Risk. A fuel leak in the presence of an ignition source increases the risk of a fire.

Power Train:automatic Transmission:gear Position Indication (prndl)
What's wrong. Mazda North American Operations (Mazda) is recalling certain model year 2008 Tribute vehicles manufactured February 9, 2007 to April 22, 2008. The affected vehicles have had their CD4E transmission replaced with remanufactured one that was rebuilt between June 11, 2015 through July 15, 2015. These service repair part transmissions have a shift lever bolt that may not be properly tightened. As a result, the shift control lever could disengage from the transmission without warning.
Risk. If the shift lever disengages from the transmission, a driver may be unable to shift gear positions and the indicated shift position may not represent the gear position the vehicle is in. Should a disengagement occur while the vehicle is being driven, when the driver goes to stop and park the vehicle, the gear selection indicator may show that the transmission is in the "PARK" position, but the vehicle transmission may not actually be in the "PARK" gear position. If the vehicle is not in the "PARK" position there is a risk the vehicle will roll away as the driver and other occupants exit the vehicle or anytime thereafter. A vehicle rollaway increases the risk of injury to exiting occupants and bystanders.

What's wrong. On certain trucks and sport utility vehicles, the windshield wiper motor may have been produced without grease applied to the output shaft gear.
Risk. After a period of continuous use on the high speed setting, lack of grease on the output shaft gear may cause the gear teeth to distort or fracture during operation resulting in the loss of wiper function. Inoperative wipers under inclement weather conditions could cause a crash due to impaired visibility.

What's wrong. Vehicle description: sport utility vehicles. The windshield wiper linkage can disengage due to a broken ball socket.
Risk. This condition will likely result in sudden and complete loss of the windshield wiper function, or of the wiper function on the driver side only, increasing the risk of a crash.

Seat Belts:front:buckle Assembly
What's wrong. On certain sport utility vehicles, a post within the safety belt buckle covers could fracture, which could affect latch function, resulting in either a "no latch" or "partial latch" condition. In the "partial latch" condition, the buckle tongue can be inserted into the buckle and appear to be engaged, but will release from the buckle without the push button being depressed when a relatively low load is applied, such as when an occupant moves slightly in the seat.
Risk. In the event of a crash, the seat occupant may not be properly restrained, increasing the risk of injury.
Fix. Dealers will have the driver and front passenger safety belt buckle cover removed and replaced with a newly designed service buckle cover. Owner notification began february 5, 2004. Owners should contact mazda at 1-800-222-5500.

What's wrong. Vehicle description: some 4x2 sport utility vehicles not equipped with an antilock braking system may have inadvertently been built with 4x4 rear hubs instead of 4x2 hubs. If the incorrect hubs are installed on a 4x2 vehicle, the rear wheels can loosen, or in some cases, separate from the vehicle.
Risk. This condition could cause loss of control of the vehicle, increasing the risk of a crash.
Fix. Dealers will inspect the rear hubs and, if necessary, replace them.
Steering: Steering Wheel/handle Bar
What's wrong. Vehicle description: sport utility vehicles. These vehicles may have a mis-assembled or incorrectly manufactured nut that attaches the steering wheel assembly to the steering column. The nut could be upside down in the steering wheel assembly, which could overstress the steering column shaft when the nut is tightened. In addition, some of the nuts may have been cracked during manufacture resulting in the nut fracturing when it is tightened during vehicle assembly.
Risk. Either of these conditions can lead to the steering wheel becoming loose or separating from the steering column, resulting in loss of steering control.
Fix. Dealers will inspect and replace the steering wheel retention armature and, if necessary, the steering column to correct this condition.

What's wrong. Vehicle description: sport utility vehicles equipped with speed control. The speed control cable can have a cracked or missing speed control cable servo cap locating tab. The locating tab could enter the servo clutch cavity and interfere with the speed control servo clutch. Also, water could enter those units that contain a cracked or missing servo cap locating tab, eventually causing corrosion that could interfere with the function of the speed control.
Risk. If either of these conditions occurs and the speed control is used, the speed control could prevent the throttle from returning to idle, resulting in a stuck throttle. A stuck throttle could result in a crash.
Fix. Dealers will inspect the speed control cables. Vehicles with speed control cables from cavities 3 and 4 will have the cable the speed control servo replaced.
Fuel System, Gasoline:delivery
What's wrong. Vehicle description: sport utility vehicles. The o-ring seals in the fuel line connector at the outlet end of the fuel filter were damaged when the line was connected to the filter during vehicle assembly. In some cases, this damage could result in a fuel odor, or possibly in fuel dripping from the connection.
Risk. Fuel leakage, in the presence of an ignition source, could result in a fire.
Fix. Dealers will inspect and, if necessary, replace the fuel line.

