Here the facts, bought a car out of state (Illinois) from Iowa with 130,131 miles on Sept 12th 2025, Mazda CX9, the dealer did not disclose any problems with the car, after 3 months of ownership, the car overheats on highway whit family including a [XXX] infant, white smoke under the hood and exhaust pipe, towed home, radiator was replaced, few days after that a coolant leak, vehicle was sent to an authorized Mazda dealership for diagnose, the technician told us, according to Mazda procedures, a cod P111A, the vehicle's engine reached unsafe level of temperature and the whole engine needs to be replaced because a cracked head cylinder. This problem is well known by Mazda because they addressed on a TSB01-002/23 and CSP11 extended warranty, there is a flaw in the design of the engine near the cylinders manifold area, that cracks the engine and requires a full replacement of the engine. its just a flaw in the design of the engine. . In January 2024, I purchased a used 2018 Mazda CX-9. The vehicle ran well until [XXX], when I discovered an engine coolant leak. My mechanic diagnosed it as a known defect involving the cylinder head. My husband contacted Mazda Customer Service (CS), and they advised us to get an official diagnosis from a Mazda dealer. We brought the car to Orland Park Mazda, where a diagnostic confirmed the defect (report attached). We later received an email from Mazda CS saying that since our car was at the dealership, Mazda would take the lead in submitting the goodwill repair claim (email attached). When we followed up with Orland Park Mazda, they informed us that no one from Mazda CS had contacted them to authorize the repair. We relayed this to Mazda CS and gave them the direct number of our service advisor, Adrian. Mazda CS said they would follow up and get back to us within 48–72 hours. On a follow-up call, I spoke with Nancy at Mazda CS, who again told me that our case manager Cindy would contact us within 48–72 hours. Instead, we received an email denying the repair, stating the vehicle exceeded the 120,000-mile limit for the extended warranty (our vehicle has 133,500 miles). While our car is outside the warranty period, this is a textbook case of a well-documented defect covered under TSBs for the 2018 CX-9. Mazda has previously approved goodwill engine replacements for other customers well beyond this mileage. We are seeking fairness and consistency in handling known product defects. Our request is that Mazda approve the engine repair under goodwill coverage as they have in many similar cases. Our Mazda CX-9, having roughly 86,000 miles, had catastrophic engine failure while driving. This is caused by coolant leaking at the cylinder head and around the exhaust manifold. This in turn caused a crack in the engine block. Mazda refused to fix this as it wasn’t a “recall” and only a tsb sent out. 1000 of people have reported this issue, causing failure while driving but Mazda will not recall the engines nor fix them under an extended warranty. After a 4 month battle with Mazda they refused to fix this in our car even though they know the engine is a bad manufacturing issue. A recall needs to be made and Mazda needs held accountable. Our exact cause was exactly what the TSB stated but Mazda stated our crack was not in the correct spot. They need a recall covering all engine block cracks due to faulty manufacturing

The infotainment screen has spider like frost inside the screen. I've checked and other models have had this same issue and have been recalled and repaired. My model year 2018 is suffering the same issue and MAZDA has lagged on this problem. Please put out a recall on this as it is a hazard when you rely on your navigation system and other components the screen assists with.

GPS touchscreen developed cracks described as inside-frosting or spide-web. Mazda had a recall for 2015-17 years models for the same problem, but they are reluctant to recall 2018 models. In 2019, Mazda discontinued selling touch-screen GPS, but it refused to extend recall for 2018 models. I took my car to the dealership and the cost of repair is $2400. The dealer did not find any damage caused by external force. Hence, this is the same recall issue with poor hardware and craftsmanship. I would like to ask NHTSA to compel Mazda to do a recall for CX9 2018 GPS touchscreen.

Infotainment screen has spider cracking from faulty screen when manufactured. Open TSB stating this and Mazda refuses to replace under warranty after 5 months of owning vehicle, spider cracks got worse this year. When i got more spider cracks i contacted mazda support since the first time I contacted a local dealer. Mazda support denied replacement because “ i do not get my car serviced through mazda”, this screen issue has nothing to do with a car being serviced. This is a safety issue as you can not see parts of the screen and will only get worse and make the whole screen inoperable when using navigation.

