The Lexus TX 500h is equipped with a 6-speed sequential-shift automatic transmission, which is paired with the Lexus DIRECT4 all-wheel drive system. This setup utilizes a 2.4-liter turbocharged hybrid powertrain to deliver 366 horsepower and 406 lb-ft of torque. During many occasions, the transmission on the 2026 Lexus TX500h would shudder during acceleration or deacceleration. I noticed it happening around 25-40mph mostly and it has happened more than once. I have demonstrated the issue to the dealer on Wed, May 20th around 10:15am Pacific but they didn't know how to address the issue. I did a ride along with the service tech from Lexus of Pleasanton and it happened again during acceleration. The dealer noted that there were no error code when they scanned the system and no observed leaks. I have a video clip but was unable to upload it. The issue with transmission shift thump when this issue happen feels like the transmission locked up. It would shake the vehicle and lose power while accelerating before it starts to gain power back in 1-2 seconds. This causes a safety issue since that pause in acceleration thus this can be dangerous while merging onto a freeway or areas with lots of traffic. I noticed the same issue on some occasions, it would shake during deceleration as well. This issue is intermittent. Maybe Lexus/Toyota can address this with the transmission shifting update. The contact owns a 2022 Lexus UX200. The contact stated that the check engine warning light was illuminated. While driving approximately 30 MPH, the vehicle stalled. While accelerating from a stop, the vehicle stalled. The contact stated that the transmission was slipping while driving at various speeds. The driver restarted the vehicle, waited an hour, and the vehicle restarted. The contact stated that the warning light disappeared. The vehicle was taken to the local dealer, who was unable to duplicate the failure. The vehicle was not diagnosed or repaired. The vehicle was taken to an independent mechanic, who informed her about a transmission code and that the vehicle was stuck in the off position. The vehicle was taken to the local dealer, who informed the contact about the DTC codes that were found. The mechanic diagnosed the vehicle and located debris in the transmission and recommended that the transmission be replaced. The vehicle was not repaired. The manufacturer was notified of the failure and offered assistance for the repair. The approximate failure mileage was 107, 000

1. I was involved in a car accident on Aug 28, 2024 when I was about to go around a curve in the road. A vehicle coming at breakneck speed and well over the posted speed limit, lost control, went over the double yellow lines and crashed into the left driver side of my Lexus. I was the driver and my left knee got crushed against the dashboard. None of my airbags, including my knee airbag deployed. My car was totaled but it is still available for inspection and held at the insurance company. The other driver was test driving a Mercedes and all their aibags deployed. 2. I suffered a communited patella fracture where my knee cap got fractured into several pieces, was taken by ambulance and had to have surgery the next day. The driver and the passenger in the other car were unscathed. 3. No. Until about two weeks ago I was under the impression that my 2015 Lexus was too old to include knee airbags. We recently found out that my car did indeed have knee airbags which did not deploy. For that matter none of the airbags deployed. 4. •My car was towed initially to the police tow lot and then to a car repair place. The car repair place along with a representative from our insurance company together decided that my car is damaged beyond repair. On 7/26/2022 approx 8:00 am I was driving on brighton clopton rd when this flat bed wrecker was backing out on highway and I was looking down to make sure my speedometer was at 30 mph all of a sudden I crashed into this bed of the wrecker, (not one) airbag deployed my knees, shins, chest bones , hurt like crazy , head, I was taken to hospital for evaluation, I attach some pictures from my crash , the officer had me to move out of drivers seat just in case the airbags did finally go off. I was in a vehicle accident on March 30. A accident that could have been avoided if the horn on the IS350 2021 was louder. The vehicle that collided into my vehicle was a Dodge Ram that was hauling a travel trailer which was what hit me. I tried honking various times to let the driver know that I was present and that they were merging onto my lane and I believe due to the weak sound of the horns installed in these vehicles the driver of the other car did not hear my warnings and collided into me
The driver-side front door window developed a crack suddenly while the vehicle was in normal use. There was no known impact, and no object was seen or heard striking the glass. The crack originated near the edge of the window close to the side mirror and spread across the glass. There were no warning messages, alerts, or prior symptoms before the failure. The failure appeared spontaneous. This condition poses a safety concern because the integrity of the side window is compromised. The crack continued to spread, which could reduce occupant protection in the event of a side impact and may lead to sudden shattering. It also has the potential to affect driver visibility. The issue was not inspected by the dealership prior to repair. The dealership advised that they could not perform the repair in-house and directed the repair to a third-party glass facility, where the window was replaced. No definitive impact point was identified prior to replacement. Based on the fracture pattern and lack of impact evidence, the failure appears consistent with a stress-related crack rather than road debris damage. Additionally, there are multiple reports from other owners describing similar cracks occurring in the same location on this vehicle model, suggesting a possible defect trend. The contact stated while at a stop sign, the brake pedal was depressed; however, the brakes failed to engage and the vehicle unintentionally accelerated which caused the front driver's side of the vehicle to crash into another vehicle. There was severe cosmetic damage however, no injuries was sustained. The air bags was deployed. No warning light was illuminated. The vehicle was towed to an independent mechanic where it was diagnosed that the vehicle was totaled. A police report was filed. The manufacturer was not notified of the failure. The failure mileage was approximately 131,000.
The ABS system failed. Mechanic stated it was the ABS actuator. Left front wheel locks up when brakes are applied with force. Safety is jeopardized since the ABS system no longer works. Cannot bring vehicle to safe stop in an emergency braking situation. The problem was reproduced by an independent service center. The vehicle had not yet been inspected by the manufacturer, police, or insurance. Lexus has been notified and said that I just missed the window on the service bulletin, which ended in March of 2024. The brake pads installed by Lexus squeak as if they need to be replaced. According to the Lexus dealer there is a service bulletin, and the fix is to replace the bad pads. I was told the brakes are fine and that the squeaking doesn't mean anything

