A faulty automatic locking retractor (ALR) sensor in the front passenger seat belt system may incorrectly report as ON when it is actually OFF, causing the suppression or improper deployment of the air bag. Risk. A loose steering knuckle fastener could separate, causing a loss of vehicle control and increasing the risk of a crash.

Fix. Dealers will inspect and, as necessary, tighten the steering knuckle fastener, free of charge. Owner notification letters were mailed November 11, 2022. Owners may contact Rivian customer service at 1-888-748-4261. Rivian's number for this recall is FSAM 997.

What's wrong. Rivian Automotive, LLC (Rivian) is recalling certain 2022 R1T and R1S vehicles. The front seat belt anchors may have been inadequately attached to the B-pillar.

Risk. An improperly secured seat belt anchor may not adequately restrain the seat occupant during a crash, increasing the risk of injury.

Fix. Rivian will inspect the front seat belt assemblies and, as necessary, properly secure the bolts, free of charge. Owner notification letters were September 30, 2022. Owners may contact Rivian customer service at 1-888-748-4261. Rivian's number for this recall is FSAM-924.

The contact owns a 2024 Rivian R1S. The contact stated that while his wife was attempting to park in a parking space, the vehicle unintendedly accelerated. The brake pedal was violently depressed however the vehicle continued to accelerate. No warning lights illuminated. The contact stated that the vehicle hit a curb in a landscaping zone and crashed into another vehicle. The air bags did not deploy during the incident. No medical attention was provided but the contact and the contact’s daughter suffered minor neck, side, and back pain. The wife also sustained minor unknown injuries, A police report was filed with the Oswego State Department, but the report was not provided. The vehicle was towed to the dealer. The dealer was contacted, and the vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and a case was opened. The approximate failure mileage was 5,000.

While driving my Rivian R1S, 2025 at highway speeds, the infotainment screen went completely black, disabling the speedometer, blind spot monitoring, lane warnings, airbag/seatbelt alerts, and navigation. I had no access to critical safety information while transporting my children, creating a dangerous situation. Rivian service confirmed this is a “known bug” affecting many vehicles, but owners were not warned. Forums show this issue is widespread. Vehicle: 2025 Rivian R1S VIN: [XXX] Purchase Date: [XXX] Component/System Affected: Passenger Airbag / Occupant Detection System (ODS) Issue Description: The passenger airbag system in my 2025 Rivian R1S intermittently fails to detect a front seat passenger, causing the airbag to remain disabled even when an adult occupant is properly seated. The airbag status indicator reads “Passenger Airbag Off” despite full occupant weight in the seat, presenting a clear safety hazard in the event of a crash. This issue occurs unpredictably during normal use. The vehicle was first brought to a Rivian Service Center from June 11 to June 14, 2025, and is currently back in service as of June 23, 2025 for the same unresolved problem. However, the Rivian service team has stated they cannot replicate the issue, despite repeated attempts and my documentation of its occurrence. Safety Risk: Failure of the passenger airbag to activate under proper seating conditions poses a serious risk of injury or death in a collision. This appears to violate FMVSS No. 208 (Occupant Crash Protection) and may stem from a malfunction in the Occupant Detection System (ODS) or associated components. My vehicle has been a service seven times, each time I have complained about my wife not being recognized by the passenger seat. She weighs 100 pounds, and I’m told that the weight limit is 65. On the latest visit they replaced the passenger seat and told me if it still does it, then it is a design flaw, and I’ll have to deal with it. Don’t find this acceptable as it affects my wife’s safety, and would like the manufacturer to rectify the issues of the airbag will function correctly.

Yesterday was the 3rd time in 2 months that the vehicle displayed a "Airbag System needs service - Drive with caution until serviced" warning. I have taken it to Rivian 2 times where each time they replaced various components of the airbag system and it was reported to be fixed. Yesterday the warning returned. I am extremely concerned that Rivian does not have the ability to properly diagnose and or fix whatever the underlying issue is regarding my airbag system. I am now scheduled to take my vehicle in for the 3rd time for them to "try to figure it out".

On five separate occasions the airbag system on my car has become inoperable. Starting in July 2025 I noticed an airbag error on the display of the vehicle. I brought the car in for service and they replaced the steering wheel. The airbag error occurred again in August, this time requiring replacement of a different wiring harness. The error occurred again and again went in for service in November, this time replacing the front seats, seatbelt control modules, and the airbag module. This past weekend the air bag alert appeared again. Rivian seems unable to fix the issue with the airbags on my vehicle creating a significant safety concern.

Front passenger airbag is inactive for long periods of time while a passenger is seated in the vehicle during operation. This is indicated by the message displaying the airbag as being off. This occurs regularly with an adult seated normally in the seat while traveling at highway speeds, for long periods of time. I reported this to Rivian and they said the car is functioning normally but it does not seem safe that the airbag is turned off with a passenger present in the vehicle during operation. I am listing the latest incident date here but this has been persistent since I purchased the car new in 2023.

the sensitivity of the airbag sensor on the passenger seat is not functioning properly in my opinion. there are times when there is a passenger present and seated, the car is showing that the passenger airbag is off. I took the car to Rivian service center. They indicated that they could not replicate the problem and that to make sure the passengers are over 110 lbs. They also noted that with future software update, they may make it better, which indicates to me that they know there is a problem. I know for a fact that my passengers have been over 110 lbs.

I pulled into my semicircle, long driveway as I do every morning, and stopped at the top by releasing my foot off the accelerator (no need for brakes as it is an EV). I opened the garage door and slowly stepped on the accelerator, but it started to go fast, and I felt out of control. I immediately released my foot off the accelerator (which should have stopped the vehicle) and slammed on the brakes (there is proof in my driveway with tire marks leading up to the accident) but still did not stop. I quickly realized I was going to crash into the garage, and possibly into the house, so I steered away as much as I could to only drive into and through the side of the garage wall. The space I had would have given me enough time to stop had the car not been driving at a fast speed. Even the neighbors heard braking and ran outside to see what was happening. There is a recording on a ring cam, and you can hear him say "I heard brakes". Oddly, the massive impact did not deploy the airbags. Also, the car usually records all incidents with time before and after (it has successfully done this before when I was rear ended and will if you get too close to anything), but for some reason it recorded what was labeled "incident" and you can only see me pulling into the driveway, the garage door going up, and then the video stops - no record of the crash. The car should have also detected the house. The car only stopped after driving through concrete and stucco, and up the stairs. House has massive damage, personal items in the garage destroyed, and the vehicle is possibly unsalvageable. It took until the next day to see if we could remove the vehicle from my house, but it is now at the service center and Rivian has been notified. I've since researched online and found countless similar incidents.

