2003 mazda mpv - transmission shifts very rough from 2nd to 3rd. Problem is intermittent and unpredictable. Dealer says the problem cannot be diagnosed and fixed unless it can be duplicated in the repair shop

The contact owned a 2002 Mazda Tribute. The contact stated that while driving at 30 mph, a pop noise was heard, the vehicle had unintended acceleration; the engine revved and sped up to 80 - 90 mph, inadvertently, and the brakes failed to engage after the contact had depressed with both feet on the brake pedal. The contact made a turn into a park and the vehicle slid, then crashed into a tree. There was smoke coming from under the hood and the contact turned off the vehicle before exiting and walking until she got assistance from a bystander, who called the police. A police report was filed. The contact did not receive medical attention at the scene but later went to the emergency room for injuries sustained with six broken ribs. The vehicle was towed to a tow yard. The dealer and manufacturer were not made aware of the failure. The failure mileage was approximately 230,000

I was driving down the interstate and my mazda cx9 caught fire! I didn't realize it since it was under the car and the car beside informed me of it. This was due to the transfer case issues that mazda had to extend the warranty on them because of so many issues with them. They are covered for 90 thousand miles or 7 years. My transfer case was already replaced once and do to issues and now since my car is past the 7 years but only has 60 thousand miles they will not pay for it. Even though the transfer case they replaced a few years ago should still be under warranty. This situation could of been a lot worse and thank goodness my children were not in the car with me

My daughter's mazda was involved in a fender bender which deployed the driver air bag, while the police officer was taking the accident report from the drivers involved, the officer noticed that the airbag was starting to smoke and then burst into flames. Only the interior of the vehicle burned from the dash to the back seat(the fire department arrived and put the fire out) I have video of the fire and I'm hoping the officer put his observations in the report. The officer said he never saw an air bag catch fire like that

Consumer rear ended another vehicle, driver's side air bag and passenger's side air bags, deployed causing a chemical to flow out and start a fire

Component: Air Bags – Knee Airbag / Trim Panel During a collision, on July 3rd with the Mazda CX‑50, the knee airbag deployed and a hard plastic trim panel from the lower dashboard was forcibly ejected. This panel struck my knee and caused a laceration across my kneecap. The injury was not caused by the crash itself but directly by the detached interior component. The trim panel appears to be part of the knee‑airbag assembly and should not detach with enough force or sharpness to cause bodily harm. I experience pain, discomfort, and ongoing issues due to the injury with tingling down my left leg when I get up to walk. I am concerned this represents a design or manufacturing defect in the knee‑airbag trim panel or its breakaway mechanism. There is contamination in the oil. So much so that it is impossible to check the oil. I've gone 3000 miles and never been able to check the oil. Even after wiping the dipstick, simply putting it back recoats the dipstick, so that the oil level cannot be read. The dealer simply changed the oil, but did not fix the problem. They said mazda knows about this, but won't do anything about it. All letters to mazda-usa are ignored. Why does mazda ignore such a potentially serious hazard? This could be a major safety hazard if the engine were to seize on the highway because of lack of oil

I wanted to report a safety issue with the 2007 mazda cx-7. There is a known problem with the AC compressor and instead of doing a recall on the product they extended the warranty to 5 years or 60,000 miles (instead of 32,000 miles). I bought my car right around the time they sent these letters out, so was unaware of what was going on. Recently I was driving the mazda and I smelled smoke. I saw some smoke coming from under the hood, and when I pulled over and opened the hood the AC compressor was on fire. Had I not been pulling into work and on the highway I would?ve been in worse shape. It was scary considering it was next to the engine which has oil, fuel, etc...going through it. There were no warning signs that there was a problem with the AC compressor until all of a sudden it caught on fire. I have made attempts to call mazda and try to get some help on this issue, and they seem to not care at all. I wanted to report this in hopes some other owners of the vehicle could be aware and not expect any help from mazda's end. It is a safety issue and it needs to be addressed/recalled

I pulled onto a dirt road because there was smoke coming from the front of my car and when I stopped I opened the hood and there was like a small fire coming from under the hood I got scared called my mother and 2 seconds later the whole front of the car was on fire

