Undersized axles and tires may have been installed, which can lead to a tire detaching. The axles installed on these vehicles were incorrectly identified as having a 7,000 pound capacity when they were only rated and designed to carry up to 6,000 pounds.
Risk. The owner of a vehicle with the incorrect axles could unknowingly overload the vehicle, possibly resulting in a loss of control or reduced braking distance, potentially increasing the risk of a crash.
Fix. All affected vehicles have been captured at the dealer level and are being repaired prior to sale. Therefore, an owner notification letter will not be issued for this campaign. The contact owns a 2022 Forest River Cedar Creek. The contact stated that while outside of his trailer, he smelled a fuel odor emitting from the trailer. The contact performed an inspection on the trailer and discovered that his LP tanks were consuming propane at an accelerated speed. The contact took the vehicle to a dealer where he informed them that he would not take possession of the trailer until the trailer was repaired. The contact stated that while the trailer was being repaired, the stove caught fire while a technician was inside the trailer; he was also informed that the fire had caused damage to the inside of the trailer which they would also repair. Upon investigation, the contact discovered that his vehicle was placed under NHTSA Campaign Number: 22V243000 (EQUIPMENT) which he linked to his failure. The manufacturer was notified about the fire but offered no assistance. The vehicle remained in possession of the dealer and had yet to be repaired. The failure mileage was approximately 35.

Dometic rv refrigerator mod. No. Rm2852 (already under recall campaign id 06e076000) to install a heat shield, does not address the defective coil cracking, only just diverts the flame front if ammonia is released and catches fire. I have experienced a catastrophic failure of the coil releasing so much ammonia that it irritated my eyes and required venting rv to rid it of the fumes. Fortunately refrigerator was in electric mode, not gas so no fire. After finding out about recall (was never informed of by dometic as required under terms of recall) and contacting dometic i was informed by their representative that under the terms of recall agreed to by them and the nhtsa they were only required to provide the heat shield free of charge, not replace the defective coils. Especially if your unit is out of warranty you are out of luck to get coil replaced at no charge if it has failed because of cracked coil. It is my opinion that this recall is flawed, in so far as it has allowed dometic to cover-up a defective product without being responsible for replacement of defective part which can result in potential fire. I therefore request that the nhtsa re-open investigation into this recall and require dometic to replace defective part(s), the entire coil assembly, that will fail or have already failed creating the fire hazard, which can result in loss of entire rv due to fire and possible injury or death of the occupants. *tr