Engine is leaking coolant apparently due to a cracked engine block caused by a manufacturing defect in the casting. Mazda has a Technical Service Bulletin, subject:Leaks at cylinder head and the number is 01-013/21. But they refuse to help cover their mistakes and now we owners are stuck with replacing an entire engine out of pocket. Shame on Mazda

My car started leaking coolant from cylinder, it is known issue for these models, unfortunately Mazda cooperation doesn’t cover the expenses, there is Service Bulletin about it, so basically so many car got affected or will affect from this problem. It is manufacturer fault, not about customer usage or mistake.

At 102,000 miles I have a cracked cylinder head

Went in to the mechanic to get a regular oil change and noticed a liquid slushing sound after starting the car up. Was informed that there is a major coolant leak somewhere in the car. Took it to Mazda dealership right away who confirmed that it is a cracked cylinder head. I got lucky that the car did not overheat on the road and happened to catch it in the nick of time. No check engine lights or low coolant lights came on at all.

The liftgate (trunk) opens sporadically. The system keeps trying to open the truck and it's causing a parasitic drain on my battery. I was stranded 2 hours from my home due to this. My trunk would sporadically beep three times, that is the only warning I received. There are safety recalls for other year of Mazda cx-9 but not my specific even though I am having the same issue.

Passenger seat heater either had a grounding issue or shorted out, causing to seat to start smoldering and catching fire, destroying the seat. There has been recalls and lawsuits for this specific issue on this model in other years, but not for this model year.

There is a recall for Mazda 2016-2017 infotainment screens where they will show signs of cracking under the plastic and the screen will reboot. I have a 2018 CX-9 and Mazda corporate acknowledged the issue but only offered 15% of the price of a replacement part instead of replacing it. They said my VIN was not part of the original recall. The infotainment system screen has developed several cracks in the upper left-hand corner of the screen. Often time these cracks look like spiderwebs or frost --it's commonly referred to as "spider cracking." This is the second time my vehicle has had this issue. At the present time, the cracks are contained to the upper left-hand corner of the screen, but they are getting larger. The vehicle has been inspected by my local MAZDA dealership and I'm awaiting a resolution. As I understand, this is a known issue with 2016-2017 model years and a TSB has been issued for these years. MAZDA should extend the TSB to 2018 and beyond.

The main infotainment display randomly exhibited "spider cracking" which obstructs the view of the material being displayed within the screen. It is unsafe because the operator is unable to see maps, time, locations, controls, reverse cameras, etc. All of these are displayed in the screen and, due to the manufacturing defect, the operator is distracted by straining to see what is displayed behind the cracking. This is a well documented issue plaguing Mazda vehicles. Mazda has admitted it as defective, and are fixing the problem in the 2016-17 models. However, the 2018 model (which has the same screen as the 2016-17) is being refused. The fix needs to be extended to the 2018 model and beyond since it iw clear this it is still a widespread issue. It is well documented that once the cracking starts, it quickly spreads to the entire screen if not repaired, causing even more danger to the operator who will be distracted while attempting to see the display behind the screen.

SERVICE BRAKES

Within 6-8 months of purchasing the car we noticed the brakes squeaking and vibrating. At that time the car was under 25,000 miles. At our maintenance check it was advised that we have the brakes resurfaced at the next appt. Back brakes were done in 3/2019 and the front were done in 6/2019. We noticed the squealing and vibration returned again. At our recent maintenance check, we asked to have the brakes and rotors inspected. We were told everything looked fine and nothing needed to be done, however we are still experiencing the vibration which is very troubling. We called another place explaining the issue and they said that it sounds like we need new rotors. Our car has mostly been sitting over the last year and a half with very few added miles. We're curious to know why a car with less than 25,000 miles on it needed the rotors resurfaced and why with less than 60,000 miles it needs new rotors. The brakes on the Mazda SUV's seem to wear rather quickly or perhaps the incorrect brakes are put on for this size of vehicle. The increased vibration seems like a safety hazard

