FORWARD IN DRIVE. The component that failed was the passenger's seat belt. It became unattached from the seat. It appears as though the bolt that holds the fastener to the seat failed. The vehicle is currently available for inspection as it has not been taken to be repaired, yet. Safety was put at risk by the seatbelt not being attached to the seat which could have resulted in serious bodily injury in the event of an accident. The problem has not been reproduced or confirmed by a dealer or independent service center, yet. The vehicle has not yet been inspected by police, insurance, or manufacturer. No warning lamps, messages or other symptoms of the problem appeared prior to failure.

The check engine light came on when I was in my way home. This morning when I started my car, the engine reved on its own and then it went to 0. This is super dangerous. The car could have stopped in the middle of the freeway. I had my car towed to a local mechanic. The error code that came up was p0192. Looks like there is a recall for the fuel pump issue for many mazda, yet mine isn’t part of it.

I WILL ATTACH THE ESTIMATE TO THIS COMPLAINT. SINCE THIS WAS AROUND THE HOLIDAYS I KNEW MY CAR WOULD BE DELAYED. THIS WAS BY FAR THE WORST EXPIERENCE I HAVE DEALT WITH FOR A MONTH AND HALF AND TODAY I REALIZED MAZDA'S NEGLIGENCE PLAYED A HUGE ROLE WITH EXPENSE AND STRESS OF MY CAR. I take the car to the mechanic and he told me the EPB is bad he changed everything disc, brakes pads,caliper and EPB motor. He said try to don't used the EPB too much or I'm going to see you again with same problem ... So I'm stop to use the EPB for long time. One day I put my car in the carwash one guy hit the EPB when he's done with the car Im get inside I turn on the car and the brake warning light is starting flashing I say fu** not again and yes EPB get stock. The contact owned a 2016 Mazda CX-5. The contact stated while driving 35 MPH, another vehicle going 110 MPH crashed into the rear of the vehicle. The air bags failed to de[ploy. The contact hit his head against the front driver's side window and he became unconscious. The contact stated upon regaining consciousness, he noticed that he was laying flat on his back as the seat had fractured. The contact also noticed that the seat belt had fractured into two. The front of the other vehicle caught on fire and the fire expanded to the rear of the contact's vehicle. The emergency units and State police patrol assisted the contact out of the vehicle through the driver's side door. The emergency units transported the contact to the hospital, where he was informed that he had sustained head, neck, and back injuries. Additionally, the contact was informed that he had sustained a stroke on the left side of his body. A police report was filed. The vehicle was towed to a tow lot, where it was totaled. The dealer and the manufacturer were not notified of the failure. The failure mileage was approximately 58,000. The VIN was unavailable.

Parked by the curb at the Ft. Lauderdale International Airport. I put the car in Park and left the car idling with the air-conditioning running.as it was 95 degrees and about 100 at around 8:00 AM. There was a shuttle van parked by the curb about 11 feet in front of me. I had to exit my car to talk to the driver in the car behind me. I walked to the car behind me and started talking to the driver. I glanced back at my Mazda CX5 and it started rolling forward. I ran up to the car and got the door open and stepped on the brake. The brakes did not engage. I repeated stepping on the brake several times and the car would not stop. I got in the drivers seat to shift the car into reverse and keep my foot on the brake pedal. The car would not shift into reverse. and by this time the Mazda had pinned to the shuttle van a lady who was trying to unload her luggage. On the sixth attempt to shift into reverse the shifter finally moved into reverse and by standers came to the aid of the lady that had been pinned to the shuttle van. Airport security and the Broward County police took the statements. The lady retrieving her baggage declined medical attention. When I checked to see what the police were reporting they did not acknowledge that I was attempting to stop a runaway vehicle. The report said that I failed to put the car in park and left it in drive. It is not possible to get out of a car that is idling and in gear without the car moving forward or backward before you can even get all the way out of the car. This incident happened about mid July in 2017. Near the end of June this year while in my driveway I left the car idling and in park, exited the car walking to the rear of the car and it dropped into reverse. Got back in the car and shut off the engine. July 2017 the local Mazda dealer inspected the vehicle and could not find as problem. Not unlikely with an intermittent electrical anomaly. There were no prior warning lamps in either incident.

