What's wrong. Mazda North American Operations (Mazda) is recalling certain model year 2017 Mazda6 vehicles manufactured August 12, 2016, to September 28, 2016 and equipped with optional accessory fog lights. Due to an incorrect combination switch being installed, the turn signals may not automatically cancel by the rotation of the steering wheel. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, "Lamps, Reflective Devices, and Associated Equipment."
Risk. A vehicle continuously driven with the turn signal flashing may result in confusion to other drivers, increasing the risk of a crash.

Steering:electric Power Assist System
What's wrong. Mazda North American Operations (Mazda) is recalling certain 2016 Mazda6 vehicles. Debris from the welding process during manufacturing of the front passenger seat frame may damage the occupant classification system (OCS) wiring.
Risk. If the wiring becomes damaged, there may be a loss of power steering assist and the deactivation of the passenger air bag, increasing the risk of a crash and the risk of injury in the event of a crash.
Fix. Mazda will notify owners, and dealers will attach a protective pad on the seat frame. If the OCS harness is damaged, insulating tape will be applied to the harness. When driving the affected vehicles in heavy rain or in deep puddles, the alternator belt may slip causing the Power Control module (PCM) to incorrectly assume failure of the charging system.
Risk. Once the PCM assumes that the charging system has failed, the vehicle will stop charging and could result in poor acceleration, loss of steering assist and windshield wiper operation, and a possible engine stall, increasing the risk of a crash.

What's wrong. Mazda North America Operations (Mazda) is recalling certain model year 2014 Mazda6 vehicles manufactured October 25, 2012, through May 9, 2013. In the affected vehicles, it is possible to fill the fuel tank beyond its rated capacity.
Risk. If the tank is overfilled, gas may flow into the charcoal canister (an evaporative emissions component) and possibly leak from the canister's external vent, increasing the risk of a fire.
Fix. Mazda will notify owners and dealers will inspect and add an adapter to the shut off valve in the fuel tank and also inspect the charcoal canister for the presence of gasoline, replacing it as necessary, free of charge. What's wrong. Mazda is recalling certain model year 2009-2013 Mazda6 vehicles manufactured February 4, 2008, through August 24, 2012. The door latch mounting screws may become loose.
Risk. If all three door latch mounting screws become excessively loose the door latch mechanism would not engage and the door might open while the vehicle is in motion increasing the risk of accident or injury.

In the affected vehicles, spiders may weave a web in the evaporative canister vent hose, blocking it and causing the fuel tank to have an excessive amount of negative pressure.
Risk. Negative pressure could cause the fuel tank to crack resulting in a fuel leak, increasing the risk of a fire.
Fix. Mazda will notify owners, and dealers will reprogram the powertrain control module and inspect and clean the canister vent line. What's wrong. Mazda is recalling certain model year 2009-2010 mazda6 vehicles manufactured from april 8, 2008, through february 8, 2010. A certain type of spider may weave a web in the evaporative canister vent line and this may cause a restriction in the line. If this occurs, the fuel tank pressure may become excessively negative when the emission control system works to purge the vapors from the canister.
Risk. As the canister is purged repeatedly during normal operation, the stress on the fuel tank may eventually result in a crack, potentially leading to fuel leakage ad an increased risk of fire.
Fix. Dealers will inspect and clean up the canister vent line, and install a spring to prevent this type of spider from entering the vent line. This service will be performed free of charge. Is recalling 2,323 owner-installed roof-mounted fit kits, models 3068 and 3069, manufactured between january 1, 2008 and february 28, 2009 that are used with thule podium foot packs, models 460 and 460r. These fit kits were sold as aftermarket equipment for use on the vehicles listed above. The bolt(s) used to secure the foot to the vehicle was manufactured to a hardness that may cause the bolt(s) to be brittle and potentially break under stress.
Risk. This may result in the bolt(s) snapping off during use or without a load. The unsecured rack and load can then come loose while driving creating a potential road hazard, possibly resulting in a vehicle crash.

What's wrong. On certain passenger vehicles, the right and/or left rear seat belts may not lock properly due to missing retaining pins on the housing.
Risk. The locking mechanism of the seat belt retractor may malfunction, increasing the risk of death or serious injury to the rear seat passengers in the event of a crash. Passengers should not sit and child seats should not be used in the right or left rear seats until this defect has been corrected.

