What's wrong. As a result of an ncap test, certain minivans fail to comply with the requirements of federal motor vehicle safety standard no. 208, "occupant crash protection." the front passenger seat air bag was not correctly wiring.
Risk. In the event of a crash, the air bag will not provide adequate protection.
Fix. Dealers will install a new wiring harness in the front passenger seat air bag system. The recall bagan march 5, 2004. Owners should contact mazda at 1-800-222-5500.

The labels show the maximum tire pressure as 240 kpa/32 psi when it should be shown as 240 kpa/35 psi. As a result of this error, the load ratios for the 205/65r15 94h and the 215/60r16 95h tires exceed the levels permitted by fmvss 120.
Risk. If the tire is operated in an under-inflated condition, the load carrying capacity of the vehicle may be compromised, increasing the risk of a crash.

Risk. It is possible that the broken actuator cable may be out of position and could interfere with the operation of the accelerator cable, preventing a decrease in engine speed while driving, increasing the risk of a crash.

Vehicle description: certain mini vans without anti-lock braking systems (abs) fail to comply with the requirements of federal motor vehicle safety standard no. 135, "passenger car brake systems." due to inappropriate brake pipe layout, these vehicles do not meet the brake stopping distance with partial failure requirements of the standard.
Risk. If partial failure occurred in the braking system, the stopping distance of the vehicle could increase, increasing the risk of a crash.

What's wrong. Vehicle description: minivans. Due to the inappropriate shape of the linkage in the rear door lock controller, accumulated dust may prevent the door lock mechanism from functioning properly, causing incomplete locking of the door when the childproof locking system is used.

What's wrong. The front safety belt buckle release buttons can break. These red plastic release buttons are marked "press." if a button breaks, pieces can fall into the buckle assembly causing the buckle to operate improperly.
Risk. The safety belts would not provide adequate protection to an occupant in a vehicle crash.
Fix. Owners should promptly check the condition and operation of both front safety belt buckles and carefully inspect the red release button for any breaks or cracks. They should ensure that both buckles are operating properly by inserting each latch plate into its buckle, tugging on the belt to make sure the latch is securely locked, and then pressing the release button. The latch plate should pop out of the buckle when the button is pressed. If either release button shows a sign of breaking or cracking or if either buckle fails to operate properly, owners should promptly contact their authorized dealer to schedule an appointment to have the buckle replaced or repaired, free of charge. The manufacturer is developing a remedy designed to prevent failure of the buttons that are not currently broken.

Risk. Inconsistent braking and premature activation of the abssystem can cause sudden stops or increased braking distances, and result inincreased risk of vehicle accident.
Fix. Replace the rear brake assemblies with assemblies whose friction characteristics do not change. This will reduce the risk of premature abs system activation.

On december 20, 2004, i tried to crank my new 2004 rx-8 to go to work. It was 19 degrees outside. The car would not crank. It had been sitting for 2 days over the weekend. Unfortunately, i took it out of gear and did not place it back in. When i got out of the car and went inside, it rolled, hitting a tree causing $3800 dollars worth of damage. The tree was hit very hard and the airbag did not deploy. Apparently, this car "supposedly" was recalled and fixed while still owed by the mazda dealership according to their records. However, after having it towed into the service department, it had not been fixed. I was never told of any of these problems or recalls prior to the purchase of the car. *ak
My daughter, danielle grande, was burned on the leg by the tailpipe of my mazda mpv minivan. Her leg touched the tailpipe because it sticks out past the bottom edge of the back bumper. I called mazda and reported the problem but they refused to fix it or take any action.
2000-2003 mazda mpv models have a coolant seepage from rear heater pipe which can cause severe engine failure and traffic hazard due to non-drivable vehicle. Mazda mpv 2002 was found to have a coolant leak in rear heater pipes and was taken to mazda repair center. Mazda advised a vehicle should not be run without coolant since it can damage the engine, and replaced the lines for a cost of $1000 (parts + labor). Mpv was driven 18 miles after completed repair, then the engine suffered a complete failure in traffic and vehicle stopped in the middle of an intersection, posing a danger to mpv passengers as well as other vehicles. Mazda repair center claims the engine failure is due to driving with low/no coolant prior to repair. Regardless, if the engine failure was due to a failed repair or due to the reason mazda states, this is a known product defect by mazda that should be issued as a recall notice to all affected mpv owners. Mazda has issued a technical service bulletin (us bulletin no: 07-002/05) stating that warranty repair is approved only if the vehicle exhibits coolant leakage and the vehicle is within the extended 7-year warranty from vehicle in-service date. Owner of 1996 mazda mpv, while traveling to ammusement park requiring long lines for parking vehicle got hot. When motor was turned off radiator pressure increased, this pressure while being released safely to the overflow bottle through the connecting hose the overflow bottle was unable to release an equal amount of pressure. This resulted in the overflow bottle top exploding and spraying scalding fluid in the face of a 13 year old boy who was standing next to the open hooded compartment. He suffered 1st & 2nd degree burns to on more than 50% of his face and some on his torso. This is the orginal factory overflow bottle, by closer inspection it appears that the input tube and output vent are different in size, the output port is smaller. This provides for the pressure in the overflow bottle to increase and exceed the snap-on-cap's holding strength. Has this been a problem in the past?*ak