While traveling at 40 mph, the contact changed lanes to pass a vehicle but when he depressed the accelerator pedal to speed up, the pedal went to the floorboard and the vehicle stalled. 2004 mazda tribute, 6 cylinder. The engine exploded upon starting, specifically the intake manifold. Upon starting, an extremely loud explosion occurred, the hood was raised about an inch and smoke and flame poured out from under the hood. I exited the vehicle and the smoked subsided and plastic parts where scattered about the parking lot. Towed the tribute to the dealer where they stated that the intake manifold blew up and it would cost over $1000 to repair with no guarantee that the engine is not damaged. On december 20, 2004, i tried to crank my new 2004 rx-8 to go to work. It was 19 degrees outside. The car would not crank. It had been sitting for 2 days over the weekend. Unfortunately, i took it out of gear and did not place it back in. When i got out of the car and went inside, it rolled, hitting a tree causing $3800 dollars worth of damage. Wife drove 2003 mazda tribute to arby's (i was meeting her there), got out of car and walked up to the entrance. As she got to arby's door, someone outside mentioned to her that her car was smoking. I started my car and the 'check engine' light flashed on-and-off. Thinking it would go away, i drove to route 3 in south shore, ma. Once on the highway, the light did not stop flashing, so i pulled over. Email referral from center for auto safety on behalf of consumer re: nhtsa recall 12v357 / pe12-09. Referral associated an april 2011 incident with the recall and included a police accident report and two photos *tgw according to the narrative from the police report, the driver of the vehicle stated she was traveling north on route 35, when the brakes on the vehicle stopped working, which caused her to lose control of the vehicle at an intersection. She attempted to make a right run from route 35 onto a bridge, but the vehicle crossed all three lanes of traffic and crashed through a guardrail and landed against a tree. The vehicle then became engulfed in flames.

When approaching a 4-way intersection during daylight hours, the light turned yellow then red. I tried to stop by pressing my brake pedal. My vehicle did not slow down, but continued to accelerate up hill. I pressed as hard as possible but my vehicle went through the intersection and i ran into the side of another vehicle. I was originally going about 30mph and when i hit the other vehicle, i was going about 25mph. My vehicle stopped after hitting the other vehicle but continued to rev higher. I was injured when my arms bent the steering wheel forward, the seat belt bruised me and my right foot (talus bone) was broken by the pressure on the brake pedal. I required surgery. I was out of work for 3 weeks and wore a cast for 7 weeks. The policeman who reported to the accident site discovered the gas pedal was half way down to the floor and stuck. Incident occurred on 10/20/2012 at 7:00 pm in the fred meyer parking lot in klamath falls, oregon. Temperature out 40 degrees. Tailgate window shattered in parking lot on my 2010 mazda tribute. I came out of fred meyer. I opened the tailgate to load a few items. I shut the tailgate and the back window exploded. Shattered glass flew over 10 feet in all directions, landed on my feet, fell into the back of the vehicle. Glass pieces were tiny shards that were very sharp. I had numerous cuts on my hands trying to pick up the large pieces. 2001 mazda tribute. Consumer writes in regards to the vehicle subframe corrosion recall notice. *smd the consumer received a recall notice in early june. However, she had already paid for the repairs. So, she submitted a request for reimbursement, along with a synopsis of the trouble she experienced. The consumer received a letter of denial from mazda, stating the parts that were replaced or service completed, were not considered covered items under the subframe corrosion recall. On december 26, 2012 while driving on the highway, the consumer noticed a decreased control over the steering. The vehicle fish-tailed multiple times in light snow, which had never occurred before. Due to her steering difficulties, she drove at 30 mph, yet still spun out for no apparent reason. No one was injured and she only hit a median which caused damage to the rear passenger side tail light and body damage. At the time, she attributed the incident to winter weather conditions. However, it made far more sense in the context of the recall. In january 2013, she continued to experience steering problems. She attempted to get an alignment, but ran into problems due to the subframe; which was the first time, she heard of trouble with the subframe. According to invoice, the caster was not adjustable without the subframe adjustment and possibly more. It was 0.3 degrees from acceptable range, may demonstrate a slight lead or pull. The consumer experienced problems for the next two years. The ball joint and right control arm were replaced. The mechanic informed the consumer the control arm looked as if it was ready to break off, due to corrosion. Also, the left front control arm, along with the struts, rotors and brake hoses, were all replaced. *jb

