In the affected vehicles, the left and right rear brackets that support the rear CNG cylinder may fail, allowing the cylinder to dislodge from the vehicle.
Risk. If the cylinder dislodges, a CNG leak may result, increasing the risk of a fire.

The affected lifts are equipped with folding link arms that may have pivot holes that are too big, allowing the bearings to move out of position.

The wheelchair lift positioning input cam may fail while the lift is in use, allowing the platform to travel higher than the vehicle's floor height.
Risk. If the wheelchair lift platform stops above the vehicle floor, the wheelchair user can tip inwards toward the vehicle when exiting the lift, increasing their risk of injury.

Adrian Steel Company (Adrian) is recalling certain model year 2012 Ford E-150, E-250 and E-350 Commercial Cargo Vans modified by Adrian between September 21, 2012, to September 24, 2012. The affected vehicles are equipped with certain propane fuel system conversion kits with an aluminum fuel line fitting manufactured by Roush CleanTech. Due to a reaction between the different metals, the affected kits may develop a propane leak where the aluminum fuel line fitting contacts the brass supply valve housing.
Risk. A propane leak in the presence of an ignition source increases the risk of a fire.
Fix. Adrian will notify owners, and dealers will replace the aluminum fuel line fittings with stainless steel fittings, free of charge. The recall began during July 2014. Owners may contact Adrian customer service at 1-800-677-2726.

Roush is recalling certain aftermarket roush clean tech liquid propane autogas fuel system kits produced between december 17, 2010, and may 6, 2011, used for installation on fleet vehicles model year 2009 through 2011 ford e-series vans/wagons/shuttles/cutaway (150, 250 and 350) with 5.4l engines ("e-series"), and my 2011 ford e-450 cutaways. The valves installed in the service port of the fuel rails of the kit were under minimum specification for the diameter. This led to insufficient compression of the o-ring, allowing leakage of propane autogas. Ford is recalling certain model year 2011 e-150, e-250, e-350 and e-450 trucks for failing to comply with the requirements of federal motor vehicle safety standard no. 207, "seating systems." it may be possible that under unusually high loads, such as during a crash, the driver power seat track worm gear may fracture.
Risk. In the event of a crash or other circumstances of severe loading, the worm gear could fracture and allow the seat to shift unrestrained forward or backward increasing the risk and/or severity of injuries to the driver.

Risk. After a period of continuous use on the high speed setting, lack of grease on the output shaft gear may cause the gear to distort or fracture during operation resulting in a loss of wiper function. Inoperative wipers under inclement weather conditions could cause a crash due to impaired visibility.

The vehicles are equipped with certain Ricon S series and K Series wheelchair lifts with an optional armored pendant cable. The armored cable may be positioned such that it does not clear the wheelchair lift.
Risk. If the cable does not clear the wheel chair lift it may contact the power lug and, if it does not have a nonconductive cover, or it has an improperly installed non-conductive cover, a short circuit may occur that could result in a fire.

What's wrong. On some of the involved vehicles, the shift rod in the transmission may not be properly seated in the retention grommet of the steering column shift lever assembly. If the shift rod were to disengage, the driver could not select the proper transmission function which would create the potential for unexpected movement of the vehicle.
Fix. Dealer will inspect the shift rod attachment and make sure it is properly seated at no cost to owner.

I was driving nw on sr626 about 45 mph when a white car pulled from stop sign striking my car in the pass side, hitting rear tire and body just in front of tire as i was swerving left to miss car. At impact my car spun 180 degrees sliding backwards into a deep ditch, on right side of road, rolling onto drivers side then back onto all four wheels. The airbags never deployed as i was tossed around inside car hitting my head, neck and left shoulder area causing injury. I did have my lap and shoulder belt on and in the correct position. *tr