On may 13, 2004, my 33-week pregnant wife pulled into my driveway with our daughter sleeping in back of our 2003 mazda mpv. My wife put the car in park, engaged the emergency brake, and left the car running. Pse&g workers requested that my wife talk with them. While she did so, the car remained stationary for 5-10 minutes. My wife then attempted to open the rear automatic door. It beeped instead. My wife opened the front door and pushed the button there to open the rear door. It beeped again. My wife checked that the gear shift was in park. It was. The car then started to roll backwards. Witnesses say they heard a ratcheting noise when the car started rolling. My wife pulled up the emergency brake to make sure that it was fully engaged. This did not stop the car. My wife was knocked to the ground and the door caught my wifes abdomen and continued to roll over her, scraping her as she tried to protect our baby. The car rolled into the street with our daughter in it. The car hit the curb and just missed being hit by a car. The rear door then popped open. A pse&g worker turned off the car and stated that the dashboard indicated that the mpv was still in park when he did so. On may 14, our mazda dealer ran diagnostic tests and advised us that it was unable to replicate the accident. Mazda took the position that nothing was wrong with the car. Mazda suggested that it was possible my wife failed to put the car fully in park. I advised mazda that this made no sense because the car was stationary in our driveway for 5-10 minutes and it was still in park when it stopped. The notion that the mpv slipped into reverse and then back into park makes no sense. Mazda has given us no adequate explanation for why this accident occurred. There are multiple complaints about 2003 mazda mpvs on the nhtsa's website about their transmissions, including down shifting between gears and gear slippage. *ak

While driving my mazda 2000 protege(automatic) home from friends on july 4 th, noticed that there was a big jerk from transmission and engine started rewing at ~4k to ~5k and the car was loosing power. I was doing 55 mph at the time and the speed came down to 40 mph. From that point on car would not go beyond 40 mph and engine still keeps rewing at that speed. I was on a highway in the car alone and was feared for life that the engine rewing with loud noice would blow on me. Had to take local roads to make it to home. The car has 37k miles on it and was gently driven. All the required maintenance work was performed. Dealer after performing the diagnostics came back with there was an internal failure with the transmission and it needs to be replaced costing over $2600.