Stopped at McDonalds drive through waiting to order with my foot on the brake and the vehicle lunged forward rearending the car stopped 5 feet in front of me ordering. I was trying to brake but it kept pushing the car in front of me then careened to the right hitting a sign and a parked car. I kept pumping the brakes and I thought it was going to run into a building when the vehicle finally stopped about 30 feet from the 2nd parked car it struck. Vehicle provided no warning before loss of control occurred. Created immediate risk of collision and serious injury. DETAILED DESCRIPTION: On November 16 2025 at approximately 5:25 PM ET, my 2025 Rivian R1S experienced sudden uncontrolled acceleration and deceleration while driving on a busy roadway. The vehicle began accelerating and decelerating erratically without driver input, creating immediate risk of rear-end collision or loss of control. No warning lights or messages appeared before the incident. The vehicle displayed "Thermal Fault Detected" and "Coolant Level Low" messages only after the erratic behavior began. This incident occurred with my family in the vehicle and created serious risk of injury or death. I was forced to have the vehicle towed. PRIOR RELATED INCIDENTS: This is not the first safety issue with this vehicle. Service Request SR-RAB3DU1AM documents "Emergency Braking Mistakenly Triggered on Regular Roadway" from August 29 2025 that remains unresolved in Rivian's system. The vehicle has required multiple service visits for thermal and cooling system failures at only 15000 miles. MANUFACTURER RESPONSE: Rivian service center claims rodent nesting caused thermal system failure leading to loss of vehicle control. If correct, this indicates critical design flaw - environmental factors should not be able to compromise vehicle control systems without warning or failsafe mechanisms. Vehicle lacks adequate: - Warning systems for thermal degradation before catastrophic failure - Failsafe mechanisms to prevent loss of control - Protection of critical systems from environmental factors SAFETY CONCERN: If thermal system damage can cause sudden loss of vehicle control without warning, this represents unreasonable safety risk. While driving the entire computer shuts down and reboots, both screens go black for several minutes. Speed display disappears, turn signals stop working, after the screens come back on there are warning lights for emergency braking disabled, cruise control disabled, and the headlights won’t come on for several minutes.

I am submitting this complaint regarding repeated ineffective repair attempts by Rivian Automotive, Inc. at its Costa Mesa Service Center on my 2025 Rivian R1S Adventure. During multiple service visits at Rivian’s Costa Mesa Service Center (January 29, June 18, July 16, and October 11, 2025), Rivian repeatedly failed to diagnose or correct the same issues affecting my 2025 Rivian R1S Adventure. • Drive Cam / Diagnostics Error: Rivian’s diagnostics incorrectly reported my drive storage as 119 GB when I have a 2 TB SSD (the maximum recommended). After five hours of testing, they failed to identify the issue. The in-car viewer still shows only partial clips, even though the full recordings are visible on a computer. • A/C Vibration and Noise: Despite Rivian tightening and insulating the A/C components, the vibration persists and is noticeable on the passenger side. This issue did not exist in the 1st generation R1S loaner vehicle, suggesting a design issue in the 2nd generation R1S, where A/C components are placed in the front compartment. • Proximity Lock and Bluetooth Delay: The vehicle frequently fails to unlock for 5–15 seconds even when I’m standing next to it. Rivian attributes this to “deep sleep mode,” which is unreasonable—no other automaker treats slow unlocking as normal. This delay is a safety concern in emergencies or bad weather. • Lighting / Camera Malfunctions: I provided video of headlights turning off while idling in a drive-thru and of the reverse camera freezing. Rivian could not replicate the issues and closed the cases. • Privacy Concern: A technician asked me to change my Rivian password and share credentials so they could log in for diagnostics. This raises privacy and data security concerns. After repeated visits and software updates (including version 2025.38.0, reissued October 21 and October 25 without listed fixes), all defects persist. These failures have caused significant inconvenience, time loss, and ongoing safety concerns.

In late May 2025, I took delivery of a new leased 2025 Rivian R1S equipped with the MaxPack battery. Since delivery, the vehicle has had intermittent electrical issues with the USB-C ports that I reported on June 12th (approx 3 weeks from taking delivery), but on July 28, 2025, it experienced a sudden and complete electrical system failure while parked. Both the high-voltage and low-voltage systems shut down, leaving the vehicle entirely inoperable. Rivian Service has reported that the incident was caused by an isolation fault, which triggered the pyrotechnic safety fuses to blow, fully disconnecting the high-voltage battery. The vehicle required towing on dollies to a Rivian Service Center, where it remains out of service pending repairs. This failure occurred within just a few months of delivery, and I have been informed by the tow operator that similar failures have occurred with other 2025 Rivian MaxPack vehicles. This defect completely disables the vehicle without warning, creating a serious safety risk if it were to occur while driving.

Winter weather led to snow buildup on the headlamps which made them useless. Fortunately this was during the day so there was plenty of sunlight. If this was at night, I would not have had any visibility and would have needed to stop every few miles to clean the headlamps. My BMW uses washer fluid to clean my headlamps and the xenon's are hot enough to melt snow. Neither of these are in the Rivian

On Monday, December 11, 2023 Myself, my girlfriend, and our two dogs were headed across the I-182 bridge coming out of Pasco, WA to Richland, WA. We were in the far left or center lane (I am not sure which). The highway is three lanes each way. We were at highway speed (the speed limit is 70 mph). About ¾ of the way across the bridge, the vehicle felt like all 4 wheels locked. The vehicle then skidded, turned 90 degree towards the concrete bridge barrier. We struck the bridge barrier nearly head on. The vehicle then rotated off the barrier and landed upright in the far right lane. Airbags fully deployed. There were numerous witnesses who reported seeing what looked like the vehicle’s wheels lock and then the events as described. No other cars were impacted. About 20 minutes prior to the crash I commented that the vehicle alignment was off. I had to keep the wheel at 10:00 or 11:00 for the vehicle to travel straight. I found that odd as the vehicle alignment was fine prior to that. Everyone walked away from the accident although both us were injured.

