Both LED headlights died within two months of each other on this relatively new vehicle. Outside the three year warranty, but only 27,000 miles (well within the 36,000 warranty period). As it's outside of the three years, the warranty already expired regardless of mileage. One headlight was grinding (fan) while the other's low beam simply died. Each headlight cost $2800 to replace. You can't simply replace a burned out bulb with this headlight design. The entire unit must be replaced at a cost of almost $3000 depending on your dealer. This is a major safety issue when headlights die while driving at night. This is not a user maintenance issue. It is not normal wear and tear. Almost $6000 dollars to replace poorly designed headlights within two months of each other on a new-ish vehicle while being a safety concern constitutes reason enough to issue a recall, and to reimburse owners for previous replacements.

RH LED headlight cooling fan failure. Diagnosed by Mazda dealer and quoted 2450 cost. Vehicle is just outside warranty coverage by 2900 miles. This is a very well known and documented problem that Mazda is not standing behind. Owners are removing the relay for their daytime running lights, rendering them inoperable in order to save wear and tear on the headlight cooling fans. This is a safety hazard.

I have a failed leveling motor on my led headlight assembly causing a constant noise as it is attempting to level. This is apparently a known problem and has had a TSA technical service bulletin of 09-015/23. My car is past the 3 yr warranty but under the 36,000 mile warranty period with only 33,000 miles. Mazda refuses to cover it since it has not had a recall yet even though it is a common and known problem. They are quoting a 1200 repair cost. Can you please help in any way and address this? Thank you

Both front headlights are making an abnormal loud whining noise which started intermittently about 6 months ago and became continuous January 2026. Took vehicle to dealership where the dealership reported that the noise may be due to contamination by a foreign matter during the manufacturing process getting inside the adaptive headlight system causing the fan motor bearings inside the headlight to wear out. MAZDA dealership in Bridgeport WV stated the fix is to replace both front headlights at a significant cost to the owner if the vehicle is outside the warranty. The only indication that there is a problem is the loud noise coming from the headlights, there has been no warning messages. MAZDA is not willing to fix the issue unless the vehicle is within warranty. My vehicle is not within the known Vins listed below having this issue and is outside of the warranty. I reached out to Mazda Corp with case # [XXX] and MAZDA Corp denied financial assistance, but stated I could work with the dealership to trade my vehicle instead of fixing the issue. MAZDA knows the issue is on these vehicles per the TSB report given to me when I took my vehicle to the dealership. Several times and most recently today 8/18/2025 at 5:30 PM my 2022 Mazda CX-5 accelerated while I had my foot on the brake and only the brake. Hard pressure on the brake kept the SUV from lurching forward. This has happened before but I did not think it would occur again. But it has and I find that this could easily cause an accident. The SUV in question has only 23,000 miles on it. An investigation and recall to fix the problem should be initiated.

It looks like a safety concern in Mazda car cx5. My car sun roof explode without any reason while in a static position in the traffic signal. The broken glass can injured the driver and copassanger .

My 2022 CX5 sunroof exploded while driving on highway, glass fragments were caught by the shade so no one was injured thankfully. But this could have easily caused bodily injuries if the shade was open. The loud explosion and glass fragments could have distracted/impair the driver, leading to a serious incident. I do not suspect it is caused by impacts from debris since there was no large vehicles around me at the time of explosion and no other impact noise was heard at the same time. There are other similar incidents regarding sunroof explosion that were posted online.

I am submitting this report to document a persistent ticking noise in my 2022 Mazda CX-5, which appears to be the same issue outlined in Mazda Service Bulletin 01-002/21 for previous model years. This issue is commonly associated with Hydraulic Lash Adjusters (HLAs), which maintain proper valve clearance. The noise occurs most prominently during cold starts and resembles the previously reported HLA ticking sound. Owners of earlier CX-5 models have attributed this issue to air bubbles or insufficient oil supply within the HLAs, with cold weather exacerbating the problem due to restricted oil circulation. Despite Mazda's claim that revisions to the engine components have resolved this issue in newer models, my experience suggests it remains unresolved in the 2022 model year. I am concerned about the long-term reliability of these engine components and the potential repair costs if the issue leads to premature wear or failure. While Mazda has not issued a Technical Service Bulletin (TSB) for the 2022 CX-5, multiple owners have reported similar ticking noises online, indicating that the problem may persist. I have a video recording of the noise but am unable to attach it to this submission. If further investigation is warranted, I would be happy to provide the footage upon request. The engine on my 2022 naturally aspirated Mazda CX5, is making a loud ticking noise when cold started. The vehicle is in pristine condition and is services regularly at the dealer with only 33,000 miles. The ticking noise is a known issue for this make and model, where Mazda has issued a TSB for faulty hydraulic valve lash. This is a distinct noise that is sharp and metallic and distracting when driving the vehicle, even after a period of time. I can hear it through my radio and it does not sound good. Evidently 2022 is not part of the TSB, yet my vehicle is still affected by this noise. Filing a complaint with NHTSA to formalize this record.