1. The airbag sensor is not working. The warning signal continually flashes. My shop had airbag system analyzed. Test results showed it is not calibrated correctly. My Mazda dealer, Luther Premier Mazda, said it is an electrical problem, and I will have to pay for analysis. I paid for this analysis when Sallas had it diagnosed. 2. Due to problems with the windshield, I paid for a replacement myself, not knowing there was a recall. I want Mazda to install a Mazda windshield and calibrate the safety systems.

I own a 2016 Mazda CX-5 Touring with AWD. I purchased it brand new from a dealer--no miles on it. It's been a great car until this Sudden Unintended Acceleration problem (about 60K miles now). It's happened 5 or 6 times--twice when in a drive thru. It seems that if I am idling (not pressing on gas or brake) the car will sometimes suddenly accelerate. I have used suggestions to brake, then place car in neutral. That seemed to resolve the problem. However, recently I pulled up to my (rural) mailbox and the car tried to accelerate out of control. I had the brake on (was on a downhill) when it happened, so this was scary. I really had to press down more on the brake pedal, then shifted to neutral, then to park. The acceleration finally stopped. I did not observe tachometer readings during this or other events, but will pay more attention if/when this happens in the future. I do not drive with both feet, and as these incidents have occurred I have been able to maintain more composure and analyze what is going on. I am not getting my foot caught in the mat and I'm not mistakenly pressing on the accelerator. I've been to my local Mazda dealer (Sport Durst in Durham) on May 13, 2022 and they were unable to replicate the problem, so could not repair or even troubleshoot it. The car was T-boned about a year ago, with some damage to the passenger front tire. Could that have anything to do with my problems? My partner feels this could be an issue with the Throttle Control Sensor (hardware) or with the software managing throttle control (much more difficult to diagnose, he says). I wanted to ask: for anyone experiencing this problem, did replacing the Throttle Control Sensor or diagnosing software problems resolve the issue? And, do you have any recommendations for me? Since the car is (barely) out of warranty, (just over 6 yrs/60,000 miles) would filing a claim with Mazda be a good approach? Would you suggest I just trade the car in?

This is a follow up to NHTSA complaint ID Number 11442992: After doing a fair amount of research the problem was deemed to be the Front Body Control Module. A new replacement was ordered (about $250). It took about 1-1/2 to 2 weeks for the module to be received and about an hour for its installation. So far the wipers seem to be working correctly although they have yet been 'used', only tested. Googling "mazda wiper problem" you will find others reporting similar problems with the Body Control Module. I found this (https://www.mazda3revolution.com/threads/for-those-suffering-wiper-motor-issues-here-is-the-fix.232623/) forum entry when I did the aforementioned google search. And finally, post replacement I have visually inspected the original module and can easily see one of the components marked "2741" with corrosion around its mounting.

The vehicle was taken to an independent mechanic and diagnosed that both the rear brake pads were worn down to 20% and needed replacement. The mechanic informed the contact there was TSB 04-004/19 and 04-005/20 related to the failure and referred the contact to a dealer. The vehicle was taken to the dealer and diagnosed that both the rear brake pads needed to be replaced from normal wear and tear; however, the vehicle was not under warranty. The vehicle was not repaired and remained at the dealer. The manufacturer was made aware of the failure and informed the contact to schedule an appointment with a dealer for diagnosis. The failure mileage was approximately 22,000.

We are having a consistent issue with our son kicking the harness release button located by his feet in the car seat. On numerous occasions we have gone to retrieve him from the seat and the harness is completely loose by kicking and releasing the trigger. This is very VERY unsafe. We have been in contact with the manufacturer via email and they have not responded to our request to remedy the situation. They advised that he may have outgrown the car seat, however he is 8 lbs under the weight limit. Additionally, the warning label on the seat says that they have not outgrown the seat until their head reaches the top of the headrest. He has come nowhere close to reaching this size. Britax offered to exchange the seat. Car seat has been replaced with another brand.