EPB WILL NOT RELEASE, OR WILL UNEXPECTEDLY AND SUDDENLY RELEASE WITH CAR ACCELERATING IN GEAR ('DRIVE' OR 'REVERSE'). I WANTED TO SAY IT HAPPENED TO US WHILE WE WERE DRIVING BACK FROM A DAYTRIP ON SATURDAY. WE HAD OUR KIDS IN THE CAR AND MY HUSBAND WAS DRIVING. ALL OF A SUDDEN THE SCBS LIGHT CAME ON AND THE CAR JUST STOPPED GIVING GAS. THE CAR BEHIND US COULDN'T TELL WE WERE SLOWING DOWN BECAUSE THERE WAS NO BRAKE LIGHT. WE WERE ALMOST REAR ENDED. THE CAR BEHIND US COULDN'T STOP IN ENOUGH TIME SO THEY JUST SWERVED ONTO THE OTHER SIDE OF US. WE HAD TO PULL THE CAR OVER AND IT WAS ALMOST NIGHTTIME. MY HUSBAND ENDED UP PUSHING THE CAR TO GET IT OFF THE ROAD AND TO A SAFE PLACE. IT WOULD ONLY GO 2 MILES PER HOUR. I HAD MY CAR TOWED TO THE DEALERSHIP, I HAVE NOT HEARD ANYTHING FROM THEM EXCEPT THEY ARE CONTINUING TO LOOK INTO IT. IT'S A 2018 MAZDA CX9 WITH ONLY 26000 MILES. IT'S NEVER BEEN IN AN ACCIDENT. I'M HOPING THAT WHATEVER THE FIX IS, IT'S COVERED UNDER WARRANTY. IF IT WAS A TRUCK OR BUS BEHIND US, THEY WOULD NOT HAVE BEEN ABLE TO STOP EITHER. I WAS PULLING INTO A PARKING SPOT IN THE UPPER PARKING LOT AT WORK. I STEPPED ON THE BRAKE PEDAL SO THAT I COULD STOP AND PARK BUT THE PEDAL WAS HARD AND HAD NO GIVE. THE CAR CONTINUED TO ROLL FORWARD OVER A CURB AND PARTLY DOWN A SMALL SET OF STAIRS THAT GO FROM THE UPPER PARKING LOT TO THE LOWER PARKING LOT. THE CAR STOPPED WHEN IT HIT A METAL RAILING AND I SHIFTED THE CAR INTO PARK.

My 2018 Mazda CX-9 with the SKYACTIV-G 2.5T engine has been diagnosed with a cracked cylinder head, cracked exhaust manifold, and cracked turbocharger at approximately 88,000 miles. This is a known design defect documented in Mazda TSB 01-002/23 and addressed by Customer Service Program CSP11, which extended the powertrain warranty to 10 years/120,000 miles for coolant leaks at the cylinder head. The TSB identifies exhaust manifold deformation and residual production stress as root causes of the cylinder head cracking. In my vehicle, the failure has extended to the exhaust manifold and turbocharger, both directly mounted to the cylinder head in the same high-stress zone. The turbocharger on this engine is a coolant-cooled unit. As coolant leaks from the cracked cylinder head, the turbocharger is deprived of its cooling supply, leading to excessive heat buildup and eventual cracking of the turbo housing. The turbo damage is a direct consequence of the cylinder head defect Mazda has already acknowledged. CSP11 covers the cylinder head but does not address consequential damage to the exhaust manifold and turbocharger caused by the same defect. I am requesting the NHTSA investigate whether Mazda's remedy is sufficient and whether a formal recall is warranted given the risk of sudden engine failure at highway speeds.

My vehicle had a coolant leak st the cylinder head around the exhaust manifold. The problem was confirmed by an independent service. No warning lamps, message or other symptoms of the problem prior to the failure did not appear. The problem was detected because coolant was dropping from the powertrain to the floor. When tha catalyst was removed it was found broken due to the high temperature but the car never display a high temperature warning lamp and the temperature display remains in normal position. The car could get on fire since no warnings or signal of high temperature was registered. After the situation occurred, I found a Technical Service Bulletin (No 01-01321) from Mazda North American Operations regarding this issue but since the problem occurred after the powertrain warranty the dealer did not accept to repair it. The probability that my car could get in fire was high since no warning lamps were display.

The vehicle has a cracked cylinder head as a result of stress put on the block from a bad exhaust manifold design. This is a known issue to Mazda and service bulletin was sent out to all their dealerships in 2021. Mazda only offered to pay a very small portion of the repair and puts the rest of the cost on the customer. Luckily I caught the issue early but this could have been dangerous if the car overheated and the engine failed on the highway. There are countless other stories of consumers having the same issue with the CX-9 and CX-5 of the same year. Many resulting in engine failure.