I was headed straight but stopped so that I could move into a left turn lane. As I got into the left turn lane and was straightening up behind the person in front of me at almost a dead stop the R1S lunged forward and accelerated at a high rate of speed hitting the car in front of me (speed control issue already known on the R1T). The camera footage verifies it. No warning lights went off (forward collision avoidance, automatic emergency braking) that there was a car in front of me and to brake (normally if there is a car in front of you that close and the vehicle thinks there will be a collision the screen goes red and the letters BRAKE come up bold in black, the car will brake if it thinks you may hit something). Of course now all my side sensors don’t work. I am still waiting on diagnostics from Rivian. Additionally, my seatbelt didn’t lock throwing me forward. I have only had this SUV two months with 848 miles. The car in front of me had substantial damage. If anyone had been in her backseat especially a child they would have been injured. My safety was in danger with the seatbelt not locking. I am waiting on the diagnostics still from Rivian. Insurance is waiting on Rivian. There had been nothing like this prior to this incident.

The contact owns a 2025 Rivian R1S. The contact stated that while attempting to reverse out of a driveway with the brake pedal still engaged, the vehicle lurched forward, crashing into the vehicle that was parked in front of her, with the entire front end of the vehicle. The vehicle then lurched to the left and crashed into another parked vehicle across the street, where it came to a stop and totaled that parked vehicle. Property damage was reported to the fence that was in front of the parked vehicle. The speed at the time of the crash was unknown. The vehicle was towed to a local auto body shop. No warning lights were illuminated prior to the crash. A police report was filed. The vehicle was not destroyed. No injuries were reported. The vehicle was repaired. The manufacturer was notified of the failure. The approximate failure mileage was 2,200.

While my wife was driving our 2025 Rivian R1S (approx. 13,500 miles), the vehicle was at a complete stop when it began to slowly roll forward on its own, followed by a sudden surge of acceleration, despite no input on the accelerator. This unexpected movement caused the vehicle to collide with a Tesla directly in front and involved another two vehicles. This incident posed a serious safety risk and could have injured pedestrians or other drivers. There were no warning lights or error messages before or during the incident. The cause is UNKNOWN at this time. Rivian has not yet inspected the vehicle, but we are requesting log preservation and a full review. This behavior has not been replicated since, and the vehicle is available for inspection upon request. We believe this may indicate a malfunction in the propulsion system, regenerative braking transition, or creep mode behavior. The incident has been reported to Rivian for investigation.

On [XXX], our 2025 Rivian R1S lurched forward while stopped at a red light, waiting to make a left turn. The driver, [XXX] , had her foot off the accelerator when the vehicle suddenly surged forward. She slammed on the brake, and the vehicle briefly bounced back — but then lurched forward again, striking the stopped vehicle in front a second time. There were no warnings or alerts on the dashboard. The incident was captured on video and shows a clear case of unintended forward acceleration. This may be linked to Rivian’s accelerator pedal recall or a software malfunction affecting braking or propulsion systems. I have video footage of this accident but was unable to upload the file. We are concerned about the safety risk this presents to other drivers and are requesting an NHTSA investigation. Vehicle was involved in a rear end collision. Vehicle was setup to have Emergency Braking enabled and used the Rivian High Regen setting (one pedal driving). In freeway traffic, the vehicle in front slowed quickly. The vehicle initiated emergency braking, but when the vehicle in front sped away, the vehicle failed to release emergency braking thus resulting in the car behind hitting my Rivian. There was no vehicle to the right of my car. I should have been able to accelerate out of the way and move to the right. It would appear the Rivian system requires a FULL STOP. I was unable to accelerate to mitigate the rear collision damage. Secondly, the vehicle acted like it did not recognize HIGH REGEN setting as a brake input. I have had multiple time where it flashed at me to brake and the HIGH REGEN setting was stopping the vehicle. This is not acceptable. Rivian was contacted and the vehicle taken for service and repair. I was informed that they reviewed the accident and that all systems checked okay. Rivian noted that I did request acceleration from the car, but I did not hold the accelerator long enough for the vehicle to allow me to regain control. I have asked what the threshold is to regain control and they were unable to answer. Minimally I would hope they can describe how their system has been designed so that I can properly drive their vehicle. Ideally, the system should display emergency information on how to override in the event it forces the car to stop on a freeway. I am sure the Rivian system is smart enough to see that I was on a freeway and I would hope smart enough to detect that I could have safely moved to the right as other cars did to avoid collision. I firmly believe the incident could have been avoided. Vehicle had all cameras enabled and full accident is recorded and available for review. Rivian also collected vehicle diagnostic logs.

Below are the diagnostics that Rivian says happened. I pulled into a parking space and as I placed my foot on the brake the car launched forward. I DID NOT hold both peddles down at the same time. The car malfunctioned and thank god I hit another parked car and not a person. The car has no been in the shop and at Rivian service center for the last 4 months. They are taking no responsibility for the events and are not returning my calls. I have read on many chat boards that this has happened to other drivers and they say it is "user error". a car with this much power should not be accidentally able to be put into launch mode. Vehicle enters a parking lot area at approximately 6:38 PM PST as vehicle speed is reduced and steering is consistent with parking lot maneuvers. Vehicle proceeds to travel at speeds below 15 mph. At approximately 6:40 PM PST the vehicle beings to turn left, then right while traveling at speeds below 7 mph. At this time, the steering wheel is returned to a centered position when the accelerator pedal is depressed to 98% application. Accelerator pedal and brake pedal are depressed simultaneously again as the vehicle comes into contact with another vehicle. Vehicle comes to a complete stop at 6:40:48 PM PST and remains in Drive. Vehicle remains in Drive and does not move for almost 8 minutes until vehicle is shifted into Reverse then Park at approximately 6:48:18 PM PST.