Just received a Warranty Extension Program - Special Service Program (SSP) D8 notice from Mazda. It states that on my vehicle the fail-safe thermostat could fail in the stuck open position and they are extending the warranty for that part to 15 years. Meaning, if it fails, they'll fix it. My issue is that if that part COULD fail, they should go ahead and do a voluntary recall, and replace the part, and be done with it. I'm thinking maybe you could require Mazda to do that.

Cylinder head cracked resulting in engine overheating with code P111A. Gasket bolts expelled due to excessive heat and pressure. Evidence of coolant spray along hose connections, lower cover and transfer case. Smoke came out of the engine compartment first followed by warnings titled 'Critical vehicle malfunction alert' 'pull over immediately' Check engine light came up shortly thereafter. Vehicle inspected by an authorized Mazda service center, confirming the engine overheat code. This is the statement from the service representative: 'Mazda wants to start with a cylinder head prior to doing an engine'. The technician's analysis indicates they received warranty approval for Entire cylinder head and head gasket replacement and all associated parts. No warning lights prior to the failure. They started just as the engine started smoking. Potentially related information: The vehicle displayed error code for a coolant valve issue 3 weeks before this issue for which a TSB already existed and Mazda replaced the part. TSB related to cylinder head defect exists for model year 2019, but not for model year 2022.

The timing chain cover gasket failed and is leaking oil at 66,000 miles. It is currently being addressed at Borgman Mazda in Grand Rapids. No crash or safety risk. There were no warning lamps, it was found by an separate inspection not from the dealership.

Cracked cylinder head, August 2023. I'm not sure if it's a safety hazard, but it seems that oil being sprayed around a hot engine could be a fire hazard. Also, since the oil light comes on with no warning, it could cause people to have to pull over on a busy interstate because you shouldn't drive with the oil light on. I had almost 14,000 miles on my 2022 Mazda CX-5 when the low engine oil light came on. I had recently had an oil change done, so the oil should not have been low. I put oil in it and drove it to the dealership. They discovered oil sprayed everywhere in the engine compartment, and discovered the cracked cylinder head. It is a known problem. The car was still under warranty so they replaced the cylinder head. There have been no changes or improvements to the cylinder head, so theoretically the replacement cylinder head could also eventually crack.

The vehicle was taken to the dealer, where the control valve body assembly and control valve were replaced; however, the failure recurred. The vehicle was taken back to the dealer, and the dealer replaced the transmission. The failure recurred, and the vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was 3,963.

Was driving down a road at around 40 MPH and a vehicle ran the light and took a turn in front of me. I applied brakes but car didn't try to apply them harder from the start and no warnings ever came up. Wasn't able to avoid collision and airbags deployed (only steering wheel airbag)

I was driving in slow moving traffic with vehicles ahead and behind. The vehicle ahead of me slowed, so I applied brakes and began to slow as well. While I was on the brakes slowing at a normal rate with plenty of distance to the vehicle ahead, my vehicle unexpectedly activated automatic emergency braking. It abruptly applied full brakes and came to a complete stop, even though traffic ahead was still rolling. There was zero chance of collision, with ample following distance, minimal speed differential, and brakes already applied. The audible alert triggered at the same time as the emergency braking, providing no advance warning. The vehicle following me was unable to stop in time and caused a rear-end collision with my vehicle. My vehicle had moderate damage, and the following vehicle had more significant damage with airbag deployment. Collision speed was approximately 5-10mph. There were no injuries.