- My 2018 Mazda CX 9 started stalling and jittering at idle and then eventually during slow driving. - Next day I got a check engine light, took to Mazda service center at the dealership I have been going to for last 6 years since I bought the car. - Service center diagnosed that there was a coolant leak through the cylinder head which seems to have cracked. - Further investigation indicated that this has been identified on many other similar Mazda engines and a Technical Service Bulletin had been issued by Mazda to Dealers. - On probing initially, dealership declined they were aware of any such problem. - The amount of repair for the engine replacement (cylinder head and other parts) is exuberant and is almost half of the value of my car, and is not expected for a low 44,000 miles car. - This had posed safety issue for my family since the stressed engine with leak in cylinder head could lead to fire and fatality. - I am awaiting response from Dealership and Mazda corporation case#[XXX] for a response especially when this issue has been filed under a class action lawsuit: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Mazda has a known technical service bulletin [XXX] which has numerous engine cylinder heads which leaks coolant causing the engine to overheat without warning. On new cars. I WAS WAITING IN A STATIONARY POSITION, AT A RED LIGHT, BEHIND SOME OTHER VEHICLES, WITH MY FOOT ON THE BRAKE PEDAL. ALL OF A SUDDEN, THE VEHICLE STARTED TO LURCH FORWARD AS IF SOMEONE HAD STEPPED HARD ON THE GAS PEDAL! I WAS STRUGGLING TO KEEP THE CAR STILL BY DEPRESSING THE BRAKE PEDAL HARDER, BUT IT WOULDN'T STOP THE CAR. I HAD TO PUT IT IN PARK AND TURN OFF THE ENGINE TO STOP THE FORWARD THRUST OF THE VEHICLE.

Infotainment screen (LCD) of Mazda CX-9 2018 has developed what looks like discoloration/scratching/frost in corner of screen about 2 years ago and it is continuing to spread over last 6 months. This screen is used for hands-free bluetooth/calling (which is now the law in Iowa) and the back-up camera which allows for safe backing. If screen is unreadable, these safety functions cannot be performed properly. I take my car into Mazda dealership for maintenance/oil changes and it has never been addressed as an issue, but I'm concerned.

CAR SHOWED IGNITION ON WHEN THE CAR WAS TURNED OFF. SAME DAY WHILE DRIVING ON HIGHWAY CAR ENGINE REVVED FOR NO REASON. ALSO WHEN SOMEONE GOT OUT OF PARKED CAR IT SAID NO KEY DETECTED BUT CAR WAS STILL TURNED ON AND KEY WAS RIGHT BESIDE ME.

The contact owns a 2018 Mazda CX-9. The contact stated that while driving at various speeds, the engine started overheating within approximately 15 minutes of driving. The contact had to pull over and wait for a while to let the engine cool down before continuing to drive. The contact stated that there was a message that the engine was overheating and to stop driving the vehicle displayed. The vehicle was taken to the dealer where it was diagnosed that coolant was leaking into the cylinders due to a fracture in the cylinder head. The contact was unsure of the cylinders with the failure. The contact was informed that the engine needed to be replaced. The vehicle had not been repaired. The contact found a Technical Service Bulletin regarding the cylinder head failure. The manufacturer was made aware of the failure and confirmed that the VIN was not covered under the TSB and that the TSB had expired in 2023. The failure mileage was 76,000.

The contact owns a 2018 Mazda CX-9. The contact stated that while the vehicle was parked, he noticed an abnormal odor inside the vehicle. Upon inspecting the vehicle, the contact found a trail of coolant. The vehicle was taken to an independent mechanic, who confirmed that coolant was leaking, and referred the contact to the dealer for a diagnostic test. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 88,080.

The contact owns a 2018 Mazda CX-9, The contact received notification of NHTSA Campaign Number: 21V875000 (Gasoline, Fuel System) however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure. VIN tool confirms parts not available.

The contact owns a 2018 Mazda CX-9. The contact stated that while pulling up to a gas pump the vehicle lost motive power and stalled. The contact added gasoline to the vehicle, restarted the vehicle, and was able to continue to drive to her residence. Several weeks later the failure recurred while the contact was pulling into a parking lot. The check engine warning light illuminated. The contact’s husband arrived at the scene and added gasoline to the vehicle. The vehicle was restarted however, the check engine warning light remained illuminated. The vehicle was taken to the local dealer to be diagnosed. The contact was informed that the fuel pump and sensor were faulty and needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 65,000.

My 2018 Mazda CX-9 experienced catastrophic engine damage after a radiator failure allowed coolant to enter the engine. The vehicle provided no dashboard warning, overheating alert, or coolant warning before the failure occurred. Modern vehicles are designed to alert drivers to overheating or cooling system failures. The lack of warning prevented me from addressing the issue and resulted in catastrophic engine damage. I believe this represents a potential safety defect in the vehicle’s cooling system monitoring and warning system.