Car is no longer under warranty and im scared they might overcharge me. $100.0 just to check it. Took it to my mechanic and he says that the ignition switch is bad. This car is laced with problems please advise ford to recall this suv. I have a 2008 mazda tribute with 13,000 miles on it. Last week i noticed the right rear wheel was tilted out or in technical terms it had extreme negative camber. I got underneath the vehicle and saw that the lower ball joint had come out of the control arm. The lower control arm is designed for the ball joint to fit in a stamped steel sleeve then bolt tight to the rear spindle once the ball joint is dislodged from the control arm the integrity of the ball joint and the control arm are compromised. I called the dealer and explained what had happened and he told me to bring it in right away for fear that the rear wheel could collapse under the vehicle. I'm well aware of what could happen but i'm more concerned that a warranty repair would not solve the cause of the problem. I'm also concerned the problem would die at the dealership and not be brought to the proper personnel attention. I'm going to take pictures of the ball joint and control arm before i take it in for the repair. I really don't feel safe driving the vehicle now that i know what could possibly happen. I can't believe that this has never happened to any other tribute or escape. I don't plan on driving this vehicle until i receive a return response from your department. *tr

My wife was in the parking lot at church waiting for a car to pull away so she could park. The car started to roll forward so she stepped harder on the brake. The car lurched ahead crashing into the car in front of her. My wife is quite sure she had her foot on the brake. As she told me about this i started remembering an event. This car had belonged to my father who has since passed away. He purchased it new. I'm not exactly sure of the date, but i believe it was in the first year of ownership. He was parking at a hospital when all of a sudden the vehicle was taking off and pushing the car in front of him. I figured he was having a senior moment and had his foot on the wrong pedal. I did some quick research today and found that others have had this problem. Maybe my father was doing all the right things and it really was the cars fault. I also see that there have been recalls on this matter. I'm not sure if any of these issues were addressed when my father had the car, but i will be checking it out. I hope you will be continuing to follow up on this. This is a scary and dangerous issue that deserves your attention. 2 complaints: i took the car to a mazda dealer 5 times starting back in 2003. 3 times for recalls ,and on each occasion had brought to the attention of the service representative e that the rpms would just all of a sudden go up rapidly, and the only why to stop it was shut it off. Of course this was always while waiting at lights when i would first start the car in the morning but now was at least one week. They put it on the computer back when it first started and said it was the pcm modular. T was also replaced along with the first recall . The car was okay for about 6 months and then started doing it again the same thing, the rpms rapidly came up- this time was already a recall so they fixed it again with another new pcm, but once again it did the same thing after about 8 months. I took it back again with another recall on a different matter, seat belt assembled, and told them the same thing was happening. By this time the warranty was out but because it had been in before for this they were able to get the mazda to cover the part again, $1200. The problem started again and i could not afford to replace this parts again. I didn't think it was diagnosed correctly from the beginning. Now it stalled without warning while i was driving the steering and everything locked because of loss of power this happened on a regular bases. . I have held off on complaining, but this has gotten out of control. I thought maybe i just got a lemon, then i read the other compalints of the same things. I am a single mom and need this fixed for good. I didn't blame the dealer for the car troubles i blamed mazda for a poorly made suv. First occurrence was while backing out of a parking spot and put the vehicle into drive. I accelerated and the car hesitated, chugged, the wrench light on dash illuminated with very slow forward momentum. Turned vehicle off and back on with no problem until the end of the parking row when i stopped for traffic, it chugged and wouldn't accelerate, wrench indicator on again. Turned vehicle off and coasted to a parking spot. Restarted a few minutes later with no problems for almost a week. Next time it happened i accelerated from a stop sign and only had crawling speed no matter what i did with the gas pedal. The wrench light was again on with the vehicle chugging and hesitating. Due to parked vehicles i had to drive almost the length of the block in this condition before having a place to pull over and move out of the long line of traffic i had created. Referral associated an april 2011 incident with the recall and included a police accident report and two photos *tgw according to the narrative from the police report, the driver of the vehicle stated she was traveling north on route 35, when the brakes on the vehicle stopped working, which caused her to lose control of the vehicle at an intersection. She attempted to make a right run from route 35 onto a bridge, but the vehicle crossed all three lanes of traffic and crashed through a guardrail and landed against a tree. The vehicle then became engulfed in flames.