Stopped at McDonalds drive through waiting to order with my foot on the brake and the vehicle lunged forward rearending the car stopped 5 feet in front of me ordering. I was trying to brake but it kept pushing the car in front of me then careened to the right hitting a sign and a parked car. I kept pumping the brakes and I thought it was going to run into a building when the vehicle finally stopped about 30 feet from the 2nd parked car it struck. My vehicle has been a service seven times, each time I have complained about my wife not being recognized by the passenger seat. She weighs 100 pounds, and I’m told that the weight limit is 65. On the latest visit they replaced the passenger seat and told me if it still does it, then it is a design flaw, and I’ll have to deal with it. Don’t find this acceptable as it affects my wife’s safety, and would like the manufacturer to rectify the issues of the airbag will function correctly.

The 15mm nut securing the 2nd row seat track, behind the passenger front seat, was not secure and the seat track moved in all directions by several millimeters. The nut was stripped and could not be tightened. This manufacturing defect was repaired by Rivian. However, given the ongoing quality issues with this Rivian, I would speculate that other safety related defects have yet to be discovered.

The driver seat has preset settings for seat adjustments but they do not have Obstruction Detection. We have a rear facing car seat that was placed behind the drivers seat that had to be replaced (by us) as the seat auto adjusted and did not detect the back of the car seat and snapped the car seat out of its base. We contact Rivian over phone and they did not have a remedy (over a year ago, cannot recall exact date). It happened several times where we had to force stop the adjustment to prevent damage to new car seat. Eventually we decided to place the new rear facing car seat behind the passenger car seat instead. The back of the drivers seat has permanent damage from it pushing against the car seat. I did not think about filing a complaint here until I read about the recent Hyundai incident.

the sensitivity of the airbag sensor on the passenger seat is not functioning properly in my opinion. there are times when there is a passenger present and seated, the car is showing that the passenger airbag is off. I took the car to Rivian service center. They indicated that they could not replicate the problem and that to make sure the passengers are over 110 lbs. They also noted that with future software update, they may make it better, which indicates to me that they know there is a problem. I know for a fact that my passengers have been over 110 lbs.

My right front passenger seat does not detect adult occupants reliably, and therefore does not enable the airbags. My wife is approximately 105 pounds and sits in the seat in a standard upright fashion without leaning back. Her legs are not crossed and are just resting on the floor. it is rare that the vehicle detects her presence and therefore she is rarely protected by the safety restraint system in the event of an accident. I brought the vehicle into Rivian Service and they inspected and said the sensor is working properly and is designed to detect occupants roughly 105 to 110 pounds. They said there is nothing that they can do about it. Every other vehicle I currently own and have owned in the past properly detects her presence for the SRS system.

The passenger side airbag shows that it is turned off about 50% of the time that my wife is riding in the passenger seat. She meets the minimum weight requirement for passenger airbag deployment. If she moves around in the seat the airbag off light will go out temporarily but usually comes back on within several miles of driving. This vehicle is available for inspection. When this happens my wife or any other passenger is at risk of not having an airbag if an accident was to occur. This issues has been looked at several times by the manufacturer and I was told that the vehicle is operating as intended. No resonable solution was offered. The only solution from the manufacturer was to have the passenger move around or adjust the seat until the airbag came on. Then have the passenger stay in that position for the duration of the ride. We’ve tried this and it doesn’t work the airbag still turns off.

Every time my wife gets into the front passenger seat the sensor in the seat doesn’t recognize her sitting down and doesn’t active the passenger side airbag also the climate control is inactive. Rivian claims that there is a seat weight limit but my wife weighs 145 lbs. this happens all the time until my wife moves around enough to activate it. Then sometimes it turns off in the middle of a drive as if there wasn’t a passenger in that seat. This has also occurred with some heavier passengers. Rivian claims nothing wrong with the vehicle. I took delivery at the end of October and the problem still continues.

The contact owned a 2025 Rivian R1S. The contact stated that while driving 24 MPH, the other vehicle switched lanes in front of the contact's vehicle. The other driver had a learner's permit. The contact depressed the brake pedal, but it traveled down to the floorboard, and the vehicle failed to respond. The vehicle was pushed into the cement divider. The vehicle came to a stop when it crashed into a light pole inside the cement divider. The contact was unsure whether there was a warning light illuminated. The air bags deployed. A police report was filed. The contact and her daughter, along with one of the passengers, were taken to the hospital by ambulance. The contact sustained minor injuries: abrasions and cuts on her forehead. And a scar on the right arm. The contact's daughter, who was sitting on the back right seat, had abrasions and, due to tachycardia, was transported to a trauma hospital due to internal bleeding. The contact's passenger, who was sitting behind the driver's seat, had multiple lacerations on the mouth, lost two front teeth, and underwent a root canal. The contact's other passenger, who was sitting in the front passenger seat, was not injured. The vehicle was towed to an unknown location. The insurance company was notified and declared the vehicle a total loss. The manufacturer was not notified of the failure. The failure mileage was 24.

Immediately after purchasing the vehicle, I got error codes thrown to service the suspension while driving to my house, specifically at 15-20mph over traffic calming bumps. This fault cleared immediately the first few times. On July 25 I submitted a service request as my family headed out on a two week, 900 mile trip. The service suspension fault had returned and disabled my ability to change modes/ride heights, blind spot sensors and cruise control, all of which I was counting on using during trip. I shut off and restarted the vehicle several times trying to clear the fault and regain functionality, but the fault inevitably returned. On July 25, Rivian added a response in the service portal that they would address the problem by July 28th. I heard nothing more. On August 1, I was driving south on HWY 57 in North Idaho. I hoped that code would have cleared, but it came back on almost immediately. After driving for about 5 miles, I decided to pull over at a visitor center parking lot to try and reset again to change modes, regain safety sensors, and cruise control. While turning left into the parking lot, another driver tried to pass on my left on a double yellow line driving above the 55mph speed limit. He collided with the front left part of my vehicle, damaging the body and breaking the front tie rod ends, leaving me unable to steer or drive forward, disabled, in the oncoming traffic lane. I asked Rivian to share data about the turn signal use, brake light, and speed data at the time of the accident with me. They did not respond in writing, but called and refused, getting quite angry during the call. After researching and observing other Rivians, I don't believe that my brake lights were on during regenerative braking. The rear, side, and blind spot sensors were disabled. Turning 1/4 second sooner or 5º sharper would have killed me.