The adjustable rails holding the rear left seat in place no longer latch. It is available for inspection. This is the most common place for a car seat in this car. In the event of a crash the seat would slide forward. The seat belt attaches to the seat so there would be little restraint on the rear passenger. The mechanic who attempted a repair stated Mazda does not sell this part, only sells a replacement for the entire seat, about $5000. There were no symptoms, when adjusting the seat the rail just stopped holding the seat firmly in place when the adjusting bar was released.

After a recent inspection conducted by Mazda Koch 33, it was brought to my attention that the engine wiring harness is defective and may potentially cause a fire or result in burns, posing a severe safety risk to me, my passengers, and others on the road. This issue is deeply concerning and unacceptable, especially considering Mazda’s reputation for reliability and safety. A fault in the engine wiring harness, particularly one that has the potential to ignite or burn, should be treated with the utmost urgency. It is troubling that such a critical component could present such danger. My vehicle has been serviced and maintained regularly, and I was not made aware of any recall or technical service bulletin that addressed this defect until the inspection. Given the nature of the defect, I believe this may be a manufacturer defect, and I request that Mazda: Immediately investigate and confirm whether this issue falls under an existing recall or known defect. Take full responsibility for replacing or repairing the affected wiring harness at no cost to me. Provide assurances in writing that my vehicle is safe to operate. Offer compensation or a loaner vehicle if the repair process renders my vehicle unusable for any period of time. Please consider this a formal request for immediate corrective action. I expect a prompt response within 10 business days of the date of this letter. If I do not receive a satisfactory resolution, I will be compelled to escalate this matter further through consumer protection agencies, such as the INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am submitting this recall request because my vehicle has experienced repeated incidents of a serious safety issue. While driving—most often on the highway—a warning appears that says “Smart System Brake Malfunction.” Immediately after, the car begins braking on its own without driver input. This has happened multiple times and, based on online reports, it appears to be affecting many Mazda vehicles across different models and years. Each time, I have been forced to pull over, turn off the car, and restart it to temporarily resolve the issue. This is extremely dangerous, especially at highway speeds, as it creates a high risk of rear-end collisions and sudden loss of vehicle control. When reported to Mazda, the concern was dismissed as a “sensor issue,” but the repeated nature and widespread reports suggest a potential systemic defect that requires urgent investigation and recall.

1. The navigation has a frosted crackle appearance framing the screen. It is difficult to read the navigation when needed. I know this has happened to other models of this vehicle and those were recalled and covered. 2. The brake senor keeps going off stating "SMART BRAKE SYSTEM MALFUNCTION". This goes off repeatedly while driving. 3. Then a second message pops up and reads "FRONT RADAR SENSOR SYSTEM MALFUNTION".

Approximately three weeks ago, a message on my dashboard said to pull over immediately. Your car is overheating. I pulled over to a nearby auto shop, which indicated that I was just low on coolant. They topped off the coolant, and everything appeared fine. Two weeks later, I got the same message on the dashboard, saying my car was overheating to pull over immediately. I pulled into a nearby auto parts store and purchased coolant for the vehicle. When I checked the reservoir, it was bone dry. I added coolant and drove the car home. The following day, I checked the reservoir to see if there was still coolant, but it was all gone. I scheduled an appointment with the Mazda dealer to have the car inspected. I also checked the Mazda Forum website to see if other vehicles of my make and model had similar issues since I had never experienced this before. I found that the overheating in vehicles of my make and model was a "known issue" with Mazda. When I took it to the dealer the next day, the service agent informed me that the "known issue" was only with the Turbo model. I explained that my vehicle is a turbo model. He said that Mazda did not have to cover any costs because no recall was issued, even though it is a known issue. Mazda is now refusing to cover the repair and has indicated that the repair and replacement of the engine will cost approximately $9800. If I had been alerted earlier, I would have been able to have the car repaired.