On several occasions my vehicle has had a sticky gas pedal. I alerted the dealership in the past advising of this same issue regarding my prior 2003 mazda tribute vehicle. My car is now at the dealership and i have been advised there are no recalls and i will be charged$357 to have a ring put on the left wheel causing a malfunction in the braking system (abs light). I purchased a 2003 mazda tribute on june 26, 2008. The car was wrecked today 7/1/08. The brakes failed to operate and ran into the back of a van in front of us. They verified that they were checked and found in good condition. On june 8, 2010 i was turning into a residential area and the car would not stop or slow down. I crashed into a median. Earlier that day i experienced a similar problem while braking but i thought it was due to the rain. When the accident occurred it had just stopped drizzling and the ground was visibly dry. I purchased the car new in 2004. I have been complaining about the brakes losing pressure since i purchased my car but the dealership always told me they were fine. I have always had to push my brakes to the floor to stop the vehicle. Sometimes when at a red light i'd sit and pump air into the brakes so they would stop properly. Yesterday i was driving in stop and go traffic and a lady stopped ahead of me but my car would not stop. I had adequate distance to stop. I had the brake pedal to the floor and it felt as if the tires were stuttering/hopping in an attempt to stop. I pressed so hard that my whole leg was in a cramp. I hit the car ahead of me damaging the bumper. I did not slide, the tires are semi-new and in great condition. Also, my rotors failed a few years ago. Had to have them replaced all round as well as another part of the braking system. I have also had the grinding noise and the air hissing from the brakes. This has been an issue for a long time and i've asked mazda about recalls and their haven't been any. I bought the car used several years ago and in the past year have experienced the steering wheel all of a sudden appear to be locking up. I looked up the recall and found that this is a known issue with the Mazda Tribute vehicles. I checked with dealer and they agreed. I contact Mazda customer service on 3 different occasions in the last month and they have issued a case number but will not acknowledge that my vehicle is a covered VIN to be fixed. They said its not part of the recall. It is and I have asked them to look into it. When I drive and pull into a parking lot such as a Starbucks and have to make a turn off a main road the vehicle steering locks up and the recall is 7614E Mazda acknowledges that the recall is correct in which the power steering torque sensor fails but refuses to cover my specific Mazda Tribute which is 2011. This range is covered and they said they wont cover it. I need the NHTSA to investigate this and contact Mazda to enforce it being covered under the recall. When this happens I am fearful that I will not be able to turn and hit a car, a person, or a building because it becomes so hard to turn. Only remedy is to shut car off and wait 20 minutes then turn back on. Sadly Mazda keeps tell me they wont cover it.

I was driving my vehicle on a city street, and the steering suddenly began to lock up. The vehicle was in motion when this occurred. I was not able to turn or control the steering wheel; it became extremely hard. I was not on my brakes, i was driving straight with my feet on the gas. This happened twice in the last 4 days. I brought the vehicle to the mazda dealership and they said the steering wheel column needed to be replaced, the torque sensor, and calibration. I read the 2011 mazda and ford escapes that was built in the kansas city factory were recalled for this same exact issue in 2014 which i did not own the vehicle at that time.
Two years ago my vehicle started loosing electrical power steering.. there is a recall for this issue… my vehicle was not experiencing this issue at the time of the recall. I have not been able to drive my vehicle because it randomly looses power steering deeming it extremely dangerous to drive.. I informed mazda who stated there system shows it was repaired during the recall will not cover the cost, but this is not true. I’m the only owner and I can’t drive it due to this issue