The belts on all 4 tires were separating and one blew out while on the roadway and a second had lost air pressure with no notification. All 4 tires on the fifth wheel were coming apart per the tire shop (aok tires).
Tire failure verified by CompassRV and P&J Towing. Belt broke causing the tire to separate and damage the outside of the RV. Safety risk: immediate swerve of a large camper. Multiple complaints on this tire through RVforum. Tire inspected by Compass RV and P&J tire and towing . No warning just a loud pop. ST235/80/R16.
All four of our castle rock st tires on our travel trailer are showing premature cracks in the sidewall and where the tread meets the sidewall. This could lead to a catastrophic failure of the tire at high speed. These tires have been in use less than 18 months.
While driving on interstate 75 in georgia, i had two trailer tires blow out approx 100 miles apart. Brand is castle rock by lionshead. Was driving well under the speed rating of 75, the trailer was also lightly loaded. Tire pressure was always checked and was correct at the time of blowouts. Tires were about 2.5 years old, about 8000 miles. Tread looked great before failure. This appears to be a common occurance and nhtsa should investigate, and get these dangerous tires off the road. I purchased a 2018 sabre cobalt 37bhq fifth wheel in may 2018. I have taken this fifth wheel on 5 trips, all of which were less than 50 miles from home. On the fifth trip i arrived at the campground and noticed the rf tire tread had come off in a large section and parts of the steel belt were exposed. I ended up replacing all four tires because i read other complaints from rv owners with similar problems with these tires (castle rock st235/80r16).*dt*jb
While driving west bound on interstate 80 about three miles outside of truckee california our back tire tread separated from the tire. It proceeded to slap against the wiring of the silde out that supplies all our power to our kitchen. It also pulled the brake wires from their housing and caused a great deal of damage. The tire never to blow and i still have it at the full psi that is recommended. The tread simply separated from the whole tire and proceeded to cause mayhem.
Heading to go camping driving 55 mph with my son and wife following us in their car, the right front trailer tire blew on my camper. This caused the fender to be torn off and some of the skirting to be bend and scuffed up from the rv. Spare was put on and we continued to the campground. Now investigating what to do to proceed. I have not notified insurance yet and may attempt to order parts for rv and repair myself. All of the tires will get replaced before my next camping trip. The rv is 1 year and a month old.
Vehicle is a 2017 5th wheel trlr.. On 7/12/2019 i was towing the trlr at 70 mph and pulled into a stop. Trlr is 2 axles and the rt. Front tire was flat with about a 7 inch strip of tread hanging loose at the junction of the tread/sidewall. I replaced the tire at a cost of $140.00. Tire pressure was 75 psi and the air temp was 90 deg. On 7/14/2019 i had another blowout with the left rear tire at 70mph. The tire came apart and damage the trailer floor and side skirting. The air temp was 77 deg. Both tires were original equipment goodyear marathon's st 235/80r16 load e. Dot 2vun 1lhv. I intend to replace the remaining marathon's as soon as possible. I have pictures of the second tire as well as trlr. Damage. Trlr is a forest river, sanibel 3591.
February 2014, we purchased a new 2013 forest river (not listed in nhtsa mfg. Selection): coachmen freedom express liberty edition, 297rldsle. Trailer came equipped with 225/75r15 d trail express - power towing tires. Not listed in your data. Dealer installed a husky 801-1200# load balancing and anti-sway bar system on our 2008 yukon xl 1/2 ton suv. We towed our rig back to fort myers 100 miles. During our trip home we had to stop several times to adjust +++ the sway bar tension because the trailer was still swaying. I discussed this with husky and they informed us that they recommend a 2nd (left) sway bar on all trailers over 24ft. So i purchased and installed a 2nd sway bar from camping world. On 4/11/14 we embarked on our 1st camping trip, destination cleveland, ohio. Before we left fort myers we checked out the entire trailer including tire pressure and set all tires to 65psi cold. During the drive we were very concerned at the vibration and sway of our rig so we stopped several times to check pressure and inspect the tires. On our 1st day we drove to lake park, ga. The next day the vibration and sway grew much worse. To be truthful we blamed georgia road quality. We were wrong! With only 653 miles on the new rig we experienced a violent tire failure at 63 mph on i-475 going around macon, ga. We then had a 2nd tire fail on the opposite side of the trailer so we replaced all 5 tires. The 2nd tire did not blow up but was contorted into an "s" shape due to ply separation. The macon tiresplus store manager said we had experienced severe tire ply separation possibly due to the trailer sitting in one place for so long (14+ months) in the hot florida sun. When we returned to i-475 we were amazed how smooth the road really was. I wish i had known the symptoms of a trailer tire failure prior to this experience. The sway and vibration we had experienced was caused by the failing tires. However, it cost us over $1200 and delays. We have photos of all the failed tires. *tr
I have a 2009 forest river sandpiper 5th wheel trailer. I have had tire problems and was told that it was due to the manufacturer undersizing the axles and springs. I have had a catastrophic blowout which could easily resulted in an accident but thankfully there were no other vehicles nearby. I had one of forest river's own dealers tell me that i had a problem with the springs being flat and that it was unsafe to operate. The tires are highway trooper tires manufactured in china and sold by greenball corporation. When searching for the tires online, i was surprised to learn that many of the postings were about the failure of the tires at about 4000 miles which is exactly when i had a blowout. My biggest concern is the flat springs and the fact that forest river used two 6000 pond axles under my 15,000 pound trailer. My wife and i live in our trailer full time traveling thruout the country. We have put over 7,000 miles on since july and i'm afraid that our travels could be dangerous but can't afford to replace out trailer which we bought brand new less than a year ago. I have contacted jeff johnson at forest river on numerous occasions over the past 4 months but can't seem to get the company to act on any of my dozen warranty problems. The date of the blown out tire is what i'm using as the incident date but i would like to see an investigation of both forest river corporation for the spring and axle problems as well as the greenball corporation for the defective tires that they are importing and selling here in the us. After finishing your form, i found that your form would not accept highway trooper under the greenball corporation for the tires and that my sandpiper was not one of your options under forest river so the form is not perfectly accurate. *ln

