(Forest River) is recalling certain 2019 Coachmen Chaparral and Shasta, 2019-2020 Coachmen Concord and Orion, 2020-2021 Coachmen Cross Trek and Forest River Berkshire, 2019-2021 Coachmen Freelander, Leprechaun and Prism, 2020 Coachmen Mirada, Pursuit and Sportscoach, Dynamax Force, DX3, Dynaquest XL and Isata and Forest River Wildwood and 2008 and 2020 Forest River Salem vehicles equipped with Dometic 3 burner cooking stoves. The saddle valve securing bolt may be overtightened, possibly damaging the o-ring seal and causing a continuous gas leak.

Risk. A gas leak can increase the risk of a fire.

Fix. Forest River will notify owners, and dealers will install a remedy kit of gaskets, washers, thread locker bolts, and two round orange labels, free of charge. The recall began January 11, 2021. Power Train:automatic Transmission15V726000

What's wrong. Forest River, Inc. (Forest River) is recalling certain model year 2016 Forest River FR3 and Georgetown, and Coachmen Mirada and Pursuit recreational vehicles manufactured August 7, 2015, to October 22, 2015. Due to an incorrect shift cable bracket and an improper shift cable adjustment, the affected vehicles can be shifted out of 'PARK' without applying the brakes.

Risk. If the vehicle can be shifted into reverse without the brakes being applied, the vehicle may unintentionally move, increasing the risk of a crash.

Fix. Forest River will notify owners, and Ford dealers will replace the transmission shift control bracket and adjust the transmission shift cable, free of charge. What's wrong. Forest River, Inc. (Forest River) is recalling certain model year 2016 Coachmen Mirada motorhomes, model MDA31FWF, manufactured August 21, 2015, to October 2, 2015. In the affected vehicles, the parking brake actuator may be out of calibration causing the parking brake system to not properly hold the vehicle even with the parking brake pedal fully depressed. Additionally, the parking brake may not disengage completely when the parking brake pedal is released.

Risk. If the parking brake does not fully engage, the vehicle may unexpectedly roll, increasing the risk of personal injury or a crash. A parking brake that drags may result in a wheel end fire.

Forest River is recalling certain model year 2013-2014 Forest River models Blue Ridge, Cardinal, Cherokee, Columbus, EVO, Georgetown, Flagstaff, Forester, Lexington, Puma, Rockwood, Sabre, Salem, Sandpiper, Sandstorm, Sierra, Stealth, Sunseeker, Wildcat, Wildwood, Work and Play and XLR; Coachmen models Brookstone, Catalina, Chaparral, Concord, Encounter, Freedom Express, Freelander, Leprechaun, Mirada, Pursuit, and Sportcoach; Prime Time models Avenger, Crusader, Lacrosse, Saniber, and Tracer; Palomino model Maverick Truck Camper; Cargo Mate model Eliminator and Shasta models Revere, Phoenix recreational vehicles manufactured from February 13, 2013, through April 18, 2013. Due to a design change in the power awning motor assembly, the motor assembly screws may unknowingly shear.

Risk. If the motor assembly screws shear, it is possible that the awning can unfurl unexpectedly, either while the vehicle is at rest or while in transit, increasing the risk of personal injury or a vehicle crash.

Fix. Forest River will notify owners and Dometic staff will replace the affected motors with motors of a different design. The safety recall began on July 29, 2013. (Forest River) is recalling certain 2014 FR3, 2009 Forest River Encore, Sportscoach Elite, and Aurora, 2008-2009 Cruise Master, 2012-2015 Aviator, 2007-2014 Georgetown, 2007-2009 Legend, 2008-2012 Ridgeview, 2008-2014 Cross Country, 2010-2014 Coachmen Encounter, 2009-2014 Pathfinder, Solera, Mirada, Prism, and Pursuit motorhomes. The affected vehicles are equipped with certain Lippert-brand Coachstep Double and Triple Electric Steps. These steps use a bolt to attach the fan gear assembly to the steps. This bolt may fracture allowing the fan gear to disengage from the steps. As a result, the steps may not remain in the expected position and may be unstable.