My car was hit from the back by another vehicle. After the collision, I parked the vehicle and turned the ignition switch off, before getting out of the vehicle, the doors automatically get locked and the vehicle does not allowed to open the door even with the key card inside. I had to open the car with the manual handle from the inside. After I went out of the car, immediately the car again was automatically locked with the key card inside. I tried to open the car several times, and even with the phone application, however its remains locked. The car never opened, so I have to use an external hard ket to get access to the inside. I am very concerned about the lock system of the vehicle because someone can get stuck inside with this system failure and if there is a fire or something, this could be a very serious problem. I have kids and pets and now I don't feel safe driving this vehicle again thinking about this behavior during an accident

She attempted to start the vehicle a third time and was successful. She drove the vehicle to her destination and when she parked the vehicle, the engine would not turn off. The contact smelled a burning odor coming from the engine. Five minutes later, the vehicle shut off. She inspected the vehicle under the hood and noticed burnt wires. The vehicle was towed to a local repair shop and the mechanic stated that there was an electrical short circuit with the starter motor, which caused the under hood fire. The manufacturer and dealer stated that her VIN was not included in any recalls; therefore, they were not liable for the repairs. The current and failure mileages were 80,000
On Juky 7, 2026 @ 10:30pm a catastrophic safety failure occurred involving a 2026 Lexus TX 350. The vehicle experienced a complete electrical and software failure while an occupant (my mother) was inside. All electronic door release buttons failed simultaneously, completely locking the occupant inside the vehicle. The emergency manual double-pull override mechanism on the interior door handles failed to mechanically release or open any of the doors. Concurrently, the climate control system locked on high heat, causing the cabin temperature to rise rapidly. Due to the total entrapment and extreme heat, the occupant began losing consciousness, and her blood pressure spiked to a dangerous level of 188/100. Family members outside the vehicle were unable to open the doors or bypass the electronic locks from the exterior. Emergency services were called to the scene. The local Fire Department had to perform an emergency extraction to rescue the occupant from the vehicle. The occupant required immediate medical attention for severe heat stress and a hypertensive crisis

When I start my car, every once in a while it acts as if it is not getting enough gas and lags. In the colder months it seems to do this more often and sometimes will not start. When the weather is warmer it will sporadically do this. I have put a new battery in thinking it was weak and this could be the cause for it to not want to start, but it has done this at least 5 times within the month that the battery was replaced. My VIN is not showing a recall for the fuel pump like the other 2016 IS200t models, but I’m afraid that this is the recall problem. What should I do if my mechanic says it is the fuel pump? Will it get replaced under the recall?