On November 27th the accelerator on my Rivian became stuck causing the car to accelerate quickly and preventing me from immediately stopping the car. There was a message stating that the regenerative braking system was not available and the accelerator pedal was stuck in the down position. When I pressed on the brake there was a terrible grinding noise and smoke coming from the front tires. I quickly realized that bringing the Rivian to a full stop would likely cause the car to catch fire. Since the road was clear I had time to discuss a plan with my family and I told them to prepare to get out of the car quickly. I pressed hard on the brakes which caused them to grind and smoke. I put the car in park and we all exited the car quickly. Flames were visible from the front tire. The car is available for inspection. If there had been other vehicles on the road or any pedestrians this would have been a life threatening incident. Rivian is claiming that I must have been pressing on both the accelerator and the brake at the same time. We had 2 adults and 3 children in the car and the incident lasted long enough for us to discuss a plan to exit the car. There is no chance that my foot was on both the accelerator and the brake. Rivian has conducted an inspection but has denied that there is an issue. There has not yet been an independent inspection. The car has been in Rivian's service center since the day after the incident. The only warning sign appeared during the incident stating that regenerative braking was not available but this did not explain why the accelerator pedal was stuck.

While my wife was driving our 2025 Rivian R1S (approx. 13,500 miles), the vehicle was at a complete stop when it began to slowly roll forward on its own, followed by a sudden surge of acceleration, despite no input on the accelerator. This unexpected movement caused the vehicle to collide with a Tesla directly in front and involved another two vehicles. This incident posed a serious safety risk and could have injured pedestrians or other drivers. There were no warning lights or error messages before or during the incident. The cause is UNKNOWN at this time. Rivian has not yet inspected the vehicle, but we are requesting log preservation and a full review. This behavior has not been replicated since, and the vehicle is available for inspection upon request. We believe this may indicate a malfunction in the propulsion system, regenerative braking transition, or creep mode behavior. The incident has been reported to Rivian for investigation.

Below are the diagnostics that Rivian says happened. I pulled into a parking space and as I placed my foot on the brake the car launched forward. I DID NOT hold both peddles down at the same time. The car malfunctioned and thank god I hit another parked car and not a person. The car has no been in the shop and at Rivian service center for the last 4 months. They are taking no responsibility for the events and are not returning my calls. I have read on many chat boards that this has happened to other drivers and they say it is "user error". a car with this much power should not be accidentally able to be put into launch mode. Vehicle enters a parking lot area at approximately 6:38 PM PST as vehicle speed is reduced and steering is consistent with parking lot maneuvers. Vehicle proceeds to travel at speeds below 15 mph. At approximately 6:40 PM PST the vehicle beings to turn left, then right while traveling at speeds below 7 mph. At this time, the steering wheel is returned to a centered position when the accelerator pedal is depressed to 98% application. Accelerator pedal and brake pedal are depressed simultaneously again as the vehicle comes into contact with another vehicle. Vehicle comes to a complete stop at 6:40:48 PM PST and remains in Drive. Vehicle remains in Drive and does not move for almost 8 minutes until vehicle is shifted into Reverse then Park at approximately 6:48:18 PM PST.

On February 1, 2026, at approximately 9:09 AM, while driving from a stop and continuing on main roads, the vehicle accelerated to extremely high speeds and would not properly slow down. When I removed my foot from the accelerator pedal, the vehicle did not decelerate as expected. Regenerative braking was nonexistent. Normally, releasing the accelerator provides noticeable deceleration, but in this incident the vehicle continued to propel forward instead of slowing. Braking was extremely difficult. The brake pedal felt abnormally stiff (“thick”) and required significant effort to depress. Even while actively braking, it felt as though the vehicle was still attempting to accelerate forward and did not slow normally. This occurred for a couple hundred yards before I was able to pull over due to safety concerns. At the same time, the vehicle was in All-Purpose drive mode, but the ride height was stuck at kneel height and would not adjust, which is abnormal. After pulling over, the vehicle would not shift into Park. I had to press the brake pedal with both feet while fully depressing it and lifting myself off the seat to keep the vehicle from moving. I then had to open the driver door in order to force the vehicle into Park. There were no warning lights or messages displayed on the dashboard. The vehicle was towed to the Rivian Service Center but has not yet been inspected because the service center is currently closed. This malfunction created a serious safety risk due to unintended acceleration, loss of normal deceleration, difficulty braking, abnormal suspension behavior, and inability to shift into Park.