Exterior Lighting:fog Lights
What's wrong. On certain mini vans and passenger vehicles, the socket holder in the fog lights is installed in the lamp casing using pressure. During operation of the fog light, the heat from the bulb can expand the socket holder. The expansion can force the socket holder rearward. After repeatedly being subject to this expansion, if the fog light is also subjected to vibration during vehicle operation, the socket holder with the bulb holder and the wiring harness may separate from the fog light casing.
Risk. Should this occur, the bulb socket and the harness can drop inside the bumper and possibly but rarely, the heat generated by the bulb can cause the bumper to burn.
Fix. Dealers will install a spring to prevent the socket holder from separating from the lamp case. Owner notification began on july 14, 2003. Owners who take their vehicles to an authorized dealer on an agreed upon service date and do not receive the free remedy within a reasonable time should contact mazda at 1-800-222-5500.

What's wrong. On certain passenger vehicles, the fuel sender unit (fsu) may have been improperly installed in the fuel tank, producing an inadequate seal, and fuel may leak.
Risk. A fire could occur in the presence of an ignition source.
Fix. Dealers will inspect the top of the fuel tank, and if necessary, repair or replace it. Owner notification began february 9, 2004. Owners should contact mazda at 1-800-222-5500.
Parking Brake:indicator Light
What's wrong. Certain passenger vehicles fail to comply with requirements of federal motor vehicle safety standard no. 135, "passenger car brake systems." during production, the float in the fluid level sensor in the brake reservoir can be forced off of the guide rail during the filling of the master cylinder.
Risk. Should this occur, it is possible that the float will not return to the guide rail and the brake fluid level warning indicator may not function.
Fix. Mazda has not yet provided a remedy for this campaign. Owner notification began july 14, 2003. Owners who take their vehicles to an authorized dealer on an agreed upon service date and do not receive the free remedy within a reasonable time should contact mazda at 1-800-222-5500.