On Saturday 3/14/26, while driving on Florida's Turnpike at highway speed, without interior dashboard warning lamps or other warning symptoms both rear tires failed simultaneously & disintegrated creating a potentially catastrophic driving hazard for those with me inside the car as well as others on the Turnpike. This type of failure occurring at the same time on both rear tires with relatively low mileage of 27,680 miles raises concern about a potential defect. The rear tires are the original factory tires. I do have photos + video evidence of the damaged two rear tires which were replaced on Monday, 3/16. I no longer have the damaged tires, however it may be an option to speak with Tire Kingdom. I have attempted resolution immediately following the incident from Lexus of West Kendall as well as Bridgestone. The dealership leadership advised "I should be happy those tires gave you almost 30,000 miles since these OEM tires should be replaced every 15,000 miles." He replaces his own tires every two years. The service manager stated the problem was that I drove over too many potholes & curbs without having my car aligned. He also said he lives two miles from his job & dodges 15 potholes daily going to work. I have already met in person with the service manager & have had multiple conversations & email exchanges with dealership management including Bridgestone. Tire size is a P275 / 70 / R18 On 10/11/2025 I experienced a belt separation on my driver side rear tire which resulted in a noticeable bulge in the tread area. I had similar issues with two other matching tires in this set and one of them blew out leading to a rapid loss of air. I am sure if I wouldn't have been hyper-aware of early tell tale signs of this potential issue from the previous incidents, this tire may have also blown out on me. This is three out of the five General ATP tires in this set of five that have failed from the same belt separation issue
225/50/18 Tires have less that 2000 miles on them and they are pealing at the treads. Literally whole chunks are falling off the tires…looks like someone has been picking and pulling off pieces of my tires

The panoramic sunroof shattered while the car was parked and there were no occupants. Lexus USA refuses to acknowledge this defect and the dealership insists there was an impact that caused this damage. There was no impact. I have asked the dealer for a written report of their determination of the point of impact and to return all broken glass to me. I do not know yet if they will comply with either request. As the car was parked, there were no immediate safety concerns, but due to the risk of additional glass falling onto the driver and passenger seats, the car was not in a drivable condition. Had this happened while driving, the sound of the shattering and the glass falling may well have resulted in a reaction from me causing immediate danger to myself and others on the road. UNKNOWN if problem has been reproduced, but this is not an isolated incident. Vehicle component had not been inspected by manufacturer, police , insurance reps or others. I purchased my vehicle 2026 Lexus tx350 from Lexus of Manchester located in [XXX]. As soon as I left the lot I noticed the hood would shake violently and almost pop up which signaled the safety latch alarm. The alarm would signal multiple times throughout a highway trip. Most noticeable around 70 mph. The dealership was helpful and set up an autobody repair where they added adhesive into the hood. Unfortunately the hood flutter is still present and it’s only a matter of time before the adhesive wears down. After conducting research Lexus issued a bulletin repair acknowledging the fault in engineering regarding the aluminum light hoods. The hood is clearly to light and this is an obvious engineering defect which should signal recalls. I have read thousands of similar cases and Toyota is failing to take accountability of this problem. They need to replace the hoods instead they are refusing to acknowledge this is a recall and sneakily sending vehicles out to autobody shops. I have read the Lexus GX and TX both have the same problem. It is not safe to have a hood flutter like that it puts way to much strain on the hood safety latches and eventually they will fail. I do find it absurd and ridiculous that a brand new 70,000 SUV has such a problem. My hope is submitting this complaint it helps force Toyota do the right thing and take accountability for their engineering defect.