While I have not been involved in an accident yet, I have discovered an issue that is believed to be the same issue that other models are covered under a recall, yet for some reason my VIN is not included. The bulleting reference number is 02-001/20 that covers MAZDA 2016-2020 CX-9 models. I have a 2018 CX-9 touring that is exhibiting the exact action of the recall which is "creaking noise from front lower arm bushing/clunking noise from front shock absorber coil spring when driving over bumps". It is only getting worse as time goes on. I am confused as to why some vehicles would be included in this recall yet my year model is experiencing the same issues but is not covered under the recall. I don't know if people are not reporting it, which is why I am filling out this report today.

the cars engine makes loud rattling noise and slips gear. the vecicle jerks hard and feels as if transmission is slipping. car hesitates when i shift into gear, followed by a jerking and jolting into gear. my brakes are replaced frequently. mazda at 65643 miles on car found excess operating noise coming from engine coolant/water pump due to internal bearing failing. they replaced the water pump. software (pyd8-18881-f) replaced. the vehicle has 74000 miles and has same problem. when stopped at a traffic signal it lunges forward at times. and shifts slow to fast gears. this problem is not resolved. the first time it was allegidely fixed was 22Dec2021 it is now 05Oct 2022

Mazda has refused to admit and repair a clearly defective screen, affecting visibility of the camera system, navigation, climate controls and infotainment. Pictures attached are several months old and the issue continues to get worse. The dealer says "corporate won't cover it", corporate customer service will not respond to repeated attempts at contact other than a generic "your VIN is not covered". Service alert direct from Mazda and pictures of my screen showing the EXACT issue are attached. For a $40k+ vehicle with less than 30k miles on it (less than 20k when the issue started), this is unacceptable. Please hold Mazda accountable.

"Spider Cracks" in both the left and right top corners that continue to spread, creating larger blind spots while in reverse. This is an issue that Mazda is aware of and impacts many models from 2014 until 2019. Even though this may not be considered a "safety feature" in it's traditional form, it has become common practice to rely on this feature while reversing the vehicle. Much like additional airbags may not be required, if they are, they should be fixed when they are proven to be faulty. Both the local dealer(Roger Beasley Austin North) and MazdaUSA refused to cover the cost of fixing the issue even though they are aware it is faulty. There is even a Service Notice that was put out by NHTSA that discusses this issue and my model is included, however I have been repeatedly told my VIN # does not qualify.

When this vehicle is low on fuel (but not empty) and under heavy acceleration, the fuel pump temporarily fails causing the engine to stall. This creates a serious safety hazard. Other 2019 Mazda models were covered for a similar defect under NHTSA Campaign Number 21V-875, however this specific VIN is questionably not included under that recall.

When I shifted into reverse the back up camera wouldn’t come on and instead a screen that said check your surroundings stayed on the screen. I reset it using the 3 button reset method and it came back on. Drove it to the grocery store, and then it sat in my driveway for 3 hours. When I went to go back in to town, the car was completely dead. No power whatsoever. We tried jumper cables and those didn’t work. Finally tried a battery charger and after an hour of charging it finally came back on. A car this new shouldn’t be having these issues.

HAPPENED TO ME TWO TIMES IN PAST THREE WEEKS, WHERE I AM COMING TO A STOP, OR HARDLY MOVING, AND CAR SUDDENLY ACCELERATES. HAD MY FOOT ON THE BRAKE AND THE CAR WAS STILL TRYING TO ACCELERATE. YESTERDAY, 4/24/21 IT TOOK ALL MY STRENGTH ON THE BRAKE TO KEEP THE CAR FROM PULLING INTO ONCOMING TRAFFIC. I HAVE BEEN DRIVING FOR 39 YEARS AND HAVE OWNED MANY CARS, AND SOMETHING LIKE THIS NEVER HAPPENED TO ME. I WILL BE BRING THE CAR DEALERSHIP TOMORROW, THE CAR HAS ONLY 10K MILES ON IT AS WE DON'T DO A LOT OF DRIVING WITH IT AS WE OWE TWO CARS. I DID NOT HAVE MY FOOT ON THE WRONG PEDAL, THIS IS VERY DANGEROUS AND IF THE DEALER DOES NOT FIND ANYTHING WITH THE CAR, I INTEND TO SELL IT, AS THIS IS PROBABLY ONE OF THE SCARIEST EXPERIENCE I HAD WITH A CAR. IT IS INTERMITTENT, SO I AM NOT SURE WHEN ITS GOING TO OCCUR, BUT TWICE IN THE LAST FEW WEEKS IS NOT COINCIDENCE ( IT ALSO HAPPENED TO MY WIFE A FEW MONTHS AGO, BUT WE THOUGHT IT COULD HAVE BEEN DUE TO SNOWY ROAD CONDITIONS). IN ONE CASE I WAS COMING UP TO A STOP SIGN, AND WAS BRAKING AND CAR WAS ACCELERATING AS I TRIED TO BREAK. I HAD TO PUT CAR IN NEUTRAL AND THE SHUT OFF CAR TO GET TO STOP. YESTERDAY, I WAS PULLING OUT OF A PARKING LOT, AND WAS WAITING FOR AN ON COMING CAR TO PASS ME BEFORE I COULD TURN ONTO ROAD, WHEN CAR WAS TRYING TO ACCELERATE, WHEN I HAD MY FOOT ON BRAKE, AND TOOK EVERYTHING TO KEEP THE CAR FROM ACCELERATING FORWARD INTO ON COMING CAR. ONCE CAR PASSED, I MADE TURN ONTO ROAD, AND ISSUE DID NOT REPEAT ON THE REMAINING MILE TO MY HOUSE. I WENT ONTO A MAZDA FORUM AND I SAW A FEW OTHERS WHO RAISED THE SAME CONCERN/ISSUE.