Vehicle provided no warning before loss of control occurred. Created immediate risk of collision and serious injury. DETAILED DESCRIPTION: On November 16 2025 at approximately 5:25 PM ET, my 2025 Rivian R1S experienced sudden uncontrolled acceleration and deceleration while driving on a busy roadway. The vehicle began accelerating and decelerating erratically without driver input, creating immediate risk of rear-end collision or loss of control. No warning lights or messages appeared before the incident. The vehicle displayed "Thermal Fault Detected" and "Coolant Level Low" messages only after the erratic behavior began. This incident occurred with my family in the vehicle and created serious risk of injury or death. I was forced to have the vehicle towed. PRIOR RELATED INCIDENTS: This is not the first safety issue with this vehicle. Service Request SR-RAB3DU1AM documents "Emergency Braking Mistakenly Triggered on Regular Roadway" from August 29 2025 that remains unresolved in Rivian's system. The vehicle has required multiple service visits for thermal and cooling system failures at only 15000 miles. MANUFACTURER RESPONSE: Rivian service center claims rodent nesting caused thermal system failure leading to loss of vehicle control. If correct, this indicates critical design flaw - environmental factors should not be able to compromise vehicle control systems without warning or failsafe mechanisms. Vehicle lacks adequate: - Warning systems for thermal degradation before catastrophic failure - Failsafe mechanisms to prevent loss of control - Protection of critical systems from environmental factors SAFETY CONCERN: If thermal system damage can cause sudden loss of vehicle control without warning, this represents unreasonable safety risk. On our drive back from Yosemite, we began descending from Groveland, CA after leaving the Rivian charging station. Due to heavy rain, the car ahead of me was moving cautiously, limiting my speed to about 20 mph while maintaining a safe distance during the descent to Moccasin, CA. Thanks to the regenerative braking, I didn’t need to apply the brakes, as the car automatically slowed when I wasn’t pressing the throttle. However, my windows started to fog up, so I turned on the defoggers. Shortly after, I noticed the car’s door locks repeatedly unlocking and locking by themselves. As we neared the bottom of the descent, I began to smell something burning from the vents. This prompted me to step on the brake pedal for the first time, but it felt unusually stiff, with little travel. The car didn’t slow down, and the burning smell intensified. I pressed the brake pedal repeatedly, but it felt stuck, and the car did not decelerate. Eventually, I was able to roll to a stop. I pulled over to a side street where the issue persisted. I turned off the defogger and contacted Rivian via SOS. We were able to replicate the brake issue on the side street: as I maneuvered the car, the brake pedal sank on its own when the defogger was on. Once I turned it off, the pedal returned to normal. There were no warning lights or alerts from the car system. When I got out of the car, the burning smell was strong near the front wheels. It was clear that my family’s safety was at risk, including 3 young children. The car was towed to a service center, where it’s being inspected. Rivian hasn’t been able to reproduce the issue yet. I believe the regenerative brakes weren’t working properly, causing the car to rely on the regular brakes for the entire descent. This likely led to brake failure at the bottom, with the burning smell originating from overheated brakes. I also suspect that the overheating caused other electrical issues, which could explain why the door locks kept cycling on and off.

I was headed straight but stopped so that I could move into a left turn lane. As I got into the left turn lane and was straightening up behind the person in front of me at almost a dead stop the R1S lunged forward and accelerated at a high rate of speed hitting the car in front of me (speed control issue already known on the R1T). The camera footage verifies it. No warning lights went off (forward collision avoidance, automatic emergency braking) that there was a car in front of me and to brake (normally if there is a car in front of you that close and the vehicle thinks there will be a collision the screen goes red and the letters BRAKE come up bold in black, the car will brake if it thinks you may hit something). Of course now all my side sensors don’t work. I am still waiting on diagnostics from Rivian. Additionally, my seatbelt didn’t lock throwing me forward. I have only had this SUV two months with 848 miles. The car in front of me had substantial damage. If anyone had been in her backseat especially a child they would have been injured. My safety was in danger with the seatbelt not locking. I am waiting on the diagnostics still from Rivian. Insurance is waiting on Rivian. There had been nothing like this prior to this incident.

The brake pedal was violently depressed however the vehicle continued to accelerate. No warning lights illuminated. The contact stated that the vehicle hit a curb in a landscaping zone and crashed into another vehicle. The air bags did not deploy during the incident. No medical attention was provided but the contact and the contact’s daughter suffered minor neck, side, and back pain. The wife also sustained minor unknown injuries, A police report was filed with the Oswego State Department, but the report was not provided. The vehicle was towed to the dealer. The dealer was contacted, and the vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and a case was opened. The approximate failure mileage was 5,000.

On February 1, 2026, at approximately 9:09 AM, while driving from a stop and continuing on main roads, the vehicle accelerated to extremely high speeds and would not properly slow down. When I removed my foot from the accelerator pedal, the vehicle did not decelerate as expected. Regenerative braking was nonexistent. Normally, releasing the accelerator provides noticeable deceleration, but in this incident the vehicle continued to propel forward instead of slowing. Braking was extremely difficult. The brake pedal felt abnormally stiff (“thick”) and required significant effort to depress. Even while actively braking, it felt as though the vehicle was still attempting to accelerate forward and did not slow normally. This occurred for a couple hundred yards before I was able to pull over due to safety concerns. At the same time, the vehicle was in All-Purpose drive mode, but the ride height was stuck at kneel height and would not adjust, which is abnormal. After pulling over, the vehicle would not shift into Park. I had to press the brake pedal with both feet while fully depressing it and lifting myself off the seat to keep the vehicle from moving. I then had to open the driver door in order to force the vehicle into Park. There were no warning lights or messages displayed on the dashboard. The vehicle was towed to the Rivian Service Center but has not yet been inspected because the service center is currently closed. This malfunction created a serious safety risk due to unintended acceleration, loss of normal deceleration, difficulty braking, abnormal suspension behavior, and inability to shift into Park.

Vehicle provided no warning before loss of control occurred. Created immediate risk of collision and serious injury. DETAILED DESCRIPTION: On November 16 2025 at approximately 5:25 PM ET, my 2025 Rivian R1S experienced sudden uncontrolled acceleration and deceleration while driving on a busy roadway. The vehicle began accelerating and decelerating erratically without driver input, creating immediate risk of rear-end collision or loss of control. No warning lights or messages appeared before the incident. The vehicle displayed "Thermal Fault Detected" and "Coolant Level Low" messages only after the erratic behavior began. This incident occurred with my family in the vehicle and created serious risk of injury or death. I was forced to have the vehicle towed. PRIOR RELATED INCIDENTS: This is not the first safety issue with this vehicle. Service Request SR-RAB3DU1AM documents "Emergency Braking Mistakenly Triggered on Regular Roadway" from August 29 2025 that remains unresolved in Rivian's system. The vehicle has required multiple service visits for thermal and cooling system failures at only 15000 miles. MANUFACTURER RESPONSE: Rivian service center claims rodent nesting caused thermal system failure leading to loss of vehicle control. If correct, this indicates critical design flaw - environmental factors should not be able to compromise vehicle control systems without warning or failsafe mechanisms. Vehicle lacks adequate: - Warning systems for thermal degradation before catastrophic failure - Failsafe mechanisms to prevent loss of control - Protection of critical systems from environmental factors SAFETY CONCERN: If thermal system damage can cause sudden loss of vehicle control without warning, this represents unreasonable safety risk. Other R1S vehicles may be at risk of similar failures.