Risk. Unstable entry and exit steps increase the risk of injury.

The refrigerator may have a defect in the boiler tube. Pressurized coolant solution could be released into an area where an ignition source (gas flame) is present.

Risk. Extended operation of a leaking cooling unit could lead to a fire from gas which might escape.

Fix. Coachmen is working with dometic in order to repair these refrigerators (please see 08e032). Dometic will repair these refrigerators by installing a secondary burn housing, a thermal fuse, and a melt fuse free of charge. Dometic has retained stericycle inc. To manage this campaign. Stericycle will assist the owner in locating dealerships or service centers and will provide assistance with scheduling of appointments. What's wrong. On certain class a motor homes, the rear hitch attachment brackets that connect the hitch assembly to the chassis frame may crack at the 90 degree bend.

Risk. If this were to occur, a complete separation of the hitch assembly from the chassis could occur, resulting in a crash due to a loss of control of the towed vehicle or trailer.

Fix. Dealers will repair the hitch assembly attachment brackets free of charge. The recall began on march 24, 2005. Owners may contact coachmen at 1-800-453-6064.

What's wrong. Coachmen is recalling 37,293 motorhomes and fifth wheel travel trailers equipped with a two-door refrigerator manufactured by the dometic corporation. The refrigerator may have a defect in the boiler tube. Pressurized coolant solution could be released into an area where an ignition source (gas flame) is present.

Risk. Extended operation of a leaking cooling unit could lead to a fire from gas which might escape.

Fix. Coachmen is working with dometic in order to repair these refrigerators (please see 08e032). Dometic will repair these refrigerators by installing a secondary burn housing, a thermal fuse, and a melt fuse free of charge. Dometic has retained stericycle inc. To manage this campaign. Stericycle will assist the owner in locating dealerships or service centers and will provide assistance with scheduling of appointments. The frame extrusion at the base of the kitchen slide room, that serves to maintain the locking mechanism for the slide room, may become loose from the body of the vehicle. When this occurs, the locking mechanism is rendered ineffective and the slide room can open unexpectedly.

Risk. Should this occur while the vehicle is in motion, the potential exists for a crash.

Fix. Dealers will install longer self-tapping screws to ensure the extrusion is secured to the steel framing. In addition, "travel locks" that, while in place, prevent the slide room from moving out under any circumstances will be supplied with all units. Owner notification began on april 28, 2003. What's wrong. On certain motorhomes, the chassis leaf spring rear shackle and/or shackle bolt could come in direct contact with the lp tank when the vehicle is in motion.

Risk. This could cause the lp tank to become damaged and potentially leak increasing the risk of a fire.

Fix. Dealers will move the mounting location of the tank out of the chassis rails, away from the chassis leaf spring rear shackle. Owner notification began on april 12, 2004. Owners should contact coachmen at 1-800-453-6064.

What's wrong. Certain motor home chassis fail to comply with the requirements of federal motor vehicle safety standard no. 105, "hydraulic and electric brake systems." the brake system warning indicator may not function as required by standard.

Risk. In some of the affected vehicles, the brake system warning indicator will not illuminate when the ignition is in the start position or for a low brake fluid condition, if the body builder relied solely upon ford's ivm compliance statement for the brake system warning indicator.

What's wrong. On motor home chassis built by workhorse and equipped with bosch brakes, the pads can drag on the rotor surface due to the pin slide.

Risk. A hang up or lock up may cause the calipers to over heat and not allow the driver to stop safely. This could cause overheating of the brake calipers which could result in a vehicle fire.

Fix. Workhorse is conducting the owner notification and remedy for this campaign. Dealers will repair the brake caliper. Owners should contact workhorse at 1-877-294-6773 or coachmen at 219-825-5821.