My 11 year old daughter was attempting to close the tailgate and didn't see there was something lodged in the crevice. The tailgate popped off and fell on her knee. She is in excruciating pain. We are at the er. This is not safe. There has to be a better safer way to attach the gate. I have owned 3 f150s and have never had this issue. Thank god my 6 year old was out of the vecinity. This could have easily injured or killed him as well.
On june 10, 2014 i left my office and traveled across the parking driveway at about 7 mph, a distance of about 150 feet where i attempted to stop(using my right foot on the brake) before making a right hand turn onto a busy fast moving street. At this point my foot hit the brake and accelerator at the same time (they are extremely close) causing me to suddenly lurch forward toward the oncoming traffic coming from my left. Fortunately i was able to quickly use my left foot to power brake and safely stop my vehicle. This was the third time in 250 miles of city driving my new vehicle that this had happened, but the first time i recorded the time and date of the incident, realizing there was a design issue. This is a serious safety problem, i can see many rear end accidents being caused by this design. Since i still own a 2004 ford e150 van, side by side measurement comparisons were easily made and i think i can see what has happened. The accelerator pedal has been raised about an inch and a quarter higher than my 2004 van and is approximately one quarter inch closer to the brake pedal. In addition because of the large engine cowling, the brake pedal is located mostly to the left of center of the center line of the steering wheel which forces the driver to effectively cross over awkwardly to hit the center of the brake pedal. This is a very unnatural motion not required on any vehicles that i looked at before contacting you. If the brake pedal is pushed down even slightly from the right hand side/edge of the brake pedal (normal habit by most drivers reviewing wear patterns on brake pedals of many vehicles at my office complex) the bottom of a man's right shoe will almost always be somewhat over the accelerator, and the potential for dual/simultaneous brake and accelerator pushing is created. *js
I opened the sun/moon roof and it would not close and after a couple of attempts it stuck in the full open position. The ford dealer said that two pins or brackets or clips had broken off the track mechanism that the sun/moon roof slides when opening or closing. The vehicle was moving when it failed and remained in the open position. Numerous repair shops in the area have told me that this is a common problem on f-150 sun/moon roofs due to the failure of the two plastic pins that become brittle and break off resulting in the sunroof failing. It is my belief that this is an occupant safety issue due to the sudden exposure to outside elements without the ability to close the sunroof. The ford dealers repair cost was nearly $2000 for a replacement of the sunroof.
(defective lock/handle) this incident occurred when the door was in a lock position and a passenger tried to open the door. Something snapped. Then the door was unable to open even after the door is unlocked. After my friend did me a favor to fix it, the same thing happened on the passenger door. The door was locked then my coworker tried to open the door before i unlocked it from the car alarm, then a component on the handle snapped. So now i have to open the door from the back or side door. *kb
Both Front Door latches have failed and will not open from the outside
Entered vehicle as usual closed door and began driving (vehicle was in motion). While in operation driving down a city street the driver side door swung open. I attempted to close door immediately but door would not close. It bounced open at every attempt. I pulled the vehicle over to the side of the road where after many attempts i was able to close the door. I attempted to open door to get into the vehicle the door latch pulled but the door would not open latch remained in pulled position and door unable to be opened at all. This malfunction appears to be a broken door latch which was recalled for vehicles (not including mine) nhtsa recall number 17v652000. However ford stated my vin was not covered under recall. I am currently awaiting a estimate for repair as it is extremely dangerous to operate the vehicle in this condition.

Recall 19s07 the 1st time was driving 72mph down the interstate and the truck downshifted causing us to be jolted and an 18 wheeler to almost rear end us. Received the recall letter from ford (literally 2 weeks after it 1st happened) for a sensor to be reset bc of truck suddenly downshifting while in motion. After dealership finished truck still was downshifting while in motion on highway or street at any speed. Took truck back and they reset again. Truck still continues to downshift while in motion. Was almost rear ended again in highway. It is hard to steer and control when it happens on the highway. Have to pull over and restart engine to be able to go again. Ford is saying we have to pay for a diagnosis, but the only problem with the truck is what it is being recalled for. Their remedy of reprograming the sensor is not working and driving the truck is dangerous. We need help please! They won't fix their faulty problem.