THIS VEHICLE IS HAS BEEN ON LEASE FOR 1 YEAR AND HAS LESS THAN 4000 MILES ON IT. INITIALLY WHILE ATTEMPTING TO START THE VEHICLE THAT HAD BEEN PARKED OVERNIGHT IT GAVE THE FOLLOWING WARNINGS: - KEYLESS ENTRY MALFUNCTION - CHARGING SYSTEM MALFUNCTION - VEHICLE OVERHEATING - BATTERY MALFUNCTION THE CAR WOULD NOT SHUT OFF UNTIL I HELD THE START/STOP BUTTON IN FOR ROUGHLY 20SECS. THE DEALER SERVICE CENTER TOOK THE CAR AND TOLD US THE BATTERY HAD A LOW CHARGE, CHARGED IT AND RETURNED IT. SEVERAL WEEKS LATER WHILE THE VEHICLE WAS IN MOTION WITH 2 ADULTS AND 2 CHILDREN ON A CITY STREET TRAVELING AT ABOUT 20MPH IT GAVE THE FOLLOWING WARNINGS: - KEYLESS ENTRY MALFUCTION - CHARGING SYSTEM MALFUCTION - VEHICLE OVERHEATING - BATTERY MALFUNCTION - PULL OVER TO SAFETY AT THIS POINT THE VEHICLE WAS STILL IN MOTION AND THE STEREO, HVAC, AND POWER STEERING CUT OUT. I WAS ABLE TO PULL THE VEHICLE OVER BUT AGAIN HAD TROUBLE SHUTTING IT OFF. I HAD TO HOLD THE START/STOP BUTTON FOR 20-40SECS. THE CAR WAS TOWED TO THE DEALER SERVICE CENTER. WE WERE TOLD BY THE SERVICE CENTER THAT A LOT OF 500 VEHICLES OF THIS YEAR AND MODEL HAD AN ENGINE HARNESS DEFECT THAT NO RECALL HAD BEEN ISSUED FOR BUT THEY WERE AWARE OF DEFECT AND HAD A FIX FOR IT. I FEEL I WAS LUCKY THAT I WAS ONLY TRAVELING 20MPH WHEN THE POWER STEERING CUT OUT. IF I HAD BEEN TRAVELING HIGHWAY SPEEDS ON A BEND THE SITUATION COULD HAVE BEEN TRAGIC AND I HAD MY CHILDREN IN THE CAR. CURRENTLY, BASED ON WHAT I'VE BEEN TOLD BY THE DEALER SERVICE AND SALES MANAGERS THERE ARE POTENTIALLY HUNDREDS OF OTHER 2018 CX-9S WHICH COULD EXPERIENCE THIS SAME MALFUNCTION WHILE IN MOTION.