My problem is with the keyless start system. The problem is that if I forget to push the start button to shut down the vehicle and open the door and leave the automobile and shut the door with the key in my possession the auto will keep running until it runs out of fuel. Wouldn't it make more sense for the vehicle to shut down after 3 minutes if the key is not present. As you know people have died from carbon monoxide poisoning by parking in their garage and forgetting to shut down the vehicle and the vehicle keeps running and filling their homes with carbon monoxide. This happened to a older couple in Sarasota Florida with a new car that they just purchased and were not familiar with the starting system. Just wanted to know just how many people have to die before something is done about this problem. I asked my Mazda dealer and they told me that the vehicle will run until out of fuel and basically said that I was stupid for being concerned.

Ebrake engages on its own when starting up the vehicle. You are unable to turn the ebrake off without restarting the car. The repair shop stated it is a problem with the computer module. A quick search of the internet shows that this is a common issue with this year's make and model. Luckily, the ebrake has not engaged itself yet while driving the vehicle.

LARGE FISH HOOK SHAPED CRACK ON WINDSHIELD OF MY 2016 MAZDA CX-5 WITH 23000 MLES. THE CRACK APPEARED ON A 55 DEGREE MORNING AFTER I PULLED MY VEHICLE OUT OF THE GARAGE AND DROVE HALF A BLOCK TO A STOP SIGN AND I WATCHED THE CRACK APPEAR. THE CRACK STARTS AT THE TOP CENTER EDGE OF THE WINDSHIELD GOES DOWN ABOUT 6 INCHES, THEN GOES INTO THE VIEW OF THE DRIVER. IT WAS ABOUT 18 INCHES THAT MORNING AND HAS GROWN TO ABOUT 30 INCHES AND A HUGE FISH HOOK SHAPE IN THE VIEW OF THE DRIVER. I WENT TO THE DEALER AND WAS TOLD IT WAS CAUSED BY A STONE IMPACT. IT WAS NOT AN IMPACT, I SHOULD KNOW, I WAS DRIVING. THE DEALER ALSO TOLD ME THAT IT WAS NOT COVERED UNDER WARRANTY, EVEN IF IT IS A DEFECTIVE WINDSHIELD OR INSTALLATION AT THE FACTORY. WITH A LITTLE RESEARCH I AM SEEING A LOT OF CX-5 OWNERS ARE REPORTING SIMILAR ISSUES WITH THE WINDSHIELDS CRACKING FOR NO APPARENT REASON. THIS IS A SAFETY CONCERN THE NHTSA NEEDS TO INVESTIGATE.