What's wrong. Coachmen is recalling 2,555 my 2000-2009 georgie boy and coachmen recreational trailers equipped with axles that include bosch zops or zoht pin slide hydraulic disc brakes. When combined with occasional or seasonal vehicle operation may experience calipers sticking in the applied position. This can result in abnormal heat generation at the wheel end causing brake drag. The driver would normally have warning of the brake drag if undetected by the driver.

Risk. The driver would normally detect the brake drag but if undetected the temperature increase at the wheel end can eventually lead to soft pedal conditions due to brake fluid boil, and possible extended stopping distance increasing the risk of a crash.

Fix. Dealers will inspect and replace the necessary components free of charge. Workhorse is handling the remedy for this campaign please see 09v-110. 2022 Mirada

The contact owns a 2022 Coachmen Mirada ES RV built on a 2021 Ford F53 Chassis. The contact stated that the rear driver's side and rear passenger's side stabilizer bar brackets supporting the rear axles had fractured in half and were hanging on the ground. The contact stated that the vehicle was previously taken to an unknown dealer to be repaired under NHTSA Campaign Number: 22V731000 (Suspension) however, the dealer informed the contact that no issues were found, and the repair was not needed. The dealer and the manufacturer were not notified of the failure. The RV was not diagnosed or repaired. The failure mileage was 8,964.

2022 Mirada

Driver side rear sway mount bracket fell off. This is well documented on the internet and has been occurring for many years on the Ford F53 chassis. Sway bar keeps sway and body roll to a minimum while driving. Without the mount, it's ability to prevent sway is diminished. If the bracket fell off during travel, the large solid piece of metal and bolts would have fallen into road and potentially cause an accident or damage to another vehicle.

2021 Mirada

2015 Mirada

My issue is with a 35' coachmen motorhome that i purchased in 2015, although the issue seems to be industry wide involving any gasoline powered class 'a' units. I personally am aware of several other makes, models of class a's with this flaw. When encountering a rough road, dips and valleys, and particularly when cornering, there is an excessive sway, list, l to rt lean or whatever you call it .depending on the severity of the 'list' it can be scary...i have measured as much as a 25 degree reading of list with an inclinometer. The industry is aware of this issue, it can't be overlooked if they drive their own product! There is a fix for this problem that should be addressed at the factory; however, the manufacturers would rather leave it to the purchaser. 'roadmaster', 'sumosprings' and other suspension enhancement companies are doing very well taking care of this problem but at a cost of $1k and up. I do not know if this flaw could be a safety issue but if not it surely must come close.

2014 Mirada

Our 2014 coachmen mirada rv has 30,800 miles and started to vibrate with noise while driving. We took the rv to a repair shop and they diagnosed problem as king pins (suspension) on our ford f53 chassis. King pins help hold everything together and normally last 175,000 miles. Knowing this we wanted a second opinion so took the rv to a different mechanic who confirmed the problem is the king pins. I researched the internet and found 7 cases with same problem on rv chat rooms so i contacted ford motor company. We were at that time a few weeks over 5 year warranty. Ford gave me the run around and said my rv dealership didn't file an extended sale warranty date and it takes one month to update the system after receiving change order. After we waited a month ford says they won't help pay for the $2,200 job because our first claim with them was closed and they'd have to start the claim today which is 2 months over warranty.

2013 Mirada

Rear sway arm mounts: while performing a pre-flight of the vehicle before a family trip, a metal component was noticed hanging underneath the rear of the vehicle. Upon further inspection it was noted that the drivers side rear clamp that holds the rear sway bar urethane bushing in place to the axle was fractured and hanging loose from the lower mount. The upper and lower bolts were still in place, but the assembly/metal had fractured 3/4 of the way up the clamp. This placed the sway bar / stabilizer bar in an unsecured situation on the drivers side of the vehicle. The vehicle was parked in a residential drive way being loaded in preparation for a family camping trip. The fracture surface had discoloration and a large rust spot which permeated the width of the fracture. The lower restraining bolt was found to be several threads loosened and the first several threads below the head were rounded. The urethane bushing appears to be damaged from the metal component unsecured movement, and it is unknown what additional stresses this placed on the down comers. The local ford dealer is being contacted for replacement/repair and the vehicle was returned to storage until repairs can be completed. This is a ford f53 chassis with factory frame extensions to support the recreational vehicle structure.