I purchased my 2013 ford f-150 v6 3.5l from jons friendly used cars back in april of 2017. The truck has been doing fine until recently (last 2 days). As i was on my way to work, the truck was delaying w/shifting, the speedometer dropped to zero and which put the truck into failsafe. After looking into this and identifying the code (p0270), it confirmed the symptoms i was experiencing. I guess i'm wondering is this going to be something that need to replace (out of pocket) or is ford or the dealership needing to get this replaced if it's been identified/posted on the tsb. How can i see if my car is covered or not?

My vehicle will be traveling at highway speed and suddenly shift to first gear locking the rear wheels. Also gets stuck in 5th gear and will not move.

The nhtsa by telling tire dealers that they can not install my mounted snow tire because they don't have tpms sensors have made driving in the winter more dangerous and unsafe for me. The system in my summer tires has been inoperable for 2 years and you know what has happened... Absolutely nothing. The cost of the sensors is very high and if you guys want me to have them so badly, then pay for them yourselves. This is just another example of nanny state government trying to make it look like they are doing something to justify their pathetic existence. So in summary you are actually making things more dangerous for drivers by denying tire retailers from installing snow tires on my vehicle because they don't have a worthless sensor that doesn't do anything to make driving safer for me. I bought this car for my daughter at loving lincoln mercury in lufkin texas. While test driving the car i noticed 1) that the brakes were making a light grinding noise while coming to a stop. 2) the low tire pressure indicator light was on. The salesman assured me they would take care of the problem before i bought it. When we picked it up they said they looked at all the brakes and they were ok. Also, the tire pressure light was no longer on, so i went ahead and bought the car. Now, just a few days after taking possession of the car the low tire pressure light is on again. The tires were made in china. The brand name of the tires is "goodride". I have read online that the chinese valve stems were recalled during certain years. I had to replace all 5 tires within 2 years of my vehicle purchase. The new tires are now just over 2 1/2 years old. One blew out on a residential street while going 35 miles/hour. The 2nd tire replaced was also separating and causing the van to vibrate just as the first one so i took it in 6 weeks after the first blowout. Leaving the tire store my van is still vibrating and now i notice the 3rd tire is buckling on the outside wall. The 4th tire does not appear to have any major issue yet. The 5th is my spare-never used. Discount said they wouldn't replace the 2 front tires until they had the same issue as the rear tires, without cost to me. These tires are from the same lot #s. I just hit the 100,000 mile mark on my 2008 focus, i bought the car new, after a little over a year at 20,000 miles i had to replace my tires, my car was shaking really bad and making alot of road noise. Then at 84,000 miles i had to put on my 4th set of tires. My miles are all highway miles, i rotate every 6000 miles, they always tell me the alignment is within factory settings. Then at 98000 miles my car started doing it again, they said my rear tires are bad, they blamed it on a bad trailing arm and they had to reset my chanber toes in the rear end, again, they said my alignment is fine. All of this was done at the ford dealership that i originally purchased the car. So now at 100,000 miles, i am getting ready to put on my 5th set of tires. This is insane. I'm afraid at some point somebody is going to have a tire blowout or something and somebody is going to get hurt. Please help to fix this problem. *tr
Factory pirelli tires tread depth after only 15191 miles on new 2008 ford focus ses---2 tires @ 2/32 and 2 tires @ 3/32. Dealer and pirelli won't accept responsibility. (car driven in local small town). Not happy! *tr

Master cylinder failed causing crash. There was no warning and after first crash, vehicle continued to plow ahead until four vehicles were involved. *ak
While applying the brakes to park the car the was a loud rifle shot sound, flash and vehicle jolt from under the hood. And the break warning light came on. During repair the mechanic found that the problem was the with the master cylinder cruse control cancelling switch. The switch blew apart and ignited the atomized break fluid that was expelled from the master cylinder. Fortunately the exploding break fluid self extinguished avoiding an engine compartment fire. Since the triggering of this event was the act of applying the brakes it was only by chance that it didn't happen at highway speeds. The event totally disabled the front brakes and at highway speeds the brake fluid mist would have likely ignited further back in the engine compartment or under the chassis. There have been numerous recalls of ford products over a 20+ year period for this switch component causing both fires and deaths but this vehicle's s/n was not on the list for getting notified. Looks like someone didn't do their "homework" on this one, i am not sure this vehicle is safe to drive. I have the repair bill for the replacement work and the gentleman showed me the blown out shell of the switch. I picked up one of the parts from our parking slab. I have notified ford by email.