Was in an accident which resulted in a lockup of vehicle and side collision with battery failure
Driving along a highway (left side of road) i crashed into large boulders of concrete scattered along the road that was caused by a previous accident. Traveling at 60 mph my vehicle launched off the ground slamming back down on all four tires. My vehicle then spun out of control crashing to the side guard rail impacting the side of the vehicle 2x's (right side of road) resulting in disabling damage to my vehicle. To my surprise the airbags did not deploy (particularly my side airbags). Later i learned that mazda (manufacture) reported a recall for my vehicle relating to a potential malfunction of side airbags. Upon my discovery i learned that mazda had learned of the defect around august 2019, but had not issued a recall notice on their website until january 1st 2020. My accident took place dec 4th 2019, and i should also add that as of yet mazda still has not made any direct attempts (email, mail, phone) to notify me of the potential hazard. Had it not been for my own inquiries i still may have not known that there was a safety issue within my vehicle especially when i needed it the most. I don't want to come off as being dramatic but that accident i was involved in could of cost my wife and i our lives. For that reason i am seeking to retrieve the event data records (edr) of my vehicle so that i may determine if the airbags failed to deploy as noted as a potential hazard or if the impact and damages where simply not substantial enough to cause the airbags to deploy. I asked my insurance company if they could assist me in gathering that data which they first claimed to be possible, but shortly after i was informed that they did not have the sophistication to do so, hence forwarding me to this website. Please advise on how i may retrieve that information, any help would be greatly appreciated. Thank you.
Tl* the contact owned a 2012 mazda 6. While driving approximately 30 mph and attempting to avoid a collision, the driver lost control of the vehicle and crashed into a tree. The front end of the vehicle was severely damaged, but the air bags did not deploy. The vehicle was destroyed and towed to an undisclosed location. The cause of the failure was not determined. The manufacturer and local dealer (rent to own outlet, 1027 westbank expy, gretna, la 70053) were notified of the failure. The driver sustained an injured left shoulder and the front passenger sustained facial and oral injuries. Medical attention was required. A police report was filed. The failure mileage was 131,000.
On april 20 my self and my 14 year old daughter was in a really bad accident and neither one of our airbag did!!!! Not come out and my myself nd my child was hurt i was rush to the hospital and i've brought my my car in december of 2017. And it happen on city streets
Takata recall- my mazda mazda6 -2009 vehicle was involved in an auto accident on in january 2015. I was injured with burns in my wrist from the airbag. I also had to go through pt to regain movement to my injured wrist. The vehicle was moving and another car cut out in front of us and hit my car. I was not cited in the police report the other party was.
2007 mazda6 on 2 lane rd. 35mph zone maybe going a little faster ran off rd. Hit rt. Curb car bounced crossing center line hitting krt bus (public bus} head on causing front of car to buckle then rolled per police report one time. Front driver airbag is the only air bag to deploy. Police not sure if driver was wearing a seatbelt but he was partially ejected through front passenger side window. The bus was traveling approx. 28-30mph. Driver of car was doa. Mass head trauma and multiple injuries are the only thing listed on death certificate. It did state however that this driver died seconds upon impact. The bus driver was transported to hosp. Our 2006 mazda 6 gt was totaled in a wreck in which it flipped 4 or more times on the highway and then into the median. The speed at the time accident occurred was 70 mph. Not one of the 6 equipped air bags was deployed. I contacted mazda customer service and was told that apparently none of the sensors were tripped and this is not uncommon. I find it hard to believe that not one of 6 would trip in an accident that violent. My insurance also didn't seem to think it was an issue. This is problem because most of us believe that our air bags will protect us in most any wreck, especially one in which you are flipping on pavement. Tl* takata recall. The contact owned a 2004 mazda 6. The contact's vehicle was included in nhtsa campaign number: 15v345000 (air bags), but had not been able to determine when the parts would become available for the recall repair for months. The vehicle was taken to the dealer, but they did not confirm if the parts were available. While driving at an unknown speed, the contact's vehicle was crashed into by another vehicle. The driver's and front passenger side air bags did not deploy. The contact stated that the other vehicle's air bags may have deployed. The driver sustained whiplash and bruising of the hands due to the force of the impact. Medical attention was not needed. A passenger sustained minor whiplash, but no other injuries or medical attention. A police report was filed. The vehicle was destroyed and towed to a yard. The vehicle was inspected by the insurance company, but it was not determined why the air bags did not deploy. The manufacturer was notified. The approximate failure mileage was 180,000. Parts distribution disconnect.
We were broadsided by a vehicle that ran a red light at full speed. The impact was on the passenger side. The car flipped over and skidded onthe passenger side then flipped back onto the drivers side and skidded before rolling over at least once. Not one air bag deployed. Thank god for the seat belts. They saved our lives. Injuries were concussions, sprained neck, injuries to lower neck, back and shoulder of passenger. We landed upside down, released our seat belts and crawled to safety with the help of police and fire. Our speed was about 25-30 miles /hour. Our car was totalled. The other driver was cited in the accident.

I have a 2016 mazda 6 gt with stock 19" dunlop tires. I bought the car with around 80 miles & at the time of the incident was around 6800 miles. I was traveling to work going around 72 mph & the left rear tire blewout causing an accident on the interstate. Tires should not blowout that quickly with proper care of the vehicle. The tire tread wore too fast.
This car is not safe and mazda has known for years. The door sash molding has caused me permanent brain damage. At age 43, i had to retire/disability due to defects mazda knew of and just inform consumer of in 2018? I was making right hand turn, legally. Someone in lane next to me decided to turn and hit side of car. I as not injure on impact. It was the three week had to drive damage car while wait for lady's insurance to pay. The metal piece that in sspa5 status stuck out and hit head on it multiple times. My neurologist was dumfounded and could not figure out where my various horrible symptoms came from. The accident occur march 2017,mazda has known for years, check mazda forum. Had this fix 3 times and is falling apart on passenger side again. As far as electrical, i will be at red light or stopped and car turn off for no reason, the bluetooth which need for directions goes in and out. It is safety concern for me due to confusion symptom of brain damage. I just received in febuary 2018 safety recall 1217f on brake issue. It state mazda concerned about safety, but again knew of issue and hat sent recall. The car has many serious manufacture defect that have caused my life to be ruin, and as i research, bodily harm to other people. Mazda, even when shown medical record and note from neurologist will not release me from my lease unless i get another mazda, which i will never do. Takata recall: i was hit from behind very hard and then hit from side by another car and knocked off freeway on to a fence. The side airbags didn't inflate and instead just slapped my body leaving red marks on my arm. I was thrown into side of car & steering wheel several times and unable to work for two months. Severe concussion with memory loss. I was driving north on mopac expressway in austin, texas at 58mph. The person who hit me said he was changing lanes & did not see me. Car was totaled. *tr
On july 18, 2018 i was in frontal impact accident. While driving on a city street. I was alone and driving i did not have a phone in my possession or in the car at that time. The vehicle that hit me was traveling at least 40 miles an hour. The upper and side airbags did not deflate, the front airbag deflated sideways and missed my face completely. My torso hit the steering wheel and my sternum was fractured. My right ankle was bruised, scraped and sprained. Because of the severe pain i was in, i was not able to get out of the vehicle on my own. I carried out of the vehicle and laid on the grass. I was taken by ambulance in severe pain and could not stand or sit up i was diagnosed with a fractured sternum and sprained right ankle. Car is totaled $11,000.00 worth of damage. I have not been able to work in 3 weeks because of the severity of the fracture on my sternum.