Just bought a 2003 mazda tribute from toyota of nicholasville 3 weeks ago. Went out to the garage to start it this morning and the thing caught fire. Put it out with fire extinguisher. The air filter area is burned and melted. My wife no longer feels safe driving this vehicle(isn't repaired yet), with our young child strapped in the car seat. I sustained small burns to my hands putting the fire out. *jb
Nhtsa recall 04v583000 on the accelerator cable. Approximately two years after having the accelerator cable replaced the problem recurred. While driving 2 mph, the vehicle accelerated uncontrollably. The driver applied the brake pedal and the vehicle continued to accelerate. The driver was not able to maintain control of the vehicle and collided into a light pole. This caused the driver to sustain minor injuries and was taken to the hospital. Upon inspection, the contact observed the front driver's seat in flames. The fire department was notified of the fire; however, the contact extinguished the flames. The vehicle was not towed. The dealer and manufacturer were not notified of the failure. There were no injuries reported. The failure mileage was approximately 187,000.

The fire department was notified and extinguished the fire. The fire department filed a report and stated that the fire was mechanical and confirmed that the vehicle had been on fire for a least an hour. The vehicle was destroyed. There were no injuries. The manufacturer was notified of the failure. The vehicle was not inspected. The failure mileage was 147,000.
My 2003 mazda tribute was recalled in may 2007 for abs voluntary safety recall. I took it in that same month and the dealer determined module connector did not need to be replaced. On march 15, 2012 abs light came on for no reason and stayed on for an hour. Took it to have it checked on march 17, code could not be determined. Upon inspection everything looked fine. I was instructed to take it to the dealership should the abs light come on again. Two days later, on march 19, 2012 i had been at work for an hour with my car parked when i was notified that my car was on fire. The car was totaled due to the fire. According to the recall of may 2007, an electrical short could cause an abs malfunction which would illuminate the abs light and could result in "burning odor, smoke, and/or fire. This could occur either when the vehicle ignition switch is in the off position or while the vehicle is being operated." my car was off when this occurred. I believe that had the module connector been replaced in 2007 when i took it in under the safety recall, this fire wouldn't have occurred. *js

My son has driven this vehicle since 2008, when we purchased it for him to use while in college. He was driving onto campus and when approaching the parking lot, he noticed abs and airbag lights coming on and off on the dashboard. As he parked he realized there was smoke coming from under the hood. He opened the hood, saw a fire, and called 911 immediately. They arrived quickly and successfully put the fire out. Firemen told him it appeared to have been electrical and he was lucky they got there as quick as they did. Insurance claim adjuster took it to a ford shop (closest) where they had it for a week and said they couldn't get parts, but were aware of a recall coming on the area the fire had occurred in (the wiring harness and abs module). It was then sent to a mazda dealer, that had it for 10 days, and finally said the needed parts were obsolete and they could not repair the vehicle. Claims adjuster checked with another ford and another mazda dealer, getting the same answers from both. By the way, when they checked on the vin, they said it was not on any mazda recalls. This was at the end of january/beginning of february 2012. So after all this, they had to total a vehicle that only had about 77,000 miles on it and was in excellent condition otherwise. The day i get home from having documents notarized for the insurance company, in my mailbox is a recall from mazda....this very one 6712a, which i had already found online days prior to the mazda dealers denying our vin was included. The recall letter states the work from a prior recall in 2007 regarding this issue had been done. We are very grateful this fire did not cause an accident or more serious emergency situation. And we are grateful our insurance company isn't holding things up due to mazda's lack of help. *tr
I started my car and the 'check engine' light flashed on-and-off. Thinking it would go away, i drove to route 3 in south shore, ma. Once on the highway, the light did not stop flashing, so i pulled over. Thirty seconds after pulling over in a rest area, white smoke appeared from under the hood. 10 seconds later, black smoke came from the hood which was accompanied with a burning smell. Luckily, someone from massdot was in the rest area and we determined that it was a car fire. We proceeded to step back and call 911 from a safe distance.