I was passing another vehicle on the road when the accelerator pedal got stuck on the all-weather floor mat and made the car virtually uncontrollable. I used the brakes to pull the car off the road and to a complete stop before engaging the parking brake and reaching down to dislodge the pedal.

Driving my Rivian R1S on a road trip with family from Seattle, WA to Ketchum, ID (about 600miles.) Stopped to recharge outside of Boise, ID for about 20mins. Started driving again and in about 5 minutes the car started warning (visual message and audible alarm). The following events then occurred: -- The warning message stated something along the line of "Pull over immediately the car will shut down imminently." -- I was driving in traffic and was able to make it through an intersection and saw a gas station in the distance. -- The power steering and braking began to go unresponsive and the car began slowing down as I drove through the intersection. I tried to guide the car to the gas station. -- While pulling in to the gas station the car could barely be controlled. Once dropping below 5mph or so it immediately shut itself down. -- The inability to effectively control the vehicle was extremely unsafe. If the intersection I had to pass through had a red light the car would likely have shut itself down and stranded our family at a very busy intersection. -- The car began flashing a ton of different warning messages. "Suspension needs to be serviced", "Front trunk unable to close please service", "Battery system overheating" are a few as an example. -- After calling customer service I reset the vehicle (using prescribed sequence of button pushes.) The car then went dark and nothing worked from there out. -- Called a tow truck and found hotel for family. Rivian was towed to the Boise service center. -- Rivian service center has been unable to diagnose or recreate the issue to date - they have been in possession of the vehicle for 17 days and counting.

I am submitting this complaint regarding repeated ineffective repair attempts by Rivian Automotive, Inc. at its Costa Mesa Service Center on my 2025 Rivian R1S Adventure. During multiple service visits at Rivian’s Costa Mesa Service Center (January 29, June 18, July 16, and October 11, 2025), Rivian repeatedly failed to diagnose or correct the same issues affecting my 2025 Rivian R1S Adventure. • Drive Cam / Diagnostics Error: Rivian’s diagnostics incorrectly reported my drive storage as 119 GB when I have a 2 TB SSD (the maximum recommended). After five hours of testing, they failed to identify the issue. The in-car viewer still shows only partial clips, even though the full recordings are visible on a computer. • A/C Vibration and Noise: Despite Rivian tightening and insulating the A/C components, the vibration persists and is noticeable on the passenger side. This issue did not exist in the 1st generation R1S loaner vehicle, suggesting a design issue in the 2nd generation R1S, where A/C components are placed in the front compartment. • Proximity Lock and Bluetooth Delay: The vehicle frequently fails to unlock for 5–15 seconds even when I’m standing next to it. Rivian attributes this to “deep sleep mode,” which is unreasonable—no other automaker treats slow unlocking as normal. This delay is a safety concern in emergencies or bad weather. • Lighting / Camera Malfunctions: I provided video of headlights turning off while idling in a drive-thru and of the reverse camera freezing. Rivian could not replicate the issues and closed the cases. • Privacy Concern: A technician asked me to change my Rivian password and share credentials so they could log in for diagnostics. This raises privacy and data security concerns. After repeated visits and software updates (including version 2025.38.0, reissued October 21 and October 25 without listed fixes), all defects persist. These failures have caused significant inconvenience, time loss, and ongoing safety concerns.

During an early season wet snow storm within minutes of driving the lights had become covered with snow and deflected the light in such a way that created a very unsafe driving environment. There was zero snow on the road because of it not being cold enough to stick but the deflection of the light and limited light on the road it was nearly impossible to see the road markings and lines to stay in the lane and created a very unsafe environment around me requiring me to drive significantly lower to try to stay on the road which could have caused an accident with other drivers who were not driving as slowly. There are a significant amount of reports of incidents like this across multiple forum platforms (ie: reddit) where drivers of the Rivian have indicated a drive that was very unsafe and something they had never experienced before. Rivian needs to create a retrofit to the existing headlamp that has a heating element of something to continue to melt the snow off the light as driving since the truck face is flat and the LEDs do not conduct enough heat to melt snow as driving.

Regarding the Safety Recall mentioned below, I reached out to Rivian Service Center. They are not giving me an ETA to fix this and no alternate is given. Is there anyways NHTSA help me by making Rivian Automotive LLC take responsibility and address the issue sooner than later ? The contact stated that occasionally, during colder weather, the headlights failed to function properly. The approximate failure mileage was 10,000.

Rivian pushed a software update to owners on the evening of 11/13/2023. This update made the driver information and infotainment screens non-functional and also disabled all driver saftey aids and any ability to turn exterior lights or windshield wipers off or on. On [XXX], I was driving my Rivian R1S on [XXX] in the far left lane at highway speed on a smooth, straight road when I suddenly and completely lost control of the vehicle without warning. The vehicle swerved right, struck a vehicle in the middle lane, then veered left, spun, and collided head-on with the guardrail on the right side of the road. I was evaluated at the scene by EMT. There were no warning lamps, messages, or any symptoms of any kind prior to the failure. The loss of control was instantaneous and without any prior indication of a problem. Review of video footage captured by the vehicle's own cameras clearly shows the rear driver-side toe link failing and the rear wheel moving in a manner inconsistent with normal operation immediately prior to loss of control. At the scene, the rear driver-side wheel was visibly sitting at an abnormal angle, inconsistent with normal alignment, and the toe link bolt was visibly absent from its mounting location. The vehicle is currently being preserved as evidence and is available for inspection. The mounting location where the toe link bolt should be seated is visible and available for examination. The vehicle has not yet been inspected by Rivian, a dealer, or any independent service center. I am aware that Rivian issued NHTSA recall 26V003 on January 8, 2026, covering 2022-2025 R1T and R1S vehicles for this exact defect — rear toe link joint separation while driving, which increases the risk of a crash without prior warning. Rivian acknowledged awareness of at least one prior crash related to this defect prior to the recall filing. My vehicle has no open recalls on record and I was never notified of recall 26V003 or any related service campaign. This incident resulted in a multi-vehicle accident on a public highway. Toe link failed on the driver rear side. My vehicle went out of control, veered over adjacent lanes, over the bike path, onto the side walk and back onto the road and side walk. I was left with a sore neck for several days, my apple watch kept trying to call 911 thinking I was in an accident. Anyone one the sidewalk or bike lane or adjacent vehicle lane would have been impacted. The problem has an outstanding recall with Rivian but my vehicle isn't part of the recall. Rivian is repairing the broken components. No warning lamps, messages, or other symptoms before the failure. I was traveling at ~50MPH through and intersection when as soon as I exited the intersection the vehicle turned to the right / banked to the right in an aggressive manner which I could not control. I went across and adjacent lane that was unoccupied sideways and onto the sidewalk then back onto the road.