Brake rotors on all four wheels measured below minimum thickness specification at only 17,000 miles, requiring complete replacement of all rotors and pads. Rotors replaced at Koch Mazda (Easton, PA) on January 7, 2026 and no longer available for inspection. SAFETY RISK: Rotors below minimum spec have reduced structural integrity and heat dissipation capacity, creating risk of warping, cracking, or failure during emergency braking or sustained braking. Under-spec rotors can cause brake fade, increased stopping distances, and potential catastrophic failure. At 17,000 miles, this represents premature failure of a critical safety system. DEALER CONFIRMATION: Koch Mazda initially charged $89.99 for diagnostics claiming "nothing wrong" despite customer providing TSB 04-003/24 documentation. After formal complaint, dealership measured rotors below minimum spec and replaced all four rotors and pads as "non-warranty goodwill repair" (value $1,184.97). Calling it "goodwill" rather than warranty avoids documenting as defect claim. Dealership stated Mazda corporate approved the goodwill repair, indicating manufacturer awareness of widespread defect being managed case-by-case rather than through recall. WARNING SYMPTOMS: Popping/clicking noise when first moving forward or reverse after sitting idle. This symptom is specifically described in Mazda TSB 04-003/24 as caused by deteriorated grease between brake pads and shims. SYSTEMIC PATTERN: Research reveals hundreds of similar complaints across 2013-2025 CX-5 model years involving premature rotor/pad failure at 11,000-35,000 miles. TSB 04-003/24 addresses this exact pattern across multiple model years, confirming manufacturer awareness. Normal rotor life is 50,000-70,000 miles; failure at 17,000 miles suggests manufacturing defect, improper heat treatment, or design flaw affecting entire CX-5 brake system across 12+ model years.

The contact stated that while driving approximately 25 mph when another vehicle abruptly stopped causing the contact to need to suddenly stop however, the brake pedal failed. The contact stated that the brake pedal traveled to the floorboard then the emergency braking system began chiming. The vehicle failed to stop causing the contact the crash into the vehicle in front. The air bags did not deploy. There was no police report. There were no injuries reported. The contact had contacted the dealer however the vehicle had not been diagnosed or repaired. The four 225/55 R19 tires’, purchased 05/02/25 and available for inspection upon request, asymmetrical tread design has 5 treads & 4 troughs across its width. The center tread and the adjacent one to its outside both have varied design water dispersal slots. When the tires are properly mounted with the word “outside” viewable on the outside of all 4 wheels and the vehicle is rolling forward, the 2 above referenced treads’ slots of the right side tires will force water away from the treads and into the adjacent troughs, but these same tread slots on the left side tires, now spun 180 degrees when mounted on the left side, will force water toward the center of the two above referenced treads, actually building up water upon the referenced treads’ surfaces, not dispersing it like on the right side tires. On wet roads, this will dangerously set up the likelihood of hydroplaning on the referenced left side treads, as well as a surface grabbing imbalance between the right and left side tires which sets things up for a spin out. When viewing the mounted tires, the above safety concerns are easy to envision. They appear to pose a risk for an accident, but Pirelli states that their tires are “engineered with advanced tread patterns that balance performance, safety, and comfort” and the tires are properly mounted as described. I have no data indicating that actual issues have been confirmed with my concerns, but I feel they are valid and that this asymmetrical tire should have also been designed and marketed as a directional tire, with left and right mirrored tread designs, which it has not been. I just noticed this tread discrepancy concern.

Odometer Fraud. The contact purchased a 2022 Mazda CX-5. The contact discovered a mileage discrepancy after purchasing the vehicle. The vehicle was a dealer sale. At the time of purchase, the vehicle mileage was 26,400. The contact stated that the dealer harassed her to buy the extended warranty. The contact stopped by the dealer and requested that they stop with the harassment. The contact stated that at the time of purchase, the dealer connected the service to the infotainment system. The contact noticed that the vehicle was being tracked and the gauges on the instrument panel were tampered with. The manufacturer was notified of the failure. The contact stated that two days later, it was discovered that the mileage was approximately 46,000. The contact suspected that the vehicle was hacked.

I have had a system malfunction, that has caused a dead battery in May 2022 and then again Aug 2022. The car was in park and I received a notification on the Mazda app about 5 hours after being in the car that stated "Critical Vehicle Malfunction Alert." I had to have my car towed and serviced at a Mazda dealer but they found no issue with the battery. This happened again in August. According to the Mazda C-X5 Manual, like all modern cars, even if something was left on (light on, door open, ACC on) it should automatically turn off after 25 minutes.