Takata recall - 2011 mazda 6 i was the passenger in a collision in november 2015 where the 2011 mazda 6 was t-boned on the driver's side by another vehicle (ford fusion), which made the mazda 6 spin several times, hop a curb, and deploy the airbags. As the passenger (front seat) i suffered injuries caused by the takata airbags and required going to the hospital following the collision. Injuries included at least three fractured/broken ribs and major bruising on my right knee. Injuries took over 6 months to heal, taking heavy doses of pain killers just to manage the pain. Lingering effects from this collision can still be felt 7 months later. The driver did not sustain major injuries aside from scrapes and a few bruises. Police reports determined that the driver of the ford fusion was responsible for the collision and was charged with careless/reckless driving. That driver failed to stop/yield in their turning lane before driving straight into the mazda 6. The mazda 6 was declared totaled by insurance. The mazda 6 was added to the takata airbag recall in may 2016, approximately 6 months after the accident had occurred.
The airbag deflated when i was involved in a car accident september 25, 2015. I suffered a fractured sternum from the airbag and severely multiple fractures in right ankle. This car was totaled. The accident was not my fault, the other car moved into my lane and i struck it head on.

My right hear has partial hearing with moderate ringing and pain, my left ear sounds like i'm sitting in a field of crickets. It makes horrible cracking and shrill whistling sounds. I am going to see my ear doctor tomorrow. The rest of me is sore but ok. I think this issue needs to be looked at as there was definitely too much pressure in the airbag. I have pictures of the steering column showing how the airbag was gone except a small piece of you need them. My mazda6 spontaneously caught on fire while it was parked on a street and completely burned down. The fault/problem cause is unknown at this time but i suspect there was wiring/electrical fault and manufacturing and design defect. Both the sheriff's department and fire department were involved in assisting me in this incident. I am still waiting for reports from law enforcement/first responders. Mazda has refused to address this situation without me providing them with an "inspection report supporting the claim a defect was present and said defect caused the fire." as i was only a lessee of the vehicle, i have no possession of the vehicle and, therefore, i cannot order an inspection on the vehicle. I was driving on the freeway after leaving class. After being on the freeway for about 5 minutes, the car starts to power down. All of the caution lights start to come on (ie: battery light, traction light, oil light and many others). As i get off the freeway, the car starts to lose more power, the fuel acceleration did not work as well and the steering became very hard because something happened to the power steering fluid. Then as i start to pull into a parking lot there is smoke coming from both sides of the closed hood. I put the car in park, turn off the acceleration, and pop the hood and i noticed it is on fire. Turning left at an intersection after coming to a stop, the driver mis-judged the right edge of the right-most lane and struck the curb. The ball joint separated from its housing in the right, lower control arm, causing the bottom of the wheel to collapse inward and the driver to lose control and impact the car in the left lane. The ball and stud were still attached to the upright clamp, but the ball joint housing was missing and not recovered. The tie rod, tie rod end and the strut did not appear to be damaged. In previous model years (2009-2013), the lower control arm appears to be a single, solid, forged steel piece with an integrated ball joint, a design for which a side force is unlikely to separate the ball from the control arm without shearing the ball stud. The lower control arm in the 2014 model year is a new design made from a stamped steel plate with a separate ball joint. The housing of the ball joint is pressed into a stamped hole with a lip in the plate. The sides of the control arm have strengthening lips that extend only to about the center of the ball joint and then stop. The outer end of the control arm is a flat plate without a strengthening lip. The flat outer end was folded back flat over the top of the control arm in this accident.