2011 Mirada

Note: we have since learned this incident has been characterized as "the death wobble" as follows: this occurred on our first "shakedown" (pun intended) trip of 300 miles taken in a new 29 ft coachmen mirada (leftover 2011 model) class a motorhome w/f53 chasis which was manufactured by ford in oct 2008. Its noteworthy that, initially, a slight shutter in coach body could be felt when highway driving over minor bumps or surface discontinuities. After driving 200 miles, we drove at 55 mph over a highway bridge expansion joint - kind of a quick double bump, which set off an extremely violent rapid shaking of the entire coach resulting in difficult steering and a harrowing experience. Thankfully, there was limited traffic, traveling in right lane, and room on shoulder to pull over. This violent shaking continued until < 5 mph. Checked tires and under chasis - all seemed ok. Continued driving home to complete the balance of trip, avoiding as many bumps as possible, and with no further incident. Rv is currently at a ford service shop for inspection and resolution. This was a very unsafe condition, and could easily have resulted in a major traffic incident, especially if there was heavy traffic and limited room to pull over. *tr updated 11/25/13

Living room slider has been in for repairs twice. 1st was 2/7/04 when it was out of alignment with only 2572 miles on coach, and 2300 of those miles were delivery distination. 2nd was7/2/04, 5 months later with only 3769 miles on coach, the slide came out when the unit was sitting and the park brake was off, then it came out when the coach was sitting with the engine running and the park brake off.*ak

The contact owns a 2017 Coachmen Mirada Motorhome built on a 2016 Ford F53 Motorhome chassis. The contact stated that while driving at an undisclosed speed, the carbon monoxide alarm alerted him that there were carbon monoxide fumes inside the RV. Upon inspection, the contact noticed that there was a hole in the firewall caused by exposed engine wires that were not sealed. 2016 Mirada

The vehicle was taken to the dealer several times for multiple repairs. 2005 Mirada

I drove my class a motorhome up my driveway after dumping my grey water tank. I went to exit through the only door and it would not open no matter what i did. I had to crawl out of a window to get out nearly injuring myself. It has been 6 hours and i have repeatedly tried to open the door from the outside and it will not open. Had there been a fire or emergency it would have been a real problem. I believe it is a trimark lock. *tr

1999 Mirada

We own a 1999 coachmen mirada motorhome with a large slideout for a dinette and sofa. We were driving up a narrow winding road in a remote area on the way to kernville, ca when, for no apparent reason the slideout extended fully (about 2 feet from the main chassis.) we were traveling at approximately 40 mph at the time and had just gone around a right bend in the road (the slideout is on the left of the coach.) if there had been a vehicle passing in the other direction, this could have potentially resulted in a fatal accident. We had to stop the coach, push the slide out in manually and block it with wood to prevent it sliding out again because it appeared to be free floating. We currently have stiers rv repairs in bakersfield, california evaluating the coach. We called coachmen service to report the problem, jason told us that failures happen! We understand us transportation secretary ray lahood is ordering a full review of motorcoach safety. We feel he needs to investigate this problem and find out if coachmen has a history of slideout failures under warranty or on later models. Please get back to me and let me know if this problem needs to be reported to any other agency. We are also going to write to coachmen and officially report the problem. 2010 Mirada