Upon turning into a parking lot with a very small incline at approximately 2 miles per hour, the rear of the fifth wheel bottomed out & scraped the pavement causing it to rip off the bottom of the back cap. As a result of the back cap being pulled away from the frame, it also caused us to have to pull the fifth wheel home without any brake lights, hazard lights or turn signals, which is a hazard to us & other drivers on the road. The rear of the fifth wheel sits entirely too low to ground & this will happen every time it is pulled to a new location unless the driveways are perfectly level with the highway.*jb

We purchased our brand new fifth wheel from a dealer and pulled it across the street to a campground for a few days. On the drive home we pulled off into a gas station with a very slight incline. The back end of the cedar creek 35lft is so low that it scrapped the pavement as we pulled in and damaged the back wall as well as all the brake wiring. Our unit had cracks in the gel coat, upon removing the nose cap we found that the welds had failed and we were told that the metal in the actual frame was to weak to hold the weight of the unit. It had to be sent back to the factory in indiana and was completely rebuilt, we did extensive research and a great number of forest river products that have lippert frames have had weld and metal failures. Our unit was in the shop for over 4 months. Had we not found the problem we could have had the nose of the fifth wheel come down on our truck and caused a serious accident. *tr
I have 2004 cedar creek fifth wheel. The bedroom up front has glass doors that go to the bottom of the floor. My 2 year old grandson trip and fell through the glass door. Luckily he fell backwards and was no seriously hurt. I felt he would have fallen head first that could have been deadly. I contacted cedar creek and they could care less.. The contact owns a 2023 Forest River XLR Toy Hauler RV. The contact stated while driving approximately 40 MPH and hauling the RV he heard an abnormal exploding sound and realized that the passenger side rear tire on the RV blew out. The contact stated no warning light was illuminated. The contact had weighed the RV and it was approximately 1,400 lbs. The contact pulled over to change the tire on the RV and noticed that the leaf-spring on the passenger was collapsing. The contact checked and observed that all the leaf-springs on the RV were collapsing, and he noticed that the passenger side front tire was wearing at an extremely accelerated rate at only 700 miles since the purchase. The contact drove the vehicle to a tire dealer and had four new tires installed on the RV. The contact stated that the tire dealer found that the tire pressure monitoring sensor banding had detached from the rim of the tire and was cutting the tire. The contact had taken the vehicle to a local dealer, where it had not yet been diagnosed and had not been repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately on the RV was 700 and on the vehicle approximately 7,600.
While towing our 5th wheel cross country on a trip in July 2020 our front hydraulic jack legs starting coming down several times while driving and contacted the road surface which could have possibly resulted in loss of control. Some how the hydraulic system was applying pressure by itself while towing .I first tried disconnecting electrical power to the hydraulic system and keeping the jack legs up with a 400 lb rated ratchet strap and the legs would still come down on its own with enough pressure to break the 400 lb rated strap. I was finally able to keep the leg secured up with 2 x 600 lb rated ratchet straps. While in Indiana we took our new 5th wheel to a Lippert componets service center and they replaced both front jacks and drained and replaced all the fluid under warranty. The service tech said this issue happened sometimes. Searching the internet it appears this a fairly common problem. This is a safety issue and Lippert should be responsible for coming up with a permanent solution.

Leaf spring bracket or axle mount came loose causing the axle to come loose on drivers side causing the whole rv to fish tail - luckily no bodily injury occurred. This happened on state rt 22 near conway south carolinia. Incident date was april 7, 2020. The dealer has provided limited information on repair - waiting for parts. Contacted factory and they also stated waiting on 3 party vendor parts. No explanation on why this happened. After some internet searching found the attached pdf and also copy of letter sent to cedar creek.