Both my back and front chassis are completely rusted through and it’s a complete screwed car. Looking online there is a recall on something like this but not for my model? The subframe of my 2009 mazda6 completely rotted out from the inside and the vehicle broke down during normal use. On wednesday, may 2nd, a mazda dealership replaced my rotors, pads, and noticed the car was pulling to one side so they corrected the alignment. During this time they identified a normal level of rust on the subframe but was not concerned by any other signs of failure. Less than 10 miles after leaving the dealership the subframe cracked, my front passenger wheel cv axil disconnected and the car was inoperable. Thankfully i was going only 35 mph, down a straight, paved city road, and not on the highway. I fear that at higher speeds the car would have gone off road or worse for when the subframe broke the car jerked heavily to the right then completely stopped. I believe the dealership provided quality service and due to the type of physical failure of rotting from the inside the danger it posed to myself and my family in the car was hidden. The dealership informs me that there is a backorder (400ish) for the part and that parts may not be available for several weeks. Searching online there seems to be many similar cases reported with models around this timeframe. Due to the type of failure, number of similar cases, age of the car, type of use, i am shocked that there is not a recall.
The front and rear suspension pops, clunks, the vehicle shakes, vibration also happens, it also causing tires to be replaced faster than normal they go bold in the center its causing now the axles to shake at night time you can see the reflection from headlights bouncing, they have tsb's on this suspension issue however not for 2018 only 14-16 mazda 6s i believe and supposedly the issue was corrected well they lied because mine is a 18 and it is a disaster, my transmission is now shifting very hard from 1st to second gear, turn wheel it makes a loud clunk sound or popping sound, go over a dip in road does the same thing, you go over a bump the vehicle feels soggy, it like dribbles, it causes the vehicle to sway.. It's horrible, also the brakes was shaking at 15k miles, found out it's tsbs on this issue and of course the dealership didn't fix for free, charged me over 600 dollars for a issue that has a tsb on.

My son took my car to school. He was going to leave after school let out, he was going to give his friend a ride home. This is a 6 speed car. His friend got in the car on the passenger side, my son was talking to his brother, he was not in the car. My son had put the keys on the driver seat, his friend put the key in the ignition and turned the key to accessories to listen to the radio, the car had the emergency brake on, and it was in first gear. When he turned the key to listen to the radio the car took off!!!! I am not sure how fast it was going but it crashed into a tree and stopped. There was no driver!! This car has had many random computer problems starting with transmission locking in low gear until you stop the car and restart. Car shuts off displaying automatic stop break got very hard and steering very hard. My daughter had an accident last week 2/2/17 due to vehicle loss of power hard breaking and steering. Called mazda with no response.

The vehicle is available for inspection. Safety Risk: The failure caused a sudden loss of propulsion while attempting to accelerate. This created an immediate risk of a rear-end collision and left the vehicle unresponsive in the path of oncoming traffic. The transmission's unpredictable shifting makes the vehicle unsafe to operate. Confirmation: The failure and the P0731 code were officially confirmed by Castle Mazda on March 2, 2026. E-brake button has corroded away and will no longer hold the car in park (car is a manual, so this is particularly concerning since this is the only way to offically keep the car in park when turned off). Car was parked in a stationary position, on a slightly slanted road. Came back and car had slightly backed up into car behind it because parking break had 'let go'. Luckily slant was small enough to not cause damage to the car behind mine, and only left a minor dent in my rear bumper. Inside of car was hot (it was a usual hot day) and seemed as though the actual parking break button, or glue that held it on, had melted. I can now no longer keep the break in park - have to either put it in first gear, or physically hold the break up with something underneath, putting my car and other at risk for car rolling back.