ON FRIDAY (JUNE 29TH, 2018) AROUND 9:29 PM, MY CAR BROKE DOWN WHILE MY FAMILY AND I WERE TRAVELING FROM ROCHESTER NY TO BOSTON MASSACHUSETTS. I WAS DRIVING DOWN THE MASSACHUSETTS TURNPIKE WHEN ALL OF A SUDDEN MY CHECK ENGINE LIGHT TURNED ON AND MY CAR JUST STARTED TO SLOW DOWN IN THE MIDDLE OF THE HIGHWAY. I WAS ABLE TO PULL OVER IN TIME AND CALLED MAZDA ROADSIDE ASSISTANCE BECAUSE AFTER THE CAR STOPPED, IT WOULDN'T TURN BACK ON. SINCE WE WERE ON A RESTRICTED HIGHWAY, MY CAR HAD TO BE TOWED AND WE WERE TAKEN TO A SAFER LOCATION. WHEN THE CAR WAS PUT DOWN, IT TURNED ON AND THE CHECK ENGINE LIGHT WAS STILL ON AND THE FUEL GAUGE SAID THAT THE CAR STILL HAD 40 MILES LEFT. THE GUY THAT TOWED OUR CAR SAID THAT IT MIGHT HAVE BEEN A FUEL ISSUE, SO WE WENT TO GET GAS. ONCE WE FILLED UP OUR TANK THE CHECK ENGINE LIGHT WAS STILL ON BUT WE WERE ABLE TO REACH OUR DESTINATION. THE NEXT MORNING, I TOOK THE CAR TO THE NEAREST MAZDA DEALERSHIP AND THEY RESTARTED THE CAR AND ACCORDING TO THEIR REPORT NOTHING WAS WRONG WITH THE CAR. THEY SAID THAT IT WAS SAFE TO DRIVE BUT THEY HAD NO CLEAR REASON AS TO WHY THE FUEL GAUGE WASN'T STATING THE CORRECT INFORMATION. AFTER THE TRIP I, REPORTED THIS ISSUE TO MAZDA AND THEY DIDN'T ANSWER ME UNTIL TWO WEEKS LATER. THEIR RESPONSE WAS THAT THERE WAS PROBABLY NOTHING WRONG WITH THE VEHICLE AND THAT THIS WAS A ONE TIME ISSUE. I DID RESEARCH ON MY OWN AND DISCOVERIES THAT SOMEONE FROM SAUDI ARABIA HAD A SIMILAR ISSUE. SINCE MAZDA HASN'T BEEN WILLING TO HELP RESOLVE THE SITUATION I AM FILING THIS COMPLAINT BECAUSE I AM NOT WILLING TO PUT MY FAMILIES LIFE IN THE SAME RISK AGAIN.

I PURCHASED A NEW MAZDA CX-9 IN SEPT 2017 BECAUSE IT HAD THE HIGHLY TOUTED SMART CITY BRAKING SYSTEM FOR WHICH I PAID THOUSANDS EXTRA. I HAD HAD A PREVIOUS REAR END COLLISION AND WANTED TO BE SURE THIS SYSTEM WOULD AT LEAST WARN ME. I SET UP SOME CARDBOARD BOXES WRAPPED IN ALUMINUM FOIL. I SLOWLY (10 MPH) DROVE THE CAR INTO THEM WITHOUT TOUCHING THE BRAKE OR ACCELERATOR. THE CAR DID NOT SLOW DOWN OR WARN ME.(I TRIED SENDING A VIDEO TO YOU FROM MY IPHONE WHICH MAY NOT GO THROUGH) I MADE AN APPOINT TO REPAIR THIS OBVIOUS FACTORY DEFECT AND SIGNIFICANT SAFETY ISSUE. THE SERVICE MANAGER AT OLYMPIA LINCOLN /MAZDA(AND PUYALLUP MILAM MAZDA) REFUSED TO REPAIR IT THEY STATED AND SHOWED ME A PRINT OUT FROM MAZDA FACTORY THAT FORBIDS REAL TIME TESTING AND STATED THAT IF THE TROUBLE LIGHT IS NOT ON THEY AREN'T ALLOWED TO REPAIR OR REAL TIME TEST IT. I THINK THAT THE FACTORY IS AFRAID OF DEALING WITH A MASSIVE SAFETY RECALL. TO COMPOUND THINGS THE ACTIVE LIVE TRAFFIC DISPLAY DOESN'T WORK EITHER WHICH MAKES MATTERS WORSE SINCE I CAN'T SEE WHERE AN ACCIDENT HAS OCCURS IN REAL TIME. I UNDERSTAND ITS DEFECTIVE IN MOST MAZDAS WITH NAVIGATION IN 2016 AND BEYOND. THE IS A CLASS ACTION LAW SUIT STARTING TO BE FILED ABOUT THIS. *BF CONSUMER STATED REGARDING DEFECTIVE AUTOMATIC BRAKING SYSTEM ON 2018 MAZDA CX-9.