Tl* the contact owns a 2005 forest river cedar creek fifth wheel rv. The axle has a spindle that was misaligned. This condition caused severe tire failure on all four tires. The forest river manufacturer was notified, and he was referred to al-ko cobar corporation. He was advised by a representative to ship the old set axle to elkhart, indiana for examination and repair. He shipped the axle back in september 9, 2007 and has not received a response. The mileage was not applicable.
I have a cedar creek 5th wheel camper. When i returned home from a trip i noticed all the wheels were tilted inward and the tires were worn to the steel core inside. I checked with the manufacturer and he said there were no recalls. I found another person on the internet with the same exact axle failures and replaced them. I believe a check of company records will find many cases of owners of this type of unit having replaced these axles at their own expense. This is a very important safety issue that should be addressed asap. I have been a mechanic for 43 years and never saw axles fail like this. *tr

We purchased a Forest River fifth wheel trailer that arrived with serious manufacturing defects across multiple systems. The electrical wiring system was dangerously faulty—exhibiting loose connections, voltage irregularities, and safety risks confirmed by technicians. The furnace system has remained nonfunctional since delivery, making the unit uninhabitable in colder temperatures. Additional issues include a broken slide-out mechanism, compromised seals, a missing pipe, water intrusion, and a missing roof panel. All components are still in our possession and available for inspection upon request. These defects posed serious safety risks to our family, especially due to the compromised electrical system, which technicians described as a potential fire hazard. The inability to heat the trailer further contributed to unsafe living conditions. The problems have been confirmed by multiple certified technicians. Their assessments documented the electrical dangers and structural failures. Forest River's Warranty Director confirms that no Pre-Delivery Inspection (PDI) was completed before the unit was delivered to the dealer, indicating a systemic inspection failure prior to sale. To our knowledge, the manufacturer has not directly inspected the trailer in person, despite our repeated requests. No inspection has been conducted by law enforcement or insurance representatives. There were no warning lamps or diagnostic messages present, as the trailer systems lacked functional indicators for these failures. Most symptoms appeared immediately upon delivery and worsened as attempts at repair failed. We are submitting this complaint because Forest River continues to avoid accountability, and the unit remains unsafe and unusable. We confirmed there is an open recall (24V395) on our VIN on forest river's website [XXX] ). Upon review of the recall instructions listed online - our RV was in fact wired incorrectly and not only needs inspected but fixed. Recall 24V395 was opened on June 5, 2024 and dealer notification occurred July 14, 2024. With this in mind, our dealer should have been aware that they sold us a new RV with an open recall. The automatic awning did not have proper backing plates installed, so the first time going out caused the awning to collapse completely against the full glass wall. The wire harness system was not the correct gauge wire, which posed a potential fire hazard. The fuse was pulled out because the motor to the faulty awning was overheating. The front entry (only) door for entering the RV and existing safely was not mounted correctly, so the owners (occupants) were locked inside during the daytime hours when the door expanded with sun/heat. The owner is disabled and was oftentimes locked inside. ALL within the warranty division and build team were aware of my disability; and did nothing to assist or mitigate the potential life-safety hazard. Forest River knew I was living in the coach for an extended period of time; and they knew I was unable to travel the rig. I/We were repeatedly told that the issues would be addressed, but they did little to nothing in assisting us by securing a qualified field team. I/We were forced to secure various RV vendors to assist, to no avail. I was forced to have it trailered into a facility; whereby the wiring issues continue and the Forest River warranty division forced me to pay the vendor over $4k for warranty items that they were supposed to cover. This rig had a poor build and Forest River refuses to take the necessary steps to cure the deficiencies. I am told by the repair shop that the wire harness became a "RECALL" item; which solidifies my concerns, as others were having the same issues. Forest River did not install backing boards for the awning that could have killed someone when it came crashing down (all caught on my Ring Doorbell), plus the array of photography taken to show the shoddy workmanship during the build. 2021 cedar creek rv. Consumer writes in regards to warranty issues. The consumer stated upon pickup of the vehicle after purchase, it experienced some warranty issues. The vehicle was returned to the owner several months later. After another four months the consumer was offered an identical replacement vehicle. The dealer did not honor the replacement and stated the vehicle had been repaired. The dealer returned the vehicle with bedroom slide still unrepaired. The slide nearly fell out of the rv. The consumer stated there were issues when attempting to level and open the slides as the power would continuosly shut down the control screen. The consumer stated while driving the vehicle there was an odor that turned out to be battery acid. The batteries were smoking and could be smelled from 1000 yards away. The consumer stated the dealer was mad aware of the failure and offered to buy the vehicle back if, the consumer signed waiver not to report the incidents. The consumer requested the vehicle be bought back in full.
On one of the first trips with the fifth wheel we were traveling and while in motion, suddenly lost all trailer brakes. We were on a town street, in the business district during heavy traffic. We were able to pull over safely and inspect the vehicle. The brake lights were functional and the fifth wheel had power. When my husband crawled under the vehicle to see if he could determine the issue he found that the electrical line to the brakes was not spliced together properly and had come apart. My husband was able to tape the appropriate wires together so that the brakes were functional and the trailer is due to be inspected by the dealer in june. *tt vin not valid updated 9/20/18*jb