While driving down highway the car suddenly will stop accelerating. Once this occurs the check engine light will turn on and shortly after the battery light will turn on. The car will not move and the AC will then stop working. The first time the car was brought to dealership (under warranty). They had to replace battery, fuel pump and fuel level indicator. They stated the fuel pump was broken as was the fuel level indicator. They were able to reproduce the problem prior to fixing it the first time. Less than a month after the car was repaired the exact same issue happened. This is a significant safety concern as I was driving on the highway with a [XXX] child in the back seat when the car stopped driving the second time. I was barely able to get over a lane and off the road before the car stopped driving. The AC then stopped working on a day where the feels like temperature was over 100. I had to lay wet cloths over my infant to prevent her from overheating while we waited for AAA and a family member to reach us. The inability to accelerate/sudden stopping of the car while driving on a highway with a speed limit of 70mph posed a significant safety risk to my life, my child’s life and potentially other cars if they weren’t able to stop. I had to wave frantically at the car in the lane next to me so that they wouldn’t hit me. No warning lights came on until after the issue (stopped accelerating) occurred. The dealership (Mazda) now has the car again and will be inspecting it. I have files from the first incident however they are in the car. The vehicle unexpectedly caught fire and the results could have been catastrophic. Almost new car under warranty. All details are in the uploaded PDF letter and photos. I have contacted Mazda and they claim they do not investigate, only insurance company will investigate. My insurance company will pay me for the total loss but does not plan to investigate. I think this is dangerous and unconscionable. Thank you for any help or guidance.

My driver side front headlight fan is malfunctioning. It stays on and makes a loud noise when the car is parked and off and the lights are still on. This is a known defect and my dealership is asking for 2k to pay for it. I am concerned the light will go out causing safety concerns. (TSB) SA01323 and 09-015/23. This should be recalled as is it not a general wear and tear issue, it is a manufacturing issue. My car is out of warranty but this should still be covered.

Transmission failure, currently reproducible but car is at dealer to be evaluated/repaired. I was safe as I was still in my driveway, however if I was in traffic I would be at risk of an accident, by the time you realize you are not moving you have already let off the brake and could be rolling, fear is that if it happens at a traffic light other drivers woudl assume I am going to accelerate but I'm not. First time this happened dealer reproduced issue, contacted Mazda, and a replacement valve body was installed. Now that it has happened again within the first 33K miles it creates a trust issue. There were no warning sign either time the transmission failed. I also do not drive aggressively, wife says I drive like a grandpa. about 10-15 seconds after failure the alarms and warning lights go off.

Mazda’s CX-5 engines, particularly in earlier models, have been known to experience issues with Hydraulic Lash Adjusters (HLAs), which are responsible for maintaining proper valve clearance. Many owners have reported a persistent tapping or ticking noise, especially during cold starts. This issue is often caused by air bubbles or insufficient oil supply to the HLAs, and cold weather exacerbates the problem as thicker oil struggles to circulate until the engine warms up. The HLAs have also faced criticism for their sensitivity to oil pressure and quality, making them prone to failure in less-than-ideal conditions. Mazda claims that replacements and redesigns have resolved these issues in newer engine models and has not issued any Technical Service Bulletins (TSBs) for models after 2021. However, I experienced the HLA “ticking” noise in my 2022 CX-5, suggesting the issue has not been fully addressed. I am concerned about the longevity of these parts and potential repair costs in the future, as Mazda seems to be dismissing the problem. I was driving to Home Depot after work around 6PM PST on [XXX] and must have fallen asleep at the wheel about a.1/4 of a mile from my house and left the road way after a turn. I hit my head so hard I had to be treated for a head injury and the car dialed 911 for me to report a crash. The airbags nor any other safety system triggered besides detecting a crash and dialing 911. My car was deemed totalled by insurance and had to go to the hospital. When driving in the wind at speeds greater than 50mph , there is an extremely loud high pitched reedy noise. I have many videos of this and Mazda won’t address it/says it’s not an issue. I travel for work in SD, a very windy state, and often listen to this sound 2+ hours straight and can’t hear any music/phone calls/audiobooks over the noise. It is dangerous because I can’t hear anything but this, and it causes a lot of stress/anxiety/overstimulation. I wish I never bought this vehicle.