My car caught on fire on [CCC] and it's completely burned down, a total loss. Following is information about my car and some summary of what happened. Car information: - Mazda CX5 2024 - Bought in June or July 2024(car payment only made 22 times so far) - Odometer 24,445mi(as of 05/15/2026) What happened: - In [XXX] Friday afternoon, I was in Bellevue and noticed an engine overheat warning. So, I stopped immediately to cool down and called Mazda Dealership(Lee Johnson Mazda) to schedule a service. - Then after 20-30 minutes, I started the car again and the warning was gone. So I drove slowly and got on [XXX] to head home in Kirkland. I was already driving slowly. - After a few minutes, overheat warning as well as engine warning started firing and noticed that smoke coming out of bonnet. - So, I pulled over on the shoulder near [XXX] and called roadside assistance. But even before I get connected to the roadside service, engine caught on fire and I had to call 911. - The car is completely burned down and it's a total loss. - State patrol wrote me a "Exchange of Information" and "Tow/Impound and Inventory Record". Why I am submitting this form: - This car is less than 2years old and has been seeing Mazda service regularly. The odometer checked by dealership on 05/15 is 24,445mi which is slightly more than 24,000 which is stated in the Lemon LAW. But still, the car is pretty brand new and around 24,000 ish. So, I'm wondering if this still qualifies for Lemon Law. - Even

The contact stated while driving 25 MPH, smoke was coming from under the hood of the vehicle, followed by the vehicle shut down and the check engine warning light illuminated. The contact's son opened the hood and flames came out from under the hood. The contact son burned his fingers but did not seek medical attention. The fire department extinguished the fire. A police report was filed. The vehicle was not diagnosed. The manufacturer was contacted, and a complaint was filed. The failure mileage was 200,000

The contact owns a 2005 mazda tribute. Upon starting the vehicle, smoke was noticed. The contact turned off the engine, but the passenger advised her to restart the engine to see from where the smoke was originating. When the vehicle was restarted, flames were seen coming from the ignition coil under the hood, passenger lift the hood and ignition went into flames. The passenger was able to extinguish the flames. The vehicle was towed to an independent mechanic where the contact was informed that the ecm, spark plugs, and six coils needed to be replaced. The vehicle was then towed to the dealer (kings colonial ford, 3565 community rd, brunswick, ga 31520), but was not diagnosed or repaired. The manufacturer was made aware of the failure. Passenger seat heater either had a grounding issue or shorted out, causing to seat to start smoldering and catching fire, destroying the seat. There has been recalls and lawsuits for this specific issue on this model in other years, but not for this model year

We had another CX-5 in the back that we used to test whether or not it was either the throttle body or the BCM ( Body Control Module ) or the PCM ( Power Control Module ) not working that is causing your vehicle to not wanting to turn on The throttle body passed we transferred it over from vehicle A ( Being the donor ) to vehicle B (Being your vehicle ) and it was still doing the same thing which it would crank but not fully turn on and that is how we ruled that part out The BCM we also transferred over from vehicle A to vehicle B and same thing it would crank but not fully turn on This is what the mechanic at Mazda Collision Center of Irving stated: The PCM has power and is communicating with everything else in the vehicle. If it was the PCM then nothing would work and we wouldn't be able to even cycle the power as if we were going to turn on the vehicle We brought this over to our Mazda Foreman and we showed him the codes it was throwing and what was going on and explained everything and he did tell us that it could be the main harness that is causing all of this he is not for sure just because he is not the mechanic involved in this but he said that is a good start. I am uncertain exactly what is wrong with the vehicle. When I took it in to be serviced, it worked. The mechanic then said he couldn't work on it anymore after trying to fix the body work because the electrical system had failed. I then had it towed to Mazda to see if they could fix the issue. A low speed rear-end collision resulted in an immediate explosion of the vehicle, causing a large external fire and smoke to enter the cabin upon impact. The structural damage to the vehicle occurred on the front-right; the fire destroyed the entire front 2/3 of the vehicle (totaling the vehicle). The concern is that such a low speed impact should not cause an explosion which could potentially have injured or killed the driver and passengers