Iwas involved in a rather serious accident in which i collided with a buick century that ran a stop sign. I was traveling 30-35 and my front end collided with the drivers side of the other car. The buick was totalled. My car was severely damaged in the front. The airbags failed to deploy. According to the owners manual they should have activated at a speed greater than 14 mph. *ak
I had an accident the 01/18/05 in mazda mpv 2001 van, the insurance company declare total lost of the car, and my complaint is because the air bags did not work, never came out, that`s why me and my family are in therapy for neck and back, i asking for help, because i called the mazda company and they do not give me any solutions, this is my first accident and, i don`t know what i have to do. *jb
Tl* - the contact was driving a mazda mpv with an odometer reading of 75000 miles, in foggy conditions on a two lane highway, her vehicle was hit from behind causing her to hit the vehicle in front of her. The contact stated that the traffic had stopped, and she was slowing down. At the time of the impact, the vehicle was traveling at approximately 20 mph. The second vehicle behind her did not stop and struck her vehicle at approximately 40 mph, according to the police. The airbags did not deploy. I am filing a complaint in regard to my vehicle's equipment that failed and was faulty in a recent automobile accident. I was traveling approximately 40-45 mph in a straight lane when a vehicle quickly entered across my path to make a left hand turn. I had no time to brake, so the other car is what stopped my motion. It was a hard hit! My mazda mpv crashed into the other person's car. During our front impact with the other car's driver side, my airbags did not deploy causing injury. Had the airbag worked, i feel i wouldn't had slid into the dash and been jerked backwards by my seat belt restraint causing injury to my back and or spinal alignment. I have been suffering every day from the accident due to this injury. In addition to the chest/lap belt being used in the passenger seat, i secured my daughter's chest during the crash with my right arm, which i believe helped her to not attain even more injury from the air bag not deploying. She was, however,injured on her leg from the storage compartment under her seat releasing on it's own from the crash and hitting her legs causing a cut that was treated at the e.r. The storage compartment should not be able to release in a wreck. I believe you will consider this problem to also be faulty in nature. I urge you to investigate these problems so that other people may be notified with this particular vehicle. I ask that you and/or mazda investigate the defects with my vehicle very soon before i have to sign the title over to the driver's insurance that was at fault. My vehicle has been declared "totaled" on 8/14 by usaa and is at a salvage yard. Please contact me soon to let me know what you plan to do about the situation. I called mazda today at 800-222-5500, was on hold for a little over 15 minutes to report this with them and ask them who made the air bags, sensor and compartment under the passenger seat. We were broadsided by a vehicle that ran a red light at full speed. The impact was on the passenger side. The car flipped over and skidded onthe passenger side then flipped back onto the drivers side and skidded before rolling over at least once. Not one air bag deployed. Thank god for the seat belts. They saved our lives. Injuries were concussions, sprained neck, injuries to lower neck, back and shoulder of passenger. The 2000 mazda mpv lx with 51,300 miles spontaneously caught on fire ,and the front half of the car was incinerated on may 11, 2007. I had driven the car approximately 15 miles back to my house, when i went inside to get a drink of water and came back out there was a burning, viscous substance dripping from underneath the engine. The engine quickly burst into flames, sending billows of smoke toward the front of my house. The fire department came and put out the flames, but not before the car was totaled. On september 26, my wife had an accident while entering in our home drive way, with the brake pedal depressed, the vehicle continued to accelerate and struck the garage door and crashed against the internal wall that divide the garage from the kitchen. We did not reported the accident to the police or the insurance to avoid further increase in the quotes insurance but our neighbors saw the accident. We had to hire a specialized door company to replace the garage door and a handyman to fix the garage side and the kitchen side. The estimated cost for the car repair is $2,000.00, new garage door $1,175.00 and the wall $250. A few days after we both this car, i had the same problem at the auto parts parking lot without consequences, just the terrible moment in what you feel that you can't stop your car. The liftgate damper pivot bolt broke, causing the liftgate to fall on my head. It only fell a couple of inches because of my height. I see there is a recall in canada for this exact same issue. I called mazda - they stated my car was not covered by the recall. I saved the old part. I replaced the 2 dampers and bolts - about 160.00 in parts. Below is the canadian recall no: 2905l, date : 04/2005, model: mazda mpv 2000-2004. Subject: liftgate stay dampers and stud bolt replacement. Summary: note: this recall is applicable to canadian vehicles only. On certain vehicles, the liftgate stay dampers and stud bolts may rust and stick together, causing the stud bolts to break off. In the worst case, this condition may allow the liftgate to suddenly drop during opening or closing, injuring the operator. Correction: dealers will replace the left and right dampers and stud bolts. *ak

This is unacceptable. The part is inadequately designed. It is a dangerous hazard and should be addressed before someone looses their life!!!!!!!