"TAKATA RECALL" ? IN MARCH 2018, I WAS TRAVELING WESTBOUND IN A MAZDA CX-5 ON AN EXPRESSWAY IN LONG ISLAND, NEW YORK. IT WAS APPROXIMATELY 9;45 PM AND THE EXPRESSWAY WAS FAIRLY EMPTY, ROADS WERE COMPLETELY CLEAR, NO RAIN, NO SNOW. OUT OF NOWHERE, A SPEEDING DRIVER SLAMMED INTO THE BACK OF MY MAZDA AT A VERY HIGH RATE OF SPEED. (SHE ADMITTED TO FOLLOWING ME TOO CLOSELY AND NOT BEING ABLE TO STOP IN TIME, SO MY CAR WAS HIT PRETTY HARD. ON POLICE REPORT, SHE WAS CITED FOR UNSAFE SPEED AMONG OTHER THINGS. I WAS ALSO INFORMED THE A POLICE OFFICER THAT SHE DID NOT HAVE A DRIVER'S LICENSE). MY CAR WAS THEN THRUST INTO THE DIVIDER (WHERE THE LEFT SIDE APPROXIMATE CORNER OF THE FRONT OF THE CAR HIT THE MEDIAN) IN THE MIDDLE OF THE EXPRESSWAY AND SPUN QUITE A FEW TIMES. NONE OF THE AIRBAGS EVER DEPLOYED. I HIT MY HEAD AT LEAST TWO TIMES AND I STARTING HAVING NECK AND BACK PAINS AMONG OTHER INJURIES. THOUGH THE CAR HAD EXTENSIVE EXTENSIVE DAMAGES (13K WAS THE LAST TIME I HEARD AND THE CAR IS STILL NOT WORKING FULLY PROPERLY) MY INSURANCE PROGRESSIVE FORCED ME TO TAKE THE CAR BACK WHILE THE OTHER INSURANCE COMPANY DOESN'T UNDERSTAND WHY THE CAR WAS NOT TOTALED. I WAS GIVEN BACK THE CAR A LITTLE OVER TWO WHOLE MONTHS AFTER THE ACCIDENT AND I WAS TOLD THE AIRBAGS WILL NOT DEPLOY IF GOD FORBID SOMETHING ELSE HAPPENS (THE AIRBAG LIGHT IS BLINKING ALL THE TIME), THE HORN DOES NOT WORK, AND NOTHING ON THE STEERING WHEEL CONTROLS WORK ANYMORE. I WAS PULLING INTO A PARKING SPACE AT NATURAL GROCERS. WHEN THE CAR WAS GOING 2-3 MPH INTO THE SPACE GETTING READY I THOUGHT TO STOP, THE ENGINE REVVED UP LIKE A JET ENGINE. THE AIR BAG DEPLOYED, MY FIRST REACTION WAS "WHAT THE HELL JUST HAPPENED". A MAN WALKING OUT OF A STORE HEARD THE HISS OF THE GAS LINE AND THEN CALLED 911. HE CAME TO THE CAR AND TOLD ME TO MOVE AWAY BECAUSE THE GAS LINE COULD BLOW UP. HE NOTICED THAT MY GLASSES WERE BACKWARDS ON THE BACK OF MY NECK AND ASKED IF I WAS HURT. SHORTLY A FIRETRUCK ARRIVED, TWO POLICE CARS, AND A STORE MANAGER. THE FIREMEN SHUT OFF THE GAS LINE. THE POLICEMAN INTERVIEWED ME AND OBSERVED THE SETTING. AFTER SOME TIME THE POLICEMAN SAID "THIS WAS NOT YOUR FAULT" AND "I AM NOT ISSUING A CITATION". SOMETHING CLEARLY WENT WRONG WITH THE CAR. I COULD BE WALKING AWAY TOO QUICKLY SOMETIMES OR SOMETHING ELSE IN MY POCKET MAY BE SHIELDING THE KEY FOB, BUT IF OTHERS ARE ALSO EXPERIENCING THIS PROBLEM, MAYBE IT IS NOT A USER ISSUE. ELECTRICAL: THE SEAT HEATER ON THE DRIVER'S SIDE TURNS ON INTERMITTENTLY WITHOUT THE POWER BEING TURNED ON. TRANSMISSION: THE RMSS CONTINUE TO INCREASE WHEN I TAKE MY FOOT OFF OF THE GAS AT HIGH SPEEDS (INTERSTATE). I DO NOT HAVE THE CRUISE CONTROL ON. THEY REV ABOUT 10,000 AND THEN COME BACK DOWN.