I only have 8000 miles on it, and the steering wheel is shaking when i touch the brakes. It also has squeaked from the day i bought it. I took it back to the dealer, they didn't hear the squeak. It is only when i back up.

The ignition key is binding in ignition cylinder when turning to start position only. Twice it has not started on the first try. Twice i heard a metal sound when starting. This is a problem with ford focus 2008, and has been brought to my attention by my auto repair shop. It was suggested to me to go direct to a ford dealer service shop which i did. The car warranty expired 5/2011 so i don't qualify for free repair on this problem. As far as i can see in researching there has not been a recall on it. I think ford should pay. *kb
Vehicle suddenly lost power, only continuing to run at a rough idle. The vehicle would not allow any acceleration. The only possible response was to limp to the shoulder of the road. After turning off the ignition the vehicle was restarted and ran as normal for the rest of the trip. Four days later the same thing occurred. Upon taking the vehicle to the ford dealer and having them tell me to continue to drive the vehicle until it occurred again and then have it towed in, i convinced them to look further into it. I was able to locate several complaints on line of the same type. The necessary repair was to replace the throttle body. The next day the dealer contacted me and said the ford hotline stated they had previous complaints and advised to replace the throttle body. This was done and the vehicle has operated correctly since the repair. *tr

While driving on highway at 55 mph car looses power, goes into limp home mode, orange wrench warning indicator appears on dash panel. Very dangerous in high traffic areas where there is room or time to pull to side of road. Pull off road, turned off engine, and then restarted and ran fine. Seems to happen unexpectedly every 3 to 4 months. No engine malfunction codes can be found on onboard diagnostic system.
Vehicle was leaking antifreeze and taken to good year. The part was not made and is not being made by any factory or by ford. Ford is aware of the problem. The service manager in rock hill,sc said there was 20 others waiting and they needed paperwork for proof and a hundred deposit for a part that isn't even made. Good year made a temporary fix. If the fix breaks the antifreeze will leak out causing the motor to seize up.
In the last two i been having a wrench coming up on the screen and truck loses power and the speed drops down to 5 miles an hour. While the truck is running in idle, it is vibrating. When i stop it and restart it goes away. It happen three times in the last two weeks. After the second time the dealer reprogramed the computer last friday. Thursday five days later it came back when i was doing 50 miles an hour. I called the dealer and he said he will change the throttle sensor out tomorrow .the times it happen i was on country road with no traffic, if i would have on the interstate no telling what would have happen with cars behind me doing 70 miles an hour.
Driving on a city street the truck started shaking and loss all acceleration, it showed a wrench on the dash screen, coasted to a safe place and turned truck off. It restarted fine. Approx 30 mins later on the highway it did the same thing, started shaking, no acceleration and the wrench showed up. Again coasted to a safe spot and restarted.

On sunday, august 20, driving s81 just before the salem exit, the 2012 ford e-150 white cargo van 4.6l romeo engine blew a spark plug out of the #3 cylinder head while accelerating from 40 mph. The planned trip started from harrisburg pa to andrews nc. To witness a solar eclipse. Engine failure happen at approximately 40mph just north of the salem exit off 81 south. There was no shop available till next day to repair damage.. Damages included a overnight stay, missing the solar eclipse of 2017 and repair and parts cost of over $300.00 spark plug was destroyed. The coil was destroyed and the cylinder head was render useless. At this time a temporary tap was made and heli coil was inserted to hold a new spark plug in place and continue our trip the next day. Since then it blew again and i have no transportation for me and my tools. I have reported this complaint to corporate ford for review. Ford stated they will respond by email wednesday december 27. To determine culpability for manufacturing flaw with inferior aluminum head design. Total damages estimate $2500 to $4000. Sincerely owner 12/22/17 submitted to cas & nhtsa. *tr

The vehicle was taken to the dealer where it was diagnosed that the tread on the cylinder head was stripped. The vehicle was not repaired. The manufacturer was notified of the failure. The approximate failure mileage was 134,000.
The gas pedal goes with gas and decelerates on its own. Brake booster does not function correctly. The motor shakes and looses horsepower.