On the first trip out, We had a tire blowout. There was no warning on the TPMS, and the tire tread peeled like a retread. It caused major damage to the rv slide out, and threw tire tread across the highway putting other motorists at risk. Forest River refused to cover any of the damage. The tire manufacturer covered less than 30% of the cost of a new tire. After the repairs, we made another trip. This time we had two more blowouts. This damaged the other side of the rv and also took out all of the brake wiring, leaving us with no brakes on the rv. One tire blew across the entire tire from side to side, the other peeled the tread like the first blowout. It also threw tire parts across the interstate. Once again there was no warning on the TPMS with either tire. We then replaced all four tires with US made Goodyear tires and repaired the rv again. The tire manufacturer says it is going to pay for partial reimbursement of the four tires. Forest River will not even acknowledge my emails to these incidents other than to ask for pictures. They no longer responded after that. No brakes, tire parts flying across the highways into other motorists and having to change tires along busy interstates are all safety concerns that Forest River refuses to address. The tires are Lionshead Ridgway Sport ST LH-001 size ST235/80R16. Since there were three incidents in total, I only put the date of the first one below. The others occured on 11/29/2022 at 50mph and 12/4/2022 at 65mph.
We have a 2017 cedar creek 38fbd with dual 7000 pound axles and oem alloy wheels. The second of my four alloy wheels cracked sometime during the 3 days we were setup at coe riverside park @ lake kanopolis, ks. I found this out when my aftermarket tpms alarmed that the tire was at 5 psi rather than near 110psi when i turned it on. If the crack and resulting pressure loss had occured while we were pulling our fifth wheel, we may have lost control of the trailer and injured not only ourselves, but others on the road. As i stated, this is the second wheel to fail. The previous failure was about two months ago with pretty much the same circumstances. We weigh our fifth wheel every so often and so far we've never had over 6700 pounds on either axle. Our tires are g rated to ~3800 pounds when in a dual axle configuration (sailun 235/85r16) that were purchased new in march 2019.
Three tires have various stages of steel coming out of the side between the tread and sidewall with only 4000 miles of use. Tire brand is trail express power touring 235/80/16 supplied by lions head tire and wheel. Company is unwilling to replace faulty tires. Trailer was being towed on the highway at 60 mph when the first tire displayed low air with bulge. Trailer was pulled over and steel was hanging out of the side of the tire and air leaking out. Upon further inspection two other tires were discovered one with wire hanging out of the side and one with just a tear/ bulge just like the others but with no wires hanging out. Lionshead tire and wheel company was contacted but no solution or replacement tires supplied. No damage to the vehicle as flat was discovered before damage was done.
While returning home from a trip we were notified by another driver we had just lost a tire. Once we pulled over we found that the entire wheel had sheared off the hub. After limping the trailer off the highway we went back and retrieved the tire. I found two of the studs with the wheel. Of the six studs 2 sheared even with the hub and four sheared inside the hub. Since we have owned the trailer the only two times i can not verify how well the studs were torqued is from the manufacture and last year when we had all four tires replaced at a dealer in pennsylvania. All other times the studs have been torqued per dexters tech data. It is very unusual with how this happened. I can provide pictures of the two recovered studs and the hub if needed. *tr

We purchased our brand new fifth wheel from a dealer and pulled it across the street to a campground for a few days. On the drive home we pulled off into a gas station with a very slight incline. The back end of the cedar creek 35lft is so low that it scrapped the pavement as we pulled in and damaged the back wall as well as all the brake wiring.