PARKED MY CAR. OPEN UP DRIVERS DOOR , RADIO AND DASH LIGHTS CAME ON. STANDING ON DRIVEWAY NOW PUSH BUTTON TO TURN OFF RADIO AND DASH. EVERYTHING CAME BACK ON. DID IT THREE TIMES AND ON THE THIRD TIME VEHICLE STARTED TO ROLL BACKWARDS. HAD TO CURLY ON THE FLOOR BARD AND DOOR WIDE OPEN. VEHICLE GAINING SPEED, ENGINE OFF, LOOK AT GEAR SHIFT SAYING IT'S IN PARK STILL. I HAD TO JUMP OUT OF VEHICLE BECAUSE I THOUGHT I WAS GOING TO DIE. NEIGHBOR CAME TO HELP ME OUT AND CALLED 911 WHO SENT AN AMBULANCE AND POLICE. PARAMEDICS ATTENDED TO MY NEEDS AND RUSHED ME TO THE HOSPITAL. POLICE ASK THE NEIGHBORS IF SOMEONE WAS INSIDE THE VEHICLE AND THEY SAID NO. POLICE REPORT SHOWS VEHICLE STILL IN PARK. VEHICLE HIT A TREE AND HIGH CENTER ON A PGE ELECTRICAL BOX. NOW VEHICLE IS AT A DEALERSHIP WHO TOLD MY FATHER UNDER THE TABLE THEY WERE ABLE TO DUPLICATE CONCERNS BUT WILL NOT NOTE IT ON THE REPAIR ORDER. THEY LOOKED AT SENSORS AND KNOW CODES APPEARED. ONCE MY FATHER MENTION HE WORKED AT A DEALERSHIP THEY MENTION THAT I MUST HAVE HAD THE VEHICLE IN REVERSE. IF I STAND OUTSIDE OF THE VEHICLE AND FOR A GOOD MINUTE TO TWO MINUTES AND IT SHOWED IN PARK THE WHOLE TIME, THERE IS SOMETHING ELECTRICAL GOING WRONG WITH THE VEHICLE. IF YOU GOOGLE RADIO IS STAYING ON WHEN VEHICLE IS SHUT OFF REGARDING THESE MAKE AND MODEL THERE ARE SEVERAL CASES OF THIS HAPPENING. PEOPLE SAY THEY HAVE TO SLAM THE VEHICLE INTO PARK TO GET THE RADIO TO WORK. SOME PEOPLE HAD A BRAKE SENSOR REPLACE WHICH FINALLY FIXED THE CONCERN....UPDATED 03/29/17 *BF *CN

TL* THE CONTACT OWNS A 2016 MAZDA CX-5. THE CONTACT RECEIVED NOTIFICATION OF NHTSA CAMPAIGN NUMBERS: 16V064000 (FUEL SYSTEM) AND 16V644000 (LATCHES/LOCKS/LINKAGES). THE PARTS FOR THE RECALL REPAIRS WERE UNAVAILABLE. THE CONTACT STATED THAT THE MANUFACTURER EXCEEDED A REASONABLE AMOUNT OF TIME FOR THE RECALL REPAIRS. THE CONTACT HAD NOT EXPERIENCED A FAILURE. THE MANUFACTURER WAS MADE AWARE OF THE ISSUE. VIN TOOL CONFIRMS PARTS NOT AVAILABLE AND PARTS DISTRIBUTION DISCONNECT.