Immediately after purchasing the vehicle, I got error codes thrown to service the suspension while driving to my house, specifically at 15-20mph over traffic calming bumps. This fault cleared immediately the first few times. On July 25 I submitted a service request as my family headed out on a two week, 900 mile trip. The service suspension fault had returned and disabled my ability to change modes/ride heights, blind spot sensors and cruise control, all of which I was counting on using during trip. I shut off and restarted the vehicle several times trying to clear the fault and regain functionality, but the fault inevitably returned. On July 25, Rivian added a response in the service portal that they would address the problem by July 28th. I heard nothing more. On August 1, I was driving south on HWY 57 in North Idaho. I hoped that code would have cleared, but it came back on almost immediately. After driving for about 5 miles, I decided to pull over at a visitor center parking lot to try and reset again to change modes, regain safety sensors, and cruise control. While turning left into the parking lot, another driver tried to pass on my left on a double yellow line driving above the 55mph speed limit. He collided with the front left part of my vehicle, damaging the body and breaking the front tie rod ends, leaving me unable to steer or drive forward, disabled, in the oncoming traffic lane. I asked Rivian to share data about the turn signal use, brake light, and speed data at the time of the accident with me. They did not respond in writing, but called and refused, getting quite angry during the call. After researching and observing other Rivians, I don't believe that my brake lights were on during regenerative braking. The rear, side, and blind spot sensors were disabled. Turning 1/4 second sooner or 5º sharper would have killed me.

I am submitting this complaint after learning through a recent TechCrunch article regarding the NHTSA investigation PE26004 into Rivian rear suspension / rear toe link failures that my incident appears substantially similar to the failures currently under investigation. On March 2, 2024, at approximately 1:29 AM, my son [XXX] was driving my 2023 Rivian R1S (VIN [XXX] ) in [XXX] . While making a normal right turn from [XXX] onto [XXX] and accelerating normally afterward at approximately 30 mph, the vehicle suddenly suffered what appeared to be a catastrophic rear suspension failure. According to the driver, the left rear portion of the vehicle suddenly jolted downward, a loud mechanical breaking sound was heard, and the vehicle immediately became uncontrollable. The rear suspension appeared to collapse instantly. The vehicle then began sliding uncontrollably across the roadway and crossed into the opposite traffic lane. The driver attempted to release the accelerator and correct steering input, but the vehicle did not respond normally. Steering control was effectively lost and the vehicle could not be controlled. The vehicle ultimately impacted a tree on the left rear/driver-side rear area before coming to rest perpendicular to the roadway. Importantly, the loss of control occurred BEFORE impact with the tree. The impact was a consequence of the apparent suspension failure and resulting loss of vehicle control, not the initiating cause of the event. There was no rain and road conditions were normal and dry. Driver-side airbags deployed and Rivian’s emergency response system automatically contacted the occupants after the crash. Police responded to the scene within minutes and a traffic crash report was generated. The vehicle had approximately 7,862 miles at the time of the incident and remained under factory warranty. clunking/thumping noise coming from front end over speed bumps like something is loose or going to fall apart. have taken car into service few times and was told its normal, its getting worse. Online forum threads have mentioned same issue , I am afraid one of these days something going to snap and cause a breakdown or even worse terrible accident.

Rivian has a 'kneel' option that uses the powered air suspension to lower the vehicle to make entry and exiting the vehicle easier for users. Yes, this can be turned off or on. When on, the function stops if a main door (driver/passenger) is open and left open the car will not continue to kneel for safety purposes. However, it does NOT stop if you are using the rear hatch to access anything. The car continues to kneel. I have hit my head multiple times on the rear hatch because I am behind the vehicle and assume a certain amount of head room and the car is still kneeling and I do not realize it and stand up like I was and hit my head. I have notified Rivian and asked for updates. I consider this a critical issue and has caused minor injury or confusion momentarily. I live on a busy street and the last thing I need is to hit my head and be knocked out. I have not heard anything from Rivian on my request and an update came out to 'fix' items but this was not included. I advised if it was not I would file an official complaint to make them take security and safety issues seriously.

While driving at city speeds, I heard a rubbing/grinding noise from the right rear of the vehicle and noticed that the suspension no longer felt normal. After stopping to inspect the vehicle, I found that the right rear wheel had shifted rearward within the wheel opening and was contacting the wheel liner. The condition appeared suddenly without prior warning messages or warning lights. I do not recall any curb strike, pothole impact, collision, off-road event, or other significant impact prior to the failure. The vehicle was towed to a manufacturer service center and inspected. The manufacturer determined the damage was impact-related and not covered under warranty. Recommended repairs include replacement of the right rear knuckle, upper longitudinal link, toe link, hub bearing, and halfshaft. The failure resulted in a significant change in rear suspension geometry and raised concerns about vehicle safety. The damaged components are currently in the possession of the manufacturer service center and may be available for inspection.

Front driver wheel had a hub bolt back out rendering the vehicle unable to drive. The car made a loud grinding noise and was unsteady. I had to pull over and was unable to drive any further. It required replacement of the hub bolt and axel. This is a known problem for rivian's and has not been addressed or checked. I reported a grinding sound when I turn 2 months ago and was told that the soonest Rivian could service my car was 2 months away. This known problem and complaint should have flaggged an immediate intervention and this needs a recall.