Driving this van is very difficult, it will not hold a straight line ,when you pass big trucks going approximately 65 mph ,the vacuum sucks it up ,and it gets more uncontrollable,causing a minor fish tale. Of course ford checked it out and found nothing wrong .m---ks could not duplicate the problem..do not drive this van if is at all windy,could become uncontrollable and unsafe . The wandering in the steering is not acceptable .i would not recommend this van for anyone. Or any business. *tr

I have replaced the tires on this vehicle 5 times in 6 years. The car was purchased in 2010, only 2 years old, and the first set of tires was replaced in 2011. Each time the tires are replaced it's because there is abnormal wear on the inside of the tires; so much damage, the steel radial from tires is sticking out. I was told i needed new sway bar links; replaced those as well, but have since put another new set of tires on this car. When i replaced these tires, i also had to replace 3 lug nuts which were broken as well. I have researched this problem; many others are experiencing this same issue. Two mechanics have tried to figure out what is causing the tires to wear prematurely and unevenly, and i still have no answer. My tires which are supposed to be good for 45,000 miles or more, depending on what i buy, and no matter what brand i buy, none have lasted 20,000 miles. The only option i have is to replace my tires approximately once a year which is unacceptable. I refuse to spend money replacing parts trying to fix the problem by "process of elimination" because no one seems to know what the root cause of the problem is. The premature, uneven tire wear is simply a "symptom" of a defect that no one seems to be able to identify. The first thing you will notice is the vibration; which continues to get worse even after alignments and tire rotations. Also, the low tire pressure light will go on because the tires lose air since they are wearing prematurely and unevenly. Once this starts happening, you may as well start saving for a new set of tires; unless ford comes clean and admits the defect and does a recall; but don't hold your breath. On this form, there is a question that says "when did this happen?" i had to choose a year so i just picked one; it has happened in 2011, 2013, 2014, 2015, and 2016.
This is a ongoing problem with this vehicles suspension since i have had this vehicle i have replaced the tires on it 4x the rear suspension cups the tires and makes it bounce i have had multiple alignments and i bought it a 52.000 miles n for the last 4 yrs i have bought many types of tires and this car has ruined them all, also strut replacement and tie rods and i just got a new rackon pinion for the steering was affected and the fluid leaked out and i highway drive, it would not perform over 65mph without shaking terribly i see more complaints about this car and this supension problem and steering and idol jerks me forward and feels like its going to stahl, the moter mounts an trani mounts are always loose or need to be replaced, an the ac went on it an because it's behind the dashboard the housing unit for the evaporator it cost 1000 to replace because it is hard to get too, the intermitten fan does not come on at all it never cycles so it smells like burnt engine coming in thru the vents that makes me sick, i have had nothing but problems since day one after i took it back after 2mons they wanted to keep it and re-do the whole engine and told me i would have to pay for it , thats when i should of reported it as a lemon, is ford going to wait until someone has a major blow-out an flip the car an die this should be a automatic recall for safety this car is so unsafe, the frame is all wrong and the rear is not aligned from the factory this is an in house problem .
The car suspension is easily unaligned and causes the car to go through tires at an alarmingly above average rate. 30,000+ mile tires do not make it 20,000 without severe damage and wear. After owning the car and taking it to many different shops the results are always the same that my cars suspension is not correct. I have had to replace 5 sets of sway bar links and at least 8+ sets of tires in the 100,000 miles i have drove the car. I have receipts for 90% of my purchases for repairs, parts, and service that all relate back to the cars suspension not functioning properly and wearing abnormally. *tr