Tl* the contact owns a 2010 coachmen mirada. The contact stated that while driving at 55 mph, he was alerted by fellow motorist that light gray smoke was emitting from his vehicle. The contact immediately pulled over and had all the occupants exit the motor home. Upon inspection, the contact saw that the transmission fluid had overflowed onto the exhaust pipe which had cause a fire. A police report was not filed and no injuries were sustained as a result of the failure. The contact was able to retrieve a fire extinguisher and extinguish the fire. The contact allowed the vehicle to cool off and continued to drive the vehicle at a low rate of speed until he reached his destination. The contact took the vehicle to an independent mechanic where it was discovered that the orifice for the transmission fluid to flow to the engine was to small; due to this design flaw, the transmission overheat which caused the failure. The vehicle was repaired by the independent mechanic. Coachmen had initially been notified of the failure and informed him that the failure was related to ford who was the maker of the chasity. The contact reached out to the ford manufacturer where he filed a claim with ford. They declined to assist was the repair of the vehicle. The failure mileage was approximately 41,000. *bf *as

2021 Mirada

The primary component that failed is the transmission system, part of the vehicle’s powertrain, which has exhibited ongoing malfunction since the purchase of the 2021 Coachmen Mirada 35ES. The vehicle has been unsafe to drive due to this unresolved transmission issue, putting the safety of the driver and others at risk. The problem has been confirmed but not fully resolved by Peach State Ford under warranty in 2021, and the vehicle was scheduled for further inspection at Gator Ford; however, repeated delays and refusals to accept the RV have hindered timely resolution. The RV has not yet undergone a complete inspection by the manufacturer or authorized service center due to these issues, but the concern has been communicated multiple times with relevant parties. Warning symptoms related to the transmission problem have been present since the initial purchase, leading to multiple service attempts and ongoing safety concerns.

2002 Mirada

General tire lmt 400, 225/70r 19.5 two separate incidents on 2002 coachmen rv with ford chassis. First on interstate rear inner dual blowout showing sidewall zipper of approx 6 inches, tire had 13000 miles on july 17, 2003. Second incident on in town roadway blowout of inner dual showing sidewall zipper pf approx 6 inches, tire had 500 miles. Notified general of both incidents, first incident took tire to general dealer stated was not fault of tire but of mouting or underinflation. Tire subsequently replaced by ford dealer. Ford notified with details. Second incident general notified directed to take tire to local ford dealer who when contacted stated dealership did not have facilities for rvs refused to set appointment for review. Based on two incidents with same problem i have serious concern about driving vehicle which has six of these tires until someone can explain what happened.*ak

My wife and i were enroute to columbus oh from newbury ma. Approximately 4 hours into the trip on rt 84 in ludington, n.y. A horrendous bang from the rear. I immediately pulled to the side of the interstate to investigate. I found a 3 1/2 inch hole in the passenger inside rear duel on the outside wall. I limped to the next exit after the highway patrols suggestion to a hess gas station and called good sam's road service. All went well from there. *jb

Tl* the contact owned a 2007 coachmen mirada. The contact stated that with no warning while driving approximately 35 mph, a fire erupted from the rear of the vehicle near the tires, to the inner cab of the mobile home. The driver and four passengers were able to exit the vehicle. A police report was filed and a fire inspection report was completed. There were no injuries reported. The road had to be blocked off while the fire inspector extinguished the flames and the vehicle was destroyed. The insurance company towed the vehicle to a yard. An authorized dealer was not contacted. The insurance company was to inspect the vehicle and the manufacturer was notified of the failure. The contact was waiting on the manufacturer to investigate what caused the failure. The contact was uncertain if the mobile home would be replaced due to the failure. The contact was uncertain if the failure was related to nhtsa campaign number: 08v279000 (equipment) and was not notified of this recall to determine if the mobile home was included or excluded. Tl* the contact owns a 2000 coachmen mirada. While driving and exiting the highway, the contact heard an abnormal noise when the brake pedal was depressed. The brake pedal felt irregular and the contact had to apply force to the brake pedal. While pulling the vehicle over, a fireman alerted the contact to exit the vehicle due to flames erupting from the two front wheels. The rear wheels were not affected by the failure. The fireman and the contact used fire extinguishers to extinguish the flames. A fire report was not provided. The contact called the manufacturer several times, but was unable to speak with anyone. The vehicle was serviced by an independent repair facility. The vehicle was parked and covered after the failure. The vehicle was excluded from nhtsa campaign numbers: 09v193000 (service brakes, hydraulic) and 09v110000 (service brakes, hydraulic), although it exhibited the same failure. The cause of the failure was not diagnosed. The vin was not available. The approximate failure mileage was 43,747.