The contact owns a 2022 Mazda CX-5. The contact stated that while driving from a stationary position, the vehicle failed to properly accelerate while depressing the accelerator pedal. The contact indicated that the vehicle would not immediately respond and would drive very slowly before accelerating. Additionally, while driving around corners the steering wheel was very difficult to maneuver, making the vehicle difficult to control. The vehicle was taken to the local dealer who was unable to identify a cause for the failure of the vehicle. The failure mileage was 100.

Unexpected acceleration ___ While stopped on a slight downgrade, RPM suddenly increased and vehicle lurched forward. Applying more pressure to brake stopped movement. I forced transmission into park. Took foot off brake, RPM still high. Tapped accelerator, RPM dropped to normal idle. Fortunately, there were no vehicles in front of mine and cross traffic was not present. Vehicle did not enter intersection. I drive single footed and shoe did not overlap brake and accelerator. The day was clear 37F, dry. Vehicle had made 3 short trips prior so engine was up to temperature, 35 miles total over 2 hours. Cruise control was not engaged and had not been used in two days. This issue has not been reported to dealer. There were no warning lights or other indications of problems. The vehicle has driven normally since. I have not been able to reproduce the issue. ____ A similar issue happened two months prior. Circumstances were not the same. Intersection was flat, engine was not up to temperature, removing pressure from brake pedal was enough to stop high RPM. Attributed instance to the shoes I was wearing. The running shoes were thicker and wider than I normally wear while driving. I believed I may have pressed both brake and accelerator without realizing it since reducing pressure corrected the problem.

When coming to a stop, car accelerated and went over curb. From a search of the internet I found examples with this and other cars where people with larger shoes had the same problem of pressing the brake and unintentionally pressing the accelerator pedal at the same time. I read of one manufacturer that programed the car to brake if both pedals are pressed at the same time and ignore the accelerator. The same thing happened in my 2015 Mazda CX5. In that case I had it checked at the dealer. The technician found nothing wrong but was able to reproduce the problem when his boot pressed both the brake pedal and accelerator because both pedals were so close together. Since this would probably be an inexpensive software change, it would be prudent for a safety recall by manufacturers that have pedals that are close to one another.

On July 22, my sister and I were traveling on I-95 going south. We stopped at the Maryland House near Aberdeen, MD. I was looking for a parking place and was driving very slowly. I slowed further to turn into a place on the right, near the south end of the parking lot and was attempting to put the car in park when the car accelerated abruptly, lurched forward, crossed over the concrete curb at the end of the parking space, and was heading right for I-95 which was straight ahead. I had never taken my foot off the brake. I tried to brake harder but that did not stop the car. I therefore turned right onto a service road to avoid crashing onto I-95. I then noticed a car coming toward me and swerved to the right back toward the parking lot, crossing over the curb at that parking spot, and slamming into a parked car and side swiping a truck. I only took my foot off the brake when the car stopped. The airbags did not deploy. After the car stopped, I was unable to turn the engine off. When I returned home, I looked on line to see if anyone else had reported a similar experience. Apparently there have been a number of similar occurrences with Mazda CX-5s (see https://www.carproblemzoo.com/mazda/cx-5/car-accelerates-on-its-own-problems.php) including a description of almost the same event with a car that also had low mileage. In several of the cases listed, the airbags likewise did not deploy. Note also that the police report may not include the origin of the incident but only the crash into the other car. I was in shock and was not able to give full information. I have asked for the complete information to be added to the report. Also of note, and possibly related: Earlier in July, an alert ‘brake, brake, brake’ sounded and there was a flashing on the dashboard that said ’’brake’. I took the car to Mazda, who could not find a problem and told me to return if it happened again.

The sunroof exploded while driving 70mph on a rural highway. There were no other cars nearby that could have kicked up a rock nor anything that could have fallen on the sunroof. The window buckled outward, not inward. The glass shattered and flew everywhere endangering the driver, passengers, and the car behind me when the window exploded. There was no warning, just a loud boom when the window gave way. My best guess is that this resulted from temperature fluctuation. I was at about 10,000 ft altitude. It froze overnight. The temperature when the window exploded was about 40 degrees, but the sun was out and strong. There was only about a 2,000 ft change in altitude while driving. I can’t imagine that the car seals well enough for the pressure change due to altitude to cause the window to burst, but that would be my second best guess, though a couple thousand feet shouldn’t have cause that big of a pressure change.