My son took my car to school. He was going to leave after school let out, he was going to give his friend a ride home. This is a 6 speed car. His friend got in the car on the passenger side, my son was talking to his brother, he was not in the car. My son had put the keys on the driver seat, his friend put the key in the ignition and turned the key to accessories to listen to the radio, the car had the emergency brake on, and it was in first gear. When he turned the key to listen to the radio the car took off!!!! I am not sure how fast it was going but it crashed into a tree and stopped. There was no driver!! There is a police report on this, and there were several witnesses. My son's friend hit his head on the windshield and it cracked the windshield. I contacted mazda headquarters to make them aware, and they seemed bothered that i even called!!!!! It is a miracle a kid was not walking by and was struck by this out of control car. *tr
The airbag deflated when i was involved in a car accident september 25, 2015. I suffered a fractured sternum from the airbag and severely multiple fractures in right ankle. This car was totaled. The accident was not my fault, the other car moved into my lane and i struck it head on.
My mazda6 spontaneously caught on fire while it was parked on a street and completely burned down. The fault/problem cause is unknown at this time but i suspect there was wiring/electrical fault and manufacturing and design defect. Both the sheriff's department and fire department were involved in assisting me in this incident. I am still waiting for reports from law enforcement/first responders. Mazda has refused to address this situation without me providing them with an "inspection report supporting the claim a defect was present and said defect caused the fire." as i was only a lessee of the vehicle, i have no possession of the vehicle and, therefore, i cannot order an inspection on the vehicle. Mazda is acting in bad-faith.

The v6 motor in my mazda 6s has been leaking oil for almost 10 of the 12 years i've owned it. The oil leaks out of the faulty seals on the timing chain cover in front of the passenger, and the oil drops onto the catalytic converter. This oil vaporizes and that vapor enters the cabin in slow speed driving and when stopped at intersections. I complained about a burning oil smell many times while the car was still under warranty, and the mazda service techs always pretended like they didn't find any problem. Years down the road, i am finding this is a rampant problem with all ford and mazda cars and suvs with this "duratec" 3.0 v6 which was made by ford. It is installed in many ford, mercury and mazda vehicles. I carry a fire extinguisher with me in the event i have an under hood fire, and i'm very worried about my health after all these years of breathing this vapor. The cost to repair this seal failure is daunting. I hope mhtsa can see that ford has been covering up this defect, endangering lives and the health of drivers as well as creating a tough financial burden on the owners to fix the issue. I gave a date on the form below, since it was required, but this has been going on since 2006 or so.

I was driving on the right lane (the slow lane) of the freeway, going around 60-65 mph. Around 4:45 p.m on a clear day, i noticed a white sedan next to me starting to change lanes into my lane. It did not see my car, because i had to veer slight right to avoid being hit by the car. My car did not handle the sudden move well and immediately started to swerve left and right uncontrollably for 1-2 seconds. I could not regain control of the car even though both my hands were on the steering wheel, trying to straighten the car. The car then lost complete control and swerved/spun all the way left (imagine making a complete left turn on the freeway), into the 3rd lane. It continued going in a counter-clockwise direction until a white van ran into my car in the middle of the freeway, stopping the car in a head-on collision. The air bags popped, smoke started coming from the car. I was amazingly unhurt for the most part. I took my hands of the steering wheel and managed to open the driver seat door to exit the vehicle. The final position of the car was facing exactly the opposite direction of traffic. Several firefighters/paramedics arrived and proceeded to clean up the debris scattered around the roads and pushed the car to the side of the freeway. The car insurance company hired forensic engineers to reconstruct the accident and inspect the car for damages. Unfortunately, there was nothing conclusive to report (although the front rotors were suspect). *tr
I was driving a hertz rented mazda 6 2006 with approximately 1,100 miles on it. Was driving at approximately 65 miles per hour when i lost control of the car. The car swerved to the right and hit a truck and swerved back to the left and hit a median. On subsequent inspection, it was found that the steering linkages were faulty and had failed. The police have recorded the incident as mechanical failure. The car was completely damaged due to impact, the side air bag had released and the front left wheel had completely come off (the last mentioned being the cause of the above accident).