2001 Mirada

Drove vehicle approximately 25 miles over flat land, no hills or mountains, to dump waste water. When we went to left rear of rv to open sewer access, smelled strong scent of burning brake fluid and could detect heat from wheel well. Allowed brakes ample time to cool down then drove home without incident. Brake smell was diminished when arrived at home. Drive vehicle short distances only three times since then, without incident. *tr

2005 Mirada

Reference: report receipt date: mar 17, 2005 nhtsa campaign number: 05v092000 component(s): trailer hitches i purchased a used coachmen mirada motorhome on april 23, 2013. After picking up the motorhome, i checked for recalls and found that there was one on the "hitch bracket assembly". In contacting the dealer i was informed that the recall had been completed and that i could call coachmen for confirmation. When i called coachmen they stated that they had not talked to anyone at the dealer and could not confirm if my unit had been checked and/or repaired. They stated that forest river rv purchased coachmen rv and they were prohibited to discuss any recalls older then 2009. I am at an impasse as i would like to know if my unit is affected. I have had another rv dealer check it and gave them information copies (see above) of what i could find on the internet. They have tried contacting coachmen and have not had any luck in getting an answer. As this is a safety recall, this second dealer said that i should not tow anything or use the hitch until this problem is resolved, i feel that this needs to be addressed and would appreciate your help. *tr

2011 Mirada

Note: we have since learned this incident has been characterized as "the death wobble" as follows: this occurred on our first "shakedown" (pun intended) trip of 300 miles taken in a new 29 ft coachmen mirada (leftover 2011 model) class a motorhome w/f53 chasis which was manufactured by ford in oct 2008. Its noteworthy that, initially, a slight shutter in coach body could be felt when highway driving over minor bumps or surface discontinuities. After driving 200 miles, we drove at 55 mph over a highway bridge expansion joint - kind of a quick double bump, which set off an extremely violent rapid shaking of the entire coach resulting in difficult steering and a harrowing experience. Thankfully, there was limited traffic, traveling in right lane, and room on shoulder to pull over. This violent shaking continued until < 5 mph. Checked tires and under chasis - all seemed ok. Continued driving home to complete the balance of trip, avoiding as many bumps as possible, and with no further incident. Rv is currently at a ford service shop for inspection and resolution. This was a very unsafe condition, and could easily have resulted in a major traffic incident, especially if there was heavy traffic and limited room to pull over. *tr updated 11/25/13

1999 Mirada

On three separate occasions (on the same trip) i experienced total loss of steering control at 60 mph due to the front wheels of my motor home hitting staggered bumps in the road that set off a harmonic vibration. The front wheels were oscillating from lock to lock (i heard them hitting the stops) with no input from the steering wheel. Braking effect was also severely reduced. I thought the vehicle was going to come apart - the noise was that dramatic! The wheels continued to oscillate until the vehicle slowed to 20 mph. Fortunately the road was straight or i would have left it for sure! I called coachmen (motorhome manufacturer) who referred me to ford (the unit has a ford sd53 chassis). Ford informed me that they had no reported problems with this unit. But they also told me that 1999 was the only year that they had not installed a steering stabilizer. Up to 1998 had one, 2000 and later had one. They were not receptive to my request that they retrofit my unit with this item at their expense and did not agree this was a safety issue! I visited my local dealer who informed me that a stabilizer kit was available for my unit. I paid for the kit which they ordered. Next day i got a call from the dealer to say the kit was no longer available! To the dealer's credit, they found one at another dealership that i went and picked up and installed myself. Has it solved the problem - not sure as i hate to drive the unit any more! Losing control of seven tons of vehicle (plus a trailer) at 60mph on a busy freeway is not something i wish to repeat!*ak