PROBLEM FIRST DETECTED BY SQUEAKING. PROBLEM GETS WORSE UNTIL DIFFICULT TO STOP WHEN BRAKING. GRUMBLING NOISE IN FRONT CAUSED BY BAD BEARINGS. PROVIDING DEALER RECORDS AND PARTS THAT HAVE BEEN ORDERED. CONCERNED THAT MAZDA HAS NOT ADDRESSED THIS ISSUE WHICH MAY NOT MANIFEST ITSELF FOR A YEAR OR MORE AFTER PURCHASE. MAZDA IS STILL SELLING THE 2016 MODELS SINCE THE 2017'S HAVE BEEN DELAYED. MAZDA HAS HAD REAR BRAKE PROBLEMS IN ITS PREVIOUS MODELS OF CX5 BUT DOES NOT ADDRESS ISSUES ON THE 2016. CONCERNED THAT SOMEONE MAY BE SERIOUSLY HURT OR KILLED BY THIS BRAKING ISSUE. CAR HAS BEEN IN SERVICE SINCE NOVEMBER 17 2016 BECAUSE SUITABLE PARTS ARE STILL ON ORDER. WHILE ATTEMPTING TO PULL INTO A PARKING SPACE WITH A 2016 MAZDA CX-5 (7000 MILES) ALL OF THE WARNING LIGHTS ON THE DASHBOARD LIT UP AND THE CAR STARTED ACCELERATING ON ITS OWN (UNINTENDED ACCELERATION). THE BRAKES WOULD NOT WORK! IN AN ATTEMPT TO AVOID THE CAR PARKED IN FRONT OF ME I SWERVED TO THE RIGHT, UNFORTUNATELY I HIT THE PARKED CAR WITH ENOUGH FORCE TO MOVED IT ABOUT 12 FEET FORWARD. MY VEHICLE CONTINUED GOING UP AN OVER A CURBED MEDIAN AND FINALLY CAME TO REST APPROXIMATELY 100 FEET AWAY WITH ENOUGH FORCE (BRAKES FINALLY WORKED) TO KNOCK MY GLASSES OFF AS MY HEAD JERKED FORWARD. VEHICLE WAS TOWED TO THE DEALER AND WAS EXAMINED AND DEEMED "WORKING AS DESIGNED". SUPPOSEDLY, INFORMATION WAS SENT TO MAZDA USA ENGINEERS, THE DEALER STATES THEY FOUND NOTHING UNUSUAL BUT HAVE NEVER GIVEN ME A WRITTEN REPORT STATING SO EVEN THOUGH I HAVE REQUESTED IT ON MULTIPLE OCCASIONS. THE DEALER RECOMMENDATION WAS TO FIX THE DAMAGE TO MY CAR ($4400.00 WORTH) AND THEN THEY WOULD INSTALL A "SPECIAL BLACK BOX" AND DRIVE THE VEHICLE FOR A FEW DAYS. AFTER DRIVING THE VEHICLE FOR 3 DAYS, THE DEALER STILL SAYS THAT NOTHING IS WRONG WITH THE VEHICLE. WE FEEL THAT MAZDA CORP SHOULD TAKE THIS VEHICLE BACK AND INSPECT IT THOROUGHLY FOR UNINTENDED ACCELERATION, AS WE DO NOT FEEL COMFORTABLE DRIVING THIS VEHICLE ANYMORE.

STOPPED ON HIGHWAY AT RED LIGHT. WENT TO PULL OUT ON GREEN, CAR HESITATED THEN IT FELT LIKE I WAS BEING HIT FROM BEHIND THE TRANSMISSION ENGAGED WITH A BIG THUD BEFORE I COULD GET MOVING. FURTHER ON DOWN THE ROAD IT WOULD WITHOUT WARNING, DOWNSHIFT THEN UPSHIFT, GOING SOMETIMES FROM 6TH GEAR TO 3RD AND THROWING ME FORWARD. EVERYTIME I WOULD STOP AND TRY TO START THERE WAS HESITATION FROM THE TRANSMISSION THEN IT WOULD SLAM INTO GEAR. WHEN I WAS STOPPED ON A SLIGHT UPGRADE, THE CAR WOULD DRIFT BACKWARD A GOOD BIT, THE ONLY WAY TO STOP IT WAS TO GIVE IT MORE GAS AND THE TRANSMISSION WOULD AGAIN SLAM INTO GEAR. DURING ALL THIS NO ENGINE OR WARNING LIGHTS OF ANY KIND CAME ON. THE MODEL I WAS DRIVING WAS LESS THAN 30 DAYS OLD WITH ONLY 3300 MILES ON IT. I AM CONCERNED, BECAUSE I TRADED IN A 2014 MAZDA CX 5 ( SAME MODEL) AND IT ALSO HAD TO HAVE THE TRANSMISSION REPLACED. MAZDA IS REPLACING THE TRANSMISSION IN THIS 2016 UNIT.

THE CAR WAS PARKED OVER THE WEEKEND IN A VERY SECLUDED COUNTY LOCATION (NO OTHER PEOPLE AROUND). I NOTICED A LARGE CRACK IN THE FRONT WINDSHIELD TWO DAYS LATER WHEN I NEXT USED THE CAR. THERE ARE NO VISIBLE CHIPS ON THE GLASS. THE CRACK SPREAD OUTWARD FROM WHERE THE REAR VIEW MIRROR IS ATTACHED. I BELIEVE THIS IS A FACTORY INSTALLATION OR DEFECTIVE PART ISSUE.