Had this Mazda CX30 as a rental car for about a week. Was pulling into the driveway, hit the brakes, and the vehicle took off full speed causing a crash. The engine was revving at max and the vehicle was accelerating as if the gas pedal was floored. No matter what pedal I hit the car just kept going until the crash and being shut off

The contact owned a 2005 mazda mazda6. The contact stated that while driving approximately 20 mph, the consumer rear ended another vehicle. After the crash, the contact noticed that the b post on the frame was on fire, there was on damage to side of car. Why did it catch fire? A tow truck appeared on the scene and extinguished the fire. The vehicle was then towed to an insurance lot where it was classified as destroyed. The police and fire department also appeared on the scene to file reports. The passenger subframe is rusted and broke the axel when driving. Glad im ok….So dangerous and the driver side is fine so bad design by ford (the tribute is like the escape)

This goes with previous form tge other thing is that my door flew of on freeway right before it jerked like stated in other form my car had just been fixed from same doors and now one flies of car doingb80 miles per hr on i15 in nevada causin the accident I had I feel repair shop needs to cover the cost of accident

TO NTSA - 2025 Mazda CX-50 Premium Turbo Plus - Purchased [XXX] - Less than 2,000 miles Driving north on [XXX] , I took exit [XXX] . When slowing to change to the right-hand lane, it surged then rapidly accelerated. Stepping on the brake did not slow it down, increased acceleration. To avoid hitting cars in my lane, I jumped the curb to the frontage road, continually pumping on the brake. The left front wheel and axle broke off and landed 20 ft away. The vehicle did not slow down; It was stopped by snapping a power pole in half. While trying to grab my purse, two Good Samaritans yelled at me to get out of the car, it was smoking and there were power lines overhead. Miraculously, I walked away. Description On 12-15-2025, in 18°F weather, the vehicle was parked on a residential sloped driveway with the manual parking brake fully engaged. While the driver was outside the vehicle, the parking brake system failed to hold the car’s weight. The vehicle rolled backward, crashed through a wooden railing, teetering on a 4-foot retaining wall, and came to rest over a culvert. Two young children were inside the vehicle at the time of the failure. Component Failure & Availability: The manual parking brake system failed to provide sufficient holding force to secure the vehicle. The rear brake calipers are currently available for inspection. Safety Risk: The safety of the two children inside the vehicle was put at extreme risk due to the unexpected movement of the car. This failure matches the exact conditions and consequences described in Safety Recall 1217F (NHTSA Campaign 17V-393), which states that "unexpected movement of the vehicle if parked on a slope" increases the risk of a collision. Reproducibility & Confirmation: The failure occurred under the specific environmental triggers (cold temperature and slope) identified in the manufacturer's own safety recall documentation. However, the manufacturer has officially refused to authorize the internal teardown of the actuator shafts—the only diagnostic method capable of verifying the rust and sliding resistance that causes this loss of holding force. My wife has a cold. She picked it up during several uncomfortable hours in the emergency room. Our driveway is on a bit of a slope. She stopped the car, put it in park (she thought), turned off the engine, and got out. Something in the dashboard compartment had blocked the shift lever and the car was actually in reverse. It started rolling backwards down the driveway. She grabbed it and tugged with all her might and actually succeeded in stopping it when it got to the street. She managed to prevent our car from rolling into our neighbor's parallel parked car immediately across from our driveway. She also managed to hurt her arm quite badly. X-rays and CAT scans and exposure to every sniffling, coughing person in the ER later, and we learned she was lucky and did not break any bones, just sprained her elbow and shoulder. Very very lucky that she didn't get knocked under the wheel by the still open door of the car! My question is this: Can automakers create a safety device to prevent this? If the engine is off and the car is not in park, and no driver is detected in the driver's seat, can they automatically apply the brake? There could be a button or switch to undo the brake for towing. It seems the technology to do this has been around for decades, but the X-ray tech, the ER admissions nurse, my wife's orthopedist, even the lady at our insurance company all had stories of similar happenings. How can such a safety device become standard?
